Purpose: To evaluate implant osseointegration in grafted autogenous bone blocks fixed with cyanoacrylate-based adhesive and screws. Also, grafted bone fixed either with an adhesive or screw was evaluated. Materials and Methods: Two surgical defects in the parietal region of rabbits (n = 12) were performed in each animal. Autogenous bone blocks obtained were fixed anteriorly with a screw or cyanoacrylate-based adhesive. After 30 and 45 days of grafting procedures, implants were placed in bone blocks. Histomorphometric and microcomputed tomography (micro-CT) analyses of the implant area were performed at 30 days after implant surgery in the screw (n = 6) and adhesive (n = 6) groups. Histomorphometric analyses of bone-grafted areas were performed at 60 and 75 days in the screw (n = 6) and adhesive (n = 6) groups. Histomorphometric evaluations were carried out in implant and grafted bone areas. The micro-CT parameters evaluated were bone-to-implant contact, bone area fraction occupancy, bone volume fraction, trabecular thickness, trabecular separation, and trabecular number. Results: No differences were observed in the micro-CT parameters for the screw and adhesive groups in all experimental periods. However, an increased quantity of immature bone volume was observed on the adhesive group in the grafted area after 75 days. The grafted area in the screw group (75 days) presented a decrease in the volume density of the immature bone compared with the screw group (60 days). There were no differences in both groups and experimental periods in soft tissue in the grafted area. Conclusion: No differences were observed in the osseointegration of implants placed in grafted autogenous bone blocks fixed with cyanoacrylate-based adhesive and screws. Aim: The purpose of this study was to investigate the biofilm effect on the hybrid ceramic-resin cement bond strength (BS) by comparing two methods. Methods: Teeth were distributed into groups (n=5), according to the resin cement (Maxcem Elite-(MC) or NX3 Nexus-(NX)) and degradation method (24h or 7 days in distilled water; 7 or 30 days incubated with biofilm and 30 days in sterile media). Treated surfaces of Vita Enamic blocks (5x6x7mm) were luted to treated or no treated dentin surfaces and light-cured. After 24h, beams were obtained (1x1x10mm) and stored accordingly. The flexural bond strength (FBS) was assessed by four-point bending test. Additional beams were obtained from new teeth (n=5), stored for 24h or 7 days in distilled water, and submitted to a microtensile bond strength (µTBS) assay. Failure modes were determined by scanning electron microscopy (100X). The flexure strength of the cements (n=10) was assessed by a four-point bending test. Data were analyzed by 1 and 2-ways ANOVA, and Tukey's test (α=0.05). Results: There was no significant difference between the degradation methods for the FBS groups. For the µTBS, the significant difference was as follows: NX 7days > NX 24h > MC 7days = MC 24h. Failure mode was mainly adhesive and mixed, but with an increase of cohesive within cement and pre-failures for the MC groups assessed by µTBS. NX had better performance than MC, regardless of the method. The treatment of diastemas should be conservative to preserve tooth structure, and porcelain veneers provide an esthetic solution with minimal tooth preparation. However, asymmetric diastemas can be difficult to treat and may require redistribution of spaces. Additionally, extensive proximal restorations may negatively impact periodontal health. Minor tooth movement with elastic separators can be used to redistribute the interdental spaces and provide space for the interdental papilla. The aim of this paper was to present a clinical treatment involving the movement of peg-shaped maxillary lateral incisors with elastic separators to rearrange the spaces of asymmetric diastemas, thereby managing the horizontal distance during rehabilitation of the smile with minimally invasive ceramic veneers. Aim: To evaluate the influence of cobalt-chromium (Co-Cr) coping fabrication methods and ceramic application on the marginal and internal fit of metal-ceramic crowns. Methods: Co-Cr copings for metal-ceramic crowns were prepared by lost wax casting or CAD-CAM machining of sintered blocks. The fit was analyzed using the silicone replica technique at four assessment points: marginal gap (MG), axial wall (AW), axio-occlusal (AO) angle, and central occlusal (CO) wall. After the initial analysis, the copings were ceramic-veneered with the layering technique, and the fit was again determined. Data were statistically analyzed by paired and unpaired Student's-t test (a=0.05). Results: Marginal and internal fit before ceramic application according to the coping manufacturing method showed significant differences only at CO (p < 0.001), with milled copings (137.98±16.71 µm) showing higher gap values than cast copings (112.86±8.57 µm). For cast copings, there were significant differences at MG (before 109.13±8.79 µm; after 102.78±7.18 µm) and CO (before 112.86±8.57 µm; after 104.07±10.63 µm) when comparing the fit before and after ceramic firing. For milled copings, there was significant difference only at AO (before 116.39±9.64 µm; after 108.54±9.26 µm). Conclusion: This study demonstrated that the coping fabrication method influenced the internal fit. Ceramic firing maintained or improved the fit of the metal-ceramic crowns. Objective: To compare the mechanical strength of rat femur after complete fracture treated absorbable and titanium plates and screws. Material and methods: A total of 10 male Wistar rats were divided in two groups, according to the femoral fracture fixation: titanium (TG) or absorbable plates (AG). A femur fracture was performed in one side after the corresponding plates of each group were adapted and fixated. The euthanasia occurred with 90 days postoperative, and the fracture femurs were compared with the opposite side control group (CG) in the mechanical tests, through the three-point bending flexural test, and the flexural strength was calculated. Using a digital caliper, the cross-section area was calculated. Data were submitted to the statistical analysis, with the significance level of 0.05. Results: The cross-section in the AG was higher than CG (p < 0.001). With respect of flexural strength, the AG was similar with CG (p = 1.00), and the TG showed lower flexural strength when compared with CG (p = 0.006) and with AG (p = 0.042). Conclusion: The group fixed with absorbable plates demonstrated increased cross-section area compared to the control group and increased flexural strength compared to the group fixed with titanium plates. Central dominance is an important element of an esthetic smile. Color, form, and size have been suggested as tools for assessing the dominance of maxillary teeth. A spectrophotometer can be used to determine the value, hue, and chroma. Correct sizing of restorations according to the central incisor dominance principle improves not only esthetics but also aspects of occlusion, such as anterior guidance. Refractory porcelain systems can effectively restore the color, shape, emergence profile, and incisal translucency. This report illustrates the esthetic and occlusal rehabilitation of the dominance of maxillary central incisors using fabricated minimal thickness refractory porcelain veneers. Treatment of the anterior teeth is challenging. Computer resources and software are available to facilitate a digital smile approach, thus allowing the dentist to plan and predict an esthetic result for a patient, particularly when an integrated multidisciplinary approach is necessary. Digital smile design has emerged as a tool in cosmetic dentistry that helps both professionals and the patient to visualize the result, in addition to facilitating a discussion between the two before establishing a final treatment decision. Here, we present a case involving a 27-year-old female who underwent multidisciplinary treatment initiated by digital planning using specific software, namely Cara Smile. The patient presented with complaints of maxillary tooth malalignment, staining, and fractures. To obtain the correct width-to-height ratio, a simulated gingival procedure was performed in the software. Gingivectomy was performed using digital simulation, and 90 days after surgery, cast and wax-up models maintaining the previously planned width-to-height ratio for the maxillary anterior teeth were made. Esthetic crowns and veneers were fabricated using the digital planning protocol. Therefore, Cara Smile is useful to facilitate the diagnosis through digital photos, improve communication between the various professionals involved in treatment, and guide the predictability of treatment and patient acceptance. Context: The aim was to evaluate the cutting efficiency of different diamond burs after successive cuts and repeated sterilization in an autoclave. The morphology and grit sizes were analyzed and correlated to cutting efficiency.Materials and Methods: Ten medium-grit diamond burs of five different manufacturers were investigated (KG, KG Sorensen; TH, Tri-Hawk; KM, Komet; HC, Heico; and FD, Frank Dental). Changes in the cutting efficiency of diamond burs on composite resin blocks were measured after five repeated cuts and after five sterilization cycles. Grit sizes were analyzed by scanning electron microscopy (SEM) and correlated to cutting efficiency. The data were statistically analyzed using 3-way ANOVA and Tukey's test (α = 0.05).Results: Significant differences were observed for diamond burs (P < 0.0001) and condition (P < 0.0001). FD presented the lowest mean cut time (21.88s), followed by KM (36.08s). TH (40.18s), HC (41.65s), and KG (42.23s) had the highest cut times. The number of cuts was not statistically significant. New burs had a significantly shorter cutting time (33.38s) when compared with the ones after sterilization cycles (39.55s). A moderate to strong positive correlation was found between diamond size and cutting time (Pearson's coefficient of 0.77).Conclusion: All diamond burs demonstrated lower cutting efficiency after repeated autoclaving. Introduction: The aim of this study was to compare root preparation of deciduous teeth with WaveOne Large (WO) and ProTaper F4 (PT) instruments with or without passive ultrasonic irrigation (PUI). Methods and Materials: Forty-eight deciduous teeth were scanned before and after root preparation and divided in four groups (n=12): WO+EDTA (WO); WO+EDTA with PUI (WOPUI); PTF+EDTA (PT); and PT+EDTA with PUI (PTPUI). Root canal enlargement by micro-computed tomography and root canal cleaning by scanning electron microscopy (SEM) were analyzed. Data were submitted to two-way ANOVA and Tukey's tests to analyze the root canal volume variation, and Kruskal-Wallis followed by Friedman and Wilcoxon tests were used to evaluate the cleaning efficacy. The level of significance was set at 0.05. Results: No significant difference occurred in total volume between groups (P>0.05). On analysis by thirds of the root canal, there was a difference in volume between WO (cervical) compared to WO and PT (apical), and PTPUI (middle and apical) (P<0.05). When cleaning of the thirds within the same group was compared, there was a significant difference in all groups (P<0.05). Among the groups, in the thirds, in the cervical a difference occurred (P=0.028), and the pairwise comparisons indicated statistically difference between WO and PT, and WO and PTPUI (P<0.05). In the pairwise comparisons among thirds, in the groups, difrences occured in all of them when compared the cervical and apical thirds (P<0.05). Conclusion: Passive ultrasonic irrigation has not improved the smear layer removal in deciduous teeth. Despite the differences in performance between WO and PT instruments, both were suitable for preparation of deciduous teeth. [ABSTRACT FROM AUTHOR]

Aim: This study evaluated the surface topography and bacterial adhesion of a hybrid ceramic and a nano ceramic resin composite after different surface finishes. Methods: Hybrid ceramic (Vita Enamic, VITA - EN) and nano ceramic resin composite (Lava Ultimate, 3M/ESPE - LV) blocks of 12 x 14 x 18 mm were cut into 1 mm slices. Each slice was divided into four specimens (6 x 7 mm) that were randomly allocated into 4 groups (n=8) according to the surface finishing: CTL - without surface finish (control); DB - wear with a diamond bur; VT - polishing system for hybrid ceramic (VITA); and DD - polishing system for ceramics (Dedeco). The specimens were analyzed regarding surface roughness parameters (Ra, Rz, Rq), sterilized and subjected to bacterial adhesion. Representative specimens from each group were observed by SEM and Confocal Laser Scanning Microscopy. Data were submitted to two-way ANOVA and Tukey's test (α=0.05). Results: EN had lower surface roughness and bacterial adhesion than LV (p<0.05), regardless of the surface finish. The highest values for all roughness parameters was observed in LVDB group, differing from the other groups, which were not significantly different. Smaller bacterial adhesion values (CFU/mL) were observed for ENDD and ENVT, which differed significantly from the other groups, except ENCTL. For LV groups there was no significant difference between the different surface finishes (p>0.05). The type of material and surface finish system significantly interfered with surface roughness parameters and bacterial adhesion. The hybrid ceramic performed better after polishing than the nano-ceramic resin. Conclusion: An adequate finishing/polishing technique should always be performed after any kind of adjustment to indirect restorations made with these materials tested. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the fracture resistance (FR), and push-out bond strengths (BS) of custom-made CAD/CAM post-and-cores manufactured with different esthetic materials. Methods: 90 single-rooted extracted teeth were selected, endodontically treated and prepared to receive the posts. The specimens were randomly divided into three equal groups according to the material: hybrid ceramic Vita Enamic (HC); nano-ceramic resin composite Lava Ultimate (RC); and experimental epoxy-resin reinforced by glass-fiber (FG). The post-and-cores were manufactured using CAD/CAM and cemented using a self-adhesive resin cement (Rely X Unicem2). A subgroup of 30 specimens (n=10) was subjected to fatigue (1,000,000 cycles at 5 Hz) and then to the FR test. Another subgroup with 60 specimens was submitted to the BS test, with and without fatigue. Data were submitted to analysis of variance (FR - one-way ANOVA; BS - two-way ANOVA) and Tukey's test (a= 0.05). Results: The FR values (mean±SD, in Newtons) were 414.5±83.9 (HC), 621.3±100.3 (RC), and 407.6±109.0 (FG), with RC showing significantly higher FR values (P< 0.05). For BS, there was no statistically significant difference among the materials, with and without fatigue (P> 0.05). The type of material used to obtain the CAD/CAM-customized post-and-cores had a significant effect on the FR, but not on the BS of the specimens. Introduction: Despite advances in adhesive dentistry, lasting bonds between indirect restorations and dentin remain a challenge. Objective: Therefore, the aim of this study was to evaluate the effect of chlorhexidine on the bond strength of self-adhesive cements to dentin after storage for 24 h and 90 days. Material and methods: Forty-eight human third molars crowns were included in polyvinyl chloride matrices using acrylic resin and worn to expose a flat dentin area. The specimens were randomly distributed into 8 groups (n = 12) in accordance with the self-adhesive cement (Rely×U200 and Smart Cem 2), the dentin treatment (application or not of 2 % chlorhexidine digluconate solution for 1 min) and the storage period (24 h or 90 d). Two cylinders' cements (1.4 × 1 mm) were constructed on the dentin surfaces of each sample, stored in distilled water for 24 h or 90 days and submitted to a micro-shear test. Subsequently, a failure analysis was performed to classify the failure into adhesive, cohesive, and mixed. Data were subjected to ANOVA and Tukey (0.05) tests. Results: There was a statistically significant difference between the materials (Rely×U200 > Smart Cem 2 - p < 0.05) regardless of the surface treatment and the storage period. Cleidocranial dysplasia (CCD), is an autosomal dominant disorder with a prevalence of 1 in 1,000,000 individuals. It is generally characterized by orofacial manifestations, including enamel hypoplasia, retained primary teeth, and impacted permanent and supernumerary teeth. The successful treatment involving a timing intervention (orthodontic-maxillofacial surgeons-restorative) is already described. However, the restorative treatment might improve the aesthetic final result in dentistry management for patients with cleidocranial dysplasia. Objective. Therefore, this clinical report presents a conservative restorative management (enamel microabrasion, dental bleaching, and direct composite resin) for aesthetic solution for a patient with CCD. Clinical Considerations. The cosmetic remodeling is a conservative, secure, and low cost therapy that can be associated with other procedures such as enamel microabrasion and dental bleaching to achieve optimal outcome. Additionally, the Golden Proportion can be used to guide dental remodeling to improve the harmony of the smile and the facial composition. Conclusions. Thus, dentists must know and be able to treat dental aesthetic problems in cleidocranial dysplasia patients. The intention of this paper is to describe a restorative approach with the cosmetic remodeling teeth (by grinding or addicting material) associated with enamel microabrasion and dental bleaching to reestablish the form, shape, and color of smile for patients with cleidocranial dysplasia. Objective: This study analyzed the influence of autogenous white adipose tissue on bone matrix development in critical-size defects created in rabbit calvaria. Materials and methods: A 15-mm-diameter defect was created in the calvaria of 42 rabbits. Twenty-one rabbits were treated with 86 mm³ of immediate transplant of fragmented white subcutaneous adipose tissue (WSAT); the others constituted the control group (sham). The animals were euthanized at 7,15, and 40 days postsurgery (n = 7), and the histological data were analyzed by histomorphometry and immunohistochemistry using the anti-adiponectin and parathyroid hormone 1-receptor (PTH1R) antibodies. Results: The calvariae treated with fragmented WSAT demonstrated significant bone formation. These results coincided with the significant presence of immunopositivity to adiponectin and PTH1R in loci, which in turn coincided with the increase in bonelike matrix deposited both in fat tissue stroma and adipocytes' cytoplasm. In contrast, the control group revealed a small amount of bone-matrix deposition and presented scarce PTH1R expression and a lack of immunostain for adiponectin. Conclusion: These results indicate that transplant of fragmented white subcutaneous adipose tissue may be an alternative to treatment of craniofacial bone deformities because adipose tissue suffers from osseous metaplasia and exhibits immunoexpression of the adiponectin and PTH1R, which are proteins associated with bone metabolism [ABSTRACT FROM AUTHOR]
