PURPOSE To generate and present the survey results on critical issues relevant to screening, diagnosis, and staging tools for prostate cancer (PCa) focused on developing countries. METHODS A total of 36 of 300 questions concern the main areas of interest of this paper: (1) screening, (2) diagnosis, and (3) staging for various risk levels of PCa in developing countries. A panel of 99 international multidisciplinary cancer experts voted on these questions to create recommendations for screening, diagnosing, and staging tools for PCa in areas of limited resources discussed in this manuscript. RESULTS The panel voted publicly but anonymously on the predefined questions. Each question was deemed consensus if 75% or more of the full panel had selected a particular answer. These answers are based on panelist opinion not a literature review or meta-analysis. For questions that refer to an area of limited resources, the recommendations consider cost-effectiveness and the possible therapies with easier and greater access. Each question had five to seven relevant answers including two nonanswers. The results were tabulated in real time. CONCLUSION The voting results and recommendations presented in this document can be used by physicians to support the screening, diagnosis, and staging of PCa in areas of limited resources. Individual clinical decision making should be supported by available data; however, as guidelines for screening, diagnosis, and staging of PCa in developing countries have not been developed, this document will serve as a point of reference when confronted with this disease.

A woman in her 50s presented with a rounded and hypervascular lesion in the right internal iliac lymph node chain, contacting with small branches of the anterior division of the internal iliac artery. Since the lesion matched the blood arterial pool in CT and the patient exhibited multiple vascular abnormalities that suggested segmental arterial mediolysis, a pseudoaneurysm hypothesis was initially made. Arteriography was realised due to the intention for embolisation of the pseudoaneurysm, but the dynamic behaviour during the exam suggested a hypervascular tumour more. An MRI was conducted, bringing new evidence, favouring the possibility of a neoplasm. The lesion excision was performed and sent to pathology. Morphological and immunohistochemical findings suggested a rare case of a fibroblastic reticular cell tumour of the internal iliac lymph node.

Pediatric mandibular tumors present an aggressive biological behavior and difficult diagnosis. A wide range of odontogenic and nonodontogenic tumors comprise the spectrum of these lesions. We report a case of a 1-year-old male child patient showing facial asymmetry symptomatic of an expansive lesion extending throughout the body and ramus of the left hemimandible with a diameter of 8 cm. The histopathological report suggested a high-grade mucoepidermoid carcinoma (MEC), recommending further immunohistochemical investigation of the ectomesenchymal or neuroectodermal origin of the tumor cells. The patient evolved with extensive bilateral pleural effusion followed by metastasis in the middle third of the right humerus, and died 2 months after the first biopsy procedure by acute renal failure with tubular necrosis, before a final inconclusive immunohistochemical report was reached. The lack of resources for less-favored regions of Brazil impairs rapid biomolecular examinations such as immunohistochemical resulting in delay of appropriate therapeutic procedures.

Mediastinal fat necrosis is an important differential diagnosis for acute chest pain in previously healthy patients. Imaging examination is essential to establish this diagnosis, as physical examination can be unhelpful and laboratory tests are non-specific. The treatment of choice is conservative, with non-steroidal anti-inflammatory drugs; surgery is reserved for a few selected cases. We present the case of a 37-year-old male patient with mediastinal fat necrosis, refractory to the conservative management and complicated by growing pleural effusion, which was treated surgically.

Objective: To evaluate the association between self-reported color and ancestry in Brazilian patients with breast cancer (BC). Methods: Ethics approval 1136/2016. This was an observational, transversal, epidemiological study, evaluating 1,215 patients with BC. DNA was extracted to evaluate ancestry. For genetic ancestry, a 46 AIM-INDEL panel was used, and the polymerase chain reaction (PCR) products were subjected to capillary electrophoresis. The ancestral profile was evaluated with Structure v.2.3.3 software, for ancestry proportion, the percentages of ancestry in the different self-referred colors. For this purpose, descriptive statistics was performed (mean ± standard deviation [minimum − maximum]). To assess differences between groups, ANOVA and Bonferroni were used. Objective: To evaluate the ancestry of breast cancer (BC) patients from different Brazilian geographical regions and to associate it with molecular subtype. Methods: Ethics approval 1136/2016. This was an observational, transversal, epidemiological study, evaluating patients with BC. Molecular characterization of BC was performed by immunohistochemistry. DNA was extracted to evaluate ancestry. For analysis of ancestry, the AIM-INDEL panel was used, with 46 primer pairs. The polymerase chain reaction (PCR) products were subjected to capillary electrophoresis, and the results were analyzed using GeneMapper 4.0 software. For inference of the ancestral profile, the data were evaluated with Structure v. 2.3.3 software. Each participant was classified into one of the following ancestries: European, African, Amerindian, and Asian. Ancestry was tested for correlations with the geographical region and the molecular subtype. The differences were compared using the chi-squared and Kruskal–Wallis tests. SPSS v.20.0 for Windows was used analysis. Results: A total of 1,330 patients were included, out of whom it was possible to evaluate ancestry in 1,127. There was a difference in race, molecular subtype, and ancestry between the geographical regions. In the South region, there were higher rates of selfreported white ethnicity, European ancestry, and HER-2-luminal tumors, which may influence age at diagnosis and yield a higher rate of early tumors. Conversely, in the North and Northeast regions, there was a higher rate of African ancestry, self-reported nonwhite ethnicity, HER-2+ tumors, and triple-negative tumors. Triple-negative and HER-2+ tumors were associated with a higher rate of advanced and metastatic disease at diagnosis, with triple-negative tumors being more frequent in young women. Conclusions: This is the largest assessment of genetic ancestry in the Brazilian BC patients. There were differences in ancestry and molecular subtype between the different regions. Knowledge of these characteristics may contribute to a better understanding of the molecular distribution of cancer in these regions.

Objetivo: Relatar um caso de schwannoma em fossa obturadora com ressecção do componente pélvico por via robótica. Detalhamento do caso: Paciente do sexo feminino, 57 anos, dá entrada em unidade de emergência do hospital com quadro de dor em região coxofemoral direita há 12 dias, evoluindo com febre e piora do quadro álgico. Os exames de imagem mostraram tumoração ocluindo a fossa obturadora direita e com extensão via forâmen obturatório ao compartimento interglúteo direito. Optado por abordagem combinada via robótica para acesso pélvico e posterior convencional para o compartimento glúteo. No procedimento cirúrgico, o acesso via robótica para a pelve possibilitou diminuição do dano aos nervos pélvicos e baixo volume de perda sanguínea. O estudo anatomopatológico evidenciou neoplasia formada pela proliferação de células fusiformes, dispostas em feixes multidirecionais, com a formação de corpúsculos de Verocay e células histiocitoides, necessitando de análise imunohistoquímica para melhor elucidação diagnóstica. Essa, por sua vez, revelou expressão difusa para proteína S-100, confirmando a hipótese de schwannoma. Considerações Finais: Dessa forma, a cirurgia robótica tende a crescer cada vez mais como alternativa na abordagem de tumores pélvicos de difícil acesso, visando a alcançar uma melhor recuperação pós-operatória e aumento da qualidade de vida do paciente.

OBJECTIVE: The 21-gene expression assay may support the decision regarding use of chemotherapy in early breast cancer. We sought to investigate the potential impact of incorporating the 21-gene expression assay into private practice in Brazil, from the perspective of third party payers. METHODS: We conducted a web-based survey with 30 (of a total of approximately 700) Brazilian medical oncologists, who were stratified by State according to the proportion of patients with breast cancer and private health insurance. We evaluated the possible treatment of first choice for patients with lymph-node-negative, estrogen-receptor-positive breast cancer, regardless of menopausal status. Interviewees were not aware of the objective of the study. Responses permitted a quantitative assessment of the care patterns regarding use of different chemotherapy regimens, type of premedication, use of growth factors, and use of intravenous antibiotics for febrile neutropenia. We calculated medication costs using the manufacturer's recommended prices. Other direct medical expenses, indirect medical costs, and non-medical costs were not included. RESULTS: Considering a hypothetical cohort of 100 patients without access to the 21-gene expression assay, the survey showed that 84 patients would receive chemotherapy. Reclassifying patient eligibility for chemotherapy according to the 21-gene expression assay would lower this number to 49. For a hypothetical cohort of 100 patients with access to the test, US$ 79,361.43 would be saved in main direct medical costs. Such results, however, would greatly vary according to tumor size: the 21-gene expression assay could increase direct medical costs in T1 tumors, and decrease costs in cases with T >2 cm. CONCLUSION: Considering the current price for the 21-gene expression assay in Brazil, our economic analysis suggests that such testing is an overall cost-saving, from the perspective of third party payers. Further, optimal use of resources would entail targeted use of the 21-gene expression assay.OBJETIVO: O índice de recorrência (IR), também conhecido como painel de 21 genes, pode apoiar decisões com relação ao uso de quimioterapia (QT) no câncer de mama precoce. Procuramos investigar o impacto potencial da incorporação do IR na prática privada no Brasil, a partir da perspectiva das fontes pagadoras. MÉTODOS: Conduzimos uma pesquisa com 30 oncologistas brasileiros (de um total de aproximadamente 700), que foram estratificados por Estado de acordo com a proporção de pacientes com câncer de mama e com cobertura pelo sistema de saúde suplementar. Avaliamos o tratamento de primeira escolha para pacientes com câncer de mama com axila negativa e expressão positiva do receptor de estrógeno, independente do estado menopausal. Os entrevistados não estavam cientes do objetivo do estudo. As respostas permitiram uma avaliação quantitativa dos padrões de cuidado, considerando o uso de diferentes regimes de QT, o tipo de pré-medicações, o uso de fatores de crescimento e o tratamento hospitalar da neutropenia febril. Calculamos o custo dos medicamentos usando o Brasíndice, e o custo do IR foi fixado em R$ 3.900,00 (MammaGene®). Outras despesas médicas diretas, custos médicos indiretos e custos não-médicos não foram considerados. RESULTADOS: Numa corte hipotética de 100 pacientes sem acesso ao teste de IR, 84 iriam receber quimioterapia. Reclassificando a elegibilidade das pacientes para QT de acordo com o IR, esse número cairia para 49. Para uma coorte hipotética de 100 pacientes com acesso ao IR, seriam economizados R$ 134.915,00 em despesas médicas diretas. CONCLUSÃO: Considerando o preço atual para avaliação do IR no Brasil, nossa análise econômica sugere que este teste economizaria custos, pela perspectiva das fontes pagadoras do setor privado. Além disso, o uso otimizado de recursos poderia requerer o emprego do painel de 21 genes de forma racional.

Juvenile xanthogranuloma (JXG) is a non-Langerhans cell histiocytosis (nonLCH). It is a benign and self-healing disorder that generally affects infants and children. Oral lesions in adult patients are rare, although the microscopic findings are similar to those observed in other locations. A 56-year-old white man presented with a chief complaint of a gingival mass that had appeared 6 months before and had grown slowly. An intraoral examination revealed the presence of a solitary, softened gingival mass affecting the mandibular lingual gingiva at the right central incisor area. A biopsy of the lesion showed multiple large macrophages and numerous giant cells of Touton type. The immunohistochemistry positivity for CD68, fascin, factor XIIIa, alpha-antitrypsin and negativity for S-100, beta-actin, CD1a, and desmin confirmed the diagnosis of JXG. The occurrence of adult oral JXG is extremely rare. It is a nonLCH that may present variable clinical and microscopic aspects, which leads to a diversity of clinical misdiagnoses. A precise diagnosis of these lesions requires an accurate evaluation of clinical, microscopic, and immunohistochemical features.

PURPOSE: To assess matrix metalloproteinase (MMP)-9 expression in pterygium lesion. METHODS: A prospective randomized clinical trial was done to evaluate the expression of matrix metalloproteinase in normal and in primary or recurrent pterygia in Tenon's capsule by immunohistochemical analysis and a computerized image analysis system. The data were submitted to statistical analysis. RESULTS: Matrix metalloproteinase expression showed no difference in normal Tenon's capsule and in primary or recurrent pterygia. CONCLUSION: The similar expression of the matrix metalloproteinase in normal Tenon's capsule and in primary or recurrent pterygia allowed us to conclude that matrix metalloproteinase is not implicated in the genesis or the recurrence of pterygium lesion.OBJETIVO: Avaliar a expressão da metaloprotease de matriz (MMP)-9 nos pterígios. MÉTODOS: Foi realizado na Faculdade de Medicina de Botucatu estudo prospectivo, aleatório, com o intuito de avaliar a expressão da metaloprotease de matriz na cápsula de Tenon normal e de pterígios primários e recidivados, usando o método da imuno-histoquímica e o sistema computadorizado de análise de imagem. Os resultados foram avaliados estatisticamente. RESULTADOS: A expressão da metaloprotease de matriz foi semelhante na cápsula de Tenon normal e nos pterígios primários e recidivados. CONCLUSÃO: A expressão da metaloprotease de matriz na cápsula de Tenon normal e nos pterígios primários ou recidivados é semelhante, o que nos leva a concluir que esta metaloprotease de matriz não esteja envolvida na gênese ou na recidiva do pterígio.

OBJECTIVE: To analyze the role of immunochemistry in serous effusions. STUDY DESIGN: We analyzed cell blocks of 18 pleural and 18 peritoneal effusions diagnosed as malignant (18), benign (14) and suspicious (4). They were immunostained by the avidin-biotin complex method with a panel of four monoclonal antibodies-CEA, Ber-EP4, LeuM1 (CD15) and p53-and, for lectins (Ulex europaeus) UEA-l, ConA and ConBr. RESULTS: Seventeen of the 18 cases of adenocarcinoma were positive for CEA (95%), 12 (66.6%) for Ber-EP4, 11 (61%) for CD15 and 11 (61%) for p53. Twelve of the 18 (66.6%) were positive for UEA-1, CEA, Ber-EP4 and CD15. UEA-1 did not react with mesothelial cells, p53 Gave a positive reaction in only one case, reactive mesothelial cells. ConA and ConBr reacted indiscriminately with benign and malignant cells; thus, it was not useful in distinguishing between these cells. CONCLUSION: In this context no antibody used alone is reliable for corroborating a diagnosis, but the selective use of a small panel of three markers (CEA, Ber-EP4 and LeuM1) can be vent useful in solving diagnostic difficulties in the cytodiagnosis of serous effusions.

OBJECTIVE This report describes a case that illustrates the limitations of the vaccination screening process for human papillomavirus (HPV) infection. CASE We report an unexpected microinvasive adenocarcinoma of the cervix arising in a young woman vaccinated against HPV as part of a clinical trial 6 years previously and followed up annually by cytology. In January 2012, at age 23 years, the patient received a cytological result of atypical squamous cells, cannot exclude high-grade cervical squamous intraepithelial lesion, and colposcopy showed slight abnormalities. Biopsy revealed an adenocarcinoma in situ. Conization of the cervix was performed, and the diagnosis was microinvasive adenocarcinoma. Polymerase chain reaction and immunohistochemical study of conization material showed positivity for HPV-18 L1, p16, and Ki-67. Retrospective analysis of the clinical trial information revealed that 7 HPV tests had been performed, and all were positive for HPV-18, including the sample collected before the first vaccination. CONCLUSIONS HPV-18 was present in the cervix before vaccination. Persistent detection of HPV-18 should be considered an important factor in treatment planning. This case demonstrates the need to vaccinate women before their first sexual contact and highlights the need for keeping adequate screening even in vaccinated individuals.

Lipid emulsions provided with total parenteral nutrition (TPN) have been associated with mononuclear phagocytic system functional changes. The aim of the present investigation was to assess the influence of TPN with added lipid emulsions on macrophage (M phi) phagocytosis. Wistar rats (n = 70) with external jugular vein cannulation were randomized into seven groups. The rats received an oral diet or six different isocaloric (1.16 kcal/mL), isonitrogenous (1.5 g/mL), and isolipidic (30% non-protein calories) TPN regimens: (a) an oral diet with intravenous infusion of saline (OS); (b) non-lipid TPN (glucose); (c) TPN with 10% long chain triacylglycerol emulsions (LCT); (d) TPN with 90% LCT and 10% fish oil (FO) emulsion; (e) TPN with 50% LCT and 50% FO; (f) TPN with 10% lipid emulsion with 50% medium chain triacylglycerol (MCT) and 50% LCT; and (g) TPN with 45% MCT, 45% LCT, and 10% FO. After 96 h of TPN or saline infusion, colloidal carbon (Pelikan, Germany) was injected intravenously at 1.0 mL/kg body weight, and the rats were killed after 3 h. Liver, spleen, and lung were weighed and prepared by immunohistochemistry analyses with the HAM-56 anti-M phi antibody. Under light microscopy, the total M phi number (MT) and the colloidal carbon phagocytic M phi number (MP) were established, and the phagocytic index was calculated as MP/MT x 100. There were no statistical (P < 0.05) differences in liver, spleen, or lung weights among the seven groups in comparison with the OS group. Non-lipid TPN inhibited spleen and lung M phi phagocytosis when compared with the OS and lipid-TPN groups. Lipid TPN supplemented with fish oil emulsion increased total liver and lung M phi number and phagocytosis. These results indicate that TPN supplemented with fish oil increases M phi phagocytosis in rats.

There is scarce information about immune function and parenteral fish oil (FO). The influence of a new parenteral lipid emulsion (LE) containing fish oil (SMOF) was experimentally evaluated on neutrophils' chemotaxis and macrophages' phagocytosis.Adult male Lewis rats (n=40) were randomized into five groups; one non-surgical control and four to receive parenteral LE or saline infusion through jugular vein catheterization: SMOF (mixture of 30% medium-chain triglycerides, 30% soybean, 25% olive and 15% fish oils); MCT/LCT (physical mixture of 50% medium-chain triglycerides and 50% soybean oil); MCT/LCT/FO (80% MCT/LCT supplemented with 20% FO) and SS (saline). In the 5th experimental day and after intravenous colloidal carbon injection, blood and tissue (liver, lung and spleen) samples were collected and immunological analyses were performed.LE didn't influence neutrophil chemotaxis. SMOF didn't influence phagocytosis (p0.05) while MCT/LCT and MCT/LCT/FO LE increased the number of liver and lung resident macrophages that had engaged in phagocytosis compared with CO-NS and SS (p0.05). Only MCT/LCT/FO increased the number of spleen resident macrophages that had engaged in phagocytosis (p0.05).LE, independently of composition, had no influence on neutrophils' chemotaxis, but showed different effect on phagocytosis by macrophages. SMOF LE had neutral effect while fish oil LE enriched with MCT/LCT LE increased resident-macrophages' phagocytosis.

Primary pulmonary lymphoma is rare. The most common histological type is the bronchus-associated lymphoid tissue lymphoma. This type of lymphoma has an indolent course and excellent response to therapy. One-third of all cases are diagnosed incidentally. However, due to the rarity of this disease, little is known about its natural history in terms of dissemination and evolution. Herein, we report the unusual case of a 61-year-old man who refused treatment after being diagnosed with bronchus-associated lymphoid tissue lymphoma and died 2 years later from massive lung infiltration without dissemination to other organs.

Malacoplakia is a granulomatous inflammatory disorder clinically and ultrasonographically very similar to prostatic adenocarcinoma. Symptoms and physical findings are similar to prostatism and in half of the patients the differential diagnosis includes malignancy, mainly because of the presence of a hard nodule on digital rectal examination. Additionally, cases of malacoplakia can show hypoechoic nodes on transrectal ultrasound mimicking adenocarcinoma. We report a case of malacoplakia of the prostate with emphasis on its similarities and differences with prostate adenocarcinoma.

Clinicamente o paciente apresentou de início cefaléia, seguida de ataxia, sonolência e paralisias múltiplas de alguns nervos cranianos, evoluindo para a morte em 8 dias. O exame anatomopatológico do encéfalo mostrou encefalite necrosante em focos múltiplos limitada ao tronco encefálico, mais acentuada na ponte e bulbo. A técnica da imunoperoxidase revelou raras células gliais com imunorreatividade intranuclear para antígeno do herpes. Raras partículas virais com as características morfológicas do herpesvírus foram identificadas nos núcleos de células nervosas em material fixado em formol a 10%. Este caso é o segundo relatado de encefalite herpética do tronco do encéfalo confirmado pelo exame anatomopatológico. Comentam-se as vias de chegada do herpesvírus ao sistema nervoso central e sua posterior disseminação, após período de latência e reativação, para a cavidade oral, região órbito-frontal, lobos temporais e tronco encefálico. A post-mortem examined case of herpetic brainstem encephalitis is presented. Clinically, the patient had cephalea followed by ataxia, drowsiness and multiple palsies of some cranial nerves, developing into death in eight days. The pathologic examination of the brain showed necrotizing encephalitis in multiple foci limited to the brainstem, more distinctly in the pons and medula oblongata. The technique of immunoperoxidase revealed rare glial cells with intranuclear immunoreactivity for herpes antigen. Rare viral particles with the morphological characteristics of the herpesvirus were identified in the nuclei of neurons in 10% formol fixed material. This is the second reported case of herpetic brainstem encephalitis confirmed by post-mortem examination. The pathway used by the virus to reach the central nervous system and its posterior dissemination to the oral cavity, the orbitofrontal region and the temporal lobes as well as to the brainstem, after a period of latency and reactivation, are discussed. A incidência do melanoma cutâneo em pacientes adultos jovens tem aumentado consideravelmente nos últimos anos. Há, contudo, carência de conhecimentos clinicopatológicos e moleculares sobre os melanomas que ocorrem nessa faixa etária. O presente estudo teve por objetivo avaliar 132 casos de melanoma cutâneo primário em pacientes com idade entre 18 e 30 anos, com ênfase no estudo das características clínicas, histopatológicas e avaliação molecular das mutações nos genes BRAF, NRAS e KIT. Em relação aos achados clínicos e histopatológicos, houve predomínio de indivíduos do sexo feminino (61,4%), sendo o tronco o sítio anatômico mais comumente envolvido (44,3%) e o melanoma extensivo superficial o tipo histológico predominante (79,5%). A mutação V600E no gene BRAF (BRAFV600E) foi analisada em 93 casos, utilizando-se a técnica de RT-PCR. Essa mutação foi identificada em 38,7% (36/93) e, estatisticamente, associada à fase vertical de crescimento (p = 0,01), infiltrado inflamatório discreto (p = 0,02) e presença de mitose intradérmica (p = 0,004). Houve, ainda, forte indício de associação com a presença de ulceração (p = 0,05). Todas essas variáveis apresentaram associação com pior prognóstico do melanoma cutâneo. Observou-se predomínio da mutação BRAFV600E em regiões anatômicas relacionadas à exposição solar intermitente. Nenhum caso de melanoma com fenômeno de regressão apresentou mutação BRAFV600E (p < 0,05). Não houve associação significativa entre BRAFV600E e sexo, tipo histológico, nível de Clark, índice de Breslow, elastose solar, invasão angiolinfática e perineural, satelitose, nevo melanocítico coexistente e sobrevida. A pesquisa de mutações NRAS, pela técnica de RT-PCR, detectou frequência de 3,95% (3/76). As três mutações encontradas foram do tipo 61K e ocorreram em pacientes do sexo masculino e em região de cabeça e pescoço. As mutações BRAFV600E e NRAS, quando presentes, eram mutuamente exclusivas. A frequência de mutações KIT, analisadas por sequenciamento, foi de 11,1% (3/27). As três mutações identificadas estavam localizadas no éxon 9 (G510, G498S e 489I). Houve concomitância de casos com mutação KIT tanto com NRAS, como com BRAFV600E. Devido ao pequeno número de casos com mutação em KIT e NRAS, não foi possível estabelecer correlações clínicas e histopatológicas com esses genes. Este estudo é o primeiro a descrever as mutações G510D e G498S no gene KIT em melanomas cutâneos. No presente estudo, a mutação BRAFV600E, em melanomas cutâneos de adultos jovens, correlacionou-se com características anatomoclínicas de pior prognóstico em relação aos melanomas selvagens para BRAFV600E The incidence of cutaneous melanoma in young adults has dramatically increased in recent years. However, there is scarce data about the clinicopathological and molecular characteristics on the melanomas occurring at this age group. The present study aimed to evaluate 132 patients aged between 18 and 30 years with primary cutaneous melanoma with emphasis on the study of clinical, histopathological characteristics and molecular evaluation of mutations in BRAF, NRAS and KIT genes. Regarding the clinical and histopathological findings, the following results were found: female predominance (61.4%), trunk was the most commonly anatomical site involved (44.3%) and superficial spreading melanoma, was the most common histological type (79.5 %). The V600E mutation in BRAF (BRAFV600E) gene was analyzed in 93 cases, using RT-PCR. It was present in 38.7% (36/93) and statistically related to the vertical growth phase (p = 0.01), mild inflammatory infiltration (p = 0.02) and the presence of intradermal mitosis (p = 0.004). There was, also, strongly evidence of an association with the presence of ulceration (p = 0.05). Worse prognosis was associated with these variables. There was a predominance of BRAFV600E mutation in anatomical regions related to intermittent sun exposure. No cases of melanoma with BRAFV600E mutation showed regression phenomenon (p < 0.05). There was no significant association between BRAFV600E and gender, histological type, Clark level, Breslow thickness, solar elastosis, angiolymphatic and perineural invasion, sattelitosis, coexisting melanocytic nevus and survival. The presence of a mutation in NRAS, by RT-PCR was seen in 3.95% (3/76) of the cases. All these three mutations were of type 61K, occurred in male patients and the head and neck region. BRAFV600E and NRAS mutations, when present, were mutually exclusive. The frequency of KIT mutations, analyzed by sequencing, was 11.1% (3/27). The three mutations identified in this gene were located in exon 9 (G510, G498S and 489I). Concomitant mutations were found between KIT and NRAS and BRAFV600E. Due to the small number of KIT and NRAS mutated cases, it was not possible to establish clinical and histopathological correlations and mutation status in these genes. This study was the first to describe the G510D and G498S mutations in KIT gene in cutaneous melanomas. O linfoma folicular é entidade clinicamente heterogênea, com carência de marcadores prognósticos que estratifiquem grupos de risco para otimização do manejo. É relativamente raro em pacientes abaixo de 40 anos. Os aspectos clínicos e patológicos desse tipo de linfoma, nessa faixa etária, assim como o comportamento biológico, são pouco conhecidos. No presente estudo, uma série de 208 pacientes entre 19-40 anos de idade foi, retrospectivamente, avaliada quanto aos achados anatomoclínicos e moleculares. Essas variáveis foram, então, correlacionadas com seguimento e sobrevida. A mediana de idade na apresentação foi de 35 anos, com leve predomínio no sexo feminino (56%). A maioria dos casos se manifestou como doença nodal (87%). Concomitância de linfoma folicular com linfoma difuso de grandes células B foi encontrada em 7 (3%) pacientes. Estudos imuno-histoquímicos revelaram expressão de CD10 (91%), BCL6 (97%), BCL2 (95%), MUM1/IRF4 (17%) e CD23 (25%). Rearranjos envolvendo os genes BCL2 e BCL6 foram encontrados em 74% e 20%, respectivamente. A sobrevida média geral estimada dos pacientes, em que foi possível o seguimento, foi de 13 anos. Presença de anemia, elevação da desidrogenase lática, acometimento de medula óssea, índice prognóstico internacional do linfoma folicular na faixa de alto risco, padrão de \"céu estrelado\", Ki-67 >= 50% e ausência do rearranjo do gene BCL2 e presença do BCL6 relacionaram-se diretamente com pior sobrevida geral. O estadiamento de Ann Arbor III/IV e MDM2 >=20% têm fortes indícios desta associação negativa com sobrevida geral. A combinação BCL2+ e BCL6- correlacionou-se com maior sobrevida média. O grau histológico determinou redução gradual da sobrevida, com semelhanças nas curvas de sobrevida entre os graus 1, 2 e 3A. Não houve diferença significativa para as curvas de sobrevida relacionando faixa etária, persistência da zona do manto, presença de áreas difusas, fibrose, expressão de CD10, BCL6 ou CD23, padrão de trama de células foliculares dendríticas ou clonalidade para cadeia leve de imunoglobulina. Esses achados revelaram que o linfoma folicular em adultos jovens apresenta similaridades com o linfoma folicular que ocorre em adultos mais velhos, incluindo a frequência de apresentação nos diversos sítios anatômicos, grau histológico e fatores prognósticos adversos Follicular lymphoma is a clinically heterogeneous group of disease and therefore with a need of characterization of prognostic markers to stratify risk groups and to optimize clinical management. It is relatively rare in patients younger than 40 years, and the clinicopathologic characteristics and biological behavior in this age group are poorly understood. In the current study, samples from a cohort of 208 patients between 19-40 years of age were evaluated retrospectively with respect to clinical, histologic and molecular characteristics. The presence of anemia, elevated lactose dehydrogenase, bone marrow involvement, high-risk follicular lymphoma international prognostic index, \"starry sky\" appearance, proliferative index >= 50%, absence of BCL2 rearrangement and presence of BCL6 rearrangement correlated with adverse overall outcome. Ann Arbor stage III/IV and MDM2 >= 20% correlated with high trend toward worse overall survival. The combination BCL2+ and BCL6- was associated with better overall survival. No impact on overall survival was observed related to age, persistence of mantle zone, presence of diffuse areas, fibrosis, expression. of CD10, BCL6 or CD23, follicular dendritic cells meshwork or clonality. O glioblastoma multiforme (GBM) é o tumor mais agressivo do Sistema Nervoso Central (SNC) e caracteriza-se por um alto poder angiogênico que está diretamente relacionado com a capacidade invasiva e inversamente com o prognóstico. A angiogênese contribui para a malignidade por prover a oxigenação e o suprimento nutricional necessários para o crescimento e invasão do tumor. O GBM prolifera sob um ambiente de hipóxia e o fator induzido por hipóxia (HIF-1) apresenta um papel fundamental na ativação da transcrição de genes alvo que favorecem a angiogênese e evitam a morte celular. O fator de crescimento do endotélio vascular (VEGF) e o fator de crescimento derivado da plaqueta C (PDGF-C) são agentes de grande interesse na angiogênese tumoral e que podem ser modulados pelo HIF-1. Foi realizado tissue microarray(TMA) de 208 casos de GBM e estudados pelo método de imunoistoquímica os marcadores HIF-1, VEGF e PDGFC. Os resultados foram correlacionados com a angiogênese avaliada através da densidade microvascular (DMV) pelo CD34, CD105, VEGF e PDGF-C, com a proliferação celular endotelial através da marcação nuclear pelo KI-67 e também com a sobrevida. A expressão tumoral do HIF-1 foi observada em 184 casos (88,5%), a do VEGF em 131 (63%) e a do PDGF-C em 160 (77%). As DMVs medianas pelo CD34, PDGF-C, VEGF e CD105 foram, respectivamente, 20, 16, 5 e 6. O presente estudo tem por objetivo avaliar as características clinicopatológicas, imunoistoquímicas e moleculares de 513 casos brasileiros de tumor estromal gastrintestinal, incluindo o estudo da expressão imunoistoquímica de CD117, CD34, proteína DOG1, actina de músculo liso, desmina, proteína S-100, antígeno Ki-67, proteína p53, molécula de adesão CD44v3, receptor para fator de crescimento epidérmico, proteína HER2 e receptor para fator de crescimento derivado de plaqueta alfa, além da análise molecular de mutações envolvendo os genes KIT e receptor para fator de crescimento derivado de plaqueta alfa e da pesquisa de amplificação dos genes receptor para fator de crescimento epidérmico e HER2 por hibridização \"in situ\" fluorescente. As características clínicas e morfológicas dos casos estudados não foram diferentes das referidas pela literatura. Houve expressão de CD117 (KIT) em 95,7% dos casos. A proteína DOG1 foi expressa em 85,9% dos tumores, sendo capaz de diagnosticar 20% dos casos com ausência de expressão de CD117 (KIT). O índice de proliferação celular determinado pelo antígeno Ki-67 foi superior nos casos classificados como de alto risco para comportamento biológico agressivo segundo critérios do \"National Institutes of Health\". A expressão da proteína p53 esteve restrita aos casos classificados como de alto risco. Não se observou expressão imunoistoquímica da molécula de adesão CD44v3. A proteína receptora para fator de crescimento epidérmico foi expressa em 84,4% dos casos, porém não se observou superexpressão da proteína HER2. A expressão imunoistoquímica da proteína receptora para fator de crescimento derivado de plaqueta alfa foi observada em 94,4% dos casos estudados, não apresentando relação com o tipo de mutação encontrado. As mutações do gene KIT foram as mais frequentemente observadas, principalmente do éxon 11. Mutações do gene receptor para fator de crescimento derivado de plaqueta alfa foram observadas em 8,1% dos casos, sendo que 54,5% dos casos com ausência de expressão imunoistoquímica de CD117 (KIT) apresentavam mutações nesse gene. The present study presents the clinicopathologic, immunohistochemical and molecular genetic features of 513 Brazilian cases of gastrointestinal stromal tumor, including the immunohistochemical expression of CD117, CD34, DOG1 protein, smooth muscle actin, desmin, S-100 protein, Ki-67 antigen, p53 protein, CD44v3 adhesion molecule, epidermal growth factor receptor, HER2 protein and platelet derived growth factor receptor alpha, the mutation analysis of KIT and platelet-derived growth factor receptor alpha genes, and the investigation of amplification of HER2 and epidermal growth factor receptor genes by fluorescence \"in situ\" hybridization. The clinicopathologic features of Brazilian gastrointestinal stromal tumors do not differ from those published from other countries. CD117 (KIT) was expressed by immunohistochemistry in 95.7% of cases. DOG1 protein was expressed in 85.9% of tumors, being able to establish the diagnosis of GIST in 20% of those cases that were negative for CD117 (KIT). Ki-67 proliferation index was higher in those cases classified as high-risk of aggressive behavior by the National Institutes of Health consensus approach. The immunohistochemical expression of p53 protein was restricted to cases classified as high-risk of aggressive behavior. The adhesion molecule CD44v3 was not expressed in any of the cases. The epidermal growth factor receptor protein was overexpressed in 84.4% of cases, and HER2 protein was not expressed in all cases. The platelet-derived growth factor receptor alpha protein was detected by immunohistochemistry in 94.4% of tumors, and there was no relationship between its expression and the mutation status. KIT mutations were the most frequent, mainly of exon 11. Plateletderived growth factor receptor alpha mutations were found in 8.1% of the cases, and 54.5% of those cases that were negative for CD117 (KIT) had mutations in platelet-derived growth factor receptor alpha gene.
