The selective cardiac myosin activator omecamtiv mecarbil has been shown to improve cardiac function in patients with heart failure with a reduced ejection fraction. Its effect on cardiovascular outcomes is unknown. METHODS. We randomly assigned 8256 patients (inpatients and outpatients) with symptomatic chronic heart failure and an ejection fraction of 35% or less to receive omecamtiv mecarbil (using pharmacokinetic-guided doses of 25 mg, 37.5 mg, or 50 mg twice daily) or placebo, in addition to standard heart-failure therapy. The primary outcome was a composite of a first heart-failure event (hospitalization or urgent visit for heart failure) or death from cardiovascular causes. RESULTS. During a median of 21.8 months, a primary-outcome event occurred in 1523 of 4120 patients (37.0%) in the omecamtiv mecarbil group and in 1607 of 4112 patients (39.1%) in the placebo group (hazard ratio, 0.92; 95% confidence interval [CI], 0.86 to 0.99; P=0.03). A total of 808 patients (19.6%) and 798 patients (19.4%), respectively, died from cardiovascular causes (hazard ratio, 1.01; 95% CI, 0.92 to 1.11). There was no significant difference between groups in the change from baseline on the Kansas City Cardiomyopathy Questionnaire total symptom score. At week 24, the change from baseline for the median N-terminal pro–B-type natriuretic peptide level was 10% lower in the omecamtiv mecarbil group than in the placebo group; the median cardiac troponin I level was 4 ng per liter higher. The frequency of cardiac ischemic and ventricular arrhythmia events was similar in the two groups. CONCLUSIONS. Among patients with heart failure and a reduced ejection, those who received omecamtiv mecarbil had a lower incidence of a composite of a heart-failure event or death from cardiovascular causes than those who received placebo. B-type natriuretic peptide (BNP) and echocardiography are potentially useful adjunct to guide management of patients with chronic heart failure (HF).Thus, the aim of this retrospective, multicenter study was to compare outcomes and renal function in outpatients with chronic HF with reduced ejection fraction (HFrEF) who underwent an echo and BNP guided or a clinically driven protocol for follow-up.In 1137 consecutive outpatients, management was guided according to echo-Doppler signs of elevated left ventricular filling pressure and BNP levels conforming to the protocol of the Network Labs Ultrasound (NEBULA) in HF Study Group in 570 (mean EF=30%), while management was clinically driven based on the institutional protocol of the HF Unit of the Cardiovascular and Thoracic Department in 567 (mean EF=33%). Propensity score, matching several confounding baseline variables, was used to match pairs based on treatment strategy. The median follow-up was 37.4months. After propensity matching, a lower incidence of death (HR 0.45, 95%CI: 0.30-0.67, p0.0001), and death or worsening renal function (HR 0.49, 95%CI 0.36-0.67, p0.0001) was apparent in echo-BNP-guided group compared to clinically-guided group. Worsening of renal function (≥0.3mg/dl increase in serum creatinine) was observed in 9.8% of echo-BNP-guided group and in 21.4% of clinical assessed group (p0.0001). The daily dose of loop diuretics did not change in echo-BNP-guided group, while it increased in 65% of patients in clinically-guided group (p0.0001).Echo and BNP guided management may improve the outcome and reduce worsening of renal function in outpatients with chronic HFrEF.

Introduzione: Nel corso della stagione influenzale, gli Operatori Sanitari (OS) rappresentano una delle categorie lavorative maggiormente investite dalla responsabilità di limitare la diffusione della patologia influenzale tra i pazienti ospedalizzati e contemporaneamente di proteggere se stessi garantendo la continuità dell'assistenza sanitaria nei periodi di maggiore assenteismo lavorativo e sovraccarico delle attività assistenziali (picco influenzale stagionale). La vaccinazione antinfluenzale, ormai riconosciuta sicura ed efficace, rappresenta ancora oggi una argomento di dibattito tra gli OS, nonostante le raccomandazioni dei Center for Disease Control and Prevention Statunitense ed Europeo a mantenere le coperture tra gli OS sopra il 75%. Numerosi studi confermano come i tassi di vaccinazione antinfluenzale rimangano generalmente bassi sia tra gli OS che in particolare tra i medici in formazione specialistica (MFS). I risultati del presente studio multicentrico cercano di indagare le attitudini e i determinanti associati con la scelta vaccinale antinfluenzale tra i MFS italiani per comprendere le cause e proporre delle soluzioni. Materiali e Metodi: Nel 2012 è stata condotta una survey tra i MFS iscritti a 18 Università Italiane (Bari, Bologna, Brescia, Catania, Catanzaro, Chieti, L'Aquila, Messina, Modena, Napoli Federico II, Palermo, Pavia, Parma, Roma Cattolica, Roma Tor Vergata, Siena, Torino, Verona). Ciascun partecipante è stato contattato tramite una mail in cui erano illustrate le motivazioni dello studio e veniva chiesto di rispondere ad un breve questionario online, anonimo ed autosomministrato che includeva domande sulle attitudini riguardanti la vaccinazione antinfluenzale. Il questionario è stato precedentemente elaborato in collaborazione con il gruppo di lavoro nazionale e successivamente testato in uno studio pilota condotto nell'Università capofila dello studio (Palermo). Results: 2.506 MFS su un totale di 10.854 hanno aderito allo studio (23.1%). L'adesione alla vaccinazione antinfluenzale per la stagione 2011/2012 è stata pari al 11,9%, dato in chiara diminuzione rispetto al 2010/2011 (21.,7%) ed al 2009/2010 (15.5%). The interpretation of the heart as a mechanical engine dates back to the teachings of Leonardo da Vinci, who was the first to apply the laws of mechanics to the function of the heart. Similar to any mechanical engine, whose performance is proportional to the power generated with respect to weight, the left ventricle can be viewed as a power generator whose performance can be related to left ventricular mass. Stress echocardiography may provide valuable information on the relationship between cardiac performance and recruited left ventricular mass that may be used in distinguishing between adaptive and maladaptive left ventricular remodeling. Peak power output-to-mass, obtained during exercise or pharmacological stress echocardiography, is a measure that reflects the number of watts that are developed by 100 g of left ventricular mass under maximal stimulation. Power output-to-mass may be calculated as left ventricular power output per 100 g of left ventricular mass: 100× left ventricular power output divided by left ventricular mass (W/100 g). A simplified formula to calculate power output-to-mass is as follows: 0.222 × cardiac output (l/min) × mean blood pressure (mmHg)/left ventricular mass (g). When the integrity of myocardial structure is compromised, a mismatch becomes apparent between maximal cardiac power output and left ventricular mass; when this occurs, a reduction of the peak power output-to-mass index is observed.

Il 2007 passerà probabilmente alla storia come l’annus horribilis dell’industria italiana del risparmio gestito che ha registrato una raccolta netta negativa di circa 53 miliardi di euro, pari a quasi il 9% del patrimonio netto in essere alla fine del 2006. E questa dinamica appare particolarmente preoccupante in quanto si è verificata al termine di un quinquennio decisamente positivo per l’andamento delle borse mondiali. Sebbene non vi siano evidenze empiriche che segnalino una stabilità della relazione tra la raccolta netta e la performance dei mercati, il divergente andamento dei due fenomeni ha indotto le associazioni di categoria, le autorità di vigilanza e gli operatori professionali ad interrogarsi sul futuro dell’industria del risparmio gestito. Le motivazioni più frequentemente addotte per spiegare la crisi del settore riguardano il trattamento fiscale penalizzante rispetto ai fondi di diritto estero, l’eccessiva trasparenza rispetto a “sostituti” disponibili sul mercato finanziario italiano (le polizze e le obbligazioni strutturate) e la struttura dell’offerta con particolare riferimento alla mancata distinzione tra produzione e distribuzione. Il tema del pricing dei fondi comuni si colloca, quindi, a pieno titolo nell’ambito del dibattito sulla più opportuna configurazione della catena del valore del risparmio gestito e, in particolare, sulla distinzione, introdotta dalla disciplina Mifid, tra attività di consulenza, collocamento ed execution only. Questa graduazione dell’intensità del servizio offerto al risparmiatore trova la propria ragion d’essere nel ruolo svolto dal soggetto che gestisce la relazione di clientela (la distribuzione) e deve quindi trascendere dalla natura e dalla tipologia del prodotto finanziario utilizzato (la produzione). Le attuali modalità di pricing dei fondi comuni di investimento, che fanno transitare dal prodotto anche la remunerazione della distribuzione - attraverso il meccanismo della retrocessione -, mal si conciliano con la nuova impostazione regolamentare che, non a caso, è focalizzata sulle modalità distributive più che su quelle produttive. Obiettivo dell’articolo è di proporre una prospettiva di ricerca nell’ambito dell’economia degli intermediari finanziari che metta in evidenza le criticità dell’attuale impostazione e che incentivi una più netta distinzione tra produzione e distribuzione.

Adjuvant Trastuzumab with chemotherapy is the gold standard for human epidermal growth factor receptor 2 (HER2)-positive early breast cancer (HER2+ EBC). Older patients have been largely under-represented in clinical trials, and few data on Trastuzumab cardiotoxicity have been reported in this subgroup.Four hundred and ninety-nine consecutive HER2+ EBC patients were treated with adjuvant trastuzumab and chemotherapy (aTrastC) at 10 Italian institutions. We evaluated disease prevalence and patient characteristics in the patients older than 60 years of age (over-60), prevalence of aTrastC cardiotoxicity and risk factors.There were 160 'over-60' patients (32%), in whom a higher prevalence of hypertension, diabetes, renal dysfunction, dyslipidemia and treatment with ACEi (40 versus 8%) and beta blockers (20 versus 8%) was found than in the younger patients (339 = 68%). Clinical heart failure occurred in 6% of the 'over-60' and in 2% of the younger patients. A reduction in left ventricular ejection fraction of10 points was detected in 33% of the 'over-60' and in 23% of the younger patients (all P0.05). aTrastC was discontinued in 10% of the 'over-60' and in 4% of the younger patients (P = 0.003), restarted in 44% of the 'over-60' and in 58% of the younger women (P = ns).In clinical practice, 32% of HER2+ EBC patients treated with aTrastC are 'over-60'. These patients have an increased cardiovascular risk profile and develop aTrastC cardiotoxicity commonly.
