- Abstract
Objective: This study examines the effect of an ecological and contextually responsive approach, during initial intake call, on engagement for multistressed families seeking child mental health services in an urban setting. Methods: Using a randomized design, parents were allocated to phone Intake As Usual (IAU) or Enhanced Engagement Phone Intake (EEPI). IAU consisted of a lengthy mandated standardized phone intake interview. EEPI, wrapped around the standard intake, was a flexible approach, attuning to the parent's concerns, help-seeking experience, and stressors and incorporating problem-solving and motivational interviewing. Results: Controlling for child gender, child age, income, and marital status, parents in the EEPI condition were 3 times more likely to attend their initial face-to-face agency appointment and subsequent treatment services. Conclusions: This study contributes to building the evidence base for early engagement approaches responsive to families' broader ecological context to ensure that vulnerable children and families access and receive benefits of treatment.

The regulation of care services has become the principal means by which the state influences provision. In this article we examine the regulation of dementia care within organizations to show how some care activities attract more regulation than others. While often perceived to be overwhelming, regulation is in fact unevenly distributed at the system, organization and, in particular, the care practice levels. In practice, some areas of care are heavily regulated, while others are less so. Drawing on research interviews with staff (N = 60) at three levels of care provider organizations—senior managers, facility managers, and direct care workers—a continuum of regulation, with regulations collecting around some care activities and not others, is developed. This is used to plot how different care activities are affected by regulation. Understanding how regulations collect around some activities and not others shows where flexibility and innovation in care are possible. This has implications for the discretion care workers can exercise in daily care, effective organizational strategies for managing care and regulation, and regulatory policy. Current debates around regulation should shift from notions of good/bad and more/less regulation to an analysis of how regulation creates areas that are closely controlled and those that allow practice innovation. Hip fractures are a common type of fragility fracture that afflict 293,000 Americans (over 5,000 per week) and 35,000 Canadians (over 670 per week) annually. Despite the large population impact the optimal fixation technique for low energy femoral neck fractures remains controversial. The primary objective of the FAITH study is to assess the impact of cancellous screw fixation versus sliding hip screws on rates of revision surgery at 24 months in individuals with femoral neck fractures. The secondary objective is to determine the impact on health-related quality of life, functional outcomes, health state utilities, fracture healing, mortality and fracture-related adverse events. FAITH is a multi-centre, multi-national randomized controlled trial utilizing minimization to determine patient allocation. Surgeons in North America, Europe, Australia, and Asia will recruit a total of at least 1,000 patients with low-energy femoral neck fractures. Using central randomization, patients will be allocated to receive surgical treatment with cancellous screws or a sliding hip screw. Patient outcomes will be assessed at one week (baseline), 10 weeks, 6, 12, 18, and 24 months post initial fixation. We will independently adjudicate revision surgery and complications within 24 months of the initial fixation. Outcome analysis will be performed using a Cox proportional hazards model and likelihood ratio test. This study represents major international efforts to definitively resolve the treatment of low-energy femoral neck fractures. This trial will not only change current Orthopaedic practice, but will also set a benchmark for the conduct of future Orthopaedic trials. Actuellement, les camps éducatifs ou les séjours de rupture font partie d'une prise en charge, originale et dynamique, mais souvent controversée, proposée régulièrement par différentes institutions, associations ou éducateurs mobiles à de nombreux adolescents ou jeunes adultes en difficultés. La Fontanelle, les Rives du Rhône, la Brigantine, Pacifique, En Passant par la montagne, proposent des expéditions, à cours ou à long terme, à la voile, à vélo et à pied, dans le désert, en forêt ou en montagne. Notre terrain de travail va s’intéresser autant à l’aspect organisationnel et pratique de cet outil d’accompagnement qu’au vécu de cette population, engagée dans ces séjours, en manque de repères identitaires et sociaux. Nous sommes partis à la recherche de réponses en essayant de comprendre le travail éducatif proposé et d’analyser cet outil dont le rendement qualitatif est difficilement justifiable. Séjour de vacances onéreux proposé à ces jeunes ou aventure hors du commun à la base de nombreux d’apprentissages ? Les résultats ont permis de mettre en lumière la perception de l’échantillon face à cette méthode éducative. Il ressort que les usagers encore inscrits dans le processus perçoivent les camps éducatifs comme favorisant le besoin de confiance, de responsabilité et de dialogue. Les ex-prestataires interrogés, quant à eux, partagent une partie de cet avis seulement, notamment pour le besoin de dialogue. En revanche, ils ont une vision plus transversale pour le besoin de responsabilité et de confiance. Ces derniers sont, pour eux, favorisés durant l’intégralité du processus éducatif institutionnel.

This paper is an attempt to re-position the perceptions and contributions of older people in contemporary Australian society. The world is undergoing a profound demographic shift, with almost every nation state experiencing population ageing on a scale that is unprecedented. This means that we have to search for new models by which to grow old and challenge stereotypes that there is only one acceptable way of doing so. The dominant view of adult ageing has recently changed from one based on dependency to one based on economic contribution. However if we are to successfully move to a new cultural environment – one that accepts age diversity and eschews age prejudice – we must consider a wide range of ways in which older adults can make a contribution. It would not be too much to say that we currently experience a poverty of meaning in our understanding of adult ageing. To address this poverty of meaning this paper draws on a series of social, cultural and historical ideas from which alternative understandings of ageing and later life are identified and discussed. Scholarly articles and texts from a range of disciplines form the basis of analysis, presenting a rich array of options for older adults. The discussion suggests that multiple sources of meaning are available to older adults, many of which counter dominant and acceptable ways of growing old in contemporary society. RÉSUMÉCette étude examine les relations entre le travail et la famille, en mettant l'accent sur les solutions politiques. La politique australienne est l'exemple de cas dans le contexte des tendances internationales. Un désaccord entre les initiatives politiques et les priorités familiales et personnelles représente un nouveau risque social associé à l'évolution démographique et socioculturelle. Les tendances contemporaines, à la fois nationaux et internationaux, montrent des solutions au "problème du vieillissement démographique" en adoptant une forme d'instrumentalisation économique. Cela limite l'identités d'âge légitimes à ceux qui sont associées avec le travail et l'activité liée au travail. Lorsqu'elle est appliqué à la vie de famille, une telle focalisation risque de réduire l'intérêt de la politique en engagement intergénérationnel à des rôles de soins non rémunérés, tandis qu'on ne tient compte du développement personnel et des priorités de la vie liées à l'âge. Le besoin d'une adaptation culturelle au vieillissement démographique est plus acceptée dans le débat politique, et est considéré ici comme une réponse efficace aux risques personnels, sociaux et économiques du vieillissement de la population et les impacts connexes sur la vie familiale.

- Abstract
The relationship between work and family is considered with an emphasis on policy solutions. Australian policy is a case example in the context of international trends. A mismatch between policy initiatives and familial and personal priorities constitutes a new social risk associated with demographic and sociocultural development. Contemporary trends, both nationally and internationally, evidence solutions to the “problem of demographic aging” by adopting a form of economic instrumentalism. This restricts legitimate age identities to those associated with work and work-related activity. When applied to family life, such a focus runs the risk of reducing policy interest in intergenerational engagement to unpaid care roles, while personal development and age-related life priorities are ignored. The need for cultural adaptation to population aging is becoming accepted in policy debate and is considered here as an effective response to the personal, social, and economic risks of population aging and associated impacts on family life.
