Zusammenfassung Hintergrund Als Bundesland mit dem höchsten Altersdurchschnitt in Deutschland und besonderen Strukturmerkmalen ländlich geprägter Gebiete sind die Folgen des demographischen Wandels bereits heute in Sachsen spürbar. Um die medizinische Versorgung von Parkinson-Patienten zu verbessern, bedarf es einer Status-quo-Analyse der aktuellen Versorgungspraxis. Ziel der Arbeit (Fragestellung) Inwieweit unterscheidet sich die Inanspruchnahme der medizinischen Leistungserbringung von Parkinson-Patienten im Vergleich von städtisch und ländlich geprägten Gebieten sowie im Vergleich von Parkinson-Patienten mit und ohne Neurologenkontakt im Beobachtungszeitraum von 2011 bis 2019? Material und Methoden Die Kohortenstudie basiert auf umfangreichen Routinedaten der Krankenkasse AOK PLUS der Jahre 2010 bis 2019 für Sachsen. Untersucht wurde eine Kohorte von insgesamt 15.744 Parkinson-Patienten (n = 67.448 Patientenjahre) und eine gematchte Vergleichskohorte (n = 674.480 Patientenjahre; Kriterien: Geburtsjahr, Geschlecht, Versicherungsjahr, Wohnsitz Stadt/Land) ohne ICD-10-Kodierung einer Bewegungsstörung. Ergebnisse Insgesamt war eine kontinuierliche Zunahme der Anzahl der Erkrankten in der dynamischen Kohorte von 2011 (n = 6829) bis 2019 (n = 8254) zu beobachten. Stadt-Land-Unterschiede zeigten sich insbesondere in der geringeren (Mit‑)Behandlung durch niedergelassene Neurologen in ländlich geprägten Gebieten. Parkinson-Patienten hatten ein 3,5- bzw. 4‑fach erhöhtes Risiko zu versterben im Vergleich zu Versicherten der Vergleichskohorte. Veränderungen der medikamentösen Parkinson-Therapie (Zunahme COMT- und MAO-Inhibitoren) sowie der Heilmittelerbringung (Zunahme Ergotherapie und Logopädie) über die Beobachtungszeit zeigten sich primär bei Parkinson-Patienten mit Neurologenkontakt. Diskussion In der Studie konnten eine erhöhte Morbidität und Mortalität bei Parkinson-Patienten identifiziert werden, die sich als Ziel für innovative Versorgungskonzepte eignen. Die zunehmende Zahl an Patienten und die beschriebenen Unterschiede dokumentieren hierfür den Bedarf. Gleichzeitig zeigen die Veränderungen in der Verordnungspraxis, dass innovative Therapien von niedergelassenen Neurologen eingesetzt werden.

Background Extremolytes enable microbes to withstand even the most extreme conditions in nature. Due to their unique protective properties, the small organic molecules, more and more, become high-value active ingredients for the cosmetics and the pharmaceutical industries. While ectoine, the industrial extremolyte flagship, has been successfully commercialized before, an economically viable route to its highly interesting derivative 5-hydroxyectoine (hydroxyectoine) is not existing. Results Here, we demonstrate high-level hydroxyectoine production, using metabolically engineered strains of C. glutamicum that express a codon-optimized, heterologous ectD gene, encoding for ectoine hydroxylase, to convert supplemented ectoine in the presence of sucrose as growth substrate into the desired derivative. Fourteen out of sixteen codon-optimized ectD variants from phylogenetically diverse bacterial and archaeal donors enabled hydroxyectoine production, showing the strategy to work almost regardless of the origin of the gene. The genes from Pseudomonas stutzeri (PST) and Mycobacterium smegmatis (MSM) worked best and enabled hydroxyectoine production up to 97% yield. Metabolic analyses revealed high enrichment of the ectoines inside the cells, which, inter alia, reduced the synthesis of other compatible solutes, including proline and trehalose. After further optimization, C. glutamicum Ptuf ectDPST achieved a titre of 74 g L−1 hydroxyectoine at 70% selectivity within 12 h, using a simple batch process. In a two-step procedure, hydroxyectoine production from ectoine, previously synthesized fermentatively with C. glutamicum ectABCopt, was successfully achieved without intermediate purification. Conclusions C. glutamicum is a well-known and industrially proven host, allowing the synthesis of commercial products with granted GRAS status, a great benefit for a safe production of hydroxyectoine as active ingredient for cosmetic and pharmaceutical applications. Because ectoine is already available at commercial scale, its use as precursor appears straightforward. In the future, two-step processes might provide hydroxyectoine de novo from sugar.

Purpose Multimorbidity leads to decreasing health-related quality of life (HRQoL). Telemedicine may help to improve HRQoL. The present study was conducted to show (I) differences in HRQoL and changes in HRQoL over time in elderly, multimorbid individuals with and without depression and/or mild cognitive impairment (MCI) using a telemonitoring application (TMA) and (II) associations between engagement with measurements by study participants using a TMA and changes in their HRQoL. Methods The present feasibility study was part of a longitudinal intervention study. Recruited general practitioners (GPs) enrolled individuals and assigned them to risk groups according to absence/presence of depression and/or MCI. Depression was assessed using the Geriatric Depression Scale (GDS-15), MCI using the Mini-Mental State Examination (MMSE), and HRQoL using the SF-12. The TMA consisted of tablets, software, and measuring devices. Measured vital data were transferred to a care and case manager for monitoring and possible intervention. Results Nine GPs recruited 177 individuals, 97 of whom were included in the HRQoL analysis. Significantly lower physical and mental component summary (PCS/MCS) scores were revealed in study participants with depression, and with both depression and MCI, compared to participants with no mental disorders. PCS scores did not differ between study dates, but MCS scores had significantly increased over time. Participants’ engagement with measurements was significantly associated with an increased MCS score, but not with the PCS score. Discussion Depression and/or MCI are negatively associated with the HRQoL of elderly, multimorbid people using a TMA. Engagement of individuals with vital data measurements via a TMA may increase their mental HRQoL. Mentally impaired people should be closely involved as co-designers and experts in development processes of TMAs to benefit from tailored solutions. An individual’s increased mental HRQoL can be a decisive factor in their engagement with a GP treatment regimen and telemonitoring processes.

The influence of environmental factors on atopic dermatitis (AD) has been investigated in many cross-sectional studies. It remains however unclear if they could influence AD development early in life. This prospective birth cohort study aimed to monitor aspects of family lifestyle and child's nutrition within a Caucasian population and to assess its association with AD development over the first 2 years of life. Genetic predisposition was evaluated based on family history and profilaggrin genotyping. Of 149 included children, 36 developed AD. Infants with a family history of atopy developed AD 2.6 times more frequently (30 of 97) than infants without atopic predisposition (6 of 52). Genotyping was carried out on 50% of the children included. Profilaggrin mutations (R501X, 2282del4, R2447X, and S3247X) were infrequent in our population. Lower incidence of AD was observed in infants exposed to a damp housing environment, lower household income, and smoking mothers with a higher but not with a lower education level.Conclusion: Family history of atopy was a significant risk factor for AD regardless of the most common, currently defined, FLG mutations. Humidity at home and passive smoking seem associated with AD development in infancy. What is Known: - Atopic dermatitis (AD) is associated with mutations in various genes of the immune system and the epidermal barrier complex in particular filaggrin (FLG) mutation. - Inherited factors alone cannot explain the rising AD; environmental factors are therefore likely to play a decisive role in this rise but the exact role that these factors may play in increasing AD risk in infancy remains unclear. Moreover, the relationship between environmental factors and AD has been the focus of mostly cross-sectional studies and not prospective studies. The consequences of demographic change are already noticeable in Saxony, the federal state with the highest average age in Germany and predominantly rural areas. In order to improve medical care for patients with Parkinson's disease (PwP), a status quo analysis of current care practice is required.To what extent does the utilization of medical services by PwP differ a) between urban and rural areas in Saxony and b) between PwP with and without neurologist contact in the observation period from 2011 to 2019?The cohort study was based on extensive routine data for Saxony from the health insurance company AOK PLUS from 2010 to 2019. A cohort of 15,744 PwP (n = 67,448 patient-years) was compared to a matched cohort (n = 674,480 patient-years; criteria: year of birth, gender, year of insurance, place of residence: urban/rural) without an ICD-10 coding of a movement disorder.Overall, there was a steady increase in the number of PwP in the dynamic cohort from 2011 (n = 6829) to 2019 (n = 8254). Urban-rural differences included a smaller proportion of patients being seen by a neurologist in rural areas. The PwP had a 3.5 to 4‑fold higher risk of dying compared to those in the comparison cohort. Changes in drug therapy for Parkinson's disease (i.e., increases in COMT and MAO inhibitors) and in remedy delivery (i.e., increases in occupational therapy and speech therapy) over the observation period were primarily seen in PwP who were seen by a neurologist.The study identified increased morbidity and mortality in PwP who are suitable targets for innovative care concepts. The increasing number of patients and the described differences document the need for this. Zusammenfassung Hintergrund Die anwenderseitige Akzeptanz ist ein wesentlicher Indikator fur den Einsatz und die Umsetzung von Telemonitoring-Anwendungen (TMA) in der Gesundheitsversorgung. Trotz diverser positiver Effekte, die bisherige Studien fur Nutzer von TMA offenlegen konnten, gibt es immer wieder Patienten, die aus unterschiedlichen Grunden ihre Teilnahme an einer Telemedizinstudie beenden oder grundsatzlich ablehnen. Zu den Grunden fur die Nichtakzeptanz und Nichtnutzung einer TMA, speziell durch Patienten mit Multimorbiditat uber 65 Jahren im hauslichen Umfeld, gibt es bisher wenig Evidenz. Um diese Forschungslucke zu schliesen, fokussiert die vorliegende Teilstudie auf patientenberichtete Motive fur eine Nichtakzeptanz und Nichtnutzung einer TMA im hauslichen Umfeld. Methoden Die vorliegende Teilstudie folgte einem gemischt-methodischen Ansatz und fokussierte die Patientenperspektive. Die quantitative Datenerhebung erfolgte uber computergestutzte Telefoninterviews bei allen Studienabbrechern und Nichtteilnehmern, die qualitative Erhebung fand in Form von Leitfadeninterviews mit Nichtnutzern ( Ergebnisse Neun Hausarztpraxen aus zwei deutschen Grosstadten rekrutierten insgesamt 177 Patienten gemas den Einschlusskriterien. Im Verlauf der Studie schieden 61 Studienteilnehmer (34,5%) aus, 80 Patienten (31,1%) lehnten eine Teilnahme an der Studie ab. Studienabbrecher und Nichtteilnehmer waren signifikant alter als aktiv teilnehmende Patienten (p = 0,004 bzw. p = 0,001). Dominierende Motive fur einen Studienabbruch waren der fehlende empfundene Zusatznutzen bzw. die fehlende inhaltliche Abwechslung der TMA auf dem Patienten-Tablet, das fehlende Interesse bzw. Bedarf an einer telemedizinischen Betreuung sowie die zu hohe zeitliche Beanspruchung durch die Studienteilnahme. Alleinlebende, ledige und verwitwete Patienten gaben signifikant mehr Schwierigkeiten in der Handhabung der Hardware (Tablet) (p = 0,040) und der Software (Motiva) (p = 0,013) an als verheiratete und in Lebensgemeinschaft lebende Patienten. Zudem wurden diese Probleme auch von weiblichen Patienten, Patienten ab 75 Jahren und Teilnehmern mit einem niedrigen Grad der Schulbildung berichtet. Schlussfolgerungen Um die Akzeptanz und den Mehrwert telemedizinischer Anwendungen fur Patienten zu erhohen, sollten zu Beginn der Entwicklung Bedarfsanalysen mit der zukunftigen Zielgruppe durchgefuhrt werden und die einzelnen Entwicklungsschritte laufend durch die Zielgruppe evaluiert werden, um eine maximale Nutzerzentrierung sicherzustellen. Die Handhabbarkeit sollte sich an den funktionellen Fahigkeiten der alteren Patienten orientieren. Zudem sollte die inhaltliche Ausgestaltung individuell zugeschnitten erfolgen, z.B. in Form eines modularen Aufbaus des Telemedizinprogramms. TMA sollten in einem angemessenen Mase eingesetzt werden, um eine Uberforderung der Patienten zu vermeiden und sich unaufdringlich in den gewohnten, alltaglichen Ablauf des Patienten einfugen. Eine einfache Handhabbarkeit der telemedizinischen Mess- und Eingabegerate ist fur eine patientenseitige Akzeptanz einer TMA ebenso wichtig wie der Abwechslungsreichtum der Angebote auf der Plattform und ein personlicher Ansprechpartner bei Technikruckfragen. Besondere Beachtung sollte alleinlebenden Patienten, Frauen, alteren Patienten uber 75 Jahren und Patienten mit niedrigem Schulbildungsgrad geschenkt werden, um auch diesen Patientengruppen einen uneingeschrankten und einfachen Zugang zu technikgestutztem Telemonitoring fur die eigene Gesundheitsversorgung zu gewahrleisten.

BACKGROUND The management of multimorbidity is complex and patients have a high burden of disease. When symptoms of dementia also appear, it becomes even more difficult for patients to cope with their everyday lives and manage their diseases. Home-based telemonitoring may support older patients with multimorbidity and mild cognitive impairment (MCI) in their regular monitoring and self-management. However, to date, there has been no investigation into whether patients with MCI are able to operate a telemonitoring app independently to manage their own diseases. This question has become even more important during the current COVID-19 pandemic to maintain high-quality medical care for this patient group. OBJECTIVE We examined the following research questions: (1) How do patients with MCI assess the usability of the telemonitoring app? (2) How do patients with MCI assess the range of functions offered by the telemonitoring app? (3) Was there an additional benefit for the patients with MCI in using the telemonitoring app? (4) Were patients with MCI able to use the telemonitoring app independently and without restrictions? (5) To what extent does previous experience with smartphones, tablets, or computers influence the perceived ease of use of the telemonitoring app? METHODS We performed a formative evaluation of a telemonitoring app. Therefore, we carried out a qualitative study and conducted guided interviews. All interviews were audio-recorded, transcribed verbatim, and analyzed using the Mayring method of structured content analysis. RESULTS Twelve patients (8 women, 4 men) were interviewed; they had an average age of 78.7 years (SD 5.6) and an average Mini-Mental State Examination score of 24.5 (SD 1.6). The interviews lasted between 17 and 75 minutes (mean 41.8 minutes, SD 19.4). Nine patients reported that the telemonitoring app was easy to use. All respondents assessed the range of functions as good or adequate. Desired functionalities mainly included more innovative and varied educational material, better fit of the telemonitoring app for specific needs of patients with MCI, and a more individually tailored content. Ten of the 12 patients stated that the telemonitoring app had an additional benefit for them. Most frequently reported benefits included increased feeling of security, appreciation of regular monitoring of vital parameters, and increased independence due to telemonitoring. Eight patients were able to operate the app independently. Participants found the app easy to use regardless of whether they had prior experience with smartphones, tablets, or computers. CONCLUSIONS The majority of examined patients with MCI were capable of operating the telemonitoring app independently. Crucial components in attaining independent use were comprehensive personal support from the start of use and appropriate design features. This study provides initial evidence that patients with MCI could increasingly be considered as a relevant user group of telemonitoring apps.

As integrated care is recognized as crucial to meet the challenges of chronic conditions such as Parkinson&rsquo, s disease (PD), integrated care networks have emerged internationally and throughout Germany. One of these networks is the Parkinson Network Eastern Saxony (PANOS). PANOS aims to deliver timely and equal care to PD patients with a collaborative intersectoral structured care pathway. Additional components encompass personalized case management, an electronic health record, and communicative and educative measures. To reach an intersectoral consensus of the future collaboration in PANOS, a structured consensus process was conducted in three sequential workshops. Community-based physicians, PD specialists, therapists, scientists and representatives of regulatory authorities and statutory health insurances were asked to rate core pathway-elements and supporting technological, personal and communicative measures. For the majority of core elements/planned measures, a consensus was reached, defined as an agreement by &gt, 75% of participants. Additionally, six representatives from all partners involved in the network-design independently assessed PANOS based on the Development Model for Integrated Care (DMIC), a validated model addressing the comprehensiveness and maturity of integrated care concepts. The results show that PANOS is currently in an early maturation state but has the potential to comprehensively represent the DMIC if all planned activities are implemented successfully. Despite the favorable high level of consensus regarding the PANOS concept and despite its potential to become a balanced integrated care concept according to the DMIC, its full implementation remains a considerable challenge.

Zusammenfassung Ziel der Studie Ziel der Studie war es, Schnittstellenprobleme zwischen stationärer, hausärztlicher und ambulant-fachspezialisierter Patientenversorgung bei älteren multimorbiden Patienten aus hausärztlicher Sicht aufzudecken. Methodik Die Datenerhebung erfolgte im Rahmen der Pilotstudie „Multimedikation und ihre Folgen für die hausärztliche Patientenversorgung in Sachsen“ und umfasste leitfadengestützte Interviews mit 7 Hausärzten. Die Interviews wurden vollständig transkribiert und inhaltlich in Anlehnung an die induktive Kategorienentwicklung nach Mayring analysiert. Ergebnisse An stationärer zu ambulant-hausärztlichen Schnittstelle zeigten sich v. a. Probleme bei der intersektoralen Kommunikation und Kooperation, dem Entlassmanagement sowie beim Medikationsmanagement. An der Schnittstelle vom Hausarzt zu ambulant-fachspezialisierten Kollegen zeigten sich Probleme hinsichtlich der freien Arztwahl, des Terminmanagements, des ärztlichen Entscheidungsfindungsprozesses, des Medikationsmanagements sowie bei der Nutzung der elektronischen Gesundheitskarte. Schnittstellenunabhängig wurden die mangelnde Kommunikation und Kooperation als zentrale Herausforderungen herausgearbeitet. Schlussfolgerung Aus hausärztlicher Sicht gibt es eine Vielzahl von Problemen zu allen angrenzenden Schnittstellen. Auffällig hierbei ist die mangelnde Kommunikation und Kooperation zwischen den verschiedenen Beteiligten. Als mögliche Ursachen zeigen sich die häufig sehr unterschiedlichen Arbeitsweisen und Rahmenbedingungen, personellen und materiellen Ressourcen sowie Therapieintentionen und -ansätze zwischen den settingspezifischen Schnittstellen. Ein möglicher Ansatz zur Verbesserung der Situation kann das seit 2016 bestehende Gesetz für sichere digitale Kommunikation und Anwendungen im Gesundheitswesen sein. Es verspricht u. a. einen sicheren und zügigen inter- und intrasektoralen Informationsaustausch von Patientendaten. Übergeordnetes Ziel der Gesundheitsversorgung in Deutschland sollte ein optimiertes und prozessorientiertes Schnittstellenmanagement sein, um Patienten einen sicheren und nahtlosen sektoralen Übergang zu gewährleisten.

Atopic dermatitis (AD) has a well-established association with skin colonization or infection by Staphylococcus aureus, which can exacerbate the disease. However, a causal relationship between specific changes in skin colonization during the first years of life and AD development still remains unclear. In this prospective birth cohort study, we aimed to characterize the association between skin colonization and AD development in 149 white infants with or without a family history of atopy. We assessed infants clinically and collected axillary and antecubital fossa skin swabs for culture-based analysis at birth and at seven time points over the first 2 years of life. We found that at age 3 months, S. aureus was more prevalent on the skin of infants who developed AD later on. S. aureus prevalence was increased on infants' skin at the time of AD onset and also 2 months before it, when compared with age-matched, unaffected infants. Furthermore, at AD onset, infants testing positive for S. aureus were younger than uncolonized subjects. In conclusion, our results suggest that specific changes in early-life skin colonization may actively contribute to clinical AD onset in infancy.

A urolitíase canina e felina constitui-se em uma das principais causas de formação de cálculos urinários, que podem ocorrer desde a pelve renal até a uretra. Estes cálculos formados alteram a fisiologia do trato urinário, variando conforme a sua composição. Os urólitos mais encontrados em cães são os de oxalato de cálcio e fosfato amoníaco magnesiano. Já em gatos, os de oxalato de cálcio são os mais prevalentes entre os sete e nove anos de idade e os de estruvita em felinos jovens. Os sinais clínicos variam entre si, sendo o método de diagnóstico mais utilizado a radiografia simples ou contrastada. Como fatores predisponentes são citados a ocorrência de infecções do trato urinário, variações no pH urinário, fatores hereditários, tipo de dieta e pouca ingestão de água. Esta revisão tem como objetivo informar sobre os diferentes tipos de urólito em cães e gatos, como ocorre a formação dos mesmos e tratamentos empregados na forma clínica e cirúrgica.

Ingrown nails are a common and painful pathology not always responding to local treatment. Destroying the matrix by application of phenol (phenolisation) has proven its superiority over the so called classic surgical methods. This is partially due to the exceptional properties of phenol: demyelinisation of the terminal nerve fibres provides long-term pain relief, the coagulation of proteins stops bleeding and the molecule has antiseptic properties. Phenolisation requires only few resources and can be widely used. Recurrence rates are particularly low at 2 %. We present this technique as the treatment of choice for ingrown nails.L’ongle incarné est une pathologie fréquente et douloureuse ne répondant pas toujours à des soins locaux. La technique par destruction chimique par phénol (phénolisation) a montré sa supériorité par rapport aux méthodes chirurgicales dites classiques. Ceci s’explique par les propriétés particulièrement intéressantes du phénol : effet antalgique à long terme par démyélinisation des fibres nerveuses terminales, une cautérisation par coagulation des protéines ainsi que des propriétés antiseptiques. La phénolisation nécessite peu de matériel et peut s’appliquer à une large frange de la population. De plus, elle présente un taux de récidives particulièrement bas à 2 %. User acceptance is a key indicator and driver for the use and implementation of telemonitoring applications (TMA) in healthcare. Despite various positive effects that previous studies have revealed for users of TMA, there are always patients who discontinue their participation in a telemedicine study or even decline participation. There is little evidence for the reasons for non-acceptance and non-use of TMA, especially in multimorbid patients at the age of 65 and over in their home environment. To close this research gap, this sub-study focuses on patient-reported reasons for non-acceptance and non-use of TMA in the home environment.This study follows a mixed-method approach and focuses on patients' perspective. Quantitative data collection took place via computer-assisted telephone interviews among all drop-outs and non-participants. Qualitative data were collected via semi-structured interviews with drop-out patients and non-users. Eligible patients were recruited consecutively by general practitioners, informed and included in the study according to the inclusion criteria. Amongst others, patients measured their vital signs (blood pressure, heart frequency, oxygen saturation, weight) via telemedical measures and sent them via tablet to a Care Coordination Center to ascertain the need for intervention. Collected data on non-acceptance and non-use of TMA were analyzed quantitatively and qualitatively.Nine general practices in two German cities included a total of 177 patients according to the inclusion criteria. During the study, 61 study participants (34.5 %) dropped out, 80 patients (31.1 %) declined participation in the study. Drop-outs and non-participants were significantly older than active participants (p=.004 and p=.001, respectively). Predominant reasons for drop-out were the lack of the perceived added value and the content-related variety of the program on the patient's tablet, the missing interest/need for telemedical monitoring as well as the time spent participating in the study. Patients living alone, single and widowed patients reported significantly more difficulties in handling the hardware (tablet) (p=.040) and the program (Motiva) (p=.013) than married and cohabiting patients. These reasons were also reported mainly by female patients, patients aged 75 years and over, and those with a low level of education.In order to increase the acceptance and the added value of TMA for patients, the individual needs of the future target group should be analyzed at the beginning of the development. To ensure maximum user centricity, individual development steps should be continuously evaluated by the target group. TMA should be adapted to the functional abilities of elderly, multimorbid patients through, e. g., an appropriate design of the content, which is tailored to patients' individual needs. TMA should be used to an appropriate degree to avoid overburdening and should fit unobtrusively into patients' usual daily routine. For patient-specific acceptance of TMA, easy handling of the telemedical measuring and input devices is as important as the variety of offers on the platform and personal contact for technical queries. Special attention should be paid to patients who live alone, women, elderly patients over 75 years of age, and poorly educated patients in order to ensure full and easy access to technology-based telemonitoring for their own healthcare.

The aim of the study was to identify interface problems between inpatient, GP and outpatient specialist care from the perspective of general practitioners in Dresden, especially in older multimorbid patients.The data were collected in the context of the pilot study "Multimedication and its Consequences for the Primary Care of Patients in Saxony" and included guided interviews with 7 general practitioners. The interviews were transcribed and analyzed according to the inductive content analysis of Mayring.At the interface of inpatient to outpatient care, several problems regarding discharge management, intersectoral communication and cooperation as well as in the management of medication were found. Concerning the interface between general practitioners and outpatient specialist care, problems were particularly marked with regard to free choice of doctors, appointment management, medical decision-making process, medication management as well as the use of the electronic health card. Regardless of the interfaces, the lack of communication could be worked out as a central challenge.There are several problems with all adjacent interfaces. What is striking here is the lack of communication and cooperation between all parties involved. Possible causes were the often very different working methods and framework conditions, human and material resources and therapeutic intentions and approaches between the different interfaces. One possible approach to improve the situation may be the law regarding secure digital communications and healthcare applications, which exists since 2016. Among other things, it promises a secure and rapid inter- and intrasectoral exchange of patient data. The overall goal of healthcare in Germany should be an optimized and process-oriented interface management in order to ensure a secure and seamless sectoral transition to patients.Ziel der Studie war es, Schnittstellenprobleme zwischen stationärer, hausärztlicher und ambulant-fachspezialisierter Patientenversorgung bei älteren multimorbiden Patienten aus hausärztlicher Sicht aufzudecken.Die Datenerhebung erfolgte im Rahmen der Pilotstudie „Multimedikation und ihre Folgen für die hausärztliche Patientenversorgung in Sachsen“ und umfasste leitfadengestützte Interviews mit 7 Hausärzten. Die Interviews wurden vollständig transkribiert und inhaltlich in Anlehnung an die induktive Kategorienentwicklung nach Mayring analysiert.An stationärer zu ambulant-hausärztlichen Schnittstelle zeigten sich v. a. Probleme bei der intersektoralen Kommunikation und Kooperation, dem Entlassmanagement sowie beim Medikationsmanagement. An der Schnittstelle vom Hausarzt zu ambulant-fachspezialisierten Kollegen zeigten sich Probleme hinsichtlich der freien Arztwahl, des Terminmanagements, des ärztlichen Entscheidungsfindungsprozesses, des Medikationsmanagements sowie bei der Nutzung der elektronischen Gesundheitskarte. Schnittstellenunabhängig wurden die mangelnde Kommunikation und Kooperation als zentrale Herausforderungen herausgearbeitet.Aus hausärztlicher Sicht gibt es eine Vielzahl von Problemen zu allen angrenzenden Schnittstellen. Auffällig hierbei ist die mangelnde Kommunikation und Kooperation zwischen den verschiedenen Beteiligten. Als mögliche Ursachen zeigen sich die häufig sehr unterschiedlichen Arbeitsweisen und Rahmenbedingungen, personellen und materiellen Ressourcen sowie Therapieintentionen und -ansätze zwischen den settingspezifischen Schnittstellen. Ein möglicher Ansatz zur Verbesserung der Situation kann das seit 2016 bestehende Gesetz für sichere digitale Kommunikation und Anwendungen im Gesundheitswesen sein. Es verspricht u. a. einen sicheren und zügigen inter- und intrasektoralen Informationsaustausch von Patientendaten. Übergeordnetes Ziel der Gesundheitsversorgung in Deutschland sollte ein optimiertes und prozessorientiertes Schnittstellenmanagement sein, um Patienten einen sicheren und nahtlosen sektoralen Übergang zu gewährleisten.

84% of online participants had previously experienced an undesirable Facebook photo.Online and offline methods are used to manage undesirable Facebook photos.Untagging is the preferred management method.Younger Facebook users are more likely to untag than older users.Agreeable individuals are more likely to use direct management methods. Facebook is the most popular social networking site (SNS) in the world. This study explored the methods individuals use to manage perceived invasions of privacy in the form of undesirable Facebook photos uploaded and tagged by other users. Limited previous research has focused solely on untagging, deletion request, privacy setting and face-to-face management methods. A series of qualitative focus group discussions identified these and further management methods, such as emailing or texting the uploader to request the removal of the image. An online quantitative questionnaire examined the relationship of the identified methods with age, gender and personality traits.Untagging was the most frequently cited management method in both the focus group discussions and online questionnaire. Contrary to previous studies, the findings suggested that women were not more likely to untag than men. Younger Facebook users were found to be more likely to untag than older users but were not more likely to use Facebook management methods. Agreeable individuals were more likely to use direct management methods. On average participants chose three methods they would use to manage these undesirable impressions. Future research should investigate the influence of the photo content and the tagged user's relationship with the uploader on the management of undesirable SNS photos.

Sweet's syndrome is a rare neutrophilic dermatosis. Typically, patients present with a sudden onset of tender erythematous skin lesions (papules, nodules, and plaques), of asymetrical distribution, located preferentially on the superior limbs, face and neck, associated whith high fever and neutrophilia. Histologicaly, there is a diffuse infiltrate of neutrophils located in the dermis. Most of the time idiopathic, it may be associated with a respiratory or gastro-intestinal infection, with inflammatory bowel disease, pregnancy and vaccination. A drug-induced form and a malignancy-associated form have been described. Based on illustrations and two clinical cases, we wish to focus on the diagnostic strategy and the management of this disease.Le syndrome de Sweet est une dermatose neutrophilique rare. Les patients présentent classiquement une apparition brutale de lésions cutanées douloureuses sous forme de papules, nodules ou plaques érythémateuses asymétriques localisées préférentiellement sur les extrémités supérieures, le visage et le cou, associées à une fièvre élevée, une neutrophilie et, à l’histologie, un infiltrat diffus de neutrophiles dans le derme. Le plus souvent idiopathique, il peut être en lien avec des infections respiratoires et gastro-intestinales, avec des maladies inflammatoires de l’intestin, avec une grossesse ou une vaccination. Des formes médicamenteuses et paranéoplasiques ont été rapportées. A l’aide d’images cliniques et de deux cas cliniques, nous souhaitons rappeler les points essentiels nécessaires au diagnostic et à la prise en charge de cette maladie.

Objectives: We evaluated the effectiveness of European chronic care programmes for type 2 diabetes mellitus (characterised by integrative care and a multicomponent framework for enhancing healthcare delivery), compared with usual diabetes care. Design: Systematic review and meta- analysis. Data sources: MEDLINE, Embase, CENTRAL and CINAHL from January 2000 to July 2015. Eligibility criteria: Randomised controlled trials focussing on (1) adults with type 2 diabetes, (2) multifaceted diabetes care interventions specifically designed for type 2 diabetes and delivered in primary or secondary care, targeting patient, physician and healthcare organisation and (3) usual diabetes care as the control intervention. Data extraction: Study characteristics, characteristics of the intervention, data on baseline demographics and changes in patient outcomes. Data analysis: Weighted mean differences in change in HbA1c and total cholesterol levels between intervention and control patients (95% CI) were estimated using a random- effects model. Results: Eight cluster randomised controlled trials were identified for inclusion (9529 patients). One year of multifaceted care improved HbA1c levels in patients with screen- detected and newly diagnosed diabetes, but not in patients with prevalent diabetes, compared to usual diabetes care. Across all seven included trials, the weighted mean difference in HbA1c change was -0.07% (95% CI -0.10 to -0.04) (-0.8 mmol/mol (95% CI -1.1 to -0.4)); I-2= 21%. The findings for total cholesterol, LDLcholesterol and blood pressure were similar to HbA1c, albeit statistical heterogeneity between studies was considerably larger. Compared to usual care, multifaceted care did not significantly change quality of life of the diabetes patient. Finally, measured for screen-detected diabetes only, the risk of macrovascular and mircovascular complications at follow-up was not significantly different between intervention and control patients. Conclusions: Effects of European multifaceted diabetes care patient outcomes are only small. Improvements are somewhat larger for screen-detected and newly diagnosed diabetes patients than for patients with prevalent diabetes.

The German healthcare system has experienced an enormous cost increase in recent years due to demographic change, the growing rates of chronic diseases and the comparatively good healthcare and health service provision. The current changes to healthcare in Germany are leading to new challenges that must be met with new ways of healthcare delivery, especially for the increasing number of patients with chronic disease. “Gesundes Kinzigtal” is a flagship-project managing a fully population-based integrated care system since 2005 with a new and innovative financing model using shared savings contracts. Since 2007, the project has offered a secondary prevention programme to reduce the development of risk factors related to the metabolic syndrome, the development of type-2-diabetes mellitus and cardiovascular diseases. Gesundes Kinzigtal strives for the Triple Aim consisting of better cost-effectiveness of care, improved patient experiences of care and improved health outcomes.

With its impressive megalithic architecture dating to the tenth to ninth millennium cal. bc, the site of Gobekli Tepe (south-east Anatolia) is singular in the cultural history of mankind. Excavations at this ritual centre produced a rich archaeofaunal assemblage dominated by skeletal remains of Persian gazelle (Gazella subgutturosa). In order to trace relevant features of this species' behaviour in early Neolithic times relating to its dietary demands, catchment area and annual round, zooarchaeological analysis was complemented by stable isotope analysis of carbon, oxygen and strontium in tooth enamel and bone apatite. Demographic profiles and stable isotope results are consistent with a seasonal presence and hence migratory behaviour in gazelle. Conceivably working events, feasting and performing rituals at Gobekli Tepe coincided with high game density in the southern Anti-Taurus piedmont.

The influence of different feedstuffs on parameters of digestion and ethology in growing rabbits (duration and number of feed intake, needed masticatory movements, pH value in stomach and small intestine, dry matter content of stomach chyme, level of destruction of cell structures in small intestine) was analysed. 384 five weeks old rabbits were fed with one of three feedstuffs: pellets (mean fibre length = 3 mm), fibre blocks (mean fibre length = 40 mm) and hay-oat-beat-ration (length of hay fibre ≥ 70 mm). The masticatory movements and duration for uptake 0.1 g of feedstuffs were measured in a special observation box. Rabbits in group cages were observed by 24 h video recording and duration and numbers of feed intakes were documented. After 8 weeks rabbits were slaughtered and pH values measured in stomach chyme (and dry matter content) and small intestine. Samples of them were taken and histologically examined (total length of villi and crypts, width of villi and degree of destruction determined by scores from 0 [= no destruction] to 3 [= severe destruction of villi]). Pellets lead to a faster feed intake with a lower number of masticatory movements. This equates a minor feeding time per feed intake and a higher amount of feedstuff in a shorter time. The dry matter content in stomach chyme increases and pH value was significantly higher there, but lower in duodenum. They also showed a significantly higher degree of destruction of villi, a shorter length and a larger width of villi than others.

Stroke is a severe clinical disorder in sickle cell disease (SCD), and few studies have evaluated transcranial Doppler (TCD) flow velocities in hemoglobin SC disease (HbSC). The guidelines for stroke risk are based on evaluations in sickle cell anemia (SCA) or HbS/β thalassemia.In this study, we compare cerebral blood flow in patients with SCD stratified by genotypes. A total of 1,664 pediatric patients with SCD underwent TCD velocity screening, and the time-averaged maximum mean velocity (TAMM) was determined in the middle cerebral artery (MCA), anterior cerebral artery (ACA), and distal intracranial internal carotid artery (ICA).Abnormal velocities were not identified in the ACA; therefore, we only use ICA and MCA velocities. TAMM from the left and right in the ICA and MCA was 134.3 ± 32.0 and 134.4 ± 32.6 cm/s in patients with SCA, and 105.2 ± 20.6 and 104.7 ± 20.0 cm/s in the patients with HbSC, respectively. Mean TAMM between right and left ICA/MCA was 134.5 ± 30.5 cm/s in the SCA group, and 104.9 ± 19.3 cm/s in the HbSC group. Notably, our data show that TCD velocities were significantly lower among the patients with HbSC compared to SCA. TAMM was negatively correlated with hemoglobin and hematocrit in both genotypes.These results suggest that a different cut-off value for abnormal TCD velocities could be considered for patients with HbSC. Additional studies are warranted to determine the actual risk of stroke in HbSC genotype associated with this possible TCD risk value.

London’s Victoria and Albert Museum, the UK’s national museum of art and design, recently created a new centre for public learning through creative design. The development process was key to the project, which has resulted in one of the most innovative and attractive learning spaces in any museum today. Research, consultation and collaboration, involving the people who are going to use the building and the architects/designers from the outset, has been an approach that has worked very successfully.

Ιntroduction: A growing number of EU citizens suffer from diabetes, posing an increasing health, social and economic burden. This burden is mostly driven by type 2 diabetes (T2DM), which is leading to a rapidly growing group of ‘older people’ (65+) with diabetes. An increase in the average life expectancy adds to this accrued number of elderly living with T2DM, experiencing a number of associated comorbidities. Management of this large group poses unique challenges, and outcomes remain far from optimal. Theory/Methods: Proposed in the mid-1990s, Chronic Care Model (CCM) identified essential elements of a health care system that encourage high-quality chronic disease care. These elements are the _community, health system, self-management support, delivery system design, decision support and clinical information systems._ The objective of the MANAGE-CARE project is to adapt and refine the CCM to ensure active ageing with T2DM, by optimizing opportunities for health, participation and security in order to enhance quality of life as people age. Based on a literature review, which assesses the state-of-the-art knowledge and evidence on existing disease management models and unmet needs and priorities of patients with T2DM, an innovative, a patient-centered chronic care management model is currently developed. After conducting a desk research, an international web-based survey has been performed. Furthermore guideline-based interviews with patients suffering from T2DM and other stakeholder groups in 15th International Conference on Integrated Care, Edinburgh, UK, March 25-27, 2015 1 International Journal of Integrated Care – Volume 15, 27 May – URN:NBN:NL:UI:10-1-117099– http://www.ijic.org/ diabetes care have been done. The results of the survey, the interviews and the assessment of unmet needs and priorities of Elderly will serve as a basis for the development of a European Practice Compendium, built by a Delphi study design methodology. In accordance to this new model and to ensure a patient-centered approach, training programs for both health professionals and patients are now under construction. To validate the education program proposed by the MANAGE-CARE model, a pilot study is planned in 2015, using focus groups and validated structured questionnaires for interviewing volunteer older patients with T2DM and their caregivers. Finally, a toolkit for implementation of the new model and transfer of the model to other chronic diseases will be constructed. Results: The preliminary outcomes of the survey identified the following major needs to be addressed by the CCM for active ageing with T2DM per dimension of the CCM. patient and family: Need to improve patient's knowledge, skills and confidence to manage demanding co-morbidities and need for more practical help with caregiving health care team: Need for a team and “1-Stop” approach to providing comprehensive care. self management support: Need to include strategies to solve problems and barriers in the selfmanagement education and need for power balance between patient and health care provider delivery system design: Need for appropriate support programs to treat depression and dementia symptoms of the elderly, as well as develop problem solving and coping skills. decision support: Need for policy/procedures for follow-up of referrals of patients who screened positive clinical information system: Need for training of Diabetes and on data analysis reporting, and utilization and large scale development of tele-health services health system: Need to develop and implement policy and procedures for the provision of CCM services, via legislative and funding actions. Discussion: MANAGE-CARE concept and preliminary findings are in line with current European policies aiming to shift from disease management to chronic care management, through change in care delivery and through partnering for change, addressing in particular older patients with multiple chronic conditions and using innovative business modeling. Diabetes can be used as a ’test-case’ for developing this innovative model. Prevention of costly complications, hospitalization and frailty in elderly with chronic diseases is directly in line with the Implementation plan of the European Innovation Partnership on Active and Healthy Ageing, aiming at an increase in healthy life years and promoting healthy ageing. Conclusions: A comprehensive approach addressing the various needs of the aspects of chronic disease management of diabetes is required in order to provide an integrated service model for active ageing. Lessons learned (from the point of view or research development): European health systems’ heterogeneity is an important challenge for the development of a care model. Therefore, a strong partnership reflecting all key stakeholder groups and relevant expertise, as well as a broad range of partners from different European countries, is required. Limitations: The outcomes of the survey need to be validated via the pilot studies planned involving elderly patients with T2DM, their caregivers and health professionals. Further Research suggestions: CCM could be more specific to address active ageing in the major chronic diseases.

OBJETIVO: Foram realizadas medidas antropométricas de recém-nascidos (RN) baianos, para definir medidas condizentes com a realidade desta região. MÉTODO: Estudo tipo corte transversal em bebês de termo e normais com as medidas de perímetro cefálico, distância biauricular, distância anteroposterior, tamanho da fontanela e índice cefálico. RESULTADOS: Foram avaliados 388 bebês, sendo 204 (52,6%) masculinos e 184 (47,4%) femininos. O perímetro cefálico variou entre 31,0 e 38,0 cm, com média de 34,4 ± 1,2. O índice cefálico variou entre 0,75 e 1,06, com média de 0,91 ± 0,05. CONCLUSÃO: As medidas antropométricas são aferidas visando à detecção precoce de anomalias. Possivelmente, estas medidas podem ser influenciadas por fatores raciais; no entanto, as medidas realizadas no Brasil em geral seguem valores de autores estrangeiros. Assim, o estudo mostra dados de uma região nordestina e salienta a importância da realização de estudos multicêntricos. OBJECTIVE: The study has been carried out through anthropometric measures of Bahian newborns, to define measures according with the reality of this region. METHOD: Study type transversal cut in term babies with the measures of cephalic perimeter, biauricular and anteroposterior distances, fontanel and cephalic index. RESULTS: 388 babies, 204 (52.6%) of males and 184 (47.4%) females had been evaluated. The cephalic perimeter varied between 31.0 and 38.0 cm, with 34.4 ± 1.2 average. The cephalic index varied between 0.75 and 1.06, with 0.91 ± 0.05 average. CONCLUSION: The anthropometric measures are surveyed aiming at precocious detention of anomalies. Possibly, these measures can be influenced by racial factors; however, the measures carried through in Brazil follow values of foreign authors. Thus, the study shows data of a Northeastern region and points out the importance of the accomplishment of multicentric studies.

O processo decisório merece destaque nas organizações, por influenciar diretamente nas estratégias delas, e por este motivo torna-se importante analisar os vieses de decisão que podem influenciá-lo. O presente estudo teve como objetivo verificar a existência do Framing effect, previsto por Kahneman e Tversky, em sua Teoria dos Prospectos. A pesquisa é do tipo Survey e foram aplicados 123 questionários aos estudantes do curso de Bacharelado em Ciências Contábeis do Instituto Federal de Educação, Ciência e Tecnologia do Paraná – IFPR, Campus Curitiba e os resultados confirmaram a existência do Framing effect.

Hintergrund: Die demographischen Veränderungen der Gesamtbevölkerung in Deutschland beeinflussen die Entwicklung der Arztzahlen in Deutschland seit Jahren negativ. Die aktuelle Versorgungslandschaft in Deutschland zeichnet sich durch eine sinkende Anzahl ärztlicher und nicht-ärztlicher Leistungserbringer aus, die einer wachsenden Anzahl von Menschen gegenüberstehen, die Versorgungsleistungen beanspruchen. Der Sicherstellungsauftrag einer flächendeckenden ambulanten Versorgung ist aufgrund der Abnahme der Anzahl an Hausarztpraxen v.a. in ländlich geprägten Gebieten gefährdet. Diverse Lösungsansätze wurden und werden aktuell umfangreich diskutiert. Ein vielversprechender Ansatz, den beschriebenen Herausforderungen zu begegnen, sind integrierte, telemedizinische Versorgungskonzepte. Fragestellungen: Die Zielstellung der Dissertation ist die zusammenfassende Darstellung des Einflusses einer im häuslichen Bereich genutzten Telemonitoring-Anwendung (TMA) auf verschiedene Outcomes in der gesamten Studienkohorte multimorbider Patienten ab 65 Jahren sowie in definierten Risikogruppen und soziodemographischen Subgruppen. Um die formulierte Zielstellung zu erreichen, wurden Gründe exploriert, die zur Nichtakzeptanz und Nichtnutzung der TMA durch die Studienteilnehmer geführt haben und mit welchen Schwierigkeiten die Nutzung verbunden war. Zudem wurde untersucht, wie adhärent die Studienpatienten im Umgang mit der TMA waren, inwieweit die patientenseitige Adhärenz und die gesundheitsbezogene Lebensqualität miteinander assoziiert waren und inwieweit sich die Lebensqualität der Studienteilnehmer während der Nutzung der TMA verändert hat. Material und Methoden: In die Gesamtstudie wurden Patienten eingeschlossen, wenn sie u.a. 65 Jahre und älter waren und mindestens zwei chronische Erkrankungen aufwiesen (Multimorbiditätskriterium). Nach Studieneinschluss wurden die Studienteilnehmer durch das Deutsche Rote Kreuz (DRK) mit der TMA ausgestattet, die von Philips Medical Systems GmbH bereitgestellt wurde. Die TMA bestand aus einem Blutdruckmessgerät mit integrierter Herzfrequenzmessung, einem Sauerstoffsättigungs-Messgerät, einer Personenwaage sowie aus einem Tablet mit der Motiva-Software. Nach Einführung in die Anwendung durch einen Techniker des DRK wurden die Studienteilnehmer in den interprofessionellen und interdisziplinären Versorgungsprozess eingebunden und begannen, ihre Vitalwerte über die telemedizinischen Geräte gemäß einem hausärztlich empfohlenen Regime zu messen sowie Fragebögen zum Gesundheitszustand zu beantworten. Die Vitaldaten wurden via Bluetooth von den Messgeräten an das Tablet und anschließend an das Care Coordination Center des DRK transferiert, die die Werte kontinuierlich auf Interventionsbedarf überprüften. Im Falle einer Überschreitung der vorab hausärztlich definierten krankheitsspezifischen Vitalwertgrenzen erfolgte eine zügige Intervention. Die erste Teilstudie wurde in einem gemischt-methodischen Ansatz durchgeführt und fokussierte die Motive für die risiko- und subgruppenspezifische Nichtakzeptanz und die Nichtteilnahme potentiell geeigneter Patienten. Die zweite Teilstudie nutzte eine quantitative Methodik und thematisierte die gruppenunspezifische Nichtakzeptanz, die patientenseitige Adhärenz sowie Herausforderungen im Umgang mit der TMA. In der dritten Teilstudie mit ebenfalls quantitativer Methodik wurden Unterschiede und Veränderungen in der gesundheitsbezogenen Lebensqualität zwischen den Studienpatienten mit und ohne Depression und/oder MCI im Verlauf der TMA-Nutzung eruiert und Assoziationen zwischen der patientenseitigen Adhärenz und Änderungen der gesundheitsbezogenen Lebensqualität untersucht. Ergebnisse: Drei Hausärzte in Dresden sowie sechs Hausärzte in Leipzig bewerteten insgesamt 257 ältere, multimorbide Patienten als zur Studienteilnahme potentiell geeignet, wovon 177 Patienten für die Studie rekrutiert wurden. Achtzig Patienten (31,1%) lehnten eine Teilnahme an der Studie ab (Nichtteilnehmer) und 61 Studienteilnehmer (34,5%) schieden im Verlauf der Studie aus (Studienabbrecher). Die Studienpatienten waren durchschnittlich 79,6 Jahre alt (SD=5,6). Studienabbrecher (81,3 Jahre (SD=5,8)) und Nichtteilnehmer (82,2 Jahre (SD=5,7)) waren signifikant älter als die aktiv an der Studie teilnehmenden Patienten (78,8 Jahre (SD=5,3)). Dominierende Abbruchgründe bei den Studienteilnehmern waren der fehlende empfundene Zusatznutzen bzw. die fehlende inhaltliche Abwechslung auf dem Tablet, kein Interesse bzw. Bedarf an einer telemedizinischen Betreuung sowie die zu hohe zeitliche Beanspruchung durch die Studienteilnahme. Alleinlebende (ledige und verwitwete) Studienpatienten gaben signifikant mehr Schwierigkeiten in der Handhabung des Tablets und der Motiva-Software an als verheiratete bzw. in Lebensgemeinschaft lebende Studienpatienten. Diese Probleme berichteten ebenso weibliche Studienpatienten, Studienteilnehmer ab 75 Jahren und jene mit einem niedrigen Schulbildungsgrad. Nichtteilnehmer nahmen aus Angst vor zu hoher zeitlicher Beanspruchung, aufgrund des fehlenden Interesses bzw. Bedarfes an einer telemedizinischen Betreuung sowie aus Angst vor Überforderung durch die eingesetzte Technologie nicht an der Studie teil. Studienpatienten hatten Schwierigkeiten im Umgang mit den Tablets (z.B. zu kleiner An-/Aus-Knopf) und der Software (z.B. persistierender Flugmodus, fehlende Dialogoption, zu kleine Symbol- und Schriftgrößen) sowie durch eine zu langsame oder instabile Internetverbindung. Im Hinblick auf das Adhärenzverhalten der Studienteilnehmer zeigte sich, dass jene, die Blutdruck und Herzfrequenz einmal wöchentlich messen sollten, dies seltener durchführten. Studienteilnehmer, die Blutdruck und Herzfrequenz mehr als einmal wöchentlich oder täglich messen sollten, maßen häufiger, als hausärztlich empfohlen. Die Adhärenz der Studienteilnehmer bezüglich ihrer Vitaldatenmessungen über die TMA war signifikant mit einer höheren psychischen Lebensqualität assoziiert, während die körperliche Lebensqualität unverändert blieb. In der Gesamtschau aller Studienpatienten verbesserte sich die psychische Lebensqualität im Gegensatz zur körperlichen Lebensqualität im Verlauf der TMA-Nutzung signifikant. Studienpatienten ohne psychische Beeinträchtigung und mit MCI wiesen im Verlauf signifikant höhere Werte für die psychische Lebensqualität auf als Patienten mit Depression und MCI. Zudem wiesen Studienpatienten mit Depression zum Nutzungsende eine signifikant niedrigere psychische Lebensqualität auf als jene mit MCI sowie Studienteilnehmer ohne psychische Beeinträchtigung. Studienpatienten mit Depression sowie mit Depression und MCI hatten eine über den Verlauf signifikant niedrigere körperliche Lebensqualität als Studienpatienten ohne psychische Beeinträchtigung. Schlussfolgerungen: Ein entscheidender Indikator und Treiber für eine effektive, nachhaltige und erfolgreiche Implementierung telemedizinischer Anwendungen sowie die Voraussetzung zur Ausschöpfung deren Potentials bleibt stets die Akzeptanz seitens der Endanwender. Die hier vorliegenden Ergebnisse zeigten, dass zu entwickelnde TMA auf die bestehende Krankheit der nutzenden Patienten zugeschnitten und entsprechend ihrer alters- bzw. krankheitsbedingten Funktionseinschränkungen gestaltet werden müssen, um eine gute Handhabbarkeit telemedizinischer Anwendungen zu erreichen. Nur dann werden Patienten Telemedizin in ihrem Alltag akzeptieren und dies als Gewinn für die eigene Gesundheitsversorgung begreifen, was sich letztlich an einer höheren gesundheitsbezogenen körperlichen und psychischen Lebensqualität abbilden lässt. Von wesentlicher Bedeutung ist der Einbezug vulnerabler Patientengruppen, z.B. mit Depression und MCI, in telemedizinische Entwicklungs- und Versorgungsprozesse, um deren Bedarfe rechtzeitig festzustellen, ihren Gesundheitszustand zu stabilisieren bzw. Verschlechterungen zu reduzieren. Die vorliegenden Befunde konnten einerseits zeigen, dass eine häuslich genutzte TMA ältere, multimorbide Menschen und ihre Versorger dabei unterstützen kann, die hausärztliche Versorgung sowie die Vernetzung von Leistungserbringern durch Telemonitoring nutzbringend zu begleiten. Andererseits können anwendungsspezifische Technikprobleme zur Verunsicherung des Patienten hinsichtlich des eigenen Gesundheitszustandes führen und die Handhabbarkeit der TMA beeinträchtigen. Die Reduzierung bzw.
