Abstract

BackgroundKnowledge of the prevalence of morbidity secondary to stroke is important for health care professionals, health care commissioners, third sector organizations, and stroke survivors to understand the likely progress of poststroke sequelae and to aid in commissioning decisions, planning care, and adjusting to life after stroke. ObjectiveThe primary aim of the Morbidity PRevalence Estimate In StrokE (MORe PREcISE) study is to determine the prevalence of morbidity secondary to a stroke, predictors of morbidity, and trends in quality of life and functional status using patient-reported outcomes, cognitive and functional assessments. MethodsA total of 500 participants will be recruited across Wales and England within 14 days following an admission to a stroke unit for either an ischemic or hemorrhagic stroke as part of a multicenter cohort study. Participants are assessed at baseline ≤14 days poststroke and subsequently at 90 (± 14) days and 180 (± 14) days poststroke. At each time point, data will be collected relating to the following domains: participant demographics, routine clinical, patient reported, cognitive status, emotional well-being, and functional ability. ResultsRecruitment commenced in October 2018 with 20 sites opened as of September 2019 and was closed on October 31, 2019. ConclusionsThe primary outcome is the prevalence of morbidity at 6 months secondary to a stroke. Further analysis will consider temporal changes in the health-related domains to describe trends among baseline, 3-, and 6-month time points. 30. Constructs for the detoxification of organophosphorous compounds

Abstract

Organophosphorous compounds (OPs) form the basis of many biocides and nerve agents, affecting millions of people each year. Current treatments for acute OP poisoning use atropine to prevent neurotransmitter overaccumulation, and oximes for rescuing phosphorylated acetylcholinesterase (AChE) prior to an irreversible ageing event, but medical care may be required for weeks. Hydrolysis of the OP phosphoester bond prevents the inhibitive phosphorylation of AchE, thus enzymes are an obvious candidate for novel treatments. Enzyme-based therapies for OP poisoning have been shown to delay mortality, but leaching of lipophilic OPs from fat deposits leads to prolonged toxicity, superseding the detoxifying effects of the enzyme due to systemic clearance. Furthermore, these same enzymes may be used for the bioremediation of waterways affected by OP pesticide runoff from crops. Here, an artificial membrane-binding protein from the OP-degrading enzyme OpdA is produced that spontaneously partitions into membranes to enhance enzyme-based therapies for OP poisoning. OpdA was expressed in Escherichia coli with high yields, and cationised with high efficiency to homogenise the surface charge for subsequent anionic surfactant engraftment, giving a supercationic enzyme, cOpdA, which maintained structure and activity, but rapidly penetrated cells, resulting in cytotoxicity. Electrostatic engraftment of an anionic polymer surfactant to the surface of the cOpdA mitigated the cytotoxicity, and the constructs were shown to remain active at the cell surface for 5 days. Crucially, the surfactant conjugation has no deleterious effects on the enzyme, and surprisingly causes a significant increase in its turnover rate and substrate affinity. Small angle X-ray scattering and molecular dynamics simulations show that the surfactant forms a highly-dynamic compact corona surrounding the enzyme. Finally, the high-yield expression system for OpdA by Escherichia coli is used in conjunction with a new bioprinting methodology to produce a 3D bacterial microreactor for OP degradation. The novel cell-laden bioink is printable at high resolutions, is not cytotoxic, and maintains its structure over several weeks. Crucially, the printed structures maintain OP-degrading activity.

Abstract

Aim: To assess the association between problematic smartphone usage and anxiety and depression in adolescents. Methods: A cross‐sectional study in five schools in the UK were included. The primary outcome was moderate anxiety (GAD‐7 ≥10) symptoms and secondary outcomes were moderate depression symptoms (PHQ‐9 ≥10) and insomnia. Problematic smartphone usage was assessed using screentime and the Smartphone Addiction Scale. A multi‐level logistic regression was fitted and adjusted Odds Ratio (aOR) with 95% confidence intervals (95% CI) reported. A mediation analysis was conducted. Results: Of the five included schools, 657 adolescents aged 16–18 years were enrolled. The median age was 17.5 years (17–18 [IQR]) and 508 (77.3%) were female. Of these 188 (28.6%) exhibited moderate anxiety and 226 (34.4%) moderate depression symptoms. Almost two thirds (421, 64.1%) have tried to cut down their smartphone use and 81 (12.5%) wanted help to reduce use. Problematic smartphone use was associated with increased anxiety (aOR = 2.03, 95% CI 1.28–3.23); depression (aOR = 2.96, 95% CI 1.80–4.86); and insomnia (aOR = 1.64, 95% CI 1.08–2.50). Screentime was not associated with anxiety (β = 0.99, 95% CI 0.91–1.08); or depression (β = 0.98, 95% CI 0.89–1.07). Problematic smartphone use had a significant direct, indirect and total effect on both anxiety and depression. 39. Amputation and advance care plans: An observational study exploring decision making and long-term outcomes in a vascular centre.

Abstract

Background: Half of those undergoing major lower limb amputation for peripheral arterial disease die within 1 year. Advance care planning reduces days in hospital and increases the chance of dying in a preferred place. Aim: To investigate the prevalence and content of advance care planning for people having a lower limb amputation due to acute or chronic limb-threatening ischaemia or diabetes. Secondary aims were to explore its association with mortality, and length of hospital stay. Design: A retrospective observational cohort study. The intervention was advance care planning. Setting/participants: Patients admitted to the South West England Major Arterial Centre between 1 January 2019 and 1 January 2021 who received unilateral or bilateral below, above, or through knee amputation due to acute or chronic limb-threatening ischaemia or diabetes. Results: 116 patients were included in the study. 20.7% (n = 24) died within 1 year. 40.5% (n = 47) had an advance care planning discussion of which all included cardiopulmonary resuscitation decisions with few exploring other options. Patients who were more likely to have advance care planning discussions were ≥75 years (aOR = 5.58, 95%CI 1.56–20.0), female (aOR = 3.24, 95%CI 1.21–8.69), and had multimorbidity (Charlson Comorbidity Index ≥5, aOR = 2.97, 95%CI 1.11–7.92). Discussions occurred more often in the emergency pathway and were predominantly initiated by physicians. Advance care planning was associated with increased mortality (aHR = 2.63, 95%CI 1.01, 5.02) and longer hospital stay (aHR = 0.52, 95%CI 0.32–0.83). Conclusions: Despite a high risk of death for all patients in the months following amputation, advance care planning occurred in fewer than half of people and mostly focused on resuscitation. Abstract

Background: Frailty is a clinical state that increases susceptibility to minor stressor events. The risk of frailty is higher in chronic conditions, such as Chronic Obstructive Pulmonary Disease (COPD). Recent studies on COPD have shown that patients living with frailty have an increased risk of mortality. The presence of cardiovascular diseases or conditions are common in COPD and may increase the risk of death. Methods: This protocol describes a European prospective cohort study of community-based people, in a stable condition with diagnosis of COPD (as defined by GOLD guidelines) across hospitals in Italy and UK. Frailty prevalence will be assessed using the Clinical Frailty Scale. At 1- and 2-year follow up, primary outcome will be the impact of frailty on the number of cardiovascular events; secondary outcomes: the influence of frailty on cardiovascular mortality, all-cause mortality, and deaths due to COPD. For the primary outcome a zero-inflated Poisson regression will compare the number of cardiovascular events at 1 year. Secondary outcomes will be analysed using the time to mortality. Discussion: This multicentre study will assess the association between frailty and cardiovascular events and mortality in population with COPD. Data collection is prospective and includes routine clinical data. This research will have important implications for the management of patients with COPD to improve their quality of care, and potentially prognosis.
