Thanks to recent advancements in numerical methods, computer power, and monitoring technology, seismic ambient noise provides precious information about the structural behavior of old buildings. The measurement of the vibrations produced by anthropic and environmental sources and their use for dynamic identification and structural health monitoring of buildings initiated an emerging, cross-disciplinary field engaging seismologists, engineers, mathematicians, and computer scientists. In this work, we employ recent deep learning techniques for time-series forecasting to inspect and detect anomalies in the large dataset recorded during a long-term monitoring campaign conducted on the San Frediano bell tower in Lucca. We frame the problem as an unsupervised anomaly detection task and train a Temporal Fusion Transformer to learn the normal dynamics of the structure. We then detect the anomalies by looking at the differences between the predicted and observed frequencies.

The Social and hUman ceNtered XR (SUN) project is focused on developing eXtended Reality (XR) solutions that integrate the physical and virtual world in a way that is convincing from a human and social perspective. In this paper, we outline the limitations that the SUN project aims to overcome, including the lack of scalable and cost-effective solutions for developing XR applications, limited solutions for mixing the virtual and physical environment, and barriers related to resource limitations of end-user devices. We also propose solutions to these limitations, including using artificial intelligence, computer vision, and sensor analysis to incrementally learn the visual and physical properties of real objects and generate convincing digital twins in the virtual environment. Additionally, the SUN project aims to provide wearable sensors and haptic interfaces to enhance natural interaction with the virtual environment and advanced solutions for user interaction. Finally, we describe three real-life scenarios in which we aim to demonstrate the proposed solutions.

- Abstract
Introduction: To safely expand the donor pool, we introduced a strategy of biopsy-guided selection and allocation to single or dual transplantation of kidneys from donors >60 years old or with hypertension, diabetes, and/or proteinuria (older/marginal donors). Here, we evaluated the long-term performance of this approach in everyday clinical practice. Methods: In this single-center cohort study, we compared outcomes of 98 patients who received one or two biopsy-evaluated grafts from older/marginal donors ("recipients") and 198 patients who received nonhistologically assessed single graft from ideal donors ("reference-recipients") from October 2004 to December 2015 at the Bergamo Transplant Center (Italy). Results: Older/marginal donors and their recipients were 27.9 and 19.3 years older than ideal donors and their reference-recipients, respectively. KDPI/KDRI and donor serum creatinine were higher and cold ischemia time longer in the recipient group. During a median follow-up of 51.9 (interquartile range 23.1-88.6) months, 11.2% of recipients died, 7.1% lost their graft, and 16.3% had biopsy-proven acute rejection (BPAR) versus 3.5, 7.6, and 17.7%, respectively, of reference-recipients. Overall death-censored graft failure (rate ratio 0.78 [95% CI 0.33-2.08]), 5-year death-censored graft survival (94.3% [87.8-100.0] vs. 94.2% [90.5-98.0]), BPAR incidence (rate ratio 0.87 [0.49-1.62]), and yearly measured glomerular filtration rate decline (1.18 ± 3.27 vs. 0.68 ± 2.42 mL/min/1.73 m2, p = 0.37) were similar between recipients and reference-recipients, respectively. Conclusions: Biopsy-guided selection and allocation of kidneys from older/marginal donors can safely increase transplant activity in clinical practice without affecting long-term outcomes. This may help manage the growing gap between organ demand and supply without affecting long-term recipient and graft outcomes.

- Abstract
Aims: Investigating whether and to what extent changes in glomerular hemodynamic parameters, beyond glomerular hyperfiltration, could predict glomerular filtration rate (GFR) decline in hypertensive, non-proteinuric type 2 diabetic patients. Materials and methods: We estimated baseline afferent (Ra) and efferent (Re) arteriolar resistances and glomerular hydrostatic pressure in 60 consecutive patients from DEMAND study, using the Gomez’ equations. Baseline renal plasma flow was measured by para-aminohippurate plasma clearance, and GFR was measured by iohexol plasma clearance at baseline and every 6 months for a median of 4.0 years [IQR 3.5–4.0 years]. Patients with a GFR decline > or ≤ 3 mL/min/1.73 m2/year were categorized as “Progressors” and “Non-progressors,” respectively. Predictors of GFR decline were studied by univariable and multivariable logistic regression analysis. Results: •The GFR declined by a median [IQR] of 4.06 [5.46–2.00] mL/min/1.73 m2/year in the study group as a whole and by 5.35 [6.60–4.48] mL/min/1.73 m2/year and 1.71 [2.14–1.33] mL/min/1.73 m2/year in Progressors and Non-progressors, considered separately. Progressors had a higher baseline Ra (3487.3 ± 1349.3 dyne•sec•cm−5 vs. 2877.0 ± 668.9 dyne•sec•cm−5, p < 0.05) and higher Ra/Re ratio (1.4 ± 0.5 vs. 1.1 ± 0.3, p < 0.01) than Non-progressors. At multivariable logistic regression analysis, Ra/Re ratio and arterial hypertension duration were independently associated with GFR decline (odds ratio [95% CI] 8.50 [1.56–46.28] and 1.14 [1.01–1.28]), respectively. Conclusions: Increased Ra/Re ratio and arterial hypertension duration predict early GFR decline in hypertensive non-proteinuric type 2 diabetic patients. These findings could be explained by glomerular hypoperfusion and chronic ischemic injury related to pre-glomerular arteriolar narrowing. Clinical trial registration: DEMAND, NCT00157586, September 12, 2005.

In this short paper, we report the activities of the Artificial Intelligence for Media and Humanities (AIMH) laboratory of the ISTI-CNR related to Industry. The massive digitalization affecting all the stages of product design, production, and control calls for data-driven algorithms helping in the coordination of humans, machines, and digital resources in Industry 4.0. In this context, we developed AI-based Computer-Vision technologies of general interest in the emergent digital paradigm of the fourth industrial revolution, fo-cusing on anomaly detection and object counting for computer-assisted testing and quality control. Moreover, in the automotive sector, we explore the use of virtual worlds to develop AI systems in otherwise practically unfeasible scenarios, showing an application for accident avoidance in self-driving car AI agents.

Artificial Intelligence (AI) is increasingly employed to develop public services that make life easier for citizens. In this abstract, we present some research topics and applications carried out by the Artificial Intelligence for Media and Humanities (AIMH) laboratory of the ISTI-CNR of Pisa about the study and development of AI-based services for Smart Cities dedicated to the interaction with the physical world through the analysis of images gathered from city cameras. Like no other sensing mechanism, networks of city cameras can 'observe' the world and simultaneously provide visual data to AI systems to extract relevant information and make/suggest decisions helping to solve many real-world problems. Specifically, we discuss some solutions in the context of smart mobility, parking monitoring, infrastructure management, and surveillance systems.

In this short paper, we report the activities of the Artificial Intelligence for Media and Humanities (AIMH) laboratory of the ISTI-CNR related to Trustworthy AI. Artificial Intelligence is becoming more and more pervasive in our society, controlling recommendation systems in social platforms as well as safety-critical systems like autonomous vehicles. In order to be safe and trustworthy, these systems require to be easily interpretable and transparent. On the other hand, it is important to spot fake examples forged by malicious AI generative models to fool humans (through fake news or deep-fakes) or other AI systems (through adversarial examples). This is required to enforce an ethical use of these powerful new technologies. Driven by these concerns, this paper presents three crucial research directions contributing to the study and the development of techniques for reliable, resilient, and explainable deep learning methods. Namely, we report the laboratory activities on the detection of adversarial examples, the use of attentive models as a way towards explainable deep learning, and the detection of deepfakes in social platforms.

As evidenced during the recent COVID-19 pandemic, there are scenarios in which ensuring compliance to a set of guidelines (such as wearing medical masks and keeping a certain physical distance among people) becomes crucial to secure a safe living environment. However, human supervision could not always guarantee this task, especially in crowded scenes. This abstract presents CrowdVisor, an embedded modular Computer Vision-based and AI-assisted system that can carry out several tasks to help monitor individual and collective human safety rules. We strive for a real-time but low-cost system, thus complying with the compute- and storage-limited resources availability typical of off-the-shelves embedded devices, where images are captured and processed directly onboard. Our solution consists of multiple modules relying on well-researched neural network components, each responsible for specific functionalities that the user can easily enable and configure. In particular, by exploiting one of these modules or combining some of them, our framework makes available many capabilities. They range from the ability to estimate the so-called social distance to the estimation of the number of people present in the monitored scene, as well as the possibility to localize and classify Personal Protective Equipment (PPE) worn by people (such as helmets and face masks). To validate our solution, we test all the functionalities that our framework makes available over two novel datasets that we collected and annotated on purpose. Experiments show that our system provides a valuable asset to monitor compliance with safety rules automatically.

Deep Learning ,Visual reasoning ,Transformer networks - Abstract
Although convolutional neural networks (CNNs) showed remarkable results in many vision tasks, they are still strained by simple yet challenging visual reasoning problems. Inspired by the recent success of the Transformer network in computer vision, in this paper, we introduce the Recurrent Vision Transformer (RViT) model. Thanks to the impact of recurrent connections and spatial attention in reasoning tasks, this network achieves competitive results on the same-different visual reasoning problems from the SVRT dataset. The weight-sharing both in spatial and depth dimensions regularizes the model, allowing it to learn using far fewer free parameters, using only 28k training samples. A comprehensive ablation study confirms the importance of a hybrid CNN + Transformer architecture and the role of the feedback connections, which iteratively refine the internal representation until a stable prediction is obtained. In the end, this study can lay the basis for a deeper understanding of the role of attention and recurrent connections for solving visual abstract reasoning tasks. - Abstract
Background Autosomal dominant polycystic kidney disease (ADPKD) is the most frequent genetically determined renal disease. In affected patients, renal function may progressively decline up to end-stage renal disease (ESRD), and approximately 10% of those with ESRD are affected by ADPKD. The somatostatin analog octreotide long-acting release (octreotide-LAR) slows renal function deterioration in patients in early stages of the disease. We evaluated the renoprotective effect of octreotide-LAR in ADPKD patients at high risk of ESRD because of later-stage ADPKD. Methods and findings We did an internally funded, parallel-group, double-blind, placebo-controlled phase III trial to assess octreotide-LAR in adults with ADPKD with glomerular filtration rate (GFR) 15–40 ml/ min/1.73 m2. Participants were randomized to receive 2 intramuscular injections of 20 mg octreotide-LAR (n = 51) or 0.9% sodium chloride solution (placebo; n = 49) every 28 days for 3 years. Central randomization was 1:1 using a computerized list stratified by center and presence or absence of diabetes or proteinuria. Co-primary short- and long-term outcomes were 1-year total kidney volume (TKV) (computed tomography scan) growth and 3-year GFR (iohexol plasma clearance) decline. Analyses were by modified intention-to-treat. Patients were recruited from 4 Italian nephrology units between October 11, 2011, and March 20, 2014, and followed up to April 14, 2017. Baseline characteristics were similar between groups. Compared to placebo, octreotide-LAR reduced median (95% CI) TKV growth from baseline by 96.8 (10.8 to 182.7) ml at 1 year (p = 0.027) and 422.6 (150.3 to 695.0) ml at 3 years (p = 0.002). Reduction in the median (95% CI) rate of GFR decline (0.56 [−0.63 to 1.75] ml/min/1.73 m2 per year) was not significant (p = 0.295). TKV analyses were adjusted for age, sex, and baseline TKV. Over a median (IQR) 36 (24 to 37) months of follow-up, 9 patients on octreotide-LAR and 21 patients on placebo progressed to

Background The efficacy and tolerability of eculizumab were assessed in REGAIN, a 26-week, phase 3, randomized, double-blind, placebo-controlled study in anti-acetylcholine receptor antibody-positive (AChR+) refractory generalized myasthenia gravis (gMG), and its open-label extension. Methods Attainment of ‘minimal symptom expression’ was evaluated using patient-reported outcome measures of gMG symptoms [MG activities of daily living scale (MG-ADL), 15-item MG quality of life questionnaire (MG-QOL15)] at the completion of REGAIN and during the open-label extension. ‘Minimal symptom expression’ was defined as MG-ADL total score of 0–1 or MG-QOL15 total score of 0–3. Results At REGAIN week 26, more eculizumab-treated patients achieved ‘minimal symptom expression’ versus placebo [MG-ADL: 21.4% vs 1.7%; difference 19.8%; 95% confidence interval (CI) 8.5, 31.0; p = 0.0007; MG-QOL15: 16.1% vs 1.7%; difference 14.4%; 95% CI 4.3, 24.6; p = 0.0069]. During the open-label extension, the proportion of patients in the placebo/eculizumab group who achieved ‘minimal symptom expression’ increased after initiating eculizumab treatment and was sustained through 130 weeks of open-label eculizumab (MG-ADL: 1.7 to 27.8%; MG-QOL15: 1.7 to 19.4%). At extension study week 130, similar proportions of patients in the eculizumab/eculizumab and placebo/eculizumab groups achieved ‘minimal symptom expression’ (MG-ADL: 22.9% and 27.8%, respectively, p = 0.7861; MG-QOL15: 14.3% and 19.4%, respectively, p = 0.7531). The long-term tolerability of eculizumab was consistent with previous reports. Conclusions Patients with AChR+ refractory gMG who receive eculizumab can achieve sustained ‘minimal symptom expression’ based on patient-reported outcomes. ‘Minimal symptom expression’ may be a useful tool in measuring therapy effectiveness in gMG. Trial registration ClinicalTrials.gov NCT01997229, NCT02301624.

OBJECTIVE: To assess whether eculizumab, a terminal complement inhibitor, improves patient- and physician-reported outcomes (evaluated using the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale, respectively) in patients with refractory anti-acetylcholine receptor antibody-positive generalized myasthenia gravis across four domains, representing ocular, bulbar, respiratory, and limb/gross motor muscle groups. METHODS: Patients with refractory anti-acetylcholine receptor antibody-positive generalized myasthenia gravis were randomized 1:1 to receive either placebo or eculizumab during the REGAIN study (NCT01997229). Patients who completed REGAIN were eligible to continue into the open-label extension trial (NCT02301624) for up to 4 years. The four domain scores of each of the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale recorded throughout REGAIN and through 130 weeks of the open-label extension were analyzed. RESULTS: Of the 125 patients who participated in REGAIN, 117 enrolled in the open-label extension; 61 had received placebo and 56 had received eculizumab during REGAIN. Patients experienced rapid improvements in total scores and all four domain scores of both the myasthenia gravis activities of daily living profile and the quantitative myasthenia gravis scale with eculizumab treatment. These improvements were sustained through 130 weeks of the open-label extension. INTERPRETATION: Eculizumab treatment elicits rapid and sustained improvements in muscle strength across ocular, bulbar, respiratory, and limb/gross motor muscle groups and in associated daily activities in patients with refractory anti-acetylcholine receptor antibody-positive generalized myasthenia gravis. ispartof: ANNALS OF CLINICAL AND TRANSLATIONAL NEUROLOGY vol:7 issue:8 pages:1327-1339 ispartof: location:United States status: published

Introduction/Aims: Individuals with refractory generalized myasthenia gravis (gMG) who have a history of rituximab use and experience persistent symptoms represent a population with unmet treatment needs. The aim of this analysis was to evaluate the efficacy and safety of eculizumab in patients with refractory anti-acetylcholine receptor antibody-positive (AChR+) gMG previously treated with rituximab. Methods: This post hoc subgroup analysis of the phase 3 REGAIN study (NCT01997229) and its open-label extension (OLE; NCT02301624) compared baseline characteristics, safety, and response to eculizumab in participants who had previously received rituximab with those who had not. Rituximab use was not permitted within the 6 months before screening or during REGAIN/OLE. Results: Of 125 REGAIN participants, 14 had received rituximab previously (7 received placebo and 7 received eculizumab). In the previous-rituximab group, 57% had used at least four other immunosuppressants compared with 16% in the no-previous-rituximab group. Myasthenia Gravis Activities of Daily Living total scores from eculizumab baseline to week 130 of eculizumab treatment improved in both the previous-rituximab and no-previous-rituximab groups (least-squares mean −4.4, standard error of the mean [SEM] 1.0 [n = 9] and least-squares mean −4.6, SEM 0.3 [n = 67], respectively; difference = 0.2, 95% confidence interval −1.88 to 2.22). In addition, in both groups, most patients who were treated with eculizumab for 130 weeks achieved a Myasthenia Gravis Foundation of America post-intervention status of minimal manifestations (66.7% and 65.0%, respectively). The eculizumab safety profile was similar between groups and consistent with its established profile. Discussion: Eculizumab is an effective therapy for patients with refractory AChR+ gMG, irrespective of whether they had received rituximab treatment previously.

Smart mobility applications, such as intelligent parking and road traffic management, are nowadays widely employed worldwide, making our cities more livable, bringing benefits to our lives, reducing costs, and improving energy usage. We propose a multi-camera system to automatically count vehicles in a parking lot using images captured by smart cameras. Unlike most of the literature on this task, which focuses on the analysis of single images, this paper proposes the use of multiple visual sources to monitor a wider parking area from different perspectives. Experiments show that our solution is robust, flexible, and can benefit from redundant information from different cameras while improving overall performance.

Background: Angiotensin converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs) prevent microalbuminuria in normoalbuminuric type 2 diabetic patients. We assessed whether combined therapy with the 2 medications may prevent microalbuminuria better than ACE inhibitor or ARB monotherapy. Methods and findings: VARIETY was a prospective, randomized, open-label, blinded endpoint (PROBE) trial evaluating whether, at similar blood pressure (BP) control, combined therapy with benazepril (10 mg/day) and valsartan (160 mg/day) would prevent microalbuminuria more effectively than benazepril (20 mg/day) or valsartan (320 mg/day) monotherapy in 612 type 2 diabetic patients with high-normal albuminuria included between July 2007 and April 2013 by the Istituto di Ricerche Farmacologiche Mario Negri IRCCS and 8 diabetology or nephrology units in Italy. Time to progression to microalbuminuria was the primary outcome. Analyses were intention to treat. Baseline characteristics were similar among groups. During a median [interquartile range, IQR] follow-up of 66 [42 to 83] months, 53 patients (27.0%) on combination therapy, 57 (28.1%) on benazepril, and 64 (31.8%) on valsartan reached microalbuminuria. Using an accelerated failure time model, the estimated acceleration factors were 1.410 (95% CI: 0.806 to 2.467, P = 0.229) for benazepril compared to combination therapy, 0.799 (95% CI: 0.422 to 1.514, P = 0.492) for benazepril compared to valsartan, and 1.665 (95% CI: 1.007 to 2.746, P = 0.047) for valsartan compared to combination therapy. Between-group differences in estimated acceleration factors were nonsignificant after adjustment for predefined confounders. BP control was similar across groups. All treatments were safe and tolerated well, with a slight excess of hyperkalemia and hypotension in the combination therapy group. The main study limitation was the lower than expected albuminuria at inclusion. Conclusions: Risk/benefit profile of study treatments was si

Purpose To evaluate the effect of eculizumab on perceived fatigue in patients with anti-acetylcholine receptor antibody-positive, refractory, generalized myasthenia gravis (MG) using the Quality of Life in Neurological Disorders (Neuro-QOL) Fatigue subscale, and to evaluate correlations between improvements in Neuro-QOL Fatigue and other clinical endpoints. Methods Neuro-QOL Fatigue, MG Activities of Daily Living (MG-ADL), Quantitative MG (QMG), and the 15-item MG Quality of Life (MG-QOL15) scales were administered during the phase 3, randomized, placebo-controlled REGAIN study (eculizumab, n = 62; placebo, n = 63) and subsequent open-label extension (OLE). Data were analyzed using repeated-measures models. Correlations between changes in Neuro-QOL Fatigue and in MG-ADL, QMG, and MG-QOL15 scores were determined at REGAIN week 26. Results At REGAIN week 26, eculizumab-treated patients showed significantly greater improvements in Neuro-QOL Fatigue scores than placebo-treated patients (consistent with improvements in MG-ADL, QMG, and MG-QOL15 scores previously reported in REGAIN). Improvements with eculizumab were sustained through OLE week 52. Correlations between Neuro-QOL Fatigue and MG-QOL15, MG-ADL, and QMG scores were strong for eculizumab-treated patients at REGAIN week 26, and strong, moderate, and weak, respectively, for placebo-treated patients. Conclusions Compared with placebo, eculizumab was associated with improvements in perceived fatigue that strongly correlated with improvements in MG-specific outcome measures. Trial ID Registration: NCT01997229, NCT02301624.

- Abstract
The ability of marine microbes to navigate toward chemical hotspots can determine their nutrient uptake and has the potential to affect the cycling of elements in the ocean. The link between bacterial navigation and nutrient cycling highlights the need to understand how chemotaxis functions in the context of marine microenvironments. Chemotaxis hinges on the stochastic binding/unbinding of molecules with surface receptors, the transduction of this information through an intracellular signaling cascade, and the activation and control of flagellar motors. The intrinsic randomness of these processes is a central challenge that cells must deal with in order to navigate, particularly under dilute conditions where noise and signal are similar in magnitude. Such conditions are ubiquitous in the ocean, where nutrient concentrations are often extremely low and subject to rapid variation in space (e.g., particulate matter, nutrient plumes) and time (e.g., diffusing sources, fluid mixing). Stochastic, biophysical models of chemotaxis have the potential to illuminate how bacteria cope with noise to efficiently navigate in such environments. At the same time, new technologies for experimentation allow for continuous interrogation—from milliseconds through to days—of bacterial responses in custom dynamic nutrient landscapes, providing unprecedented access to the behavior of chemotactic cells in microenvironments engineered to mimic those cells navigate in the wild. These recent theoretical and experimental developments have created an opportunity to derive population-level uptake from single-cell motility characteristics in ways that could inform the next generation of marine biogeochemical cycling models.

Big data ,Machine learning ,Manufacturing companies ,Image analysis - Abstract
In the era of Big Data, manufacturing companies are overwhelmed by a lot of disorganized information: the large amount of digital content that is increasingly available in the manufacturing process makes the retrieval of accurate information a critical issue. In this context, and thanks also to the Industry 4.0 campaign, the Italian manufacturing industries have made a lot of effort to ameliorate their knowledge management system using the most recent technologies, like big data analysis and machine learning methods. This paper presents the on-going work done within the ADA project, with special emphasis on the specific image analysis work carried out to extract information from images contained in the so different document of the manufacturing companies, partners of the project.

We propose a simple and effective methodology to index and retrieve image features without the need for a time-consuming codebook learning step. We employ a scalar quantization approach combined with Surrogate Text Representation (STR) to perform large-scale image retrieval relying on the latest text search engine technologies. Experiments on large-scale image retrieval benchmarks show that we improve the effectiveness-efficiency trade-off of current STR approaches while performing comparably to state-of-the-art main-memory methods without requiring a codebook learning procedure.

The astonishing and cryptic effectiveness of Deep Neural Networks comes with the critical vulnerability to adversarial inputs - samples maliciously crafted to confuse and hinder machine learning models. Insights into the internal representations learned by deep models can help to explain their decisions and estimate their confidence, which can enable us to trace, characterise, and filter out adversarial attacks.

In this article, we present a scenario for monitoring the occupancy of parking spaces in the historical city of Lucca (Italy) based on the use of intelligent cameras and the most modern technologies of artificial intelligence. The system is designed to use different smart-camera prototypes: where the connection to the power grid is available, we propose a powerful embedded hardware solution that exploits a Deep Neural Network. Otherwise, a fully autonomous energy-harvesting node based on a low-energy custom board employing lightweight image analysis algorithms is considered.

Objectives: The aim of this study was to compare the durabilities of efavirenz (EFV) and rilpivirine (RPV) in combination with tenofovir/emtricitabine (TDF/FTC) in first-line regimens. Methods: A multicentre prospective and observational study was carried out. We included all patients participating in the Italian Cohort Naive Antiretrovirals (ICONA) Foundation Study who started first-line combination antiretroviral therapy (cART) with TDF/FTC in combination with RPV or EFV, with a baseline viral load < 100 000 HIV-1 RNA copies/mL. Survival analyses using Kaplan–Meier (KM) curves and Cox regression with time-fixed covariates at baseline were employed. Results: Overall, 1490 ART-naïve patients were included in the study, of whom 704 were initiating their first cART with EFV and 786 with RPV. Patients treated with EFV, compared with those on RPV, were older [median 36 (interquartile range (IQR) 30–43) years vs. 33 (IQR 27–39) years, respectively; P < 0.001], were more frequently at Centers for Disease Control and Prevention (CDC) stage C (3.1% vs. 1.4%, respectively; P = 0.024), and had a lower median baseline CD4 count [340 (IQR 257–421) cells/μL vs. 447 (IQR 347–580) cells/μL, respectively; P < 0.001] and a higher median viral load [4.38 (IQR 3.92–4.74) log10 copies/mL vs. 4.23 (IQR 3.81–4.59) log10 copies/mL, respectively], (P = 0.004). A total of 343 patients discontinued at least one drug of those included in the first cART regimen, more often EFV (26%) than RPV (13%), by 2 years (P < 0.0001). After adjustment, patients treated with EFV were more likely to discontinue at least one drug for any cause [relative hazard (RH) 4.09; 95% confidence interval (CI) 2.89–5.80], for toxicity (RH 2.23; 95% CI 1.05–4.73) for intolerance (RH 5.17; 95% CI 2.66–10.07) and for proactive switch (RH 10.96; 95% CI 3.17–37.87) than those starting RPV. Conclusions: In our nonrandomized comparison, RPV was better tolerated, less toxic and showed longer durability than EFV, without a significant difference in rates of discontinuation because of failures.
