O presente artigo analisa (e publica) uma até agora desconhecida lista das fontes de receita do arcebispo de Lisboa em 1465-1466 que esteve guardada no Museu Britânico. Combinada com outra documentação, este registo amplia os conhecimentos sobre a organização económica das dioceses portuguesas. A informação desta fonte oferece uma rara oportunidade de observar as alterações na agricultura portuguesa nos séculos XIV e XV.

The characterization of ceramic foams via X-ray microtomography imaging method is often restricted to a general overview of samples, either to perform qualitative or quantitative analyses. To assess the foam with a focus on some specific components of its structure, such as hollow struts’ cavities and open cells, the generalized characterizations must be overcome. This work presents image analysis methodologies, based on open-source software, tools, and plugins, to achieve the aforementioned characterizations. SiC and ZrO2 foams samples were analyzed and some of the results were compared with those accomplished with pieces of commercial software. The results show good agreement between open-source and commercial software applications, indicating that the presented methodologies can be freely applied by any researcher to analyze their foams.

Current research applies an energy-based design model to improve performance in existing modern buildings, in Rio de Janeiro, from the 1940’s, improving these buildings’ shading systems. This article proposes a methodology tested through a case study, the Nova Cintra building. The methodology starts by analysing the original shading system performance, regarding insolation, illuminance and air temperature. Using these results, proposes two computacional methods to improve performance: (1) a combinatorial modelling process, recombining the existing shading systems positions in the building’s north façade; and (2) a transformation process, using parametric and algorithmic–parametric modelling, to improve the existing shading systems performance. Both processes use optimization algorithms. The results of these modelling and optimization methods are compared with the results of the original system and suggests an improvement between 111.1% and 590.4% for insolation; between 360.9% and 84.4% for illuminance; and between 2.9% and 3.0% for air temperature, considering winter and summer solstices. This improvement aims at reducing the buildings’ energy consumption and foresees the production of renewable energy from solar harvesting, to mitigate climate change.

Objectives: The aim of the present systematic review was to evaluate the dimensional influence of the epithelialized tissue graft harvested from the palate in the postoperative pain. Material and Methods: Research was conducted in electronic databases Cochrane Library, Embase, LILACS, PubMed, Scopus, and Web of Science upwards May 15, 2022. Studies that reported the influence of graft dimensions of palatal epithelized harvesting on postoperative pain were eligible. The evaluation was made using the methodological quality assessment by Joanna Briggs Institute Critical Appraisal Checklist for randomized clinical trials and non-randomized studies and the level of evidence according to GRADE. Results: Four studies were included. The clinical and methodological heterogeneity among studies led to an analysed narrative. The postoperative pain was assessed during the period of 1 to 28 postoperative days. It was determined by using visual analog score in three studies, while the evaluation was performed indirectly based on analgesics intake in one study. According to three studies, bigger graft sizes were associated with higher postoperative pain. The methodological quality assessment categorized two study as high (one randomized control trial and one non-randomized), and two as moderate (one randomized control trial and one non-randomized). The data was considered moderate. Conclusions: Based on the moderate certainty level, bigger graft sizes of palatal epithelized harvesting appear to promote more postoperative pain. Understanding the postoperative pain as a response to a graft extension may assist some clinical decisions regarding the surgical periodontal and peri-implant planning.

Abstract Introduction: Kidney transplant (KT) recipients have a high risk for adverse outcomes from infections, such as COVID-19. Methods: We have retrospectively reviewed all KT recipients with documented COVID-19 between March 1, 2020, and March 15, 2021, and analyzed patients’ characteristics, clinical course, treatment, and outcomes. Results: We identified 123 patients, 72% were male, with a mean age of 54.5±13.0 years. Twenty percent were asymptomatic, 7% had a nosocomial transmission, and 36% of the remainder required hospitalization. Almost all admitted patients received oxygen, 30% required invasive mechanical ventilation (IMV), more than a half had acute kidney injury, with 10% requiring dialysis, and 20% died. Incidence was comparable to that of the Portuguese population, but the mortality rate was almost four times higher (SMR of 3.768 (95% CI:1.723-7.154). Higher body mass index (OR 1.275, P=0.001), lower baseline graft function (OR 0.968, P=0.015), and nosocomial transmission (OR 13.836, P=0.019) were associated with oxygen demand, whereas female gender (OR 3.801, P=0.031) and lower baseline kidney graft function (OR 0.955, P=0.005), but not body mass index, were associated with IMV and/or death. Conclusion: Mortality rate in KT patients was higher than in the general population and lower baseline kidney function was the most consistent marker for adverse outcomes.

The History of Political Ideas by the German-American philosopher Eric Voegelin (1901-1985) is a monumental work of around 2,600 pages. It remained unpublished during his lifetime, and it came to light through the American edition (1997-1999) and the now completed Portuguese edition (2012-2018). Being the author of the first world edition of an abridged version of the History of Political Ideas ; the translator of the first three volumes of the 2012-2018 Portuguese edition; and the author of The civil philosophy of Eric Voegelin (my 1990 Ph. D diss.) I consider that the History of Political Ideas challenges the present climate of opinion: it subverts the dominant corrosive forces of moral relativism, intolerant neo-positivism, end-of-history obsessions, postmodernist deconstructions, agnosticism, nihilism, new age religions, and the all-pervasive ideology of money. Eric Voegelin achieves all this leading his readers from Antiquity to Modern Age. His monumental work begins with the “spiritual disintegration” of the Greek world, after the peak of Plato and Aristotle, a disintegration that ushered a long process of transition in the self-understanding of man in the Mediterranean world. The series goes through Middle Ages , R enaissance and Reformation as Voegelin analyzes the collapse of imperial Christianity, which led to the rise of autonomous reason and sectarian revolts that reached full development in later centuries. A new form of modern human consciousness replaced the Christian understanding of a divinely created closed cosmos. The collection ends - in a suspensive way - with “The Crisis and the Apocalypse of Man” focused on thinkers such as Comte, Bakunine and Marx; although they experienced true epiphanies, they become self-obsessed to the detriment of the world to which they refer. Such “Apocalypse of Man” must now be challenged, albeit with methodologies and hermeneutic principles other than those that Voegelin himself abandoned some decades ago.

Innovative technologies for digital design and manufacture have spread with globalization, providing improved working tools and processes for architecture. These technologies open up different professional opportunities, forging alternative relations with society, especially for new generations and emerging countries. This article reflects on novel collaborative experiences between Ibero-American Universities, accounting for the intensive use of digital technologies in architecture. To bridge technological gaps, the authors describe collaborative efforts to produce full-size constructions, using networking, which results in a hybridization of these “new media” adapted to local and cultural contexts. These initiatives arise from an attempt to overcome the lack of resources at local Universities and their rigid institutional processes, along with their concerns about promoting global connectivity for new generations, leading to informal and collective actions that break with traditional teaching, applying advanced techniques in unseen collective and creative processes. These experiences reveal an architectural work distributed among all the participants that collaborate in the conceptualization, programming, management, and execution of the design, with diverse hybrid practices, and a powerful collective synergy. This also results in new expanded proposals that unfold new relationships with the environment and the community. Thus, these actions tend, by themselves, to integrate and project new horizons in architectural collaboration. This article aims at mapping these spatial-temporal actions in Architecture from the global south (Santos 2014).

Este trabalho propõe uma reconstrução crítica da conquista cristã de Tavira. De acordo com a fonte narrativa mais antiga (a Crónica de Portugal de 1419), este acontecimento deu-se em 1242 e foi uma consequência acidental do massacre pela população muçulmana dos ditos “Sete Mártires” de Tavira (seis freires de Santiago e um mercador). Recorrendo a materiais novos e outros há muito conhecidos, propõe que Tavira tenha sido ocupada na sequência de uma campanha a 11 de junho de 1239, enquanto o “Martírio” ocorreu num dia 9 de julho cerca de uma década depois, possivelmente em resultado de um confronto entre a população de Tavira e cavaleiros castelhanos de Santiago. Estas propostas e datas constituem contributos importantes para três áreas: a crítica da Crónica de Portugal de 1419, o estabelecimento de uma sequência fiável dos acontecimentos militares e a memória e identidade da sociedade criada pela Reconquista.

A correspondência (1643-1649) entre a princesa Isabel da Boémia e René Descartes, só integralmente conhecida em finais do século XIX, é reveladora de preocupações comuns e de afeição mútua. O presente artigo visa recuperar o lugar de Isabel na evolução da filosofia em geral, e da antropologia cartesiana em particular. Aplica-se a hermenêutica a uma conversação que não teria cabimento numa obra formal, de modo a entender como temas do quotidiano se tornaram objeto de reflexão e, inversamente, essa reflexão veio lançar luz sobre esses mesmos temas. Na sequência de estudos de Henriques (1996) e dos contributos marcantes de Shapiro (1999) e Jeffery (2018) sugere-se como a teoria das emoções de Isabel proporcionou a Descartes a evolução do tratamento da dualidade de corpo e alma no Tratado sobre as paixões da alma (1649), já fora do escopo deste artigo.

O presente artigo explora a compreensão da individualidade como um dos principais motivos pelo qual Karl Polanyi e a sua obra, que em muito ultrapassa A Grande Transformação, se tornaram uma referência nas ciências humanas. A primeira vaga de polanyianos na década de 1960, consistia em antropólogos e economistas que investigavam as dinâmicas de sociedades pré-capitalistas. Uma segunda vaga, na década de 1980, consistia em sociólogos, juristas e politólogos. Desde a crise de 2007-2008, Polanyi foi assumido como um tribuno contra o globalismo e contribuiu para um novo paradigma de análise, uma nova visão geral (Übersichtsproblem) da liberdade social e da responsabilidade pessoal em sociedades complexas. O artigo pretende revelar como o conceito dialógico de individualidade, originado pela conversação filosófica de Polanyi com a doutrina social cristã, proporciona os conceitos de terra, trabalho e dinheiro desmercantilizados, patentes em A Grande Transformação e facilita a leitura crítica dos entrosamentos atuais entre economia, ética, sociedade e estado.

Patients who are candidates for a second kidney transplant (SKT) frequently have a higher level of panel reactive antibodies (PRA). We assessed the allosensitisation change after a first graft failure (GF), its predictors and impact on retransplantat outcomes. We retrospectively selected 140 adult patients who received a SKT. Recipient and donor characteristics were analyzed. We defined the delta PRA (dPRA) as the difference between peak PRA before the SKT and first one (cohort median value = +10%). Logistic regression analysis was used to determine risk factors for dPRA ≥ 10% and acute rejection (AR) in the SKT. Univariable and multivariable Cox analysis was applied to assess independent predictors of second GF. Risk factors for dPRA ≥ 10% at SKT were AR (OR = 2.57; P = 0.022), first graft survival

We explore Ernst -Wolfgang Böckenförde’s (1930-2019) political thought that intends to justify public law as the condition of freedom, because it establishes limits. This vision presupposes an anthropology, never fully unfolded in his work but, nevertheless, very present when he reflects as a jurist with philosophical concerns. One year after his death, among unprecedented world changes and huge uncertainties, it is relevant to revisit and evaluate his lasting contribution. Here, our interest focuses exclusively on the notion of the State and some of its implications; we will leave for another paper his interpretation of the historical evolution of law and the Constitution, his position on issues of bioethics, and his vision of the European Union project and globalization.

Abstract The occurrence of ascites after Renal Transplant (RT) is infrequent, and may be a consequence of surgical or medical complications. Case report: 61 year-old, male, history of arterial hypertension, tongue carcinoma and alcoholic habits 12-20g/day. He had chronic kidney disease secondary to autosomal dominant polycystic kidney disease, without hepatic polycystic disease. He underwent cadaver donor RT in September 2017. He had delayed graft function by surgically corrected renal artery stenosis. He was admitted in January 2018 for ascites de novo, with no response to diuretics. HE had visible abdominal collateral circulation. Graft dysfunction, adequate tacrolinemia, Innocent urinary sediment, mild anemia, without thrombocytopenia. Serum albumin 4.0g / dL. Normal hepatic biochemistry. Peritoneal fluid with transudate characteristics and serum albumin gradient > 1.1. Ultrasound showed hepatomegaly, permeable vascular axes, without splenomegaly. Mycophenolate mofetil was suspended, with reduced remaining immunosuppression. He maintained refractory ascites: excluded infectious, metabolic, autoimmune and neoplastic etiologies. No nephrotic proteinuria and no heart failure. MRI: micronodules compatible with bile cysts. Upper Digestive Tract Endoscopy did not show gastroesophageal varicose veins. Normal abdominal lymphoscintigraphy. He underwent exploratory laparoscopy with liver biopsy: incomplete septal cirrhosis of probable vascular etiology some dilated bile ducts. He maintained progressive RT dysfunction and restarted hemodialysis. The proposed direct measurement of portal pressure was delayed by ascites resolution. There was further recovery of the graft function. Discussion: Incomplete septal cirrhosis is an uncommon cause of non-cirrhotic portal hypertension. Its definition is not well known, morphological and pathophysiological. We have not found published cases of post-RT ascites secondary to this pathology, described as possibly associated with drugs, immune alterations, infections, hypercoagulability and genetic predisposition.

Até 30% dos pares de dador vivo não são transplantados por incompatibilidade do grupo ABO e/ou do sistema Human Leukocyte Antigen (HLA). Os programas de doação renal cruzada surgiram como uma estratégia para tentar ultrapassar estas barreiras. Em Portugal o Programa Nacional de Doação Renal Cruzada (PNDRC) foi legislado em 2010 e o primeiro transplante renal cruzado ocorreu em 2013. Até ao momento foram efetuados 22 transplantes deste tipo. Objetivo: O presente trabalho visa avaliar as caraterísticas e perfil evolutivo dos doentes submetidos a transplante renal cruzado em Portugal. Métodos: Os autores apresentam um estudo observacional retrospetcivo com análise dos respetivos doentes. Resultados: Da amostra total, a maioria dos recetores era do sexo masculino (55%), com idade mediana 53 anos. A poliquistose renal foi a etiologia da doença renal mais comum (18%) e a maioria dos doentes encontrava-se previamente em programa crónico de hemodiálise (68%). Três doentes apresentaram Calculated Panel Reactive Antibody (PRAc) superior a 98% e 10 PRAc superior a 80%. Foi realizada indução de imunossupressão com Anti-Thymocyte Globulin (ATG) em 50% dos doentes e imunomodulação com Rituximab e/ou plasmaferese em 15%. Todos os recetores evoluíram com função imediata do enxerto. Não se registaram complicaçães major, com eventos minor em 15%. Num tempo mediano de follow-up de 2 7 [ 2-46] meses, não se verificou nenhum caso de rejeição celular aguda e a penas um de rejeição humoral. As sobrevidas do dador e recetor foram ambas 100%. Conclusão: A apresentação destes resultados preliminares excelentes visa estimular o aumento do número de pares a incluir no PNDRC e um maior número de transplantes efetuados no programa. Tal como uma parte significativa desta amostra, o transplante renal cruzado pode constituir uma possibilidade para doentes com incompatibilidade do sistema HLA e/ou do grupo ABO. A doação renal cruzada em muito engrandece a doação em vida, oferecendo a pares incompatíveis e selecionados, uma oportunidade de transplantação renal com sucesso.

Background: Histiocytic sarcoma (HS) is a rare hematologic neoplasm with a few hundred cases having been described to date. Case Presentation: We report the case of a 56-year-old woman with a history of hepatitis C infection and chronic kidney disease (CKD), submitted to a kidney transplant in 1984, under maintenance immunosuppression with prednisone and azathioprine. Patient presented with a relentlessly growing mass on her right front thorax. It was painless, smooth, and adherent to the deep muscle. Laboratory studies were unremarkable. Ultrasonography and computerized tomography (CT) scan revealed a highly vascularized heterogeneous mass (8×9 cm), with a necrotic centre. Positron emission tomography (PET) scan demonstrated multiple thoracic, abdominal, and pelvic nodules. Histology revealed a highly undifferentiated HS (vimentin, CD68, CD99, and CD4 positive). In spite of having started treatment with etoposide and thalidomide, no clinical response was achieved and the patient died three months later. Conclusions: To the authors’ knowledge, this is the first described case of HS in a solid organ transplant patient.
