- Abstract
This article focuses on the trends and challenges faced by the Portuguese penitentiary system between 2000 and 2017, taking into account the Portuguese Penal Law, penal statistics, national and international reports, and studies regarding the Portuguese prison system. The description of the changes in the Penal Law helps us to understand, on one hand, the changes in penal statistics over time and, on the other hand, the inconsistencies between the law in books and law in practice.

No. of bitstreams: 0 Previous issue date: 2017-12-16 Wellcome Trust AstraZeneca Background Underweight, overweight, and obesity in childhood and adolescence are associated with adverse health consequences throughout the life-course. Our aim was to estimate worldwide trends in mean body-mass index (BMI) and a comprehensive set of BMI categories that cover underweight to obesity in children and adolescents, and to compare trends with those of adults. Methods We pooled 2416 population-based studies with measurements of height and weight on 128.9 million participants aged 5 years and older, including 31.5 million aged 5-19 years. We used a Bayesian hierarchical model to estimate trends from 1975 to 2016 in 200 countries for mean BMI and for prevalence of BMI in the following categories for children and adolescents aged 5-19 years: more than 2 SD below the median of the WHO growth reference for children and adolescents (referred to as moderate and severe underweight hereafter), 2 SD to more than 1 SD below the median (mild underweight), 1 SD below the median to 1 SD above the median (healthy weight), more than 1 SD to 2 SD above the median (overweight but not obese), and more than 2 SD above the median (obesity). Findings Regional change in age-standardised mean BMI in girls from 1975 to 2016 ranged from virtually no change (-0.01 kg/m (2) per decade; 95% credible interval -0.42 to 0.39, posterior probability [PP] of the observed decrease being a true decrease= 0.5098) in eastern Europe to an increase of 1.00 kg/m(2) per decade (0.69-1.35, PP> 0.9999) in central Latin America and an increase of 0.95 kg/m (2) per decade (0.64-1.25, PP> 0.9999) in Polynesia and Micronesia. The range for boys was from a non-significant increase of 0.09 kg/m(2) per decade (-0.33 to 0.49, PP= 0.6926) in eastern Europe to an increase of 0.77 kg/m (2) per decade (0.50-1.06, PP> 0.9999) in Polynesia and Micronesia. Trends in mean BMI have recently flattened in northwestern Europe and the high-income English-speaking and Asia-Pacific regions for both sexes, southwestern Europe for boys, and central and Andean Latin America for girls. By contrast, the rise in BMI has accelerated in east and south Asia for both sexes, and southeast Asia for boys. Global age-standardised prevalence of obesity increased from 0.7% (0.4-1.2) in 1975 to 5.6% (4.8-6.5) in 2016 in girls, and from 0.9% (0.5-1.3) in 1975 to 7.8% (6.7-9.1) in 2016 in boys; the prevalence of moderate and severe underweight decreased from 9.2% (6.0-12.9) in 1975 to 8.4% (6.8-10.1) in 2016 in girls and from 14.8% (10.4-19.5) in 1975 to 12.4% (10.3-14.5) in 2016 in boys. Prevalence of moderate and severe underweight was highest in India, at 22.7% (16.7-29.6) among girls and 30.7% (23.5-38.0) among boys. Prevalence of obesity was more than 30% in girls in Nauru, the Cook Islands, and Palau; and boys in the Cook Islands, Nauru, Palau, Niue, and American Samoa in 2016. Prevalence of obesity was about 20% or more in several countries in Polynesia and Micronesia, the Middle East and north Africa, the Caribbean, and the USA. In 2016, 75 (44-117) million girls and 117 (70-178) million boys worldwide were moderately or severely underweight. In the same year, 50 (24-89) million girls and 74 (39-125) million boys worldwide were obese. Interpretation The rising trends in children's and adolescents' BMI have plateaued in many high-income countries, albeit at high levels, but have accelerated in parts of Asia, with trends no longer correlated with those of adults. Background Raised blood pressure is an important risk factor for cardiovascular diseases and chronic kidney disease. We estimated worldwide trends in mean systolic and mean diastolic blood pressure, and the prevalence of, and number of people with, raised blood pressure, defined as systolic blood pressure of 140 mm Hg or higher or diastolic blood pressure of 90 mm Hg or higher. Methods For this analysis, we pooled national, subnational, or community population-based studies that had measured blood pressure in adults aged 18 years and older. We used a Bayesian hierarchical model to estimate trends from 1975 to 2015 in mean systolic and mean diastolic blood pressure, and the prevalence of raised blood pressure for 200 countries. We calculated the contributions of changes in prevalence versus population growth and ageing to the increase in the number of adults with raised blood pressure. Findings We pooled 1479 studies that had measured the blood pressures of 19.1 million adults. Global age-standardised mean systolic blood pressure in 2015 was 127.0 mm Hg (95% credible interval 125.7-128.3) in men and 122.3 mm Hg (121.0-123.6) in women; age-standardised mean diastolic blood pressure was 78.7 mm Hg (77.9-79.5) for men and 76.7 mm Hg (75.9-77.6) for women. Global age-standardised prevalence of raised blood pressure was 24.1% (21.4-27.1) in men and 20.1% (17.8-22.5) in women in 2015. Mean systolic and mean diastolic blood pressure decreased substantially from 1975 to 2015 in high-income western and Asia Pacific countries, moving these countries from having some of the highest worldwide blood pressure in 1975 to the lowest in 2015. Mean blood pressure also decreased in women in central and eastern Europe, Latin America and the Caribbean, and, more recently, central Asia, Middle East, and north Africa, but the estimated trends in these super-regions had larger uncertainty than in high-income super-regions. By contrast, mean blood pressure might have increased in east and southeast Asia, south Asia, Oceania, and sub-Saharan Africa. In 2015, central and eastern Europe, sub-Saharan Africa, and south Asia had the highest blood pressure levels. Prevalence of raised blood pressure decreased in high-income and some middle-income countries; it remained unchanged elsewhere. The number of adults with raised blood pressure increased from 594 million in 1975 to 1.13 billion in 2015, with the increase largely in low-income and middle-income countries. The global increase in the number of adults with raised blood pressure is a net effect of increase due to population growth and ageing, and decrease due to declining age-specific prevalence. Interpretation During the past four decades, the highest worldwide blood pressure levels have shifted from high-income countries to low-income countries in south Asia and sub-Saharan Africa due to opposite trends, while blood pressure has been persistently high in central and eastern Europe. 9. Better outcomes of peritoneal dialysis in diabetic patients in spite of risk of loss of autonomy for home dialysis

Introduction: Diabetes mellitus is a leading cause of chronic renal failure, challenging therapy strategies. Patients with diabetes may benefit from peritoneal dialysis (PD) but higher technique failure is feared. Our purpose was to critically evaluate clinical outcomes of this modality in diabetics, in order to find quality improvement strategies in these risk patients. Methods: A registry-based study of 432 incident patients, 23% with diabetes, from a university hospital PD program was performed. Traditional methods (Kaplan-Meier, Cox models) and innovative survival analysis, taking competing risks into account, were performed, as well as exploring the trends in cohorts according to the decade of PD start. Results: In spite of the detrimental effect of diabetes in patient survival compared to non-diabetics (77%, 52% vs 86%, 71%, at 2 and 4 years, respectively; p < 0.0001) the hazard ratio (HR) for death decreased in the more contemporary cohort (0.303, 95% confidence interval (CI) 0.150 - 0.614, p < 0.001). It is noteworthy that diabetes was not associated with lower technique survival: 74%, 51% vs 79%, 57%, at 2 and 4 years, respectively (p = not significant (NS)). On multivariate analysis, diabetes was an independent predictor for mortality, but not for technique failure. The hazard ratio (HR) for technique failure also decreased in the more recent cohort (0.566, 95% CI 0.348 - 0.919, p = 0.021). Among reasons for transfer to hemodialysis, proportion of ultrafiltration failure was similar between groups (26% vs 22%, p = NS), but drop-out due to loss of autonomy occurred more in the group with diabetes (23% vs 5%, p = 0.004), mainly due to ischemic stroke. The hospitalization rate was also higher in diabetic patients (1.39 vs 0.84 episodes per patient-year (E/PY), p = 0.004) but the peritonitis rate was not increased (0.53 vs 0.61 E/PY, p = NS). Conclusion: PD was an effective long-term renal replacement therapy in diabetics, without higher rates of technique failure, ultrafiltration failure or peritonitis. Better outcomes were achieved in the contemporary cohort. The menace of autonomy loss due to stroke and higher hospitalization rates enhance the need for assisted PD strategies and better control of comorbidities.

Abstract —To investigate the role of the autonomic nervous system (ANS) in the generation of the circadian blood pressure (BP) variation, the degree of impairment of the ANS was related to the results of ambulatory BP recordings in 212 patients with progressive autonomic failure due to familial amyloid polyneuropathy. On the basis of BP and/or heart rate (HR) responses to the Valsalva maneuver, 60° head-up tilting, deep-breathing tests, and plasma norepinephrine levels, 4 groups of patients were distinguished. In all patients and in 38 age-matched control subjects, ambulatory BP was monitored. Patients of group I (n=40, aged 32±3 y), with no evidence yet of impairment of their ANS, had circadian BP and HR variations indistinguishable from controls. Patients of group II (n=41, aged 34±5 y) had a variable degree of impairment of their parasympathetic ANS, but their sympathetic ANS was still intact. Autonomic dysfunction seems to play a central role in the pathophysiology of neurocardiogenic syncope (NCS) but conflicting data have recently become available. We evaluated autonomic nervous system (ANS) function (heart rate variability (HRV), systolic blood pressure variability (SBPV) and baroreceptor gain (BRG)) and non-invasive haemodynamics (cardiac output and total peripheral resistance) in patients with neurocardiogenic syncope. Retrospectively, we evaluated 12 NCS patients (positive head-up tilt without pharmacological provocation) in the basal state and at initial tilt, 12 non-NCS patients with tilt-negative syncope and 12 aged-matched normal controls. Prospectively, we evaluated 16 NCS patients to analyse the haemodynamics and ANS activity throughout the tilt test (beginning of tilt and before syncope occurs). HRV and SBPV were accessed by fast Fourier transforms (FFT) and spontaneous BRG by temporal sequences, slope and a index. Modelflow was used to quantify the non-invasive haemodynamics. None of the autonomic and haemodynamic parameters at baseline or in the first 10 min of tilt was different among the respective NCS, non-NCS syncope and normal control groups, except for SBP, which was higher at baseline in controls. Abstract —To clarify the role of the sympathetic nervous system in the development of cyclosporine A (CsA)-induced rise in blood pressure (BP), the effects of CsA on 24-hour ambulatory BP (ABP) were studied in patients with familial amyloid polyneuropathy (FAP) who underwent a liver transplantation. On the basis of autonomic function tests, patients with absent or mild-to-moderate sympathetic damage (Group A, n=11, age 29 to 43 years, disease duration 2 to 6 years) and patients with severe sympathetic damage (Group B, n=9, age 27 to 38 years, disease duration 3 to 9 years) were identified. Both groups were followed for 1 year. The daily doses of CsA and the CsA whole blood trough levels between the groups did not differ. - Abstract
BACKGROUND: Diabetes has been defined on the basis of different biomarkers, including fasting plasma glucose (FPG), 2-h plasma glucose in an oral glucose tolerance test (2hOGTT), and HbA1c. We assessed the effect of different diagnostic definitions on both the population prevalence of diabetes and the classification of previously undiagnosed individuals as having diabetes versus not having diabetes in a pooled analysis of data from population-based health examination surveys in different regions. METHODS: We used data from 96 population-based health examination surveys that had measured at least two of the biomarkers used for defining diabetes. Diabetes was defined using HbA1c (HbA1c ≥6·5% or history of diabetes diagnosis or using insulin or oral hypoglycaemic drugs) compared with either FPG only or FPG-or-2hOGTT definitions (FPG ≥7·0 mmol/L or 2hOGTT ≥11·1 mmol/L or history of diabetes or using insulin or oral hypoglycaemic drugs). We calculated diabetes prevalence, taking into account complex survey design and survey sample weights. We compared the prevalences of diabetes using different definitions graphically and by regression analyses. We calculated sensitivity and specificity of diabetes diagnosis based on HbA1c compared with diagnosis based on glucose among previously undiagnosed individuals (ie, excluding those with history of diabetes or using insulin or oral hypoglycaemic drugs). We calculated sensitivity and specificity in each survey, and then pooled results using a random-effects model. We assessed the sources of heterogeneity of sensitivity by meta-regressions for study characteristics selected a priori. FINDINGS: Population prevalence of diabetes based on FPG-or-2hOGTT was correlated with prevalence based on FPG alone (r=0·98), but was higher by 2-6 percentage points at different prevalence levels. Disabling orthostatic hypotension, due to insufficiency of the autonomic nervous system, is a common complication of type I familial amyloidotic polyneuropathy (FAP). We investigated whether oral treatment with L-threo-3,4-dihydroxyphenylserine (L-threo-Dops), a noradrenaline precursor, might be of therapeutical benefit. In twenty untreated FAP patients, aged 33 to 44 years, who, because of severe orthostatic hypotension, were bedridden or constrained to a sitting life, supine and erect blood pressure (BP), plasma noradrenaline and tilting time, defined as the interval (s) between the beginning of a 60 degrees head-up tilt and the occurrence of orthostatic symptoms (dizziness, blurred vision or near syncope) were determined before and at repeated intervals during oral treatment with L-threo-Dops, 100 mg bid, for 6 months. Before treatment supine mean BP was 80 (76-85) mmHg (mean and 95% CI), supine plasma noradrenaline was low, 59 (41-77) pg/ml and tilting time ranged from 38 to 118 s. In response to tilt, mean BP immediately fell by 36 (31-41) mmHg, whereas plasma noradrenaline increased by only 11 (0-21) pg/ml (p = 0.05). After 3 to 5 days of treatment with L-threo-Dops all patients experienced marked improvement of their orthostatic tolerance as reflected by their ability to walk freely around. This effect sustained throughout the six months of treatment. Plasma noradrenaline increased moderately by 37 (11-63) pg/ml (p = 0.02) and supine mean BP increased by 8.6 (5.8-12.4) mmHg (p0.001) during chronic treatment. Supine or nocturnal hypertension did not develop, the fall in mean BP in response to tilt diminished by 12.5 (6.5-17.3) mmHg (p0.001) and tilting time became longer than 600 s in all patients. Because of its efficacy, its sustained duration of action and the lack of side effects, L-threo-Dops is advocated to improve orthostatic tolerance in patients with autonomic insufficiency due to FAP.

16. Improving crunchiness and crispness of fried squid rings through innovative tempura coatings: addition of alcohol and CO2 incubation.

This study aimed to determine the effect of different uncommon tempura formulations (incubated with CO
2 and with added ethanol) on physical, chemical and sensory characteristics of fried coated squids, immediately after frying and also after 48 h of refrigeration storage and subsequent oven reheating. Ethanol addition led to lower levels of moisture and higher of fat in the fried coating, regardless the use of CO2 . There were no difference in instrumental colour parameters among all four battered samples. Ethanol added tempuras showed higher crispness and lower maximum force than their water counterparts in an instrumental texture evaluation. Fried squids coated with ethanol added tempuras were rated as crispier and crunchier, even after 2 days of storage and further reheating. Overall, partial substitution of water by ethanol appears as an interesting strategy to increase crispness in tempura coated fried products, especially when the products are stored and reheated before consumption. Mean hourly parameters obtained from all beats (long series) were compared with those obtained from a sample of 512 beats extracted each hour (short series) in nine presumably normal subjects. For both the short and long series, the spectral components, very low frequency, (VLF), low frequency (LF), and high frequency (HF), and time-domain indices (such as the Ewing statistic [PNN50] and RR standard deviation [SD-RR]), have been estimated. The spectral components LF and HF, estimated from the short and long series, were not significantly different, whereas significant differences were found between VLF, SD--RR, and PNN50. In both the short and long series, a strong correlation was found between LF and SD-RR and between HF and PNN50. The results suggest that, over a period of 24 hours, hourly LF and HF spectral components can be obtained using a single series of 512 beats every hour, with a great advantage over the evaluation of the mean hourly parameters. This method would be particularly useful in the study of circadian heart rate spectral analysis in Holter recordings with multiple artifacts or ectopic beats, and in general, when analysis of the entire 24-hour series is not feasible.

Procedeu-se a uma revisão da literatura sobre os conceitos actuais de tratamento da endometriose associada a infertilidade. A terapêutica médica isolada não parece ser útil no tratamento da infertilidade. A cirurgia laparoscópica permite melhorar a probabilidade de ocorrência de gravidez espontânea ou com auxílio de técnicas de procriação medicamente assistida (PMA). Um estudo prospectivo demonstrou o efeito benéfico do tratamento cirúrgico da endometriose I e II. A cirurgia dos endometriomas do ovário com dimensões superiores a 3 cm não está associada a diminuição da reserva folicular do ovário, permitindo a ocorrência de gravidezes espontâneas, principalmente durante o primeiro ano após a sua realização. Quando se planeia a realização de técnicas de PMA, a utilização de protocolos de GnRH ultra-longos está indicada nos estádios III e IV de endometriose, por aumentar significativamente as taxas de gravidez evolutiva, benefício que não se verifica nos estádios I e II de endometriose.

A laqueação tubária bilateral (LTB) é um dos métodos de contracepção mais solicitados, sendo considerado definitivo. Contudo em cerca 3-5% de todas as mulheres que realizaram LTB vêm posteriormente a desejar uma nova gravidez. O objectivo deste trabalho foi avaliar os resultados obtidos após recanalização tubária realizadas na UMR da MAC e fazer uma breve revisão das condutas seguidas actualmente na abordagem desta situação. Materiais e métodos: Foram avaliadas 15 cirurgias para recanalização tubária por laparotomia no período de Janeiro de 1995 a Março de 2002. A idade média das mulheres à data da recanalização tubária foi de 35.3 anos. Em 3 casos observou-se a presença de 1 ou mais factores de infertilidade associados. Todos os casais foram submetidos a uma avaliação pré-operatória criteriosa. Resultados: Tendo em consideração apenas a 1ª gravidez conseguida verificou-se uma taxa de 62.5% de gravidez intra-uterina (GIU). De acordo com o método de LTB utilizado a probabilidade de ocorrer gravidez mostrou-se diferente sendo maior quando esta foi realizada com anéis de Yoon (1/1) e Pomeroy (6/6) versus nenhum caso de gravidez quando electrocoagulação (0/3). Registaram-se 66.7% GIU nas anastomoses istmo-istmicas vs 50% nas anastomoses istmo-ampolar. A HSG pós-operatória revelou-se um método preditivo da ocorrência de uma gravidez quando dentro dos parâmetros considerados normais. Conclusão: O método de LTB sempre que possível deve ser realizado na zona ístmica e nos casos com a menor destruição possível de trompa. A recanalização tubária e fertilização in vitro (FIV) devem ser considerados procedimentos complementares, sendo a adequada selecção das pacientes crucial para a escolha da melhor abordagem terapêutica.

O Síndrome de Klinefelter é uma das causas mais frequentes de hipogonadismo masculino. As manifestações clínicas do Síndrome de Klinefelter variam consideravelmente de indivíduo para indivíduo. Assim, alguns doentes têm alterações no desenvolvimento dos caracteres sexuais secundários, enquanto que outros não se distinguem de indivíduos saudáveis. Os autores avaliaram a variabilidade de apresentação clínica em 25 doentes com S. de Klinefelter, diagnosticados numa consulta de infertilidade.

As anomalias da cavidade uterina e, nomeadamente os pólipos endometriais, são uma importante causa de subfertilidade. Com o objectivo de determinar um impacto do pólipo endometrial na ocorrência de a gravidez, avaliaram-se retrospectivamente 74 mulheres, da consulta de infertilidade da maternidade Dr. Alfredo da Costa, com aquele diagnóstico e que foram ou não sujeitas a cirurgia correctiva, entre Janeiro de 1997 e Dezembro de 2000. 20 das mulheres apresentavam factores adicionais de infertilidade das quais 14 foram submetidas a técnicas de procriação medicamente assistida (PMA). Considerando as mulheres sem factores adicionais de infertilidade e com factores de infertilidade submetidas a PMA, ocorreram 10 gravidezes entre as 31 mulheres que realizaram polipectomia e 6 entre as 33 que não efectuaram esta cirurgia correctiva, não tendo sida esta diferença estatisticamente significativa. Ocorreram 3 abortos nas mulheres não sujeitas a polipectomia. Não foram detectadas diferenças entre os pólipos com mais ou menos 2cm. Neste estudo parece existir uma relação positiva entre a polipectomia e a ocorrência de gravidez, no entanto para melhor avaliação sugere-se o prolongamento do período e a determinação objectiva do número de gravidezes e o seu desfecho.

Introdução: A Técnica de fertilização in vitro-transferência embrionária (FIV-TE) foi desenvolvida para permitir a gravidez em mulheres com patologia tubária. Inúmeros artigos foram publicados referindo que no grupo de Infertilidade Feminina por Factor Tubário isolado (FTi), o sub-grupo de Hidrossalpinge (HS), apresentava piores resultados nos ciclos FIV-TE. Material e Métodos: Estudo retrospectivo de 99 ciclos FIV-TE efectuados em 88 mulheres com FTi entre Janeiro de 1997 e Dezembro de 1999, na Unidade de Medicina da Reprodução da Maternidade Dr. Alfredo da Costa (MAC). O objectivo foi comparar as diferentes taxas de gravidez entre o subgrupo de Hidrossalpinge (n=28) e o grupo de FTi sem hidrossalpinge (n=71). Resultados: A análise estatística entre o grupo HS e o grupo de Fti sem HS relativamente aos parâmetros do protocolo FIV foi estatisticamente não significativa. Discussão: Um grande número de estudos na literatura confirmam o efeito negativo do HS nos resultados da técnica FIV-TE, relativamente às outras causas de factor tubário, excepto os estudos de Sharara et al (1996) e Bazar et al (1995). Provavelmente as diferentes técnicas para o diagnóstico do HS resultam em diferentes percentagens de diagnóstico da patologia, interferindo nos resultados finais. Conclusão: Na população em estudo na MAC o efeito deletério do HS não demonstrou significado estatístico. No entanto, em valor absoluto a taxa de gravidez é discretamente inferior.

Introdução: Desde há vários anos que a inseminação intra-uterina intra-conjugal (IAC), com ou sem estimulação ovárica, vem sido usada no tratamento da infertilidade. No entanto, o seu uso permanece controverso. Material e métodos: Efectuou-se uma análise retrospectiva (1997-1999) de 114 ciclos de IAC com estimulação ovárica controlada em 66 casais, com o objectivo de determinar a eficácia da IAC e identificar variáveis significativas predictíveis do seu sucesso. Analisou-se o protocolo de estimulação, taxa de gravidez, resultado da gravidez e complicações da terapêutica. Resultados: A taxa de gravidez foi de 10,5% por ciclo e de 18% por casal, sendo a taxa de gravidez múltipla de 25% e a de aborto 0%. Metade de todas as gravidezes resultantes ocorreram no primeiro ciclo de IAC. A análise estatística identificou duas variáveis significativas: número de folículos e duração da infertilidade. Baixas doses de FSH parecem prevenir a gravidez múltipla e o síndrome de hiperestimulação ovárica. Conclusão: Concluímos que uma selecção criteriosa das pacientes associada a estimulação ovárica adequada é fundamental para o sucesso da IAC e que esta técnica constitui um tratamento eficaz para algumas formas de infertilidade.

O verdadeiro contributo das malformações uterinas para o insucesso reprodutivo é difícil de avaliar; Não existem certezas absolutas de que a sua correcção cirúrgica melhore o resultado de uma futura gravidez. Os autores fazem uma revisão teórica sobre anomalias müllerianas, sua embriogénese, classificação, apresentação clínica, diagnóstico e indicação cirúrgica.

Foram avaliadas 233 laparoscopias realizadas na Unidade de Medicina de Reprodução da Maternidade Dr. Alfredo da Costa no período compreendido entre Janeiro de 1996 e Dezembro de 1997. A prevalência de endometriose foi de 36% com uma incidência de 28,5% de Quistos Endometriais. Confirmou-se na laparoscopia o diagnóstico ecográfico de Quisto Endometrial em 95,8% dos casos. Foram analisados os resultados da atitude terapêutica dos 24 Quistos Endometriais submetidos a laparoscopia cirúrgica. A taxa de recidiva foi de 16,6%. Avaliar as crenças acerca dos graus e riscos atribuídos pelos universitários a diferentes práticas sexuais e comparálas com a atribuição feita por especialistas em AIDS, constituíram o objetivo deste estudo. Um questionário composto por 25 itens (Escala de Probabilidade do tipo Likert), referentes à práticas/hábitos sexuais foi aplicado a alunos dos cursos de graduação em Enfermagem e Obstetrícia, Medicina, Psicologia, Farmácia-Bioquímica, que aquiesceram em respondê-lo. Através deanálise fatorial, usando-se o Sistema Varimax de Rotação, 25 itens foram distribuídos em sete fatores, sendo cinco itens excluídos. Dos 20 itens, 5 foram analisados neste trabalho, compondo dois fatores. O Fator X foi constituído pelos itens 1(sexo vaginal com preservativo) e 2(sexo anal com preservativo). No Fator Y foram alocados os itens: 3(relação com pessoa do sexo oposto), 4(relação vaginal sem preservativo) e 5(sexo anal sem preservativo). Em 80% dos 5 itens, observou-se que os estudantes apresentam conhecimento compatível ao preconizado pelos especialistas. Entretanto faz-se necessária a educação continuada a estes alunos, considerando-os enquanto pessoa e futuro profissional prestador de assistência aos indivíduos infectados pelo HIV ou com AIDS. To detect the beliefs concerning the risk levels related to several sexual practices/habits and the HIV infection among undergraduate students and to compare their opinion with the AIDS experts were the objectives of this study. A 25- item questionnaire (Likert Probability Scale) about sexual practices/habits were answered by undergraduate students enrolled in the courses of nursing, medical school, pharmacy-biochemistry, psychology. Based on Fatorial analysis and by using the Varimax Rotation System, 25 itens were distributed in 7 factors, and 5 itens were excluded. From the 20 itens, 5 were reunited in 2 factors and studied in this paper. Factor X reunited the itens 1 (vaginal intercourse with condom) and 2 (anal sex with condom). In Factor Y the itens included were: 3 (heterosexual intercourse), 4 (vaginal intercourse without condom) and 5 (anal intercourse without condom). In 80% of the 5 itens, it was observed that the students have knowledge similar to the experts"s evidences But, continuing education is necessary to these people, as human beings and as future health care givers to the HIV infected/AIDS patients.

A apresentação pélvica ocorre em 3% das gravidezes únicas de termo e está associada ao aumento do risco de complicações maternas e fetais. Os autores fizeram uma avaliação retrospectiva de 118 partos pélvicos por via vaginal. Foram considerados os recém-nascidos com pesos compreendidos entre 2000 e 4000 g, de gestação maior ou igual a 34 semanas. Comparou-se a morbilidade materna e do recém-nascido com apresentação pélvica, em 118 partos vaginais e 244 cesarianas. Constatou-se que a morbilidade neonatal major foi significativamente superior no parto vaginal (5,9%), em relação à cesariana (0,4%). A morbilidade materna foi ligeiramente superior na cesariana (3,6%), em relação ao parto vaginal (1,6%), sem significado estatístico.

The objective of this study was to assess the influence of day-night cycle and sleep-awake period on the circadian pattern of heart rate variability (HRV). Twelve male oil refinery security shift workers, aged 39 +/- 7 years, were studied with 24-hour Holter monitor recordings during morning and night work periods. Hourly HRV parameters in the time and frequency domains were evaluated. For both shifts, all HRV parameters during awake or work periods were found not to be statistically different. In both day and night work shifts, the very low frequency and high-frequency components of HRV and the proportion of differences in successive R-R intervals greater than 50 ms increased during the sleep period, while the low frequency/high frequency ratio decreased. The low-frequency component in absolute units and the SD of the R-R interval did not show any variation in either shifts for the different periods. These results suggest that the circadian pattern of HRV seems to be predominantly related to sleep (supine) and wakefulness (standing) and remains independent of night-day cycle.

Foram avaliados todos os internamentos no Serviço de Ginecologia da MAC de 2 de Janeiro a 19 de Setembro de 1991. O critério de selecção baseou-se no registo de morbilidade intra-operatória e no prolongamento de tempo de internamento. Foram estudados os dados relativos à idade, factores de risco associados, risco anestésico, tipo de intervenção cirúrgica, tempo operatório, tipo de complicações e média de tempo de internamento. Verificou-se uma morbilidade global de 5,6%, tendo-se constatado uma maior incidência de complicações no grupo das histerectomias abdominais (16,8%). Nos casos com morbilidade 72% apresentavam um ou mais factores de risco associados. Ocorreram 5 lesões de órgão (0,73%), 33 complicações pós-operatórias (4,8%) e 7 reintervenções (1%). A média de internamento neste grupo de doentes foi de 11 dias.

Spectral analysis of R-R variability has been done in twenty subjects with Familial Amyloidotic Polyneuropathy in order to define intensity, chronology and discriminative capacity of this method in parasympathetic and sympathetic disturbances. The subjects were subdivided in 3 groups. Group I with far advanced disease, group II in early stages of the disease and group III with asymptomatic carriers. The R-R spectral analysis was calculated after fast Fourier transform in supine and after 60 tilt. Valsalva manoeuvre, deep breathing and standing were performed in groups II and III. The results showed a very severe autonomic disturbance in group I.The parasympathetic system seems to be early affected even in some asymptomatic carriers. Heart rate spectral analysis seems to be more sensitive for detection of autonomic disturbances than other autonomic tests.
