Objective: The standard surgical treatment of advanced ovarian carcinoma is primary debulking surgery (PDS) aiming to complete cytoreduction. The need to achieve complete cytoreduction has shifted the surgical paradigm to more complex procedures, whose impact on morbidity is controversial. The objective of this retrospective analysis is to explore the impact of extensive PDS on morbidity and oncologic outcomes in a real-world scenario. Methods: A retrospective single-center analysis was performed on 137 patients with advanced high-grade ovarian carcinoma (HGOC) who received PDS in 2015–2020. Patients treated in 2015–2017 (Group 1) were compared to patients treated in 2018–2020 (Group 2). The two periods were chosen according to the higher complexity of surgical procedures introduced in 2018. Results: The increase in complete cytoreduction observed in Group2 (RD 0: 33 % vs 61 %, p = 0,008) was related to a higher surgical complexity (Aletti Score: 4 vs 6, p = 0,003) and did not reflect an increase in peri-operative complications (CCI: 20,9 vs 20,9, p = 0,11). After a median FUP of 44 months, PFS and OS at 24 months were 33,60 % vs 47,33 % (p = 0,288) and 72,10 % vs 80,37 % (p = 0,022) in Group 1 and 2, respectively. Conclusions: An extensive surgical effort leads to a significant increase in complete cytoreduction with acceptable morbidity. Arm-in-arm with novel maintenance therapies, it contributes to increasing the outcomes of patients with advanced HGOC.

The benthic mucilage producing microalga Chrysophaeum taylorii Lewis and Bryan (Pelagophyceae) has recently received attention for its rapid spread in the Mediterranean Sea, where its blooms have remarkable detrimental effects. So far no information on C. taylorii response to multiple stressors, especially in terms of mucilage hyperproduction, is available in the literature yet, and a manipulative field experiment in this topic was designed in Tavolara Punta Coda Cavallo Marine Protected Area. The aim of the study was to test the effects of nutrient enrichment (addition of nutrients), mechanical disturbance (partial and total benthic organisms removal) and hydrodynamics (increased water turbulence) on C. taylorii cell density and mucilage abundance. To the purpose, the three above mentioned stressors were simulated and the three treatments were assigned to 20 × 20 cm plots following a full-factorial design (n = 3). Interactive effects of the three stressors affected significantly both benthic C. taylorii cell density and mucilage cover although differently. Mechanical disturbance and high hydrodynamics produced consistent effects on cell density and mucilage production (i.e. the former factor enhancing and the latter decreasing). Nutrient enrichment on the contrary led to contrasting effects, promoting cell abundance and inhibiting mucilage production. Therefore, important mucilage blooms are expected in oligotrophic sheltered coastal locations where barren areas are present.

Elephants are the biggest living terrestrial animal, weighing up to five tons and measuring up to three metres at the withers. These exceptional dimensions provide certain advantages (e.g. the mass-specific energetic cost of locomotion is decreased) but also disadvantages (e.g. forces are proportional to body volume while supportive tissue strength depends on their cross-sectional area, which makes elephants relatively more fragile than smaller animals). In order to understand better how body size affects gait mechanics the movement of the centre of mass (COM) of 34 Asian elephants (Elephas maximus) was studied over their entire speed range of 0.4-5.0 m s(-1) with force platforms. The mass-specific mechanical work required to maintain the movements of the COM per unit distance is ~0.2 J kg(-1) m(-1) (about 1/3 of the average of other animals ranging in size from a 35 g kangaroo rat to a 70 kg human). At low speeds this work is reduced by a pendulum-like exchange between the kinetic and potential energies of the COM, with a maximum energy exchange of ~60% at 1.4 m s(-1). At high speeds, elephants use a bouncing mechanism with little exchange between kinetic and potential energies of the COM, although without an aerial phase. Elephants increase speed while reducing the vertical oscillation of the COM from about 3 cm to 1 cm.

The muscle-tendon work performed during locomotion can, in principle, be measured from the mechanical energy of the centre of mass of the whole body and the kinetic energy due to the movements of the body segments relative to the centre of mass of the body. Problems arise when calculating the muscle-tendon work from increases in mechanical energy, largely in correctly attributing these increases either to energy transfer or to muscle-tendon work. In this study, the kinetic and gravitational potential energy of the centre of mass of the whole human body was measured (using a force platform) simultaneously with calculation of the kinetic and potential energy of the body segments due to their movements relative to the body centre of mass (using cinematography) at different speeds of walking and running. Upper and lower boundaries to the total work were determined by including or excluding possible energy transfers between the segments of each limb, between the limbs and between the centre of mass of the body and the limbs. It appears that the muscle-tendon work of locomotion is most accurately measured when energy transfers are only included between segments of the same limb, but not among the limbs or between the limbs and the centre of mass of the whole body.

1. At high running speeds, the step frequency becomes lower than the apparent natural frequency of the body's bouncing system. This is due to a relative increase of the vertical component of the muscular push and requires a greater power to maintain the motion of the centre of gravity, Wext. However, the reduction of the step frequency leads to a decrease of the power to accelerate the limbs relatively to the centre of gravity, Wint, and, possibly, of the total power Wtot = Wext + Wint. 2. In this study we measured Wext using a force platform, Wint by motion picture analysis, and calculated Wtot during human running at six given speeds (from 5 to 21 km h-1) maintained with different step frequencies dictated by a metronome. The power was calculated by dividing the positive work done at each step by the duration of the step (step-average power) and by the duration of the positive work phase (push-average power). 3. Also in running, as in walking, a change of the step frequency at a given speed has opposite effects on Wext, which decreases with increasing step frequency, and Wint, which increases with frequency; in addition, a step frequency exists at which Wtot reaches a minimum. However, the frequency for a minimum of Wtot decreases with speed in running, whereas it increases with speed in walking. This is true for both the step-average and the push-average powers. 4. The frequency minimizing the step-average power equals the freely chosen step frequency at about 13 km h-1: it is higher at lower speeds and lower at higher speeds. The frequency minimizing the push-average power approaches the freely chosen step frequency at high speeds (around 22 km h-1 for our subjects). 5. It is concluded that the increase of the vertical push does reduce the step-average power, but that a limit is set by the increase of the push-average power. Between 13 and 22 km h-1 the freely chosen step frequency is intermediate between a frequency minimizing the step-average power, eventually
