Historically, medicinal plants have always had an important place in medicine. Medicinal plants processing represents a great challenge, due to their compounds sensitive to the environmental conditions that surround and degrade them. Mostly of these plants require to be dry to preserve its safety and medicinal properties; therefore, for proper drying, it is necessary to use sustainable devices that protect the desirable characteristics of plants from direct radiation. In this work, the kinetics of dehydration of three medicinal plants are presented in an indirect solar dryer. In addition, the experimental results were adjusted to nine mostly used models, to estimate the drying conditions required to achieve a desired final moisture content. Modified Page and Page were the models with better fit to experimental results. Furthermore, a computational simulation of temperature evolution and distribution inside the dryer is presented. These results agree with those obtained experimentally.

Simple Summary Computer-aided diagnosis systems to improve significant prostate cancer (PCa) diagnoses are being reported in the literature. These methods are based on either deep learning or radiomics. However, there is a lack of scientific evidence comparing these methods on the same external validation sets. The aim of our study was to compare the performance of a deep-learning model with the performance of a radiomics model for significant-PCa diagnosis on various cohorts. We collected multiparametric magnetic resonance images and pathology data from four patient cohorts (644 patients in total). One of the cohorts was used to develop a deep-learning model and a radiomics model. Both models were tested on the three remaining cohorts. The comparison shows that whereas the performance of the deep-learning model was higher on the training cohort, the radiomics model outperformed the deep-learning model in all the testing cohorts, making it a more accurate tool with which to detect clinically significant prostate cancer. Abstract The computer-aided analysis of prostate multiparametric MRI (mpMRI) could improve significant-prostate-cancer (PCa) detection. Various deep-learning- and radiomics-based methods for significant-PCa segmentation or classification have been reported in the literature. To be able to assess the generalizability of the performance of these methods, using various external data sets is crucial. While both deep-learning and radiomics approaches have been compared based on the same data set of one center, the comparison of the performances of both approaches on various data sets from different centers and different scanners is lacking. The goal of this study was to compare the performance of a deep-learning model with the performance of a radiomics model for the significant-PCa diagnosis of the cohorts of various patients. We included the data from two consecutive patient cohorts from our own center (n = 371 patients), and two external sets of which one was a publicly available patient cohort (n = 195 patients) and the other contained data from patients from two hospitals (n = 79 patients). Using multiparametric MRI (mpMRI), the radiologist tumor delineations and pathology reports were collected for all patients. During training, one of our patient cohorts (n = 271 patients) was used for both the deep-learning- and radiomics-model development, and the three remaining cohorts (n = 374 patients) were kept as unseen test sets. The performances of the models were assessed in terms of their area under the receiver-operating-characteristic curve (AUC). Whereas the internal cross-validation showed a higher AUC for the deep-learning approach, the radiomics model obtained AUCs of 0.88, 0.91 and 0.65 on the independent test sets compared to AUCs of 0.70, 0.73 and 0.44 for the deep-learning model. Our radiomics model that was based on delineated regions resulted in a more accurate tool for significant-PCa classification in the three unseen test sets when compared to a fully automated deep-learning model.

- Abstract
The majority of clinical geneticists in Chile work in the Metropolitan Region (78%). To expand the area of Telemedicine and support the management of the Ministry of Health, we present this Telegenetics development project that includes innovation of assistance and educational nature directed to regions. The implementation of the National Registry of Congenital Anomalies in Chile (RENACH) in the public and private systems, in December 2015, and the obligation to record and describe the anomalies in all newborns, constitutes a favorable scenario that would benefit from the support of clinical geneticists. This proposal brings together a team of 18 specialists and 6 fellows, professionals from different Universities and / or Hospitals of Health Services, in a collaborative project in the area of clinical genetics, which, supported by the HCUCH + CIMT Telemedicine project, will contribute to two regions of Chile better tools for the diagnosis and comprehensive management of newborn patients with congenital anomalies. It can serve as a pilot for a new way to support the registration of malformations throughout Chile and teach clinical genetics concepts. The expected benefits are to improve the quality of care and health management in patients with little-known diseases.

Radiomics applied in MRI has shown promising results in classifying prostate cancer lesions. However, many papers describe single-center studies without external validation. The issues of using radiomics models on unseen data have not yet been sufficiently addressed. The aim of this study is to evaluate the generalizability of radiomics models for prostate cancer classification and to compare the performance of these models to the performance of radiologists. Multiparametric MRI, photographs and histology of radical prostatectomy specimens, and pathology reports of 107 patients were obtained from three healthcare centers in the Netherlands. By spatially correlating the MRI with histology, 204 lesions were identified. For each lesion, radiomics features were extracted from the MRI data. Radiomics models for discriminating high-grade (Gleason score ≥ 7) versus low-grade lesions were automatically generated using open-source machine learning software. The performance was tested both in a single-center setting through cross-validation and in a multi-center setting using the two unseen datasets as external validation. For comparison with clinical practice, a multi-center classifier was tested and compared with the Prostate Imaging Reporting and Data System version 2 (PIRADS v2) scoring performed by two expert radiologists. The three single-center models obtained a mean AUC of 0.75, which decreased to 0.54 when the model was applied to the external data, the radiologists obtained a mean AUC of 0.46. In the multi-center setting, the radiomics model obtained a mean AUC of 0.75 while the radiologists obtained a mean AUC of 0.47 on the same subset. While radiomics models have a decent performance when tested on data from the same center(s), they may show a significant drop in performance when applied to external data. On a multi-center dataset our radiomics model outperformed the radiologists, and thus, may represent a more accurate alternative for malignancy prediction.

Background: Approval of hypomethylating agents in patients with chronic myelomonocytic leukaemia is based on trials done in patients with myelodysplastic syndromes. We aimed to investigate whether hypomethylating agents provide a benefit in subgroups of patients with chronic myelomonocytic leukaemia compared with other treatments. Methods: For this retrospective cohort study, data were retrieved between Nov 30, 2017, and Jan 5, 2019, from 38 centres in the USA and Europe. We included non-selected, consecutive patients diagnosed with chronic myelomonocytic leukaemia, who received chronic myelomonocytic leukaemia-directed therapy. Patients with acute myeloid leukaemia according to 2016 WHO criteria at initial diagnosis (ie, ≥20% blasts in the bone marrow or peripheral blood) or with unavailability of treatment data were excluded. Outcomes assessed included overall survival, time to next treatment, and time to transformation to acute myeloid leukaemia. Analyses were adjusted by age, sex, platelet count, and Chronic myelomonocytic leukaemia-Specific Prognostic Scoring System (CPSS). Patients were grouped by first received treatment with either hydroxyurea, hypomethylating agents, or intensive chemotherapy, and stratified by risk according to blast count, French-American-British subtype, CPSS, WHO 2016 subtype, and the eligibility criteria of the DACOTA trial (NCT02214407). Findings: 949 patients diagnosed with chronic myelomonocytic leukaemia between April 13, 1981, and Oct 26, 2018, were included. Median follow-up was 23·4 months (IQR 11·5–42·3) from diagnosis and 16·2 months (6·6–31·6) from start of first-line treatment. 412 (43%) of 949 patients received hypomethylating agents as first treatment, 391 (41%) hydroxyurea, and 83 (9%) intensive chemotherapy. Adjusted median overall survival for patients treated with hydroxyurea versus hypomethylating agents was 15·6 months (95% CI 13·1–17·3) versus 20·7 months (17·9–23·4); hazard ratio (HR) 1·39 (1·17–1·65; p=0·0002) and 14·0 months (9·8–17·2) versus 20·7 months (17·9–23·4; HR 1·55 [1·16–2·05]; p=0·0027) for those treated with intensive chemotherapy versus hypomethylating agents. In patients with myeloproliferative chronic myelomonocytic leukaemia (myeloproliferative CMML), median overall survival was 12·6 months (10·7–15·0) versus 17·6 months (14·8–21·5; HR 1·38 [1·12–1·70]; p=0·0027) for patients treated with hydroxyurea versus hypomethylating agents, and 12·3 months (8·4–16·6) versus 17·6 months (14·8–21·5; HR 1·44 [1·02–2·03]; p=0·040) for intensive chemotherapy versus hypomethylating agents. Background: Experimental and clinical data suggest that reducing inflammation without affecting lipid levels may reduce the risk of cardiovascular disease. Yet, the inflammatory hypothesis of atherothrombosis has remained unproved. Methods: We conducted a randomized, double-blind trial of canakinumab, a therapeutic monoclonal antibody targeting interleukin-1β, involving 10,061 patients with previous myocardial infarction and a high-sensitivity C-reactive protein level of 2 mg or more per liter. The trial compared three doses of canakinumab (50 mg, 150 mg, and 300 mg, administered subcutaneously every 3 months) with placebo. The primary efficacy end point was nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. RESULTS: At 48 months, the median reduction from baseline in the high-sensitivity C-reactive protein level was 26 percentage points greater in the group that received the 50-mg dose of canakinumab, 37 percentage points greater in the 150-mg group, and 41 percentage points greater in the 300-mg group than in the placebo group. Canakinumab did not reduce lipid levels from baseline. At a median follow-up of 3.7 years, the incidence rate for the primary end point was 4.50 events per 100 person-years in the placebo group, 4.11 events per 100 person-years in the 50-mg group, 3.86 events per 100 person-years in the 150-mg group, and 3.90 events per 100 person-years in the 300-mg group. The hazard ratios as compared with placebo were as follows: in the 50-mg group, 0.93 (95% confidence interval [CI], 0.80 to 1.07; P = 0.30); in the 150-mg group, 0.85 (95% CI, 0.74 to 0.98; P = 0.021); and in the 300-mg group, 0.86 (95% CI, 0.75 to 0.99; P = 0.031). The 150-mg dose, but not the other doses, met the prespecified multiplicity-adjusted threshold for statistical significance for the primary end point and the secondary end point that additionally included hospitalization for unstable angina that led to urgent revascularization (hazard ratio vs. placebo, 0.83; 95% CI, 0.73 to 0.95; P = 0.005). Canakinumab was associated with a higher incidence of fatal infection than was placebo. There was no significant difference in all-cause mortality (hazard ratio for all canakinumab doses vs. placebo, 0.94; 95% CI, 0.83 to 1.06; P = 0.31). Conclusions: Antiinflammatory therapy targeting the interleukin-1β innate immunity pathway with canakinumab at a dose of 150 mg every 3 months led to a significantly lower rate of recurrent cardiovascular events than placebo, independent of lipid-level lowering. We studied HLA class I (HLA-A, -B) and class II (HLA-DRB1, -DQB1) allele groups and alleles by PCR-SSP based typing in a total of 15,318 mixed ancestry Mexicans from all the states of the country divided into 78 sample sets, providing information regarding allelic and haplotypic frequencies and their linkage disequilibrium, as well as admixture estimates and genetic substructure. We identified the presence of 4268 unique HLA extended haplotypes across Mexico and find that the ten most frequent (HF > 1%) HLA haplotypes with significant linkage disequilibrium (Δ’≥0.1) in Mexico (accounting for 20% of the haplotypic diversity of the country) are of primarily Native American ancestry (A*02~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*08~DQB1*04, A*68~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*14~DQB1*03:01, A*24~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*04~DQB1*03:02, A*02~B*40:02~DRB1*04~DQB1*03:02, A*68~B*35~DRB1*04~DQB1*03:02, A*02~B*15:01~DRB1*04~DQB1*03:02). Admixture estimates obtained by a maximum likelihood method using HLA-A/-B/-DRB1 as genetic estimators revealed that the main genetic components in Mexico as a whole are Native American (ranging from 37.8% in the northern part of the country to 81.5% in the southeastern region) and European (ranging from 11.5% in the southeast to 62.6% in northern Mexico). African admixture ranged from 0.0 to 12.7% not following any specific pattern. We were able to detect three major immunogenetic clusters correlating with genetic diversity and differential admixture within Mexico: North, Central and Southeast, which is in accordance with previous reports using genome-wide data. Our findings provide insights into the population immunogenetic substructure of the whole country and add to the knowledge of mixed ancestry Latin American population genetics, important for disease association studies, detection of demographic signatures on population variation and improved allocation of public health resources. La orquídea Cattleya trianae Linden & Rchb.f. es reconocida como flor nacional de Colombia y se encuentra en peligro crítico, al presentar una reducción estimada mayor al 80% en los últimos 100 años, debido a la disminución en la calidad del hábitat y niveles altos de explotación o recolección. La familia Orchidaceae es una de las que mayor número de especies posee en el reino Plantae, con aproximadamente 900 géneros, de los cuales, el 38% es endémico de Colombia, concentrado en la región Andina, con 87,2%. Esta investigación buscó profundizar en el tema de la propagación como mecanismo de conservación, para lo cual, se determinó el efecto de la 6-Bencilaminopurina (6-BAP), sobre el desarrollo in vitro, en Fusagasugá (Cundinamarca), en un diseño de bloques completamente al azar, con 3 repeticiones. Cuerpos protocórmicos provenientes de semillas recolectadas en Pacho (Cundinamarca) fueron sembrados en medio básico Murashige & Skoog, enriquecido con 4 concentraciones de 6-BAP. Los resultados mostraron respuestas diferenciales a la adición de la citoquinina, ya que, con la concentración más alta, se obtuvo el mayor porcentaje de callos, con la de 0,05mg.L-1, el mayor porcentaje de brotes y sin la aplicación del regulador de crecimiento, el mayor porcentaje de raíz. The orchid Cattleya trianae Linden & Rchb.f. is recognized as the national flower of Colombia, the species is in a critical danger, presenting an estimated reduction of more than 80% in the last 100 years due to the decrease in habitat quality and high levels of exploitation or harvesting. The Orchidaceae family is one of the largest number of species in the Plant Kingdom, with approximately 900 genera, of which 38% are endemic in Colombia, concentrated in the Andean region with 87.2%. This research sought to deepen in the topic of the propagation as mechanism of protection for their conservation, for which the effect of cytokinin 6-benzylaminopurine (6-BAP) on development was determined in vitro in Fusagasugá (Cundinamarca), in a completely randomized block design with 3 replicates. The protocorm-like bodies from seeds collected in Pacho (Cundinamarca) were sown in a basic medium Murashige and Skoog enriched with 4 concentrations of 6-BAP. The results showed differential responses to the addition of cytokinin, as the highest concentration was obtained the highest percentage of callus, with the 0.05mg. The content of this article is the analysis of missionary processes of a chosen company for research, with the aim of reviewing its performance on information systems. The purpose of the project is to adjust the COBIT v5 reference model, based on a goals cascade, so that they focus on their own TI areas' processes, from Small and Medium Enterprises (PYME) of the health sector, reviewing the actual status of PYME taken as a study case, identifying processes, applicable tools and controls, and prioritizing these processes within the government framework. All this, in order to have PYMES of the health sector own a government model available, to allow them the development of actions, focused towards a continuous improvement of the organization, validating procedures for internal processes, and being aligned with the business corporate purpose, and/or making an internal auditing of TI, based on best practices with support and recognition, allowing an appropriate use of technology in the business processes.

Energy saving and environmental impact are two elements that must be considered in any sustainable process or proposal aimed at economic, social and cultural development. Moringa is a plant that has generated great interest in recent years due to its medicinal properties attributed, its leaves are an important source of food due to its high nutritional value. This work presents the characteristics of the dehydration of Moringa leaves using a solar dryer type greenhouse, open sun and electric oven. The tests were carried out at the Facultad de Ingenieria of the Universidad Autonoma de Campeche, in Campeche, Mexico, located at 19o51'00" north latitude and 90o31'59" west longitude, with a warm-humid climate, average values of solar irradiation of 980 W/m2. The best drying time was obtained in the greenhouse dryer, reaching up to 59 oC inside the drying chamber and with average drying times of 4 hours. The percentage of moisture in fresh and dry moringa leaves on average was measured at 82.29 and 4.09, respectively. The results show the technical feasibility of the solar drying of the moringa leaf guaranteeing a final product of quality and an important energy economy. Prostate cancer (PCa) is currently the second leading cause of cancer-related death in men. Systematic biopsies are the standard of care for PCa diagnosis. However, biopsies are invasive and prone to sampling errors. With magnetic resonance imaging (MRI) the whole prostate tissue can be visualized non-invasively. In this study we evaluate a radiomics approach to classify suspected lesions, into highgrade and low-grade PCa. The data comprised MRI, histology of radical prostatectomy specimens and pathology reports of 40 patients. Histology and MRI were correlated obtaining 72 lesions. Features were extracted to train a Support Vector Machine as classifier. Our experiments were performed in a fully automated framework, using 100x random split cross-validation, including extensive algorithm selection and hyperparameter optimization on the training set in each cross-validation. Our method achieved an AUC of 77[0.66–0.87], sensitivity 0.74[0.57–0.91] and specificity of 0.66[0.50–0.82], demonstrating the potential of radiomics to classify PCa lesion based on MRI.

Resumen: En el presente artículo se aborda el problema Flexible Job Shop Scheduling (FJSSP) con restricciones de transporte, con el objetivo de minimizar el makespan realizando la secuenciación y asignación de máquinas. Se llevó a cabo una revisión bibliográfica para orientar la metodología a utilizar, y a partir de allí, se decidió abordar el problema con un algoritmo genético, validando su efectividad a través de la comparación de los resultados obtenidos con distintas instancias propuestas en la literatura. Los resultados obtenidos muestran que el algoritmo genético propuesto es eficiente en las diferentes configuraciones del Job Shop clásico probadas, y para el Job Shop flexible con restricciones de transportes se presentan soluciones muy aproximadas a las mejores encontradas hasta el día de hoy. Abstract: We solved the Flexible Job Shop Scheduling Problem (FJSSP) with transportation constraints in order to minimize the makespan by sequencing and assigning machines. A bibliographic review was made in order to guide the methodology to be used. From there, we approached the problem with a genetic algorithm. We validated its effectiveness by comparing the results obtained with different instances proposed in the literature. The results obtained show that the proposed genetic algorithm is efficient in the different classic Job Shop configurations tested. The algorithm is able to find very approximate solutions to the best found to date for the Flexible Job Shop Scheduling Problem with transportation constraints. Resumo: Resolvemos o Problema de Agendamento de Oficina de Trabalho Flexível (FJSSP) com restrições de transporte para minimizar o makespan por sequenciamento e atribuição de máquinas. Uma revisão bibliográfica foi realizada para orientar a metodologia a ser utilizada. A partir daí, abordamos o problema com um algoritmo genético. Nós validamos sua eficácia comparando os resultados obtidos com diferentes instâncias propostas na literatura. Os resultados obtidos mostram que o algoritmo genético proposto é eficiente nas diferentes configurações clássicas de Job Shop testadas. O algoritmo é capaz de encontrar soluções muito aproximadas para o melhor encontrado até o momento para o Problema de Agendamento de Oficina de Trabalho Flexível com restrições de transporte.

Esta investigación se basó en el Modelo de Otto Kernberg y colaboradores, quienes consideran que la Identidad del Yo es el criterio central a la hora de evaluar la Organización de la Personalidad. Este trabajo expondrá los principales fundamentos teóricos y clínicos en referencia a la Identidad del Yo de este autor. En esta muestra, encontramos un alto grado de concordancia entre el diagnóstico de Trastorno de Personalidad según DSM y la presencia de Difusión de Identidad a través de la STIPO Dimensión Identidad y, por otra parte, Identidad Integrada en pacientes que no presentaron Trastorno de Personalidad según DSM.

This paper will expose the main theoretical and clinical foundations in reference to the Identity of the Self of this author. According to the results obtained in the sample, there is a high degree of concordance between the diagnosis of Personality Disorder according to DSM-IV diagnosis of Identity Diffusion through STIPO Identity Dimension.
