Abstract

OBJECTIVES: In agreement with other autoimmune diseases, systemic sclerosis (SSc) is associated with a strong sex bias. However, unlike lupus, the effects of sex on disease phenotype and prognosis are poorly known. Therefore, we aimed to determine sex effects on outcomes. METHOD: We performed a prospective observational study using the latest 2013 data extract from the EULAR scleroderma trials and research (EUSTAR) cohort. We looked at (i) sex influence on disease characteristics at baseline and (ii) then focused on patients with at least 2 years of follow-up to estimate the effects of sex on disease progression and survival. RESULTS: 9182 patients with SSc were available (1321 men) for the baseline analyses. Abstract

It is a misconception to think of or refer to rheumatoid arthritis simply as “arthritis”. This is a mistake commonly made by laypersons who when asked if they suffer from any condition, they answer by saying “I suffer from arthritis” without specifying which type of arthritis. English literature also tends to reflect the same mistake, and we often come across narrative references to walking sticks implying the fictional character has “arthritis”. English literature also tends to synonymously associate arthritis with old age. However, this is not always the case. The first thing to note about rheumatoid arthritis is that there are different types of arthritis and not all are strictly associated with old age. Certain types of arthritis can occur in younger patients, even in children.

Abstract

Background: There is some impairment of the ability of patients with Alzheimer’s disease (AD) to perceive emotions, but this ability seems relatively preserved compared to the impairment of other cognitive domains. Few studies have focused on the link between emotional processing and other cognitive functions, such as memory or attention. Objective: This study was designed to investigate whether the emotional content of a text can influence memory in patients affected by AD and whether this effect is related to attentional processes as measured by event-related potentials (ERP). Methods: All subjects were administered neuropsychological tests and a logical memory test including emotional and nonemotional material. ERP were recorded during an attention task. Results: AD patients had better immediate recall of sad and, to a lesser extent, happy stories than of neutral stories. This difference also affected multiple choice recognition and identification (immediate and delayed) of emotional content. The amplitude of both P300 and mismatch negativity was significantly decreased in the group as a whole. There was no correlation between P300 parameters and performance on the memory tasks, whatever their emotional content. Conclusions: The results show relatively preserved emotional processing in patients with AD and suggest that the emotional content of a context can influence memory performance. We found no evidence that this effect is mediated by attention as measured by ERP.

Abstract

Giant cell arteritis, a systemic vasculitis that preferentially targets large and medium-sized arteries, can produce a wide variety of clinical features which may be classified into 2 groups: A) Systemic: malaise, fatigue, anorexia, weight loss, fever, sweats, arthralgias and, when associated with polymyalgia rheumatica, myalgias and stiffness of the neck, shoulders low back, hips and thighs. B) Arteritic: features resulting from ischaemia or necrosis of structures supplied by the involved blood vessels. Several different arteries can be involved with a corresponding wide range of clinical features. The most frequently affected arteries are the branches of the carotid artery with headache being the most common symptom (70–90%) [1] due to involvement of the temporal artery, whilst the most dreaded feature is blindness due to involvement of the ophthalmic artery (6–36%) [2]. Raynaud’s phenomenon as a result of subclavian artery involvement is rare [3]. One such case is described.

Abstract

In patients with rheumatoid arthritis, significant positive correlations were found between urinary pyridinium crosslinks and C-reactive protein (CRJP), erythrocyte sedimentation rate (ESR) and articular index. Also an inverse correlation was observed between pyridinium crosslinks excretion and grip strength. Glucocorticoid therapy, equivalent to daily doses of 7.5mg of prednisolone or less, did not appear to have deleterious effect on bone metabolism in these patients as measured by urinary pyridinium crosslinks.

Abstract

We present the results of a preliminary survey carried out on 46 patients with Paget’s disease of bone in Malta, 40 of whom were seen at a general medical outpatient clinic and a further 6 at a primary health care centre over the first 6 months of 1997. Various aspects of the disease have been analysed: prevalence, age and sex distribution, familial aggregates, mode of presentation, complications, pattern of bone involvement, a semi-quantitative assay of disease activity as assessed by bone scintigraphy and serum alkaline phosphatase (SAP) levels, and the factors that influenced disease activity. The results have been compared to previous published surveys on Paget’s disease in different countries.

Abstract

Toxic epidermal necrolysis (TEN) is an uncommon condition (Figure 1) with an estimated prevalence of 1.2/million each year. This life-threatening condition is frequently drug-induced (Figure 2) and is characterised by epidermal erythema and inflammation, superficial necrosis, and skin erosions resulting in the skin being shed in large sheets [1]. Infection and metabolic failure are invariable consequences and mortality can approach 70% with an average of 30% in reported cases [2].

Although the division of medicine into specialties according to different systems is convenient, it is also artificial: the different systems interact and many diseases overlap both in their pathological features as well as their clinical expression. Many examples of such interactions are seen in the connective tissue disorders, where rheumatological and dermatological manifestations may be prominent features. In some of them the skin rash may be a diagnostic marker (e.g., systemic lupus erythematosus, dermatomyositis). Joint involvement can also be found in “primary” skin disorders such as psoriasis; certain infections can produce both skin and joint manifestations including a number of fairly common viral disorders as well as Lyme borelliosis and the acquired immune deficiency syndrome (A.I.D.S.) The skin may also be the major target of toxicity from a number of drugs, particularly those that are used in the management of rheumatic disorders.
