Background: Allergic rhinitis (AR) is a highly prevalent disease. We aimed to assess the symptoms that physicians who see patients with AR perceive as the most bothersome in their patients. Methods: We performed a cross-sectional study based on an online questionnaire sent to all members of the Allergic Rhinitis and its Impact on Asthma (ARIA) initiative. The survey included questions on the physicians' perceptions of patients’ AR symptoms as well as of their own AR symptoms. Results: Among 401 respondents, 155 (38.7%) reported having AR. ARIA members reported nasal symptoms to be the most frequent (89.7%) and bothersome (80.0%) symptoms experienced by themselves. Likewise, nasal symptoms were reported by ARIA members as the most frequent (94.8% in members with AR vs 96.0% in members without AR) and bothersome (57.0% in members with AR vs 67.9% in members without AR) in their patients. We found a significant association (p = 0.001) between physicians’ own symptoms and those perceived as the most bothersome in their patients. Conclusion: Physicians perceive nasal symptoms to be the most frequent and the most bothersome symptoms in AR patients. The physicians' personal experiences with AR may influence their perception of patients’ symptoms.

In the present work the temperature effect on rheological properties of Portuguese honeys was studied for the first time. Two unifloral honeys – heather and rosemary – and a polyflower honey were analyzed. All honeys showed flow independence over time and behaved as Newtonian fluids at the studied temperature and shear rate ranges. For all honeys it was found that the viscosity decreased with temperature and the rosemary honey was the one that always presented the lowest viscosity at 30°C (6120 mPa·s), 50°C (603 mPa·s) and 70°C (145 mPa·s). The temperature dependence of viscosity was well described by the K0×eAT-B$K_0 \times e^{{A \over {T - B}}} $ equation. Nevertheless, good regression coefficients were also obtained when fitting the experimental data to the Arrhenius model, showing the rosemary honey to be less temperature sensitive. The results obtained in this study are of great interest to beekeepers and industrials that handle and prepare eatable honey-based products because they will be better informed about the best type of honey to use.

Background: Aedes aegypti is the main vector of the virus causing Dengue fever, a disease that has increased dramatically in importance in recent decades, affecting many tropical and sub-tropical areas of the globe. It is known that viruses and other parasites can potentially alter vector behavior. We investigated whether infection with Dengue virus modifies the behavior of Aedes aegypti females with respect to their activity level. Methods/Principal Findings: We carried out intrathoracic Dengue 2 virus (DENV-2) infections in Aedes aegypti females and recorded their locomotor activity behavior. We observed an increase of up to ∼50% in the activity of infected mosquitoes compared to the uninfected controls. Conclusions: Dengue infection alters mosquito locomotor activity behavior. We speculate that the higher levels of activity observed in infected Aedes aegypti females might involve the circadian clock. Further studies are needed to assess whether this behavioral change could have implications for the dynamics of Dengue virus transmission. [ABSTRACT FROM AUTHOR]

Research background: Competition laws have become widespread and as a rule have a sanctioning nature. DANVERS refers an exponential increase in legal systems that introduced competition laws in recent years, accounting for more than 127 jurisdictions hereto. Specifically concerning the trend of criminalizing cartels, SHAFFER and NESBITT consider it due to transnational interests, and SÁNCHEZ defends a link between the economic subsystem as the engine of globalization and the economic law with the latter – where economic criminal law, competition law, financial markets law, among others are included - being subject to strong international influences, and possibly being moreover a sign of democracy. More than 30 jurisdictions already have criminalized cartels and STEPHAN mentions that about 60 jurisdictions already present internal legal solutions combining a reinforcement of sanctions with the figure of leniency, in a “carrot and stick” approach to destabilizing and deterring cartels. Purpose of the article: Considering the cartel criminalization trend in competition law, it is paramount to identify the underlying legal interests as a (global) recognized fundamental value, even if there are particularities adjusted to each legal order, and the purpose of the article is to identify the legal interests and adscription criteria that may be considered and retained as common in competition enforcement. Methods: normative analyses of the law. Findings & Value added: To discuss how to harmonize, in an international legal environment, competition enforcement.

We report the case of a 29-year old female who was evaluated because of a thyroid tumor. The initial pathological classification was an insular thyroid carcinoma. There was strong involvement in the neck, mediastinum and lungs. Three years after receiving specific therapy for her thyroid neoplasia, she developed a Cushing syndrome and liver lesions suggestive of metastases from the primary tumor. A review of the previous pathological material revealed a medullary thyroid carcinoma producing ACTH, instead of the insular carcinoma. Based on this case a review of the literature is presented.

Through examination of specimens collected in 12 locations confirmed the presence of the genus Latrodectus distinguishing six morphotypes in six departments and four ecoregions of Bolivia. The results imply that there is a risk of accidents caused by the bite of these spiders.

The two linear position sensors used to determine the position of the European Organization for Nuclear Research; Large Hadron Collider collimator’s jaws with respect to the beam are the linear variable differential transformer and the ironless inductive position sensor. The latter was designed as an alternative to the former since the linear variable differential transformer exhibits a position error in magnetic environments. The ironless inductive position sensor is an air cored, high-precision linear position sensor, which is by design immune to external DC or slowly varying magnetic fields. Since the ironless inductive position sensor is required to have no on-board electronics, the raw signal has to be carried through long cable lengths and this may lead to performance degradation. This paper focuses on a set of experimental measurements conducted to assess the ironless inductive position sensor’s sensitivity at different frequencies with long cable lengths. This is critical for the sensor`s correct operation in the Large Hadron Collider`s collimators. Furthermore, to gain a better understanding, the ironless inductive position sensor’s frequency response is compared with a commercial off-the-shelf linear variable differential transformer.

Abstract: There are no test standarizaded for the antifungal susceptibility of dermatophytes. The aim of this study is to probe the well diffusion technique as antifungal susceptibility test for dermatophites. One hundred species of dermatophytes were isolated and identified (74 Trichophyton rubrum, 18 Trichophyton mentagrophytes, 1 Trichophyton tonsurans, 5 Microsporum canis and 2 Epidermophyton floccosum).in the Medical Micology Departament of TMI. The antifungal sensitivity of the isolates of the dermatophytes to the drugs was carried out using the well diffusion technique. The antifungal agents tested were griseofulvin, terbinafine, itraconazole and fluconazole. Results were interpreted according to NCCLS breakpoints. The inoculum suspension was standardized by weight measurement. Agar Staib was considered the most suitable, because 97% of the isolates grew on it. This is the first time the inoculum suspension of dermatophytes is standardized by weight measurement. Terbinafine was found to be the most effective drug, followed by Griseofulvin and Itraconazole. Fluconazole were not found to be effective. Resumen: No existe hasta los momentos una prueba estandarizada para realizar la susceptibilidad antifúngica de los dermatofitos. El objetivo de este estudio fue implementar la técnica de los pozos de difusión para el estudio de la susceptibilidad de los dermatofitos. Se utilizaron un total de 100 aislados de dermatofitos (74 Trichophyton rubrum, 18 Trichophyton mentagrophytes, 1 Trichophyton tonsurans, 5 Microsporum canis y 2 Epidermophyton floccosum) provenientes de muestras clínicas aisladas e identificadas en la Sección de Micología Médica, IMT. Se empleó la técnica de los pozos de difusión con los siguientes antimicóticos: Griseofulvina, Terbinafina, Fluconazol e Itraconazol. Se aplicaron los criterios de interpretación acordados por la NCCLS. El inóculo, se estandarizó utilizando el peso como unidad. El medio Agar Staib demostró ser el más adecuado, al obtenerse un mejor crecimiento de los aislados en un 97 %. Esta es la primera vez, que se utiliza el peso como medida para cuantificar el inóculo en los dermatofitos. La mayor sensibilidad se evidenció ante Terbinafina seguido de Griseofulvina e Itraconazol. No se demostró sensibilidad alguna ante el Fluconazol. Sin embargo, se requieren más estudios en los cuales se relacione la susceptibilidad “in vitro” con los resultados “in vivo”.

Este estudo visa determinar a freqüência, correlacionar possíveis agentes causais e monitorizar a ocorrência de malformações múltiplas na população de Pelotas, Rio Grande do Sul, Brasil. Abrange todos os nascimentos ocorridos entre 1990 e 2002 nas maternidades de Pelotas, com peso superior a 500g. Para cada recém-nascido malformado (caso), tomou-se um neonato vivo (controle), pareado a ele, sem malformação e de igual sexo. Formou-se um banco de dados mediante o preenchimento dos formulários-modelo ECLAMC - MONITOR edição 1982, que foram tabulados pelo programa SPSS. Para a análise estatística, utilizou-se o Teste t de Student e chi2. No período em estudo nasceram 71.500 crianças. Dentre essas, 0,11% recém-nascidos apresentaram malformações múltiplas. Foram encontrados resultados significativos para o peso, gemelaridade e nascimento, a etnia dos antepassados, a idade paterna, o número de abortos e natimortos prévios. Em Pelotas, a ocorrência de recém-nascidos que apresentavam malformações ao nascer, no período do estudo, foi de 1,37%. A freqüência de recém-nascidos com malformações múltiplas é de 8,1%, predominantemente no sexo feminino e nos nascimentos ocorridos no inverno.

Descrevem-se o estado nutricional, o padrão de consumo alimentar, a ingestão energética (IE) e a freqüência da atividade física de lazer de 65 operários (19 a 58 anos de idade) do sexo masculino na perspectiva de construção de um sistema de vigilância (SISVAN) numa empresa de metalurgia do Rio de Janeiro, Brasil. O IMC médio foi de 25,0kg/m² (44,6% tinham sobrepeso - SP), a IE, de 3.209,1kcal/dia com 17,8; 21,8 e 60,6% de proteínas, lipídios e glicídios, respectivamente. Os alimentos mais freqüentemente consumidos foram arroz, feijão, pão, café, açúcar, manteiga, suco, leite, refrigerante, bife e frango. O consumo de leguminosas, hortaliças, frutas, carnes e ovos ultrapassou o recomendado. Dos trabalhadores com SP, 79,0% apresentaram balanço energético (BE) negativo. Os resultados mostraram um paradoxo a ser enfrentado pelo SISVAN: alta prevalência de SP, mas com BE negativo e com dieta com padrão qualitativo bem variado; e, ainda, prática de alguma atividade física de lazer por metade dos trabalhadores. Pode ser que esse aparente paradoxo seja causado por superestimativa dos requerimentos energéticos ou subestimativa da ingestão, fatores que não podem ser determinados na presente análise.

OBJETIVO: apresentar os achados ginecológicos mais freqüentes de uma coorte de 300 mulheres infectadas pelo HIV, acompanhadas ambulatorialmente. MÉTODOS: estudo prospectivo e descritivo de mulheres HIV-soropositivas atendidas em clínica ginecológica, no período de novembro de 1996 a dezembro de 2002. As pacientes foram submetidas ao protocolo de atendimento ginecológico que incluiu anamnese, exame ginecológico, colpocitologia oncótica (Papanicolaou), pesquisa de HPV (PCR) e colposcopia. A biópsia cervical, quando necessária, foi realizada. Os dados foram armazenados e analisados no Epi-Info, versão 6.0. RESULTADOS: a idade média foi 34,5 anos. Destaca-se o pequeno número de parceiros sexuais, média de três parceiros, e a predominância do contágio heterossexual: 271 (90,6%) pacientes adquiriram o vírus por meio do contato sexual com seus parceiros. Foi alta a prevalência de neoplasias intra-epiteliais cervicais (NIC), representando 21,7% do total da amostra. Dentre as 109 pacientes submetidas a PCR encontraram-se 89 (81,7%) com algum genótipo do HPV. Esfregaço inflamatório à citologia esteve presente em 69% das pacientes. CONCLUSÕES: a infecção pelo HIV se associa com freqüência a NIC e a processos infecciosos genitais, em especial o HPV.

The 2,2'-bipyridine has been entitled as the most widely used ligand. Nowadays there is a large variety of known molecules comprising at least two 2,2'-bipyridine units and the number of applications in many areas such as catalysis, new materials, optoeletronics and electrochemistry have increased very much in the past decades. Nevertheless, there is no article that gives an overview of the main synthetic methods for obtaining the substituted 2,2'-bipyridines, generally non available. This article presents a synthetic discussion about the three different methods (coupling reaction, ciclo-functionalization and functionalization of the heteroaromatic rings of 2,2'-bipyridine) for preparing these heterocyclic compounds and also provides a practical and fundamental guide, for obtaining more than eighty different symmetric and unsymmetrical substituted 2,2'-bipyridines, shown in a table with the corresponding references.

Neste artigo são examinadas algumas dimensões centrais na vida dos jovens de 15 a 24 anos, apreendidas em pesquisa coordenada pela Unesco, sob o título Cultivando vidas. Desarmando violências. Tais dimensões foram consideradas centrais, de acordo com o acervo de trabalhos sobre jovens em situações de pobreza no Brasil, informações oficiais disponíveis e o que sentem os próprios jovens, pais e educadores. A amostra constitui-se de jovens residentes em capitais e em alguns outros municípios. A análise macrorreferenciada é entremeada de discussões dos agentes sobre o sentido, percepção e importância das dimensões examinadas¹.

OBJETIVO: Uma das principais preocupações mundiais quanto ao uso de medicamentos está relacionada à utilização de antimicrobianos. Nos países em desenvolvimento, poucos recursos são empregados na monitorização de ações sobre o uso racional de medicamentos. São, também, limitados os dados sobre o uso daqueles agentes em hospitais. Assim realizou-se estudo para determinar os padrões de uso de antimicrobianos em hospital num período de sete anos (1990 a 1996). MÉTODOS: O estudo foi realizado em um hospital universitário, terciário, com 690 leitos, localizado em Porto Alegre, RS. Os registros hospitalares foram revisados visando identificar o consumo de antimicrobianos por pacientes internados, sendo os resultados expressos em dose diária definida (DDD) por 100 leitos-dia. A análise de conglomerados foi realizada para determinar as tendências de consumo dos agentes individuais. RESULTADOS: O consumo de antimicrobianos aumentou com o correr dos anos, passando de 83,8 DDD por 100 leitos-dia, em 1990, a 124,58 DDD por 100 leitos-dia em 1996. O grupo de medicamentos mais utilizado foi de penicilinas (39,6%), seguido por cefalosporinas (15,0%), aminoglicosídeos (14,4%), sulfonamidas (12,8%), glicopeptídeos (3,6%) e lincosaminas (3,1%). Estes grupos foram responsáveis por cerca de 90% do consumo. A análise de conglomerados do uso de antimicrobianos mostrou 13 grupos principais de tendência de consumo. CONCLUSÕES: O consumo de antimicrobianos cresceu no período avaliado, sendo expressivamente mais alto em comparação com o relatado em outros estudos. Quando novas alternativas terapêuticas foram disponibilizadas no hospital, o uso de medicamentos antigos decresceu e, em alguns casos, existiu manutenção dos níveis de consumo. Quando foi realizada intervenção específica como uma campanha para o uso correto de cefoxitina, as mudanças esperadas ocorreram.

From fruits of Pterodon polygalaeflorus (Leguminosae) the new diterpene 6alpha-hydroxyvouacapane, the known diterpenoids vouacapane-6alpha,7beta,14beta,19-tetraol and methyl 6alpha,7beta-dihydroxyvouacapan-17b-oate and the flavonoid taxifolin were isolated. Structural determinations were accomplished by spectroscopic analysis, including two-dimensional NMR, chemical transformation of the new diterpene and comparison with literature data.

FUNDAMENTO: Chlamydia trachomatis es actualmente el patógeno más prevalente de los causantes de Enfermedades de transmisión sexual en países desarrollados, alcanzando en algunos de ellos proporciones epidémicas. Dichas infecciones pueden dar lugar a un considerable número de complicaciones, muchas de ellas de importantes consecuencias. Dado que además, un gran número de casos cursan de manera asintomática, existe un consenso generalizado en la conveniencia de realizar programas de cribaje. El objetivo de este estudio es llevar a cabo la primera fase en la evaluación del programa de cribaje de infección asintomática por C. trachomatis en las mujeres que acuden a un centro de planificación familiar. MÉTODOS: Estudio descriptivo para determinar la prevalencia de cada año del período estudiado, análisis de la tendencia temporal de la misma y valoración de la efectividad del programa en términos de reducción proporcional de prevalencia. Se incluyeron un total de 6.746 mujeres que acudieron al centro de planificación familiar para revisión ordinaria o consejo de anticoncepción entre enero de 1991 y diciembre de 1993. RESULTADOS: El programa mostró una efectividad global del 80%, reduciéndose la prevalencia de 5.1% en el año 90 a 1.0% en el año 93. Se observó un descenso significativo en todas las categorías analizadas, a excepción de las mujeres que utilizaban preservativo y las tituladas superiores. CONCLUSIONES: El programa alcanza sus objetivos con una alta efectividad. No obstante, sería aconsejable realizar una valoración de su eficiencia incluyendo el análisis de los costes, ya que al disminuir la prevalencia el valor predictivo positivo de la prueba disminuye notablemente, lo que encarece la detección de un caso.

There are many ways to get students involved in the English learning process. Telling stories is one of the most effective ones. This article attempts to gather some pedagogical and psychological reasons why this kind of activity is important, not only to improve students vocabulary and make them achieve a feeling of self confidence but to let them develop social values.

Abstract Background Idjwi, an island of approximately 220,000 people, is located in eastern DRC and functions semi-autonomously under the governance of two kings (mwamis). At more than 8 live births per woman, Idjwi has one of the highest total fertility rates (TFRs) in the world. Rapid population growth has led to widespread environmental degradation and food insecurity. Meanwhile family planning services are largely unavailable. Methods At the invitation of local leaders, we conducted a representative survey of 2,078 households in accordance with MEASURE DHS protocols, and performed ethnographic interviews and focus groups with key informants and vulnerable subpopulations. Modelling proximate determinates of fertility, we evaluated how the introduction of contraceptives and/or extended periods of breastfeeding could reduce the TFR. Results Over half of all women reported an unmet need for spacing or limiting births, and nearly 70% named a specific modern method of contraception they would prefer to use; pills (25.4%) and injectables (26.5%) were most desired. We predicted that an increased length of breastfeeding (from 10 to 21 months) or an increase in contraceptive prevalence (from 1% to 30%), or a combination of both could reduce TFR on Idjwi to 6, the average desired number of children. Increasing contraceptive prevalence to 15% could reduce unmet need for contraception by 8%. Conclusions To meet women’s need and desire for fertility control, we recommend adding family planning services at health centers with NGO support, pursuing a community health worker program, promoting extended breastfeeding, and implementing programs to end sexual- and gender-based violence toward women.

Abstract The use of Global Positioning Systems (GPS) and Geographical Information Systems (GIS) in disease surveys and reporting is becoming increasingly routine, enabling a better understanding of spatial epidemiology and the improvement of surveillance and control strategies. In turn, the greater availability of spatially referenced epidemiological data is driving the rapid expansion of disease mapping and spatial modeling methods, which are becoming increasingly detailed and sophisticated, with rigorous handling of uncertainties. This expansion has, however, not been matched by advancements in the development of spatial datasets of human population distribution that accompany disease maps or spatial models. Where risks are heterogeneous across population groups or space or dependent on transmission between individuals, spatial data on human population distributions and demographic structures are required to estimate infectious disease risks, burdens, and dynamics. The disease impact in terms of morbidity, mortality, and speed of spread varies substantially with demographic profiles, so that identifying the most exposed or affected populations becomes a key aspect of planning and targeting interventions. Subnational breakdowns of population counts by age and sex are routinely collected during national censuses and maintained in finer detail within microcensus data. Moreover, demographic and health surveys continue to collect representative and contemporary samples from clusters of communities in low-income countries where census data may be less detailed and not collected regularly. Together, these freely available datasets form a rich resource for quantifying and understanding the spatial variations in the sizes and distributions of those most at risk of disease in low income regions, yet at present, they remain unconnected data scattered across national statistical offices and websites. In this paper we discuss the deficiencies of existing spatial population datasets and their limitations on epidemiological analyses. We review sources of detailed, contemporary, freely available and relevant spatial demographic data focusing on low income regions where such data are often sparse and highlight the value of incorporating these through a set of examples of their application in disease studies. Moreover, the importance of acknowledging, measuring, and accounting for uncertainty in spatial demographic datasets is outlined. Finally, a strategy for building an open-access database of spatial demographic data that is tailored to epidemiological applications is put forward.

Abstract Background Built environment features of neighborhoods may be related to obesity among adolescents and potentially related to obesity-related health disparities. The purpose of this study was to investigate spatial relationships between various built environment features and body mass index (BMI) z-score among adolescents, and to investigate if race/ethnicity modifies these relationships. A secondary objective was to evaluate the sensitivity of findings to the spatial scale of analysis (i.e. 400- and 800-meter street network buffers). Methods Data come from the 2008 Boston Youth Survey, a school-based sample of public high school students in Boston, MA. Analyses include data collected from students who had georeferenced residential information and complete and valid data to compute BMI z-score (n = 1,034). We built a spatial database using GIS with various features related to access to walking destinations and to community design. Spatial autocorrelation in key study variables was calculated with the Global Moran’s I statistic. We fit conventional ordinary least squares (OLS) regression and spatial simultaneous autoregressive error models that control for the spatial autocorrelation in the data as appropriate. Models were conducted using the total sample of adolescents as well as including an interaction term for race/ethnicity, adjusting for several potential individual- and neighborhood-level confounders and clustering of students within schools. Results We found significant positive spatial autocorrelation in the built environment features examined (Global Moran’s I most ≥ 0.60; all p = 0.001) but not in BMI z-score (Global Moran’s I = 0.07, p = 0.28). Because we found significant spatial autocorrelation in our OLS regression residuals, we fit spatial autoregressive models. Most built environment features were not associated with BMI z-score. Density of bus stops was associated with a higher BMI z-score among Whites (Coefficient: 0.029, p p Conclusion Some relationships between the built environment and adolescent BMI z-score were in the unexpected direction. Our findings overall suggest that the built environment does not explain a large proportion of the variation in adolescent BMI z-score or racial disparities in adolescent obesity. However, there are some differences by race/ethnicity that require further research among adolescents.

Abstract Background In Brazil dengue has been a major public health problem since DENV-1 introduction and spread in 1986. After a low or silent co-circulation, DENV-1 re-emerged in 2009 causing a major epidemic in the country in 2010 and 2011. In this study, the phylogeny of DENV-1 strains isolated in RJ after its first introduction in 1986 and after its emergence in 2009 and 2010 was performed in order to document possible evolutionary patterns or introductions in a re-emergent virus. Findings The analysis of the E gene sequences demonstrated that DENV-1 isolated during 2009/2010 still belong to genotype V (Americas/Africa) but grouping in a distinct clade (lineage II) of that represented by earlier DENV-1 (lineage I). However, strains isolated in 2011 grouped together forming another distinct clade (lineage III). Conclusions The monitoring of DENV is important to observe the spread of potentially virulent strains as well to evaluate its impact over the population during an outbreak. Whether explosive epidemics reported in Brazil caused mainly by DENV-1 was due to lineage replacement, or due the population susceptibility to this serotype which has not circulated for almost a decade or even due to the occurrence of secondary infections in a hyperendemic country, is not clear. This is the first report of multiple lineages of DENV-1 detected in Brazil.

Abstract Background Genetic plasticity may be understood as the ability of a functional gene network to tolerate alterations in its components or structure. Usually, the studies involving gene modifications in the course of the evolution are concerned to nucleotide sequence alterations in closely related species. However, the analysis of large scale data about the distribution of gene families in non-exclusively closely related species can provide insights on how plastic or how conserved a given gene family is. Here, we analyze the abundance and diversity of all Eukaryotic Clusters of Orthologous Groups (KOG) present in STRING database, resulting in a total of 4,850 KOGs. This dataset comprises 481,421 proteins distributed among 55 eukaryotes. Results We propose an index to evaluate the evolutionary plasticity and conservation of an orthologous group based on its abundance and diversity across eukaryotes. To further KOG plasticity analysis, we estimate the evolutionary distance average among all proteins which take part in the same orthologous group. As a result, we found a strong correlation between the evolutionary distance average and the proposed evolutionary plasticity index. Additionally, we found low evolutionary plasticity in Saccharomyces cerevisiae genes associated with inviability and Mus musculus genes associated with early lethality. At last, we plot the evolutionary plasticity value in different gene networks from yeast and humans. As a result, it was possible to discriminate among higher and lower plastic areas of the gene networks analyzed. Conclusions The distribution of gene families brings valuable information on evolutionary plasticity which might be related with genetic plasticity. Accordingly, it is possible to discriminate among conserved and plastic orthologous groups by evaluating their abundance and diversity across eukaryotes. Opportunistic networking represents a promising paradigm for support of communications, specifically in infrastructureless scenarios such as remote areas communications. In principle in opportunistic environments, we would like to make available all the applications thought for traditional wired and wireless networks like file-sharing and content distribution. In this paper, we present a delay-tolerant scenario for file sharing applications in rural areas, where an opportunistic approach is exploited. In order to support communications, we compare two peer-to-peer (P2P) schemes initially conceived for wireless networks and prove their applicability and usefulness to a DTN scenario, where replication of resources can be used to improve the lookup performance and the network can be occasionally connected by means of a data mule. Simulation results show the suitability of the schemes and allow to derive interesting design guidelines on the convenience and applicability of such approaches.

Abstract Background In Mali, malaria is the leading cause of death and the primary cause of outpatient visits for children under five. The twin towns of Mopti and Sévaré have historically had high under-five mortality. This paper investigates the changing malaria burden in children under five in these two towns for the years 1998-2006, and the likely contribution of previous interventions aimed at reducing malaria. Methods A retrospective analysis of daily outpatient consultation records from urban community health centres (CSCOMs) located in Mopti and Sévaré for the years 1998-2006 was conducted. Risk factors for a diagnosis of presumptive malaria, using logistic regression and trends in presumptive malaria diagnostic rates, were assessed using multilevel analysis. Results Between 1998-2006, presumptive malaria accounted for 33.8% of all recorded consultation diagnoses (10,123 out of 29,915). The monthly presumptive malaria diagnostic rate for children under five decreased by 66% (average of 8 diagnoses per month per 1,000 children in 1998 to 2.7 diagnoses per month in 2006). The multi-level analysis related 37% of this decrease to the distribution of bed net treatment kits initiated in May of 2001. Children of the Fulani (Peuhl) ethnicity had significantly lower odds of a presumptive malaria diagnosis when compared to children of other ethnic groups. Conclusions Presumptive malaria diagnostic rates have decreased between 1998-2006 among health care-seeking children under five in Mopti and Sévaré. A bed net treatment kit intervention conducted in 2001 is likely to have contributed to this decline. The results corroborate previous findings that suggest that the Fulani ethnicity is protective against malaria. The findings are useful to encourage dialogue around the urban malaria situation in Mali, particularly in the context of achieving the target of reducing malaria morbidity in children younger than five by 50% by 2011 as compared to levels in 2000.

Abstract Background Our goal is to use gene therapy to alleviate pain by targeting glial cells. In an animal model of facial pain we tested the effect of transfecting the glutamic acid decarboxylase (GAD) gene into satellite glial cells (SGCs) of the trigeminal ganglion by using a serotype 5 adenovector with high tropisms for glial cells. We postulated that GABA produced from the expression of GAD would reduce pain behavior by acting on GABA receptors on neurons within the ganglion. Results Injection of adenoviral vectors (AdGAD65) directly into the trigeminal ganglion leads to sustained expression of the GAD65 isoform over the 4 weeks observation period. Immunohistochemical analysis showed that adenovirus-mediated GAD65 expression and GABA synthesis were mainly in SGCs. GABAA and GABAB receptors were both seen in sensory neurons, yet only GABAA receptors decorated the neuronal surface. GABA receptors were not found on SGCs. Six days after injection of AdGAD65 into the trigeminal ganglion, there was a statistically significant decrease of pain behavior in the orofacial formalin test, a model of inflammatory pain. Rats injected with control virus (AdGFP or AdLacZ) had no reduction in their pain behavior. AdGAD65-dependent analgesia was blocked by bicuculline, a selective GABAA receptor antagonist, but not by CGP46381, a selective GABAB receptor antagonist. Conclusion Transfection of glial cells in the trigeminal ganglion with the GAD gene blocks pain behavior by acting on GABAA receptors on neuronal perikarya.

Abstract Background Historically, environmental management has brought important achievements in malaria control and overall improvements of health conditions. Currently, however, implementation is often considered not to be cost-effective. A community-based environmental management for malaria control was conducted in Dar es Salaam between 2005 and 2007. After community sensitization, two drains were cleaned followed by maintenance. This paper assessed the impact of the intervention on community awareness, prevalence of malaria infection, and Anopheles larval presence in drains. Methods A survey was conducted in neighbourhoods adjacent to cleaned drains; for comparison, neighbourhoods adjacent to two drains treated with larvicides and two drains under no intervention were also surveyed. Data routinely collected by the Urban Malaria Control Programme were also used. Diverse impacts were evaluated through comparison of means, odds ratios (OR), logistic regression, and time trends calculated by moving averages. Results Individual awareness of health risks and intervention goals were significantly higher among sensitized neighbourhoods. A reduction in the odds of malaria infection during the post-cleaning period in intervention neighbourhoods was observed when compared to the pre-cleaning period (OR = 0.12, 95% CI 0.05–0.3, p < 0.001). During the post-cleaning period, a higher risk of infection (OR = 1.7, 95% CI 1.1–2.4, p = 0.0069) was observed in neighbourhoods under no intervention compared to intervention ones. Eighteen months after the initial cleaning, one of the drains was still clean due to continued maintenance efforts (it contained no waste materials and the water was flowing at normal velocity). A three-month moving average of the percentage of water habitats in that drain containing pupae and/or Anopheles larvae indicated a decline in larval density. In the other drain, lack of proper resources and local commitment limited success. Conclusion Although environmental management was historically coordinated by authoritarian/colonial regimes or by industries/corporations, its successful implementation as part of an integrated vector management framework for malaria control under democratic governments can be possible if four conditions are observed: political will and commitment, community sensitization and participation, provision of financial resources for initial cleaning and structural repairs, and inter-sectoral collaboration. Such effort not only is expected to reduce malaria transmission, but has the potential to empower communities, improve health and environmental conditions, and ultimately contribute to poverty alleviation and sustainable development.

Abstract Background Research on the motivational model proposed by Self-Determination Theory (SDT) provides theoretically sound insights into reasons why people adopt and maintain exercise and other health behaviors, and allows for a meaningful analysis of the motivational processes involved in behavioral self-regulation. Although obesity is notoriously difficult to reverse and its recidivism is high, adopting and maintaining a physically active lifestyle is arguably the most effective strategy to counteract it in the long-term. The purposes of this study are twofold: i) to describe a 3-year randomized controlled trial (RCT) aimed at testing a novel obesity treatment program based on SDT, and ii) to present the rationale behind SDT's utility in facilitating and explaining health behavior change, especially physical activity/exercise, during obesity treatment. Methods Study design, recruitment, inclusion criteria, measurements, and a detailed description of the intervention (general format, goals for the participants, intervention curriculum, and main SDT strategies) are presented. The intervention consists of a 1-year group behavioral program for overweight and moderately obese women, aged 25 to 50 (and pre-menopausal), recruited from the community at large through media advertisement. Participants in the intervention group meet weekly or bi-weekly with a multidisciplinary intervention team (30 2 h sessions in total), and go through a program covering most topics considered critical for successful weight control. These topics and especially their delivery were adapted to comply with SDT and Motivational Interviewing guidelines. Comparison group receive a general health education curriculum. After the program, all subjects are follow-up for a period of 2 years. Discussion Results from this RCT will contribute to a better understanding of how motivational characteristics, particularly those related to physical activity/exercise behavioral self-regulation, influence treatment success, while exploring the utility of Self-Determination Theory for promoting health behavior change in the context of obesity. Abstract Background Half of the population of Africa will soon live in towns and cities where it can be protected from malaria by controlling aquatic stages of mosquitoes. Rigorous but affordable and scaleable methods for mapping and managing mosquito habitats are required to enable effective larval control in urban Africa. Methods A simple community-based mapping procedure that requires no electronic devices in the field was developed to facilitate routine larval surveillance in Dar es Salaam, Tanzania. The mapping procedure included (1) community-based development of sketch maps and (2) verification of sketch maps through technical teams using laminated aerial photographs in the field which were later digitized and analysed using Geographical Information Systems (GIS). Results Three urban wards of Dar es Salaam were comprehensively mapped, covering an area of 16.8 km2. Over thirty percent of this area were not included in preliminary community-based sketch mapping, mostly because they were areas that do not appear on local government residential lists. The use of aerial photographs and basic GIS allowed rapid identification and inclusion of these key areas, as well as more equal distribution of the workload of malaria control field staff. Conclusion The procedure developed enables complete coverage of targeted areas with larval control through comprehensive spatial coverage with community-derived sketch maps. The procedure is practical, affordable, and requires minimal technical skills. This approach can be readily integrated into malaria vector control programmes, scaled up to towns and cities all over Tanzania and adapted to urban settings elsewhere in Africa.
