Observational prospective population-based incidence study. The main objective of this study was to assess the incidence of traumatic spinal cord injuries (TSCIs) and incidence rates, in order to provide estimates by age, gender, characteristics and cause. This study was conducted at acute-care spinal cord injury (SCI) hospitals and SCI centers from 11 Italian regions, between 1 October 2013 and 30 September 2014. Data of all consecutive patients with acute TSCI who met the inclusion criteria were obtained through case reporting by clinicians. The data were collected into a web database. Incidence rates and incidence rate ratios were calculated and stratified by age, gender, cause, level and completeness. From 50% of the entire population of Italy, 445 new cases of TSCI were included. The crude incidence rate of TSCI was 14.7 cases per million per year (95% CI: 13.4–16.4); the overall male to female ratio was 4:1 and the mean age was 54. Complete information was available in 85% of the sample and revealed tetraplegia in 58% and incomplete lesion in 67% of cases. The leading cause of TSCI was falls (40.9%) followed by road traffic accidents (33.5%). The leading cause was falls for patients over 55 and road traffic accidents for patients under 55. The changing trend of TSCI epidemiology concerns the increase in the average age of TSCI people and the increase of both cervical and incomplete lesions. The etiology shows the primacy of falls over road traffic accidents and suggests the need for a change in prevention policies.

- Abstract
Familial growth hormone deficiency provides an opportunity to identify new genetic causes of short stature. Here we combine linkage analysis with whole-genome resequencing in patients with growth hormone deficiency and maternally inherited gingival fibromatosis. We report that patients from three unrelated families harbor either of two missense mutations, c.347G>T p.(Arg116Leu) or c.1106C>T p.(Pro369Leu), in KCNQ1, a gene previously implicated in the long QT interval syndrome. Kcnq1 is expressed in hypothalamic GHRH neurons and pituitary somatotropes. Co-expressing KCNQ1 with the KCNE2 β-subunit shows that both KCNQ1 mutants increase current levels in patch clamp analyses and are associated with reduced pituitary hormone secretion from AtT-20 cells. In conclusion, our results reveal a role for the KCNQ1 potassium channel in the regulation of human growth, and show that growth hormone deficiency associated with maternally inherited gingival fibromatosis is an allelic disorder with cardiac arrhythmia syndromes caused by KCNQ1 mutations.

Objectives: ACST-2 is currently the largest trial ever conducted to compare carotid artery stenting (CAS) with carotid endarterectomy (CEA) in patients with severe asymptomatic carotid stenosis requiring revascularization. Methods: Patients are entered into ACST-2 when revascularization is felt to be clearly indicated, when CEA and CAS are both possible, but where there is substantial uncertainty as to which is most appropriate. Trial surgeons and interventionalists are expected to use their usual techniques and CE-approved devices. We report baseline characteristics and blinded combined interim results for 30-day mortality and major morbidity for 986 patients in the ongoing trial up to September 2012. Results: A total of 986 patients (687 men, 299 women), mean age 68.7 years (SD ± 8.1) were randomized equally to CEA or CAS. Most (96%) had ipsilateral stenosis of 70-99% (median 80%) with contralateral stenoses of 50-99% in 30% and contralateral occlusion in 8%. Patients were on appropriate medical treatment. For 691 patients undergoing intervention with at least 1-month follow-up and Rankin scoring at 6 months for any stroke, the overall serious cardiovascular event rate of periprocedural (within 30 days) disabling stroke, fatal myocardial infarction, and death at 30 days was 1.0%. Conclusions: Early ACST-2 results suggest contemporary carotid intervention for asymptomatic stenosis has a low risk of serious morbidity and mortality, on par with other recent trials. The trial continues to recruit, to monitor periprocedural events and all types of stroke, aiming to randomize up to 5,000 patients to determine any differential outcomes between interventions. Background and Purpose— The mechanisms underlying the relationship between migraine and ischemic stroke remain uncertain. The aim of the present study was to investigate the predictive value of major cardiovascular risk factors, cardiac interatrial abnormalities, and additional biological markers on migraine subtypes in young adults with ischemic stroke. Methods— Ischemic stroke patients aged 45 years or younger were consecutively enrolled as part of the Italian Project on Stroke in Young Adults. A comprehensive evaluation was performed including assessment of self-reported migraine and cardiovascular risk factors, interatrial right-to-left shunt, and genotyping to detect factor V Leiden and the G20210A mutation in the prothrombin gene. Results— Nine hundred eighty-one patients (mean age, 36.0±7.6 years; 50.7% women) were included. The risk of migraine with aura increased with decreasing number of cardiovascular risk factors (OR, 0.50; 95% CI, 0.24–0.99 for 2 factors or more), increasing number of thrombophilic variants (OR, 2.21; 95% CI, 1.05–4.68 for carriers of at least 1 of the 2), and the presence of right-to-left shunt (OR, 2.41; 95% CI, 1.37–3.45), as compared to patients without migraine. None of these factors had influence on the risk of migraine without aura. Conclusions— In young adults with ischemic stroke, low cardiovascular risk profile, right-to-left shunt, and an underlying procoagulant state are predictors of migraine with aura. The biological effects of these factors should be considered in future studies aimed at investigating the mechanisms linking migraine to brain ischemia.

Cette bactérie développe des résistances à la plupart des antibiothérapies conventionnelles, c’est pourquoi trouver de nouvelles approches thérapeutiques est un enjeu sociétal. Une de ces approches est de cibler des facteurs de virulence de PA impliqués dans la formation du biofilm, grâce à des molécules multivalentes de fortes affinités appelées glycoclusters. Parmi ces facteurs de virulence, PA-IL est une lectine de PA qui reconnait spécifiquement le D-galactose. Afin de cribler les interactions lectine/glycoclusters, il a été conçu un microsystème d’analyse basée sur une technologie biopuce. Cet outil permet une analyse à haut débit tout en utilisant que quelques picomoles d’échantillon. Les interactions lectine/glycoclusters sont évaluées par une mesure de fluorescence, d’IC50 et de Kd1. Le criblage sur biopuce de plus de 150 galactoclusters a permis de mettre en évidence que des molécules présentant un aglycone phényle proche du galactose étaient affins pour PA-IL2. L’influence de 4 aglycones aromatiques sur l’interaction avec PA-IL a été étudié par des mesures d’IC50 et de Kd dans le but de mieux comprendre les paramètres structures/affinité et notamment le rôle de l’aglycone aromatique. Il est ressortit, que les galactoclusters possédant un aglycone de type naphtyle ou biphényle augmentaient significativement l’interaction avec PA-IL. - Abstract
Background: The kidney-brain interaction has been a topic of growing interest. Past studies of the effect of kidney function on intracerebral hemorrhage (ICH) outcomes have yielded inconsistent findings. Although the second, main phase of the Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERACT2) suggests the effectiveness of early intensive blood pressure (BP) lowering in improving functional recovery after ICH, the balance of potential benefits and harms of this treatment in those with decreased kidney function remains uncertain. Study Design: Secondary analysis of INTERACT2, which randomly assigned patients with ICH with elevated systolic BP (SBP) to intensive (target SBP < 140 mm Hg) or contemporaneous guideline-based (target SBP < 180 mm Hg) BP management. Setting & Participants: 2,823 patients from 144 clinical hospitals in 21 countries. Predictors Admission estimated glomerular filtration rates (eGFRs) of patients were categorized into 3 groups based on the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) creatinine equation: normal or high, mildly decreased, and moderately to severely decreased (>90, 60-90, and <60 mL/min/1.73 m2, respectively). Outcome(s): The effect of admission eGFR on the primary outcome of death or major disability at 90 days (defined as modified Rankin Scale scores of 3-6) was analyzed using a multivariable logistic regression model. Potential effect modification of intensive BP lowering treatment by admission eGFR was assessed by interaction terms. Result(s): Of 2,623 included participants, 912 (35%) and 280 (11%) had mildly and moderately/severely decreased eGFRs, respectively. Patients with moderately/severely decreased eGFRs had the greatest risk for death or major disability at 90 days (adjusted OR, 1.82; 95% CI, 1.28-2.61). De nouvelles approches thérapeutiques sont basées sur l’inhibition de facteurs de virulence notamment certaines lectines. Les lectines sont des protéines qui reconnaissent spécifiquement des motifs saccharidiques. Les glycoclusters sont des molécules de synthèse multivalentes (portant plusieurs motifs saccharidiques) capables d’inhiber l’action de ces lectines bactériennes. Pour étudier les interactions lectines-glycoclusters, nous avons conçu un microsystème d'analyse basé sur une technologie biopuce. Les glycoclusters portant une étiquette ADN sont immobilisés par hybridation spécifique avec le brin complémentaire d'une puce à ADN (DNA Directed Immobilization, DDI). Cette technologie permet de réaliser une étude multiplexée et une analyse à haut débit de ces interactions tout en utilisant une très faible quantité d’échantillon (quelques pmoles)1. Cet outil permet également d'évaluer l'affinité de chaque glycocluster pour une lectine donnée par une mesure de fluorescence, d’IC50 et de Kd2. Le DDI-glycocluster array permet donc un criblage de glycoclusters capables d’interagir avec des lectines de Pseudomonas aeruginosa qui seraient impliquées dans la formation du biofilm, dans le but de trouver de nouveaux agents anti-infectieux. Nous avons principalement étudié PA-IL qui reconnait spécifiquement le D-Galactose. Le criblage d’une banque de 150 galactoclusters a montré que ceux possédants un cœur centré mannose portant des bras longs et flexibles ainsi qu’un aglycone phényle étaient affins pour PA-IL3. Par la suite, l’influence d'autres aglycones aromatiques sur l’interaction avec PA-IL a été étudié par des mesures d’IC50 et de Kd. De cette étude est ressortit, que les galactoclusters possédant un aglycone type naphtyle et biphényle augmentaient significativement l’interaction avec PA-IL. Nous étudions ici Pseudomonas aeruginosa qui est une bactérie opportuniste impliquée dans 10-30% des maladies nosocomiales par le fait qu’elle a développé des résistances aux antibiothérapies conventionnelles. Le DDI-glycoarray permet donc un criblage d’une banque de glycoclusters capable d’interagir avec une lectine de Pseudomonas aeruginosa dans le but de trouver de nouveaux agents anti-infectieux. Nous avons principalement étudié PA-IL, une lectine de Pseudomonas aeruginosa reconnaissant spécifiquement le D-Galactose3. Le DDI-glycoarray a été utilisé pour déterminer l’affinité d’environs 150 glycoclusters vis-à-vis de PA-IL. Dans cette étude, l’affinité du glycocluster pour PA-IL est importante lorsque la mesure de fluorescence émise par la lectine est grande, l’IC50 est grand et le Kd petit. Ces trois mesures permettent le classement des glycoclusters les uns par rapport aux autres vis-à-vis de leur affinité pour PA-IL. Elles permettent également de déterminer les paramètres de configurations des glycolcusters qui sont important pour leur interaction avec PA-IL. De cette étude est donc ressortit que des glycoclusters à cœur centré mannose portant des bras longs et flexibles ainsi qu’un cycle aromatique près du galactose terminal augmenté significativement l’interaction avec PA-IL. Aggressive forms of multiple sclerosis (MS) represent a limited group of demyelinating diseases that rapidly progress to severe disability. Currently available therapies are poorly effective against these clinical entities. Recently, it has been demonstrated that intense immunosuppression followed by autologous haematopoietic stem cell transplantation (AHSCT) can affect the clinical course of individuals with severe MS and completely abrogate the inflammatory activity detected by MRI. We report the result of the Italian phase 2 GITMO study, a multicentre study in which 21 MS patients, who were rapidly deteriorating and not responding to the usual therapeutic strategies, were treated with this procedure. The clinical effect of the treatment is long lasting, with a striking abrogation of inflammation detected by MRI findings. These results support a role for intense immunosuppression followed by ASCT as treatment in rapidly evolving MS cases unresponsive to conventional therapies.

In patients with ischemic stroke, endovascular treatment results in a higher rate of recanalization of the affected cerebral artery than systemic intravenous thrombolytic therapy. However, comparison of the clinical efficacy of the two approaches is needed.We randomly assigned 362 patients with acute ischemic stroke, within 4.5 hours after onset, to endovascular therapy (intraarterial thrombolysis with recombinant tissue plasminogen activator [t-PA], mechanical clot disruption or retrieval, or a combination of these approaches) or intravenous t-PA. Treatments were to be given as soon as possible after randomization. The primary outcome was survival free of disability (defined as a modified Rankin score of 0 or 1 on a scale of 0 to 6, with 0 indicating no symptoms, 1 no clinically significant disability despite symptoms, and 6 death) at 3 months.A total of 181 patients were assigned to receive endovascular therapy, and 181 intravenous t-PA. The median time from stroke onset to the start of treatment was 3.75 hours for endovascular therapy and 2.75 hours for intravenous t-PA (P0.001). At 3 months, 55 patients in the endovascular-therapy group (30.4%) and 63 in the intravenous t-PA group (34.8%) were alive without disability (odds ratio adjusted for age, sex, stroke severity, and atrial fibrillation status at baseline, 0.71; 95% confidence interval, 0.44 to 1.14; P=0.16). Fatal or nonfatal symptomatic intracranial hemorrhage within 7 days occurred in 6% of the patients in each group, and there were no significant differences between groups in the rates of other serious adverse events or the case fatality rate.The results of this trial in patients with acute ischemic stroke indicate that endovascular therapy is not superior to standard treatment with intravenous t-PA.
