After applying inclusion and exclusion criteria, a total of 82 patients (67 inpatients and 15 ambulatory) were identified for final analysis. The primary outcome measured was postoperative Coronavirus (COVID-19) status. Secondary outcome measures included length of stay and discharge disposition.Results: The mean age and length of stay in the hospital group was 59.5 years (± 21.7) and 4.3 days (± 4.6), respectively, versus 47.9 years (± 9.8) in the ambulatory group. 7.3% (6/82) of the inpatients subsequently tested or screened positive for COVID-19 at 2 weeks post-operatively, compared to 0/15 ambulatory patients (P=0.58). Of the 6 inpatients who tested positive, 4 (66.7%) were discharged to a rehabilitation center. Diabetes (P=0.05), hypertension (P=0.02), and congestive heart failure (P=0.005) were associated with transmission.Conclusion: In this analysis, there was a nosocomial transmission rate of 7% compared to zero in the ambulatory surgery center, however this was not found to be statistically significant. This data supports the use of precautions such as frequent screening, hand washing, and masks to reduce transmission when COVID-19 rates are high. There is a lower risk of nosocomial COVID-19 transmission for patients treated as an outpatient and elective surgical procedures may be safer in this setting.Keywords: coronavirus, pandemic, infection, hospital transmission

Abstract

The REGAIN (Regional versus General Anesthesia for Promoting Independence after Hip Fracture) trial found similar ambulation and survival at 60 days with spinal versus general anesthesia for hip fracture surgery. Trial outcomes evaluating pain, prescription analgesic use, and patient satisfaction have not yet been reported.To compare pain, analgesic use, and satisfaction after hip fracture surgery with spinal versus general anesthesia.Preplanned secondary analysis of a pragmatic randomized trial. (ClinicalTrials.gov: NCT02507505).46 U.S. and Canadian hospitals.Patients aged 50 years or older undergoing hip fracture surgery.Spinal or general anesthesia.Pain on postoperative days 1 through 3; 60-, 180-, and 365-day pain and prescription analgesic use; and satisfaction with care.A total of 1600 patients were enrolled. The average age was 78 years, and 77% were women. A total of 73.5% (1050 of 1428) of patients reported severe pain during the first 24 hours after surgery. Worst pain over the first 24 hours after surgery was greater with spinal anesthesia (rated from 0 [no pain] to 10 [worst pain imaginable]; mean difference, 0.40 [95% CI, 0.12 to 0.68]). Pain did not differ across groups at other time points. Prescription analgesic use at 60 days occurred in 25% (141 of 563) and 18.8% (108 of 574) of patients assigned to spinal and general anesthesia, respectively (relative risk, 1.33 [CI, 1.06 to 1.65]). Satisfaction was similar across groups.Missing outcome data and multiple outcomes assessed.Severe pain is common after hip fracture. Abstract

SUMMARY Distal radius fractures are one of the most common fractures seen in orthopaedics. These fractures may be treated surgically or conservatively depending on patient-related and radiographic factors. Displaced fractures should be reduced in the acute setting to better align the fracture fragments and a splint applied to hold the fracture in this position. Fractures that are acceptably reduced may be treated conservatively with casting and close radiographic follow-up to ensure maintained alignment. In this video, we describe our technique for closed reduction of distal radius fractures and review important factors that guide treatment.

Summary Engaging patients—defined broadly as individuals with lived experience of a given condition, family members, caregivers, and the organisations that represent them—as partners in research is a priority for policymakers, funders, and the public. Nonetheless, formal efforts to engage patients are absent from most studies, and models to support meaningful patient engagement in clinical anaesthesia research have not been previously described. Here, we review our experience in developing and implementing a multifaceted patient engagement strategy within the Regional Versus General Anesthesia for Promoting Independence After Hip Fracture (REGAIN) surgery trial, an ongoing randomised trial comparing spinal vs general anaesthesia for hip fracture surgery in 1600 older adults across 45 hospitals in the USA and Canada. This strategy engaged patients and their representatives at both the level of overall trial oversight and at the level of individual recruiting sites. Activities spanned a continuum ranging from events designed to elicit patients' input on key decisions to longitudinal collaborations that empowered patients to actively participate in decision-making related to trial design and management. Engagement activities were highly acceptable to participants and led to concrete changes in the design and conduct of the REGAIN trial. The REGAIN experience offers a model for future efforts to engage patients as partners in clinical anaesthesia research, and highlights potential opportunities for investigators to increase the relevance of anaesthesia studies by incorporating patient voices and perspectives into the research process.

5. Variation in Diagnosis of Compartment Syndrome by Surgeons Treating Tibial Shaft Fractures

Abstract

Background: It is agreed that missed compartment syndrome is associated with significant morbidity, but controversy regarding its diagnosis remains. To our knowledge, no one has analyzed the effect of individual surgeon variation on the diagnosis of compartment syndrome. Methods: We analyzed a consecutive cohort of patients with tibial shaft fractures at our level I trauma center (n = 386 fractures). We identified all patients who were diagnosed as having compartment syndrome and who therefore underwent fasciotomy. The surgeon of record for each patient was recorded. Surgeons took call on random nights. All the surgeons were full-time orthopedic trauma surgeons. Patients with "prophylactic" fasciotomies were not included. Results were analyzed by conducting analysis of variance and the Kruskal-Wallis H test. Results: Even though all the surgeons practiced at the same hospital during the same time period, wide variation existed in the rate of diagnosis and treatment of compartment syndrome. The rate ranged from a maximum of 24% to a minimum of 2% of the tibial fractures being diagnosed with compartment syndrome, depending on the surgeon. The differences were highly statistically significant (p < 0.005, Kruskal-Wallis H test). The surgeons' use of compartment pressure checks also varied (p < 0.05, Kruskal-Wallis H test) and seemed to approximately parallel the rate of compartment syndrome diagnosis. Conclusions: The diagnosis of compartment syndrome is difficult, and the data reported herein show that significant practice variation is likely, even within a single institution. It is unknown what the "true" rate of compartment syndrome should be, considering that a rate that is too high indicates unnecessary surgery and a rate that is too low means missing a devastating injury. Our data indicate lack of consensus in practice regarding the diagnosis of compartment syndrome, even at a high-volume level I trauma center, and emphasize the possibility of false-positive results of compartment pressure checks in clinical practice.

Abstract

Pelvic ring disruption that results in a locked pubic symphysis is an unusual injury. A locked pubic symphysis is defined as a compression of the pelvic ring, with the intact pubis becoming trapped against the contralateral pubis. Although the injury pattern is well recognized in clinical practice, to the best of our knowledge, only eight patients with this injury have been reported in the English-language literature1-8 since its original description by Eggers in 19523. A proposed mechanism for a locked symphysis is forced hyperextension and adduction of the hip1 resulting from a lateral compression force to the pelvis. This injury causes a rupture of the ligaments that normally stabilize the symphysis. The superior, anterior, and posterior ligaments are weak, while the arcuate (or inferior) ligament is thought to confer most of the stability to this fibrocartilaginous joint9. After rupture of these ligaments, the displaced pelvic bone may lie either anterior or posterior to the contralateral pubis. Because of the direction and magnitude of this pubic displacement, the injury pattern has been associated with urethral injury2,3,5,7. Although an initial attempt at closed reduction has been advocated6, there is otherwise only limited guidance in the literature regarding the treatment of a locked symphysis. One technique for closed reduction involves using the femur as a lever by locking it in flexion, abduction, and external rotation. The iliofemoral ligaments are thought to hold the femoral head within the acetabulum and to allow reduction with a gentle abduction and rocking motion of the affected extremity3. Abstract

Clopidogrel (Plavix®) may influence patient safety during fracture surgery. Our study examines the incidence of complications for patients undergoing hemiarthroplasty on clopidogrel therapy. All patients, who underwent hemiarthroplasty between 2005 and 2011 were retrospectively reviewed. Patients were placed in two comparative groups based on the use of clopidogrel antiplatelet therapy. Abstract

Intramedullary nailing (IMN) of tibial shaft fractures is an excellent treatment option associated with low complication rates. Suprapatellar intramedullary nailing (SIMN) is a viable alternative treatment to traditional IMN, especially for proximal fractures of the tibia. There are, however, clinical concerns regarding SIMN removal that pose a challenge to a successful outcome. This article describes a novel percutaneous technique for the removal of a SIMN using the same instruments and incision used for nail insertion. This technical innovation offers a simple, reliable, and minimally invasive approach for the removal of any IMN, including nails inserted via a suprapatellar approach. The technique is demonstrated here on a cadaveric specimen as well as with a sawbones model.
