Abstract

The continuous advance of the automotive industry is leading to the emergence of more advanced driver assistance systems that enable the automation of certain tasks and that are undoubtedly aimed at achieving vehicles in which the driving task can be completely delegated. All these advances will bring changes in the paradigm of the automotive market, as is the case of insurance. For this reason, CESVIMAP and the Universidad Carlos III de Madrid are working on an Autonomous Testing pLatform for insurAnce reSearch (ATLAS) to study this technology and obtain first-hand knowledge about the responsibilities of each of the agents involved in the development of the vehicles of the future. This work gathers part of the advancements made in ATLAS, which have made it possible to have an autonomous vehicle with which to perform tests in real environments and demonstrations bringing the vehicle closer to future users. As a result of this work, and in collaboration with the Johannes Kepler University Linz, the impact, degree of acceptance and confidence of users in autonomous vehicles has been studied once they have taken a trip on board a fully autonomous vehicle such as ATLAS. This study has found that, while most users would be willing to use an autonomous vehicle, the same users are concerned about the use of this type of technology. Thus, understanding the reasons for this concern can help define the future of autonomous cars.

9. Domestic violence screening in a public mental health service: A qualitative examination of mental health clinician responses to DFV.

Abstract

Accessible Summary: What is known on the subject?: Mental health clients experience higher estimated rates of domestic violence, yet mental health services are less likely to screen for domestic violence. What the paper adds to existing knowledge?: This paper qualitatively explores the perspectives and experiences of mental health practitioners in inpatient and community teams in a publicly funded hospital and health service (i.e. public mental health service).Mental health practitioners described a lack of domestic violence training, as well as a lack of knowledge of domestic violence and support mechanisms for victims, when domestic violence is disclosed by clients.The paper highlights the unique difficulties and barriers experienced by clinicians in screening for domestic violence while also dealing with clients suffering a mental health crisis. What are the implications for practice?: The paper sheds more light on the issue of domestic violence in mental health in terms of screening, and identifies avenues for improvement in mental health services; particularly the need for staff training and education. Introduction: Domestic violence is particularly prevalent within mental health client groups, though screening for domestic violence within mental health services is often overlooked. Aim: To investigate the experiences and opinions of domestic violence screening by mental health clinicians in a publicly funded hospital and health service. Methods: Semi‐structured interviews were conducted with twelve clinicians working in publicly funded mental health services in Queensland, Australia. Transcripts were thematically analysed. Results: Four main themes emerged from the data: staff training and experience; prioritizing domestic violence screening; attitudes to domestic violence; and victim support. Participants discussed a lack of training for, or expectations of, domestic violence screening in mental health services. They also highlighted a lack of resources enabling them to appropriately respond when domestic violence was identified. Discussion: Education, attitudes and resources relating to domestic violence are major factors that should be addressed for the successful screening and treatment of clients in mental health. Implications for Practice: This paper informs services of the gaps in knowledge and care around domestic violence and mental health. A less medicalized approach to the treatment of mental health should be adopted, and domestic violence training introduced for all healthcare practitioners, to improve client outcomes. Abstract

Introduction Domestic violence (DV) is a major cause of morbidity worldwide. The ED is a location recommended for opportunistic screening. However, screening within EDs remains irregular. Objective To examine intrinsic and extrinsic barriers to routine screening in Australian EDs, while describing actions taken after identification of DV. Methods Emergency clinicians at nine public hospitals participated in an anonymous online survey. Factor analysis was performed to identify principal components around attitudes and beliefs towards screening. Results In total, 496 emergency clinicians participated. Universal screening was uncommon; less than 2% of respondents reported screening all adults or all women. Although willing, nearly half (45%) reported not knowing how to screen. High patient load and no single rooms were ‘very or severely limiting’ for 88% of respondents, respectively, while 24/7 social work and interpreter services, and online/written DV protocols were top enablers. Factor analysis identified four distinct intrinsic belief components: (1) screening is not futile and could be done in ED, (2) screening will not cause harm, (3) there is a duty to screen and (4) I am willing to screen. Conclusion This study describes a culture of Queensland ED clinicians that believe DV screening in ED is important and interventions are effective. Most ED clinicians are willing to screen. In this setting, availability of social work and interpreter services are important mitigating resources. Clinician education focusing on duty to screen, coupled with a built-in screening tool, and e-links to a local management protocol may improve the uptake of screening and subsequently increase detection.

17. How do children interpret and manage their disturbing obsessive intrusive thoughts?

Abstract

Introduction. Cognitive models of obsessive-compulsive disorder rest on three main assumptions: a) obsessive intrusive thoughts (OITs) are almost universal; b) negative appraisals of OITs lead to emotional distress and interference; and c) unhelpful control strategies contribute to the escalation of OITs into clinical obsessions. These assumptions have received ample empirical support from research with adult samples, while studies in children are promising but scarce. The aim of this study was to describe the emotional distress, interference, negative appraisals and control efforts of those children that report having experienced recent and frequent OITs. Method. 49 children (28 girls, mean age 9.1 years, range 8-10) were recruited from a community sample and individually interviewed using the Children’s Anxious Thoughts Interview. First, an interviewer explored whether the children had ever experienced OITs in six content domains, and the frequency and last date of appearance of the thought were recorded. Then, children were asked to appraise any OITs that they experienced recently (in the last 3 months) and frequently (rated at least 50 in a 100 mm. scale). Children appraised their OITs using a visual analogue scale (100 mm.), in the following domains: 1) emotional reaction (sad, angry, scared, nervous, surprised, and upset); 2) cognitive appraisals (belief the OIT was important, distracting or could come true; and 3) how effective their attempts to manage the OIT were. Strategies used to cope with the thought were explored with an open question and a list of examples from which participants selected the ones they used. Results. From the initial 49 children, 14 children (9 girls, mean age 9.2) reported at least one recent and frequent OIT. The highest-rated emotional reactions were “nervous” (mean = 75.3), “surprised” (68.6) and “scared” (65.4); emotions related to anxiety were also the most frequently mentioned when freely describing how they felt after an OIT. OITs were considered moderately upsetting (64.6), for reasons mostly related to fears of something bad happening (n=7) or their emotional impact (n=5). On average, children regarded the OITs as important (79.6) and distracting (71.4), but were only moderately convinced that it could come true (51.4). The strategies most frequently reported by the children included thought control, either through suppression, distraction or substitution; however, only 3 children rated their attempts at suppression as clearly successful. Compulsive behaviours were reported by only 4 out of 14 children. Discussion. Non-clinical children between 8 and 10 years old described their OITs as upsetting due to their anxiety-provoking qualities. OITs were managed through, mostly unsuccessful, thought control strategies. These results are consistent with the cognitive model of OCD and show that children in this age group can report unwanted intrusive thoughts with obsessive contents if interviewed by a developmentally appropriate instrument such as the CATI. 18. Do children experience obsessional intrusive thoughts?

Abstract

Introduction. Obsessive-compulsive disorder (OCD) is a disabling condition that often begins in childhood. Cognitive behavioural therapy for paediatric OCD is effective and often considered the treatment of choice. While cognitive models of OCD have improved our understanding and interventions in adult populations, it has scarcely been investigated in children. Those models assume that obsessions are extreme variants of unwanted obsessional intrusive thoughts (OITs), which are almost universally experienced in the general populations. This assumption has received empirical support in adult samples but there are almost no studies analysing the presence of OITs in preadolescents. Method. 49 children (28 girls and 21 boys, mean age 9.1 years, range 8-10 years) were recruited from a community sample (two public schools in Spain). Participants were individually interviewed using the newly developed Children’s Anxious Thoughts Interview (CATI). Using the first part of the CATI, an interviewer explored whether participants had ever experienced OITs in six content domains. For each thought reported, an example was requested and subsequently analysed to determine whether it met criteria to be considered an OIT. The frequency (marked in a 100 mm visual analogue scale ranging from hardly ever to every day) and last date that the thought was experienced were also recorded. Participants also completed self-report measures of anxiety (SCAS) and obsessive-compulsive symptoms (ChOCI-R). Results. 33 children endorsed having experienced at least one OIT. The most frequently endorsed content related to harm (n=18) followed by doubt (n=14), superstition (n=4), order (n=2) and aggression (n=1). No contamination OITs were reported. Only 4 children endorsed more than one OIT, with the most frequent combination being harm and doubt. Of 33 children who had experienced at least one OIT, 14 reported having had one in the last 3 months and with a frequency of at least 50 (scale from 0 to 100). Children were divided into three groups according to frequency and recency of OIT experiences: frequent and recent OITs (n=14); OITs that were not frequent or recent (n=19); no OITs. Group scores on SCAS and ChOCI-R were compared using ANOVA. Group differences (p> .05) were found for both instruments. Post-hoc comparisons indicated that those children with frequent and recent OITs experienced significantly higher anxiety (SCAS) and more serious obsessions and compulsions (as shown by the interference scales of the ChOCI-R). Discussion. Our results show that children as young as 8-9 years old are able to report thoughts with the formal characteristics of intrusions and contents characteristics of OCD, although their prevalence may be not as high as in adults. Whilst the occasional recognition of an OIT was not associated with anxiety or OCD symptoms, recent and frequent experiences of OITs was associated with higher anxiety and OCD symptoms. These results are compatible with the cognitive model of OCD, which posits that it is not the experience of OITs themselves that lead to distress but certain pathological appraisals and coping mechanisms. 19. EsTOCma: A Mental Health Application for Enhancing Mental Health Literacy About Obsessive-compulsive Disorder and Reduce Stigma: Protocol for a Randomized Controlled Trial

Abstract

Introduction: Obsessive-compulsive disorder (OCD) is a debilitating heterogeneous condition and one of the 10 leading causes of disability worldwide, affecting approximately 2%-2.3% of the population. However, there is a long delay in seeking treatment, and between 38% and 89.8% of OCD sufferers neither ask for nor receive treatment for their symptoms. Studies on OCD report that insufficient mental health literacy about OCD and the associated stigma would explain the delayed treatment-seeking behavior of OCD patients. With the aim of increasing OCD mental health literacy and reducing the associated stigma, a gamified mental health mobile application (app) called esTOCma was developed. The objective of the study is to describe the randomized control trial that will test the efficacy of the esTOCma app in a Spanish non-clinical sample. The trial is registered in ClinicalTrials.gov with the identifier NCT04777292. Method: A total of 300 participants from the community will be randomly allocated to one of two conditions: undertaking immediate-use or delayed-use (waiting list control group) of the esTOCma mobile app. In this app, participants are asked to fight the OCD stigma monster in ten missions on 8-10 days. They are asked to liberate 10 characters from the OCD stigma monster called EsTOCma. The app is organized in 10 missions with three levels that involve different methodological approaches: psychoeducation, contact, and cognitive restructuring. Furthermore, participants will be asked to complete a set of questionnaires at baseline, at the end of the app, and at 3 and 6 months from baseline: Mental Health Literacy about OCD (MHL-OCD), Attribution Questionnaire (AQ-27), Social Distance Scale (SDS), and General Help-Seeking questionnaire (GHSQ). Results: It is expected that after using the app, participants will have greater knowledge about obsessive-compulsive disorder (MHL-OCD), be more likely to ask for help when having OCD symptoms (GHSQ), and show lower stigma attitudes (AQ-27) and social distance (SDS), and these changes will be maintained at 3- and 6-month follow-ups. Discussion: If esTOCma proves to be effective, widespread use of the app could improve access to mental health services and early help-seeking in people suffering from OCD, which, in turn, could reduce the personal and financial costs associated with OCD. This makes a focus on service engagement and recovery essential on many levels as the social and personal impacts of early psychosis can be considerable and far reaching (McGorry, 2015). Gaining a better understanding of youth early psychosis, the experiences of young people attending early psychosis community services, and the influences that work to keep them engaged is important for clinical and personal recovery, and a global challenge for social work practice. This study used a mixed methods methodology to identify the factors that contributed to consumers’ decision-making processes of whether or not to attend and engage with a community public early psychosis service. In doing so the role of social work in promoting autonomy and self-determination, while addressing the social and demographic factors that often impact on service engagement and recovery was also considered. The two phases in the study were underpinned by self-determination and ecological theory. The first phase involved an analysis of five years of data extracted from the Queensland Health Consumer Integrated Mental Health Application (CIMHA) pertaining to young people referred to the Early Psychosis Service on the Gold Coast. The second phase consisted of two consumer focus groups which aimed to explore themes that had been identified in the first phase as well as to investigate the key findings in more depth. Results suggested that social support, substance use, the therapeutic relationship and the transition period from hospital to community service were all influential in impacting on service engagement decisions. Solving criminal cases can be a complicated task, as there are numerous elements to consider when analyzing a body as its identification, reason for death, time of death, among others; this is where forensic entomology, a specialty in development whose potential cannot be neglected, its effective-ness in recognizing a deceased through the insect’s present providing abundant information about its state of decomposi-tion, death interval, and even evidence some events that led to death. Knowing which insects are found, their stage of evolution through their growth and/or development rates allows professionals to identify post-mortem characteristics. Thanks to this science, it is even possible to provide a timeline of the events, with the help of the post mortem interval during the first 72 hours, in addition there are other elements such as the identification of the types of insects, the amount present in the body, as well as its state of growth and development. There is also the modeling of the crime scene temperatures, these are compared with estimates of the meteorological condition to determine the circumstantial environment of the corpse, until determining if the event actually occurred in that place or the event was in another location and the deceased was moved to where he was found. In general, forensic entomology has great versatility and application. Despite this, in Latin America its boom is not as strong as expected, as few countries have done research on the matter, always ac-companied by the police institute and the educational and/or university entity for theoretical support. On the other hand, there are countries such as Mexico and Guatemala whose scientific advances are still very few and, if any, are found only in universities and research centers.

Abstract

La presente investigación plantea una alternativa de solución al problema actual que viene atravesando las localidades de Bellavista, Limón y El Porvenir, correspondiente al ámbito de cobertura de la EPS EMAPA San Martín S.A. – Unidad Operativa de Bellavista, respecto al sistema de abastecimiento de agua potable, que consiste en un conjunto de propuestas básicas con la finalidad de evaluar, controlar, reducir y recuperar los volúmenes de agua perdidos en todo el sistema de distribución. Mediante el mejoramiento del sistema de abastecimiento, se garantiza una eficiente gestión de la EPS, traduciéndose esto en una mejor estabilidad económica para la empresa y un óptimo servicio para toda la población. Se plantea la implementación de programas de evaluación y reducción de pérdidas de agua en la jurisdicción de la Unidad Operativa de Bellavista; de igual modo esta investigación se desarrolló con el fin de aportar bases que aporten a lograr un mejor manejo del líquido vital en las demás EPS del País. Este trabajo se centra en la reducción de las pérdidas de agua, causado por fugas de toda índole, mala operación del sistema, falta de mantenimiento y otros problemas; por ello se propone un plan de acción para disminuir estas pérdidas a partir de un plan piloto en el distrito de Bellavista, con el fin de evaluarlo y analizar estos resultados para posteriormente aplicarlo en las demás áreas del ámbito de la Unidad Operativa de Bellavista. The following investigation proposes an alternative solution to the current problem that has been going through the localities of Bellavista, Limón and El Porvenir, corresponding to the scope of coverage of the EPS EMAPA San Martín S.A. - Operational unit of Bellavista, regarding the potable water supply system, which consists of a set of basic proposals with the purpose of evaluating, controlling, reducing and recovering the volumes of water lost throughout the distribution system. Through the improvement of the supply system, efficient management of the EPS is guaranteed, translating this into a better economic stability for the company and an optimal service for the entire population. The implementation of programs to evaluate and reduce water losses is proposed in the jurisdiction of the Operational Unit of Bellavista; In the same way, this research was developed with the purpose of providing bases that contribute to achieve a better management of the vital liquid in the other EPS of the Country. This work focuses on the reduction of water losses, caused by leaks of all kinds, poor system operation, lack of maintenance and other problems; therefore, an action plan is proposed to reduce these losses from a pilot plan in the Bellavista district, in order to evaluate and analyze these results and then apply it in the other areas of the Bellavista Operational Unit.

Abstract

A month after Covid- 19 burst into Spanish society and the Spanish market, the issue exploded, both in academic and in commercial circles, as to whether the brutal impact suffered by "normal life " (lockdowns, confinements, almost disappearance of the commercial activity but for the essentials for life) should correspond to the subject matter of the ' rebus sic stantibus " theory, currently existent in Spanish case law but barely applicable in practice for at least a century. The first struggle focused on the rents due by commercial tenants where their undertakings were subject to mandatory lockdown Recently, the issue has extended to the realm of pure money financial debts created by commercial loans, when the debtor objects to the inability of servicing the debt because of the commercial balance shortfall caused by the depressed economy resulting from Covid. Abstract

Royal Decree Act 9/2017, transposing Directive 2014/104, has not only brought about a fundamental amendment to the Competition Act but also incorporates original solutions to the law of damages. This legal analysis is concerned exclusively with matters of joint and several liability among infringers. This legal analysis reveals, both at the European and Spanish level, the enormous difficulty in understanding some rules related to the personal cover of damages, the contribution among coinfringers and the reimbursement after payment by a co-debtor. This situation is more remarkable because the true core of the difficulty- i.e. the limits posed by causation rules-has not been addressed by both legislators. [ABSTRACT FROM AUTHOR]

Abstract

International audience; Background: Ofatumumab, a subcutaneous anti-CD20 monoclonal antibody, selectively depletes B cells. Teriflunomide, an oral inhibitor of pyrimidine synthesis, reduces T-cell and B-cell activation. The relative effects of these two drugs in patients with multiple sclerosis are not known.Methods: In two double-blind, double-dummy, phase 3 trials, we randomly assigned patients with relapsing multiple sclerosis to receive subcutaneous ofatumumab (20 mg every 4 weeks after 20-mg loading doses at days 1, 7, and 14) or oral teriflunomide (14 mg daily) for up to 30 months. The primary end point was the annualized relapse rate. Secondary end points included disability worsening confirmed at 3 months or 6 months, disability improvement confirmed at 6 months, the number of gadolinium-enhancing lesions per T1-weighted magnetic resonance imaging (MRI) scan, the annualized rate of new or enlarging lesions on T2-weighted MRI, serum neurofilament light chain levels at month 3, and change in brain volume.Results: Overall, 946 patients were assigned to receive ofatumumab and 936 to receive teriflunomide; the median follow-up was 1.6 years. Abstract

Introduction: Domestic violence (DV) is a major cause of morbidity worldwide. The ED is a location recommended for opportunistic screening. However, screening within EDs remains irregular., Objective: To examine intrinsic and extrinsic barriers to routine screening in Australian EDs, while describing actions taken after identification of DV., Methods: Emergency clinicians at nine public hospitals participated in an anonymous online survey. Factor analysis was performed to identify principal components around attitudes and beliefs towards screening., Results: In total, 496 emergency clinicians participated. Universal screening was uncommon; less than 2% of respondents reported screening all adults or all women. Although willing, nearly half (45%) reported not knowing how to screen. High patient load and no single rooms were 'very or severely limiting' for 88% of respondents, respectively, while 24/7 social work and interpreter services, and online/written DV protocols were top enablers. Factor analysis identified four distinct intrinsic belief components: (1) screening is not futile and could be done in ED, (2) screening will not cause harm, (3) there is a duty to screen and (4) I am willing to screen., Conclusion: This study describes a culture of Queensland ED clinicians that believe DV screening in ED is important and interventions are effective. Most ED clinicians are willing to screen. In this setting, availability of social work and interpreter services are important mitigating resources.
