Presentaré formalmente la reología como una filosofía precisa. Hoy hemos de habérnoslas con nuestras herramientas que no pueden ser, por razones que veremos, ni la epistemología ni la ontología clásicas; requerimos de «noología» y «reología», filosofías incoadas en el pensamiento de Xavier Zubiri. Teniendo a la noología por realismo de partida en el que «realidad es» modo de quedar las cosas en la aprehensión, y a la reología por realismo de llegada en el que la constitución estructural de las cosas «es realidad», la metafísica queda purgada de lastres vetustos. Todo se presenta en dos partes: I) Escasez de reólogos, expondremos qué se entiende por «noología» y por «reología». II) Realidad, expondremos el problema de los «dos» realismos («de partida» y «de llegada»), lo concretizaremos en la filosofía de Zubiri y veremos que «ambos» son más bien uno solo. Concluiremos que, si queremos tener una filosofía pertinente para el conocimiento contemporáneo del mundo sin limitarnos a un estudio escolar de un autor, hemos de hacer reología.

We present cosmological constraints from a gravitational lensing mass map covering 9400 deg ^2 reconstructed from measurements of the cosmic microwave background (CMB) made by the Atacama Cosmology Telescope (ACT) from 2017 to 2021. In combination with measurements of baryon acoustic oscillations and big bang nucleosynthesis, we obtain the clustering amplitude σ _8 = 0.819 ± 0.015 at 1.8% precision, ${S}_{8}\equiv {\sigma }_{8}{({{\rm{\Omega }}}_{{\rm{m}}}/0.3)}^{0.5}=0.840\pm 0.028$ , and the Hubble constant H _0 = (68.3 ± 1.1) km s ^−1 Mpc ^−1 at 1.6% precision. A joint constraint with Planck CMB lensing yields σ _8 = 0.812 ± 0.013, ${S}_{8}\equiv {\sigma }_{8}{({{\rm{\Omega }}}_{{\rm{m}}}/0.3)}^{0.5}=0.831\pm 0.023$ , and H _0 = (68.1 ± 1.0) km s ^−1 Mpc ^−1 . These measurements agree with ΛCDM extrapolations from the CMB anisotropies measured by Planck. We revisit constraints from the KiDS, DES, and HSC galaxy surveys with a uniform set of assumptions and find that S _8 from all three are lower than that from ACT+Planck lensing by levels ranging from 1.7 σ to 2.1 σ . This motivates further measurements and comparison, not just between the CMB anisotropies and galaxy lensing but also between CMB lensing probing z ∼ 0.5–5 on mostly linear scales and galaxy lensing at z ∼ 0.5 on smaller scales. We combine with CMB anisotropies to constrain extensions of ΛCDM, limiting neutrino masses to ∑ m _ν < 0.13 eV (95% c.l.), for example. We describe the mass map and related data products that will enable a wide array of cross-correlation science. Our results provide independent confirmation that the universe is spatially flat, conforms with general relativity, and is described remarkably well by the ΛCDM model, while paving a promising path for neutrino physics with lensing from upcoming ground-based CMB surveys.

We present new measurements of cosmic microwave background (CMB) lensing over 9400 deg ^2 of the sky. These lensing measurements are derived from the Atacama Cosmology Telescope (ACT) Data Release 6 (DR6) CMB data set, which consists of five seasons of ACT CMB temperature and polarization observations. We determine the amplitude of the CMB lensing power spectrum at 2.3% precision (43 σ significance) using a novel pipeline that minimizes sensitivity to foregrounds and to noise properties. To ensure that our results are robust, we analyze an extensive set of null tests, consistency tests, and systematic error estimates and employ a blinded analysis framework. Our CMB lensing power spectrum measurement provides constraints on the amplitude of cosmic structure that do not depend on Planck or galaxy survey data, thus giving independent information about large-scale structure growth and potential tensions in structure measurements. The baseline spectrum is well fit by a lensing amplitude of A _lens = 1.013 ± 0.023 relative to the Planck 2018 CMB power spectra best-fit ΛCDM model and A _lens = 1.005 ± 0.023 relative to the ACT DR4 + WMAP best-fit model. From our lensing power spectrum measurement, we derive constraints on the parameter combination ${S}_{8}^{\mathrm{CMBL}}\equiv {\sigma }_{8}{\left({{\rm{\Omega }}}_{m}/0.3\right)}^{0.25}$ of ${S}_{8}^{\mathrm{CMBL}}=0.818\pm 0.022$ from ACT DR6 CMB lensing alone and ${S}_{8}^{\mathrm{CMBL}}=0.813\pm 0.018$ when combining ACT DR6 and Planck NPIPE CMB lensing power spectra. These results are in excellent agreement with ΛCDM model constraints from Planck or ACT DR4 + WMAP CMB power spectrum measurements. Our lensing measurements from redshifts z ∼ 0.5–5 are thus fully consistent with ΛCDM structure growth predictions based on CMB anisotropies probing primarily z ∼ 1100. We find no evidence for a suppression of the amplitude of cosmic structure at low redshifts.

This study examines the economic viability of an emerging technology for potential upscaling and commercialization in a specific location: the village of New Stuyahok, Alaska. The proposed technology is hydrothermal liquefaction, which utilizes kelp macroalgae and fishing waste as feedstock. These materials were chosen due to their easy availability in the village and their alignment with the local economy. The economic evaluation is based on the net present value (NPV) and sensitivity models. Different feedstock ratios (on a dry basis), such as 100:0, 50:50, and 30:70 of kelp and fishing waste, respectively, were evaluated to determine the optimal combination. The results indicated that the process is economically viable only when a high proportion of fishing waste is used. This can be ascribed to the constrained output yield of the kelp biomass and the relatively negligible influence exerted by alginate production on the NPV. However, the ratio 50:50 appears to be economically promising if the costs can be reduced by at least 13.5% or the benefits can be increased by 12.1%. Nevertheless, government support could play a crucial role in expediting the implementation of this technology once it becomes market-ready. This means being practical, scalable, and economically viable, enabling reduced investments or increased benefits that signify its readiness. Utilizing such a tool offers valuable insights into the framework of the proposed technology and the use of local natural resources.

Antecedentes y Objetivo: Los estragos causados por la pandemia COVID-19 alcanzaron también la educación médica perdiéndose la enseñanza tutorial que ejercíamos en nuestro Instituto. Método: Se implementaron estrategias a través de las tecnologías de la información y de la comunicación que se describen en este artículo. Resultados: Con las estrategias llevadas a cabo se pudo continuar con toda la educación teórica de manera virtual a distancia, incluyendo el pase cotidiano de visita a los enfermos, la presentación de casos clínicos, el análisis a distancia de los métodos de laboratorio y gabinete, así como la enseñanza de la exploración física y la adquisición de los métodos de ayuda diagnóstica de los gabinetes. El número de asistentes a la sesión general semanaria del Instituto incremento de 100 asistentes presenciales a 500 virtuales (promedio) con reproducciones de las mismas en un promedio de 5,000. Se discuten las ventajas e inconvenientes de estas nuevas técnicas y cuáles son los desafíos para el futuro. Conclusiones: Sin duda alguna, la pandemia ha causado daño a la enseñanza de dos generaciones de estudiantes de la medicina en los años 2020 y 2021 y aún seguimos temerosos de que en el 2022 persista. A nuestro juicio la educación médica implementada por nosotros no es lo ideal, pero por lo menos constituye un buen paliativo del daño que la pandemia produjo en la educación médica.

Insatisfechos ante las antropologías filosóficas disponibles, en este artículo proponemos el esbozo de una antropología fundamental e invitamos a continuar la empresa. La crítica a las antropologías tradicionales es común en otros autores y nos ayuda a justificar la elaboración de una nueva antropología que calificamos de “reológica”. Sus primeros objetivos son 1) mostrar que la res humana es “física” y por tanto no “dual” como han creído casi todas las antropologías y 2) mostrar que si hay algo fundamentalmente humano no está en las categorías clásicas de “psique”, “reflexividad”, etc., sino en lo que llamamos “experienciar”, i.e. probar físicamente realidad, algo que no hacen el resto de res. Se trata de una propuesta metafísica y no “psicologista”. Por tanto, explicaremos brevemente qué es reología y, usándola como herramienta, esbozaremos algunas directrices que delimiten una futura investigación reológica de la realidad humana.

En el asunto de las realidades materiales ha de profundizarse por vía tanto científica como filosófica. Por la primera, en esta ocasión nos serviremos de la química. Por la segunda, justificaremos que el acceso no puede ser la ontología, sino la reología. Iniciaremos con una noología mínima de las cosas materiales para poder ahondar en ellas. Entonces pondremos a debatir la ontología y la reología: hablaremos de la idea de “sustancia” química, los pluralismos ontológicos y otros resabios, para dar paso a una muestra de cómo la química y la reología se acoplan trabajando. Químicos y reólogos no están uno al servicio del otro, sino ambos al servicio de una investigación de realidad. Así, si por trabajar con las ciencias la reología es una metafísica físicamente responsable, es posible que la química (y otras) por hacerlo con la reología devengan metafísicamente responsables.

Este artículo sostiene la tesis de que la realidad es fundamentalmente estructural sin cabida para «substancias». En una breve primera parte expondremos la reología (una metafísica físicamente responsable) como el marco desde el cual sostenerlo. En una segunda haremos: 1) una exposición de la idea de realidad como substancia, 2) criticaremos el punto anterior, 3) desarrollaremos cómo la realidad es fundamentalmente estructura, 4) mostraremos que esta estructura es siempre estructuración. Por último y previo a concluir, dejaremos algunos problemas abiertos propios de investigaciones reológicas venideras

Abstract Background Sweetpotato is an important staple food crop worldwide. The genotype mainly influences the nutritional quality of its storage roots, but environmental conditions could produce significant variations in chemical composition and quality. The aim of this study was to characterize sweetpotato diversity of 20 selected genotypes and identify harvest time (90, 120, and 150 days after planting-DAP) and environmental effects on quality attributes estimated by proximate analysis (dry matter, ash, crude fiber, total protein, and total soluble solids). Red (R), green (G), and blue (B) (RGB) analysis was used to characterize the genotypes phenotypically. Results The results of the current study revealed that flesh color was associated with proximate composition. RGB analysis showed that low B pixel values were present in yellow–orange- and purple-fleshed genotypes, which simultaneously exhibited high total protein content (TPC), ash content (AC), and crude fiber (CF), while cream- and white-fleshed genotypes showed high B pixel values and were related to high dry matter content (DMC). In these genotypes, the high DMC was maintained or increased through harvest time, however, a reduction in the proportion of accumulated AC and TPC was observed. On the other hand, in the pigmented genotypes, DMC increased up to 120 DAP, showing major stability in terms of AC and TPC. Regardless of harvest time, more intense rain events 30 days before harvest, affected the storage of DMC in roots negatively, while AC, TPC, and CF were accumulated more efficiently, or their proportion increased. High temperatures increased total soluble solids (TSS) accumulation and reduced AC, TPC, and CF accumulation. The combined analysis, according to the additive main effects and multiplicative interaction (AMMI) model, confirmed these results. Conclusions These findings indicated that white-fleshed genotypes contain high DMC, although, with lower TPC, AC, and CF contents compared to yellow–orange- and purple-fleshed genotypes. Although there is an increase in DMC in extended harvests (in orange-fleshed genotypes up to 120 DAP), the presence of pre-harvest rain promotes its translocation and loss. On the contrary, TPC, AC, and CF can be kept stable or even increase, except in environments with high temperatures that induce low accumulation. The current study provides a better understanding of the nutritional response of sweetpotato diversity under several growing conditions, which can be recognized and used integrally to improve food quality.

The diamonds production has a great impact on the economy of the Republic of Angola, in which Catoca plant is of particular relevance. The autogenous grinding plays an important role in the technology of processing kimberlitic ores, as it has the purpose of achieving the release at the coarsest possible sizes. In order to evaluate the influence of the rotation speed and the pulp density on the energy efficiency indicators of the mill, a series of tests was designed considering three speed levels (55, 65 and 80 % of the critical speed) and two levels of dilution (dilute pulps, 45-50 % solids, and dense pulps 55-60 % solid). A sampling system was organized in the industrial facility that allowed evaluating the size distribution of the particles in the mill discharge and the power consumed by its motor and its capacity were recorded. At the same time, the specific energy consumed by the mill was evaluated. The results show that both operational factors have a decisive influence on the indicators studied. The higher the density of the pulp and the rotational speed of the mill, the larger the size of the mill outlet. The speed of rotation strongly influences the energy consumption of the installation.

SARS-CoV-2 uses the ACE2 receptor and the cellular protease TMPRSS2 for entry into target cells. The present study aimed to establish if the TMPRSS2 polymorphisms are associated with COVID-19 disease. The study included 609 patients with COVID-19 confirmed by RT-PCR test and 291 individuals negative for the SARS-CoV-2 infection confirmed by RT-PCR test and without antibodies anti-SARS-CoV-2. Four TMPRSS2 polymorphisms (rs12329760, rs2298659, rs456298, and rs462574) were determined using the 5′exonuclease TaqMan assays. Under different inheritance models, the rs2298659 (pcodominant2 = 0.018, precessive = 0.006, padditive = 0.019), rs456298 (pcodominant1 = 0.014, pcodominant2 = 0.004; pdominant = 0.009, precessive = 0.004, padditive = 0.0009), and rs462574 (pcodominant1 = 0.017, pcodominant2 = 0.004, pdominant = 0.041, precessive = 0.002, padditive = 0.003) polymorphisms were associated with high risk of developing COVID-19. Two risks (ATGC and GAAC) and two protectives (GAGC and GAGT) haplotypes were detected. High levels of lactic acid dehydrogenase (LDH) were observed in patients with the rs462574AA and rs456298TT genotypes (p = 0.005 and p = 0.020, respectively), whereas, high heart rate was present in patients with the rs462574AA genotype (p = 0.028). Our data suggest that the rs2298659, rs456298, and rs462574 polymorphisms independently and as haplotypes are associated with the risk of COVID-19. The rs456298 and rs462574 genotypes are related to high levels of LDH and heart rate.

A deep review of the state of the electromobility in urban public transport by bus was conducted from all different charging strategies, types of chargers, and e-buses with a general overview and SWOT analysis. A review of five case studies worldwide was also conducted and a real case study with real data was shown in depth: EMT Madrid, where all chargers and charging systems were developed in a single operation center. Total Cost of Ownership (TCO) from the literature and from the case study for e-buses were shown as compared with different bus technologies.

Background: TK2 is a nuclear gene encoding the mitochondrial matrix protein thymidine kinase 2 (TK2), a critical enzyme in the mitochondrial nucleotide salvage pathway. Deficiency of TK2 activity causes mitochondrial DNA (mtDNA) depletion, which in humans manifests predominantly as a mitochondrial myopathy with onset typically in infancy and childhood. We previously showed that oral treatment of the Tk2 H126N knock-in mouse model (Tk2−/−) with the TK2 substrates, deoxycytidine (dCtd) and thymidine (dThd), delayed disease onset and prolonged median survival by 3-fold. Nevertheless, dCtd + dThd treated Tk2−/− mice showed mtDNA depletion in brain as early as postnatal day 13 and in virtually all other tissues at age 29 days. Methods: To enhance mechanistic understanding and efficacy of dCtd + dThd therapy, we studied the bioavailability of dCtd and dThd in various tissues as well as levels of the cytosolic enzymes, TK1 and dCK that convert the deoxynucleosides into dCMP and dTMP. Findings: Parenteral treatment relative to oral treatment produced higher levels of dCtd and dThd and improved mtDNA levels in liver and heart, but did not ameliorate molecular defects in brain or prolong survival. Down-regulation of TK1 correlated with temporal- and tissue-specificity of response to dCtd + dThd. Finally, we observed in human infant and adult muscle expression of TK1 and dCK, which account for the long-term efficacy to dCtd + dThd therapy in TK2 deficient patients. Interpretations: These data indicate that the cytosolic pyrimidine salvage pathway enzymes TK1 and dCK are critical for therapeutic efficacy of deoxynucleoside therapy for Tk2 deficiency. The importance of phosphate solubilizing rhizobacteria (PSB) has been well-document as an option for enhancing sustainable agriculture. As a particular group of plant growth promoting rhizobacteria (PGPR), PSB play an important role in the soil phosphorus cycle, increasing the bioavailability for growth and plant development. This study analyses the plant growth promoting effects of 5 strains (BN0009, BN0013, BN0015, BN0024, and BN0035) out of 180 isolated from Jarava frigida (Phil.) F.Rojas (Poaceae), a native grass from the Andean Atacama desert from North of Chile. The five bacterial isolated (BN strains) were identified as non-pathogenic Erwinia sp. and show a high phosphate solubilization capacity for Ca(PO4) ranging from 608.9 to 781.4 mg/L. Strains IAA production varies between 23.5 and 35.9 mg/L, siderophores, phosphatase (alkaline and acid) production was also observed, but none of the five isolated presented antagonism against plant pathogens Botrytis sp. and Sclerotinia sp. All isolates enhanced seed germination in Lactuca sativa and Solanum lycopersicum (excepting BN009). Additionally, all strains stimulated the early root elongation and seedling development in lettuce and tomato. Pot experiments displayed that BN0015, BN0024, and BN0035 significantly promote plant growth regarding root and leaf area, root and leaf weight, as well as leaf number compared with non-treated plants. In a field experiment with lettuce and two fertilization treatments (50 and 100% of the recommended crop fertilization), BN0024 application improved crop productivity compared to respective control. P content in plants with bacterial inoculations increased significantly compared to control in either fertilization treatment, suggesting an improved nutrient uptake. Also, lettuce with 50% fertilization and inoculation with BN0024 equate productivity with the control 100% fertilization. Finally, we discuss these results in the context of applicability to enhance the agroecosystem productivity in arid and semiarid zones.

Mineral processing and metallurgy production centers may leave a far-reaching fingerprint of soil contamination. This scenario is particularly relevant in the mining district of Linares (Southern Spain), where former industrial sites are now dedicated to other land uses. Within this context, we selected five sectors of concern in Linares region, which are currently used as agricultural and residential areas. The study began with an edaphic characterization, including grain-size fractioning and soil chemical analyses, which were complemented by mineralogical and sequential extraction information. Anomalous soil concentrations of As, Cd, Cu, Pb, and Zn were found, with higher values than the admissible regional guideline limits. Moreover, chemical speciation indicated that in general, Pb, Zn, and Cd were highly available and bound mainly to the carbonate fraction. In addition, health risk assessment evidenced potential threats by Pb and As. Regarding remediation approaches, we observed that, in soils affected by mining and ore dressing activities, the clay and silt size fractions contained the highest pollution load, making them suitable for a size classification treatment. By contrast, in areas affected by metallurgical activity, pollutants were prone to be evenly distributed among all grain sizes, thereby complicating the implementation of such remediation strategies.

Resumen: Introducción y objetivos: Adalimumab (ADA), anticuerpo anti-TNF-α monoclonal recombinante de origen humano, generalmente se emplea como tratamiento de segunda línea en niños con enfermedad de Crohn (EC) que no han respondido o han perdido respuesta a infliximab (IFX). En las series publicadas más del 70% de los pacientes habían sido tratados inicialmente con IFX. Los datos sobre la eficacia a corto y a largo plazo de ADA en pacientes naïve a anti-TNF son muy limitados. El objetivo del presente estudio es describir nuestra experiencia con ADA como tratamiento anti-TNF de primera línea en niños con EC. Material y método: Estudio multicéntrico, retrospectivo que incluye pacientes con EC tratados con ADA como anti-TNF de primera línea. Resultados: Se incluyeron 62 pacientes (34 varones) con una edad media de 13,0 ± 2,4 años, un tiempo de evolución de la enfermedad de 7,3 meses (RIQ 2,7-21) y un wPCDAI de 35 puntos (RIQ 24,3-47,5). En el momento de comenzar ADA, 58 pacientes (93,5%) estaban recibiendo tratamiento inmunomodulador. A las 12 semanas de tratamiento el 80,6% (50/62) habían alcanzado la remisión clínica, así como el 95% (57/60) a las 52 semanas. Ocho pacientes (13%) presentaron efectos adversos. Se constató un incremento significativo de los z-scores de talla, velocidad de crecimiento e índice de masa corporal (IMC) a las 52 semanas de tratamiento, en especial en aquellos con retraso de crecimiento. Conclusiones: El tratamiento con ADA favorece una remisión clínica prolongada en pacientes naïve a anti-TNF. El tratamiento con ADA mejora la velocidad de crecimiento en niños con EC y retraso de crecimiento al inicio del tratamiento. Se constató un incremento significativo de los z-scores de talla, velocidad de crecimiento e índice de masa corporal (IMC) a las 52 semanas de tratamiento, en especial en aquellos con retraso de crecimiento. Conclusiones: El tratamiento con ADA favorece una remisión clínica prolongada en pacientes naïve a anti-TNF. El tratamiento con ADA mejora la velocidad de crecimiento en niños con EC y retraso de crecimiento al inicio del tratamiento.

Background: Research has demonstrated that parent-child attachment security and school connectedness (SC) are protective factors against substance use, depression, and suicidality during adolescence. However, past research has examined these factors independently, and little is known about how attachment security and SC work in conjunction to reduce risk. Objective: The present study examined the moderating role of SC on the relations between parent-adolescent attachment (security, anxiety, and avoidance) and substance use, depression, and suicidality among at-risk adolescents. Method: Using a cross-sectional design, 480 community-based adolescents (60.5% female; M[subscript age] = 14.86) aged 12-18 years self-reported parent-adolescent attachment, adolescent substance use, depression, and suicidality. Results: High levels of attachment security in conjunction with high SC predicted the lowest risk for adolescent substance use. Several sex differences were found: SC significantly moderated the relation between attachment security and depressive symptoms in female adolescents and suicidality in male adolescents. Results also revealed that the moderating of role of SC differed in relation to attachment anxiety versus attachment avoidance for female versus male adolescents. Conclusion: Findings point to the importance of testing associations between multiple dimensions of attachment and SC on several well-established mental health outcomes in a sample of high-risk adolescents.

A bioslurry reactor was designed and used to treat loamy clay soil polluted with polycyclic aromatic hydrocarbons (PAHs). To this end, biostimulation alone, or combined with bioaugmentation with two bacterial strains (Rhodocccus erythropolis and Pseudomonas stuzeri) previously isolated from the polluted site, was applied. The PAH concentrations decreased notably after 15 days in all of the treatments. The concentrations of the two- and three-ring compounds fell by >80%, and, remarkably, the four- to six-ring PAHs also showed a marked decrease (>70%). These results thus indicate the capacity of bioslurry treatments to improve, notably, the degradation yields obtained in a previous real-scale remediation carried out using biopiles. In this sense, the remarkable results for recalcitrant PAHs can be attributed to the increase pollutants’ bioavailability achieves in the slurry bioreactors. Regarding bioaugmentation, although treatment with R. erythropolis led to a somewhat greater reduction of lighter PAHs at 15 days, the most time-effective treatment was achieved using P. stutzeri, which led to an 84% depletion of total PAHs in only three days. The effects of microbial degradation of other organic compounds were also monitored by means of combined qualitative and quantitative gas chromatography mass spectrometry (GC–MS) tools, as was the evolution of microbial populations, which was analyzed by culture and molecular fingerprinting experiments. On the basis of our findings, bioslurry technology emerges as a rapid and operative option for the remediation of polluted sites, especially for fine soil fractions with a high load of recalcitrant pollutants.

Non-invasive brain stimulation using focused ultrasound has many potential applications as a research and clinical tool, including its incorporation as either an extracorporeal or implantable neural prosthetic. To this end, we investigated the effect of focused ultrasound (FUS) combined with systemically administered microbubbles on visual-motor decision-making behavior in monkeys. We applied FUS to the putamen in one hemisphere to open the blood-brain barrier (BBB), and then tested behavioral performance 3–4 h later. On days when the monkeys were treated with FUS, their decisions were faster and more accurate than days without sonication. The performance improvement suggested both a shift in the decision criterion and an enhancement of the use of sensory evidence in the decision process. FUS also interacted with the effect of a low dose of haloperidol. The findings indicate that a two-minute application of FUS can have a sustained impact on performance of complex cognitive tasks, and may increase the efficacy of psychoactive medications. The results lend further support to the idea that the dorsal striatum plays an integral role in evidence- and reward-based decision-making, and provide motivation for incorporating FUS into cognitive neural prosthetic devices.

Se presentó una mujer de 74 años al servicio de urgencias del HRAEBI, ISSSTE, el día 2 de septiembre de 2010 con datos de desequilibrio hidroelectrolítico, hipotensión y conjuntivitis bilateral. Sus signos vitales durante su estancia en urgencias fueron: tensión arterial, 80/40 mmHg; temperatura, 38.5 °C; frecuencia respiratoria, 22 rpm; frecuencia cardíaca 89 lpm Se interconsulta al servicio de Oftalmología quienes diagnosticaron celulitis orbitaria bilateral, con no percepción de luz en ambos ojos e igualmente ausencia de reflejos pupilares. Se inició tratamiento con antibióticos intravenosos y se solicitó tomografía computarizada de órbitas y cerebro. Se internó para estabilizar su estado general; durante su estancia se diagnosticó diabetes mellitus y estado inmunosupresivo por datos de candidiasis oral y otros síntomas. La tomografía mostró celulitis orbitaria bilateral con formación de abscesos orbitarios, sin datos de proceso infeccioso a nivel bucal o de senos paranasales. La evolución de la celulitis fue tórpida a pesar de la terapia antibiótica cuádruple (cefotaxima, vancomicina, metronidazol y clindamicina; posteriormente imipenem, clindamicina y vancomicina) con desarrollo de endoftalmitis bilateral por lo cual fue necesario eviscerar ambos ojos. Se tomó cultivo directo del vítreo y 3 días después se encontró como germen causal Klebsiella pneumoniae.

Introduction: The conventional 24-hour pH monitoring is the gold standard for the diagnosis of gastro-esophageal reflux (GER), a possible cause of Apparent Life Threatening Episodes (ALTE). However, multichannel intraluminal impedance (MII) may provide advantages. Objectives: Comparison of the results of MII and pH monitoring in patients undergoing MII-pH monitoring in the 3-year study period because of having suffered from ALTE. Material and methods: Prospective study of MII-pH monitoring performed in our unit to infants < 12 months of age admitted for ALTE for a 3-year period. Results: Thirty nine patients studied. 2,692 pH monitoring episodes, with median of 24 (IQ: 15-44) episodes/patient, 1.30 (IQ: 0.80-2.60) reflux/hour, 1 (IQ: 0-4) reflux episode > 5 min per patient and clearance of 1.20 (IQ: 0.70-2.20) min/reflux. With pH monitoring analysis, 14 children (35.9 %) could have been diagnosed as GER (8 mild, 4 moderate and 2 severe) based on the classical criteria. MII identified a total of 8,895 events; only 3,219 among them were refluxes, with a median of 75 (IQ: 54-111) per patient, 1.30 (IQ: 1.3-2.6) episodes/hour). With MII-pH monitoring combination there were 21.60 (SD 15.21) acid reflux episodes, 67.33 weekly acid (SD 32.09) and 3.34 (SD 7.23) non-acid, being finally diagnosed 33 patients as GER. Conclusions: The association of pH monitoring and MII provides additional information that improves GER diagnostic performance without posing any additional risk to the infant patient. The non-acid/weekly acid refluxes, not detected by pH monitoring, account for a high percentage of episodes, this may have diagnostic and therapeutic significance, especially in infants. Further studies are needed to assess the normality of MMI in pediatric patients.

Forcing ventilation is the most widely used system to remove noxious gases from a working face during tunnel construction. This system creates a region near the face (dead zone), in which ventilation takes place by natural diffusion, rather than being directly swept by the air current. Despite the extensive use of this system, there is still a lack of parametrical studies discerning the main parameters affecting its formation as well as a correlation indicating their interrelation. With this aim in mind, computational fluid dynamics (CFDs) models were used to define the dead zone based on the airflow field patterns. The formation of counter vortices, which although maintain the movement of air hinder its renewal, allowed us to discuss the old paradigm of defining the dead zone as a very low air velocity zone. Moreover, further simulations using a model of air mixed with NO2 offered an idea of NO2 concentrations over time and distance to the face, allowing us to derive at a more realistic equation for the effective distance. The results given here confirm the degree of conservativism of present-day regulations and may assist engineers to improve ventilation efficiency in tunnels by modifying the duct end-to-face distance.

Objectives: to describe the efficacy and safety of adalimumab (ADA) in inducing clinical remission and reducing inflammation of intestinal mucosa in children with Crohn's disease (CD). Methods: we carried out a descriptive, observational study with all patients diagnosed with CD and treated with ADA between January 2007 and March 2013. Disease activity was determined using the Pdiatric Crohn's Disease Activity Index (PCDAI), and the degree of mucosa inflammation by fecal calprotectin (FC). Results: sixteen patients were included. Mean age at diagnosis was 10.6 ± 2.5 years, with a mean age at start of ADA treatment of 12.4 ± 1.8 years, and a median of 1.4 years (IQR 0.5-3) duration from CD diagnosis to start of treatment. Twelve patients were naïve to anti-TNF-α. The PCDAI score at start of ADA treatment was significantly reduced at 12 weeks of follow-up (31.25 IQR 26.8-37.5 vs. 1.2 IQR 0.0-5.0; p = 0.001). Similarly, the FC level decreased at 12 weeks (749 µg/g IQR 514-898 vs. 126 µg/g IQR 67.7-239.2; p = 0.02). Surgery was performed in 4 patients. Adverse events were reported in 4 patients. One patient developed lymphoma at 4 years of ADA treatment in monotherapy. Conclusions: ADA has been shown to be effective in children with moderate-to-severe CD. Treatment benefits should be weighed against side effects. Multicenter longitudinal studies with longer follow-up periods are required to determine the true efficacy and safety of long-term ADA treatment.

Soil washing is a remediation technology based on the adaptation of techniques habitually used in mineral processing to recover contaminants from polluted soils. Its main scope is the reduction of the total volume of contaminated soil, which could later be treated by biological, chemical, or thermal procedures. Since the contaminants are usually associated to the soil finer fractions, direct classification is the most widely employed technique. Moreover, other procedures, such as indirect classification, density and magnetic separation, as well as froth flotation are also extended. Soil washing is usually applied by means of on-site mobile plants which are located in the study sites. However, previously to these field applications, a detailed soil characterization and some trials are required in order to optimize the process. In this work, soils affected by heavy metals as a result of mining, metallurgical, and industrial activities are evaluated and pilot-scale treated. The diversity of soils and residues treated, together with the variety of the methodologies employed enabled the elaboration of a general feasibility protocol.

Former industrial sites are now dedicated to other land uses in the Linares mining district. Here we selected five residential/farming areas (squares of 1 km2 each) and sought to evaluate the levels of contamination by Potentially Toxic Elements (PTEs) of the soils, and also to offer an insight into the threat these pollutants may pose to human health or the environment by means of risk assessment. High concentrations especially of Pb, and also of As, Cd, Cu and Zn were found in quantities that are considerably bioavailable. Moreover, risk assessment revealed unacceptable concentrations for Pb and As in all the areas as well as for Cd and Cu in some squares.

Adaptamos el método universal de predicción por Krigeado a contextos con datos inciertos o imprecisos. Empleando la teoría de conjuntos difusos, desarrollamos un Krigeado difuso niversal que utilizamos para estimar tipos de calidades en un depósito de cuarzo. El método modela la tendencia de los datos por combinación lineal de funciones que dependen de las coordenadas de cada punto, que son números fuzzy. La dependencia espacial entre los datos es determinada por medio de covariogramas no difusos. El método propuesto proporciona úmeros difusos y varianzas no difusas en puntos no muestreados. Finalmente, los números difusos estimados son convertidos en calidades de cuarzo y representados gráfi camente. Por medio del modelo obtenido empleando números difusos ha sido posible efectuar minería selectiva del depósito mineral dependiendo de la demanda del mercado para los tres productos explotados.

El presente artículo está basado en una investigación cuantitativa, llevada a cabo por los autores, la cual tuvo como objetivo principal determinar las competencias profesionales y emprendedoras con las que debe contar el estudiante de la electiva en Traducción de la Universidad EAN, al finalizar su programa académico, para estar en la capacidad de responder con calidad, emprendimiento y compromiso a la demanda laboral de compañías colombianas y extranjeras.

Focused Ultrasound (FUS) coupled with intravenous administration of microbubbles (MB) is a non-invasive technique that has been shown to reliably open (increase the permeability of) the blood-brain barrier (BBB) in multiple in vivo models including non-human primates (NHP). This procedure has shown promise for clinical and basic science applications, yet the safety and potential neurological effects of long term application in NHP requires further investigation under parameters shown to be efficacious in that species (500 kHz, 200-400 kPa, 4-5 μm MB, 2 minute sonication). In this study, we repeatedly opened the BBB in the caudate and putamen regions of the basal ganglia of 4 NHP using FUS with systemically-administered MB over 4-20 months. We assessed the safety of the FUS with MB procedure using MRI to detect edema or hemorrhaging in the brain. Contrast enhanced T1-weighted MRI sequences showed a 98% success rate for openings in the targeted regions. T2-weighted and SWI sequences indicated a lack edema in the majority of the cases. We investigated potential neurological effects of the FUS with MB procedure through quantitative cognitive testing of' visual, cognitive, motivational, and motor function using a random dot motion task with reward magnitude bias presented on a touchpanel display. Reaction times during the task significantly increased on the day of the FUS with MB procedure. This increase returned to baseline within 4-5 days after the procedure. Visual motion discrimination thresholds were unaffected. Our results indicate FUS with MB can be a safe method for repeated opening of the BBB at the basal ganglia in NHP for up to 20 months without any long-term negative physiological or neurological effects with the parameters used.

Modeling nanoscale capacitance presents particular challenge because of dynamic contribution from electrodes, which can usually be neglected in modeling macroscopic capacitance and nanoscale conductance. We present a model to calculate capacitances of nano-gap configurations and define effective capacitances of nanoscale structures. The model is implemented by using a classical atomic charge-dipole approximation and applied to calculate capacitance of a carbon nanotube nano-gap and effective capacitance of a buckyball inside the nano-gap. Our results show that capacitance of the carbon nanotube nano-gap increases with length of electrodes which demonstrates the important roles played by the electrodes in dynamic properties of nanoscale circuits.

Se determinaron las concentraciones de progesterona plasmática (P4) de 30 lechonas de reemplazo de cinco meses de edad y 15 cerdas gestantes de un rebaño comercial con manejo intensivo ubicado en el Estado Carabobo Venezuela, con la finalidad de evaluar la concentración de progesterona (P4) como hormona reproductiva y su influencia sobre la pubertad y la gestación de estas hembras porcinas, usando la técnica de enzimoinmunoanálisis (ELISA) como herramienta diagnóstica de la hormona. En el caso de las lechonas de reemplazo, las concentraciones de P4 para cada uno de los días del muestreo no presentaron significancia entre ellas (P > 0,05), observándose valores mínimos de 0,5 ng/ml y 30,0 ng/ml como valor máximo en la fase luteal del ciclo estral. Los valores promedios se presentaron dentro del rango 0,5 ng/ml y 9,62 ng/ml. El análisis de regresión lineal demostró la siguiente ecuación: Y=0,6329X-1,6276, con una correlación mediana entre los días del muestreo y las concentraciones de P4 en ese momento (R2 0,5219). La curva general de P4 demostró en las dos terceras partes iniciales del período de muestreo, una escasa actividad cíclica ovárica (≤  1,0 ng/ml de P4); sólo a partir del último tercio del muestreo (7½ meses de edad) se observa significancia en la misma (P < 0,05) denotando inicio de actividad cíclica ovárica (pubertad) (>2 ng/ml hasta 10 ng/ml de P4), donde la actividad luteal fue más rítmica con ciclos amplios, diferenciándose claramente la fase luteal vs fase folicular del ciclo estral, indicando actividad regular incipiente con ciclos regulares. Con este dato fue mejorado el manejo de estas lechonas de reemplazo, ubicándolas al servicio a una edad más adecuada y temprana que la usada tradicionalmente en la granja. En cerdas gestantes se obtuvieron valores máximos promedio de 29,4 ng/ml y mínimos de 1,2 ng/ml post parto. La regresión lineal para este caso fue Y = -2,3691X+30,017, con una correlación mediana entre la concentración de P4 y el día del muestreo (R2 0,307). La curva de P4 se presentó con valores altos mantenidos hasta el parto (valor más bajo de P4) observándose un período parto concepción de 45 días, en este caso el período fue adecuado. Se obtuvo baja tasa de mortalidad embrionaria. Se recomienda evaluar hormonalmente por un período más largo las lechonas de reemplazo para poder asegurar un plan de manejo óptimo en las mismas en diferentes épocas del año. Â

Introduction: Metastatic Crohn's disease (MCD) is an extraintestinal manifestation of Crohn's disease, with biopsy as fundamental diagnostic tool. There are few References to MCD in children, with a 0.5-1% estimated incidence in adults. There is no consensus about its therapeutic approach. We describe our diagnostic and therapeutic experience in MCD. Case Reports: Four cases of MCD are described in our Pediatric Gastroenterology Unit in a tertiary care hospital. The age at diagnosis was between 7 and 13 years. Lesions appeared before the diagnosis of Crohn's disease in three of them, and during the course of the disease in another one, with genital location in three patients and bilateral pretibial region in the other. All four cases demonstrated non-caseificant granulomas on biopsy. Only two patients used exclusive enteral nutrition therapy with complete resolution, while other two cases received a combination of therapies (corticosteroids, azathioprine, tacrolimus, infliximab and adalimumab) because of recurrence. Only one case required surgery after poor clinical control. Discussion: The MCD is infrequent but must always be included in the differential diagnosis of cutaneous lesions in Crohn's disease, considering it could be the debut of the disease. We will rely on biopsy anyway for definitive diagnosis. In this series the genital region is verified as the most commonly affected in children. The therapeutic approach does not differ from the management of intestinal involvement.

Intoxications in pediatric age represent a frequent cause of visit to the hospital emergency unit. Herb-made products can be toxic for the infant. The neurotoxic properties of the star anise (Illicium verum) have been widely described, although it is a classic product used to treat the infantile colic. Hepatic failure due to the consumption of anise herb elaborated infusions is presented as an exceptional finding in our environment. A case of a 4-month-old infant with hypertransaminasemia, severe coagulopathy, non ketotic hypoglycemia, moderated metabolic acidosis and neurologic symptoms such as seizures and nistagmus is described. After discarding infectious, metabolic and autoimmune etiology and through a meticulous anamnesis, the family referred having administered in the last two months a daily star anise and green anise (Pimpinella anisum) infusion to the patient. It is important to emphasize the serious risk of administering homemade herb infusions to infants.
