Recent works in contexts like the Internet of Things (IoT) and large-scale Cyber-Physical Systems (CPS) propose the idea of programming distributed systems by focussing on their global behaviour across space and time. In this view, a potentially vast and heterogeneous set of devices is considered as an "aggregate" to be programmed as a whole, while abstracting away the details of individual behaviour and exchange of messages, which are expressed declaratively. One such a paradigm, known as aggregate programming, builds on computational models inspired by field-based coordination. Existing models such as the field calculus capture interaction with neighbours by a so-called "neighbouring field" (a map from neighbours to values). This requires ad-hoc mechanisms to smoothly compose with standard values, thus complicating programming and introducing clutter in aggregate programs, libraries and domain-specific languages (DSLs). To address this key issue we introduce the novel notion of "computation against a neighbour", whereby the evaluation of certain subexpressions of the aggregate program are affected by recent corresponding evaluations in neighbours. We capture this notion in the neighbours calculus (NC), a new field calculus variant which is shown to smoothly support declarative specification of interaction with neighbours, and correspondingly facilitate the embedding of field computations as internal DSLs in common general-purpose programming languages -- as exemplified by a Scala implementation, called ScaFi. This paper formalises NC, thoroughly compares it with respect to the classic field calculus, and shows its expressiveness by means of a case study in edge computing, developed in ScaFi.

- Abstract
Evidence against the use of prophylactic drain after gastrectomy are increasing and ERAS guidelines suggest the benefit of drain avoidance. Nevertheless, it is unclear whether this practice is still widespread. We conducted a survey among Italian surgeons through the Italian Gastric Cancer Research Group and the Polispecialistic Society of Young Surgeons, aiming to understand the current use of prophylactic drain. A 28-item questionnaire-based survey was developed to analyze the current practice and the individual opinion about the use of prophylactic drain after gastrectomy. Groups based on age, experience and unit volume were separately analyzed. Response of 104 surgeons from 73 surgical units were collected. A standardized ERAS protocol for gastrectomy was applied by 42% of the respondents. Most of the surgeons, regardless of age, experience, or unit volume, declared to routinely place one or more drain after gastrectomy. Only 2 (1.9%) and 7 surgeons (6.7%) belonging to high volume units, do not routinely place drains after total and subtotal gastrectomy, respectively. More than 60% of the participants remove the drain on postoperative day 4–6 after performing an assessment of the anastomosis integrity. Interestingly, less than half of the surgeons believe that drain is the main tool for leak management, and this percentage further drops among younger surgeons. On the other hand, drain’s role seems to be more defined for duodenal stump leak treatment, with almost 50% of the surgeons recognizing its importance. Routine use of prophylactic drain after gastrectomy is still a widespread practice even if younger surgeons are more persuaded that it could not be advantageous.

Contains fulltext : 296536.pdf (Publisher’s version ) (Closed access), IMPORTANCE: Understanding the learning curve of a new complex surgical technique helps to reduce potential patient harm. Current series on the learning curve of minimally invasive distal pancreatectomy (MIDP) are mostly small, single-center series, thus providing limited data. OBJECTIVE: To evaluate the length of pooled learning curves of MIDP in experienced centers. DESIGN, SETTING, AND PARTICIPANTS: This international, multicenter, retrospective cohort study included MIDP procedures performed from January 1, 2006, through June 30, 2019, in 26 European centers from 8 countries that each performed more than 15 distal pancreatectomies annually, with an overall experience exceeding 50 MIDP procedures. Consecutive patients who underwent elective laparoscopic or robotic distal pancreatectomy for all indications were included. Data were analyzed between September 1, 2021, and May 1, 2022. EXPOSURES: The learning curve for MIDP was estimated by pooling data from all centers. MAIN OUTCOMES AND MEASURES: The learning curve was assessed for the primary textbook outcome (TBO), which is a composite measure that reflects optimal outcome, and for surgical mastery. Generalized additive models and a 2-piece linear model with a break point were used to estimate the learning curve length of MIDP. Case mix-expected probabilities were plotted and compared with observed outcomes to assess the association of changing case mix with outcomes. The learning curve also was assessed for the secondary outcomes of operation time, intraoperative blood loss, conversion to open rate, and postoperative pancreatic fistula grade B/C. RESULTS: From a total of 2610 MIDP procedures, the learning curve analysis was conducted on 2041 procedures (mean [SD] patient age, 58 [15.3] years; among 2040 with reported sex, 1249 were female [61.2%] and 791 male [38.8%]). This retrospective analysis of the prospective IGOMIPS registry reports on 1191 minimally invasive pancreatic resections (MIPR) performed in Italy between 2019 and 2022, including 668 distal pancreatectomies (DP) (55.7%), 435 pancreatoduodenectomies (PD) (36.3%), 44 total pancreatectomies (3.7%), 36 tumor enucleations (3.0%), and 8 central pancreatectomies (0.7%). Spleen-preserving DP was performed in 109 patients (16.3%). Overall incidence of severe complications (Clavien-Dindo & GE; 3) was 17.6% with a 90-day mortality of 1.9%. This registry analysis provided some important information. First, robotic assistance was preferred for all MIPR but DP with splenectomy. Second, robotic assistance reduced conversion to open surgery and blood loss in comparison to laparoscopy. Robotic PD was also associated with lower incidence of severe postoperative complications and a trend toward lower mortality. Fourth, the annual cut-off of & GE; 20 MIPR and & GE; 20 MIPD improved selected outcome measures. Fifth, most MIPR were performed by a single surgeon. Sixth, only two-thirds of the centers performed spleen-preserving DP. Seventh, DP with splenectomy was associated with higher conversion rate when compared to spleen-preserving DP. Eighth, the use of pancreatojejunostomy was the prevalent reconstruction in PD. Ninth, final histology was similar for MIPR performed at high- and low-volume centers, but neoadjuvant chemotherapy was used more frequently at high-volume centers. Finally, this registry analysis raises important concerns about the reliability of R1 assessment underscoring the importance of standardized pathology of pancreatic specimens. In conclusion, MIPR can be safely implemented on a national scale. Further analyses are required to understand nuances of implementation of MIPR in Italy.

- Abstract
Objectives: To determine the reproducibility of the National Comprehensive Cancer Network (NCCN) resectability status classification for pancreatic cancer. Background: The NCCN classification defines 3 resectability classes (resectable, borderline resectable, locally advanced), according to vascular invasion. It is used to recommend different approaches and stratify patients during clinical trials. Methods: Prospective, multicenter, observational study (trial ID: NCT03673423). Main outcome measure was the interobserver agreement of tumor assignment to different resectability classes and quantification of vascular invasion degrees. Agreement was measured by Fleiss' k (k = 1 perfect agreement; k = 0 agreement by chance). Sixty-nine computed tomography (CT) scans of pathologically confirmed pancreatic adenocarcinoma were independently reviewed in a blinded fashion by 22 observers from 11 hospitals (11 surgeons and 11 radiologists). Rating differences between surgeons or radiologists and between hospitals with different volumes (≥60 or <60 resections/year) were assessed. Results: Complete agreement among 22 observers was recorded in 5 CT scans (7.2%), whereas 25 CT scans (36.2%) were variously assigned to all 3 resectability classes. Interobserver agreement varied from fair to moderate (Fleiss' k range: 0.282-0.555), with the lowest agreement for borderline resectable tumors. Assessing vascular contact ≤180° had the lowest agreement for all vessels (k range: 0.196-0.362). The highest concordance was recorded for venous invasion >180° (k range: 0.619-0.756). Neither reviewers' specialty nor hospital volume influenced the agreement. Conclusions: There is high variability in the assignment to resectability categories, which may compromise the reliability of treatments recommendations and the evidence of trials stratifying patients in resectability classes. Criteria should be revised to allow a reproducible classification.

Engineering and programming approaches for collective adaptive systems often leverage ensemble-like abstractions to characterise a subset of devices as a domain for a given task or computation. In this paper, we address the problem of programming the dynamic evolution of distributed computational aggregates, through neighbour-based coordination. This is a problem of interest, since several situated activities (especially in large-scale settings) require decentralised collaboration, and need to be sustained by limited subsets of devices. These subsets may vary dynamically due to delegation, completion of local contributions, exhaustion of resources, failure, or change in the device set induced by the openness of system boundaries. In order to study and develop how distributed aggregates progressively take form by local coordination, we build on the field-based framework of aggregate processes, and extend it with techniques to support more expressive evolution dynamics. We propose novel algorithms for more effective propagation and closure of the boundaries of dynamic aggregates, based on statistics on the information speed and a notion of progressive closure through wave-like propagation. We verify the proposed techniques by simulation of a paradigmatic case study of multihop message delivery in mobile settings, and show increased performance and success rate with respect to previous work.

- Abstract
The safety of minimally invasive procedures during COVID pandemic remains hotly debated, especially in a country, like Italy, where minimally invasive techniques have progressively and pervasively entered clinical practice, in both the hepatobiliary and pancreatic community. A nationwide snapshot of the management of HPB minimally invasive surgery activity during COVID-19 pandemic is provided: a survey was developed and conducted within AICEP (Italian Association of HepatoBilioPancreatic Surgeons) with the final aim of conveying the experience, knowledge, and opinions into a unitary report enabling more efficient crisis management. Results from the survey (81 respondents) show that, in Italian hospitals, minimally invasive surgery maintains its role despite the COVID-19 pandemic, with the registered reduction of cases being proportional to the overall reduction of the HPB surgical activity. Respondents agree that the switch from minimally invasive to open technique can be considered as a valid option for cases with a high technical complexity. Several issues merit specific attention: screening for virus positivity should be universally performed; only expert surgical teams should operate on positive patients and specific technical measures to lower the biological risk of contamination during surgery must be followed. Future studies specifically designed to establish the true risks in minimally invasive surgery are suggested. Furthermore, a standard and univocal process of prioritization of patients from Regional Healthcare Systems is advisable.

We aimed to determine the impact of pre-operative isolation on postoperative pulmonary complications after elective surgery during the global SARS-CoV-2 pandemic. We performed an international prospective cohort study including patients undergoing elective surgery in October 2020. Isolation was defined as the period before surgery during which patients did not leave their house or receive visitors from outside their household. The primary outcome was postoperative pulmonary complications, adjusted in multivariable models for measured confounders. Pre-defined sub-group analyses were performed for the primary outcome. A total of 96,454 patients from 114 countries were included and overall, 26,948 (27.9%) patients isolated before surgery. Postoperative pulmonary complications were recorded in 1947 (2.0%) patients of which 227 (11.7%) were associated with SARS-CoV-2 infection. Patients who isolated pre-operatively were older, had more respiratory comorbidities and were more commonly from areas of high SARS-CoV-2 incidence and high-income countries. Although the overall rates of postoperative pulmonary complications were similar in those that isolated and those that did not (2.1% vs 2.0%, respectively), isolation was associated with higher rates of postoperative pulmonary complications after adjustment (adjusted OR 1.20, 95%CI 1.05–1.36, p=0.005). Sensitivity analyses revealed no further differences when patients were categorised by: pre-operative testing; use of COVID-19-free pathways; or community SARS-CoV-2 prevalence. The rate of postoperative pulmonary complications increased with periods of isolation longer than 3days, with an OR (95%CI) at 4–7days or ≥8days of 1.25 (1.04–1.48), p=0.015 and 1.31 (1.11–1.55), p=0.001, respectively. Isolation before elective surgery might be associated with a small but clinically important increased risk of postoperative pulmonary complications. Longer periods of isolation showed no reduction in the risk of postoperative pulmonary complications. These findings have significant implications for global provision of elective surgical care. - Abstract
Introduction: With the prolongation of life expectancy, an increasing number of elderly patients are evaluated for pancreatic surgery. However, the influence of increasing age on outcomes after pancreaticoduodenectomy (PD) is still unclear, especially in octogenarians. Aim of this study is to evaluate the perioperative characteristics and outcomes of octogenarians undergoing PD. Methods: Data for 812 patients undergoing PD between 2019 and 2020 in 10 referral centers in Italy were reviewed. Patients aged 80 years or older were matched based on nearest neighbor propensity scores in a 1:1 ratio to patients younger than 80 years. Propensity scores were calculated using 7 perioperative variables including gender, ASA score, neoadjuvant treatment (NAT), biliary stent positioning, type of surgical approach (open, laparoscopic, robot-assisted), associated vascular resections, type of lesion. Perioperative characteristics and short-term postoperative outcomes were compared before and after matching. Results: Overall, 81 (10%) patients had 80 years or more. Before matching, octogenarians had a higher rate of ASA score≥ 3 (n = 35, 43.2% vs. n = 207, 28.3%; p = 0.005) and less frequently underwent NAT (n = 11, 13.6% vs. n = 213, 29.1%; p = 0.003). Matching was successfully performed for 70 octogenarians. After matching, no differences in preoperative and intraoperative characteristics were found. Postoperatively, ICU admission was more frequent in octogenarians (50% vs 30%; p = 0.01). Although in-hospital mortality was higher in octogenarians before matching (7.4% vs 2.9% in the younger cohort; p = 0.03), no difference was noted between the matched cohorts (p = 0.36). Postoperative morbidity was comparable between groups in the whole and selected populations. At the multivariate analysis, chronological age was not recognized as a prognostic factor for cumulative major complications, while ASA ≥3 was the only confirmed influencing feature (OR 2.98; 95%CI: 1.6–6.8; p = 0.009). - Abstract
Few evidences are present on the consequences of coronavirus disease 2019 (COVID-19) pandemic on pancreatic surgery. Aim of this study is to evaluate how COVID-19 influenced the diagnostic and therapeutic pathways of surgical pancreatic diseases. A comparative analysis of surgical volumes and clinical, surgical and perioperative outcomes in ten Italian referral centers was conducted between the first semester 2020 and 2019. One thousand four hundred and twenty-three consecutive patients were included in the analysis: 638 from 2020 and 785 from 2019. Surgical volume in 2020 decreased by 18.7% (p < 0.0001). Benign/precursors diseases (− 43.4%; p < 0.0001) and neuroendocrine tumors (− 33.6%; p = 0.008) were the less treated diseases. No difference was reported in terms of discussed cases at the multidisciplinary tumor board (p = 0.43), mean time between diagnosis and neoadjuvant treatment (p = 0.91), indication to surgery and surgical resection (p = 0.35). Laparoscopic and robot-assisted procedures dropped by 45.4% and 61.9%, respectively, during the lockdown weeks of 2020. No difference was documented for post-operative intensive care unit accesses (p = 0.23) and post-operative mortality (p = 0.06). The surgical volume decrease in 2020 will potentially lead, in the near future, to the diagnosis of a higher rate of advanced stage diseases. However, the reassessment of the Italian Health Service kept guarantying an adequate level of care in tertiary referral centers. Clinicaltrials.gov ID: NCT04380766.

Importance: Data about the optimal timing for the initiation of peptide receptor radionuclide therapy (PRRT) for advanced, well-differentiated enteropancreatic neuroendocrine tumors are lacking. Objective: To evaluate the association of upfront PRRT vs upfront chemotherapy or targeted therapy with progression-free survival (PFS) among patients with advanced enteropancreatic neuroendocrine tumors who experienced disease progression after treatment with somatostatin analogues (SSAs). Design, setting, and participants: This retrospective, multicenter cohort study analyzed the clinical records from 25 Italian oncology centers for patients aged 18 years or older who had unresectable, locally advanced or metastatic, well-differentiated, grades 1 to 3 enteropancreatic neuroendocrine tumors and received either PRRT or chemotherapy or targeted therapy after experiencing disease progression after treatment with SSAs between January 24, 2000, and July 1, 2020. Propensity score matching was done to minimize the selection bias. Exposures: Upfront PRRT or upfront chemotherapy or targeted therapy. Main outcomes and measures: The main outcome was the difference in PFS among patients who received upfront PRRT vs among those who received upfront chemotherapy or targeted therapy. A secondary outcome was the difference in overall survival between these groups. Hazard ratios (HRs) were fitted in a multivariable Cox proportional hazards regression model to adjust for relevant factors associated with PFS and were corrected for interaction with these factors. Results: Of 508 evaluated patients (mean ([SD] age, 55.7 [0.5] years; 278 [54.7%] were male), 329 (64.8%) received upfront PRRT and 179 (35.2%) received upfront chemotherapy or targeted therapy. The matched group included 222 patients (124 [55.9%] male; mean [SD] age, 56.1 [0.8] years), with 111 in each treatment group. An authentic educational process should lead to learn the beauty of living together, to promote the desire to know the other and to weave solidarity plots of Peace; a process in-volving the integrity of the person, supporting integrity (spiritual, corporeal, mental and emotional awareness). Educational concerns call for knowledge and action, question what it is necessary to know and what it is necessary to know how to do to promote and preser-ve integrity in relationships. Peace is an action that expresses full awareness, full deci-sion-making capacity and self-mastery, respecting and considering the Other. As the Pa-nikkarrian reading suggests – recovering its Latin etymological root alter – the other is not the adversary, but the other of two, the other part of oneself, that through which one's identity matures in a relational sense. The existential nature of the human being – as well as of every existing form and force – is constitutively relational and unitary (Schroeder 2002). This awareness is the aim of an educational experimentation adherent to the strong and urgent existential questions, projected towards the full realisation of humanity – a quality implicit in the human being, but which needs to be cultivated in order to be able to express itself, as problematised by the human and pedagogical experience of Korczak (2017a). Observing the relationality of existence and the unity of mind-body-spirit – valued also within the Montessori pedagogical model (Cives 2009) sensitive to the construction of peace and knowledge – the religious dimension is grasped in its being a fundamental expe-rience of interconnection; religiosity is an expression of being in a condition of conjunction – as its etymological root, from the Latin re-ligo (re-join, re-connect) – helps us to consi-der.

The proposal presented here refers to the realization of a professional training workshop structured on corporeal-meditative practices – ongoing since a.y. 2010, at the University of Bologna. It aims to explore the transformative breadth of an education through gesture, in the consideration of the mind-body unity. Method, principles and techniques come from the Far Eastern philosophical-experiential disciplines: Qi Gong and Meditation. The method involves the person in its entirety, contributing significantly to the psycho-physical well-being, facilitating access to internal resources, in motivational and decision-making. The joint exercise of body and mind refines the perceptive field and the quality of attention - such as awareness and meta-awareness - promoting transversal skills of decoding and managing the emotional-affective dimension, which are crucial within life skills, with particular regard to relationship and self-actualization.

This contribution is intended to reflect on the value of silence as a condition for critical, creative, aesthetic and contemplative thinking and feeling, and as a matrix of listening that is capable of welcoming a renewed vision of oneself and the world, which is not subject to habit. The question that prompts my reflection is: “towards what end should we promote the exercise of thought, expression and communication?”. At the root of this question lies the consideration of how it is necessary to educate regarding awareness of the movement of one’s own thought – and not only to focus on its contents – and regarding the awareness of the body of words, so that speech effectively becomes creative gesture.

L’emergenza sanitaria che stiamo attraversando è arrivata come un’onda: ha travolto e scaraventato in una realtà sconosciuta. Ha colpito nella sua globalità la sfera esistenziale di ciascuno, con una violenza tale da restituire con acuta immediatezza la rete di interdipendenza in cui la vita si situa. Nell’esperienza individuale si è colta l’unità interdipendente tra corpo-mente-spirito, e nella più ampia rete collettiva si è sperimentato come sia reale la dimensione di interconnessione umana, naturale, sociale. Alla luce di questa realtà – non nuova ad una certa prospettiva filosofico-scientifica, ma fino ad ora, evidentemente, poco considerata – si intende proporre una riflessione in ordine alla questione della capacità di accettare, attraversare e cavalcare (come opportunità) la condizione di dinamica trasformazione come traccia specifica della condizione umana. Riflettere su come meglio caratterizzare l’impegno di un pensiero e di un’azione pedagogica respons-abile ossia capace di sostenere la complessità e rispondere alla trasformazione delle richieste educative con lettura critica e azione efficace. The health emergency that we are going through has come like a wave: it has swept and thrown us into an unknown reality. It has struck in its totality the existential sphere of each person, with such violence that it has returned with acute immediacy the network of interdependence in which life is situated. In the individual experience we have grasped the interdependent unity between body-mind-spirit, and in the wider collective network we have experienced how real is the dimension of human, natural, social interconnection. In the light of this reality - not new to a certain philosophical-scientific perspective, but evidently little considered so far - we intend to propose a reflection on the question of the ability to accept, cross and ride (as an opportunity) the condition of dynamic transformation as a specific trace of the human condition. This issue aim to reflect on how better characterize the commitment of a responsible pedagogical thought and action, in the view of respnsibilty as an ability of sustaining complexity and responding to the transformation of educational demands with a critical reading and an effective action.

L’ascolto: esercizio di conoscenza, sensibilità e dialogo. Esprimersi è un atto creativo di apertura e comunicazione verso di sé e il mondo. E’ possibile farlo attraverso l’idea, la parola e l’azione che sono le dimensioni costitutive della persona, nella pienezza del suo esserci est-etico (Dewey). Dalla prospettiva estetica il pensare, il sentire e l’agire vengono colti nella loro più profonda sinergia e ancor più la persona stessa viene riconosciuta nella sua inalienabile unità di mente-corpo. L’esercizio pieno e libero del pensiero non può situarsi in esperienze separate dalla valorizzazione dell’universo sensoriale e corporeo, che nutrono il mondo delle idee e che concretamente si adoperano affinché l’idea e il pensiero possano prendere corpo nella relazione con gli altri. La relazione può essere pensata in termini dialogali, in cui sia solida, fine e profonda la capacità di ascoltarsi e ascoltare, dove il pensare sia un’azione in cui maturare lo “spazio” per ragionare, riflettere, creare, trasformare (Panikkar). Est-eticamente concepito il pensiero è movimento e forma che richiede attenzione al suo sorgere, al suo essere creativo e generativo di dialogo fecondo. La proposta è di concedere una riflessione sul valore dell’ascolto come disposizione matrice allo sviluppo del pensare e del sentire, del discernimento e della sensibilità; ma soprattutto decifrare l’ascolto come esercizio della capcità di fare spazio per ac-cogliere: disporsi in silenziosa attenzione e apertura al sorgere della domanda, al sorgere dell’incontro, al sorgere del sentire, dell’immaginare, del pensare come fosse per la prima volta (Krishnamurti). Sotto questa luce, la riflessione porta a riconoscere il silenzio come esperienza sensoriale trasversale in grado di esercitare la capacità dell’ascolto a fondamento dello sviluppo del pensiero critico, creativo, premuroso (Montessori). Tenendo presente il contributo dell’approccio olistico all’educazione (Mancini), quello della P4wC (Kohan) e quello dell’educazione attraverso l’arte (Munari) il contributo intende tessere assieme riflessioni interpretative e ipotesi esperienziali per la valorizzazione dell’ascolto come: conoscenza di sé, riconoscimento dell’altro, regolazione dell’attenzione, incontro con lo stupore, rispetto per la vita, consonanza con la natura, apprezzamento della bellezza.
