Abstract

This paper presents a comparison of models for extreme rainfall and flood values. Based on a large set of thousands of rainfall and discharge data, the French ExtraFlo project showed that design estimate of extreme values based on the fitting of a distribution on a limited sample of maximum values is very inaccurate when only a few decades of observation are available. Some interesting alternatives are possible, using a regional approach, or historical data when available, or simulation methods, or sedimentological or geochemical signatures of flood deposits.

Exploration and development of natural gas resources includes the use of hydraulic fracturing, a process that can potentially contaminate domestic water sources. While industry seeks access to Bureau of Land Management leases for new wells, their proximity to communities can have serious negative consequences on their health and life style, raising questions of how sites are selected and concerns about environmental justice. This case scenario explores the manner in which the residents of Delta County, Colorado, organized in response to proposed lease sales and how grassroots mobilization uses technology and proactive mechanisms to affect public policy. 20. Detection of Intramolecular Associations in Hydrophobically Modified Pectin Derivatives Using Fluorescent Probes

Abstract

Hydrophobically modified pectin derivatives were prepared by immobilization of long alkyl chains (C12−C18) at various substitution ratios, using two different synthetic pathways, one affording covalent fixation and the other one leading to a mere ionic association. These derivatives display an associative tendency in semidilute as well as in dilute aqueous solutions. This phenomenon, which stems from both intra- and intermolecular interactions between hydrophobic groups, results in the formation of hydrophobic microdomains. The latter can be characterized, especially in the dilute regime, thanks to fluorescence spectroscopy. Fluorescent molecular rotors, as well as pyrene, a classical fluorescence probe of widespread use, witness the variations of the medium polarity. In addition, they can also provide further information, particularly about the local cohesion of the microenvironment of the probe, without performing any complementary experiment, for example, the addition of quenchers together with the fluorescent probe in the polymer solutions. CAC values derived from polarity changes (CACpolarity), using the molecular rotor as well as pyrene as the fluorescence probes, are significantly different from those determined from the cohesion of the microenvironment (CACcohesion), accessible only with the molecular rotor. This latter type of fluorophore may therefore enable us to determine more accurately the actual critical aggregation concentration.

21. Amyotrophic Lateral Sclerosis Presenting with Sleep Hypopnea Syndrome

Abstract

Described is a 67-year-old man whose initial symptoms evoked an obesity-hypoventilation syndrome. Polysomnography showed hypopneas associated with O2desaturation episodes, and no apnea; maximal changes were noted during REM sleep. A few months later, in spite of marked weight loss, acute alveolar hypoventilation occurred and necessitated mechanical ventilatory support. Tracheostomy was performed. The patient appeared to be dependent on nocturnal ventilatory assistance. Diaphragmatic paralysis was noted in addition to clinical and electrodiagnostic evidence of amyotrophic lateral sclerosis. While the patient was not ventilated, a nocturnal recording of SaO2again revealed desaturation episodes partly corrected by O22 L/min administered through the tracheostomy tube. With volume-controlled ventilation, desaturations completely disappeared, although no oxygen enrichment of the air was provided. We speculate that sleep disorders with hypopneas and O2desaturation episodes were the initial symptoms of amyotrophic lateral sclerosis. This leads us to suggest that nonspecific respiratory muscle fatigue frequently seen in COPD might be included in the hypothetic causes of nocturnal hypoxemia.

Abstract

Post-operative cognitive dysfunction (POCD) has been reported after a variety of surgical procedures. POCD is associated with a decline in performance of activities of daily living of elderly patients and can cause substantial damage to family and/or to social support systems. The incidence of POCD in the first week after surgery is 23% in patients between 60 and 69 years of age and 29% in patients older than 70. Cognitive dysfunction was still present in 14% of patients over 70 at three month after surgery. The risk of POCD increases with age, and the type of surgery is also important since there is very low incidence of POCD after minor surgery. For many years, it has been known that general anaesthesia is associated with persistent changes in gene expression in the brain for at least 72 hours. These observed modifications suggest an interesting hypothesis to explain the side effects of anaesthetic agents on cognitive dysfunction, particularly in the elderly. The inflammatory response to surgery is consistent with the hypothesis that inflammation contributes to cognitive decline in the elderly. Most of the drugs administered during anaesthesia interact with the cerebral cholinergic system, which seems to be impaired with ageing. One can hypothesize that this cholinergic dysfunction is a potent factor in the pathogenesis of POCD. These findings have implications for the information provided before obtaining consent from elderly patients prior to surgery; a careful evaluation of mental status is mandatory for all elderly patients undergoing general anaesthesia. Perioperative physicians should be familiar with the prevention, diagnosis, and management of postoperative cognitive dysfunction.
