In vivo measurements of patellofemoral joint contact area are scarce. Patellofemoral contact area has been measured in living people under static conditions with the knee held at fixed angles between 0 and 60° of flexion. No previous study to our knowledge has measured patellofemoral contact area in vivo during dynamic activity. The aim of this study was to measure and compare patellofemoral joint contact area in healthy people across a range of daily activities. Mobile biplane X-ray imaging was used to measure 3D tibiofemoral and patellofemoral kinematics in level walking, downhill walking, stair ascent, stair descent, and open-chain (non-weightbearing) knee flexion and knee extension. The kinematic data were combined with magnetic resonance imaging to determine patellofemoral joint contact area at each time point during each activity. The knee flexion angle explained, respectively, 83%, 80%, and 72% of the variability in the total, lateral, and medial patellofemoral contact areas measured across all participants and all activities. Total, lateral, and medial patellofemoral contact areas increased from 0 to 60° of knee flexion and then decreased as the flexion angle increased further, up to ~ 120°. Patellofemoral contact area was less sensitive to the type of activity and hence joint load. The lateral patellofemoral contact area was larger than the medial patellofemoral contact area throughout the range of knee flexion in all activities (p < 0.001). Knowledge of patellofemoral contact area during daily activities like walking improves our understanding of patellofemoral joint biomechanics and will assist in validating computational models of the patellofemoral joint. We investigated the effects of isolated meniscectomy on tibiofemoral skeletal kinematics and cartilage contact arthrokinematics in vivo . We recruited nine patients who had undergone isolated medial or lateral meniscectomy, and used a dynamic stereo-radiography (DSX) system to image the patients’ knee motion during decline walking. A volumetric model-based tracking process determined 3D tibiofemoral kinematics from the recorded DSX images. Cartilage contact arthrokinematics was derived from the intersection between tibial and femoral cartilage models co-registered to the bones. The kinematics and arthrokinematics were analyzed for early stance and loading response phase (30% of a gait cycle), comparing the affected and intact knees. Results showed that four patients with medial meniscectomy had significantly greater contact centroid excursions in the meniscectomized medial compartments while five patients with lateral meniscectomy had significantly greater cartilage contact area and lateral shift of contact centroid path in the meniscectomized lateral compartments, comparing to those of the same compartments in the contralateral intact knees. No consistent difference however was identified in the skeletal kinematics. The current study demonstrated that cartilage-based intra-articular arthrokinematics is more sensitive and insightful than the skeletal kinematics in assessing the meniscectomy effects. Background Numerous studies have reported on the tibiofemoral articular cartilage contact kinematics, however, no data has been reported on the articular cartilage geometry at the contact area. This study investigated the in-vivo tibiofemoral articular cartilage contact biomechanics during a dynamic step-up motion. Methods Ten healthy subjects were imaged using a validated magnetic resonance and dual fluoroscopic imaging technique during a step-up motion. Three-dimensional bone and cartilage models were constructed from the magnetic resonance images. The cartilage contact along the motion path was analyzed, including cartilage contact location and the cartilage surface geometry at the contact area. Findings The cartilage contact excursions were similar in anteroposterior and mediolateral directions in the medial and lateral compartments of the tibia plateau ( P > 0.05). Both medial and lateral compartments were under convex (femur) to convex (tibia) contact in the sagittal plane, and under convex (femur) to concave (tibia) contact in the coronal plane. The medial tibial articular contact radius was larger than the lateral side in the sagittal plane along the motion path ( P < 0.001). Interpretations These data revealed that both the medial and lateral compartments of the knee experienced convex (femur) to convex (tibia) contact in sagittal plane (or anteroposterior direction) during the dynamic step-up motion. These data could provide new insight into the in-vivo cartilage contact biomechanics research, and may provide guidelines for development of anatomical total knee arthroplasties that are aimed to reproduce normal knee joint kinematics. This study was to investigate the in vivo tibiofemoral cartilage contact locations before and after anterior cruciate ligament (ACL) reconstruction at 6 and 36 months. Ten patients with unilateral ACL injury were included. A step-up motion was analyzed using a combined magnetic resonance modeling and dual fluoroscopic imaging techniques. The preoperative (i.e. ACL deficient and healthy contralateral) and postoperative cartilage contact locations at 6 and 36 months were analyzed. Similar patterns of the cartilage contact locations during the step-up motion were found for the preoperative and postoperative knee states as compared to the preoperative healthy contralateral side. At the end of step-up motion, the medial contact locations at postoperative 36 months were more anterior when compared to the preoperative healthy contralateral ( p =0.02) and 6 months postoperative knee states ( p =0.01). The changes of the cartilage contact locations at 36 months after ACL reconstruction compared to the healthy contralateral side were strongly correlated with the changes at 6 months postoperatively. This study showed that the tibiofemoral cartilage contact locations of the knee changes with time after ACL reconstruction, implying an ongoing recovery process within the 36 months after the surgery. There could be an association between the short-term (6 months) and longer-term (36 months) contact kinematics after ACL reconstruction. The purpose of this study was to create a kinematic model of the knee joint with six degrees of freedom (DOF) and evaluate the effect of medial collateral ligament (MCL) and lateral collateral ligament (LCL) rupture on cartilage contact point distribution on the tibia during flexion. We hypothesized that collateral ligament contributions vary over six DOF of knee joint articulation and affect the cartilage contact point distribution during joint articulation. The ligament contributions and distribution of joint cartilage contact points cannot be fully assessed with simplified joint models or invasive experiments. Therefore, we developed a new model in which the tibia and femur centers of mass were determined from their surface geometry, and the displacement of the moving tibia was determined from the displacements of the attached ligaments. Compared to the intact knee, the tibia with the LCL removed had higher medial translation and lower valgus rotation. The tibia with the MCL removed had higher lateral translation and higher valgus rotation than the intact knee. At 0, 30, and 60, the tibia with the LCL removed had more internal rotation than the intact knee. Understanding six DOF knee joint kinematics with integration of ligament contributions and cartilage contact positions is useful for the diagnosis of ligament injuries and the design of articulating surfaces for total arthroplasty. Combining accurate bone kinematics data from biplane radiography with cartilage models from magnetic resonance imaging, it is possible to estimate tibiofemoral cartilage contact area and centroid location. Proper validation of such estimates, however, has not been performed under loading conditions approximating functional tasks, such as gait, squatting, and stair descent. The goal of this study was to perform an in vitro validation to resolve the accuracy of cartilage contact estimations in comparison to a laser scanning gold standard. Results demonstrated acceptable reliability and accuracy for both contact area and centroid location estimates. Root mean square errors in contact area averaged 8.4% and 4.4% of the medial and lateral compartmental areas, respectively. Modified Sorensen-Dice agreement scores of contact regions averaged 0.81 ± 0.07 for medial and 0.83 ± 0.07 for lateral compartments. These validated methods have applications for in vivo assessment of a variety of patient populations and physical activities, and may lead to greater understanding of the relationships between knee cartilage function, effects of joint injury and treatment, and the development of osteoarthritis. To understand the mechanical consequences of knee injury requires a detailed analysis of the effect of that injury on joint contact mechanics during activities of daily living. Three-dimensional (3D) knee joint geometric models have been combined with knee joint kinematics to dynamically estimate the location of joint contact during physiological activities--using a weighted center of proximity (WCoP) method. However, the relationship between the estimated WCoP and the actual location of contact has not been defined. The objective of this study was to assess the relationship between knee joint contact location as estimated using the image-based WCoP method, and a directly measured weighted center of contact (WCoC) method during simulated walking. To achieve this goal, we created knee specific models of six human cadaveric knees from magnetic resonance imaging. All knees were then subjected to physiolo-gical loads on a knee simulator intended to mimic gait. Knee joint motion was captured using a motion capture system. Knee joint contact stresses were synchronously recorded using a thin electronic sensor throughout gait, and used to compute WCoC for the medial and lateral plateaus of each knee. WCoP was calculated by combining knee kinematics with the MRI-based knee specific model. Both metrics were compared throughout gait using linear regression. The anteroposterior (AP) location of WCoP was significantly correlated with that of WCoC on both tibial plateaus in all specimens (p < 0.01, 95% confidence interval of Pearson's coefficient r > 0), but the correlation was not significant in the mediolateral (ML) direction for 4/6 knees (p > 0.05). Our study demonstrates that while the location of joint contact obtained from 3D knee joint contact model, using the WCoP method, is significantly correlated with the location of actual contact stresses in the AP direction, that relationship is less certain in the ML direction. Achieving high flexion is an objective of contemporary total knee arthoplasty; however little is known on the knee biomechanics at high flexion under weight-bearing conditions. This study investigates the 6DOF kinematics and tibiofemoral cartilage contact biomechanics of the knee during weight-bearing flexion from full extension to maximal flexion. Eight knees from seven healthy subjects with no history of injuries or chronic pain were recruited. The knees were MRI scanned to create 3D models of the tibia and femur, including their articular cartilage surfaces. The subjects were then imaged using a dual fluoroscopic image system while performing a weight-bearing quasi-static single-legged lunge from full extension to maximal flexion. The 6DOF kinematics and the articular cartilage contact locations were measured along the flexion path of the knee. The result indicated that the internal tibial rotation increased sharply at low flexion angles (full extension to 30°), maintained a small variation in the middle range of flexion (30-120°, and then sharply increased again at high flexion angles (120° to maximal flexion). The contact point moved similarly in the medial and lateral compartments before 120° of flexion, but less on the medial compartment at high flexion angles. The results indicated that the knee motion could not be described using one character in the entire range of flexion, especially in high flexion. The knee kinematic data in the entire range of flexion of the knee could be instrumental for designing new knee prostheses to achieve physical high flexion and improving rehabilitation protocols after knee injuries. Summary: Objective: The knowledge of in-vivo cartilage contact biomechanics is important to the understanding of the pathogenesis of joint diseases such as osteoarthritis. This study investigated the in-vivo contact areas of human talocrural joint under weightbearing conditions that simulated the stance phase of walking using a combined magnetic resonance (MR) and dual-orthogonal fluoroscopic imaging technique. Design: Nine healthy ankles of living subjects were recruited for this study. The in-vivo talocrural joint positions were recorded using the dual-orthogonal fluoroscopic images at three ankle positions that simulated those occurring during the stance phase of walking: heel strike, mid-stance, and toe off. Three-dimensional (3D) models of the talocrural joints were created from MR images and used to reproduce the in-vivo ankle positions recorded on the fluoroscopic images. The talocrural cartilage contact area was defined as the overlap area of the distal tibial and the proximal talar cartilage surfaces. The method was validated using an in-vitro experimental setup to evaluate its accuracy in determination of cartilage contact area. Results: The validation study demonstrated that the articular cartilage contact area of the talocrural joint determined using the imaging technique was approximately 4% lower than that of the experimental measurement. In the nine living ankles, the average cartilage coverage area was 964.9±156.1mm2 on the distal tibia and 1304.8±208.4mm2 on the proximal talus. The average talocrural cartilage contact areas were 272.7±61.1mm2 at heel strike, 416.8±51.7mm2 at mid-stance, and 335.7±64.5mm2 at toe off. The contact area at mid-stance was significantly larger than those at heel strike and toe off, while the contact area at toe off was significantly larger than that at heel strike. Conclusion: The combined dual fluoroscopic and MR imaging technique was shown to be capable of determining in-vivo talocrural cartilage contact areas. During the simulated stance phase of walking, the contact areas were less than 44% and 31% of the cartilage coverage areas of the distal tibia and the proximal talus, respectively. Abstract: Objective. The objective of this study was to investigate whether regions of cartilage in the tibiofemoral joint where cartilage-to-cartilage contact occurred was thicker than other regions. Design. In vivo human subjects. Background. The thickness of the cartilage in the knee has been investigated in various studies. However, the factors that influence the thickness distribution within the joint remain unclear. Methods. Six healthy living knees (5 male, 1 female, average age=27) were scanned using magnetic resonance imaging. Three-dimensional models of the tibial and femoral cartilage layers were created. The cartilage thickness distribution was compared between regions where cartilage-to-cartilage contact was observed during in vivo weightbearing flexion and regions with no contact. Results. The regions with cartilage-to-cartilage contact were significantly thicker than the regions without cartilage-to-cartilage contact (P &#60;0.05). On the medial condyle, the cartilage-to-cartilage contact regions were up to 40% thicker than regions with no contact. On the lateral femoral condyle, the maximum difference between these regions was 20%. On the tibial plateau, the maximal differences between regions with and without cartilage-to-cartilage contact were found to be 40% on the medial side and 50% on the lateral side. Conclusions. The data suggested that in normal knees, the cartilage was thicker in regions where cartilage-to-cartilage contact was present. Future studies should investigate the effects of in vivo loading on cartilage maintenance and growth. This paper quantified the motion of the tibiofemoral contact points during in vivo weight bearing flexion using MRI- based 3D knee models and two orthogonal fluoroscopic images. The contact points on the medial and lateral tibial plateau were calculated by finding the centroid of the intersection of the tibial and femoral cartilage layers and by using the bony geometry alone. Our results indicate that the medial femoral condyle remains in the central portion of the tibial plateau and the lateral condyle translates posteriorly with increasing flexion. Using the bony contact model increased the total translation of the medial and lateral condyles by 250 and 55%, respectively, compared to the cartilage contact model. These results suggest that using the bony geometry alone may not accurately represent the articular surfaces of the knee. Articular cartilage geometry may have to be used to accurately quantify tibiofemoral contact. [Copyright &y& Elsevier]
