1. Building Community Capacity for Integrating Engineering in Rural Middle School Science Classrooms

Abstract

Broadening participation in engineering is an important national priority and has led to increasing demands for engineering content to be integrated into traditional K-12 curriculum. However, expecting teachers to incorporate engineering into their classrooms without additional training or resources is unreasonable. Partnering teachers with industry partners is one promising way to prioritize integrated science and engineering content while also introducing youth to possible career paths. In this programmatic article, we introduce the Partnering with Educators and Engineers in Rural Schools (PEERS) project that focuses on the collaborative design, implementation, and study of recurrent hands-on engineering activities with middle school youth in three rural communities in or near Appalachia. We discuss the curricular priorities of the program as well as preliminary findings on both student-focused and capacity-building metrics across the partnerships. Key discussion points include (1) a need to distill goals for engineering outreach by wrestling with what success might really look like for middle-school youth engagement with engineering and (2) cultivating community capacity to better support education systems and the simultaneous potential for and challenges of collaborating to build such infrastructure.

6. North Atlantic Aerosol Single Scattering Albedos: TARFOX and ACE-2 Results and Their Relation to Radiative Effects Derived from Satellite Optical Depths

Abstract

Bergstrom and Russell estimated direct solar radiative flux changes caused by atmospheric aerosols over the mid-latitude North Atlantic Ocean under cloud-free and cloudy conditions. They excluded African dust aerosols, primarily by restricting calculations to latitudes 25-60 N. As inputs they used midvisible aerosol optical depth (AOD) maps derived from AVHRR satellite measurements and aerosol intensive properties determined primarily in the 1996 IGAC Troposheric Aerosol Radiative Forcing Observational Experiment (TARFOX). Those aerosol intensive properties, which included optical depth wavelength dependence and spectra of single scattering albedo (SSA) and scattering asymmetry parameter, were also checked against initial properties from the 1997 North Atlantic Aerosol Characterization Experiment (ACE 2). Bergstrom and Russell investigated the sensitivity of their derived flux changes to assumed input parameters, including midvisible AOD, SSA, and scattering asymmetry parameter. Although the sensitivity of net flux change at the tropopause to SSA was moderate over the ocean (e.g., a SSA uncertainty of 0.07 produced a flux-change uncertainty of 21%), the sensitivity over common land surfaces can be much larger. Also, flux changes within and below the aerosol layer, which affect atmospheric stability, heating rates, and cloud formation and persistence, are quite sensitive to aerosol SSA. Therefore, this paper focuses on the question: "What have we learned from TARFOX and ACE 2 regarding aerosol single scattering albedo?" Three techniques were used in TARFOX to determine midvisible SSA. One of these derived SSA as a best-fit parameter in comparing radiative flux changes measured by airborne pyranometer to those computed from aerosol properties. Another technique combined airborne measurements of aerosol scattering and absorption by nephelometer and absorption photometer. A third technique obtained SSA from best-fit complex refractive indices derived by comparing vertical profiles of lidar backscatter, sunphotometer extinction, and relative size distribution. In ACE 2 midvisible SSA was determined both as a best-fit parameter in comparing measured and calculated flux changes at the surface and by combining nephelometer and absorption photometer measurements. The nephelometer/absorption-photometer results were obtained on the ACE 2 ship (10 m asl), at the Sagres, Portugal site at 50 m asl, and also on the Pelican aircraft. This paper presents and compares the TARFOX and ACE 2 SSA results from the above techniques for different situations (e.g., marine vs continental flows, "clean" vs polluted conditions). It also discusses the strengths and limitations of the techniques, including whether they describe the aerosol in its ambient state or as perturbed by sampling processes; whether they describe the aerosol at the surface, as a function of altitude, or integrated over a column; the ease of acquiring representative data sets; results obtained in tests of consistency with radiative flux changes, and the likelihood of various artifacts and errors.

Abstract

Aerosol single scattering albedo w (the ratio of scattering to extinction) is important in determining aerosol climatic effects, in explaining relationships between calculated and measured radiative fluxes, and in retrieving aerosol optical depths from satellite radiances. Recently, two experiments in the North Atlantic region, TARFOX and ACE-2, determined aerosol w by a variety of techniques. The techniques included fitting of calculated to measured fluxes; retrievals of w from skylight radiances; best fits of complex refractive index to profiles of backscatter, extinction, and size distribution; and in situ measurements of scattering and absorption at the surface and aloft. Both TARFOX and ACE-2 found a fairly wide range of values for w at midvisible wavelengths, with 0.85 less than wmidvis less than 0.99 for the marine aerosol impacted by continental pollution. Frequency distributions of w could usually be approximated by lognormals in wmax-w, with some occurrence of bimodality, suggesting the influence of different aerosol sources or processing. In both TARFOX and ACE-2, closure tests between measured and calculated radiative fluxes yielded best-fit values of wmidvis of 0.90+/-0.04 for the polluted boundary layer. Although these results have the virtue of describing the column aerosol unperturbed by sampling, they are subject to questions about representativeness and possible artifacts (e.g., unknown gas absorption). The other techniques gave larger values for wmidvis for the polluted boundary layer, with a typical result of wmidvis = 0.95+/-0.04, Current uncertainties in vv are large in terms of climate effects. 9. Why surgeons prefer not to care for trauma patients

Regionalized trauma systems have significantly reduced the number of preventable trauma deaths. This type of regionalized care requires a large enough pool of surgeons dedicated to caring for the injured patient. Many factors influence a surgeons' interest in trauma care. Factors negatively influencing physicians attitudes include: interruption of elective surgical practice; lower reimbursement for trauma care; risk of physician liability; and increased cost of malpractice insurance. A survey was undertaken in the state of Washington to document the opinions of surgeons on treatment of trauma patients. There were 754 questionnaires mailed with a response rate of 65 percent; 423 were able to be used for analysis. Thirty-nine percent of those responding said they preferred not to treat trauma patients. These surgeons tended to be older, to practice in cities, and to feel that responding to a call to treat a trauma patient negatively affected their elective practice. They strongly believed reimbursement for treatment of trauma patients was not equal to that of treatment of nontrauma patients. Care of the trauma patient was viewed as requiring more time and carrying an increased medicolegal risk. The results of this study suggest that negative attitudes and resistance to treating trauma patients are based on many factors, particularly physician age and the impact on elective practice. It may be beneficial to recruit younger surgeons interested in trauma care to serve as salaried staff traumatologists in urban and community hospitals. 10. Rapid diagnosis of thoracic aortic transection using intravenous digital subtraction angiography

Abstract

Rupture of the thoracic aorta associated with blunt trauma remains a frequently lethal injury. Although increasing numbers of patients with ruptured aortas are surviving to reach the hospital, the in-hospital mortality attending this injury remains high. Death due to transected aorta has been related to a delay in diagnosis. In an attempt to decrease the time necessary for diagnosis of this injury, we studied 50 patients using intravenous digital subtraction angiography (IVDSA) and conventional biplane angiography. We found that IVDSA was significantly faster than conventional biplane angiography, and that when IVDSA films are of diagnostic quality, they are sufficient to reliably demonstrate the presence of traumatic aortic transection. Our study was too small to establish whether IVDSA is a sufficiently sensitive test to exclude aortic injury. Further studies in this area need to be performed., One frequently lethal result of blunt trauma is rupture of the thoracic aorta, a segment of the aorta, which is the main vessel carrying blood from the heart to the body. This complication is one of the most frequent causes of death at the scene of motor vehicle accidents, and its incidence has increased in recent years. Due to better pre-hospital care, more patients survive to reach the hospital, but hospital mortality remains high. One cause of the high death rate is the length of time required for conventional biplane angiography, a method of viewing the blood vessels under X-ray, to evaluate the thoracic aorta. It is possible that another angiographic approach, intravenous digital subtraction angiography (IVDSA), could enhance the speed of this screening process. This was evaluated by studying 50 patients who underwent IVDSA and conventional biplane angiography. IVDSA required an average of 18 minutes; in five patients, the thoracic aorta was seen to be transected (cut), and the patients were immediately taken to the operating room. The diagnostic phase for these patients required 16 minutes, on average. IVDSA results were either not of acceptable quality, or appeared normal, for the remaining 45 patients. These patients underwent conventional biplane angiography, which required an additional 71 minutes. Two patients with aortic transections were identified in this group. Reasons for the poor-quality IVDSA obtained are discussed. Some researchers have suggested that CT (computerized tomography) or MRI (magnetic resonance imaging) be used for the diagnosis of thoracic aorta rupture. These suggestions are discussed and discarded. IVDSA offers several advantages over other imaging techniques, including IADSA (intra-arterial DSA), because it may be performed so quickly. The approaches taken to improve the method's image quality are outlined. Presently, the main use for IVDSA is as a screening tool to identify, but not to exclude, aortic injury. The Colorado State University (CSU) Accelerator Facility will include a 6-MeV L-Band electron linear accelerator (linac) with a free-electron laser (FEL) system capable of producing Terahertz (THz) radiation, a laser laboratory, a microwave test stand, and a magnetic test stand. The photocathode drive linac will be used in conjunction with a hybrid undulator capable of producing THz radiation. Details of the systems used in CSU Accelerator Facility are discussed

Abstract

This study determined the association between proximal gastrointestinal (GI) colonization and the development of intensive care unit (ICU)-acquired infection and multiple organ failure (MOF) in a population of critically ill surgical patients.ICU-acquired infection in association with progressive organ system dysfunction is an important cause of morbidity and mortality in critical surgical illness. Oropharyngeal and gastric colonization with the characteristic infecting species is common, but its association with ICU morbidity is poorly defined.A prospective cohort study of 41 surgical ICU patients was undertaken. Specimens of gastric and upper small bowel fluid were obtained for quantitative culture; the severity of organ dysfunction was quantitated by a numeric score.One or more episodes of ICU-acquired infection developed in 33 patients and involved at least one organism concomitantly cultured from the upper GI tract in all but 3. The most common organisms causing ICU-acquired infection--Candida, Streptococcus faecalis, Pseudomonas, and coagulase-negative Staphylococci--were also the most common species colonizing the proximal GI tract. Gut colonization correlated with the development of invasive infection within 1 week of culture for Pseudomonas (90% vs. 13% in noncolonized patients, p0.0001) or Staphylococcus epidermidis (80% vs. 6%, p0.0001); a weaker association was seen for colonization with Candida. Infections associated with GI colonization included pneumonia (16 patients), wound infection (12 patients), urinary tract infection (11 patients), recurrent (tertiary) peritonitis (11 patients), and bacteremia (10 patients). ICU mortality was greater for patients colonized with Pseudomonas (70% vs. 26%, p = 0.03); organ dysfunction was most marked in patients colonized with one or more of the following: Candida, Pseudomonas, or S. epidermidis.The upper GI tract is an important reservoir of the organisms causing ICU-acquired infection. Pathologic GI colonization is associated with the development of MOF in the critically ill surgical patient.
