The objective was to review, among individuals of different skin colors and ethnic groups, how the effectiveness and safety of drugs could be influenced by pharmacokinetics variations resulting from genetic polymorphisms in the cytochrome P-450 enzyme system (CYP-450). This is a systematic review study, with a descriptive and observational approach. The systematic review protocol was registered on the international platform Prospective Register of Systematic Reviews under the identification number 25581111. Data collection was performed on Medical Literature Analysis and Retrieval System Online (MedLine) through PubMed, Latin American Literature, and the Caribbean Health Sciences (LILACS), as well as the Scientific Electronic Library Online (SciELO). Six CY450 genetic polymorphisms that may affect pharmacodynamic and pharmacokinetics parameters of therapy in drug users of different ethnic and skin color was identified and described to drugs as Olanzapine, Efavirenz, and Irinotecan. However, the results found did not demonstrate the existence of clinically significant differences when evaluating the aspects of safety and effectiveness of the drugs, despite the identification of polymorphisms in the CYP450 enzyme system that influenced these parameters. The findings point to the identification of individual genotypes, without ethnic or skin color cut, to provide a better application of precision medicine, thus helping to make therapeutic decisions in clinical practice in the light Evidence-Based medicine. El objetivo fue revisar, entre individuos de diferentes colores de piel y grupos étnicos, cómo la efectividad y la seguridad de los medicamentos pueden verse influenciadas por variaciones farmacocinéticas resultantes de polimorfismos genéticos en el sistema enzimático del citocromo P-450 (CYP450). Se trata de un estudio de revisión sistemática, con enfoque descriptivo y observacional. El protocolo de revisión sistemática se registró en la plataforma internacional Prospective Register of Systematic Reviews con el número de identificación 25581. La recolección de datos se realizó en las plataformas Medical Literature Analysis and Retrieval System Online (MedLine) a través de PubMed, Latin American Literature and Caribbean in Health Sciences (LILACS), así como la Biblioteca Científica Electrónica en Línea (SciELO). Fue posible identificar y describir seis polimorfismos genéticos de CYP450 que parecen afectar los parámetros farmacocinéticos y farmacodinámicos de la farmacoterapia en usuarios de medicamentos de diferentes colores de piel y grupos étnicos para los medicamentos Olanzapina, Efavirenz e Irinotecan. Sin embargo, los resultados encontrados no demostraron la existencia de diferencias clínicamente significativas al evaluar los aspectos de seguridad y eficacia del fármaco, a pesar de la identificación de polimorfismos en el sistema enzimático CYP450 que influían en estos parámetros. Los hallazgos apuntan a la identificación de genotipos individuales, sin etnia ni color de piel, para brindar una mejor aplicación de la medicina de precisión, ayudando así a la toma de decisiones terapéuticas en la práctica clínica a la luz de la Evidencia Basada en Salud. Objetivou-se revisar, dentre indivíduos de diferentes cores de pele e grupos étnicos, como a efetividade e a segurança de medicamentos pode ser influenciada por variações farmacocinéticas resultantes de polimorfismos genéticos no sistema enzimático do citocromo P-450 (CYP450). Trata-se de um estudo do tipo revisão sistemática, com abordagem descritiva e observacional. O protocolo da revisão sistemática foi submetido a registro na plataforma internacional Prospective Register of Systematic Reviews sob o número de identificação 25581. A coleta de dados foi realizada nas plataformas Medical Literature Analysis and Retrieval System Online (MedLine) através da PubMed, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), bem como na Scientific Eletronic Library Online (SciELO). Foi possível identificar e descrever seis polimorfismos genéticos do CYP450 que parecem afetar os parâmetros farmacocinéticos e farmacodinâmicos da farmacoterapia em usuários de medicamentos de diferentes cores de pele e grupos étnicos para os medicamentos Olanzapina, Efavirenz e Irinotecano. No entanto, os resultados encontrados não demonstraram a existência de diferenças clinicamente significativas quando avaliados os aspectos de segurança e efetividade de medicamento, apesar de serem identificados os polimorfismos no sistema enzimático do CYP450 que influenciavam nesses parâmetros. Os achados apontam para a identificação dos genótipos individuais, sem recorte étnico ou de cor da pele, a fim de propiciar uma melhor aplicação da medicina de precisão, auxiliando, assim, na tomada de decisões terapêuticas na prática clínica sob à luz da Saúde Baseada em Evidências.

In this household-based seroepidemiological survey, we analyzed the dynamics of SARS-CoV-2 seroprevalence during the first year of the COVID-19 pandemic in Sergipe State, Northeast Brazil, the poorest region of the country. A total of 16,547 individuals were tested using a rapid IgM−IgG antibody test and fluorescence immunoassay (FIA). Seroprevalence rates were presented according to age, sex, and geographic region. A comparative analysis was performed between the results obtained in July 2020 (peak of the first wave), August – November 2020 (end of the first wave), and February – March 2021 (beginning of the second wave). Seroprevalence rates in the three phases were estimated at 9.3% (95% CI 8.5–10.1), 12.0% (95% CI 11.2–12.9) and 15.4% (95% CI 14.5–16.4). At the end of the first wave, there was a rise in seroprevalence in the countryside (p < 0.001). At the beginning of the second wave, we found an increase in seroprevalence among women (p < 0.001), adults aged 20 to 59 years (p < 0.001), and the elderly (p < 0.001). In this phase, we found an increase in estimates both in metropolitan areas and in the countryside (p < 0.001). This study showed an increase in SARS-CoV-2 seroprevalence over the first year of the pandemic, with approximately one in six people having anti-SARS-CoV-2 antibodies at the beginning of the second wave of COVID-19. Furthermore, our results suggest a rapid spread of COVID-19 from metropolitan areas to the countryside during the first months of the pandemic. [ABSTRACT FROM AUTHOR]

Cysteine proteases are an important class of enzymes involved in several essential biological processes. For this reason, these enzymes are important therapeutic targets, and therefore can be used in enzymatic assays for the search of hits. The most widely used methods in the search of enzyme inhibitors are spectrophotometric, however they have relatively high operational costs. For this reason, simpler, and cheaper methods can be useful in the search for enzyme inhibitors. In this work, a simple and low-cost semi-quantitative method was optimized and validated using papain and skimmed powdered milk (casein) as a substrate. The ideal condition for the method was the one that used powdered milk, 0.250%, papain solution, 9.4 mg mL-1 and incubation time, 18 h. The method validation was performed using the inhibitor E64 and herb extracts, in different concentrations. After carrying out the experiments and statistical analyses, a high reproducibility can be observed, with relative standard deviations below 2.1%. Plant extracts were used to test the method and the results indicated that these extracts are promising for the search of cysteine protease inhibitors. The method developed here has simplicity and reproducibility and has great application, as preliminary study, in the search for inhibitors of cysteine proteases.

Bacillus subtilis spores have received growing attention regarding potential biotechnological applications, including the use as probiotics and in vaccine formulations. B. subtilis spores have also been shown to behave as particulate vaccine adjuvants, promoting the increase of antibody responses after co-administration with antigens either admixed or adsorbed on the spore surface. In this study, we further evaluated the immune modulatory properties of B. subtilis spores using a recombinant HIV gag p24 protein as a model antigen. The adjuvant effects of B. subtilis spores were not affected by the genetic background of the mouse lineage and did not induce significant inflammatory or deleterious effects after parenteral administration. Our results demonstrated that co-administration, but not adsorption to the spore surface, enhanced the immunogenicity of that target antigen after subcutaneous administration to BALB/c and C57BL/6 mice. Spores promoted activation of antigen presenting cells as demonstrated by the upregulation of MHC and CD40 molecules and enhanced secretion of pro-inflammatory cytokines by murine dendritic cells. In addition, in vivo studies indicated a direct role of the innate immunity on the immunomodulatory properties of B. subtilis spores, as demonstrated by the lack of adjuvant effects on MyD88 and TLR2 knockout mouse strains. Conselho Nacional de Pesquisa e Desenvolvimento Científico e Tecnológico - CNPq Ginger (Zingiber officinale Roscoe) is a medicinal plant known for its anti-inflammatory, antitumoral, antioxidant and antimicrobial properties. The objective of this study was to optimize the extraction of sesquiterpenes (α-curcumene, α-zingiberene, α-farnesene, β-sesquifelandrene and β-bisabolene), to evaluate their antimicrobial and antibiotic potential against Staphylococcus aureus, Streptococcus mutans, S. agalactiae, Escherichia coli and Pseudomonas aeruginosa, in addition to their antiproteic activity. Analyzes of the extracted compounds were performed by GC-MS. Antimicrobial and biofilm formation assays were performed by the 96 well plate microdilution technique. In addition to assessing biofilm inhibition, the hydrophobicity index was also determined as an indirect way of assessing the ability of microorganisms to adhere to host tissues and abiotic surfaces. The best conditions for obtaining the sesquiterpene-enriched fraction (SEF) were: extraction time: 4 min, plant / solvent ratio: 1:20 (m / v) and microwave power: 400 W. The SEF presented a MIC of 2000 μg / mL compared to S. mutans and was ineffective in inhibiting the growth of other bacteria. Nevertheless, SEF (250 μg / mL) inhibited the biofilm formation of S. aureus, S. agalactiae and E. coli by 50%, 80% and 80%, respectively. In addition, and E. coli bacteria by 61.86%, 73.44% and 49.10%, respectively. FES (500 μg / mL) was able to reduce papain enzymatic activity by 71.5%, with an IC50 of 386.8 μg / mL. The results showed that SEF had potential to combat the production of bacterial biofilms. Thus, these substances are good candidates for drug prototypes used to treat infections caused by multi-resistant bacteria. To our knowledge, this is the first time that the SEF antimicrobial and antibiofilm activity has been studied. O gengibre (Zingiber officinale Roscoe) é uma planta medicinal conhecida por suas propriedades anti-inflamatórias, antitumorais, antioxidantes e antimicrobianas. O objetivo deste trabalho foi otimizar a extração de sesquiterpenos (α-curcumeno, α-zingibereno, α-farneseno, β-sesquifelandreno e β-bisaboleno), avaliar seu potencial antimicrobiano e antibiofilme contra Staphylococcus aureus, Streptococcus mutans, S. agalactiae, Escherichia coli e Pseudomonas aeruginosa, além da sua atividade antiproteásica. As análises dos compostos extraídos foram realizadas por CG-ES. Os ensaios antimicrobianos e formação de biofilmes foram realizados pela técnica de microdiluição em placas de 96 poços. Além de avaliar a inibição do biofilme, o índice de hidrofobicidade também foi determinado como uma forma indireta de avaliar a capacidade de adesão dos micro-organismos aos tecidos hospedeiros e superfícies abióticas. As melhores condições para obtenção da fração enriquecida em sesquiterpenos (FES) foram: tempo de extração: 4 min, razão planta/solvente: 1:20 (m/v) e potência de micro-ondas: 400 W. A FES apresentou CIM de 2000 μg/mL frente a S. mutans e foi inefetivo na inibição do crescimento das outras bactérias. Apesar disso a FES (250 g/mL) inibiu a formação de biofilme de S. aureus, S. agalactiae E. coli em 50%, 80% e 80%, respectivamente. Ademais, a FES (250 g/mL) reduziu o índice de hidrofobicidade das bactérias S. aureus, S. agalactiae e E. coli em 61,86%, 73,44% e 49,10%, respectivamente. A FES (500 μg/mL) foi capaz de reduzir em 71,5% a atividade enzimática da papaína, apresentando IC50 386,8 μg/mL. Os resultados mostraram que a FES apresentou potencial no combate à produção de biofilmes bacterianos. Desse modo, essas substâncias representam-se como bons candidatos a protótipos de medicamentos utilizados no tratamento de infecções causadas por bactérias multirresistentes.
