Abstract Background The immature platelet fraction (IPF), a parameter obtained by the Sysmex XN‐1000V analyzer, is used in humans to differentiate between central (CEN) and peripheral (PER) thrombocytopenia (TP) but has not been evaluated in small animals. Objectives Compare IPF between healthy, clinical non‐TP and TP dogs and cats, study IPF in different causes of TP in dogs and cats and, establish IPF reference intervals (RIs), and study the effect of age and sex on IPF in healthy dogs and cats. Animals A total of 3281 dogs and 726 cats. Methods Retrospective review of medical records. Animals were classified as nonthrombocytopenic (healthy group and group of clinical patients without TP [NTP]) or TP. These latter animals were subclassified as pseudothrombocytopenia (PSE), CEN and PER, based on evaluation of platelet clumps, estimated platelet count in blood smears and final diagnosis. Blood samples were evaluated using a Sysmex XN‐1000V with a specific platelet channel (PLT‐F). Results The IPF was significantly different between each subtype of TP in both species. Universities are often described as institutions that can promote the wellbeing of their local populations. This is because they are central for advancing human development aims which support the aspirations of students and the communities from which they come. Nevertheless, we know this potential can be constrained by historical processes of oppression and negation of indigenous ways of being and doing. Applying the Capabilities Approach and Human Development paradigm as a normative framework for the outcomes of university education in the South African context, we argue for a focus on the centrality of capabilities (real freedoms) in assessing how well universities are doing to support student wellbeing. We pay special attention to one capability which we see as architectonic for other freedoms, which is Ubuntu. While Ubuntu is generally understood as a moral philosophy, in this paper we articulate it as a valued capability in the space of higher education. We also argue that it is a capability that has transformative and decolonial potential that can enable universities to promote student wellbeing if the conditions to practice it are in place. Drawing from data collected through qualitative and participatory approaches in two longitudinal research projects that were carried out between 2016 and 2021 with undergraduate students in diverse universities in South Africa, we show that Ubuntu informs students’ conceptions of humanity and their aspirations for ‘a good life’ and that it can inform the vision of transformed and decolonised university spaces that reflect indigenous ways of being and indigenous ways of seeing the world.

IntroductionExtracorporal Photophoresis (ECP) is in clinical use for steroid-refractory and steroid-dependent acute GVHD (SR-aGVHD). Based on recent Phase-III study results, ruxolitinib has become the new standard of care for SR-aGVHD. Our aim was to collect comparative data between ruxolitinib and ECP in SR-aGVHD in order to improve the evidence base for clinical decision making. MethodsWe asked EBMT centers if they were willing to participate in this study by completing a data form (Med-C) with detailed information on GVHD grading, -therapy, -dosing, -response and complications for each included patient.Results31 centers responded positively (14%) and we included all patients receiving alloSCT between 1/2017-7/2019 and treated with ECP or ruxolitinib for SR-aGVHD grades II-IV from these centers. We identified 53 and 40 patients with grades II-IV SR-aGVHD who were treated with ECP and ruxolitinib, respectively. We performed multivariate analyses adjusted on grading and type of SR-aGVHD (steroid dependent vs. refractory). At day+90 after initiation of treatment for SR-aGVHD we found no statistically significant differences in overall response. The odds ratio in the ruxolitinib group to achieve overall response vs. the ECP group was 1.13 (95% CI = [0.41; 3.22], p = 0.81). In line, we detected no statistically significant differences in overall survival, progression-free survival, non-relapse mortality and relapse incidence.DiscussionThe clinical significance is limited by the retrospective study design and the current data can’t replace prospective studies on ECP in SR-aGVHD. However, the present results contribute to the accumulating evidence on ECP as an effective treatment option in SR-aGVHD.

Climate-sensitive hazards, including extreme heat, wildfire smoke, flooding, and rising sea levels, can jeopardize the health of all populations. However, certain population groups are more vulnerable to harm from these hazards. While youth–particularly those from historically marginalized groups and communities–are among those at the highest risk, their abilities may be underutilized in local climate adaptation efforts. This exploratory research aimed to identify opportunities for youth involvement in climate and environmental solutions in their historically marginalized communities. Specifically, this study (1) investigated youth risk perceptions and impacts of various climate-sensitive hazards (e.g. extreme heat, wildfire smoke, flooding, and sea level rise); (2) examined current barriers, resource needs, and opportunities for youth to engage in climate and environmental solution work; and (3) explored current methods and spaces where youth and community leaders can support youth-led climate-related initiatives. Through focus group discussions with youth and in-depth interviews with community leaders who are directly with youth, this study identified opportunities to leverage youth experiences, perceptions, and assets to promote a healthy and resilient community in the face of various compounding climate-related threats. Results showed that while youth recognize the changing climate and associated health impacts, they require more financial resources and support from local decision-makers to maintain their engagement and promote community resilience. Engaging youth in climate action and community resilience involves more than just centering youth voices and perspectives–it requires intentional collaboration, capacity-building, organizing, granting decision-making power, and other strategies to produce inclusive, intersectional, and sustainable solutions.

The Sysmex XN-1000V analyzer can identify those reticulocytes with high RNA content and fluorescence, providing the immature reticulocyte fraction (IRF). While this parameter has been used in human medicine to identify the cause of anemia, few studies have focused on its use in veterinary medicine. In this study, we determined the IRF and related reticulocyte parameters in a large population of non-anemic and anemic dogs and cats (subclassified depending on the origin of their anemia). The IRF was significantly higher in hemolytic anemias compared to hemorrhagic ones in both species. Moreover, the IRF was significantly lower in dogs and cats with bone marrow failure than in other non-regenerative anemias and in both groups compared to pre-regenerative anemias. The accurate cut-off values for the differential in regenerative anemias and reference ranges for both species using the Sysmex XN-1000V are also reported. The measurement of the IRF in this analyzer can help clinicians to further classify the type of anemia in both species.

BackgroundAlthough adherence to the Mediterranean and antioxidant-rich diets during pregnancy is suggested to improve maternal-fetal health by reducing oxidative stress, yet there is no study available.ObjectiveWe examined whether maternal dietary patterns in pregnancy impact the biomarkers of oxidative stress in mothers and their offspring.MethodsStudy population included 642 mothers and 335 newborns of the “Nutrition in Early Life and Asthma” (NELA) birth cohort. Maternal diet during pregnancy was assessed by a validated food frequency questionnaire and a priori-defined dietary indices (relative Mediterranean Diet [rMED], alternative Mediterranean Diet [aMED], Dietary Approach to Stop Hypertension [DASH], Alternate Healthy Index [AHEI], and AHEI-2010) were calculated. Biomarkers measured were: hydroperoxides, carbonyl groups, and 8-hydroxydeoxyguanosine (8OHdG) determined in maternal blood and newborn cord blood, and urinary maternal and offspring 15-F2t-isoprostane. Multivariate linear regression models were performed.ResultsMaternal rMED score was inversely associated with the maternal levels of 8OHdG at mid-pregnancy (beta per 1-point increase = −1.61; 95% CI −2.82, −0.39) and the newborn levels of hydroperoxides (beta per 1-point increase = −4.54; 95% CI −9.32, 0.25). High vs. low maternal rMED score was marginally associated with the decreased levels of 8OHdG in newborns (beta = −9.17; 95% CI −19.9, 1.63; p for trend 0.079). Maternal DASH score tended to be inversely associated with maternal urinary 15-F2t-isoprostane (beta per 1-point increase = −0.69; 95% CI, −1.44, 0.06). High vs. low maternal AHEI score was associated with reduced offspring urinary levels of 15-F2t-isoprostane (beta = −20.2; 95% CI −38.0, −2.46; p for trend 0.026).ConclusionThese results suggest that maternal adherence to healthy dietary patterns during pregnancy may reduce DNA damage and lipid oxidation in mothers and offspring.

Summary: The COVID-19 pandemic is having a major impact on the mental health of populations in the Americas. Studies show high rates of depression and anxiety, among other psychological symptoms, particularly among women, young people, those with pre-existing mental health conditions, health workers, and persons living in vulnerable conditions. Mental health systems and services have also been severely disrupted. A lack of financial and human resource investments in mental health services, limited implementation of the decentralized community-based care approach and policies to address the mental health gap prior to the pandemic, have all contributed to the current crisis. Countries must urgently strengthen their mental health responses to COVID-19 by taking actions to scale up mental health and psychosocial support services for all, reach marginalized and at-risk populations, and build back better mental health systems and services for the future.Editorial Disclaimer: This translation in Spanish was submitted by the authors and we reproduce it as supplied. Objective: We assessed the performance characteristics of the Panbio COVID-19 Rapid Antigen Test Device in field conditions and its potential use as a point-of-care test for isolation and cohorting patients at risk. Methods: We reviewed our laboratory records from January 2021 to May 2021 to identify subjects with a Panbio COVID-19 Rapid Antigen Test Device (index test) and an reverse transcriptase-polymerase chain reaction (RT-PCR) (reference test) test carried out on the same date. Results: In total, 634 subjects met inclusion criteria. Three (0.2%) samples of the index test were not interpretable. A total of 51 (8.08%) subjects had SARS-CoV-2 infection. The index test had sensitivity 78.85% (95% CI: 66.30-88.94%), specificity 91.58% (95% CI 89.02-93.71), and accuracy 90.54% (95% CI 87.99-92.70%). A total of 11 (1.73%) subjects (one asymptomatic, five with respiratory symptoms, and five with pneumonia) had a false-negative result of the index test with median (range) cycle threshold RT-PCR of 25 (21-30), indicating the presence of high viral load in the nasopharyngeal sample. Conclusions: The Panbio COVID-19 Rapid Antigen Test Device cannot replace SARS-CoV-2 RT-PCR for infection control purposes.

(1) Background: Large cohort studies of patients with COVID-19 treated with remdesivir have reported improved clinical outcomes, but data on older patients are scarce. Objective: This work aims to assess the potential benefit of remdesivir in unvaccinated very old patients hospitalized with COVID-19; (2) Methods: This is a retrospective analysis of patients ≥ 80 years hospitalized in Spain between 15 July and 31 December 2020 (SEMI-COVID-19 Registry). Differences in 30-day all-cause mortality were adjusted using a multivariable regression analysis. (3) Results: Of the 4331 patients admitted, 1312 (30.3%) were ≥80 years. Very old patients treated with remdesivir (n: 140, 10.7%) had a lower mortality rate than those not treated with remdesivir (OR (95% CI): 0.45 (0.29–0.69)). After multivariable adjustment by age, sex, and variables associated with lower mortality (place of COVID-19 acquisition; degree of dependence; comorbidities; dementia; duration of symptoms; admission qSOFA; chest X-ray; D-dimer; and treatment with corticosteroids, tocilizumab, beta-lactams, macrolides, and high-flow nasal canula oxygen), the use of remdesivir remained associated with a lower 30-day all-cause mortality rate (adjusted OR (95% CI): 0.40 (0.22–0.61) (p < 0.001)). (4) Conclusions: Remdesivir may reduce mortality in very old patients hospitalized with COVID-19.

Sclerodermoid chronic graft-versus-host disease (scGVHD) is a severe complication of allogeneic haema­- topoietic stem cell transplantation. The aim of this study was to investigate the usefulness of high-frequency ultrasound of the skin in assessing the inflammatory patterns and prognosis of patients with scGVHD. A prospective study was carried out with patients who developed scGVHD in the period June 2016 to April 2018. Clinical and ultrasound examinations were performed on the first visit and at 6-month follow-up. A total of 24 patients were included in the study. A 6-month follow-up high-frequency ultrasound of the skin was performed on 20 of the 24 patients. Abnormal B-mode findings in high-frequency ultrasound of the skin consisted of hypoechogenic dermis, hypoechogenicity of septa and hyperechogenicity of lobules in hypodermis. No differences were observed in these basal parameters between treatment progressive/non-responding and inactive/responding scGVHD groups of patients. Basal Doppler showing increased vascular flow with a systolic peak ≥10 cm/s and a vascular resistance index ≥ 0.70 was observed only in those patients who developed progressive/non-responding scGVHD (62.5% vs 0% p = 0.006). In conclusion, Doppler ultrasound is a useful tool to assess the inflammatory activity and outcome of scGVHD. These findings could enhance patient management and help to guide treatment decisions.

BackgroundCampylobacter jejuni is the most common bacterial cause of human infectious intestinal disease.MethodsWe genome sequenced 601 human C. jejuni isolates, obtained from two large prospective studies of infectious intestinal disease (IID1 [isolates from 1993-1996; n = 293] and IID2 [isolates from 2008-2009; n = 93]), the INTEGRATE project [isolates from 2016-2017; n = 52] and the ENIGMA project [isolates from 2017; n = 163].ResultsThere was a significant increase in the prevalence of the T86I mutation conferring resistance to fluoroquinolone between each of the three later studies (IID2, INTEGRATE and ENIGMA) and IID1. Although the distribution of major multilocus sequence types (STs) was similar between the studies, there were changes in both the abundance of minority STs associated with the T86I mutation, and the abundance of clones within single STs associated with the T86I mutation.DiscussionFour population-based studies of community diarrhoea over a 25 year period revealed an increase over time in the prevalence of the T86I amongst isolates of C. jejuni associated with human gastrointestinal disease in the UK. Although associated with many STs, much of the increase is due to the expansion of clones associated with the resistance mutation.

In the discussion of the findings, we explain how ubuntu underpins the ways students tend to relate to each other -- as interdependent partners of a learning community, rather than as independent individuals who happen to be in the same learning environment. Building on these descriptions of deeply relational ways of being at university, ways that embrace an African indigenous worldview, we argue that creating the conditions for students to achieve the capability of ubuntu has decolonial potential.

La medida del rendimiento de cualquier unidad de decisión es un objetivo ciertamente deseable para conocer si la aplicación de sus recursos económicos (inputs) es adecuada en cada momento para conseguir su objetivo productivo. En este sentido, las entidades sin fines lucrativos no son ajenas a esta necesidad de gestión, presentando dificultades añadidas la medida de su desempeño, dado que por su carácter de «no lucrativas» su objetivo último no es la maximización de beneficio, sino la consecución de fines de carácter social, en ocasiones difícilmente cuantificables.En el contexto descrito, el presente trabajo evalúa, dentro del sector de las entidades sin fines lucrativos, el rendimiento de las fundaciones estatales de tipo asistencial radicadas en España, mediante la aplicación de la técnica no paramétrica del análisis envolvente de datos (DEA), capaz de determinar la frontera de «buenas prácticas» de las unidades evaluadas a partir de un modelo de optimización matemática, al tiempo que permite estudiar las variaciones del cambio productivo a través del cálculo del índice de productividad total de los factores de Malmquist basado en la metodología DEA mencionada.El estudio se complementa con un análisis DEA en 2 etapas aplicando un modelo de regresión logit para evaluar la posible asociación de covariables de relevancia en el sector de las fundaciones, tales como su tamaño, liquidez, endeudamiento y antigüedad, con el rendimiento de dichas organizaciones previamente calculado.

Objectives: A decrease in blood cell counts, especially lymphocytes and eosinophils, has been described in patients with serious Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), but there is no knowledge of their potential role of the recovery in these patients’ prognosis. This article aims to analyse the effect of blood cell depletion and blood cell recovery on mortality due to COVID-19. Design: This work was a retrospective, multicentre cohort study of 9644 hospitalised patients with confirmed COVID-19 from the Spanish Society of Internal Medicine’s SEMI-COVID-19 Registry. Setting: This study examined patients hospitalised in 147 hospitals throughout Spain. Participants: This work analysed 9644 patients (57.12% male) out of a cohort of 12,826 patients ≥18 years of age hospitalised with COVID-19 in Spain included in the SEMI-COVID-19 Registry as of 29 May 2020. Main outcome measures: The main outcome measure of this work is the effect of blood cell depletion and blood cell recovery on mortality due to COVID-19. Univariate analysis was performed to determine possible predictors of death, and then multivariate analysis was carried out to control for potential confounders. Results: An increase in the eosinophil count on the seventh day of hospitalisation was associated with a better prognosis, including lower mortality rates (5.2% vs. 22.6% in non-recoverers, OR 0.234; 95% CI, 0.154 to 0.354) and lower complication rates, especially regarding the development of acute respiratory distress syndrome (8% vs. 20.1%, p = 0.000) and ICU admission (5.4% vs. 10.8%, p = 0.000). Lymphocyte recovery was found to have no effect on prognosis. Treatment with inhaled or systemic glucocorticoids was not found to be a confounding factor. Conclusion: Eosinophil recovery in patients with COVID-19 who required hospitalisation had an independent prognostic value for all-cause mortality and a milder course.

Este artículo analiza la relación entre los conocimientos indígenas y los occidentales en el sistema de educación intercultural bilingüe (EIB) en el Ecuador. El texto muestra que la tensión entre ambos tipos de saberes refleja diferencias en la forma en la que entienden el papel de la educación formal las bases y los líderes del movimiento indígena. El artículo también argumenta que los aliados de los movimientos indígenas (los grupos religiosos, los académicos, las ONG y otros) y las comunidades tienen distintos objetivos educativos. Mientras que los aliados y los líderes ven la EIB como un espacio para preservar la lengua y la cultura indígenas, los padres de familia y los estudiantes aspiran a que el sistema educativo formal les de acceso a conocimientos occidentales como la lecto-escritura en castellano, el inglés y la computación. Sin embargo, las comunidades no desprecian los saberes indígenas basados en la narrativa oral y la sabiduría de los ancianos, sino que no consideran que el espacio para la reproducción de estos saberes sea la educación formal. Estos conocimientos tienden a reproducirse fuera de las instituciones educativas en los ámbitos de la familia y la comunidad. Los datos de este estudio se recogieron de forma colaborativa por la autora y un grupo de investigadores indígenas. Debido a esto, el texto aporta puntos de vista internos y críticos que complementan los de otros estudios.

We compare two approaches given to the parallelogram law as a fundamental notion in eighteenth century mechanics. The authors we study are Kant and d’Alembert and we use the context created by Newton’s Principia as our point of departure.

The high-affinity IgE receptor (Fcε RI) is a heterotetramer of three subunits: Fcε RIα, Fcε RIβ and Fcε RIγ (αβγ2) encoded by three genes designated as FCER1A, FCER1B (MS4A2) and FCER1G, respectively. Recent evidence points to FCERI gene variability as a relevant factor in the risk of developing allergic diseases. Because Fcε RI plays a key role in the events downstream of the triggering factors in immunological response, we hypothesized that FCERI gene variants might be related with the risk of, or with the clinical response to, selective (IgE mediated) non-steroidal anti-inflammatory (NSAID) hypersensitivity.From a cohort of 314 patients suffering from selective hypersensitivity to metamizole, ibuprofen, diclofenac, paracetamol, acetylsalicylic acid (ASA), propifenazone, naproxen, ketoprofen, dexketoprofen, etofenamate, aceclofenac, etoricoxib, dexibuprofen, indomethacin, oxyphenylbutazone or piroxicam, and 585 unrelated healthy controls that tolerated these NSAIDs, we analyzed the putative effects of the FCERI SNPs FCER1A rs2494262, rs2427837 and rs2251746; FCER1B rs1441586, rs569108 and rs512555; FCER1G rs11587213, rs2070901 and rs11421. Furthermore, in order to identify additional genetic markers which might be associated with the risk of developing selective NSAID hypersensitivity, or which may modify the putative association of FCERI gene variations with risk, we analyzed polymorphisms known to affect histamine synthesis or metabolism, such as rs17740607, rs2073440, rs1801105, rs2052129, rs10156191, rs1049742 and rs1049793 in the HDC, HNMT and DAO genes.No major genetic associations with risk or with clinical presentation, and no gene-gene interactions, or gene-phenotype interactions (including age, gender, IgE concentration, antecedents of atopy, culprit drug or clinical presentation) were identified in patients. Purpose: The following investigation aims to highlight strengths and weaknesses of the computer science degree from the part of students.Design/methodology/approach: This paper presents the results of a quantitative investigation that sheds light on student’s motivations to pursue or reject computer science.Findings: The study identified the reasons that cause this rejection and suggest them as opportunities to enhance the domain in the future.Practical implications: This study is a preliminary phase of a larger research goal which aims to identify the strategies presented to computer science education using technology to foster interest in the domain among students and professionals in Europe.Originality/value: The declining number of computer science students in Europe is producing negative consequences in the technology field that affect other sectors like the economic and educational ones. In order to analyze this situation and in view of providing solutions to stop the decline, it is primordial to understand the reasons that retain students to choose this domain of study or that attract and retain them in the field.

Different types of graft-versus-host disease prophylaxis have been proposed in the setting of reduced intensity and non-myeloablative allogeneic stem cell transplantation. An alternative combination with sirolimus and tacrolimus has recently been tested although comparative studies against the classical combination of a calcineurin inhibitor and mycophenolate mofetil or methotrexate are lacking. We describe the results of a prospective, multicenter trial using sirolimus + tacrolimus as immunoprophylaxis, and compare this approach with our previous experience using cyclosporine + mycophenolate in the setting of unrelated donor transplantation setting after reduced-intensity conditioning. Forty-five patients received cyclosporine + mycophenolate between 2002 and mid-2007, while the subsequent 50 patients, who were transplanted from late 2007, were given sirolimus + tacrolimus. No significant differences were observed in terms of hematopoietic recovery or acute graft-versus-host disease overall, although gastrointestinal acute graft-versus-host disease grade ≥2 was more common in the cyclosporine + mycophenolate group (55% versus 21%, respectively, P=0.003). The 1-year cumulative incidence of chronic graft-versus-host disease was 50% versus 90% for the patients treated with the sirolimus- versus cyclosporine-based regimen, respectively (P

Several applications to represent classical or fuzzy data in databases have been developed in the last two decades. However, these representations present some limitations specially related with the system portability and complexity. Ontologies provides a mechanism to represent data in an implementation-independent and web-accessible way. To get advantage of this, in this paper, an ontology, that represents fuzzy relational database model, has been redefined to communicate users or applications with fuzzy data stored in fuzzy databases. The communication channel established between the ontology and any Relational Database Management System (RDBMS) is analysed in depth throughout the text to justify some of the advantages of the system: expressiveness, portability and platform heterogeneity. Moreover, some tools have been developed to define and manage fuzzy and classical data in relational databases using this ontology. Even an application that performs fuzzy queries using the same technology is included in this proposal together with some examples using real databases.

This trial evaluated the feasibility and efficacy of the infusion of mesenchymal stem cells expanded using human serum for the treatment of refractory acute or chronic graft-versus-host disease. Twenty-eight expansions were started. In 22, a minimum of more than 1x106 mesenchymal stem cells/kg were obtained after a median of 26 days; this threshold was not obtained in the remaining cases. Ten patients received cells for the treatment of refractory or relapsed acute graft-versus-host disease and 8 for chronic disease. One patient treated for acute graft-versus-host disease obtained a complete response, 6 had a partial response and 3 did not respond. One of the chronic patients achieved complete remision, 3 a partial response, and 4 did not respond. The current study supports the use of this approach in less heavily treated patients for both acute and chronic graft-versus-host disease. Lichen sclerosus (LS) is a chronic inflammatory mucocutaneous disease that often affects the anogenital area; oral mucosal lesions are extremely rare. A 52-year-old woman presented for evaluation of an 8-year history of a persistent whitish plaque in the buccal mucosa. Intraoral examination revealed multiple elevated whitish plaques diffusely distributed in the buccal mucosa associated with an area of tissue atrophy. Although both leukoplakia and lichen planus were considered, incisional biopsy and later, full excision confirmed the histopathological diagnosis of oral LS. After 6 months of follow-up, there are no clinical signs of relapse. This case highlights the importance of clinical and histopathological findings for the correct diagnosis and treatment of oral LS.
