Aim: To assess the association between 25OHD levels and renal allograft function and rejection episodes. Background(s): 25OHD deficiency has been associated with increased mortality and morbidity in CKD patients. Vitamin D has known immunomodulatory properties, as such a role in renal allograft outcomes has been proposed. Method(s): 25OHD levels were measured at the time of transplantation in incident RTRs, levels of <50 nmol/L were considered deficient. The eGFR was calculated at days 3,7,14 and months 1 and 3. All rejection episodes were biopsy proven. Result(s): 151 transplants were performed (102 cadaveric, 32 living, 9 ABOi and 8 SPK). The predominant causes of ESKD were GN 29.8%, DM 19.9%, reflux 12.6% and IgA 12.5%. Most (>95%) patients received IL2RA induction with Tacrolimus, Mycophenolate and Prednisolone maintenance immunosuppression. The median 25OHD level was 49 nmol/L, with 25OHD deficiency seen in 50.3% (95% CI, 42.3-58.4). Thirty-four (22.5%) patients had a rejection episode, 47.1% were antibody-mediated and 32.2% cell-mediated rejection. Whilst patients with rejection had a lower mean 25OHD level (49.2 vs. 57.1, p = 0.12), 25OHD deficiency was not associated with rejection OR 1.33 (95% CI, 0.62-2.87, p = 0.46). Those who were 25OHD deficient had better renal function at 1-month (eGFR 56.0 vs. 51.8, p = 0.28) and 3-months (eGFR 60.1 vs 55.6, p = 0.24), however this was not significant. Longitudinal regression models adjusted for time, showed no significant association between 25(OH)D and creatinine (p = 0.11) or eGFR (p = 0.08). Conclusion(s): In this short term study 25(OH)D was not significantly associated with rejection, and the apparent decrease in renal function in those with higher baseline 25(OH)D levels was not significant. Longer follow-up and greater numbers are needed to explore these associations further.
