2. Development and validation of a vero cell-based suspension method for the detection of Zika virus

Objective. To develop and validate a cell suspension method using Vero 76 cells for culturing Zika virus (ZIKV) based on infection of detached freshly seeded cells. Material and methods. Three different multiplicities of ZIKV infection were used to develop and compare this novel method to the standard confluent cell monolayer method. In addition, we preliminary validated the cell suspension method using well-characterized ZIKV positive and negative clinical samples. The standard confluent cell monolayer method was used as the reference method, and viral isolation was confirmed by a ZIKV-specific RT-PCR. We estimated the sensitivity, and its 95% confidence intervals, for the cell suspension method. Also, a technical comparison of the cell suspension method against the cell monolayer method was performed. Results. Our findings suggest that both the viral load and replication of ZIKV were comparable between both monolayer- and suspension-infection methods. Although both methods were suitable for culturing and isolating ZIKV, the cell suspension method was easier, cheaper, and quicker as well as a sensitive isolation technique. The cell suspension method was significantly more sensitive in detecting Zika in inconclusive cases by RT-PCR, with a fourfold increase compared to the confluent cell monolayer method. Conclusion. The cell suspension method has the potential to be an effective method for cultivating and isolating ZIKV and its application is potentially useful in both research and clinical settings.

Objetivo. Describir la experiencia inicial en ablación de arritmias cardiacas empleando mapeo 3D en el Instituto Nacional Cardiovascular INCOR (Lima-Perú). Materiales y métodos. Estudio descriptivo, retrospectivo, donde se recolectaron datos de historias clínicas de todos los pacientes en los cuales se realizó ablación empleando mapeo 3D, desde julio de 2017 a diciembre de 2019. Este procedimiento se realizó a pacientes sintomáticos y refractarios a terapia antiarrítmica. Resultados. Se recolectaron datos de 123 pacientes (mediana de edad: 46 años, 64,2% varones), con una mediana del tiempo de enfermedad de 6 años. Entre las arritmias tratadas 19% tuvieron fibrilación auricular; 17,5% taquicardia auricular; 17,5% arritmias ventriculares idiopáticas; 16,6% síndrome de Wolf Parkinson White/taquicardia por reentrada auriculoventricular; 11,1% arritmias ventriculares del sistema de conducción His-Purkinje; 9,5% taquicardia ventricular asociada con cicatriz; 6,4% flutter atrial y 2,4% taquicardia intranodal. La mediana del tiempo de fluoroscopía fue de 26 min. La ablación tuvo éxito agudo en el 95,9% de los casos; las complicaciones agudas se observaron en 4,8%, y la sobrevida libre de recurrencias durante el primer año de seguimiento fue de 74%. Conclusiones. Nuestra experiencia en ablación de arritmias cardiacas empleando mapeo 3D presentó una alta frecuencia de ablación exitosa aguda y baja frecuencia de complicaciones, así como una sobrevida libre de recurrencias del 74%, estimada al año de seguimiento.

Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. Esta es una investigación cualitativa de tipo exploratorio, que tiene como objetivo examinar las Representaciones sociales acerca de la adopción en parejas sin hijos y con posibilidades de adoptar, para lo cual se aplicará una entrevista semiestructurada a 50 parejas, además de una ficha de caracterización sociodemográfica elaborada para esta investigación. Se concluye que las parejas tienen una representación sesgada de la adopción, y prejuicios respecto a la edad, la salud, la historia de los niños y niñas, que determinan las resistencias y representaciones respecto a la adopción. Asimismo, las parejas anteponen el proyecto de vida personal, la estabilidad económica, dificultades en los procesos adoptivos, y supuestas preferencias por familias extranjeras, como criterios que dan forma a la resistencia a adoptar, aun cuando identifican que la adopción es un proceso necesario en la sociedad que debe ser fortalecido y asumido por más personas.

During April–June 2014 in a malaria-endemic rural community close to the city of Iquitos in Peru, we detected evidence of Guaroa virus (GROV) infection in 14 febrile persons, of whom 6 also had evidence of Plasmodium vivax malaria. Cases were discovered through a long-term febrile illness surveillance network at local participating health facilities. GROV cases were identified by using a combination of seroconversion and virus isolation, and malaria was diagnosed by thick smear and PCR. GROV mono-infections manifested as nonspecific febrile illness and were clinically indistinguishable from GROV and P. vivax co-infections. This cluster of cases highlights the potential for GROV transmission in the rural Peruvian Amazon, particularly in areas where malaria is endemic. Further study of similar areas of the Amazon may provide insights into the extent of GROV transmission in the Amazon basin.

Introducción. El presente artículo resume la guía de práctica clínica (GPC) para el manejo de pacientes con fibrilación auricular (FA) en el Seguro Social del Perú (EsSalud). Objetivo. Proveer recomendaciones clínicas basadas en evidencia para el manejo de pacientes con FA en EsSalud. Métodos. Se conformó un grupo elaborador (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 9 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y ,cuando fue considerado pertinente, estudios primarios en Medline durante el 2017. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y los flujogramas de la GPC. Finalmente, la GPC fue aprobada con Resolución N° 91 – IETSI – ESSALUD – 2018. Resultados. La presente GPC abordó 9 preguntas clínicas, divididas en tres temas: prevención de eventos tromboembólicos, estrategias de control y manejo de FA en emergencias. En base a dichas preguntas se formularon 12 recomendaciones (6 recomendaciones fuertes y 6 recomendaciones condicionales), 29 puntos de buena práctica clínica, y 2 algoritmos. Conclusión. El presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para manejo de pacientes con FA en EsSalud.

9. Evaluation of Two Serological Assays for Diagnosing Zika Virus Infection

Zika virus (ZIKV) emerged and spread rapidly in South American countries during 2015. Efforts to diagnose ZIKV infection using serological tools were challenging in dengue-endemic areas because of antigenic similarities between both viruses. Here, we assessed the performance of an in-house developed IgM antibody capture enzyme-linked immunosorbent assay (MAC-ELISA) and the plaque reduction neutralization test (PRNT) to diagnose ZIKV infection. Acute and convalescent paired serum samples from 51 patients who presented with clinical symptoms suggestive of an arbovirus illness in dengue-endemic areas of Honduras, Venezuela, Colombia and Peru were used in the assessment. Samples were tested for ZIKV, dengue and chikungunya virus using a variety of laboratory techniques. The results for the ZIKV-RNA screening and seroconversion detected by the microneutralization test were used to construct a composite reference standard. The overall sensitivity and specificity for the MAC-ELISA were 93.5% and 100.0%, respectively. Contrastingly, the overall sensitivity and specificity for the PRNT were 96.8% and 95.0%, respectively. Restricting the analysis according to IgM or neutralizing antibodies against dengue, the performances of both serological assays were adequate. The findings of this study reveal that the MAC-ELISA and PRNT would provide initial reliable laboratory diagnostic assays for ZIKV infection in dengue-endemic areas.

Despite the lack of evidence for symptomatic human infection with Maguari virus (MAGV), its close relation to Cache Valley virus (CVV), which does infect humans, remains a concern. We sequenced the complete genome of a MAGV-like isolate (OBS6657) obtained from a febrile patient in Pucallpa, Ucayali, Peru, in 1998. To facilitate its classification, we generated additional full-length sequences for the MAGV prototype strain, 3 additional MAGV-like isolates, and the closely related CVV (7 strains), Tlacotalpan (1 strain), Playas (3 strains), and Fort Sherman (1 strain) viruses. The OBS6657 isolate is similar to the MAGV prototype, whereas 2 of the other MAGV-like isolates are located on a distinct branch and most likely warrant classification as a separate virus species and 1 is, in fact, a misclassified CVV strain. Our findings provide clear evidence that MAGV can cause human disease.

ABSTRACT Mayaro virus (MAYV), Venezuelan equine encephalitis virus (VEEV), and chikungunya virus (CHIKV) are vector-borne alphaviruses that cocirculate in South America. Human infections by these viruses are frequently underdiagnosed or misdiagnosed, especially in areas with high dengue virus endemicity. Disease may progress to debilitating arthralgia (MAYV, CHIKV), encephalitis (VEEV), and death. Few standardized serological assays exist for specific human alphavirus infection detection, and antigen cross-reactivity can be problematic. Therefore, serological platforms that aid in the specific detection of multiple alphavirus infections will greatly expand disease surveillance for these emerging infections. In this study, serum samples from South American patients with PCR- and/or isolation-confirmed infections caused by MAYV, VEEV, and CHIKV were examined by using a protein microarray assembled with recombinant capsid, envelope protein 1 (E1), and E2 from nine New and Old World alphaviruses. Notably, specific antibody recognition of E1 was observed only with MAYV infections, whereas E2 was specifically targeted by antibodies from all of the alphavirus infections investigated, with evidence of cross-reactivity to E2 of o’nyong-nyong virus only in CHIKV-infected patient serum samples. Our findings suggest that alphavirus structural protein microarrays can distinguish infections caused by MAYV, VEEV, and CHIKV and that this multiplexed serological platform could be useful for high-throughput disease surveillance. IMPORTANCE Mayaro, chikungunya, and Venezuelan equine encephalitis viruses are closely related alphaviruses that are spread by mosquitos, causing diseases that produce similar influenza-like symptoms or more severe illnesses. Moreover, alphavirus infection symptoms can be similar to those of dengue or Zika disease, leading to underreporting of cases and potential misdiagnoses. New methods that can be used to detect antibody responses to multiple alphaviruses within the same assay would greatly aid disease surveillance efforts. However, possible antibody cross-reactivity between viruses can reduce the quality of laboratory results. Our results demonstrate that antibody responses to multiple alphaviruses can be specifically quantified within the same assay by using selected recombinant protein antigens and further show that Mayaro virus infections result in unique responses to viral envelope proteins.

While studying respiratory infections of unknown etiology we detected Saffold virus in an oropharyngeal swab collected from a two-year-old female suffering from diarrhea and respiratory illness. The full viral genome recovered by deep sequencing showed 98% identity to a previously described Saffold strain isolated in Japan. Phylogenetic analysis confirmed the Peruvian Saffold strain belongs to genotype 3 and is most closely related to strains that have circulated in Asia. This is the first documented case report of Saffold virus in Peru and the only complete genomic characterization of a Saffold-3 isolate from the Americas.

Our genetic analyses of uncharacterized bunyaviruses isolated in Peru identified a possible reassortant virus containing small and large gene segment sequences closely related to the Caraparu virus and a medium gene segment sequence potentially derived from an unidentified group C orthobunyavirus. Neutralization tests confirmed serologic distinction among the newly identified virus and the prototype and Caraparu strains. This virus, named Itaya, was isolated in 1999 and 2006 from febrile patients in the cities of Iquitos and Yurimaguas in Peru. The geographic distance between the 2 cases suggests that the Itaya virus could be widely distributed throughout the Amazon basin in northeastern Peru. Identification of a new Orthobunyavirus species that causes febrile disease in humans reinforces the need to expand viral disease surveillance in tropical regions of South America.

To better describe the genetic diversity of hantaviruses associated with human illness in South America, we screened blood samples from febrile patients in Chapare Province in central Bolivia during 2008–2009 for recent hantavirus infection. Hantavirus RNA was detected in 3 patients, including 1 who died. Partial RNA sequences of small and medium segments from the 3 patients were most closely related to Andes virus lineages but distinct (1 hantaviruses; the highest prevalence was among agricultural workers. Because of the high level of human exposure to hantavirus strains and the severity of resulting disease, additional studies are warranted to determine the reservoirs, ecologic range, and public health effect of this novel strain of hantavirus.

Este articulo muestra la importancia que la tradición jurídica y la organización de un sistema judicial; reclutamiento, garantías de inmovilidad, presupuesto y prescripciones salariales, pueden tener en la mayor o menor independencia del judicial. Del mismo modo, presenta cómo los estudios que sobre independencia judicial han sido realizados en Colombia han privilegiado a la Corte Constitucional y en menor medida a la Corte Suprema de Justicia. Finalmente el estudio concluye que pese a toda una serie de garantías normativas que buscan preservar la independencia de los jueces en Colombia, ésta es muy variable, y depende en últimas de la voluntad del Ejecutivo.

After a human rabies outbreak in southeastern Peru, we collected bats to estimate the prevalence of rabies in various species. Among 165 bats from 6 genera and 10 species, 10.3% were antibody positive; antibody prevalence was similar in vampire and nonvampire bats. Thus, nonvampire bats may also be a source for human rabies in Peru.

En Colombia se asiste a la explosión del número de denuncias por faltas de todo tipo en contra de magistrados de la Corte Suprema de Justicia y del Consejo Superior de la Judicatura. También, se acrecienta la sensación de que los jueces cuentan con suficientes arreglos que garantizan su independencia, sin que a la par se asegure que estos respondan por sus acciones y omisiones. El presente documento busca mostrar cómo pese a la existencia de un cuerpo normativo que busca asegurar que los altos jueces rindan cuentas, en la práctica estos mecanismos se muestran poco eficaces. La fractura al sistema de rendición de cuentas puede rastrearse en la deficiente gestión del ente que por ley controla a los togados: la Comisión de Investigación y Acusación de la Cámara de Representantes.

Since Venezuelan equine encephalitis virus (VEEV) was isolated in Peru in 1942, >70 isolates have been obtained from mosquitoes, humans, and sylvatic mammals primarily in the Amazon region. To investigate genetic relationships among the Peru VEEV isolates and between the Peru isolates and other VEEV strains, a fragment of the PE2 gene was amplified and analyzed by single-stranded conformation polymorphism. Representatives of seven genotypes underwent sequencing and phylogenetic analysis. The results identified four VEE complex lineages that cocirculate in the Amazon region: subtypes ID (Panama and Colombia/Venezuela genotypes), IIIC, and a new, proposed subtype IIID, which was isolated from a febrile human, mosquitoes, and spiny rats. Both ID lineages and the IIID subtype are associated with febrile human illness. Most of the subtype ID isolates belonged to the Panama genotype, but the Colombia/Venezuela genotype, which is phylogenetically related to epizootic strains, also continues to circulate in the Amazon basin.

Mayaro virus (MAYV), an alphavirus similar to chikungunya virus (CHIKV), causes an acute debilitating disease which results in the development of long-term arthralgia in more than 50% of infected individuals. Currently, the immune response and its role in the development of MAYV-induced persistent arthralgia remain unknown. In this study, we evaluated the immune response of individuals with confirmed MAYV infection in a one-year longitudinal study carried out in Loreto, Peru. We report that MAYV infection elicits robust immune responses that result in the development of a strong neutralizing antibody response and the secretion of pro-inflammatory immune mediators. The composition of these inflammatory mediators, in some cases, differed to those previously observed for CHIKV. Key mediators such as IL-13, IL-7 and VEGF were strongly induced following MAYV infection and were significantly increased in subjects that eventually developed persistent arthralgia. Although a strong neutralizing antibody response was observed in all subjects, it was not sufficient to prevent the long-term outcomes of MAYV infection. This study provides initial immunologic insight that may eventually contribute to prognostic tools and therapeutic treatments against this emerging pathogen.

Oropouche (ORO) virus, a member of the Simbu serogroup, is one of the few human pathogens in the Orthobunyavirus genus in the family Bunyaviridae. Genetic analyses of ORO-like strains from Iquitos, Peru, identified a novel reassortant containing the S and L segments of ORO virus and the M segment of a novel Simbu serogroup virus. This new pathogen, which we named Iquitos (IQT) virus, was first isolated during 1999 from a febrile patient in Iquitos, an Amazonian city in Peru. Subsequently, the virus was identified as the cause of outbreaks of "Oropouche fever" during 2005 and 2006 in Iquitos. In addition to the identification of 17 isolates of IQT virus between 1999 and 2006, surveys for neutralizing antibody among Iquitos residents revealed prevalence rates of 14.9% for ORO virus and 15.4% for IQT virus. Limited studies indicate that prior infection with ORO virus does not seem to protect against disease caused with the IQT virus infection. Identification of a new Orthobunyavirus human pathogen in the Amazon region of Peru highlights the need for strengthening surveillance activities and laboratory capabilities, and investigating the emergence of new pathogens in tropical regions of South America.

BACKGROUND: Arthropod-borne viruses (arboviruses) are among the most common agents of human febrile illness worldwide and the most important emerging pathogens, causing multiple notable epidemics of human disease over recent decades. Despite the public health relevance, little is know about the geographic distribution, relative impact, and risk factors for arbovirus infection in many regions of the world. Our objectives were to describe the arboviruses associated with acute undifferentiated febrile illness in participating clinics in four countries in South America and to provide detailed epidemiological analysis of arbovirus infection in Iquitos, Peru, where more extensive monitoring was conducted. METHODOLOGY/FINDINGS: A clinic-based syndromic surveillance system was implemented in 13 locations in Ecuador, Peru, Bolivia, and Paraguay. Serum samples and demographic information were collected from febrile participants reporting to local health clinics or hospitals. Acute-phase sera were tested for viral infection by immunofluorescence assay or RT-PCR, while acute- and convalescent-phase sera were tested for pathogen-specific IgM by ELISA. Between May 2000 and December 2007, 20,880 participants were included in the study, with evidence for recent arbovirus infection detected for 6,793 (32.5%). Dengue viruses (Flavivirus) were the most common arbovirus infections, totaling 26.0% of febrile episodes, with DENV-3 as the most common serotype. Alphavirus (Venezuelan equine encephalitis virus [VEEV] and Mayaro virus [MAYV]) and Orthobunyavirus (Oropouche virus [OROV], Group C viruses, and Guaroa virus) infections were both observed in approximately 3% of febrile episodes. In Iquitos, risk factors for VEEV and MAYV infection included being male and reporting to a rural (vs urban) clinic. In contrast, OROV infection was similar between sexes and type of clinic. CONCLUSIONS/SIGNIFICANCE: Our data provide a better understanding of the geographic range of arboviruses in South America and highlight the diversity of pathogens in circulation. These arboviruses are currently significant causes of human illness in endemic regions but also have potential for further expansion. Our data provide a basis for analyzing changes in their ecology and epidemiology.

Relata-se o caso de uma paciente portadora de cardiomiopatia chagásica, internada no Hospital Beneficência Portuguesa, com insuficiência cardíaca congestiva classe funcional IV e arritmia ventricular complexa. Após avaliaçao, iniciou-se o protocolo para a realizaçao de transplante cardíaco. A paciente apresentou fibrilaçao ventricular, sendo recuperada por manobras de ressuscitaçao cárdio-pulmonar. Submetida a um estudo eletrofisiológico invasivo, desencadearam-se duas taquicardias ventriculares de origens distintas, mal toleradas hemodinamicamente, sendo indicado o implante de um cardioversor-desfibrilador. Reutilizou-se por 84 dias uma prótese com 3 meses de uso, explantada de outro paciente, até a realizaçao do transplante cardíaco, o que foi fundamental para garantir a sobrevida da paciente nesse período. Verificou-se que é possível reutilizar cardioversores-desfibriladores implantáveis uma vez que o alto custo da prótese, na época nao era coberto pelo SUS, o que impossibilitava o acesso da grande maioria dos pacientes a esse tipo de terapia. Justifica-se assim a adoçao dessa conduta, principalmente nos casos em que a perspectiva de uso do aparelho é por tempo limitado.

Objetivo. Describir la calidad de vida en pacientes con arritmias ventriculares idiopáticas, tratados en el Instituto Nacional Cardiovascular INCOR de Lima-Perú. Materiales y métodos. Estudio analítico y transversal de pacientes con arritmias ventriculares idiopáticas, tratados mediante ablación 3D o terapia antiarrítmica entre julio 2017 y diciembre de 2019 a quienes se aplicó el cuestionario de salud SF-36 para evaluar la calidad de vida relacionada con la salud. Resultados. Se incluyeron a 52 pacientes con arritmias ventriculares idiopáticas (34 fueron a ablación 3D y 18 solo terapia antiarrítmica). El porcentaje de recurrencia (14,7% vs. 50%, p=0,01) y efectos adversos (0% vs. 22,2%, p=0,011) fueron menores en el grupo ablación 3D comparado con el grupo antiarrítmico. The COVID-19 pandemic has caused a severe economic crisis that led to instability in the labor market, putting at risk the welfare of workers. Some workers fell into unemployment and self-employed workers had to face a partial or total stoppage of their activities. In this framework, unemployed people could find a way out of unemployment in entrepreneurship. As for self-employed workers, they need support to reactive economically. This paper aims to study the entrepreneurial intention of unemployed people and the need of training and financial support of self-employed workers. To do so, Probit models are estimated using a national survey on workers in Ecuador. The results show that the underlying characteristics that motivate the entrepreneurial intention differ across existent unemployed people and COVID-19 unemployed people. COVID-19 unemployed women are more likely to have entrepreneurial intentions than men. Less educated existent unemployed people are more likely to have entrepreneurial intentions than more educated ones. The results regarding the need of support of self-employed people show that people that need financial support also need training support and vice-versa. The COVID-19 affectation on economic sectors has a relevant effect on the need for financial support.

Introducción: Una de las principales causas de muerte por infecciones respiratorias es la neumonía adquirida en la comunidad, esta enfermedad afecta de forma habitual a los lactantes por su débil sistema inmunológico. En la actualidad no existe estadística exacta que indique la prevalencia de esta enfermedad en Latinoamérica, por lo que este trabajo tiene la finalidad de buscar información con respecto a la problemática. Objetivos: Realizar una revisión bibliográfica sistemática sobre la prevalencia de la neumonía en pacientes pediátricos en Latinoamérica durante el periodo 2017-2022, con la finalidad de identificar los factores etiológicos inmiscuidos para el desarrollo de la neumonía, los tipos de neumonía, el cuadro clínico, la inmunización preventiva y las posibles intervenciones para el manejo de la enfermedad. Metodología: En este apartado se ha seguido el método PRISMA con el fin de encontrar artículos que cumplan con los estándares requeridos. Las bases de datos utilizadas fueron: Scopus, Web of Science o Springer; bases de datos regionales como Redalyc y Scielo. Resultados: La neumonía en pacientes pediátricos de Latinoamérica inciden como factor primordial por su prevalencia debido a que, en países como Cuba, el 37,5% de las neumonías ocasionan mayor impacto dentro de los pacientes pediátricos de una edad inferior a 5 años del sexo masculino al igual que en el Ecuador con un 53,6%, se detectó que factores asociados a esta patología se encuentran los factores etiológicos y de riesgos los cuales se encuentran inmersos por la asepsia de la ventilación, la vulnerabilidad en el sistema inmunológico, etc. Así mismo, el 81,6% de las neumonías se dan por origen viral y el 32,14% por nosocomiales, por último, la vacuna establece mejorar la inmunización y reducir la mortalidad entre un 20% a 47% de la población pediátrica en Latinoamérica. Conclusión: Es prevalente la neumonía en pacientes pediátricos de Latinoamérica en países como; Chile, Uruguay, Perú, y Ecuador. De esta manera, se ha establecido que los factores de riesgo como la desnutrición, la prematurez e infecciones, hacen vulnerables a los niños, frente a agentes como streptococcus neumoniae y enterobacter cloacae, desarrollando una clínica comúnmente basada en secreciones traqueales, hipertermia, y disnea. Finalmente, la inmunización es el parámetro preventivo de la neumonía.

Resumen Introduccion El lupus eritematoso sistemico (LES) es una enfermedad cronica que afecta a multiples sistemas, entre ellos el sistema nervioso, con eventos neurologicos y psiquiatricos que podrian influir en la adherencia al tratamiento y los resultados a largo plazo de la enfermedad. Objetivo Evaluar la adherencia al tratamiento de los pacientes con trastornos neuropsiquiatricos difusos asociados con el LES. Metodologia Se llevo a cabo un estudio descriptivo transversal que evaluo el grado de adherencia al tratamiento con el test de Morisky en pacientes con trastornos neuropsiquiatricos difusos asociados con LES (alteracion cognitiva, psicosis, trastornos del animo y ansiedad). Resultados Se incluyo a 60 pacientes con trastornos neuropsiquiatricos difusos asociados con LES, de los cuales 13 (21,6%) tuvieron adherencia alta, 9 (15%) adherencia media y 38 (63,3%) presentaron baja adherencia al tratamiento. Conclusion En este estudio se identifico que mas de la mitad de los participantes (63,3%) presentaron baja adherencia al tratamiento.

AIMS: We examined the effect of growth hormone (GH) counter-regulation on carbohydrate metabolism in individuals with life-long diminished insulin secretion (DIS).METHODS: Adults homozygous for the E180 splice site mutation of GHR [Laron syndrome (LS)], adults with a gain-of-function mutation in CDKN1c [Guevara-Rosenbloom syndrome (GRS)], and controls were evaluated for body composition, leptin, total and high molecular weight (HMW) adiponectin, insulin-like growth factor (IGF) axis molecules, and a 5-hour oral glucose tolerance test (OGTT), with measurements of glucose, insulin, glucagon, ghrelin, pancreatic polypeptide, gastric inhibitory peptide, glucagon-like peptide-1, peptide YY, and islet amyloid polypeptide (IAPP).RESULTS: Both syndromic cohorts displayed DIS during OGTT. LS subjects had higher serum concentrations of total and HMW adiponectin, and lower levels of IGF-I, IGF-II, and IGF-Binding Protein-3 than individuals in other study groups. Furthermore, they displayed normal glycemic responses during OGTT with the lowest IAPP secretion. In contrast, individuals with GRS had higher levels of protein glycation, deficient glucose control during OGTT, and increased secretion of IAPP.CONCLUSIONS: A distinct metabolic phenotype depending on GH counter-regulatory status, associates with diabetes development and excess glucose-induced IAPP secretion.

Este estudio determina los factores que afectan la productividad laboral de las empresas en Ecuador, incorporando factores externos a la empresa como las economías de aglomeración. Utilizando la información a nivel de empresas de la encuesta de ciencia, tecnología e innovación de los años 2011 y 2014, se emplea un modelo de regresión múltiple y se estima la productividad a través del método de mínimos cuadrados ordinarios. Los resultados indican que un aumento de un punto porcentual de la proporción de trabajadores con educación terciaria aumenta la productividad laboral en 1%. Si las empresas exportan o se adhieren a un grupo empresarial, su productividad laboral aumentaría en 41% y 27,7%, respectivamente. Otros factores que aumentan la productividad son el estatus multiplanta y la inversión de capital fijo. Además de los factores individuales, los factores externos como la densidad de empresas y la competencia en un sector y región dados pueden incentivar la productividad individual de las empresas.

The migration phenomenon has important implications in both the receiving and sending countries. Regarding developing countries, much less attention has been devoted to their role as receiving countries. This study focuses on Ecuador, a developing country whose migration inflows have significantly increased since its economy formally dollarized. The aim is to know how residents’ perceived life satisfaction is affected by international migration not only at an aggregated level but also at a disaggregated level (by gender, occupation, and nationality). To do so, a multiple-choice econometric model, a generalized ordinal logit, is estimated using pooled data from 2014 to 2015. Results evidence that the effect of international migration on life satisfaction is not linear: it depends on the individuals’ life satisfaction level. Moreover, results evidence that immigrants’ gender, nationality, and occupation matter when studying residents’ perceived life satisfaction. An increase in the share of male migrants is associated with low levels of life satisfaction of residents. While the increase of the share of some nationalities is associated with lower levels of life satisfaction, the increase of others is associated with higher or intermediate levels of life satisfaction. Interestingly, an increase in the share of working immigrants is not associated with low levels of life satisfaction of locals.

Utilizando un modelo de estimación de elección binaria de 2 etapas, se determinan los factores que motivan a las empresas a adoptar innovaciones previamente desarrolladas en productos y procesos. Se utilizó una muestra de 8 025 empresas ecuatorianas provenientes de la Encuesta de Actividades de Ciencia, Tecnología e Innovación, en sus ediciones 2009-2011 y 2012-2014. Los resultados concluyen que el recurso cognitivo de las empresas, las actitudes colaborativas con clientes y proveedores y los canales de comunicación que estas tienen influyen positivamente en la adopción de innovación. El tamaño de la empresa resultó tener una relación en forma de U, mientras que el nivel de exportaciones resultó no tener ningún efecto.

The Ecuadorian cohort of subjects with LS has taught us valuable lessons since the late 80's. We have learned about migration of Sephardic Jews to our country, their isolation in remote hamlets and further inbreeding. These geographical, historical and social determinants induced dissemination of a growth hormone (GH) receptor mutation which widely occurred in those almost inaccessible villages. Consequently, the world's largest Laron syndrome (LS) cohort emerged in Loja and El Oro, two of the southern provinces of Ecuador. We have been fortunate to study these patients since 1987. New clinical features derived from GH insensitivity, their growth patterns as well as treatment with exogenous insulin-like growth factor I (IGF-I) have been reported. Novel biochemical characteristics in the field of GH insensitivity, IGFs, IGF binding proteins (BP) and their clinical correlates have also been described. In the last few years, studies on the morbidity and mortality of Ecuadorian LS adults surprisingly demonstrated that despite obesity, they had lower incidence of diabetes and cancer than their relatives. These events were linked to their metabolic phenotype of elevated but ineffective GH concentrations and low circulating IGF-I and IGFBP-3. It was also noted that absent GH counter-regulation induces a decrease in insulin resistance (IR), which results in low but highly efficient insulin levels which properly handle metabolic substrates. We propose that the combination of low IGF-I signaling, decreased IR, and efficient serum insulin concentrations are reasonable explanations for the diminished incidence of diabetes and cancer in these subjects.
