Hepatitis D virus (HDV) infection is highly prevalent in patients with chronic hepatitis B (CHB). AASLD guidelines recommend a risk-based screening approach. Our aim was to ascertain if the risk-based approach leads to appropriate HDV screening, identify targets to improve screening rates, and study HDV clinical burden. CHB patients screened for HDV from 01/2016 to 12/2021 were identified. Level of training and specialty of providers ordering HDV screening tests were determined. HDV seropositive (HDV+) patient charts were reviewed for the presence of individual risk factors per the AASLD guidelines to determine if they met screening criteria. The severity of liver disease at the time of HDV screening was compared between the HDV+ group and a matched (based on age, hepatitis B e antigen status, BMI and sex) HDV seronegative (HDV-) group. During the study period, 1444/11,190 CHB patients were screened for HDV. Most screening tests were ordered by gastroenterology (90.2%) specialists and attending physicians (80.5%). HDV+ rate was 88/1444 (6%), and 72 HDV+ patients had complete information for analysis. 18% of HDV+ patients would be missed by a risk-based screening approach due to unreported or negative risk factors (see Table). A significantly higher number of HDV+ patients had developed significant fibrosis (p = 0.001) and cirrhosis (p 0.01) by the time of screening than HDV- (n = 67) patients. In conclusion, targeted interventions are needed towards trainees and primary care clinics to improve screening rates. Current risk-based criteria do not appropriately screen for HDV. It is time for universal screening of HDV in CHB patients.

El estudio se trata de una evaluación de posturas forzadas en los en los puestos de trabajo del área administrativa del “HOSPITAL BASICO GUAMOTE”, los cuales tienen como actividad principal la coordinación, organización y abastecimiento de cada uno de los procesos a su cargo, obligándolo a permanecer gran parte de su jornada laboral en posición sentada realizando sus actividades laborales cotidianas. En el estudio se realiza una descripción general de las actividades que se realizan en cada puesto de trabajo, además de una evaluación inicial de los riesgos existentes. Para lo cual se analiza los factores de riesgo como Frecuencia de movimientos, Duración de la postura, Posturas de tronco, Posturas de cuello, Posturas de la extremidad superior, Posturas de la extremidad inferior, e identificar las posturas forzadas mediante la aplicación de check list para la identificación de posturas forzadas publicado por el Instituto de Seguridad, Salud y Bienestar en el Trabajo basado en las normas UNE-EN 1005-4:2005+A1:2009 e ISO 11226:2000. mediante el cual se pudo determinar que existen un total de 19 posturas forzadas. Posteriormente se estimó el riesgo de padecer desórdenes corporales relacionados con el trabajo por adoptar posturas forzadas como resultado de las actividades desempeñadas a lo largo de la jornada laboral mediante el método Rapid Entire Body Assessment) que ha sido desarrollado por Hignett y McAtamney. (Nottingham, 2000) obteniendo como resultado que 4 trabajadores presentan un riesgo medio alto, y 3 con riesgo alto, en el cual la necesidad de intervención es pronta y necesaria, Porponiendo medidas destinadas a prevenir la adopción de estas posturas forzadas, reduciendo o eliminando el riesgo ergonómico encontrado y la postura adoptada en los puestos de trabajo por la adopción de posturas incorrectas.

Para que los Objetivos de Desarrollo Sostenible planteados por la OMS hasta el año 2030 se cumplan, se debe resolver la problemática desde su estructura en el ámbito de la salud y el bienestar. El propósito de la presente publicación es beneficiarse de la retrospectiva para establecer adecuadas prácticas de atención médica que garanticen que ésta sea de calidad y en favor de la mejoría del paciente. La atención médica se encuentra descrita en el marco de parámetros nacionales e internacionales desarrollados por entidades de salud como: MSP, ODNA, OMS/OPS, USDA, CDC. Con un enfoque integral y holístico del paciente en edad pediátrica, enmarcado en la promoción y prevención de la salud, características de la atención primaria de la salud. La presente atención médica, busca además de ser preventiva y curativa, caracterizar la calidad de atención médica, para procurar la autoevaluación y articular la trilogía técnico-administrativo-médica, para de este modo buscar las estrategias para mejorar la calidad de la atención en salud.

This article focuses on the work and efforts put forth by the University of Houston’s Recovering the US Hispanic Literary Heritage program (Recovery) to create the first digital humanities center for US Latina/o Research: #usLdh. Recovery is a program to locate, preserve, and make available the written legacy of Latinas/os in the United States since colonial times until 1960. Through 27 years of successful work Recovery has not only been able to inscribe the excluded history of Latinas/os, but also has created an inclusive and vast digital repository that facilitates scholarship in this area of studies. This article focuses on the importance of recovery work in the writing, teaching, and understanding of history and considers how local personal archives have helped to fill in the gaps of mainstream history. We will detail the goals and challenges of this mission, as well as the importance of educating the community in digital methods that preserve and disseminate minority voices.

INTRODUCTION: Epstein- Barr virus (EBV) is a herpes virus with a majority of its primary presentations being sub-clinical It is estimated that 90%-95% of adults are positive for infection with studies suggesting that primary infection occurs by the age of 20 EBV presentation of hepatitis accounted for 10 mg/dL) Ultrasound revealed a 13 7cm heterogeneous textured liver without biliary or common ductal dilation Multiple echogenic lesions were seen, likely hemangiomas All viral panels were negative aside from the EBV panel, which was positive for IgM and IgG Patient was diagnosed with EBV hepatitis and was provided supportive treatment with NAC therapy During the patient's hospital course, his weakness and nausea persisted but pain resolved as LFTs started to trended down The total bilirubin remained elevated at discharge INR was stable throughout his stay AST/ALT had decreased to 190/195 and with improvement, he was discharged home with hepatology clinic follow up DISCUSSION: EBV hepatitis typically presents with fever, lymphadenopathy, sore throat, lymphocytosis, and splenomegaly with auto resolution over 8 weeks Our patient did not have the typical symptoms, but presented with jaundice, cholestatic liver injury with significant elevations in liver panels Treatment is supportive care Our patient completed NAC therapy and with improvement was discharged home with close with close monitoring but was unfortunately lost to follow up due to the Covid pandemic

Context: patients with chronic diseases spend between 5% and 18% of the individual or family budget for health in European countries. These expenditures could be higher in Ecuador.Objective: to carry out an analysis of the economic cost of medicines used for psychiatric pathologiesMethodology: descriptive study in 439 patients diagnosed with schizophrenia, bipolar disorder, depression and addictions, to determine the cost of treatment according to the pathology diagnosed, and the number of drugs required in their treatment.Results: 95% of subjects come from the sierra region, 2,2% come from the coast and 1,8% from the east. Female patients predominate (53%), with an average age of 36,7 ± 15,8 years. 68% are unemployed and only 20% have inadequate employment. Regarding the diagnosis, 28,6% corresponds to anxiety, 27,2% to depression,21% consume substances, 16,3% are schizophrenic and 6.7% have bipolar disorder. 80% of individuals have adherence to the treatment of one month. Most pathologies merit a treatment with two drugs, which implies an average monthly cost of 30,67 ± 24,94 dollars. Schizophrenia is the pathology that has a higher cost of treatment with a monthly value of 51,28 ± 85,04 dollars Conclusion: 80% of the patients studied require pharmacological treatment of more than 30 days; the majority of patients surveyed, 28,6%, suffer from generalized anxiety syndrome, 40% of all patients need at least two medications simultaneously, bipolar disorder is the pathology that demands the highest monthly cost, with more than US $ 112 with three medications for simultaneous use, which represents 29% of their income if you earn a unified minimum remuneration in Ecuador, to 2017. Estas cifras pueden ser mayores en Ecuador.Objetivo: realizar un análisis del costo económico de los medicamentos usados para patologías psiquiátricas.Metodología: estudio descriptivo en 439 pacientes con diagnóstico de esquizofrenia, trastorno bipolar, depresión y adicciones para determinar el costo del tratamiento según la patología diagnosticada y el número de fármacos requeridos en su tratamiento.Resultados: el 95% de sujetos provienen de la región sierra, el 2,2% provienen de la costa y el 1,8% del oriente. Predominan pacientes del sexo femenino (53%), con una edad promedio de 36,7±15,8 años. El 68% es desempleado y apenas el 20% tiene un empleo inadecuado. Respecto al diagnóstico, el 28,6% corresponde a ansiedad, 27,2% a depresión, 21% consumen sustancias, 16,3% es esquizofrénico y el 6,7% tiene trastorno bipolar. El 80% de individuos tiene adherencia al tratamiento de un mes. La mayoría de patologías ameritan un tratamiento con dos fármacos, lo que implica un costo mensual promedio 30,67±24,94 dólares. La esquizofrenia es la patología que tiene un mayor costo de tratamiento con un valor mensual de 51,28±85,04 dólaresConclusión: el 80% de los pacientes estudiados requiere tratamiento farmacológico superior a 30 días; la mayoría de pacientes encuestados, el 28,6%, sufre síndrome de ansiedad generalizada, el 40% del total de pacientes necesita al menos dos medicamentos de forma simultánea, el trastorno bipolar es la patología que demanda mayor costo mensual, con más de US$ 112 con tres medicamentos de uso simultáneo, lo que representa el 29% de sus ingresos si ganase un remuneración mínima unificada en Ecuador, al 2017
