Abstract

In February 2017, the blazar OJ~287 underwent a period of intense multiwavelength activity. It reached a new historic peak in the soft X-ray (0.3-10 keV) band, as measured by Swift-XRT. This event coincides with a very-high-energy (VHE) $\gamma$-ray outburst that led VERITAS to detect emission above 100 GeV, with a detection significance of $10\sigma$ (from 2016 December 9 to 2017 March 31). The time-averaged VHE $\gamma$-ray spectrum was consistent with a soft power law ($\Gamma = -3.81 \pm 0.26$) and an integral flux corresponding to $\sim2.4\%$ that of the Crab Nebula above the same energy. Contemporaneous data from multiple instruments across the electromagnetic spectrum reveal complex flaring behavior, primarily in the soft X-ray and VHE bands. To investigate the possible origin of such an event, our study focuses on three distinct activity states: before, during, and after the February 2017 peak. The spectral energy distributions during these periods suggest the presence of at least two non-thermal emission zones, with the more compact one responsible for the observed flare. Broadband modeling results and observations of a new radio knot in the jet of OJ~287 in 2017 are consistent with a flare originating from a strong recollimation shock outside the radio core., Comment: 21 pages, 7 figures, accepted for publication in The Astrophysical Journal

Abstract

We present geometric and dynamical modeling of the broad line region for the multi-wavelength reverberation mapping campaign focused on NGC 5548 in 2014. The dataset includes photometric and spectroscopic monitoring in the optical and ultraviolet, covering the H$\beta$, C IV, and Ly$\alpha$ broad emission lines. We find an extended disk-like H$\beta$ BLR with a mixture of near-circular and outflowing gas trajectories, while the C IV and Ly$\alpha$ BLRs are much less extended and resemble shell-like structures. There is clear radial structure in the BLR, with C IV and Ly$\alpha$ emission arising at smaller radii than the H$\beta$ emission. Using the three lines, we make three independent black hole mass measurements, all of which are consistent. Combining these results gives a joint inference of $\log_{10}(M_{\rm BH}/M_\odot) = 7.64^{+0.21}_{-0.18}$. We examine the effect of using the $V$ band instead of the UV continuum light curve on the results and find a size difference that is consistent with the measured UV-optical time lag, but the other structural and kinematic parameters remain unchanged, suggesting that the $V$ band is a suitable proxy for the ionizing continuum when exploring the BLR structure and kinematics. Finally, we compare the H$\beta$ results to similar models of data obtained in 2008 when the AGN was at a lower luminosity state. We find that the size of the emitting region increased during this time period, but the geometry and black hole mass remain unchanged, which confirms that the BLR kinematics suitably gauge the gravitational field of the central black hole., Comment: 26 pages, 19 figures, 1 table, accepted for publication in ApJ

Abstract

The radio source 3C 264, hosted by the giant elliptical galaxy NGC 3862, was observed with VERITAS between February 2017 and May 2019. These deep observations resulted in the discovery of very-high-energy (VHE; E $>100$ GeV) $\gamma$-ray emission from this active galaxy. An analysis of $\sim$57 hours of quality-selected live time yields a detection at the position of the source, corresponding to a statistical significance of 7.8 standard deviations above background. The observed VHE flux is variable on monthly time scales, with an elevated flux seen in 2018 observations. The VHE emission during this elevated state is well-characterized by a power-law spectrum with a photon index $\Gamma = 2.20 \pm 0.27$ and flux F($>315$ GeV) = ($7.6\pm 1.2_{\mathrm stat} \pm 2.3_{\mathrm syst})\times 10^{-13}$ cm$^{-2}$ s$^{-1}$, or approximately 0.7% of the Crab Nebula flux above the same threshold. 3C 264 ($z = 0.0217$) is the most distant radio galaxy detected at VHE, and the elevated state is thought to be similar to that of the famously outbursting jet in M 87. Consequently, extensive contemporaneous multi-wavelength data were acquired in 2018 at the time of the VHE high state. Abstract

We report velocity-delay maps for prominent broad emission lines, Ly_alpha, CIV, HeII and H_beta, in the spectrum of NGC5548. The emission-line responses inhabit the interior of a virial envelope. The velocity-delay maps reveal stratified ionization structure. The HeII response inside 5-10 light-days has a broad single-peaked velocity profile. The Ly_alpha, CIV, and H_beta responses peak inside 10 light-days, extend outside 20 light-days, and exhibit a velocity profile with two peaks separated by 5000 km/s in the 10 to 20 light-day delay range. The velocity-delay maps show that the M-shaped lag vs velocity structure found in previous cross-correlation analysis is the signature of a Keplerian disk with a well-defined outer edge at R=20 light-days. The outer wings of the M arise from the virial envelope, and the U-shaped interior of the M is the lower half of an ellipse in the velocity-delay plane. The far-side response is weaker than that from the near side, so that we see clearly the lower half, but only faintly the upper half, of the velocity--delay ellipse. The delay tau=(R/c)(1-sin(i))=5 light-days at line center is from the near edge of the inclined ring, giving the inclination i=45 deg. A black hole mass of M=7x10^7 Msun is consistent with the velocity-delay structure. A barber-pole pattern with stripes moving from red to blue across the CIV and possibly Ly_alpha line profiles suggests the presence of azimuthal structure rotating around the far side of the broad-line region and may be the signature of precession or orbital motion of structures in the inner disk. Further HST observations of NGC 5548 over a multi-year timespan but with a cadence of perhaps 10 days rather than 1 day could help to clarify the nature of this new AGN phenomenon., Comment: 19 pages, 9 figures, ApJ in press

Abstract

We report on variability and correlation studies using multiwavelength observations of the blazar Mrk 421 during the month of February, 2010 when an extraordinary flare reaching a level of $\sim$27~Crab Units above 1~TeV was measured in very-high-energy (VHE) $\gamma$-rays with the VERITAS observatory. This is the highest flux state for Mrk 421 ever observed in VHE $\gamma$-rays. Data are analyzed from a coordinated campaign across multiple instruments including VHE $\gamma$-ray (VERITAS, MAGIC), high-energy (HE) $\gamma$-ray (Fermi-LAT), X-ray (Swift}, RXTE, MAXI), optical (including the GASP-WEBT collaboration and polarization data) and radio (Mets\"ahovi, OVRO, UMRAO). Light curves are produced spanning multiple days before and after the peak of the VHE flare, including over several flare `decline' epochs. The main flare statistics allow 2-minute time bins to be constructed in both the VHE and optical bands enabling a cross-correlation analysis that shows evidence for an optical lag of $\sim$25-55 minutes, the first time-lagged correlation between these bands reported on such short timescales. Limits on the Doppler factor ($\delta \gtrsim 33$) and the size of the emission region ($ \delta^{-1}R_B \lesssim 3.8\times 10^{13}\,\,\mbox{cm}$) are obtained from the fast variability observed by VERITAS during the main flare. Analysis of 10-minute-binned VHE and X-ray data over the decline epochs shows an extraordinary range of behavior in the flux-flux relationship: from linear to quadratic to lack of correlation to anti-correlation. Abstract

We model the ultraviolet spectra of the Seyfert 1 galaxy NGC~5548 obtained with the Hubble Space Telescope during the 6-month reverberation-mapping campaign in 2014. Our model of the emission from NGC 5548 corrects for overlying absorption and deblends the individual emission lines. Using the modeled spectra, we measure the response to continuum variations for the deblended and absorption-corrected individual broad emission lines, the velocity-dependent profiles of Ly$\alpha$ and C IV, and the narrow and broad intrinsic absorption features. We find that the time lags for the corrected emission lines are comparable to those for the original data. The velocity-binned lag profiles of Ly$\alpha$ and C IV have a double-peaked structure indicative of a truncated Keplerian disk. The narrow absorption lines show delayed response to continuum variations corresponding to recombination in gas with a density of $\sim 10^5~\rm cm^{-3}$. The high-ionization narrow absorption lines decorrelate from continuum variations during the same period as the broad emission lines. Analyzing the response of these absorption lines during this period shows that the ionizing flux is diminished in strength relative to the far-ultraviolet continuum. The broad absorption lines associated with the X-ray obscurer decrease in strength during this same time interval. The appearance of X-ray obscuration in $\sim\,2012$ corresponds with an increase in the luminosity of NGC 5548 following an extended low state. We suggest that the obscurer is a disk wind triggered by the brightening of NGC 5548 following the decrease in size of the broad-line region during the preceding low-luminosity state., Comment: 50 pages, 30 figures, uses aastex62.cls. Abstract

During the Space Telescope and Optical Reverberation Mapping Project (STORM) observations of NGC 5548, the continuum and emission-line variability became de-correlated during the second half of the 6-month long observing campaign. Here we present Swift and Chandra X-ray spectra of NGC 5548 obtained as a part of the campaign. The Swift spectra show that excess flux (relative to a power-law continuum) in the soft X-ray band appears before the start of the anomalous emission-line behavior, peaks during the period of the anomaly, and then declines. This is a model-independent result suggesting that the soft excess is related to the anomaly. We divide the Swift data into on- and off-anomaly spectra to characterize the soft excess via spectral fitting. The cause of the spectral differences is likely due to a change in the intrinsic spectrum rather than being due to variable obscuration or partial covering. The Chandra spectra have lower signal-to-noise ratios, but are consistent with Swift data. Our preferred model of the soft excess is emission from an optically thick, warm Comptonizing corona, the effective optical depth of which increases during the anomaly. This model simultaneously explains all the three observations: the UV emission line flux decrease, the soft-excess increase, and the emission line anomaly., Comment: ApJ in press. Abstract

We present the results of an optical spectroscopic monitoring program targeting NGC 5548 as part of a larger multi-wavelength reverberation mapping campaign. The campaign spanned six months and achieved an almost daily cadence with observations from five ground-based telescopes. The H$\beta$ and He II $\lambda$4686 broad emission-line light curves lag that of the 5100 $\AA$ optical continuum by $4.17^{+0.36}_{-0.36}$ days and $0.79^{+0.35}_{-0.34}$ days, respectively. The H$\beta$ lag relative to the 1158 $\AA$ ultraviolet continuum light curve measured by the Hubble Space Telescope is roughly $\sim$50% longer than that measured against the optical continuum, and the lag difference is consistent with the observed lag between the optical and ultraviolet continua. This suggests that the characteristic radius of the broad-line region is $\sim$50% larger than the value inferred from optical data alone. We also measured velocity-resolved emission-line lags for H$\beta$ and found a complex velocity-lag structure with shorter lags in the line wings, indicative of a broad-line region dominated by Keplerian motion. The responses of both the H$\beta$ and He II $\lambda$4686 emission lines to the driving continuum changed significantly halfway through the campaign, a phenomenon also observed for C IV, Ly $\alpha$, He II(+O III]), and Si IV(+O IV]) during the same monitoring period. Finally, given the optical luminosity of NGC 5548 during our campaign, the measured H$\beta$ lag is a factor of five shorter than the expected value implied by the $R_\mathrm{BLR} - L_\mathrm{AGN}$ relation based on the past behavior of NGC 5548., Comment: 22 pages, 13 figures, accepted to ApJ

Abstract

We conduct a multiwavelength continuum variability study of the Seyfert 1 galaxy NGC 5548 to investigate the temperature structure of its accretion disk. The 19 overlapping continuum light curves (1158 to 9157 angstroms) combine simultaneous HST , Swift , and ground-based observations over a 180 day period from 2014 January to July. Light-curve variability is interpreted as the reverberation response of the accretion disk to irradiation by a central time-varying point source. Our model yields the disk inclination, i, temperature T1 at 1 light day from the black hole, and a temperature-radius slope, alpha. We also infer the driving light curve and find that it correlates poorly with both the hard and soft X-ray light curves, suggesting that the X-rays alone may not drive the ultraviolet and optical variability over the observing period. We also decompose the light curves into bright, faint, and mean accretion-disk spectra. Abstract

During an intensive Hubble Space Telescope (HST) Cosmic Origins Spectrograph (COS) UV monitoring campaign of the Seyfert~1 galaxy NGC 5548 performed from 2014 February to July, the normally highly correlated far-UV continuum and broad emission-line variations decorrelated for ~60 to 70 days, starting ~75 days after the first HST/COS observation. Following this anomalous state, the flux and variability of the broad emission lines returned to a more normal state. This transient behavior, characterised by significant deficits in flux and equivalent width of the strong broad UV emission lines, is the first of its kind to be unambiguously identified in an active galactic nucleus reverberation mapping campaign. The largest corresponding emission-line flux deficits occurred for the high-ionization collisionally excited lines, C IV and Si IV(+O IV]), and also He II(+O III]), while the anomaly in Ly-alpha was substantially smaller. This pattern of behavior indicates a depletion in the flux of photons with E_{\rm ph} > 54 eV, relative to those near 13.6 eV. We suggest two plausible mechanisms for the observed behavior: (i) temporary obscuration of the ionizing continuum incident upon BLR clouds by a moving veil of material lying between the inner accretion disk and inner BLR, perhaps resulting from an episodic ejection of material from the disk, or (ii) a temporary change in the intrinsic ionizing continuum spectral energy distribution resulting in a deficit of ionizing photons with energies > 54 eV, possibly due to a transient restructuring of the Comptonizing atmosphere above the disk. Abstract

The BL Lac object 1ES 1011+496 was discovered at Very High Energy gamma-rays by MAGIC in spring 2007. Before that the source was little studied in different wavelengths. Therefore a multi-wavelength (MWL) campaign was organized in spring 2008. Along MAGIC, the MWL campaign included the Metsahovi radio observatory, Bell and KVA optical telescopes and the Swift and AGILE satellites. MAGIC observations span from March to May, 2008 for a total of 27.9 hours, of which 19.4 hours remained after quality cuts. The light curve showed no significant variability. The differential VHE spectrum could be described with a power-law function. Both results were similar to those obtained during the discovery. Swift XRT observations revealed an X-ray flare, characterized by a harder when brighter trend, as is typical for high synchrotron peak BL Lac objects (HBL). Strong optical variability was found during the campaign, but no conclusion on the connection between the optical and VHE gamma-ray bands could be drawn. The contemporaneous SED shows a synchrotron dominated source, unlike concluded in previous work based on nonsimultaneous data, and is well described by a standard one zone synchrotron self Compton model. We also performed a study on the source classification. While the optical and X-ray data taken during our campaign show typical characteristics of an HBL, we suggest, based on archival data, that 1ES 1011+496 is actually a borderline case between intermediate and high synchrotron peak frequency BL Lac objects., Comment: 13 pages, accepted for publication in MNRAS

Abstract

We present ground-based optical photometric monitoring data for NGC 5548, part of an extended multi-wavelength reverberation mapping campaign. The light curves have nearly daily cadence from 2014 January to July in nine filters (\emph{BVRI} and \emph{ugriz}). Combined with ultraviolet data from the \emph{Hubble Space Telescope} and \emph{Swift}, we confirm significant time delays between the continuum bands as a function of wavelength, extending the wavelength coverage from 1158\,\AA\ to the $z$ band ($\sim\!9160$\,\AA). We find that the lags at wavelengths longer than the {\it V} band are equal to or greater than the lags of high-ionization-state emission lines (such as He\,{\sc ii}\,$\lambda 1640$ and $\lambda 4686$), suggesting that the continuum-emitting source is of a physical size comparable to the inner broad-line region (BLR). The trend of lag with wavelength is broadly consistent with the prediction for continuum reprocessing by an accretion disk with $\tau \propto \lambda^{4/3}$. However, the lags also imply a disk radius that is 3 times larger than the prediction from standard thin-disk theory, assuming that the bolometric luminosity is 10\% of the Eddington luminosity ($L = 0.1L_{\rm Edd}$). Using optical spectra from the Large Binocular Telescope, we estimate the bias of the interband continuum lags due to BLR emission observed in the filters. We find that the bias for filters with high levels of BLR contamination ($\sim\! 20\%$) can be important for the shortest continuum lags, and likely has a significant impact on the {\it u} and {\it U} bands owing to Balmer continuum emission., Comment: 26 pages, 10 figures, accepted to ApJ. Abstract

[Abridged] Context. To construct and interpret the spectral energy distribution (SED) of BL Lacertae objects, simultaneous broad-band observations are mandatory. Aims. We present the results of a dedicated multi-wavelength study of the high-frequency peaked BL Lacertae (HBL) object and known TeV emitter 1ES 2344+514 by means of a pre-organised campaign. Methods. The observations were conducted during simultaneous visibility windows of MAGIC and AGILE in late 2008. The measurements were complemented by Mets\"ahovi, RATAN-600, KVA+Tuorla, Swift and VLBA pointings. Additional coverage was provided by the ongoing long-term F-GAMMA and MOJAVE programs, the OVRO 40-m and CrAO telescopes as well as the Fermi satellite. The obtained SEDs are modelled using a one-zone as well as a self-consistent two-zone synchrotron self-Compton model. Results. 1ES 2344+514 was found at very low flux states in both X-rays and very high energy gamma rays. Variability was detected in the low frequency radio and X-ray bands only, where for the latter a small flare was observed. The X-ray flare was possibly caused by shock acceleration characterised by similar cooling and acceleration time scales. MOJAVE VLBA monitoring reveals a static jet whose components are stable over time scales of eleven years, contrary to previous findings. There appears to be no significant correlation between the 15 GHz and R-band monitoring light curves. The observations presented here constitute the first multi-wavelength campaign on 1ES 2344+514 from radio to VHE energies and one of the few simultaneous SEDs during low activity states. The quasi-simultaneous Fermi-LAT data poses some challenges for SED modelling, but in general the SEDs are described well by both applied models. The resulting parameters are typical for TeV emitting HBLs. Abstract

We report on TeV gamma-ray observations of the blazar Mrk 421 (redshift of 0.031) with the VERITAS observatory and the Whipple 10m Cherenkov telescope. The excellent sensitivity of VERITAS allowed us to sample the TeV gamma-ray fluxes and energy spectra with unprecedented accuracy where Mrk 421 was detected in each of the pointings. A total of 47.3 hrs of VERITAS and 96 hrs of Whipple 10m data were acquired between January 2006 and June 2008. We present the results of a study of the TeV gamma-ray energy spectra as a function of time, and for different flux levels. On May 2nd and 3rd, 2008, bright TeV gamma-ray flares were detected with fluxes reaching the level of 10 Crab. The TeV gamma-ray data were complemented with radio, optical, and X-ray observations, with flux variability found in all bands except for the radio waveband. The combination of the RXTE and Swift X-ray data reveal spectral hardening with increasing flux levels, often correlated with an increase of the source activity in TeV gamma-rays. Abstract

Since September 2005, the Whipple 10m Gamma-ray Telescope has been operated primarily as a blazar monitor. The five Northern Hemisphere blazars that have already been detected at the Whipple Observatory, Markarian 421, H1426+428, Markarian 501, 1ES 1959+650 and 1ES 2344+514, are monitored routinely each night that they are visible. We report on the Markarian 421 observations taken from November 2005 to June 2006 in the gamma-ray, X-ray, optical and radio bands. During this time, Markarian 421 was found to be variable at all wavelengths probed. Both the variability and the correlations among different energy regimes are studied in detail here. A tentative correlation, with large spread, was measured between the X-ray and gamma-ray bands, while no clear correlation was evident among the other energy bands. In addition to this, the well-sampled spectral energy distribution of Markarian 421 (1101+384) is presented for three different activity levels. In September 2005, the observing program of the Whipple 10 m gamma-ray telescope was redefined to be dedicated almost exclusively to AGN monitoring. Since then the five Northern Hemisphere blazars that had already been detected at Whipple are monitored routinely each night that they are visible. Thanks to the efforts of a large number of multiwavelength collaborators, the first year of this program has been very successful. We present the results of a long-term monitoring campaign of the H\alpha line in a sample of bright OB-supergiants (O7.5-B9) that aims at detecting rotationally modulated changes potentially related to the existence of large-scale wind structures. A total of 22 objects were monitor ed during 36 nights spread over 6 months in 2001-2002. Coordinated broad-band photometric observations were also obtained for some targets. Conspicuous evidence for variability in H\alpha is found for the stars displaying a feature contaminated by wind emission. Most changes take place on a daily time-scale, although hourly variations are also occasionally detected. Convincing evidence for a cyclical pattern of variability in H\alpha has been found in 2 stars: HD 14134 and HD 42087 (periodic signals are also detected in other stars, but independent confirmation is required). Rotational modulation is suggested from the similarity between the observed recurrence time-scales (in the range 13-25 days) and estimated periods of stellar rotation. We call attention to the atypical case of HD 14134 which exhibits a clear 12.8-d periodicity both in the photometric and in the spectroscopic data sets. This places this object among a handful of early-type stars where one may observe a clear link between extended wind structures and photospheric disturbances. Abstract

We report on two BeppoSAX observations of BL Lac (2200+420) performed respectively in June and December 1999, as part of a ToO program to monitor blazars in high states of activity. During both runs the source has been detected up to 100 keV, but it showed quite different spectra: in June it was concave with a very hard component above 5-6 keV (alpha_1 ~ 1.6; alpha_2 ~ 0.15); in December it was well fitted by a single power law (alpha ~ 0.6). During the first BeppoSAX observation BL Lac showed an astonishing variability episode: the 0.3 - 2 keV flux doubled in ~ 20 minutes, while the flux above 4 keV was almost constant. This frequency-dependent event is one of the shortest ever recordered for BL Lac objects and places lower limits on the dimension and the magnetic field of the emitting region and on the energy of the synchrotron radiating electrons. A similar but less extreme behaviour is detected also in optical light curves, that display non-simultaneous, smaller fluctuations of ~ 20% in 20 min. We fit the spectral energy distributions with a homogeneous, one-zone model to constrain the emission region in a very simple but effective SSC + external Compton scenario, highlightning the importance of the location of the emitting region with respect to the Broad Line Region and the relative spectral shape dependence. Abstract

Light curves of 3C 279 are presented in optical (R-band), X-rays (RXTE/PCA), and gamma rays (CGRO/EGRET) for 1999 Jan-Feb and 2000 Jan-Mar. During both of those epochs the gamma-ray levels were high, and all three observed bands demonstrated substantial variation, on time scales as short as one day. Correlation analyses provided no consistent pattern, although a rather significant optical/gamma-ray correlation was seen in 1999, with a gamma-ray lag of ~2.5 days, and there are other suggestions of correlations in the light curves. For comparison, correlation analysis is also presented for the gamma-ray and X-ray light curves during the large gamma ray flare in 1996 Feb and the two gamma-bright weeks leading up to it; the correlation at that time was strong, with a gamma-ray/X-ray offset of no more than 1 day., Comment: 20 pages, including 7 figures; accepted by The Astrophysical Journal

Abstract

Dermis fibroblasts are very sensitive to penetrating UVA radiation and induce photo-damage. To protect skin cells against this environmental damage, there is an urgent need for effective compounds, specifically targeting UVA-induced mitochondrial injury. This study aimed to analyze the effect of carnosine on the proteome of UVA-irradiated human skin fibroblast, cultured in a three-dimensional (3D) biological system recapitulating dermal compartment as a test system to investigate the altered cellular pathways after 48 h and 7 days of culture with or without carnosine treatment. The obtained results indicate that UVA dysregulates Oxidative Phosphorylation, the Fibrosis Signaling Pathway, Glycolysis I and Nrf2-mediated Oxidative Stress Response. Carnosine exercises provide a protective function against the harmful effects of UVA radiation by activating the Nrf2 pathway with the upregulations of some ROS-detoxifying enzymes such as the glutathione S-transferase (GST) protein family. Additionally, carnosine regulates the activation of the Epithelial Adherens Junction and Wound Healing Signaling Pathway by mediating the activation of structural proteins such as vinculin and zyxin as well as fibronectin 1 and collagen type XVIII alpha 1 chain against UVA-induced changes.

Abstract

The Italian Radical Cystectomy Registry (RIC) is an observational prospective study aiming to understand clinical variables and patient characteristics associated with short- and long-term outcomes among bladder cancer (BC) patients undergoing radical cystectomy (RC). Moreover, it compares the effectiveness of three RC techniques - open, robotic, and laparoscopic.From 2017 to 2020, 1400 patients were enrolled at one of the 28 centers across Italy. Patient characteristics, as well as preoperative, postoperative, and follow-up (3, 6, 12, and 24 months) clinical variables and outcomes were collected.Preoperatively, it was found that patients undergoing robotic procedures were younger (p.001) and more likely to have undergone preoperative neoadjuvant chemotherapy (p.001) and BCG instillation (p.001). Hypertension was the most common comorbidity among all patients (55%), and overall, patients undergoing open and laparoscopic RC had a higher Charlson Comorbidities Index (CCI) compared to robotic RC (p.001). Finally, laparoscopic patients had a lower G-stage classification (p=.003) and open patients had a higher ASA score (p.001).The present study summarizes the characteristic of patients included in the RIC. Future results will provide invaluable information about outcomes among BC patients undergoing RC. This will inform physicians about the best techniques and course of care based on patient clinical factors and characteristics.

Abstract

Introduction: We aimed to compare the outcomes of open vs robotic partial nephrectomy (PN), focusing on predictors of Trifecta failure in patients with highly complex renal masses. Patients and methods: We queried the prospectively collected database from the SIB International Consortium, including 507 consecutive patients with cT1-2N0M0 renal masses treated at 16 high-volume referral centres, to select those with highly complex (PADUA score =10) tumors undergoing PN. RT was classified as enucleation, enucleoresection or resection according to the SIB score. Trifecta was defined as achievement of negative surgical margins, no acute kidney injury and no Clavien-Dindo grade =2 postoperative surgical complications. Multivariable logistic regression analysis was used to assess independent predictors of Trifecta failure. Results: 113 patients were included. Patients undergoing open PN (n = 47, 41.6%) and robotic PN (n = 66, 58.4%) were comparable in baseline characteristics. RT was classified as enucleation, enucleoresection and resection in 46.9%, 34.0% and 19.1% of open PN, and in 50.0%, 40.9% and 9.1% of robotic PN (p = 0.28). Trifecta was achieved in significantly more patients after robotic PN (69.7% vs. 42.6%, p = 0.004). On multivariable analysis, surgical approach (open vs robotic, OR: 2.62; 95%CI: 1.11–6.15, p = 0.027) and tumor complexity (OR for each additional unit of the PADUA score: 2.27; 95%CI: 1.27–4.06, p = 0.006) were significant predictors of Trifecta failure, while RT was not. The study is limited by lack of randomization; as such, selection bias and confounding cannot be entirely ruled out. Preclinical animal models have provided a great deal of information on potential common pathogenic mechanisms underlying these two clinical identities. Although the efficacy of the most commonly used treatments for LUTS/BPH is well defined, the negative impact of these treatments on sexual function – in particular, on ED – has triggered the search for new treatment options. In this regard, a new role for phosphodiesterase type 5 inhibitors in the treatment of LUTS/BPH and ED has been claimed. Tadalafil is one of the most extensively investigated phosphodiesterase type 5 inhibitors for this new indication. All evidence reported to date suggests that tadalafil 5 mg once daily is a safe and effective treatment option for both LUTS/BPH and ED.
