2. Implementation of an interprofessional education program in a community teaching hospital and its impact on student perceptions of other healthcare professions.

In this single-center, prospective study we evaluated the impact of an interprofessional education program (IPE) on healthcare students' perceptions of other healthcare professions. The program consisted of four one-hour, roundtable, case-based sessions with students and several facilitators from medicine, nursing, pharmacy, and physician assistant programs. Included students were 18 years of age or older and currently enrolled in a healthcare program during the study time frame. The primary outcome of student perceptions of other healthcare professions was measured by baseline and follow-up surveys using the Adapted Attitudes Toward Interprofessional Health Care Teams scale. Perceptions of students who participated in the IPEP (intervention group) were compared to similar healthcare program students who did not participate in the program (control group). Overall, the intervention group had significantly higher perceptions of other healthcare professions comparing pre-intervention to post -intervention data (pre-intervention mean ± SD of 57.2 ± 5.24; post-intervention mean 60.7 ± 5.63; p =.02). This improvement in perceptions was also seen when comparing the post-intervention group to the control group (control mean 56.7 ± 5.1; post-intervention mean 60.7 ± 5.63; p =.008). 4. Paroxetine-induced galactorrhea

Introduction Antidepressant-induced galactorrhea and increases in prolactin levels have been sporadically reported among SSRI-related side effects. Objectives Current rapport presents a case of 39 y.o. female who developed several adverse effects on paroxetine - including galactorrhoea - which improved on discontinuation of the drug. Methods Case discussion of 39-year old woman who was treated with paroxetine for her panic disorder and developed galactorrhoea with hyperprolactinemia that resolved upon discontinuation of the drug. Additionally, authors performed the literature search using PubMed and Embase to review the similar cases and used PDSP Database to assess the latest pharmacodynamic (PD) properties of paroxetine and other SSRI’s. Results Literature review (1966–2020) revealed 24 prior published case reports of SSRI-induced galactorrhea in users of paroxetine (n=4), escitalopram (n=4), sertraline (n=2), citalopram (n=2), fluoxetine (n=3), fluvoxamine (n=2) and other non-assessable reports (n=7). Elevated prolactin levels were mostly observed with paroxetine and escitalopram and rarely with fluoxetine, fluvoxamine and sertraline. PD-assessment showed the highest binding affinity of paroxetine and escitalopram to SERT (kPi = 0.07-0.2 and 0.8-1.1 nmol/L respectively) compared to other SSRI’s, in absence of other relevant PD-properties Conclusions Increasing body of evidence shows that galactorrhea does occur among paroxetine female users. Pharmacodynamic mechanism of action is poorly understood but given the modern insights in relationship in serotonin and dopamine circuits, we suggest that strong SERT inhibitory properties of paroxetine might lead to a tonic suppressive influence on dopamine neurotransmission. This physiological link may explain an increase in prolactin levels through dopamine depletion in the tuberoinfundibular pathway. Disclosure No significant relationships.

Specific group dynamics of Open Mic follow, including the role of parallel process, mutual aid and reciprocity, are addressed, with attention to their relevance for adult learners. 8. Taking Care of the Puerto Rican Patient: Historical Perspectives, Health Status, and Health Care Access

Introduction Hispanics are the largest minority group in the US at 18% of the population, of which Puerto Ricans are the second largest subgroup. Puerto Ricans have poorer health status than other US Hispanic and non-Hispanic populations. Thus, health care providers need to know about and distinguish the health care problems of Puerto Ricans to improve their health. Although there are some published curricula addressing how to provide health care to Hispanic populations, none address the specific needs of Puerto Ricans. Methods We developed a 60-minute interactive workshop consisting of a PowerPoint presentation and case discussion aimed at increasing health care providers' knowledge and understanding of the historical perspective that led to Puerto Rican identity, health issues and disparities, and the health care access problems of mainland and islander Puerto Ricans. Evaluation consisted of pre- and postworkshop questionnaires. Results There were a total of 64 participants with diverse ethnoracial identities including medical students, residents, faculty, physicians, researchers, administrators, and students/faculty from nursing, occupational therapy, genetic counseling, biomedical sciences, and social work programs. A comparison of pre- and postworkshop data showed a statistically significant increase in participants' confidence in meeting all learning objectives. Participants positively commented on the interactive nature of the workshop, the case discussion, and the historical perspective provided. Discussion With the increasing migration of Puerto Ricans to the US mainland this module can uniquely improve the preparation of current and future health care providers to provide competent care to Puerto Rican patients.

Background:Most secondary transmission of COVID-19 is occurring in a hospital setting. To decrease person-to-person contact, health care providers have built many isolation wards. However, out-of-hospital professionals cannot access patient information, which has greatly reduced the efficiency of treatment; it is inconvenient for health care professionals to issue a case discussion with professionals from other wards. This article mainly introduces a mobile telehealth system (MTS) applied to facilitate patient information presentation and case discussion. Materials and Methods:The MTS searches patient information, which is stored in hospital intranet, and uses five modules to display patient information. By a request/response module and a real-time interaction module, we successfully conducted case discussions. In addition, we took measures in three areas to prevent patient information leakage. Results:The system uses mobile collaboration technology to present patient information and support case discussion. MTS was officially launched for 37 days, during which it has been used 3,061 times. Conclusions:The building of the MTS not only provides convenience and benefit for health care professionals, but also reduces person-to-person contact. [ABSTRACT FROM AUTHOR]

Background: The reflecting team is a way of exploring clinical or educational dilemmas. A key difference from traditional case discussion is that the case presenter is not actively involved in the group discussion but instead listens to the ideas and thoughts generated, without contributing to the narrative. The origins of the reflecting team are in family therapy and there is little evidence on how successfully the technique can be transposed to clinical or educational practice. Aim: To evaluate the effectiveness of the reflecting team as an educational intervention. Design and setting: Interpretive action research involving 10 GPs who were either enrolled on the Induction and Refresher Scheme or who were members of a learning set of GP educators. Method: Interview data were audio-recorded, transcribed verbatim and thematic analysis was used to identify themes. Results: The reflecting team appeared to be an effective and acceptable intervention. Key features of the reflecting team included the opening up of new perspectives, feeling validated by peers, the need for ground rules and the perception of the reflecting team as a practical tool. Conclusions: The reflecting team is a new way of undertaking case discussion in GP education and seems to offer some advantages over traditional case discussion. It lends itself well to learning groups and it is possible that the idea might be transposed to clinical settings too. [ABSTRACT FROM AUTHOR]

在後現代社會中,培養師資生能以多元觀點來理解學生學習與教師教學的複雜性十分重要。多元觀點幫助師資生更貼近學生學習與教師教學的全貌,有助其看見學生的潛能、重新界定問題與解決問題。本研究旨在探討不同案例教學法對師資生案例解決問題多元觀點的影響。本研究為準實驗研究,以臺灣東部某大學50名師資生為研究對象,分別進行實驗組班級案例討論(26人)與控制組案例影片(24人)兩種案例教學。研究結果顯示,「案例討論」比「案例影片」更能增進師資生案例問題解決之多元觀點,且達到顯著差異。 In the post-modern society, it is very important to cultivate preservice students’ pluralistic views of student learning and teacher teaching in order to catch its complexity. Multi-perspective help preservice teachers see closely the whole picture of student learning and teacher teaching, which would help them to see the students’ potential, redefine and solve problems. This study examines the influence of two case-based instructions (case discussion and case video) on pre-service teachers’ problem solving multi-perspective. The participants in this quasi-experimental research were two classes of pre-service teachers with experimental group 26 students in class case discussion and control group 24 students in case video in an eastern national university of Taiwan. Result shows that compared to watching case video, the case discussion significantly improve pre-service teachers’ multi-perspective on case problem solving.

Studies have reported that persons with developmental disabilities have the lowest rate of labor force participation, relative to other disabilities due to various factors. This paper presents two cases studies of persons with ID who were successfully provided a hybrid supported employment approach of ‘train and place model’ and ‘place and train model’ by the Psychiatric Rehabilitation Services (PRS) team. Conducted a retrospective file review highlighting the process of supported employment approach adapted for persons with ID. The ethical approval was obtained from the Institute Ethical Committee. The hybrid supported employment approach focuses on enhancing client’s vocational potential and skills, prepare for job ready, find a suitable job placement. Further, the approach helped in improving the sense of independence self-esteem and quality of life of the clients. A hybrid supported employment approach could be an effective method in aiding persons with developmental disabilities in India seek, get, and keep jobs; it will also help them deal with unique challenges they face in the workplace as well as loss of or gaps in employment. Involvement of families in the intervention will help minimize negative expressed emotions and distress.

23. Developing a Web-Based Asynchronous Case Discussion Format on Social Media to Teach Clinical Reasoning: Mixed Methods Study.

Background: Case-based learning conferences are valuable to trainees, but growing clinical demands hinder consistent attendance. Social media increasingly acts as a venue for trainees to supplement their education asynchronously. We designed and implemented a web-based asynchronous clinical case discussion series on the Twitter social media platform to fill this educational gap., Objective: The aim of this mixed methods study is to examine the nature of interactions among web-based case discussion participants and assess local attitudes regarding the educational intervention., Methods: Starting in February 2018, we posted clinical vignettes to a dedicated Twitter account with the prompt "What else do you want to know?" to stimulate discussion. The authors replied in real time when case discussion participants requested additional details. Additional data about the case were posted at regular intervals to the discussion thread to advance the overall case discussion. Participants were asked to explain their reasoning and support their conclusions when appropriate. Web-based engagement was assessed using Twitter Analytics. Participants' posts were qualitatively analyzed for themes, with special attention to examples of using clinical reasoning skills. A codebook of types of participant posts and interactions was refined iteratively. Local engagement and attitudes at our institution were assessed by surveying internal medicine trainees (n=182) and faculty (n=165) after 6 months., Results: Over a 6-month period, 11 live case discussions were engaged with by users 1773 times. A total of 86 Twitter profiles spanning 22 US states and 6 countries contributed to discussions among participants and the authors. Participants from all training levels were present, ranging from students to faculty. Interactions among participants and the case moderators were most commonly driven by clinical reasoning, including hypothesis-driven information gathering, discussing the differential diagnosis, and data interpretation or organization. Of 71 respondents to the local survey, 29 (41%) reported having a Twitter account. Of the 29 respondents with Twitter accounts, 17 (59%) reported participating in the case discussions. Respondents agreed that case participation increased both their clinical reasoning skills (15/17, 88%) and clinical knowledge (13/17, 76%)., Conclusions: A social media-based serialized case discussion was a feasible asynchronous teaching method for engaging web-based learners of all levels in a clinical reasoning discussion. - Abstract
Objective: To survey the current status of teaching case discussion, so as to provide reference for better discussion of teaching cases in the future and further improvement of teaching quality. Methods: Questionnaire survey was performed in the study. Participants in the survey were students who were engaged in the training of clinical pharmacists supported by the Health and Family Planning Commission, including those who were receiving or had completed the training. Results: A total of 97 copies of questionnaire were issued and 97 copies that were duly retrieved were all valid. The participants of the survey were those who once took part in the discussion of teaching cases. Fifty-nine pharmacists (61%) came from the training base of clinical pharmacists, where teaching case discussions were routinely held, and the discussions that were held in 75 units (77%) were attended by multi-disciplinary professionals, and in 70 units (72%), discussion were held once every week. Through teaching case discussion, 89 pharmacists (92%) thought that their clinical thinking ability and analytical ability were improved to a certain extent, and 84 pharmacists (87%) held that their literature retrieval ability was more or less increased. The main problems that still existed among them were: 69 pharmacists (71%) were not familiar with the diagnosis and treatment principles of the diseases which were beyond their own profession, 62 pharmacists (64%) were lacking in the pharmacological knowledge beyond their own profession, and 12 pharmacists (12%) had little interest in the discussion. Conclusion: The survey results indicate that pharmacists could surely benefit from teaching case discussion and their various abilities could all be improved to a more or less degree. However, there still exist many problems, and the form and contents of teaching case discussion remain to be further improved. [ABSTRACT FROM AUTHOR]

ملخص بحث محددات استخدام الاخصائى الاجتماعى لمؤتمر الحالة فى المجال النفسى تتعدد مجالات الممارسة المهنية لطريقة خدمة الفرد، ومنها المجال النفسى الذى يعد من أهم مجالات الممارسة المهنية، ولا يخفي علي أحد من المتخصصين في مجال الصحة النفسية أن العلاقة بين انتشار الأمراض النفسية وما قد تحدثه من نتائج وآثار سلبية علي المريض نفسه وأسرته ومجتمعه، أهمها الضعف أو الخلل في أداء المريض النفسي والمحيطين به لأدوارهم المختلفة، ولما کان الأخصائي الاجتماعي أحد أعضاء فريق العلاج في المجال النفسي، وأن کثيرا من المرضي النفسيين قد تلعب الظروف الاجتماعية والأسرية دورا في إصابتهم ببعض الأمراض النفسية أو قد ينتج عن الإصابة بها مشکلات اجتماعية ونفسية عديدة، فإن فهم الأخصائي الاجتماعي لطبيعة تلک الأمراض وأنواعها وأسبابها والمشکلات التي قد تنتج عنها وطرق وأساليب التعامل مع نسق المريض وأسرته وفريق العلاج وحدود ومتطلبات دوره وما يتصل بذلک من توافر المعارف العلمية والمهارات المهنية. وتعتبر اجتماعات مناقشة الحالة وسيلة هامة لتنسيق المعلومات التي تجمع عن الحالة وتفسيرها، ويتم في هذه الاجتماعات عرض الحالة من کل جوانبها واستعراض الجوانب الإرشادية والعلاجية المناسبة للحالة، ويعد مؤتمر الحالة فرصة لفريق العمل مع الحالة الفردية لتبادل المعلومات والآراء والخبرات، ومناقشة خطط العلاج، وتعديلها حسب متطلبات الموقف، ووضع الخطط البديلة، ويساعد استعداد الاخصائيين الاجتماعيين للعمل مع الحالة الفردية المعروضة على مؤتمر الحالة من جمع معلومات عن الحالة، وتحديد جوانب القوة فى شخصية العميل، واستکمال مناطق الدراسة طبقا لطبيعية الحالة، ويتطلب عمل الاخصائيين الاجتماعيين أثناء انعقاد مؤتمر الحالة مناقشة مشکلات الحالات الفردية المعروضة عليهم، وتقديم تفسير منطقى للعوامل التى أدت إلى حدوثها، واقتراح أسلوب العمل المناسب معها، ويتطلب عمل الاخصائيين الاجتماعيين أيضا مع الحالة الفردية بعد الانتهاء من مؤتمر الحالة تحديد جوانب القصور في تنفيذ الخطة العلاجية، ومتابعة سير الخطة المتفق عليها، واتساقا مع الطرح السابق الذى يؤکد الأهمية التى يمثلها مؤتمر الحالة في المجال النفسى؛ إلا أن الواقع يؤکد ندرة فى الکتابات والدراسات الأجنبية والعربية التى تناولت الموضوع؛ لذا فقد تحددت القضية الرئيسية للبحث فى الوقوف على محددات استخدام الاخصائيين الاجتماعيين لمؤتمر الحالة فى المجال النفسى. يعد هذا البحث من نمط البحوث الوصفية، اعتمادا على المنهج الوصفى، تم تطبيقه على جميع الاخصائيين الاجتماعيين العاملين بالمستشفيات النفسية بإقليم القاهرة الکبرى، والبالغ عددهم(87) اخصائى اجتماعى، واعتمدت الباحثة في البحث الميداني على مقياس محددات استخدام الاخصائى الاجتماعى لمؤتمر الحالة فى المجال النفسى، وقد وتوصل البحث إلى الإجابة على تساؤلات الدراسة والتى تمثلت في انخفاض استعداد الاخصائيين الاجتماعيين لاستخدام مؤتمر الحالة في المجال النفسى، وضعف قدراتهم على تنفيذ المهام المطلوبة منهم أثناء انعقاد المؤتمر، وضعف قدراتهم على تنفيذ المهام المطلوبة منهم بعد انعقاد مؤتمر الحالة، وأکدت نتائح البحث وجود بعض المعوقات التى تحول دون قدرة الاخصائيين الاجتماعيين على استخدام مؤتمر الحالة، وخلص البحث إلى عدد من المقترحات لتطوير قدرة الاخصائيين الاجتماعيين على استخدام مؤتمر الحالة في المجال النفسى. This research is of the type of descriptive research, depending on the descriptive approach, it was applied to all social workers working in psychiatric hospitals in the Greater Cairo Region, and the number of (87) social workers, and the researcher adopted in the field research on the scale of the determinants of the use of the social worker for the case conference in the psychological field The research found an answer to the study’s questions, which were represented in the low willingness of social workers to use the case conference in the psychological field, their weak ability to carry out the tasks required of them during the conference, and their weak ability to carry out the tasks required of them after the case conference was held, and the results of the research confirmed the existence of Some of the obstacles that prevent social workers from using the case conference, and the research concluded a number of proposals to develop the capacity of social workers to use the case conference in the psychological field.

Multidisciplinary tumor boards (TBs) is an integral part of cancer care. Emerging evidence shows that effective TB implementation is crucial. It remains largely unknown how digital solutions can assist effective TB conduction. This study aimed to evaluate the impact of a digital solution on case discussion during TB meetings in four cancer types: Breast, Gastrointestinal (GI), Ear, Nose & Throat (ENT), and Hematopathology. A prospective study was performed to evaluate case discussion time during TB meetings pre- and post-solution implementation, at an US academic healthcare cancer center. Data were recorded by a Nurse Navigator for each case during TB meetings. Case discussion times were recorded for 2312 patients, at a total of 286 TB meetings. Significant decreases were observed in the average case discussion time for the breast and GI TBs. We observed a trend for reduction in discussion time variance for all TBs, suggesting the potential of the digital solution to standardize case discussion via provision of uniform case presentation and data access. Postponement rate decreased from 23 to 10% for ENT TB. This study demonstrated that the digital solution enhanced effective TB implementation, with heterogeneity across cancer types.

The aim of this study was to determine the benefits of RCD on nurses themselves, this literature through identification from the Pubmed database, Science direct and online wiley, use the keywords 'Reflection' and 'Case' and 'Nursing'. The method of searching articles uses PICOT technique, Prism Flow diagram, abstraction and synthetic data. Through fulltext screening, double publication and eligibility, 455 research articles were found. The next step is to screen through the inclusion criteria and exclusion criteria so that the final result of the article found is 4 articles. Articles have a lot to explain about the benefits RCD for nurses, RCD can add to the knowledge of nurses, minimize the gap theory and practice so that errors can be resolved. This review of the literature was conducted to define avoidant restrictive food intake disorder (ARFID) and provide the current evidence-based treatment modalities and implications for Nurse Practitioners. A specific case is used to illustrate the daily struggles of a pediatric patient with ARFID that include bullying, self-doubt, and anxiety related to eating. There is a need for increased awareness of the disorder to increase identification of the disorder, enhance the research on treatment modalities for the disorder, and most importantly, to increase the quality of life for those with the disorder and their family members.

Current gendered interventions for couples experiencing intimate partner violence often do not address the complex systemic issues, nor allow for couple therapy. Experience of working in men's behaviour change group programs (or batterer programs) and with heterosexual couples, indicates this is not enough. Change options for men perpetrating domestic violence and support for partners are generally provided by individual services or gendered groups, located in separate agencies. This article initially poses some of the inadequacies of this gendered, 'one size fits all' ideology, which may compromise safety when couples stay together or there is shared parenting. It then explores further questions and possibilities raised by utilising a systemic lens in working with partners and families. It is possible to work systemically with a couple without compromising safety and accountability, where there is flexibility at intake, a both/and approach, ongoing assessment of risk, collaboration between professionals, and good transitions between individual and conjoint work. This article describes a systemic practice framework for working with partners, utilising individual, group, and conjoint sessions in a recursive four phase approach. [ABSTRACT FROM AUTHOR]

36. Application of case discussions to improve anatomy learning in Syria

Aims: Designing a new approach for learning gross anatomy to improve students′ motivation to study anatomy and to enable them to learn independently through case discussion. Materials and Methods: The study included newly registered students in the first academic year. The total number of students was 165, who were divided by alphabetical order into 15 groups of 11 students. Each group was led by one faculty member and each faculty member lead 3 groups. Each group met twice a week for 2 weeks to discuss one case related to the upper limb anatomy. Students took pre- and posttests and completed an opinion questionnaire about the case discussions. Results: The pretest score shows that 20% of the students received grades of 60% or above and that 80% received grades less than 60%. The posttest showed that 45% of the students received grades of 60% or above and that 55% received grades less than 60%. There was a significant difference between the pre- and posttest for grades 60% (P = 0.0023). In addition, 17% of students achieved the same results (less than 60%) in both the pre- and posttests. The questionnaire revealed that all students stated that the discussion method was useful in their learning process, helped them to increase their motivation to study anatomy (85%), know the usefulness of studying anatomy (84%), and understand the problems (91%). Conclusions: The implementation of the case discussion in teaching anatomy can increase the students′ understanding and motivate them to learn.

Background: Poor communication between physicians and nursing staff could result in inadequate interprofessional collaboration with negative effects on patient health. In order to ensure optimal health care for patients, it is important to strengthen interprofessional communication and collaboration between physicians and nurses during their education. Aim: The aim of this project is to foster communication for medical and nursing students through interprofessional case discussions and simulated ward rounds as a form of training.Method: In 2013-15 a total of 39 nursing students and 22 medical students participated in eight seminars, each covering case discussions and simulated ward rounds. The seminar was evaluated based on student assessment of the educational objectives.Results: Students who voluntarily signed up for the seminar profited from the interprofessional interaction and gathered positive experiences working in a team.Conclusion: Through practicing case discussions and ward rounds as a group, interprofessional communication could be fostered between medical and nursing students. Students took advantage of the opportunity to ask those from other profession questions and realized that interprofessional interaction can lead to improved health care.

The role of research ethics consultants in biomedical research has increased to the point that they have an advisory capacity at all research institutes. For such professionals, we have established an educational system, which includes teaching materials, training methods, and nationwide educational workshops. These workshops have served to examine the developed system’s usefulness and to provide realistic training for consultant candidates. In addition, we have used the current workshop to encourage clinical research investigators (and related personnel) to participate. Subsequently, we examined its usefulness as an opportunity to provide exposure to research ethics. In October 2019, we held a 1-day pilot workshop in Tokushima, Japan, which included a basic lecture in research ethics. During the lecture, two sets of materials were used for case discussion: case 1, covering issues related to a clinical trial, and case 2, covering issues related to human biological specimens. At the end of the workshop, a 30-item self-reporting anonymous questionnaire was provided. Of the 13 total participants, 9 (70%) were clinical research investigators and related personnel, while 6 (46%) had no direct intention to act as consultants. Respondents indicated that case 2 was more difficult than case 1. However, both cases were generally accepted as educational materials; thus, satisfaction was expressed in relation to both. As the evaluations of the cases were generally positive, we will further examine the usefulness of participation in the workshop in the cultivation of research ethics in the investigator community.

Trauma is widespread, and its symptoms can adversely impact wellbeing at end of life, a time when hospice seeks to maximize quality of life. This article reviews research on trauma at end of life, provides an overview of trauma-informed principles, and explores possibilities for applying trauma-informed care through an illustrative case study of a patient at end of life. The case discussion applies findings from the literature using Feldman's Stepwise Psychosocial Palliative Care model as a roadmap. As shown in the case study, trauma-related symptoms may complicate care, making it an important subject of clinical attention for interdisciplinary hospice team members. As part of this team, social workers are particularly well suited to provide more targeted interventions where indicated, though all members of the team should take a trauma-informed approach. Lastly, this article reflects on the need for organizations to take a systems-level approach when implementing trauma-informed care and suggests implications for practice through a universal approach to trauma and the need for trauma-specific assessments and interventions at end-of-life, along with areas for future research.

Few studies have examined social work student outcomes after engagement in interprofessional (IP) learning experiences. We examined self-reported attitudes and skills self-efficacy among social work students before and after their engagement in IP Forums. The data comes from social work students who participated in 2016-2018 Fall IP Forums focusing on opioid use disorder. Using the Interprofessional Attitudes Scale (IPAS) and the Interprofessional Collaborative Competency Attainment Scale, revised (ICCAS), we assessed students’ self-reported attitudes (IPAS) and skills self-efficacy (ICCAS) before and after the Fall IP Forums. Paired t-tests identified significant changes in IPAS and ICCAS scores. The introduction of newer anticoagulants requires clinicians to fully appreciate, interpret, and correctly apply the use of coagulation assays, such as the prothrombin time (PT), activated partial thromboplastin time (APTT), and anti-factor Xa assays. For oral vitamin K antagonists, the international normalized ratio (INR) is a predictor of anticoagulation intensity. However, for direct oral anticoagulants (DOACs), the PT or INR and APTT are unable to quantify the level of anticoagulation intensity as there is a poor correlation between plasma concentration of DOAC with these routine coagulation assays and that significant anticoagulant effect may still be present despite normal or near normal results for these routine assays. In the USA, there are 5 DOACs available including dabigatran, a direct thrombin inhibitor and 4 direct factor Xa inhibitors (FXaI), each with varying indications, doses, pharmacodynamic, and pharmacokinetic characteristics. A thorough understanding of these properties aids in the management of the periprocedural or bleeding patient. In this first section of this manuscript, we will review the laboratory tests that are commonly performed for assessing a patient’s coagulation status with known DOAC exposure. The second section will describe 3 real-world challenging case studies in DOAC-treated patients, with a focus on presenting clinical queries, interpretation of baseline laboratory tests with interpretations, followed by the case discussion to combine interpretation of appropriate laboratory tests with clinical patient considerations in an effort to guide clinical decision-making.

Background: Most secondary transmission of COVID-19 is occurring in a hospital setting. To decrease person-to-person contact, health care providers have built many isolation wards. However, out-of-hospital professionals cannot access patient information, which has greatly reduced the efficiency of treatment; it is inconvenient for health care professionals to issue a case discussion with professionals from other wards. This article mainly introduces a mobile telehealth system (MTS) applied to facilitate patient information presentation and case discussion. Materials and Methods: The MTS searches patient information, which is stored in hospital intranet, and uses five modules to display patient information. By a request/response module and a real-time interaction module, we successfully conducted case discussions. In addition, we took measures in three areas to prevent patient information leakage. Results: The system uses mobile collaboration technology to present patient information and support case discussion. MTS was officially launched for 37 days, during which it has been used 3,061 times. Conclusions: The building of the MTS not only provides convenience and benefit for health care professionals, but also reduces person-to-person contact.

Severe stroke and neurodegenerative diseases often cause limitations in communication and willing capability. Decision processes in these conditions assume primarily a positive medical indication for any intervention. If not obtainable from an individual by itself, by a disposal or by a legal custodian, the presumed will of a patient has to be detected carefully. Evidence can be raised by an interview of relatives or an individual case discussion in a local ethical comitee. Stroke and dementia can raise the need for palliative care, especially a sufficient analgesia as well as other severe illnesses. Pain in demented persons is often underrated and undertreated. The diagnosis of dementia alone does not limit the indication for curative therapy in general. Ethical comitees or ethical visits are helpful instruments to find out an adequate decision in difficult situations.

Background and purpose Upon graduation and licensing, pharmacists work very closely with pharmacy technicians. Despite this, opportunities for learning together as students are limited. We developed and implemented a pilot intraprofessional event for pharmacy and pharmacy technician students. The purpose of this study was to evaluate the perceived value and learner confidence through analysis of participant feedback. Education activity and setting Pharmacy students from the University of Waterloo School of Pharmacy and pharmacy technician students from Lambton College participated in an intraprofessional event that included a three-station practice objective structured clinical exam (OSCE) and a case discussion regarding a methadone dispensing error, followed by a facilitated debrief. Upon completion of the event, students were invited to complete an online feedback questionnaire. Findings Twenty-one pharmacy students and 22 pharmacy technician students participated in the event. Twenty-one students completed the questionnaire, for a response rate of 49%. The majority of respondents agreed or strongly agreed that the event enhanced learning and confidence in working together to provide interprofessional care. Students seemed to find the OSCE to be particularly valuable. Feedback suggestions for improvement indicated a desire for more activities and time allocated to the event. Summary We designed and implemented a pilot intraprofessional event that was well-received by pharmacy students and pharmacy technician students. This supports the development of future similar events.

In adolescents and young women, there is limited data on the type of replacement, route of administration, and ideal doses to be used in systemic hormone therapy administered for the treatment of hypogonadism. In particular, management of patients with complicated systemic diseases or at risk of thrombophilia may present significant challenges. We present a case of a 15-year-old adolescent girl with hypogonadism and coexisting medical conditions, who was evaluated for systemic hormone therapy.
