Abstract

Patients with advanced coronary artery disease (CAD) are poor candidates for coronary revascularization. A coronary artery that is diffusely infltrated with poor distal runoff is typically not amenable to percutaneous [...], Background: With a growing population of patients with advanced coronary artery disease (CAD), many of whom have undergone prior percutaneous coronary interventions, coronary endarterectomy (CE) allows for the extension of revascularization in patients with otherwise limited options. Whether adjunctive CE associated with standard surgery, combined with contemporary antiplatelet therapy, improves outcomes remains largely unknown. Methods: We studied 147 consecutive patients who underwent 154 adjunctive CE procedures for advanced CAD between January 2015 and January 2018. We used computed tomography angiography (CTA) in a subgroup of 32 consecutive patients who underwent CE during coronary artery bypass grafting after June 2016 to assess graft and coronary patency. Results: Patients (mean age 67 [+ or -] SD 10 yr) underwent CE of the right (102 patients), the left anterior descending (LAD, 22 patients) and the circumfex (17 patients) coronary arteries. Seven patients (5%) experienced a procedural myocardial infarction and there were no perioperative deaths. Among the 32 patients who underwent CTA 3 months after surgery, the mean patency of the endarterectomized coronary arteries and bypass grafts was 90% and 88%, respectively. All 6 arterial grafts on the LAD artery were patent. The mean survival rate and the mean rate of freedom from major adverse cardiovascular events was 95% [+ or -] 2% and 95% [+ or -] 6%, respectively. The patency rate was 100 % for patients evaluated at 3-year follow up. Conclusion: Coronary endarterectomy offers a surgical option for patients with diffuse CAD who may be unsuitable for coronary bypass alone. Grafts and endarterectomized coronary artery patency remain adequate and explain the excellent patient survival and the freedom rate from major adverse cardiovascular events. Contexte : Avec une population croissante de patients atteints de coronaropathie avancee (CA), dont plusieurs ont deja subi des interventions coronariennes percutanees, l'endarteriectomie coronarienne (EC) permet d'ameliorer la revascularisation chez les patients pour qui les options sont par ailleurs limitees. On ignore si l'EC d'appoint associee a la chirurgie standard et alliee au traitement antiplaquettaire moderne permet d'ameliorer les resultats. Methodes : Nous avons etudie 147 patients consecutifs qui ont subi 154 EC d'appoint pour une coronaropathie avancee entre janvier 2015 et janvier 2018. Nous avons utilise la coronarographie chez un sous-groupe de 32 patients consecutifs soumis a une EC durant un pontage aortocoronarien apres juin 2016 pour evaluer la permeabilite du greffon et des coronaires. Resultats : Les patients (age moyen 67 ans [+ or -] ecart-type [E-T] 10 ans) ont subi une EC de la coronaire droite (102 patients), de la descendante anterieure gauche (22 patients) et de la circonfexe (17 patients). Sept patients (5 %) ont subi un infarctus du myocarde durant l'intervention et on n'a deplore aucun deces perioperatoire. Parmi les 32 patients qui ont subi une coronarographie 3 mois apres la chirurgie, la permeabilite moyenne des coronaires soumises a l'endarteriectomie et des greffons etait de 90 % et de 88 %, respectivement. Les 6 greffons arteriels de la descendante anterieure etaient permeables. Les taux moyens de survie et de survie sans evenement cardiovasculaire majeur ont ete de 95 % [+ or -] 2 % et de 95 % [+ or -] 6 %, respectivement. Le taux de permeabilite etait de 100 % chez les patients evalues au bout d'un suivi de 3 ans. Conclusion : L'endarteriectomie coronarienne offre une option chirurgicale aux patients qui ont une coronaropathie diffuse et qui seraient de mauvais candidats au pontage aortocoronarien seul. La permeabilite des greffons et des coronaires endarteriectomisees reste adequate et explique l'excellente survie des patients et l'absence d'evenements cardiovasculaires majeurs.

Abstract

An increasing number of residents are choosing nonsurgical specialties, a trend explained mainly by residents' desire for more control over their lifestyle. (1-3) The decreased number of applicants in surgical [...], Background: Medical students are increasingly choosing nonsurgical specialties; observership programs can address factors influencing them toward surgical careers by allowing preclerkship exposure and mentorship, and correcting misconceptions. The aims of this study were to assess the influence of a peer-led observership program at the Universite de Montreal on the career choices of preclinical medical students and to determine the factors associated with a positive observership experience. Methods: We used a quasi-experimental convergent mixed-methods questionnaire design. From Nov. 19 to Dec. 31, 2018, and Mar. 1 to Apr. 4, 2019, all medical students participating in the observership program were eligible for the study; there were no ineligibility criteria. Using a prospective purposive sampling method, we recruited students via the email sent to confirm their observership. In the week after their observership, we invited the students by email to complete a postintervention survey. We used nonparametric tests to evaluate the impact of the observership on participants' career choices and an inductive data-driven thematic analysis to analyze their responses. Results: Of the 204 students who participated, 157 provided consent, of whom 85 (54.1%) completed questionnaires both before and after the observership. The majority of participants were interested in a surgical specialty before (72 [85%]) and after (68 [84%]) the observership. There was no significant change in the students' choices of surgical specialties after the observership. However, most (68 [81%]) reported being more interested in a surgical career as a result of the observership, which allowed them to see that the type of practice they considered was congruent with a surgical career. Their perceptions of the field of surgery became positive, particularly regarding its pace and atmosphere and the humanistic patient-doctor relationship it required. The experience was influenced by surgeons' and teams' attitudes toward students, knowledge-sharing and quality of exposure. Participants mentioned that their willingness to participate was in part responsible for the success of their experience. Conclusion: This observership program allowed an early, positive introduction of students to surgery while challenging stereotypes. It provided a better understanding of surgery, enabling participants to consider this field and potentially influencing their residency application. Contexte: De plus en plus d'étudiants en médecine choisissent des spécialités non chirurgicales; les stages d'observation peuvent servir à leur rendre la carrière de chirurgien plus attirante en leur offrant une exposition et un mentorat précliniques et en corrigeant certaines idées préconçues. Cette étude avait pour but d'évaluer l'influence d'un stage d'observation mené par des pairs à l'Université de Montréal sur les choix de carrière chez les étudiants en médecine à l'étape préclinique et déterminer quels facteurs sont associés à une expérience positive lors du stage d'observation. Méthodes : Nous avons utilisé un type de questionnaire à méthodes mixtes convergent quasi-expérimental. Du 19 novembre au 31 décembre 2018 et du 1er mars au 4 avril 2019, tous les étudiants en médecine participant au stage d'observation étaient admissibles à l'étude; il n'y avait aucun critère d'exclusion. A l'aide d'une méthode d'échantillonnage dirigé prospectif, nous avons recruté les étudiants à même le courriel de confirmation de leur stage d'observation. La semaine suivant leur stage d'observation, nous les avons invités par courriel à répondre à un questionnaire. Nous avons utilisé des tests non paramétriques pour mesurer l'impact du stage d'observation sur leurs choix de carrière et une analyse thématique inductive guidée par des données pour analyser leurs réponses. Résultats: Parmi les 204 étudiants qui ont participé, 157 ont fourni leur consentement, dont 85 (54,1%) ont répondu aux questionnaires avant et après le stage d'observation. La majorité des participants étaient intéressés par une spécialité chirurgicale avant (72 [85%]) et après (68 [84%]) le stage d'observation. On n'a noté aucun changement significatif dans les choix de spécialité chirurgicale des étudiants après le stage d'observation. Par contre, la plupart (68 [81%]) se sont dits plus intéressés par une carrière en chirurgie par suite du stage d'observation, qui leur a permis de voir que le type de pratique auquel ils aspiraient s'harmonisait bien avec une carrière en chirurgie. Leurs perceptions du domaine de la chirurgie sont devenues positives, particulièrement au plan du rythme, de l'ambiance et du lien patient-médecin empreint d'humanité qui est requis. Leur expérience a été influencée par l'attitude des chirurgiens et des équipes qui les ont accueillis, par le partage des connaissances et la qualité de leur présence. Les participants ont mentionné que leur volonté de participer était en partie responsable de la réussite de leur expérience. Conclusion : Pour les étudiants, ce stage d'observation a constitué une introduction positive au milieu chirurgical tout en ébranlant leurs préjugés. Il leur a permis de mieux comprendre ce qu'est la chirurgie et d'envisager de faire carrière dans ce domaine, et pourrait avoir influé sur leur choix de résidence.

Abstract

Heart transplantation (HT) remains the treatment of choice for patients with end-stage heart failure refractory to optimal medical therapy. Donor shortages and increasing wait times for transplantation have driven teams [...], Background: The decision about whether to use venoarterial extracorporeal membrane oxygenation (VA-ECMO) in patients with cardiac graft dysfunction (GD) is usually made on a case-by-case basis and is guided by the team's experience. We aimed to determine the incidence of VA-ECMO use after heart transplantation (HT), to assess early- and long-term outcomes and to assess risk factors for the need for VA-ECMO and early mortality in these patients. Methods: We included adults who underwent heart transplantation at 3 cardiac centres who met the most recent International Society for Heart and Lung Transplantation definition of graft dysfunction (GD) over a 10-year period. Pretransplant, intraoperative and posttransplant characteristics of the heart recipients as well as donor characteristics were analyzed and compared among recipients with GD treated with and without VA-ECMO. Results: There were 135 patients with GD in this study, of whom 66 were treated with VA-ECMO and 69 were not. The mean follow-up averaged 81.2 months (standard deviation 36 mo, range 0-184 mo); follow-up was complete in 100% of patients. The overall incidence of GD (30%) and of VA-ECMO use increased over the study period. We did not identify any predictive pretransplantation factors for VA-ECMO use, but patients who required VA-ECMO had higher serum lactate levels and higher inotropes doses after HT. The overall survival rates were 83% and 42% at 1 year and 78% and 40% at 5 years among patients who received only medical treatment and those who received VA-ECMO, respectively. Delayed initiation of VA-ECMO and postoperative bleeding were strongly associated with increased in-hospital mortality. Conclusion: The incidence of GD increased over the study period, and the need for VA-ECMO among patients with GD remains difficult to predict. In-hospital mortality decreased over time but remained high among patients who required VA-ECMO, especially among patients with delayed initiation of VA-ECMO. Contexte : La décision d'utiliser l'oxygénation extracorporelle (ECMO, pour extracorporeal membrane oxygenation) veino-artérielle chez les patients qui présentent une dysfonction du greffon (DG) cardiaque se prend habituellement au cas par cas et est guidée par l'expérience de l'équipe. Nous avons voulu déterminer l'incidence de l'utilisation de l'ECMO après une transplantation cardiaque (TC) afin d'en évaluer les résultats à court et à long terme, d'identifier les facteurs de risque de recours à l'ECMO et de mesurer la mortalité précoce chez ces patients. Méthodes : Nous avons inclus des adultes qui avaient subi une transplantation cardiaque dans 3 centres de cardiologie et qui répondaient à la plus récente définition de dysfonction du greffon (DG) cardiaque de l'International Society for Heart and Lung Transplantation sur une période de 10 ans. Nous avons analysé les caractéristiques pré-, per- et post-transplantation des receveurs et les caractéristiques des donneurs et nous les avons comparées chez les receveurs ayant manifesté une DG, traités ou non par ECMO. Résultats : Nous avons dénombré 135 patients ayant présenté une DG au cours de cette étude; 66 ont été traités par ECMO, contre 69 qui ne l'ont pas été. Le suivi moyen était de 81,2 mois (écart-type 36 mois, plage 0-184 mois); le suivi a été mené à terme chez 100% des patients. L'incidence globale de la DG (30%) et du recours à l'ECMO ont augmenté au cours de la période de l'étude. Nous n'avons pas identifié de facteurs prédictifs pré-transplantation pour ce qui est du recours à l'ECMO, mais les patients qui l'ont utilisée présentaient des taux plus élevés de lactase sérique et avaient reçu des doses d'inotropes plus élevés après la transplantation. Les taux de survie globaux ont été de 83% et de 42% à 1 an et de 78% et 40% à 5 ans chez les patients qui n'ont reçu que le traitement médical et qui ont reçu l'ECMO, respectivement. L'instauration retardée de l'ECMO et les saignements post-opératoires ont été étroitement associés à une mortalité perhospitalière accrue. Conclusion : L'incidence de la DG a augmenté au cours de période de l'étude et le recours à l'ECMO veino-artérielle chez les patients présentant une DG reste difficile à prédire. La mortalité perhospitalière a diminué avec le temps, mais est demeurée élevée chez les patients qui ont eu besoin de l'ECMO, surtout si son instauration avait été retardé.

Abstract

Although the department of surgery of the Universite de Montreal was officially chartered in 1961, it had been in place since 1951, under the leadership of Dr. Jacques Bruneau and [...], The Department of Surgery of the Universite de Montreal was officially chartered in 1961, but the structure had been in place since since 1951. The department grew as a fusion of hospital-based surgery training programs from the largest French-speaking hospitals in Montreal. Currently 448 professors (135 women and 313 men) teach in the department. The research activity, both clinical and applied, is in strong progression. The Department of Surgery is the largest French and bilingual training centre in Canada and North America. In 2021 the department will celebrate its 70th anniversary. As members, we should be proud of the work achieved by our predecessors and by the current rank of professors, teachers and researchers. The department strives to promote the essential role of and highlight the rewards and benefits of academic surgery.
