Our aim was to assess the gain in time to start PORT and the G2–G3 acute–subacute toxicity rate of whole breast adjuvant hypofractionated radiotherapy (AH-RT) administered up-front to the third-generation adjuvant CT (A-CT) in high-risk nodal positive BC in a preliminary report at 2 years.Methods: This retrospective study analysed the duration of treatment and safety of AH-RT administered up-front to A-CT in high-risk nodal positive BC patients (pts). Data on 45 pts treated between 2022– 2023 were collected. All pts underwent the third-generation A-CT after AH-RT 15– 5 fractions with or without a boost. Acute toxicity was scored according to CTCAE v5.0 for skin, pulmonary, and cardiac adverse events. Univariate and multivariate analyses were conducted to assess significant prognosticators for skin/lung/heart acute toxicities in the AH-RT 5– 15 fractions arms and CT (p < 0.005).Results: A reduction in the time to PORT initiation and overall adjuvant treatment time was recorded. RT was initiated 5 median weeks after surgery, and A-CT was performed 9 median weeks after surgery. The median duration of the entire adjuvant treatment was 35 weeks after surgery. At 6 months mean follow-up, no significant differences in G2–G3 toxicity were noted between the different hypofractionated RT arms, irrespective of the CT schedules, irradiated volumes, or boost (SIB or sequential) in univariate and multivariate analyses. In the multivariate analysis, no significant effects in CT schedules and AH-RT 5– 15 arms for skin/lung acute toxicities (p = 0.077 and p = 0.68; 0.67 and 0.87, respectively) were recorded.Conclusion: As a new PORT approach in BC, AH-RT up-front to the third-generation A-CT appeared safe with a low acute toxicity profile, providing an advantage in shortening the time from surgery to PORT initiation and the overall adjuvant treatment time.Keywords: PORT, third-generation chemotherapy, hypofractionated radiotherapy

Radiation recall syndrome is a limited field inflammatory reaction that occurs in a volume that was irradiated several months or years previously before being induced by a triggering factor. The most frequently reported phenomena are skin reactions; however, other organs could be involved, such as the lungs in radiation recall pneumonitis (RRP). It is a well-described inflammatory reaction that occurs within a pulmonary volume that was irradiated several months or years previously via radiotherapy (RT), triggered by factors such as drugs, including chemotherapy agents, immunotherapy, or vaccination. Indeed, during the COVID-19 pandemic, RRP following anti-COVID-19 vaccination or SARS-CoV2 infection was recently reported. ACE receptor-rich tissues such as lung or skin tissues were mainly involved. Herein, we present a case of RRP triggered by COVID-19 pulmonary infection in a woman who previously underwent adjuvant breast cancer radiotherapy. Although symptoms were typical, pulmonary CT findings depicted a unique distribution of ground-glass opacities (GGOs) throughout the previous radiation portals and mirror-like the radiation fields. A set of new nanohybrid polymeric formulations containing silicon compounds (like T8 silsesquioxanes or SiC nanoparticles) and small organic molecules (like azulene) have been deposited on adhesive aluminium tape, characterized and exposed to the outdoor environment of Casaccia (Rome), monitoring their temperature. Results of the first month of external campaign show that they exhibit PDRC effects.

We conducted a retrospective analysis in inpatients with confirmed COVID-19 on the anti-inflammatory and antithrombotic effects of enoxaparin and fondaparinux at prophylactic doses.Methods: This retrospective cohort study used patients with confirmed COVID-19 during the first months of the Italian outbreak from February 18 to April 30, 2020. Our aim was to compare clinical characteristics, prophylactic treatment, markers of inflammation, and thrombotic outcomes in inpatients positive for SARS-CoV2 during hospitalization associated with thromboprophylaxis with enoxaparin (40 mg or 60 mg once daily) or fondaparinux (2.5 mg once daily). Statistical analysis was conducted with using MatLab R2016B and ad hoc functions.Results: There were no significatant differences in clinical characteristics between patients that used enoxaparin or fondaparinux as thromboprophylaxis for SARS-CoV2. No differences were found in D-dimer and fibrinogen levels either, which were used as markers of inflammation during the infection at testing on admission and after 3 weeks.Significant differences in CRP, IL6, and LDH were found in patients after 21 days’ treatment.Discussion: Increased levels of fibrinogen and D-dimer in patients with confirmed COVID-19 have been reported in several studies. Our results showed that anti-inflammatory effects of fondaparinux and enoxaparin after 3 weeks of prophylactic treatment were similar when levels of fibrinogen and D-dimer were considered. Reduced levels of circulating progenitor cells (CPCs) and endothelial progenitor cells (EPCs) have been indicated as a risk factor for adverse cardiovascular outcomes and death in people at high cardiovascular risk. The aim of the present study was to evaluate the change in CPCs and EPCs levels in a population of young type 1 diabetic patients treated with intensive insulin regimen over a period of 2 years.Patients and Methods: A total of 204 type 1 diabetic patients, of whom 84 treated with insulin pump (CSII) and 120 with multiple daily insulin injections (MDI), completed a 2-year follow-up. Clinical measurements, including the indices of glycemic control and glucose variability, were collected at baseline and after 2 years. Both CPC and EPC cell count were assessed by flow cytometry.Results: Mean age of participants was 24.5 years and mean diabetes duration was 13.6 years. After 2 years, we found a significant reduction of HbA1c (− 0.3% versus baseline, P < 0.001), associated with decrease in mean amplitude of glucose excursion (MAGE) (− 0.5 mmol/L versus baseline, P< 0.001), continuous overall net glycemic action (CONGA) (− 0.2 mmol/L versus baseline, P=0.006), and blood glucose standard deviation (BGSD) (− 0.2 mmol/L versus baseline, P< 0.001). The number of all EPCs phenotypes, but not CPC cell count, significantly raised up in the entire population, with higher increase in CSII group. MAGE resulted as an independent predictor for increased levels of both CD34+ (P = 0.020) and CD34+KDR+ (P = 0.004) cell count in the whole population.Conclusion: Over a 2-year follow-up, young type 1 diabetic patients showed an increase in circulating EPCs levels, which was higher in patients with CSII. 7. On Classical Determinate Truth

Abstract

{The paper studies classical, type-free theories of truth and determinateness. Recently, Volker Halbach and Kentaro Fujimoto proposed a novel approach to classical determinate truth, in which determinateness is axiomatized by a primitive predicate. In the paper we propose a different strategy to develop theories of classical determinate truth in Halbach and Fujimoto's sense featuring a \emph{defined} determinateness predicate. This puts our theories of classical determinate truth in continuity with a standard approach to determinateness by authors such as Feferman and Reinhardt. The theories entail all principles of Fujimoto and Halbach's theories, and are proof-theoretically equivalent to Halbach and Fujimoto's CD+. They will be shown to be logically equivalent to a class of natural theories of truth, the \emph{classical closures of Kripke-Feferman truth}. The analysis of the proposed theories will also provide new insights on Fujimoto and Halbach's theories: we show that the latter cannot prove most of the axioms of the classical closures of Kripke-Feferman truth. This entails that, unlike what happens in our theories of truth and determinateness, Fujimoto and Halbach's \emph{inner theories} -- the sentences living under two layers of truth -- cannot be closed under standard logical rules of inference.}

8. Full Helicon thruster modeling

Abstract

This research work is part of a PhD project aiming to design a new helicon plasma thruster for cargo space missions, within a collaboration between the School of Aerospace Engineering of La Sapienza University and the ENEA Frascati research center. A 0-D global design model of the thruster is first established to identify the main operational conditions of the thruster, starting from a targeted performance in terms of specific impulse and thrust. These operational conditions are tested with dedicated simulations on the electromagnetic plasma-wave interaction within the ionization chamber, and on the electrostatic plasma acceleration within the magnetic nozzle. The former are carried out with a finite element electromagnetic code and confirm the technical feasibility of the identified operational RF power with a predefined chamber geometry. The latter are obtained with an electrostatic particle-in-cell code, which allows to estimate more precisely the achieved thruster performance. A sub-optimal thruster with an absorbed plasma power of around 1 kW, a thrust of around 10 mN and a specific impulse higher than 1000 s is successfully simulated. The established simulation framework will serve as the basis for the next optimization of the thruster geometry and operational conditions, with the ultimate goal of building and testing a new helicon thruster prototype.

Abstract

BackgroundMachine learning algorithms have been drawing attention at the joining of pathology and radiology in prostate cancer research. However, due to their algorithmic learning complexity and the variability of their architecture, there is an ongoing need to analyze their performance. ObjectiveThis study assesses the source of heterogeneity and the performance of machine learning applied to radiomic, genomic, and clinical biomarkers for the diagnosis of prostate cancer. One research focus of this study was on clearly identifying problems and issues related to the implementation of machine learning in clinical studies. MethodsFollowing the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) protocol, 816 titles were identified from the PubMed, Scopus, and OvidSP databases. Studies that used machine learning to detect prostate cancer and provided performance measures were included in our analysis. The quality of the eligible studies was assessed using the QUADAS-2 (quality assessment of diagnostic accuracy studies–version 2) tool. The hierarchical multivariate model was applied to the pooled data in a meta-analysis. To investigate the heterogeneity among studies, I2 statistics were performed along with visual evaluation of coupled forest plots. Due to the internal heterogeneity among machine learning algorithms, subgroup analysis was carried out to investigate the diagnostic capability of machine learning systems in clinical practice. ResultsIn the final analysis, 37 studies were included, of which 29 entered the meta-analysis pooling. The analysis of machine learning methods to detect prostate cancer reveals the limited usage of the methods and the lack of standards that hinder the implementation of machine learning in clinical applications. ConclusionsThe performance of machine learning for diagnosis of prostate cancer was considered satisfactory for several studies investigating the multiparametric magnetic resonance imaging and urine biomarkers; however, given the limitations indicated in our study, further studies are warranted to extend the potential use of machine learning to clinical settings. Recommendations on the use of machine learning techniques were also provided to help researchers to design robust studies to facilitate evidence generation from the use of radiomic and genomic biomarkers.

Abstract

The simulation of electronic properties is a pivotal issue in modern electronic structure theory, driving significant efforts over the past decades to develop protocols for computing energy derivatives. In this work, we address this problem by developing a strategy to integrate the quantum phase estimation algorithm within a fully differentiable framework. This is accomplished by devising a smooth estimator able to tackle arbitrary initial states. We provide analytical expressions to characterize the statistics and algorithmic cost of this estimator. Furthermore, we provide numerical evidence that the estimation accuracy is retained when an arbitrary state is considered and that it exceeds the one of standard majority rule. We explicitly use this procedure to estimate chemically relevant quantities, demonstrating our approach through ground-state and triplet excited state geometry optimization with simulations involving up to 19 qubits. This work paves the way for new quantum algorithms that combine interference methods and quantum differentiable programming.

We show that optimal control of the electron dynamics is able to prepare molecular ground states, within chemical accuracy, with evolution times approaching the bounds imposed by quantum mechanics. We propose a specific parameterization of the molecular evolution only in terms of interaction already present in the molecular Hamiltonian. Thus, the proposed method solely utilizes quantum simulation routines, retaining their favourable scalings. Due to the intimate relationships between variational quantum algorithms and optimal control we compare, when possible, our results with state-of-the-art methods in literature. We found that the number of parameters needed to reach chemical accuracy and algorithmic scaling are in line with compact adaptive strategies to build variational ansatze. The algorithm, which is also suitable for quantum simulators, is implemented emulating a digital quantum processor (up to 16 qubits) and tested on different molecules and geometries spanning different degrees of electron correlation.

Abstract

One of the main challenges concerning next generation tokamaks (such as DEMO) will be the development of a heat and power exhaust system able to withstand the large loads expected in the divertor region. A dedicated Divertor Tokamak Test (DTT) facility has been proposed in the EUROfusion Roadmap, with the aim of testing unconventional solutions, such as advanced magnetic configurations and liquid metal divertors. Magnetic control of alternative plasma configurations, such as the X-Divertor, will play a key role in the solution of the heat exhaust and yet can be a challenging point, due to increased sensitivity introduced by secondary x-points. To overcome the complications introduced by secondary x-points in advanced plasma shapes, magnetic control in DTT is achieved by resolving to the eXtreme Shape Controller, in order to control both the plasma shape and the secondary x-point position.

The impact of the many psychological difficulties linked to the migraine without aura (MoA) and recurrent headache attacks, such as anger and separation anxiety, on self-esteem has not yet been well investigated. The aims of the present study were to assess self-esteem levels in an objective way and to verify their possible relationship and correlation with the frequency and intensity of migraine attacks, in a population of children and adolescents affected by MoA.Methods: The study population was comprised of 185 children (88 males [M], 97 females [F]) aged between 6 and 12 years (mean 9.04 ± 2.41 years) referred consecutively for MoA to the Center for Childhood Headache, Clinic of Child and Adolescent Neuropsychiatry, Second University of Naples and of 203 healthy controls (95 M, 108 F) with mean age 9.16 ± 2.37 years, recruited from schools in Campania. The monthly headache frequency and the mean headache duration were assessed from daily headache diaries kept by all the children, and MoA intensity was assessed on a VAS (visual analog scale). To further evaluate their level of self-concept, all subjects filled out the Multidimensional Self-Concept Scale (MSCS).Results: The two study groups were comparable for age (P = 0.621), sex (P = 0.960), and z-score BMI (P = 0.102). The MoA group showed a significant reduction in the MSCS total score (P < 0.001) and in the Social (P < 0.001), Affect (P < 0.001), Family (P < 0.001), and Physical (P < 0.001) domains of the MSCS compared with the control group. The Pearson's correlation analysis showed a significantly negative relationship between MoA clinical characteristics and MSCS scores, and similarly the frequency of attacks was significantly negatively related with the Social (r = −0.3176; P < 0.001), Competence (r = −0.2349; P = 0.001), Physical (r = −0.2378; P = 0.001), and total (r = −0.2825; P < 0.001) scores of the MSCS. On the other hand, the MoA duration was significantly negatively related with the Social (r = −0.1878; P = 0.01), Competence (r = −0.2270; P = 0.002), Physical (r = −0.1976; P = 0.007), and total (r = −0.1903; P = 0.009) scores of the MSCS.Conclusion: Our study first identified differences in self-esteem levels, with an objective tool, in children affected by MoA compared with controls, suggesting the need for evaluation of self-esteem for better psychological pediatric management of children with migraine.Keywords: children, self-esteem, MSCS

Abstract

The Divertor Tokamak Test (DTT) facility will be equipped with in-vessel divertor coils able to locally modify the flux surfaces in the divertor region. When the first DTT divertor had not been selected, four in-vessel divertor coils were considered, with 10 turns each, fed by independent 4-quadrant SCR (thyristor) 0.5 kV - 5 kA - power supplies. This configuration was able to meet the design criteria in terms of requested performance and operational constraints, but one of the coils is not compatible with the geometry of the first DTT divertor, and another coil, scarcely efficient, can be removed. This paper discusses the design criteria, illustrates the possible revised layout, and illustrates the performance that can be achieved.
