3. Age-discriminated IVF Access and Evidence-based Ageism:Is There a Better Way?

Abstract

Access to state-funded fertility treatments is age-restricted in many countries based on epidemiological evidence showing age-associated fertility decline and aimed at administering scarce resources. In this article, we consider whether age-related restrictions can be considered ageist and what this entails for a normative appraisal of access criteria. We use the UK as a case study due to the state-funded and centrally regulated nature of in vitro fertilization (IVF) provision. We begin by reviewing concepts of ageism and age discrimination in gerontological scholarship and contend that it is analytically useful to differentiate between them when considering age-restricted health services. We then argue that criteria to access IVF could be considered indirectly ageist so far as they rely on an age-related evidence base that manifests ageist categorizations of persons. Lastly, we examine whether there could be more normatively desirable alternatives to devise criteria to access fertility treatment, considering “lifestyle” as a potential candidate. We conclude, however, that lifestyle-based discrimination is problematic because, unlike age-based discrimination, it risks exacerbating existing socioeconomic and ethnic inequalities. © The Author(s) 2021.

Abstract

Abstract

Accumulating evidence suggests that modifications of gut function and microbiota composition might play a pivotal role in the pathophysiology of several cardiovascular diseases, including heart failure (HF). In this study we systematically analysed gut microbiota composition, intestinal barrier integrity, intestinal and serum cytokines and serum endotoxin levels in C57BL/6 mice undergoing pressure overload by transverse aortic constriction (TAC) for 1 and 4 weeks. Compared to sham-operated animals, TAC induced prompt and strong weakening of intestinal barrier integrity, long-lasting decrease of colon anti-inflammatory cytokine levels, significant increases of serum levels of bacterial lipopolysaccharide and proinflammatory cytokines. TAC also exerted effects on microbiota composition, inducing significant differences in bacterial genera inside Actinobacteria, Firmicutes, Proteobacteria and TM7 phyla as shown by 16S rDNA sequencing of fecal samples from TAC or sham mice. These results suggest that gut modifications represent an important element to be considered in the development and progression of cardiac dysfunction in response to TAC and support this animal model as a valuable tool to establish the role and mechanisms of gut-heart crosstalk in HF. Evidence arising in this field might identify new treatment options targeting gut integrity and microbiota components to face adverse cardiac events.

18. Lesbian motherhood and mitochondrial replacement techniques: reproductive freedom and genetic kinship

Abstract

In this paper, we argue that lesbian couples who wish to have children who are genetically related to both of them should be allowed access to mitochondrial replacement techniques (MRTs). Firstly, we provide a brief explanation of mitochondrial diseases and MRTs. We then present the reasons why MRTs are not, by nature, therapeutic. The upshot of the view that MRTs are non-therapeutic techniques is that their therapeutic potential cannot be invoked for restricting their use only to those cases where a mitochondrial DNA (mtDNA) disease could be ‘cured’. We then argue that a positive case for MRTs is justified by an appeal to reproductive freedom, and that the criteria to access these techniques should hence be extended to include lesbian couples who wish to share genetic parenthood. Finally, we consider a potential objection to our argument: that the desire to have genetically related kin is not a morally sufficient reason to allow lesbian couples to access MRTs.

19. Non-essential treatment?:Sub-fertility in the time of COVID-19 (and beyond)

Abstract

The extremely difficult circumstances that fertility patients find themselves in force them to reconcile with acute losses and profound uncertainties. Sometimes what hurts the most is the loss of the child desired and imagined (Lesnik-Oberstein, 2008), which becomes a tangible loss in the event of a failed implantation or a miscarriage. Sometimes it is the loss of a partner due to the strain that fertility treatments can put on a relationship (Martins et al., 2014). In other instances, uncertainty about the outcome of treatment can become unbearable. The outbreak of SARS-CoV-2 in the first few months of 2020 confronted fertility patients with a new set of losses and uncertainties that came suddenly and unexpectedly. Due to the implementation of stringent social distancing measures and the suspension of ‘non-essential’ medical treatments and procedures, fertility treatments were halted in the USA, the UK and other European countries, (Ferguson, 2020; Miller, 2020). At the time of writing, many countries have slowly moved towards allowing fertility clinics to resume their activities. However, given the disruption caused, my contention in this brief commentary is that it is necessary to engage in conversations on the social value of offering fertility treatments and on whether it is justifiable to suspend them in the event of a health crisis of this proportion.

22. Regulating genome editing: for an enlightened democratic governance

Abstract

How should we regulate genome editing in the face of persistent substantive disagreement about the moral status of this technology and its applications? In this paper, we aim to contribute to resolving this question. We first present two diametrically opposed possible approaches to the regulation of genome editing. A first approach, which we refer to as “elitist,” is inspired by Joshua Greene’s work in moral psychology. It aims to derive at an abstract theoretical level what preferences people would have if they were committed to implementing public policies regulating genome editing in a context of ethical pluralism. The second approach, which we refer to as the democratic approach, defended by Francoise Baylis and Sheila Jasanoff et al., emphasizes the importance of including the public’s expressed attitudes in the regulation of genome editing. After pointing out a serious shortcoming with each of these approaches, we propose our own favored approach—the “enlightened democracy” approach—which attempts to combine the strengths of the elitist and democratic approaches while avoiding their weaknesses.

23. Lag length selection for unit root tests in the presence of nonstationary volatility

Abstract

A number of recent papers have focused on the problem of testing for a unit root in the case where the driving shocks may be unconditionally heteroskedastic. These papers have, however, taken the lag length in the unit root test regression to be a deterministic function of the sample size, rather than data-determined, the latter being standard empirical practice. We investigate the finite sample impact of unconditional heteroskedasticity on conventional data-dependent lag selection methods in augmented Dickey-Fuller type regressions and propose new lag selection criteria which allow for unconditional heteroskedasticity. Standard lag selection methods are shown to have a tendency to over-fit the lag order under heteroskedasticity, resulting in significant power losses in the (wild bootstrap implementation of the) augmented Dickey-Fuller tests under the alternative. The proposed new lag selection criteria are shown to avoid this problem yet deliver unit root tests with almost identical finite sample properties as the corresponding tests based on conventional lag selection when the shocks are homoskedastic.

Abstract

We show that full risk sharing may not be at odd with the idea that changes in regional consumption display error-correcting dynamics, in line with the idea that information and transaction costs stemming from interregional portfolio diversification and labor movements induced by permanent income shocks may delay the adjustment process. Using Italian data over the period 1960-2001 it is found that regional per capita consumptions match the proposed error-correcting structure.

Abstract

Mitochondria are dynamic organelles that frequently undergo fission and fusion processes, imbalances in which have recently emerged as important etiological factors in obesity and insulin-resistance. The present works aimed to evaluate the association between mitochondrial dynamic behaviour and hepatic steatosis development when rats were fed with high fat diet for different periods. To this end, hepatic lipid accumulation was monitored in rats fed a control diet (10% J/J, N rats) or a high-fat diet (40% J/J, L rats) for 6 or 24 weeks. Mitochondrial fatty acid oxidation rates and the content of fusion (MFN2, OPA1) and fission (DRP1, Fis1) proteins (by western blot and immunohistochemical analysis) were assessed. After 6 weeks, L rats showed increased hepatic lipid accumulation and beta-oxidation associated with a shift toward mitochondrial fission processes (decreased MFN2 content and increased DRP1 and Fis1 content). This fission phenotype, by enhancing mitochondrial surface area, may improve fatty acids intake and, together with the increase in beta-oxidation, may be useful to avoid excessive fat accumulation. After 24 weeks, the further increase in hepatic lipid accumulation was associated with beta-oxidation impairment and a fission phenotype with a decrease in OPA1 content, too. Bearing in mind the role of OPA1 in stabilizing cristae structure and preventing autophagia, its decrease may be useful to eliminate functional impaired mitochondria and minimize cellular damage.

Abstract

Aims: We analyze how the inflammatory state in adipose tissue caused by a condition of chronically positive energy balance can lead to insulin resistance first in adipose tissue, then in all insulin-sensitive tissues.Data synthesis: Chronic nutrient overload causes an increase in adipose depots that, if adipose tissue expandability is tow, are characterized by an increased presence of hypertrophic adipocytes. This adipocyte hypertrophy is a possible stress condition for the endoplasmic reticulum (ER) that would lead to a proinflammatory state in adipose tissue. In this condition, ER stress would activate metabolic pathways that trigger insulin resistance, release of macrophage chemoattractant proteins, and in chronic inflammation, the death of the hypertrophic adipocyte. The infiltrated macrophages in turn release inflammatory proteins causing further recruitment of macrophages to adipose tissue and the release of inflammatory cytokines. Following these events, insulin resistance becomes extended to all adipose tissue. Insulin-resistant adipocytes, characterized by low liposynthetic capacity and high lipolytic capacity, cause increased release of free fatty acids (FFA). FFA released by lipolitic adipocytes may also activate Toll-like receptors 4 and then chemokines and cytokines release amplifying insulin resistance, lipolysis and inflammation in all. adipose tissue. Moreover, increased circulating FFA levels, reduced circulating adiponectin levels and leptin resistance lead to decreased lipid oxidation in non-adipose tissues, thereby triggering ectopic accumulation of lipids, lipotoxicity and insulin resistance.Conclusion: All. the conditions that increase circulating fatty acids and cause lipid overloading (obesity, lipoatrophy, lipodystrophy, catabolic states, etc.) induce a lipotoxic state in non-adipose tissues that gives rise to insulin resistance.

The ARGO-YBJ experiment is designed for the detection of Extensive Air Showers (EAS) in the primary energy range 100 GeV–10 TeV. Its full-coverage feature allows to lower the energy threshold about one order of magnitude with respect to sampling EAS detectors. Here the performance of the RPC array is described and the current status of the experiment is presented.

Abstract

The ARGO-YBJ detector is an extensive air shower array consisting of a carpet of Resistive Plate Chambers (RPCs). Very high energy (VHE) gamma ray astronomy is the main scientific goal of this Chinese-Italian experiment. High altitude and full coverage ensure a low energy threshold (few hundreds of GeV for primary photons), high duty-cycle and large field of view allow a continuous sky survey in the declination range from −10° to +70°. Also many features of the high energy cosmic rays are studied by ARGO-YBJ exploiting the unprecented shower reconstruction.

Abstract

It is well established that the shocks driving many key macro-economic and financial variables display time-varying volatility. In this paper we consider estimation and hypothesis testing on the coefficients of the co-integrating relations and the adjustment coefficients in vector autoregressions driven by both conditional and unconditional heteroskedasticity of a quite general and unknown form in the shocks. We show that the conventional results in Johansen (1996) for the maximum likelihood estimators and associated likelihood ratio tests derived under homoskedasticity do not in general hold in the presence of heteroskedasticity. As a consequence, standard confidence intervals and tests of hypothesis on these coefficients are potentially unreliable. Solutions to this inference problem based on Wald tests (using a "sandwich" estimator of the variance matrix) and on the use of the wild bootstrap are discussed. These do not require the practitioner to specify a parametric model for volatility, or to assume that the pattern of volatility is common to, or independent across, the vector of series under analysis. We formally establish the conditions under which these methods are asymptotically valid. A Monte Carlo simulation study demonstrates that significant improvements in finite sample size can be obtained by the bootstrap over the corresponding asymptotic tests in both heteroskedastic and homoskedastic environments. An application to the term structure of interest rates in the US illustrates the difference between standard and bootstrap inferences regarding hypotheses on the co-integrating vectors and adjustment coefficients.

Abstract

In this work, estimation and testing methods for Brownian motions in the presence of two reflecting barriers are analysed. By deriving the autocovariance function of the reflected Brownian motion, the asymptotic distributions of ordinary least squares esti-mators and Dickey-Fuller test statistics are obtained and the presence of inconsistency problems is evaluated analytically. Abstract

La consistenza del tipo genetico caprino autoctono 'Cilentana' si aggira intorno ai 6000 capi ed è diffuso soprattutto nelle aree della provincia di Salerno e, principalmente, nel terrotorio del Cilento e del Vallo di Diano. L'allevamento è rivolto principalmente alla produzione della carne e del latte, dal quale si ottiene un particolare formaggio denominato 'cacioricotta'. La ricerca ha lo scopo di caratterizzare dal punto di vista somatico la popolazione caprina allevata nel Cilento al fine di stabilire gli standards minimi richiesti per l'iscrizione al registro anagrafico e per iniziare i controlli funzionali per la produzione del latte. Lo studio è stato effettuato su 91 becchi e 1.745 capre distribuiti in 67 allevamenti ubicati in 28 comuni della provincia di Salerno. I risultati dell'indagine hanno evidenziato che le misure somatiche variano notevolmente sia in relazione all'allevamento e, quindi, alla regione agraria considerata, che in relazione all'età dei soggetti e alle altre caratteristiche fanerotiche quali il tipo di mantello, il portamento dell'orecchio e, per le femmine, il tipo di mammella

50. Bootstrap Determination of the Co-Integration Rank in Heteroskedastic VAR Models

Abstract

In a recent paper Cavaliere et al. (2012) develop bootstrap implementations of the (pseudo-) likelihood ratio (PLR) co-integration rank test and associated sequential rank determination procedure of Johansen (1996). The bootstrap samples are constructed using the restricted parameter estimates of the underlying vector autoregressive (VAR) model which obtain under the reduced rank null hypothesis. They propose methods based on an independent and individual distributed (i.i.d.) bootstrap resampling scheme and establish the validity of their proposed bootstrap procedures in the context of a co-integrated VAR model with i.i.d. innovations. In this paper we investigate the properties of their bootstrap procedures, together with analogous procedures based on a wild bootstrap resampling scheme, when time-varying behavior is present in either the conditional or unconditional variance of the innovations. We show that the bootstrap PLR tests are asymptotically correctly sized and, moreover, that the probability that the associated bootstrap sequential procedures select a rank smaller than the true rank converges to zero. This result is shown to hold for both the i.i.d. and wild bootstrap variants under conditional heteroskedasticity but only for the latter under unconditional heteroskedasticity. Monte Carlo evidence is reported which suggests that the bootstrap approach of Cavaliere et al. (2012) significantly improves upon the finite sample performance of corresponding procedures based on either the asymptotic PLR test or an alternative bootstrap method (where the short run dynamics in the VAR model are estimated unrestrictedly) for a variety of conditionally and unconditionally heteroskedastic innovation processes
