Abstract

Background: Ailments such as diarrhoea and antibiotic-associated gut symptoms are generally self-managed using probiotics. Real-world data on reasons behind self-medication with over-the-counter (OTC) products and patient-reported outcomes can be investigated strategically by the pharmacists. Objective: This study evaluates the use of Bacillus clausii (Enterogermina®) at the Italian community pharmacies among self-medicating patients, their treatment habits and perceived benefits. Design: This is a multicentre, prospective, non-interventional study which included two visits [at screening (T0) and end of the study (T1) when symptoms had subsided, ≤ 30 days from T0]. Patients who were already inclined to buy B. clausii were enrolled and instructed to complete a questionnaire at T0 and T1. The primary objective was to evaluate the reasons for taking B. clausii. Secondary objectives assessed treatment duration, perceived effectiveness, quality of life (QoL), treatment satisfaction and safety outcomes. Results: Overall, 268 patients were enrolled; 99.6% of them were evaluated at T0 and 97.4% at T1, and safety was evaluated in 97.8% who had ≥ 1 dose of B. clausii. At T0, mean age was 50.7 years and majority were females (62.2%). In the interview, main reason stated for using B. clausii at T0 was diarrhoea (56.93%), followed by other gastrointestinal symptoms. Treatment duration was shorter in those with diarrhoea or abdominal pain versus those with constipation or abdominal tension. More than 90% perceived their symptoms to have improved or improved very much. Overall QoL improved in all the aspects measured. Treatment satisfaction was reported by nearly 90% of patients as satisfied, very satisfied or extremely satisfied. No adverse events were reported. Conclusion: This is the first pharmacy-based study in Italy that evaluated the real-world usage of an OTC probiotic containing B. clausii among self-medicating adults. Abstract

BACKGROUND Baveno-VI guidelines recommend that patients with compensated cirrhosis with liver stiffness by transient elastography (LSM-TE) 150,000/mm do not need an esophagogastroduodenoscopy (EGD) to screen for varices, since the risk of having varices needing treatment (VNT) is 110,000/mm), and other criteria in predicting the absence of VNT. METHODS This was a multicenter cross-sectional study in four referral hospitals. We retrospectively analyzed data from 227 patients with compensated advanced chronic liver disease (cACLD) due to PBC (n = 147) and PSC (n = 80) that had paired EGD and LSM-TE. We calculated false negative rate (FNR) and number of saved endoscopies for each prediction rule. RESULTS Prevalence of VNT was 13%. Baveno-VI criteria had a 0% FNR in PBC and PSC, saving 39 and 30% of EGDs, respectively. In PBC the other LSM-TE-based criteria resulted in FNRs >5%. In PSC the expanded Baveno criteria had an adequate performance. In both conditions LSM-TE-independent criteria resulted in an acceptable FNR but saved less EGDs. CONCLUSIONS Baveno-VI criteria can be applied in patients with cACLD due to ChLDs, which would result in saving 30-40% of EGDs. Expanded criteria in PBC would lead to FNRs >5%.

Abstract

Background & Aims: Controlled attenuation parameter (CAP) for steatosis assessment has not been validated in compensated advanced chronic liver disease compensated advanced chronic liver disease (cACLD). We primarily aimed at assessing the accuracy of CAP for the diagnosis and quantification of steatosis in cACLD. Secondary aim: to assess the validity of non‐invasive criteria for cACLD according to liver stiffness measurement (LSM). Methods: This is a single‐centre retrospective study including patients with cACLD defined as LSM ≥10 kPa, CAP measurement and liver biopsy (reference standard for steatosis and fibrosis) observed in 06/2015‐06/2017. Steatosis was graded as S0 (<5%), S1 (5%‐32%), S2 (33%‐66%) and S3 (>66%). The diagnostic performance of CAP for any grade of steatosis and for high‐grade steatosis (≥S2) was studied. Results: Among 461 consecutive patients, 111 with LSM‐based diagnosis of cACLD were included (63% male, median age 55 years, median body mass index 28.1 Kg/m2, aetiology: 32% non‐alcoholic fatty liver disease/non‐alcoholic steatohepatitis, 32% alcohol or viral + metabolic syndrome, 15% viral, 6% autoimmune, 4% alcohol, 11% others). Median LSM and CAP were 16.1 kPa and 277 dB/m respectively. On liver biopsy, steatosis was found in 88/111 patients (79%); 44 patients (43 with metabolic syndrome) had high‐grade steatosis. CAP was accurate in identifying any grade of steatosis (area under the receiving operating characteristic curves 0.847; 95% CI 0.767‐0.926, P <.0001), and ≥S2 steatosis (0.860; 95% CI 0.788‐0.932, P <.0001). CAP performed similarly in patients with CAP‐ interquartile range (IQR) ≥ or <40 dB/m. Conclusions: Steatosis is frequent in patients with cACLD and metabolic syndrome. CAP diagnostic accuracy for any steatosis and high‐grade steatosis is good in this population. A CAP‐IQR ≥40 dB/m does not impair CAP diagnostic accuracy in cACLD. [ABSTRACT FROM AUTHOR]

18. Optimizing H2Production from Waste Tires via Combined Steam Gasification and Catalytic Reforming

Abstract

The disposal of waste tires represents a relevant problem within the waste management strategy of the European community: more than 300 000 000 tires are estimated to reach their end of life each year in the 27 member states of the European Union and comparable amounts are found in North America, Latin America, Asia, and the Middle East. The global total is ∼1 000 000 000 and rising each year. (Source: European Tyre Recycling Association (ETRA), 2006.) It is well-known that scrap tires possess high volatiles and low ash contents, together with a heating value that is higher than coal and biomass. These properties make them an ideal material for alternative thermal processes, such as pyrolysis and gasification, which can be finalized both to energy and material recovery. Within this frame, the present work is related to experimental tests and has obtained results of a combined process of scrap tire steam gasification and syngas catalytic reforming, with the aim of exploring the possible utilization of syngas for fuel cell applications. Four catalysts have been used for the experimental tests: two natural mineral products (olivine and dolomite) and two commercial nickel-based catalysts. Experimental data show that whether olivine or dolomite is used directly into the reactor to carry out the steam gasification, the char and gas yields increase with respect to the sole tire gasification (the char production varies from 41.2% w/w without catalysts to 59% w/w and 47.9% w/w, using olivine and dolomite, respectively; the gas production varies from 60.8% w/w for the sole tire gasification to 63.5% w/w with olivine and 84% w/w with dolomite). Then, while the olivine shows a stronger effect on the char production, the dolomite seems to be more effective on the gas yield. Moreover, both the catalysts promote a higher hydrogen production, which varies from 51.6 vol % for the sole tire gasification to 65.6 vol % and 57 vol % using, respectively, dolomite or olivine, basically because of the enhanced cracking of methane and the other hydrocarbons (the methane content decreases from 27.6 vol % for the sole tire gasification to 11.3 vol % and 20.8 vol %, using dolomite or olivine, respectively). Regarding both dry and steam reforming, the experimental tests show that the catalytic step, tested by varying the temperature, the catalytic substrates, and the reactive atmosphere, promote the production of a high hydrogen-rich gas, already at the lower tested temperature. It has been seen that the stronger effect for the increase of hydrogen content is for steam reforming condition and using a commercial nickel catalyst instead of Ni/olivine: under such conditions, the hydrogen content increases, from 51.6 vol % before the reforming up to 78 vol % at 650 °C. With regard to gas production, a strong increase of the flow, mostly due to the effect of the cracking reactions, is registered as well and, more in detail, the gas production increases from 0.8 m3kg–1fed, before the reforming, up to 1.0 m3kg–1fed and 1.5 m3kg–1fed, respectively, for dry and steam reforming at 750 °C and using Ni olivine catalyst, and up to 1.12 m3kg–1fed and 1.91 m3kg–1fed, for dry and steam reforming at 750 °C, respectively, and using a commercial Ni catalyst. The adopted operating conditions allow one to obtain an appreciable amount of char, whose high carbon content suggest its further exploitation both as activated carbon (after activation process) and as a carbon source for synthesis reactions.

It remains uncertain if this tool can be used in patients with cholestatic liver diseases (ChLDs): primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC). These patients may have a pre-sinusoidal component of portal hypertension that could affect the performance of this rule. In this study we evaluated the performance of Baveno-VI, expanded Baveno-VI (LSM-TE <25 kPa and platelets >110,000/mm(3)), and other criteria in predicting the absence of VNT., Methods: This was a multicenter cross-sectional study in four referral hospitals. We retrospectively analyzed data from 227 patients with compensated advanced chronic liver disease (cACLD) due to PBC (n = 147) and PSC (n = 80) that had paired EGD and LSM-TE. We calculated false negative rate (FNR) and number of saved endoscopies for each prediction rule., Results: Prevalence of VNT was 13%. Baveno-VI criteria had a 0% FNR in PBC and PSC, saving 39 and 30% of EGDs, respectively. In PBC the other LSM-TE-based criteria resulted in FNRs >5%. In PSC the expanded Baveno criteria had an adequate performance. In both conditions LSM-TE-independent criteria resulted in an acceptable FNR but saved less EGDs., Conclusions: Baveno-VI criteria can be applied in patients with cACLD due to ChLDs, which would result in saving 30-40% of EGDs. Expanded criteria in PBC would lead to FNRs >5%.
