Nowadays, 70% of patients in Europe and the USA are affected by a p16 + , potentially HPV driven oropharyngeal squamous cell carcinoma. However, despite the improved survival rate in this group, the quality-of-life remains low in cases which neck dissection took place. In this vein, in recent years, some surgeons have considered to avoid dissection of level IIB, proposing a supra-selective non-IIb neck dissection.A retrospective, longitudinal, multicentric study was conducted, including patients with pathologically confirmed primary HPV + or HPV - OPSCC who went through surgical treatment for the primary lesion and neck dissection.141 patients were included. Among them, 99 (70.2%) were male and 42 (29.8%) were female. The mean age was 62 ± 9 years (range 36-81). The most frequent anatomical location was the tonsil in 63 (44.7%) of patients. The most common approach was the classic transoral oropharyngectomy in 51 (36.2%) patients. Immunohistochemistry for p16 was positive in 62 (44%) patients. One-hundred and five (74.5%) patients received a unilateral ND, and a 36 (25.5%) a bilateral ND. Of those, a 12.8% (18/141) of patients were level IIb LN + . According to our results, level IIb ND should be considered in patients underwent therapeutic ND with positive LN metastasis in level IIa (OR = 9.83; 95% CI 3.463-27.917) or III (OR = 6.25; 95% CI 2.158-18.143), advanced (T3/T4) oropharyngeal primary tumors (OR = 3.38; 95% CI 1.366-8.405), and patients with ENE (OR = 6.56; 95% CI 2.182-19.770), regardless of p16 status.According to our results, level IIb ND should be considered in patients who underwent therapeutic ND with positive LN metastasis in level IIa or III, advanced oropharyngeal primary tumors, and patients with ENE, independently of p16 status. Prospective data are necessary to definitively ensure the safety of omitting ipsilateral or contralateral level IIb ND in cN - patients with early stage disease.

The gracilis muscle free flap has gained popularity in head and neck reconstruction due to minimal donor-site morbidity, reliable vascular pedicle, strong muscular component, and possibility to perform nerve coaptation. However, almost all the existing evidence in the literature is related to its use for facial palsy reanimation. The aim of this study was therefore to review and provide a comprehensive summary of all the possible indications and outcomes of this versatile free flap in head neck reconstructive surgery.A systematic review of the literature was conducted including articles from 1970 to 2019. All articles were examined and described.Twenty-seven papers published between 1994 and 2019 were identified for analysis. The evidence highlights the use of the gracilis muscle free flap for parotid, forehead and midface defects, oral tongue, oral sphincter, lower and upper lip, cheek, and oral commissure defects, among others, as the most common defects reconstructed.This flap represents an easy to harvest and versatile free flap with low donor-site morbidity and multiple proven uses in headneck reconstruction. We therefore encourage reconstructive surgeons to include this flap in their armoury, either as a first or as a second-line option.

Introduction Oral cavity squamous cell carcinoma (OCSCC) is the most common malignancy in the oral cavity. Two types of mandibular resections have been described: the segmental mandibulectomy and the marginal mandibulectomy. Both may have a different impact over the quality of life, oncological prognosis, and functional or aesthetic result. Objectives The aim of this study was to systematically explore the literature to determine the survival outcomes and disease control rates in patients who underwent segmental or marginal mandibulectomy for OCSCC with histological evidence of cortical and medullary bone invasion. Data Synthesis This review involved a systematic search of the electronic databases MEDLINE/PUBMED, Google Scholar, Ovid Medline, Embase, and Scopus including articles from 1985 to 2019. Fifteen articles were included for qualitative analysis and 11 articles were considered for meta-analysis calculations. All of them correspond to retrospective cohort studies. Conclusion This systematic review reveals the low-level evidence regarding the impact over local control or survival according to the type of mandibulectomy. Our results need to be considered with precaution according to the limited evidence available. We just found difference regarding the 5-year disease-free survival, and a tendency in favor of segmental mandibulectomy was confirmed when medullary invasion was evident.

Over the last decade, technological growth has developed new devices for more precise surgery based on improved maneuverability, minimally invasive approaches, and magnification of the operating field. In this context, the exoscope has opened a new phase for more accurate and safer microsurgery, improving the perception of the volume of objects and the depth of structures for planning, targeting, and controlling fine movements. The exoscope could be used for middle ear, transcanal, transmastoid, and craniotomy procedures that require two-handed dissection, both to perform both totally VITOM-based techniques and coupled to traditional procedures with an operating microscope or endoscope. In addition, the VITOM 3D system allows the surgeon to work with high-definition images, which is essential in facial nerve surgery or submandibular salivary stone or tear surgery approaches, where magnification plays a fundamental role in surgical success and in reducing operating times. The 3D exoscope approach could also be included in traditional transoral procedures for oropharyngeal carcinoma. The exoscope may provide a relevant approach in teaching surgeons and nurses, allowing adequate training in non-oncological surgical procedures such as a tonsillectomy or lateral pharyngoplasty.

Resumen La cirugia transoral de la orofaringe ha presentado una importante evolucion en los ultimos anos, al ampliarse las opciones quirurgicas disponibles (TORS, TOUSS, TOLS), la capacidad de resecar lesiones cada vez mas extensas y el numero de pacientes que se beneficia de ellas. Este hecho ha provocado una evolucion en la reconstruccion de los defectos quirurgicos tras cirugia transoral. Este articulo tiene como objetivo la revision de las posibles tecnicas reconstructivas, las indicaciones y factores que tener en cuenta previamente a la cirugia, ademas de proponer un protocolo para valorar la necesidad de reconstruccion transoral sobre la base de la profundidad del defecto, la estadificacion y el tratamiento previamente recibido.

Headneck surgery encompasses a variety of surgical approaches for benign and malignant conditions. Due to the complexity in treating patients with head and neck pathology, it is necessary to adhere to basic surgical principles to decrease complications. Among them, surgical site infection can be prevented using a surgery quality protocol including the correct use of antibiotics and optimization of nutritional status.A survey was sent through the YO-IFOS and SEORL-CCC international mailing list.A total of 435 surgeons completed the survey. Of the respondents, 97.7% confirm that they scrub their hands before surgery, 40.9% respondents recommend nutritional support according to sign and symptoms, 60.9% use of antibiotic prophylaxis in clean surgery and just 9.2% use clindamycin in combination.This survey has broadened the scope regarding HN surgical safety around the globe. Identifying innovative ways in which surgical care may be improved is mandatory.

Introduction: Radial forearm free flap (RFFF) and supraclavicular artery island flap (SCAIF) are some of the most common fasciocutaneous flaps used for head and neck (H&N) reconstruction. Material and methods: A retrospective analysis of clinical data and outcomes of 31 consecutive patients who underwent H&N reconstruction using either SCAIF or RFFF over a three-year period, aiming to compare the surgical outcomes of the SCAIF and the RFFF in H&N reconstruction. Results: Thirty-two flaps were performed in 31 patients (17 SCAIFs and 15 RFFFs). There was no difference in patient demographics between both groups. Hospital stay was longer in the SCAIF group (30.7 ± 18.2 days (min: 9/max: 60) versus 19.2 ± 15.8 days (min: 7/max: 72). Patients who underwent reconstruction with a SCAIF had shorter reconstructive procedure time; 74.4 min (min: 60/max: 93) versus 147.8 min (min: 140/max: 187). Overall morbidity was not significantly different (SCAIF 52.7% vs RFFF 39.9%, p = NS). Global flap survival was higher without statistical significance in the RFFF group (100%) versus the SCAIF group (70.7%). Conclusion: Despite the advantages related to the use of SCAIF like regarding the time spent in the reconstructive procedure. In our experience, the RFFF continues to be the most successful technique with similar perioperative outcomes and fewer complication rates. In this vein, both techniques can be reasonably used to reconstruct post-ablative H&N defects. However, in our experience, the use of SCAIF may lengthen hospital length of stay probably due to the augmented risk of flap failure.

International audience; We investigated safety and efficacy of transoral robotic surgery (TORS) for base of tongue (BOT) reduction in obstructive sleep apnea syndrome (OSAS) patients. PubMed, Cochrane Library, and Scopus were searched. A meta-analysis was performed. Random effects models were used. Thirty-one cohorts met our criteria (1693 patients). The analysis was based mostly on retrospective studies. The summary estimate of the reduction of Apnea–Hypoxia Index (AHI) was 24.25 abnormal events per hour (95% CI: 21.69–26.81) and reduction of Epworth Sleepiness Scale (ESS) was 7.92 (95% CI: 6.50–9.34). The summary estimate of increase in lowest O2 saturation was 6.04% (95% CI: 3.05–9.03). The success rate of TORS BOT reduction, either alone or combined with other procedures, was 69% (95% CI: 64–79). The majority of studies reported low level of evidence but suggested that TORS BOT reduction may be a safe procedure associated with improvement of AHI, ESS, and lowest O2 saturation.

There are no previously described training models for learning or teaching how to remove lithiasis from the salivary ducts. Therefore, we present a new simulation model to enable us to faithfully represent the process of endoscopic lithiasis extraction by sialoendoscopy.A simulation model was developed using a pig's head, omeprazole spheres were used to simulate lithiasis in the various ducts of each salivary gland and a Dormia basket was used to train in extraction of the lithiasis model.Twenty-seven residents in training and/or young specialists were successfully trained in this technique using this model. Twenty-six (96.3%) considered the model useful for training in the use of baskets; all of them were able to capture the omeprazole sphere in the salivary duct. A satisfaction rate of 92.25 out of 100 points was obtained through an anonymous survey.We describe a novel simulation model using omeprazole spheres, which allows the surgeon to practice how to diagnose and treat obstructive pathology of the salivary glands in a risk-free environment guaranteeing the reproducibility of the technique in conditions similar to those of normal practice.

Desmoplastic small round cell tumor represents an unusual variety of tumors that affects mostly children and young males. In this report we present a case of a desmoplastic small round cell tumor involving the parotid gland of a young male. The tumor was excised, and definitive histology showed features of desmoplastic small round cell tumor confirming the diagnosis.

Resumen Introduccion El carcinoma sarcomatoide puede aparecer en cualquier parte del cuerpo, siendo las glandulas salivales mayores su principal localizacion en cabeza y cuello, pero en la laringe representa aproximadamente un 1%. Cuenta con componentes epiteliales y mesenquimales, lo que ha llevado a plantear multiples teorias acerca de su origen. Por esto su diagnostico anatomopatologico puede ser un reto. Caso clinico Presentamos un caso clinico de un varon de 76 anos fumador que consulta por disfonia. Se observa una lesion en cuerdas vocales sin adenopatias ni metastasis. Se le realiza microcirugia laringea con escision completa de la lesion, y el diagnostico anatomopatologico es de carcinosarcoma, mostrando positividad intensa y difusa para vimentina y focal para AE1-AE3, CK5 y p63. El paciente recibe tratamiento complementario con radioterapia. Discusion El carcinoma sarcomatoide tiende a manifestarse con sintomas obstructivos como la disfonia. Su pronostico depende del estadio y de si hay o no metastasis. Suelen ser positivos los marcadores epiteliales citoqueratina (AE1-AE3), antigenos de membrana epitelial (EMA), Ki 67 y marcadores mesenquimales como vimentina, desmina y S-100. En cuanto al tratamiento, se recomienda de entrada una biopsia por escision seguida o no de radioterapia complementaria, aunque la radioterapia sola tambien ha tenido exito (T2-T1). En estadios T3-T4 pueden ser tratados con reseccion local, laringectomia parcial, total con o sin vaciamiento, seguida de radioquimioterapia concomitante.

Three-dimensional (3D) printing is an additive manufacturing process. This technology provides us with the opportunity to create 3D structures by adding material on a layer-by-layer basis, using different kinds of materials such as ceramics, metals, plastics, and polymers. Nowadays, tissue engineering investigations are taking place on a widespread basis in the fields of regeneration, restoration, or replacement of defective or injured functional living organs and tissues. For this reason, it is important to understand the basic concept of 3D bioprinting as a tool for producing a 3D structure combining living cells and biomaterials and controlling cell proliferation, attachment, and migration within 3D structures. There are a variety of applications for additive manufacturing printing technology available to surgeons at this moment, like scaled models for preoperative planning based prosthetics or custom implants and biocompatible scaffolds. Moreover, this technology can be used as a tool to improve surgical and medical education, by using simulation models and utilising its potential to replicate complex anatomy by employing distinct materials that mimic the characteristics of the native tissue in an effort to increase patient safety through repetition of common procedures.

Introduction: There are different complications related to the submandibular gland resection, like damage to the mandibular branch of the facial nerve, lingual or hypoglossal nerve. Moreover, subjective complaints like dry mouth, altered sensation around the scar and taste alterations were described. Patients and methods: A prospective, non-randomized study was performed including 20 patients treated with submandibular gland resection. Results: The most common etiology was sialolithiasis affecting 9 (45%) patients. One (5%) patient presented a lingual nerve paresis, 2 (10%) patients presented a transient marginal mandibular branch paresis and no hypoglossal nerve alteration was observed. Factors like the size of the lithiasis (P = 0.293), size of the tumor (P = 0.445) or type of pathology (P = 0.694) were not related with neurological complications. Altered sensation in the skin around a scar was the most severely weighted problem (55% and 33.3%), and xerostomia was the second one (33% and 27.3%) at one month and at six months after surgery, respectively. Conclusion: The submandibular gland resection remains to be a safe technique with a low rate of surgical and neurological complications. However, patients should be informed about sequelae like xerostomia, altered sensation in the skin or pain around the scar, which may be discomforting. The cosmetic result can be satisfactory, but a surgical scar can be avoided with another approach. Although rare, the risk of MBFN, lingual and hypoglossal nerve damage should always be included in the preoperative information on surgical risks.

Objective Cochlear implants (CI) are electronic devices that enable the auditory rehabilitation and the management of individuals with severe to profound bilateral hearing loss, and nowadays, advanced age is not considered a contraindication for cochlear implantation and several studies have shown that older adults do benefit from CI, with improvements in hearing abilities and quality of life. Methods Retrospective analysis of patients older than 18 years who underwent cochlear implant surgery in a tertiary academic centre. Results 57 patients met the inclusion criteria: 25 (43.9%) male and 32 (56.1%) female. Total percentage of minor complication was 24.6% and major complication was 17.5%. The most common minor complication in our series was vestibular disorder, and the most common major complication was device failure. No correlation was found among age, previous meningitis, anatomical variables or comorbidities with the appearance of complications. Conclusion Cochlear implantation is a safe surgical technique for rehabilitation of severe to profound sensorineural hearing loss. According to our results, neither the age over 65 years nor the presence of comorbidities does have a direct impact over the complication rates in our patients.

Resumen Introduccion En el 80% de los casos de infeccion por Mycobacterium tuberculosis existe afectacion pulmonar, sin embargo hasta en un 20% de casos puede haber compromiso extrapulmonar. En el area otorrinolaringologica la localizacion mas frecuente es la linfadenitis cervical, que afecta aproximadamente al 95% de los casos. Materiales y metodos Estudio retrospectivo en pacientes que acudieron a consulta de ORL en un hospital terciario por sintomatologia en cabeza y cuello y que fueron diagnosticados de tuberculosis, entre diciembre del ano 2007 y diciembre del ano 2013. Resultados Un total de 73 pacientes fueron incluidos, 41 (56,2%) hombres y 32 (43,8%) mujeres (ratio H/M = 1,28), con una edad promedio de 39,4 anos (±26,5 anos; min: 1/max 88). Un total de 53 (72,6%) casos correspondieron a linfadenopatia cervical, 11 (15%) a tuberculosis laringea, 3 (4,1%) a tuberculosis hipofaringea, y los restantes 6 en otras localizaciones. Del total, 14 (19,2%) pacientes eran VIH positivo y 10 (13,7%) tenian antecedentes de contacto con familiares que habian sufrido tuberculosis pulmonar. En 51 (69,8%) de los casos se realizo una PCR como medida de confirmacion, siendo positiva en 47 de estos (92,1%). Conclusion La similitud de la tuberculosis con enfermedades de mal pronostico y la dificultad que entrana su diagnostico hacen necesario tener en cuenta la tuberculosis a la hora de explorar a pacientes con lesiones granulomatosas o ulcerativas del area ORL. Al analizar nuestros resultados, la incidencia de tuberculosis segun su localizacion en el area ORL es similar a la reportada en la literatura. Es importante hacer mencion especial del uso de la PCR en nuestro estudio y las ventajas que su aplicacion significa para el diagnostico.

Introduction Pulmonary involvement exists in 80% of cases of infection with Mycobacterium tuberculosis; however, in up to 20% of cases there may be extra-pulmonary involvement. In the ENT area, the most common site is cervical lymphadenitis, affecting approximately 95% of cases. Materials and methods This was a retrospective study of patients attending an ENT department in a tertiary hospital for head and neck symptoms that were diagnosed with tuberculosis (TB), between December 2007 and December 2013. Results The study included 73 patients, 41 (56.2%) males and 32 (43.8%) females (M/F ratio=1.28), with a mean age of 39.4 years (±26.5 years; Min 1, Max 88). There were 53 (72.6%) cases of cervical lymphadenopathy, 11 (15%) of laryngeal tuberculosis 3 (4.1%) of hypopharyngeal tuberculosis, and six cases in other locations. Of the total, 14 (19.2%) patients were HIV positive and 10 (13.7%) had a history of contact with relatives who had suffered pulmonary tuberculosis. PCR was performed in 51 (69.8%) cases to confirm being positive, of which 47 (92.1%) cases were. Conclusion The similarity of tuberculosis to diseases of poor prognosis and the difficulty of its diagnosis make considering tuberculosis necessary when exploring patients with ulcerative or granulomatous ENT lesions. Analysing our results, the incidence of TB according to its ENT area location is similar to that reported in the literature. It is important to make special mention of the use of PCR in our study and the benefits that its implementation means for diagnosis.

Introduction The emergence of flexible nasofibrolaryngoscopy in recent decades has facilitated the development of Otolaryngology and the nasofibrolaryngoscope has become an essential diagnostic tool for the otolaryngologist. However, its use is not without discomfort for the patient, which is why various options for topical anaesthesia have been proposed during the development of the technique. Materials and methods This was a prospective, double blind, crossover study in patients with no history of nasal disease, through which we compared the use of topical anaesthetic (Lidocaine) + epinephrine, oxymetazoline and placebo. Using a visual analogue scale, the degree of discomfort was assessed during each scan. Results We evaluated 18 patients, 10 women (55.6%) and 8 men (44.4%), with a mean age of 28 ± 4 years. The degree of disturbance generated by the scan with the application of lidocaine–epinephrine was 1.94; with oxymetazoline it was 3.78, and with placebo, 4.61. When the lidocaine–epinephrine solution was compared with oxymetazoline, there was a statistical significance in favour of lidocaine–epinephrine ( P P Conclusions In this study we demonstrated that using lidocaine associated with epinephrine as a topical anaesthetic prior to flexible nasofibrolaryngoscopy decreases pain and feeling of discomfort for the patient. Consequently, we can recommend the use of topical nasal anaesthesia when performing this exploration.

ResumenHoy en día la tuberculosis es una de las más importantes infecciones reemergentes en el mundo. Hasta un 20% de las formas de presentación son extratorácicas. Sin embargo, la presencia de tuberculosis a nivel parotídeo es infrecuente incluso en países subdesarrollados, donde la incidencia de esta enfermedad es mayor. Presentamos un paciente de 83 años que acude por tumoración en región parotídea de 4 semanas de evolución sin antecedentes personales. La exploración clínica revela una masa en cola de parótida izquierda de aproximadamente 3×2cm, no dolorosa a la palpación y sin evidencia de adenopatías a nivel cervical. Tras el estudio radiológico y punción con aguja fina, no se obtienen datos concluyentes, por lo que se procede a realizar exéresis. Se envía muestra para estudio de PCR, se confirma diagnóstico de tuberculosis parotídea. Tras recibir quimioterapia antituberculosa complementaria, el paciente es dado de alta por remisión de la enfermedad.AbstractTuberculosis is currently one of the most important re-emerging infections worldwide. Up to 20% of cases are extra-thoracic. However, the presence of parotid tuberculosis is uncommon even in developing countries where the incidence of this disease is higher. The case of an 83 year-old patient is presented who had a tumour in the parotid region of 4 weeks onset, and with no previous personal history. Clinical examination revealed a mass of approximately 3×2cm in the left parotid. It was not painful on palpation and there was no evidence of cervical lymph node involvement. After inconclusive radiological examination and fine needle aspiration, an excision of the lesion was performed, and the specimen sent for a PCR study. This confirmed the diagnosis of parotid tuberculosis, and after receiving additional tuberculosis treatment the patient was discharged due to remission of the disease.

BACKGROUND. Functional endoscopic sinus surgery (FESS) is a reliable option in the treatment of sinus pathology, but the presence of the anatomical variant and difficult cases like massive polyposis or revision FESS can generate some problems to surgeons. MATERIAL AND METHODS. After performing an unciformectomy, a partial anterior ethmoidectomy and maxillary ostium antrostomy, we slide a cottonoid back to the basal lamella of the middle turbinate with a Cottle dissector and introduce it in the superior meatus. After that, we return to the middle meatus and proceed to open the basal lamella finding the cottonoid placed there previously. RESULTS. An easy technique, safe and reproducible, that allows us to advance in our dissection, avoiding damaging important structures. CONCLUSION. In this paper we present a safe way to approach the posterior ethmoidal cells complex in the classic way through the basal lamella of the middle turbinate, under the guidance of a cottonoid, a safe and easy maneuver to do this procedure in the beginning of our formation or in complex cases.

Transoral surgery of the oropharynx has seen an important evolution in recent years, expanding the surgical options available (TORS, TOUSS, TOLS). The capacity to resect increasingly extensive lesions and the number of patients who benefit from them. This fact has led to an evolution in the reconstruction of surgical defects after transoral surgery. This article aims to review the possible reconstructive techniques, indications and factors to be taken into account prior to surgery, proposing a protocol to assess the need for transoral reconstruction based on the depth of the defect, the staging, and previous treatment.

Introduction: Carotid blowout syndrome is an uncommon complication for patient treated by head and neck tumors, and related to a high mortality rate. Objective: The aim of this study was to study the risk of carotid blowout in a large cohort of patients treated only by larynx cancer. Methods: Retrospective analysis of patients older than 18 years, treated by larynx cancer who developed a carotid blowout syndrome in a tertiary academic centre. Results: 197 patients met the inclusion criteria, 192 (98.4%) were male and 5 (1.6%) were female. 6 (3%) patients developed a carotid blowout syndrome, 4 patients had a carotid blowout syndrome located in the internal carotid artery and 2 in the common carotid artery. According to the type of rupture, 3 patients suffer a type I, 2 patients a type III and 1 patient a type II. Five of those patients had previously undergone radiotherapy and all patients underwent total laryngectomy. We found a statistical correlation between open surgical procedures (p = 0.004) and radiotherapy (p = 0.023) and the development of a carotid blowout syndrome. Conclusion: Carotid blowout syndrome is an uncommon complication in patients treated by larynx tumours. According to our results, patient underwent radiotherapy and patients treated with open surgical procedures with pharyngeal opening have a major risk to develop this kind of complication. Resumo Introdução: A síndrome da ruptura da carótida é uma complicação incomum no paciente em tratamento para tumores de cabeça e pescoço, relacionada com uma alta taxa de mortalidade. Objetivo: O objetivo deste estudo foi estudar o risco de ruptura da carótida em uma grande coorte de pacientes tratados isoladamente por um câncer de laringe. Método: Análise retrospectiva de pacientes com mais de 18 anos, tratados por câncer de laringe em um centro de assistência terciária, que desenvolveram a síndrome da ruptura da carótida. Resultados: Ao todo, 197 pacientes atenderam aos critérios de inclusão, 192 (98,4%) eram do sexo masculino e 5 (1,6%) eram do sexo feminino. 6 (3%) desenvolveram síndrome da ruptura da carótida, 4 tiveram síndrome da ruptura da carótida localizada na artéria carótida interna e 2 na artéria carótida comum. De acordo com o tipo de ruptura, 3 pacientes apresentaram síndrome da ruptura da carótida tipo I, 2 pacientes, síndrome da ruptura da carótida Tipo III e um tipo II. Cinco desses pacientes haviam sido previamente tratados com radioterapia e todos os pacientes foram submetidos a laringectomia total. Encontrou-se uma correlação estatística entre procedimentos cirúrgicos abertos (p = 0,004) e radioterapia (p = 0,023) e o desenvolvimento de síndrome da ruptura da carótida. Conclusão: A síndrome de ruptura da carótida é uma complicação rara em pacientes tratados para tumores de laringe. De acordo com nossos resultados, pacientes submetidos a radioterapia e pacientes tratados com procedimentos cirúrgicos abertos com abertura da faringe apresentam um risco maior de desenvolver essa complicação.

Introduction Squamous cell carcinoma of the larynx is currently the second most common malignancy of the airway after lung cancer, and hypopharyngeal cancer accounts for fewer than 5% of head and neck cancers. The nonsurgical options for patients with this disease are related to significant long-term toxicities and the need for persistent tracheostomy, which adversely affects the quality of life of these patients. Objective To evaluate the need for tracheostomy, and the influence of this in the overall and specific survival rates of patients diagnosed with all stages of laryngeal carcinoma treated by chemoradiotherapy. Methods A retrospective study of patients diagnosed with laryngeal carcinoma was performed according to the criteria of the Union for International Cancer Control (UICC) and the American Joint Committee on Cancer (AJCC) 7th edition, in a tertiary hospital. Results A total of 21 patients were evaluated, 8 patients required a tracheotomy (31%) during the treatment protocol, 7 (35%) men and 1 (100%) women. According to subsite 4/4 patient with glottis cancer (p ≤ 0.001), 2/10 patients with supra glottis cancer and 2/7 patients with hypopharyngeal cancer. During follow up, just in 1 patient was possible to close the tracheostomy. Conclusion Persistent tracheostomy dependence after primary chemoradiation increases significantly the morbidity, and decreases the quality of life of those patients. Patients with glottis cancer are prone to need a tracheostomy, but no statistical difference regarding the oncological stage and the need for a tracheostomy were detected. A more thorough selection of the patients is needed to improve the quality of life and reduce permanent tracheostomy dependence.

Introduction Pharyngocutaneous fistula is the most frequent complication after total laryngectomy. Its incidence varies between 9%–25% in post primary total laryngectomy patients, to 14%–57% in salvage laryngectomy post radiotherapy or post chemotherapy+radiotherapy. The pectoralis major myofascial flap (PMMF) is postulated as a useful tool to decrease the incidence of this complication. Materials and method Retrospective analysis of a group of patients treated by salvage laryngectomy, associated or not with pharyngeal closure reinforcement with PMMF. Results Twenty patients were included, 18 males (90%) and 2 females (10%), in 10 of whom the PMMF was used. The average age was 66.65 years. Seventeen (85%) had a laryngeal tumour and 3 (15%) had a hypopharyngeal tumour. Eight (80%) patients in the non-PMMF group had postoperative fistula, whereas only 2 (20%) patients in the PMMF group had a fistula during the postoperative period (P=.005). The mean time for fistula closure was significantly shorter in the cases where PMMF flap was used (16±11 days vs. 76.8±67 days, P=.001), as was hospital stay (19.6±18 days vs. 83.9±77 days, P=.001). Conclusion The use of PMMF in our series is associated with a lower rate of post salvage laryngectomy fistulas in patients treated primarily by organ preservation protocol for laryngeal/hypopharyngeal cancer. In turn, it promotes local healing by decreasing the mean duration of fistula closure and the mean hospital stay.

BACKGORUND. Simulation models have slowly been gaining a place in training and surgical skills, before attending the operating theatre. This paper presents a model that allows the trainee to practise the functional endoscopic sinus surgery (FESS) training in a safe, non-patient facing environment. MATERIAL AND METHODS. The method can be practised away from the operating theatre and the skills developed can then be transferred to the operating theatre. To create our FESS simulator, we use a cardboard tube and sheet, scissors, markers, cutter, foam, needles, catheters, plasticine, an inflatable balloon, a 10g lumbar puncture needle, one 5 cc syringe, two Blakesley forceps (straight and angled) and a 0° and 30° Hopkins rigid optics. In this simulator you can practise a puncture of the inferior turbinate, the endoscopic depth, remove a polyp, balloon dilation, a partial ethmoidectomy. RESULTS. We describe a cheap and easy to build FESS training model, which allows the resident to practise this technique in a risk-free environment, while guaranteeing the reproducibility of the technique under similar conditions. CONCLUSION. The high cost of digital simulators does not allow possible spreading this technology in every department. For that reason, the purpose of this paper is the acquisition of a set of skills that allow the resident to go to the operating room with clear concepts about the basic techniques.

Acronyms and abbreviations are frequently used in otorhinolaryngology and other medical specialties. CO2 laser-assisted transoral surgery of the pharynx, the larynx and the upper airway is a family of commonly performed surgical procedures termed transoral laser microsurgery (TLM). The abbreviation TLM can be confusing because of alternative modes of delivery. Classification and definition of the different types of procedures, performed transorally or transnasally, are proposed by the Working Committee for Nomenclature of the European Laryngological Society, emphasizing the type of laser used and the way this laser is transmitted. What is usually called TLM, would more clearly be defined as CO2 laser transoral microsurgery or CO2 TOLMS or CO2 laser transoral surgery only (with a handpiece) would be defined as CO2 TOLS. KTP transnasal flexible laser surgery would be KTP TNFLS. Transoral use of the flexible CO2 wave-guide with a handpiece would be a CO2 TOFLS. One can argue that these clarifications are not necessary and that the abbreviation TLM for transoral laser microsurgery is more than sufficient. But this is not the case. Laser surgery, office-based laser surgery and microsurgery are frequently and erroneously interchanged for one another. These classifications allow for a clear understanding of what was performed and what the results meant.

Introduction Transoral laser microsurgery (TLM) has established itself as an effective option in the management of malignant tumors of the glottis, supraglottis, and hypopharynx. Nonetheless, TLM is not a harmless technique. Complications such as bleeding, dyspnea, or ignition of the air may appear in this type of surgery. Objective The aim of this study is to describe the complications that occurred in a group of patients treated for glottic and supraglottic carcinomas in all stages by TLM. Methods This study is a retrospective analysis of patients diagnosed with squamous cell carcinoma of the glottis and supraglottis for all stages (T1, T2, T3, T4), N -/ + , M -/+ treated with TLM between January 2009 and March 2012 in a tertiary hospital. Results Ninety-eight patients met the inclusion criteria, which had undergone a total of 131 interventions. Ninety-four (95.9%) patients were male and 4 (4.1%) were female. The mean age was 64.2 years (± 10.7 years = min 45; max 88). The presence of intraoperative complications was low, affecting only 2% of patients. Immediate postoperative complications occurred in 6.1%, whereas delayed complications affected 13.2% of patients, without any of them being fatal. Conclusion TLM has shown good oncologic results and low complication rate compared with traditional open surgery during intervention, in the immediate and delayed postoperative period and in the long-term with respect to radiotherapy.

Resume Introduction La mise en place d’un drain de tympanostomie est peut-etre la procedure ambulatoire la plus frequente en oto-chirurgie, et la premiere qu’entreprend le jeune oto-chirurgien. Elle demande un grand savoir-faire et ce n’est qu’a la suite d’une courbe d’apprentissage exigeante que le jeune medecin sera pret a la mettre en œuvre. Note technique Nous decrivons un simulateur fait-maison pour la mise en place de drains de tympanostomie. Il comporte une porte-seringue en bois et une piece de latex qui simule la membrane tympanique. Resultats Le simulateur est facile et peu couteux a construire. Il permet la simulation pratique et sans risque de la mise en place de drains de tympanostomie. La technique est reproductible et ainsi permet au chirurgien en formation de s’entrainer a l’ecart des patients. Conclusion Aucun simulateur ne reproduit parfaitement les conditions de la chirurgie reelle ; neanmoins, le Vigo Grommet Trainer est un tres bon moyen d’acquerir et de developper le savoir-faire necessaire a la mise en place des drains de tympanostomie au bloc operatoire.

Introduction Transtympanic grommet placement is perhaps the most common otologic outpatient procedure and is the junior resident's first step in otologic surgery. Drain placement requires a high level of skill and only after painstaking practice, will the young physician be prepared to perform the procedure. Technical note We describe a home-made training model for grommet placement, consisting of a wooden base holding a syringe, with a sheet of latex simulating the tympanic membrane. Results The model is cheap and easy to build. It allows ear tube (grommet) placement to be simulated in a practical and risk-free manner. The technique is reproducible, allowing the young physician to develop his or her skills without patient involvement. Conclusion Although a simulator cannot perfectly replicate surgery, the Vigo grommet trainer is an excellent tool to provide valuable practice in acquiring and developing the skills needed to perform drain placement in the operating theater.

The treatment of supraglottic carcinoma remains a controversial issue. Five accepted surgical and non-surgical oncological treatments have been currently established: standard horizontal supraglottic laryngectomy (HSL), supraglottic CO2 laser microsurgery (TLM), transoral robotic surgery, radiotherapy alone and radiotherapy in combination with chemotherapy. Some studies have shown that complications of head and neck surgeries increase significantly in patients over 65 years compared to younger patients. We designed a retrospective analysis to assess the rate of complications and functional outcomes of patients treated by TLM and HSL in cases of T1-T3 supraglottic squamous cell carcinomas (SCC) in a tertiary University Hospital. Results were compared between patients younger and older than 65 years. We found significant differences in the rate of aspiration pneumonia (p = 0.026), mean time to decannulation (p = 0.001) and mean hospital stay (p = 0.007) in patients treated by TLM, which was higher and longer in the group of patients over 65 years of age. Regarding HPL, we only found significant differences in the mean time to decannulation (p = 0.001), which was longer in the group of patients younger than 65 years. According to our results, TLM or HPL can both be a safe surgical option for patients older than 65 years, but previous evaluation of lung function before surgery is mandatory because of an increased risk of aspiration pneumonia in patients with lung problems, especially when treated by TLM. Concerning functional outcomes in patients older than 65 years, TLM reduces the postoperative rate of tracheostomy, mean time required for decannulation and mean hospital stay compared with HPL. However, no significant difference in the occurrence of aspiration pneumonia, dysphagia or in the mean length of NGT feeding was found.Il trattamento dei carcinomi sopraglottici rappresenta al momento una problematica controversa. Al momento sono disponibili 5 differenti opzioni di trattamento: la tradizionale laringectomia orizzontale sopraglottica (HSL), la laringectomia sopraglottica con laser CO2 (TLM), l’approccio mediante chirurgia robotica, la radioterapia e la radioterapia in combinazione con la radiochemioterapia. Alcuni studi hanno evidenziato come il rate di complicanze della chirurgia del distretto testa collo aumenti significativamente nei soggetti al di sopra dei 65 anni. Abbiamo pertanto voluto effettuare uno studio retrospettivo che confrontasse i risultati funzionali e il rate di complicanze nei pazienti al di sotto e al di sopra dei 65 anni, affetti da carcinomi squamocellulari sopraglottici T1-T3, sottoposti a laringectomia orizzontale sopraglottica tradizionale (HSL) o a TLM presso un Polo ospedaliero Universitario di terzo livello. Il gruppo di pazienti al di sopra dei 65 anni sottoposto a TLM ha mostrato differenze statisticamente significative per quanto riguarda il rate di polmonite ab ingestis (p = 0.026), tempo medio necessario al decannulamento (p = 0.001) e tempo medio di ricovero ospedaliero (p = 0.007) tutti risultati più lunghi rispetto a quelli osservati nei pazienti al di sotto dei 65 anni. Per quanto riguarda la chirurgia open, abbiamo rilevato una differenza statisticamente significativa solo per il tempo medio necessario per il decannulamento (p = 0.001) risultato essere più lungo nel gruppo di pazienti più giovani di 65 anni. In accordo coi nostri risultati sia la TLM che l’approccio open rappresentano opzioni sicure nei pazienti al di sopra dei 65 anni, ma in cosiderazione dell’aumentato rischio di polmonite ab ingestis è mandatoria un adeguata valutazione della funzionalità polmonare preoperatoria, in particolare nei pazienti trattati con TLM. Nei pazienti al di sopra dei 65 anni inoltre la TLM rispetto all’approccio open riduce il rate di tracheostomie postoperatorie, il tempo medio necessario al decannulamento e il tempo medio di ricovero. Tuttavia non è stata rilevata alcuna differenza significativa in termini di rate di polmonite ab ingestis, disfagia o tempo medio di utilizzo del sondino naso gastrico.
