The most common etiologies of lymphoproliferation are represented by infectious diseases and lymphoid malignancies. However, it is increasingly recognized that lymphoproliferative features can be the presenting sign of rare conditions, including inborn errors of immunity (IEI) and inborn errors of metabolism (IEM). Among IEI, lymphoproliferation is frequently observed in autoimmune lymphoproliferative syndrome (ALPS) and related disorders, common variable immunodeficiency (CVID), activated phosphoinositide 3-kinase δ syndrome, and Epstein-Barr virus (EBV)-related disorders. Gaucher disease and Niemann-Pick disease are the most common IEMs that can present with isolated lymphoproliferative features. Notably, other rare conditions, such as sarcoidosis, Castleman disease, systemic autoimmune diseases, and autoinflammatory disorders, should be considered in the differential diagnosis of patients with persistent lymphoproliferation when infectious and malignant diseases have been reasonably ruled out. The clinical features of lymphoproliferative diseases, as well as the associated clinical findings and data deriving from imaging and first-level laboratory investigations, could significantly help in providing the correct diagnostic suspicion for the underlying etiology. This paper reviews the most relevant diseases associated with lymphoproliferation, including infectious diseases, hematological malignancies, IEI, and IEM. 3. MYC overexpression and SMARCA4 loss cooperate to drive medulloblastoma formation in mice

Abstract Group 3 medulloblastoma is one of the most aggressive types of childhood brain tumors. Roughly 30% of cases carry genetic alterations in MYC, SMARCA4, or both genes combined. While overexpression of MYC has previously been shown to drive medulloblastoma formation in mice, the functional significance of SMARCA4 mutations and their suitability as a therapeutic target remain largely unclear. To address this issue, we combined overexpression of MYC with a loss of SMARCA4 in granule cell precursors. Both alterations did not increase proliferation of granule cell precursors in vitro. However, combined MYC overexpression and SMARCA4 loss successfully induced tumor formation in vivo after orthotopic transplantation in recipient mice. Resulting tumors displayed anaplastic histology and exclusively consisted of SMARCA4-negative cells although a mixture of recombined and non-recombined cells was injected. These observations provide first evidence for a tumor-promoting role of a SMARCA4 deficiency in the development of medulloblastoma. In comparing the transcriptome of tumors to the cells of origin and an established Sonic Hedgehog medulloblastoma model, we gathered first hints on deregulated gene expression that could be specifically involved in SMARCA4/MYC driven tumorigenesis. Finally, an integration of RNA sequencing and DNA methylation data of murine tumors with human samples revealed a high resemblance to human Group 3 medulloblastoma on the molecular level. Altogether, the development of SMARCA4-deficient medulloblastomas in mice paves the way to deciphering the role of frequently occurring SMARCA4 alterations in Group 3 medulloblastoma with the perspective to explore targeted therapeutic options.

Abstract Pediatric high-grade gliomas of the subclass MYCN (HGG-MYCN) are highly aggressive tumors frequently carrying MYCN amplifications, TP53 mutations, or both alterations. Due to their rarity, such tumors have only recently been identified as a distinct entity, and biological as well as clinical characteristics have not been addressed specifically. To gain insights into tumorigenesis and molecular profiles of these tumors, and to ultimately suggest alternative treatment options, we generated a genetically engineered mouse model by breeding hGFAP-cre::Trp53 Fl/Fl ::lsl-MYCN mice. All mice developed aggressive forebrain tumors early in their lifetime that mimic human HGG-MYCN regarding histology, DNA methylation, and gene expression. Single-cell RNA sequencing revealed a high intratumoral heterogeneity with neuronal and oligodendroglial lineage signatures. High-throughput drug screening using both mouse and human tumor cells finally indicated high efficacy of Doxorubicin, Irinotecan, and Etoposide as possible therapy options that children with HGG-MYCN might benefit from.

The Intelligence of Hands Attributing artificiality solely to technological intelligences certainly means distinguishing their capabilities from those of the human mind, whose biological origin, however, runs the risk of becoming synonymous with an immutable nature. Adopting a palaeontological perspective, the contribution, conversely, seeks to draw attention to the close evolutionary link between our brains and the practices of the living body. Such a reflection is intended to recall not only the constructed nature of our intelligences, but also to show how, in removing the plasticity of the body, Cartesian Modernity was instead inspired by a machinic model of the brain that rendered the mind an immaterial and static entity. Therefore, trying to probe the presence of this approach between the lines of some conceptions of A.I. is a necessary premise both to read human intelligere in terms of a specific articulation of the artificial and to reposition its relationship with other machinic intelligences in the more horizontal perspective of a respectful heterogeneity.

6. On the dark matter origin of an LDMX signal

Fixed target experiments where beam electrons are focused upon a thin target have shown great potential for probing new physics, including the sub-GeV dark matter (DM) paradigm. However, a signal in future experiments such as the light dark matter experiment (LDMX) would require an independent validation to assert its DM origin. To this end, we propose to combine LDMX and next generation DM direct detection (DD) data in a four-step analysis strategy, which we here illustrate with Monte Carlo simulations. In the first step, the hypothetical LDMX signal (i.e. an excess in the final state electron energy and transverse momentum distributions) is $\textit{recorded}$. In the second step, a DM DD experiment operates with increasing exposure to test the DM origin of the LDMX signal. Here, LDMX and DD data are simulated. In the third step, a posterior probability density function (pdf) for the DM model parameters is extracted from the DD data, and used to $\textit{predict}$ the electron recoil energy and transverse momentum distributions at LDMX. In the last step, $\textit{predicted}$ and $\textit{recorded}$ electron recoil energy and transverse momentum distributions are compared in a chi-square test. We present the results of this comparison in terms of a threshold exposure that a DD experiment has to operate with to assert whether $\textit{predicted}$ and $\textit{recorded}$ distributions $\textit{can}$ be statistically dependent. We find that this threshold exposure grows with the DM particle mass, $m_\chi$. It varies from 0.012 kg-year for a DM mass of $m_\chi=4$ MeV to 1 kg-year for $m_\chi=25$ MeV, which is or will soon be within reach.

7. Large Language Models Reflect the Ideology of their Creators

Large language models (LLMs) are trained on vast amounts of data to generate natural language, enabling them to perform tasks like text summarization and question answering. These models have become popular in artificial intelligence (AI) assistants like ChatGPT and already play an influential role in how humans access information. However, the behavior of LLMs varies depending on their design, training, and use. In this paper, we uncover notable diversity in the ideological stance exhibited across different LLMs and languages in which they are accessed. We do this by prompting a diverse panel of popular LLMs to describe a large number of prominent and controversial personalities from recent world history, both in English and in Chinese. By identifying and analyzing moral assessments reflected in the generated descriptions, we find consistent normative differences between how the same LLM responds in Chinese compared to English. Similarly, we identify normative disagreements between Western and non-Western LLMs about prominent actors in geopolitical conflicts. Furthermore, popularly hypothesized disparities in political goals among Western models are reflected in significant normative differences related to inclusion, social inequality, and political scandals. Our results show that the ideological stance of an LLM often reflects the worldview of its creators. This raises important concerns around technological and regulatory efforts with the stated aim of making LLMs ideologically `unbiased', and it poses risks for political instrumentalization.

BACKGROUND: Rib fractures are one of the most common traumatic injuries and may result in significant morbidity and mortality. Despite growing evidence, technological advances and increasing acceptance, surgical stabilization of rib fractures (SSRF) remains not uniformly considered in trauma centers. Indications, contraindications, appropriate timing, surgical approaches and utilized implants are part of an ongoing debate. The present position paper, which is endorsed by the World Society of Emergency Surgery (WSES), and supported by the Chest Wall Injury Society, aims to provide a review of the literature investigating the use of SSRF in rib fracture management to develop graded position statements, providing an updated guide and reference for SSRF. METHODS: This position paper was developed according to the WSES methodology. A steering committee performed the literature review and drafted the position paper. An international panel of experts then critically revised the manuscript and discussed it in detail, to develop a consensus on the position statements. RESULTS: A total of 287 studies (systematic reviews, randomized clinical trial, prospective and retrospective comparative studies, case series, original articles) have been selected from an initial pool of 9928 studies. Thirty-nine graded position statements were put forward to address eight crucial aspects of SSRF: surgical indications, contraindications, optimal timing of surgery, preoperative imaging evaluation, rib fracture sites for surgical fixation, management of concurrent thoracic injuries, surgical approach, stabilization methods and material selection. CONCLUSION: This consensus document addresses the key focus questions on surgical treatment of rib fractures. The expert recommendations clarify current evidences on SSRF indications, timing, operative planning, approaches and techniques, with the aim to guide clinicians in optimizing the management of rib fractures, to improve patient outcomes and direct future research.

Furthermore, it has been reported that for every one-hundred patients admitted to hospital, seven patients in high-income economies and ten in emerging and low-income economies acquire at least one type of HCAI. Currently, almost all pathogenic microorganisms have developed antimicrobial resistance, and few new antimicrobials are being developed and brought to market. The literature search for this narrative review was performed by searching bibliographic databases (including Google Scholar and PubMed) using the search terms: “Strategies,” “Prevention,” and “Healthcare-Associated Infections,” followed by snowballing references cited by critical articles. We found that although hand hygiene is a centuries-old concept, it is still the primary strategy used around the world to prevent HCAIs. It forms one of a bundle of approaches used to clean and maintain a safe hospital environment and to stop the transmission of contagious and infectious microorganisms, including multidrug-resistant microbes. Finally, antibiotic stewardship also has a crucial role in reducing the impact of HCAIs through conserving currently available antimicrobials.Keywords: prevention, hand hygiene, environmental hygiene, surveillance, antibiotic stewardship

Machine learning models can inherit biases from their training data, leading to discriminatory or inaccurate predictions. This is particularly concerning with the increasing use of large, unsupervised datasets for training foundational models. Traditionally, demographic biases within these datasets have not been well-understood, limiting our ability to understand how they propagate to the models themselves. To address this issue, this paper investigates the propagation of demographic biases from datasets into machine learning models. We focus on the gender demographic component, analyzing two types of bias: representational and stereotypical. For our analysis, we consider the domain of facial expression recognition (FER), a field known to exhibit biases in most popular datasets. We use Affectnet, one of the largest FER datasets, as our baseline for carefully designing and generating subsets that incorporate varying strengths of both representational and stereotypical bias. Subsequently, we train several models on these biased subsets, evaluating their performance on a common test set to assess the propagation of bias into the models' predictions. Our results show that representational bias has a weaker impact than expected. Models exhibit a good generalization ability even in the absence of one gender in the training dataset. Conversely, stereotypical bias has a significantly stronger impact, primarily concentrated on the biased class, although it can also influence predictions for unbiased classes. Sub-GeV dark matter (DM) particles produced via thermal freeze-out evade many of the strong constraints on heavier DM candidates but at the same time face a multitude of new constraints from laboratory experiments, astrophysical observations and cosmological data. In this work we combine all of these constraints in order to perform frequentist and Bayesian global analyses of fermionic and scalar sub-GeV DM coupled to a dark photon with kinetic mixing. For fermionic DM, we find viable parameter regions close to the dark photon resonance, which expand significantly when including a particle-antiparticle asymmetry. For scalar DM, the velocity-dependent annihilation cross section evades the strongest constraints even in the symmetric case. Using Bayesian model comparison, we show that both asymmetric fermionic DM and symmetric scalar DM are preferred over symmetric fermionic DM due to the reduced fine-tuning penalty. Finally, we explore the discovery prospects of near-future experiments both in the full parameter space and for specific benchmark points. We find that the most commonly used benchmark scenarios are already in tension with existing constraints and propose a new benchmark point that can be targeted with future searches., Comment: 59 pages, 5 tables, 24 figures

The integration of renewable energy sources into the electricity system can contribute to the development of a low-carbon economy. However, due to the intermittency and non-programmability of these sources, problems related to the management of local electricity grids may occur. A possible solution or limitation to these issues is given by the electrical storage. In addition, in the next future, domestic micro-grids are expected to play a fundamental role in electric power networks, driving both the academic and industrial research interests in developing highly efficient and reliable conversion and storage technologies. In this study, the behavior of a small-scale hybrid energy system for hydrogen production and storage has been predicted, by means of a developed calculation model, and the operational strategy of the system has been optimized with the aim to maximize the hydrogen production. In addition, with the aim to maximize the overall solar-to-hydrogen chain efficiency, the whole system model has been applied to different operating scenarios, to identify the optimal management strategy to control it.

15. Dark Matter-induced electron excitations in silicon and germanium with Deep Learning

We train a deep neural network (DNN) to output rates of dark matter (DM) induced electron excitations in silicon and germanium detectors. Our DNN provides a massive speedup of around $5$ orders of magnitude relative to existing methods (i.e. QEdark-EFT), allowing for extensive parameter scans in the event of an observed DM signal. The network is also lighter and simpler to use than alternative computational frameworks based on a direct calculation of the DM-induced excitation rate. We combine the non-relativistic effective theory of dark matter (DM) - electron interactions with linear response theory to obtain a formalism that fully accounts for screening and collective excitations in DM-induced electronic transition rate calculations for general DM-electron interactions. In the same way that the response of a dielectric material to an external electric field in electrodynamics is described by the dielectric function, so in our formalism the response of a detector material to a DM perturbation is described by a set of generalised susceptibilities which can be directly related to densities and currents arising from the non-relativistic expansion of the Dirac Hamiltonian. We apply our formalism to assess the sensitivity of non-spin-polarised detectors, and find that in-medium effects significantly affect the experimental sensitivity if DM couples to the detector's electron density, while being decoupled from other densities and currents. BACKGROUND: The trauma mortality rate is higher in the elderly compared with younger patients. Ageing is associated with physiological changes in multiple systems and correlated with frailty. Frailty is a risk factor for mortality in elderly trauma patients. We aim to provide evidence-based guidelines for the management of geriatric trauma patients to improve it and reduce futile procedures. METHODS: Six working groups of expert acute care and trauma surgeons reviewed extensively the literature according to the topic and the PICO question assigned. Statements and recommendations were assessed according to the GRADE methodology and approved by a consensus of experts in the field at the 10th international congress of the WSES in 2023. RESULTS: The management of elderly trauma patients requires knowledge of ageing physiology, a focused triage, including drug history, frailty assessment, nutritional status, and early activation of trauma protocol to improve outcomes. Acute trauma pain in the elderly has to be managed in a multimodal analgesic approach, to avoid side effects of opioid use. Antibiotic prophylaxis is recommended in penetrating (abdominal, thoracic) trauma, in severely burned and in open fractures elderly patients to decrease septic complications. Antibiotics are not recommended in blunt trauma in the absence of signs of sepsis and septic shock. Venous thromboembolism prophylaxis with LMWH or UFH should be administrated as soon as possible in high and moderate-risk elderly trauma patients according to the renal function, weight of the patient and bleeding risk. A palliative care team should be involved as soon as possible to discuss the end of life in a multidisciplinary approach considering the patients directives, family feelings and representatives desires, and all decisions should be shared. CONCLUSIONS: The management of elderly trauma patients requires knowledge of ageing physiology, a focused triage based on assessing frailty and early activation of trauma protocol to improve outcomes. Geriatric Intensive Care Units are needed to care for elderly and frail trauma patients in a multidisciplinary approach to decrease mortality and improve outcomes.

In the last few years, Artificial Intelligence systems have become increasingly widespread. Unfortunately, these systems can share many biases with human decision-making, including demographic biases. Often, these biases can be traced back to the data used for training, where large uncurated datasets have become the norm. Despite our knowledge of these biases, we still lack general tools to detect and quantify them, as well as to compare the biases in different datasets. Thus, in this work, we propose DSAP (Demographic Similarity from Auxiliary Profiles), a two-step methodology for comparing the demographic composition of two datasets. DSAP can be deployed in three key applications: to detect and characterize demographic blind spots and bias issues across datasets, to measure dataset demographic bias in single datasets, and to measure dataset demographic shift in deployment scenarios. An essential feature of DSAP is its ability to robustly analyze datasets without explicit demographic labels, offering simplicity and interpretability for a wide range of situations. To show the usefulness of the proposed methodology, we consider the Facial Expression Recognition task, where demographic bias has previously been found. In order to pursue the goals of de-carbonization and renewable sources exploitation, an efficient, cost-effective and environmental friendly employment of energy systems is mandatory. In this context, the ERA-Net funded Project ZEHTC (Zero Emission Hydrogen Turbine Center) has been developed, aimed at the realization of a pilot plant consisting in the world first gas turbine test facility making use of the power produced during turbine tests – along with renewables (solar in particular) – for hydrogen production and batteries storage. The produced hydrogen will be locally used to run the gas turbine, thus reducing its environmental impact. The aim of this paper is to present the results of the first steps of the project, consisting in the definition of an optimized grid and in the development of a calculation model for the optimal energy systems design. To this respect, the typology of energy systems to be included into the grid (in addition to the current set-up of the existing turbine test center) will be presented. In addition, the mathematical models specifically developed for each component, as well as the developed operational logic of the entire grid, will be presented and discussed in order to set the optimal size of each systems.

Abstract Background The quality of Big Data analysis in medicine and surgery heavily depends on the methods used for clinical data collection, organization, and storage. The Knowledge Graph (KG) represents knowledge through a semantic model, enhancing connections between diverse and complex information. While it can improve the quality of health data collection, it has limitations that can be addressed by the Decentralized (blockchain-powered) Knowledge Graph (DKG). We report our experience in developing a DKG to organize data and knowledge in the field of emergency surgery. Methods and results The authors leveraged the cyb.ai protocol, a decentralized protocol within the Cosmos network, to develop the Emergency Surgery DKG. They populated the DKG with relevant information using publications from the World Society of Emergency Surgery (WSES) featured in the World Journal of Emergency Surgery (WJES). The result was the Decentralized Knowledge Graph (DKG) for the WSES-WJES bibliography. Conclusions Utilizing a DKG enables more effective structuring and organization of medical knowledge. This facilitates a deeper understanding of the interrelationships between various aspects of medicine and surgery, ultimately enhancing the diagnosis and treatment of different diseases. The system’s design aims to be inclusive and user-friendly, providing access to high-quality surgical knowledge for healthcare providers worldwide, regardless of their technological capabilities or geographical location. As the DKG evolves, ongoing attention to user feedback, regulatory frameworks, and ethical considerations will be critical to its long-term success and global impact in the surgical field.

Third Space. Kant’s phenomenal and relativistic Position This paper shows a comparison and try to track down some points of contact between several Kantian assumptions about space and some elements from the formulation of Einstein’s Relativity. By operating a significant shift to his contemporary geometry, the young Kant believes directing as an internal property of the figure. Starting from this premise and through the famous example of incongruous opposites, the philosopher shows that the inner reason of directive diversity between hands leads to the need for a new concept of space. It will be called relative and phenomenal this third type of space that Kant aims. Related and non-relational because it results from the immediate relationship that a certain object has with the sides of our body. Einstein’s space exhibits not only a relative nature, but also a non-Euclidean character having its determined geometry determined from masses and their speed, from the characters of dynamism, of discontinuity and variability. In this direction, you can find some interesting similarities with Kant.

42. Spin-1 Thermal Targets for Dark Matter Searches at Beam Dump and Fixed Target Experiments

The current framework for dark matter searches at beam dump and fixed target experiments relies on four benchmark models, the complex scalar, inelastic scalar, pseudo-Dirac and finally, Majorana DM models. While this approach has so far been successful in the interpretation of the available data, it a priori excludes the possibility that DM is made of spin-1 particles -- a restriction which is neither theoretically nor experimentally justified. In this work we extend the current landscape of sub-GeV DM models to a set of models for spin-1 DM, including a family of simplified models (involving one DM candidate and one mediator -- the dark photon) and an ultraviolet complete model based on a non-abelian gauge group where DM is a spin-1 Strongly Interacting Massive Particle. For each of these models, we calculate the DM relic density, the expected number of signal events at beam dump experiments, the rate of energy injection in the early universe thermal bath and in the Intergalactic Medium, as well as the helicity amplitudes for forward processes subject to the unitary bound. We then compare these predictions with experimental results from Planck, CMB surveys, IGM temperature observations, LSND, MiniBooNE, NA64, and BaBar and with available projections from LDMX and Belle II. Through this comparison, we identify the regions in the parameter space of the models considered in this work where DM is simultaneously thermally produced, compatible with present observations, and within reach at Belle II and LDMX. We find that the simplified models are strongly constrained by current beam dump experiments and the unitarity bound, and will thus be conclusively probed in the first stage of LDMX data taking. We also find that the SIMP model explored in this work predicts the observed DM abundance, is compatible with current observations and within reach at LDMX in a wide region of the parameter space.

BACKGROUND: Laparoscopy is widely adopted across nearly all surgical subspecialties in the elective setting. Initially finding indication in minor abdominal emergencies, it has gradually become the standard approach in the majority of elective general surgery procedures. Despite many technological advances and increasing acceptance, the laparoscopic approach remains underutilized in emergency general surgery and in abdominal trauma. Emergency laparotomy continues to carry a high morbidity and mortality. In recent years, there has been a growing interest from emergency and trauma surgeons in adopting minimally invasive surgery approaches in the acute surgical setting. The present position paper, supported by the World Society of Emergency Surgery (WSES), aims to provide a review of the literature to reach a consensus on the indications and benefits of a laparoscopic-first approach in patients requiring emergency abdominal surgery for general surgery emergencies or abdominal trauma. METHODS: This position paper was developed according to the WSES methodology. A steering committee performed the literature review and drafted the position paper. An international panel of 54 experts then critically revised the manuscript and discussed it in detail, to develop a consensus on a position statement. RESULTS: A total of 323 studies (systematic review and meta-analysis, randomized clinical trial, retrospective comparative cohort studies, case series) have been selected from an initial pool of 7409 studies. Evidence demonstrates several benefits of the laparoscopic approach in stable patients undergoing emergency abdominal surgery for general surgical emergencies or abdominal trauma. The selection of a stable patient seems to be of paramount importance for a safe adoption of a laparoscopic approach. In hemodynamically stable patients, the laparoscopic approach was found to be safe, feasible and effective as a therapeutic tool or helpful to identify further management steps and needs, resulting in improved outcomes, regardless of conversion. Appropriate patient selection, surgeon experience and rigorous minimally invasive surgical training, remain crucial factors to increase the adoption of laparoscopy in emergency general surgery and abdominal trauma. CONCLUSIONS: The WSES expert panel suggests laparoscopy as the first approach for stable patients undergoing emergency abdominal surgery for general surgery emergencies and abdominal trauma.

Enhanced perioperative care protocols become the standard of care in elective surgery with a significant improvement in patients outcome. The key element of the enhanced perioperative care protocol is the multimodal and interdisciplinary approach targeted to the patient, focused on a holistic approach to reduce surgical stress and improve perioperative recovery. Enhanced perioperative care in emergency general surgery is still a debated topic with little evidence available. The present position paper illustrates the existing evidence about perioperative care in emergency surgery patients with a focus on each perioperative intervention in the preoperative, intraoperative and postoperative phase. For each item was proposed and approved a statement by the WSES collaborative group.
