1. The Role of Gender in the Onset, Development and Impact of Type 2 Diabetes Mellitus and Its Co-Morbidities

Abstract

Almost half a billion people worldwide are living with diabetes mellitus (DM). Complications associated with DM are common and approximately half of those people with DM suffer from at least one comorbidity. There is high mortality, morbidity and cost associated with these comorbidities which include cardiovascular disease, retinopathy, nephropathy, neuropathy and osteopathy. Gender influences the relative risk of developing complications from DM via differing mechanisms – both directly and indirectly. Generally, an increased relative risk of cardiovascular disease and kidney disease is noticed in women with DM compared to the non-DM context, where rates of both are much higher in men. Men appear to be at greater risk of diabetic retinopathy and also of insensate diabetic neuropathy, whereas women suffer from an increased rate of painful diabetic neuropathy compared to men. These differences are not clear cut and vary regionally and temporally, indicating that the field would benefit from further research on both the epidemiology and physiological mechanism of the observed patterns. These differences should be taken into account in treatment programmes for DM and its comorbidities.

2. DARE Project at the End of 1970. Dictionary of American Regional English. Final Report.

Abstract

Progress made in the DARE Project, begun in 1965 with the purpose of producing a "Dictionary of American Regional English," is described. Phase 1, the collection of necessary materials, has been completed. Ten Spoken Sources of this material and 12 Written Sources are listed and annotated. It is estimated that the grand total of items in the file will be more than 2,500,000, and the number of entries in the Dictionary should be over 100,000. Phase 2, editing and producing "DARE," is scheduled for completion in 1976. When completed, DARE is expected to have two distinct parts: (1) a conventional alphabetical word-list; and (2) a summary of the data from 1,000 questionnaires completed by communities in 50 states. A short list is provided of words that do not appear in existing dictionaries which DARE will treat. A sample of what the data summary will list is provided. (DB)

Abstract

Objective: The risk-benefit profile of antidepressant medications in bipolar disorder is controversial. When conclusive evidence is lacking, expert consensus can guide treatment decisions. The International Society for Bipolar Disorders (ISBD) convened a task force to seek consensus recommendations on the use of antidepressants in bipolar disorders.Method: Anexpert task force iteratively developed consensus through serial consensusbased revisions using the Delphi method. Initial survey items were based on systematic review of the literature. Subsequent surveys included new or reworded items and items that needed to be rerated. This process resulted in the final ISBD Task Force clinical recommendations on antidepressant use in bipolar disorder. Results: There is striking incongruity between the wide use of and the weak evidence base for the efficacy and safety of antidepressant drugs in bipolar disorder. Few well-designed, long-term trials of prophylactic benefits have been conducted, and there is insufficient evidence for treatment benefits with antidepressants combined with mood stabilizers. A major concern is the risk for mood switch to hypomania, mania, and mixed states. Integrating the evidence and the experience of the task force members, a consensus was reached on 12 statements on the use of antidepressants in bipolar disorder. Conclusions: Because of limited data, the task force could not make broad statements endorsing antidepressant use but acknowledged that individual bipolar patients may benefit from antidepressants. Regarding safety, serotonin reuptake inhibitors and bupropion may have lower rates of manic switch than tricyclic and tetracyclic antidepressants and norepinephrine-serotonin reuptake inhibitors. The frequency and severity of antidepressant-associated mood elevations appear to be greater in bipolar I than bipolar II disorder. Hence, in bipolar I patients antidepressants should be prescribed only as an adjunct to moodstabilizing medications.

•Plasmid ampCβ-lactamase genes were detected in 19% of phenotypic AmpC-producing Escherichia coliisolates.•The CIT group was the most common plasmid ampCfamily type (n=14).•PFGE typing and MLST analysis revealed these isolates were not clnally related, with the exception of two isolates that had an identical sequence type (ST720).•Establishing the prevalence and clonality of Escherichia colihabouring plasmid-mediated AmpC is important in order to implement evidence-based infection control measures.
