Introdução: A obturação dos canais radiculares desempenha indispensável papel na preservação do tratamento endodôntico, sendo o cimento responsável por importantes funções, como vedação, sepultamento de bactérias remanescentes e preenchimento de irregularidades. Entretanto, para a obtenção do sucesso no tratamento e excelente selamento em longo prazo, é necessário que o cimento contemple algumas propriedades físicas, encaixando-se nos parâmetros para um cimento endodôntico ideal. Objetivo: O objetivo do presente estudo foi avaliar quantitativamente a solubilidade e o escoamento de dois cimentos resinosos, Sealer Plus (MK Life, Porto Alegre, Brasil) e AH Plus (Dentsply, Konstanz, Alemanha). Métodos: Para a realização dos testes físicos, foi empregada a especificação #57 da American Dental Association (ADA) para materiais obturadores endodônticos. Para análise estatística no teste de escoamento, foi utilizada o teste t para amostras independentes e a solubilidade dos cimentos foi expressa em números percentuais. Resultados: A capacidade de escoamento dos cimentos analisados foi semelhante, não apresentando diferença estatística. Quanto à solubilidade, o Sealer Plus® teve sua massa ligeiramente aumentada após contato com a umidade, enquanto o AH Plus® teve sua massa diminuída. Conclusão: Ambos os materiais estão de acordo com as especificações preconizadas pela norma #57 da ADA, a qual exige que a sua solubilidade não deve ser superior a 3% de sua massa original e que o diâmetro do disco formado seja superior a 25 milímetros.

A fístula cutânea de origem odontogênica é decorrente da infecção crônica que pode comprometer a estética do paciente e quando mal disgnóstica, por médicos e/ou cirurgiões-dentistas, pode ocasionar alteração estética e transtornos psicológicos ao paciente. O presente relato de caso descreve o correto diagnóstico e o tratamento de uma paciente que foi anteriormente encaminhada a diferentes profissionais de saúde, sendo inclusive submetida a biopsia após hipóteses diagnósticas incorretas. Após avaliação clínica e de exames radiográficos periapicais e tomografia computadorizada de feixe cônico, solicitados por um endodontista, o caso foi diagnosticado de maneira adequada e um plano de tratamento foi proposto. O tratamento endodôntico não-cirúrgico foi realizado, proporcionando a resolução da fístula em curto período de tempo. O presente caso clínico reforça a necessidade de se realizar o diagnóstico diferencial de patologias associadas as fístulas cutâneas ocorridas na região facial ou cervical, e da importância da colaboração interprofissional, inclusive considerando a etiologia odontogênica.

Objetivo: este estudo reporta um caso clínico de reabsorção radicular interna em um dente com necrose pulpar que foi submetido ao tratamento endodôntico no qual, na fase de preparo químico-mecânico, foi utilizada uma técnica suplementar de irrigação com o instrumento XP-Endo Finisher. Métodos: após radiografia para diagnóstico e realização dos testes semiotécnicos, fez-se a instrumentação com sistema Protaper Universal, ativação da substância irrigadora utilizada (digluconato de clorexidina a 2%), com o sistema XP-Endo Finisher, remoção da smear layer, com EDTA a 17% ativada com o mesmo sistema, inserção de medicação intracanal Calen, com paramonoclorofenol, e selamento provisório da coroa dentária, com coltosol. Na segunda sessão, realizou-se a obturação utilizando a técnica de Schilder associada à técnica híbrida de Tagger. Conclusão: o sistema XP-Endo Finisher se mostrou eficaz como técnica suplementar de irrigação, capaz de favorecer a limpeza do sistema de canais radiculares.

Introduction This systematic review aimed to compare the effectiveness of sodium hypochlorite and chlorhexidine for root canal disinfection during root canal therapy. Methods A literature search for clinical trials was made on the PubMed (MEDLINE), Web of Knowledge, SCOPUS, and Science Direct databases and in the reference lists of the identified articles up to January 2015. Quality assessment of the selected studies was performed according to the Consolidated Standards of Reporting Trials statement. Results One clinical trial and 4 randomized clinical trials were selected from the 172 articles initially identified. There was heterogeneity in the laboratory methods used to assess the root canal disinfection as well as in the concentrations of the irrigants used. Therefore, meta-analysis was not performed. Two studies reported effective and similar reductions in bacterial levels for both irrigants. Sodium hypochlorite was more effective than chlorhexidine in reducing microorganisms in 1 study, and another reported opposite findings. Both root irrigants were ineffective in eliminating endotoxins from necrotic pulp root canals in 1 study. Trial design and information regarding randomization procedures were not clearly described in the clinical trials. No study compared laboratory results with clinical outcomes. Conclusions The available evidence on this topic is scarce, and the findings of studies were not consistent. Additional randomized clinical trials using clinical outcomes to compare the use of sodium hypochlorite and chlorhexidine during root canal therapy are needed.

BACKGROUND This study compared the incidence of deviation along curved canals after preparation with two nickel-titanium (NiTi) rotary systems, Twisted File Adaptive and BT-RaCe. MATERIAL AND METHODS Forty resin training blocks with curved canals were filled with ink and divided into two groups according to the instrumentation technique. Preinstrumentation images were acquired by using a stereomicroscope. The canals were up to an instrument #35/0.04. Postinstrumentation images were captured using the same conditions, and the images were superimposed. The amount of resin removed was measured at 8 different points, beginning at the apical terminus of the canal. Differences in the mesial and distal aspects were measured to evaluate the occurrence of deviation. The Student's-t test was used for comparison of the intragroup deviation. Intergroup analysis was performed by using one-way ANOVA for each level. For multiple comparisons, the Bonferroni test was used and a cutoff for significance was set at 5%. RESULTS Intragroup analysis showed that both instrumentation techniques promoted some deviation at all levels. The search for new instruments to promote an appropriate cervical preparation has led to the development of new rotary instruments such as TripleGates. However, to the best of the authors' knowledge, there is no study evaluating TripleGates effect on the “risk zone” of mandibular molars. Objectives : The aim of this study was to evaluate the effects of a crown-down sequence of Gates-Glidden and TripleGates burs on the remaining cervical dentin thickness and the total amount of dentin removed from the root canals during the instrumentation by using cone beam computed tomography. The number of separated instruments was also evaluated. Material and Methods : Mesial roots of 40 mandibular first molars were divided into 2 equal groups: crown-down sequence of Gates-Glidden (#3, #2, #1) and TripleGates burs. Cervical dentin thickness and canal area were measured before and after instrumentation by using cone beam computed tomography and image analysis software. Student’s t-test was used to determine significant differences at p0.05) were observed between the instruments, regarding the root canal area and dentin wall thickness. Conclusion : Both tested instruments used for cervical preparation were safe to be used in the mesial root canal of mandibular molars.

A etiologia das lesões perirradiculares e suas diferentes manifestações clínicas já estão bastante esclarecidas, porém o tratamento dos cistos radiculares ainda é um assunto controverso na Odontologia. O objetivo deste trabalho é realizar uma análise crítica, baseada na literatura, sobre o tratamento de cistos radiculares, buscando evidências que demonstrem que os cistos radiculares podem ser eliminados após tratamento endodôntico. Os cistos podem ser divididos em: cistos em bolsa e cistos verdadeiros, sendo que os cistos em bolsa respondem ao tratamento endodôntico, enquanto os cistos verdadeiros somente podem ser tratados através da cirurgia perirradicular. Pode-se concluir que sendo o agente microbiano o responsável pelas lesões perirradiculares, a maioria destas lesões, incluindo os cistos, regridem após a intervenção endodôntica não cirúrgica. DOI: 10.18363/rbo.v71i1.511 The etiology of apical periodontitis and its different clinical manifestations are already well versed, but the treatment of radicular cysts is still a controversial subject in Dentistry. The e aim of this study is to perform a critical analysis based on literature regarding treatment of radicular cysts, seeking evidence demonstrating that the radicular cysts can be eliminated after endodontic treatment. The cysts can be divided in: Apical Pocket Cysts and true cysts. Apical pocket cysts respond to endodontic treatment, while true cysts can only be treated by periradicular surgery. It can be concluded that since the microbial agent is responsible for the apical periodontitis, the majority of these lesions, including cysts.

Orientador: Caio Cézar Randi Ferraz Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo deste estudo in vitro foi avaliar o selamento coronário e resistência de união do cimento resinoso AH Plus associado a um protocolo de hibridização da dentina radicular com Scotchbond Multi Purpose (SBMP) após o uso de diferentes substâncias químicas auxiliares endodônticas (Hipoclorito de Sódio 5,25%, Gluconato de Clorexidina 2%, EDTA 17% e ácido fosfórico 37%). Para avaliar o selamento, foram utilizadas raízes palatinas de molares superiores divididas em 18 grupos (n=10) que foram tratadas com diferentes substâncias químicas, obturadas com cimento endodôntico AH Plus associado a cones resinosos ou a cones de guta-percha. Em alguns grupos obturados com AH Plus + cones resinosos, a dentina foi hibridizada antes da aplicação do material obturador. O selamento coronário foi avaliado pelo método de filtração de fluidos. Um espécime de cada grupo foi levado ao MEV para avaliação morfológica da interface de união entre os materiais obturadores e a dentina radicular. Para a avaliação da resistência de união do cimento à dentina radicular foram utilizadas 60 raízes de dentes bovinos, cortadas longitudinalmente, que receberam tratamento com as substâncias químicas auxiliares utilizadas na avaliação do selamento (n=5). Foram criadas seis perfurações nas raízes selecionadas, duas por terço, onde foi inserido o cimento AH Plus, após o tratamento da dentina com o sistema adesivo SBMP. Após a presa do cimento, os corpos-de-prova foram submetidos ao teste de resistência ao cisalhamento por extrusão ?Push out?. Posteriormente, o modo de fratura foi analisado em lupa estereoscópica 32X. Um espécime de cada grupo foi avaliado no MEV para confirmação das análises feitas pela lupa. A análise do modo de fratura demonstrou (22,77%) fraturas adesivas, (11,67%) fraturas coesivas em cimento e (65,55%) fraturas mistas para o protocolo SBMP/ AH Plus, enquanto que para o AH Plus a distribuição foi (86,11%) coesivas em cimento e (13,89%) mistas. A análise estatística (Dixon-Grubbs-Neuman, Kolmogorov-Smirnov, Cochran e Tukey HSD) demonstrou que o selamento coronário, após a hibridização dentinária com o SBMP e obturação com cone Resinoso + AH Plus, foi superior ao promovido pela guta-percha associada ao AH Plus, para todos os protocolos de irrigação (p ? 0,05). Para a resistência de união, a aplicação do SBMP se mostrou desfavorável, pois diminuiu a resistência de união do AH Plus (p ? 0,05). As substâncias químicas agiram diferentemente sobre a resistência de união nos grupos onde foi aplicado o adesivo, sendo observado que o tratamento dentinário com clorexidina e soro apresentou melhores valores de resistência quando foram associados ao condicionamento dentinário com EDTA 17%, diferentemente do que ocorreu nos grupos onde se aplicou hipoclorito de sódio 5,25% que apresentou melhores resultados com a utilização do ácido fosfórico 37%. Houve correlação inversa entre resistência de união e capacidade de selamento. Pode-se concluir que o uso do sistema adesivo Scotchbond Multi Purpose melhorou a capacidade de selamento coronário do AH Plus para todos os protocolos de irrigação utilizados, e que a resistência de união deste cimento foi diminuída quando o adesivo foi aplicado, sendo influenciada pelo tipo de condicionamento e substâncias químicas Abstract: The aim of this in vitro study was to evaluate the sealing ability and bond strength of AH Plus sealer associated with a hybridization protocol of root dentin with ScotchbondMulti Purpose (SBMP) after the use to different endodontic auxiliary chemical substances (5.25% sodium hypochlorite and 2% Chlorhexidine and 17 EDTA) in One hundred-eighty palatal roots of maxillary molars were selected and divided into 18 groups according to the dentin protocol treatment (n=10), filled with AH Plus sealer associated with resinous cones or gutta-percha in order to evaluate the sealing ability by fluid filtration (FLODEC system). In s groups that were used in the AH Plus + resinous cones, was hybridized dentin before applying the sealing material. One specime from each group was taken to a SEM morphology analysis of the bonded interfaces between sealers and dentin. For the bond strength test, 60 root bovine teeth were selected and treated with each auxiliary chemical substances selected (n=5). Six holes were poked in each root, being two in the cervical, middle and apical third of the root. The roles were filled with AH Plus sealer with or without the adhesive system, and subsequently, submitted to push out test. Thereafter, the fracture mode was examined using stereomicroscope (x32). One specimen from each was SEM analyzed. The fracture mode methodology revealed a 22.77% of adhesive, 11.67% of cohesive and 65.55% of mixture fracture for the SBMP / AH Plus protocol. In contrast, the AH Plus protocol indicated 86.11% of cohesive and 13.89% of mixed fracture. Statistical analysis (Dixon-Grubs-Neuman, Kolmogorov-Smirnov, Cochran and Tukey HSD) showed that the ScotchBond Multi Purpose (SBMP) + Resilon cone + AH Plus sealer group promoted a higher sealing ability than the gutta-percha + AH Plus sealer group (p < 0.05), regardless the auxiliary chemical substance.
