2. Unraveling the Anxious Mind: Anxiety, Worry, and Frontal Engagement in Sustained Attention Versus Off-Task Processing

Much remains unknown regarding the relationship between anxiety, worry, sustained attention, and frontal function. Here, we addressed this using a sustained attention task adapted for functional magnetic resonance imaging. Participants responded to presentation of simple stimuli, withholding responses to an infrequent “No Go” stimulus. Dorsolateral prefrontal cortex (DLPFC) activity to “Go” trials, and dorsal anterior cingulate (dACC) activity to “No Go” trials were associated with faster error-free performance; consistent with DLPFC and dACC facilitating proactive and reactive control, respectively. Trait anxiety was linked to reduced recruitment of these regions, slower error-free performance, and decreased frontal-thalamo-striatal connectivity. This indicates an association between trait anxiety and impoverished frontal control of attention, even when external distractors are absent. In task blocks where commission errors were made, greater DLPFC-precuneus and DLPFC-posterior cingulate connectivity were associated with both trait anxiety and worry, indicative of increased off-task thought. Notably, unlike trait anxiety, worry was not linked to reduced frontal-striatal-thalamo connectivity, impoverished frontal recruitment, or slowed responding during blocks without commission errors, contrary to accounts proposing a direct causal link between worry and impoverished attentional control. This leads us to propose a new model of the relationship between anxiety, worry and frontal engagement in attentional control versus off-task thought.

The transition of activism in the late 1960s from a non-violent, citizenship-based appeal for Civil Rights to a Nationalist, potentially violent call for revolution, marked a shift to a more radical and confrontational politics of social change. Hidden in this history are the narratives of women’s participation which dramatically revise the current historical record in these ground-breaking social movements. During this period, women and men organised for social change, often around identity-based issues, and challenged the status quo. This work examines two organisations which emerged in the late sixties as vanguards of an era defined by the self-determined chants of ‘black and red power’, a time of social and political rebellion against the leaders of the waning Civil Rights movement and an increasingly repressive government. This thesis seeks to foreground the hitherto unknown involvement of women in male-identified organisations such as the Black Panther Party and the American Indian Movement. It will highlight previously untold stories of key women activists in these two organisations. Not only will it demonstrate that women comprised a majority of the participants, but also that they performed all manner of functions ranging from high-level negotiations to meal preparation. Contemporary coverage of both organisations in the media obscured such involvement. The majority of the groups that defined themselves as revolutionary or radical were unable to deal with issues of gender inequality within their ranks. Many of these groups espoused a rhetorical philosophy of equality yet they were frequently unable to match such ideals in practice. This was certainly the case for the BPP and AIM. By equating liberation with manhood, women in these groups found themselves not only struggling for the cause but also competing with oppressive notions of masculinity. Women’s liberationists failed to offer any common cause, focusing on race-specific issues and advocating the separation of sexes which alienated women of color.

Genetic alterations in signaling pathways that control cell-cycle progression, apoptosis, and cell growth are common hallmarks of cancer, but the extent, mechanisms, and co-occurrence of alterations in these pathways differ between individual tumors and tumor types. Using mutations, copy-number changes, mRNA expression, gene fusions and DNA methylation in 9,125 tumors profiled by The Cancer Genome Atlas (TCGA), we analyzed the mechanisms and patterns of somatic alterations in ten canonical pathways: cell cycle, Hippo, Myc, Notch, Nrf2, PI-3-Kinase/Akt, RTK-RAS, TGFβ signaling, p53 and β-catenin/Wnt. We charted the detailed landscape of pathway alterations in 33 cancer types, stratified into 64 subtypes, and identified patterns of co-occurrence and mutual exclusivity. Eighty-nine percent of tumors had at least one driver alteration in these pathways, and 57% percent of tumors had at least one alteration potentially targetable by currently available drugs. Thirty percent of tumors had multiple targetable alterations, indicating opportunities for combination therapy. An integrated analysis of genetic alterations in 10 signaling pathways in >9,000 tumors profiled by TCGA highlights significant representation of individual and co-occurring actionable alterations in these pathways, suggesting opportunities for targeted and combination therapies. The U.S. Geological Survey initiated a research effort in 2014 to define the geologic framework of the Delmarva Peninsula inner continental shelf, which included new data collection and assembly of relevant extant datasets. Between 2006 and 2011, Science Applications International Corporation, under contract to the National Oceanic and Atmospheric Administration National Ocean Service, carried out 23 hydrographic surveys covering more than 4,100 square kilometers of the continental shelf using Reson multibeam echosounders and Klein towed sidescan sonars to update nautical charts along the Delmarva Peninsula. Acoustic backscatter data from these instruments are valuable for characterizing aspects of shallow geologic framework, including seafloor geology, sediment transport pathways, and marine resources. The data cover an area that extends from the entrance of Delaware Bay, Delaware, south to Parramore Island, Virginia, in water depths of about 3 to 35 meters below mean lower low water. Data were collected along lines spaced 40 meters apart, resulting in 40 to 100 percent seafloor coverage for multibeam bathymetry. Processed bathymetric data within the Delmarva Peninsula study area are available through a National Ocean Service interactive map interface, but towed sidescan data products are limited, and multibeam backscatter data products have not been available in the past. The U.S. Geological Survey obtained raw Reson multibeam data files from Science Applications International Corporation and the National Oceanic and Atmospheric Administration for 20 hydrographic surveys and extracted backscatter data using the Fledermaus Geocoder Toolbox from Quality Positioning Service. The backscatter mosaics produced by the U.S. Geological Survey for the inner continental shelf of the Delmarva Peninsula using National Oceanic and Atmospheric Administration data increased regional geophysical surveying efficiency, collaboration among government agencies, and the area over which geologic data can be interpreted by the U.S. Geological Survey. This report describes the methods by which the backscatter data were extracted and processed and includes backscatter mosaics and interpolated bathymetric surfaces. It is known that the lymph node yield in open cystectomy is variable and dependent, in some part, upon surgeon experience. This study, the largest of its kind reporting on outcomes associated with robot-assisted radical cystectomy, demonstrates that lymph node yields in experienced hands at the time of robot-assisted radical cystectomy is comparable to that seen in open series. OBJECTIVE To evaluate the incidence of, and predictors for, lymphadenectomy in patients undergoing robot-assisted radical cystectomy (RARC) for bladder cancer. PATIENTS AND METHODS Utilizing the International Robotic Cystectomy Consortium (IRCC) database, 527 patients were identified who underwent RARC at 15 institutions from 2003 to 2009. After stratification by age group, sex, pathological T stage, nodal status, sequential case number, institutional volume and surgeon volume, logistic regression was used to correlate variables to the likelihood of undergoing lymphadenectomy (defined as >= 10 nodes removed). RESULTS Of the 527 patients, 437 (82.9%) underwent lymphadenectomy. A mean of 17.8 (range 0-68) lymph nodes were examined. Tumour stage, sequential case number, institution volume and surgeon volume were significantly associated with the likelihood of undergoing lymphadenectomy. Surgeon volume was most significantly associated with lymphadenectomy on multivariate analysis. High-volume surgeons (> 20 cases) were almost three times more likely to perform lymphadenectomy than lower-volume surgeons, all other variables being constant [odds ratio (OR) = 2.37; 95% confidence interval (CI) = 1.39-4.05; P = 0.002]. CONCLUSION The rates of lymphadenectomy at RARC for advanced bladder cancer are similar to those of open cystectomy series using a large, multi-institutional cohort. There does, however, appear to be a learning curve associated with the performance of lymphadenectomy at RARC.
