Aims The aim of this study was to determine the contemporary use of reperfusion therapy in the European Society of Cardiology (ESC) member and affiliated countries and adherence to ESC clinical practice guidelines in patients with ST-elevation myocardial infarction (STEMI). Methods and results Prospective cohort (EURObservational Research Programme STEMI Registry) of hospitalized STEMI patients with symptom onset Conclusions The use of reperfusion therapy for STEMI in the ESC member and affiliated countries was high. Primary PCI was the most frequently used treatment and associated total in-hospital mortality was below 5%. However, there was geographic variation in the use of primary PCI, which was associated with differences in in-hospital mortality.

Body condition score has been shown to be highly correlated to the degree of body fatness in slaughtering experiments involving a pre-established range of body condition score in sheep at the same physiological stage,but there is a lack of evidence in animals at different physiological conditions. The aim of the present work was to evaluate the usefulness of subjectively assessed body condition score to predict the proportion of chemical fat in the body of Romanov x Rasa ewes at different stages of their reproductive cycle. An adaptation of Russel et al.(1969) condition scoring system was used in wich animals were scored on a 0 to 5 scale to the nearest 0.25 condition unit.

Las necesidades nutritivas de las ovejas varían a lo largo de su ciclo reproductivo, siendo elevadas al final de la gestación y primera fase de la lactación y próximas al mantenimiento en el resto de ios periodos. Como se verá en el transcurso de este seminario (Guada, 1991) el aporte de nutrientes no se ajusta siempre a esta evolución de las necesidades, bien por razones fisiológicas, debido a la limitada capacidad de ingestión al final de la gestación e inicio de la lactación, o bien por razones de disponibilidad de alimentos como consecuencia de la variación estacional de la producción de pastos. Para amortiguar el defase entre necesidades y aporte de nutrientes, las ovejas movilizan y utilizan sus propias reservas corporales, pudiendo perder hasta el 50 % de sus reservas iipídicas durante la gestación (Russel et al., 1968) y llegar prácticamente a la deplección de grasa a mitad de la lactación (Sykes, 1974), cuando se mantienen en condiciones de explotación extensiva. Por consiguiente no se puede establecer una relación causal entre aportes y producción si no se conoce en que medida las movilizaciones de las reservas corporales contribuyen al desarrollo del feto y anexos o a la producción de leche. Para el establecimiento de estas relaciones es precisó disponer de un método fiable de estimación de la composición corporal o estado de reservas de los animales a lo largo del ciclo reproductivo.

Aims The Acute Cardiac Care Association (ACCA)–European Association of Percutaneous Coronary Intervention (EAPCI) Registry on ST-elevation myocardial infarction (STEMI) of the EurObservational programme (EORP) of the European Society of Cardiology (ESC) registry aimed to determine the current state of the use of reperfusion therapy in ESC member and ESC affiliated countries and the adherence to ESC STEMI guidelines in patients with STEMI. Methods and results Between 1 January 2015 and 31 March 2018, a total of 11 462 patients admitted with an initial diagnosis of STEMI according to the 2012 ESC STEMI guidelines were enrolled. Individual patient data were collected across 196 centres and 29 countries. Among the centres, there were 136 percutaneous coronary intervention centres and 91 with cardiac surgery on-site. The majority of centres (129/196) were part of a STEMI network. The main objective of this study was to describe the demographic, clinical, and angiographic characteristics of patients with STEMI. Other objectives include to assess management patterns and in particular the current use of reperfusion therapies and to evaluate how recommendations of most recent STEMI European guidelines regarding reperfusion therapies and adjunctive pharmacological and non-pharmacological treatments are adopted in clinical practice and how their application can impact on patients’ outcomes. Patients will be followed for 1 year after admission. Conclusion The ESC ACCA-EAPCI EORP ACS STEMI registry is an international registry of care and outcomes of patients hospitalized with STEMI. It will provide insights into the contemporary patient profile, management patterns, and 1-year outcome of patients with STEMI.

Special Issue: Focused Ultrasound in Parkinson's Disease., [Background] Single nucleotide polymorphisms (SNPs) in the α‐synuclein (SNCA ) gene are associated with differential risk and age at onset (AAO) of both idiopathic and Leucine‐rich repeat kinase 2 (LRRK2)‐associated Parkinson's disease (PD). Yet potential combinatory or synergistic effects among several modulatory SNPs for PD risk or AAO remain largely underexplored., [Objectives] The mechanistic target of rapamycin (mTOR ) signaling pathway is functionally impaired in PD. Here we explored whether SNPs in the mTOR pathway, alone or by epistatic interaction with known susceptibility factors, can modulate PD risk and AAO., [Methods] Based on functional relevance, we selected a total of 64 SNPs mapping to a total of 57 genes from the mTOR pathway and genotyped a discovery series cohort encompassing 898 PD patients and 921 controls. In addition, we also found a 3-loci epistatic combination of RPTOR rs11868112 and RPS6KA2 rs6456121 with SNCA rs356219, which was associated (odds ratio = 2.89; P < .0001) with differential AAO. The latter was further validated (odds ratio = 1.56; P = 0.046-0.047) in the International Parkinson's Disease Genomics Consortium cohort. Conclusions These findings indicate that genetic variability in the mTOR pathway contributes to SNCA effects in a nonlinear epistatic manner to modulate differential AAO in PD, unraveling the contribution of this cascade in the pathogenesis of the disease. In secrecy for decades, the terrorist attacks of in 2001 in the heart of our cities have brought the intelligence services and their role in the fight against terrorism closer to a large sector of the population. Post 9/11 studies have shown how knowledge helps to mitigate stress. In this project, the process of preparing two animated videos is described. They offer material to parents and educators that will help to inform young people that, alongside the police and the military, there is another democratic institution with people in charge of protecting them. The field work was conducted with 455 students of between 8 and 16 years old from various Spanish cities. In all, 83% of students considered that the story was of interest as well as the way it was communicated. In conclusion, the animated videos were seen by the schoolchildren as an appropriate means of communicating the role of security institutions.

18. In vitro acidification potential and fermentation pattern of cereal grains incubated with inoculum from animals given forage or concentrate based diets

This work aimed to study the acidification and fermentation pattern of maize (M1, M2, M3), barley (B1, B2, B3) and sorghum (S1, S2, S3) varieties depending on the rumen environment (inoculum from forage or concentrate diets, FI or CI), in 10 h incubation series with a low buffered medium. With CI, gas volume from barley was similar to maize (P > 0.05) except at 4 and 6 h, when M2 and M1 were lower. Barley or maize varieties did not differ in gas production (P > 0.05). After 10 h, barley and maize produced on average 82.5 and 73.0 mL/g organic matter (OM), whereas S1, S2 and S3 rendered 68.4, 31.1 and 39.7 mL/g. With FI, differences between barley and maize were detected after 6 h (61.0, 35.3 and 14.1 mL/g OM at 10 h for barley, maize and sorghum). Among inocula, pH with CI was lower than with FI (P < 0.001). Incubation pH at 10 h was 5.51, 5.21 and 5.00 with CI, and 5.52, 5.85 and 5.91 with FI, for sorghum, maize and barley varieties. Gas production and total volatile fatty acid (VFA) concentration were higher (P < 0.001) with CI than FI (67.3 vs 36.8 mL/g and 31.0 vs 19.9 mmol/L at 10 and 8 h). Butyrate and valerate proportions were higher with CI (P < 0.001), whereas acetate and branched chain VFA were lower (P < 0.001). Fermentative activity against vitreous starch such as maize or sorghum was lower for forage than concentrate inoculum. Study of fermentation of starch-rich substrates in a low buffered medium gives a more realistic picture than conventionally buffered conditions.

19. Effect of dietary fat to starch content on fecal microbiota composition and activity in dogs

- Abstract
Dietary fat is known to modulate the hindgut microbiota in rodents; however, there is no clear evidence on the impact of high-fat diets on canine gut microbiota. The purpose of this study was to investigate the effect of feeding of diets differing in the amount of ME provided by fat and starch on the composition and activity of canine fecal microbiota. Twelve adult (3 to 7 yr of age) spayed Beagle dogs received a low-fat-high-starch diet (LF-HS; approximately 23%, 42%, and 25% ME provided by fat, starch, and CP, respectively) and a high-fat-low-starch diet (HF-LS; approximately 43%, 22%, and 25% ME provided by fat, starch, and CP, respectively) following a 2-period crossover arrangement. The higher amount of fat in the HF-LS diet was provided by lard, whereas the higher amount of starch in the LF-HS diet was provided primarily by maize and broken rice. Each period lasted 7 wk and included 4 wk for diet adaptation. Dogs were fed to meet their daily energy requirements (set at 480 kJ ME/kg BW0.75). Fecal samples were collected on weeks 5 and 6 of each period for the analysis of bacterial richness, diversity, and composition [by Ion-Torrent next-generation sequencing], bile acids, ammonia, and VFA. Additional fecal samples were collected from four dogs per diet and period to use as inocula for in vitro fermentation using xylan and pectin as substrates. Gas production was measured at 2, 4, 6, 9, 12, and 24 h of incubation. On week 7, blood samples were collected at 0-and 180-min postfeeding for the analysis of bacterial lipopolysaccharide (LPS). Feeding the HF-LS diet led to a greater (P < 0.05) fecal bile acid concentration compared with the LF-HS diet. Bacterial richness and diversity did not differ between diets (P > 0.10). However, dogs showed a lower relative abundance of Prevotella (P < 0.01), Solobacterium (P < 0.05), and Coprobacillus (P. 0.05) when fed of the HF-LS diet. Fecal ammonia and VFA contents were not affected by diet (P > 0.10). Relative to the LF-HS diet, in vitro fermentation of xylan using feces of dogs fed the HF-LS diet produced less gas at 6 h (P < 0.01) and 9 h (P < 0.05). Blood LPS did not increase at 180-min postfeeding with either diet (P < 0.10). These findings indicate that feeding a HF-LS diet to dogs does not affect bacterial diversity or fermentative end products in feces, but may have a negative impact on Prevotella and xylan fermentation.

- Abstract
Introduction There is a need for fast-acting, non-injection antiagitation treatments that are well tolerated and can be used outside of healthcare facilities. In phase II/III trials, an inhaled formulation of loxapine (ADASUVE (R)), a well-established, first-generation antipsychotic agent, provided rapid control of mild to moderate agitation in the hospital setting. The present study was designed to investigate the safety and efficacy of inhaled loxapine when self-administered outside the hospital setting. Methods and analysis This phase IV, multicentre, single-arm, open-label clinical trial is being conducted in five countries in Europe: Spain, Germany, Norway, Romania and Austria. The aim is to include approximately 500 patients with schizophrenia or bipolar disorder who previously received and responded well to inhaled loxapine in the hospital setting. Eligible patients will be followed up for 6 months from baseline. They will be given a 10 mg dose of inhaled loxapine to self-administer outside the hospital setting to treat an agitation episode, should one occur. Patients will also be given a short-acting beta-agonist bronchodilator for treatment of possible severe respiratory side effects. The primary endpoint is incidence of serious adverse events (AEs) and respiratory AEs of special interest related to use of inhaled loxapine outside the hospital setting. Secondary endpoints include incidence of other AEs, Clinical Global Impression-Improvement scores up to 2 hours after self-administration of inhaled loxapine, time to improvement of agitation, patient satisfaction with treatment, treatment outcomes according to agitation severity and concordance between the patient (or a family member/caregiver) and the physician in scoring of agitation severity and the decision to self-administer inhaled loxapine. Ethics and dissemination The protocol received ethics committee approval in the participating countries between January and August 2016. The results of this study will be disseminated through one or more scientific papers.

Although studies in rodents and humans have evidenced a weaker effect of fat in comparison to carbohydrates on the suppression of food intake, very few studies have been carried out in this field in dogs. This study investigates the effects of a high-carbohydrate (HC) and a high-fat (HF) diets on subsequent food intake and blood satiety-related hormones in dogs. Diets differed mainly in their starch (442 vs. 271 g/kg dry matter) and fat (99.3 vs. 214 g/kg dry matter) contents. Twelve Beagle dogs received the experimental diets at maintenance energy requirements in two experimental periods, following a cross-over arrangement. In week 7 of each period, blood concentrations of active ghrelin, glucagon-like peptide (GLP-1), peptide YY, insulin, and glucose were determined before and at 30, 60, 120, 180, and 360 min post-feeding. The following week, intake of a challenge food offered 180 min after the HC and HF diets was recorded over two days. In comparison to the dogs on the HC diet, those on the HF diet had a higher basal concentration of GLP-1 (p =.010) and a higher total area under the curve over 180 min post-prandial (tAUC) (p =.031). Dogs on the HC diet showed a higher elevation of ghrelin at 180 min (p =.033) and of insulin at 360 min (p =.041), although ghrelin and insulin tAUC did not differ between the two diets (p ˃.10). Diet had no effect on challenge food intake (p ˃.10), which correlated with the tAUC of ghrelin (r =.514, p =.010), insulin (r = −.595, p =.002), and glucose (r = −.516, p =.010). Feeding a diet high in carbohydrate or fat at these inclusion levels does not affect the feeding response at 180 min post-prandial, suggesting a similar short-term satiating capacity.

Animal sterilization is suggested to promote food overconsumption, although it is unknown whether this effect is mediated by variations in satiety-related hormones, which are released in response to food intake. The aim of this study was to evaluate the effect of sterilization and of the main energy-delivery nutrients, fat and nonstructural carbohydrates, on food intake, blood concentration of satiety-related hormones, and activity level in dogs. In a 2-phase experiment (phase I [Ph.I], 74 d, and Ph.II, 84 d), 12 female Beagle dogs were assigned to a control group (intact in both phases) and a sterilization group (spayed 20 d before Ph.II). In each phase, dogs received a high-carbohydrate (HC) diet (313 and 105 g/kg DM starch and fat, respectively) and a high-fat (HF) diet (191 and 213 g/kg DM starch and fat, respectively), both high in total dietary fiber (>200 g/kg DM) and providing 27% ME as protein, in 2 consecutive periods following a crossover arrangement. During each period, dogs’ voluntary DMI and activity level were recorded during 5 d. Then, energy allowance was restricted to 0.7 maintenance and the level of intake of a common challenge food offered 4 h after feeding the experimental diets (challenge food intake [ChFI]) was used as an index of the satiety state of dogs. Blood concentration of active ghrelin, cholecystokinin (CCK), total peptide YY (PYY), and insulin were determined before and 15, 60, 120, 240, and 360 min after feeding. SUMMARY The type and processing of cereals for beef cattle are key factors to maximize performance without increasing the risk of ruminal acidosis. The current work studied the effect of grinding (G) or dry-rolling (R) the cereals in a concentrate based on maize (M) or barley (B) on the rumen environment of eight 6-month-old Holstein calves, which received either M or B for 70 days, offered as G and R in two consecutive periods. Daily feed intake pattern, rumen fermentation traits, total rumen bacteria and relative proportions of Streptococcus bovis and Selenomonas ruminantium were characterized twice in each period at 0, 4 and 8 h after feeding. Dry-rolling promoted a 0·25 reduction of concentrate intake during the first 4 h after feeding. Neither cereal type nor its processing form promoted differences in ruminal fermentation at 0 and 4 h; however, 8 h after feeding R cereals resulted in higher rumen pH (6·17 v. 5·71) and lower concentration of lactic acid (88·1 v. 156 mg/l) and volatile fatty acids (only in diet B, 118 v. 164 mm/l) compared with G. Little effect of dietary factors in the target rumen microorganisms were observed. The more balanced fermentation observed with R cereals may be explained by the lower concentrate intake promoted during the first 4 h, coupled with a more difficult access of bacterial enzymes to their starch, thus reducing its fermentation rate. Therefore, feeding dry-rolled cereals may be useful to reduce the risk of acidosis by regulating the intake pattern of the concentrate and/or its fermentation rate.

The aim of this study was to investigate the effect of presence or absence of protozoa on rumen fermentation and efficiency of microbial protein synthesis under different diets. Of 20 twin paired lambs, 1 lamb of each pair was isolated from the ewe within 24 h after birth and reared in a protozoa-free environment (n = 10), whereas their respective twin-siblings remained with the ewe (faunated, n = 10). When lambs reached 6 mo of age, 5 animals of each group were randomly allocated to 1 of 2 experimental diets consisting of either alfalfa hay as the sole diet, or 50:50 mixed with ground barley grain according to a 2 × 2 factorial arrangement of treatments. After 15 d of adaptation to the diet, the animals were euthanized and total rumen and abomasal contents were sampled to estimate rumen microbial synthesis using C31 alkane as flow marker. Different (15N and purine bases) and a novel (recombinant DNA sequences) microbial markers, combined with several microbial reference extracts (rumen protozoa, liquid and solid associated bacteria) were evaluated. Absence of rumen protozoa modified the rumen fermentation pattern and decreased total tract OM and NDF digestibility in 2.0 and 5.1 percentage points, respectively. The effect of defaunation on microbial N flow was weak, however, and was dependent on the microbial marker and microbial reference extract considered. Faunated lambs fed with mixed diet showed the greatest rumen protozoal concentration and the least efficient microbial protein synthesis (29% less than the other treatments), whereas protozoa-free lambs fed with mixed diet presented the smallest ammonia concentration and 34% greater efficiency of N utilization than the other treatments. Although 15N gave the most precise estimates of microbial synthesis, the use of recombinant DNA sequences represents an alternative that allows separate quantification of the bacteria and protozoa contributions. This marker showed that presence of protozoa decrease the bacterial-N flow through the abomasum by 33%, whereas the protozoa-N contribution to the microbial N flow increased from 1.9 to 14.1% when barley grain was added to the alfalfa hay. Absolute data related to intestinal flow must be treated with caution because the limitations of the sampling and maker system employed. Takayasu's arteritis is an inflammatory disease that produces a narrowing of the aorta and its major branches. The transcranial doppler findings and its value have not been clearly determined. We describe a patient with a history of orthostatic syncopal attacks, and transitory ischemic attacks. Arteriography showed severe narrowings and occlusions of multiple large branches of the aortic arch. Transcranial doppler showed a low flow velocity in both middle cerebral arteries, low pulsatility index and damping in the waveform. After surgery, transcranial doppler ipsilateral to the graft became almost normal and the collateralization pattern had changed. This case provides further evidence that the Takayasu's arteritis leads to relevant changes in the transcranial doppler waveform and that this technique can help in the follow-up of these patients.
