Abstract

Background: Microvascular dysfunction is an often overlooked feature of hypertrophic cardiomyopathy (HCM). Our aim was to assess the association between microvascular dysfunction, wall thickness, tissue characteristics and myocardial deformation in HCM patients, by analyzing individual myocardial segments. Methods: Prospective assessment including cardiac magnetic resonance to assess wall thickness, T1 and T2 mapping, extracellular volume, late gadolinium enhancement (LGE) and stress perfusion. Results were stratified according to the 16 American Heart Association segments. Results: Seventy-five patients were recruited (1200 segments), 63% male, mean age 54.6±14.8 years, maximal wall thickness of 20.22±4.6 mm. Among the 424 segments (35%) with perfusion defects, 24% had defects only in the endocardial layer and 12% in both endocardial and epicardial layers. Perfusion defects were more often detected in hypertrophied segments (64%). Among the 660 segments with normal wall thickness, 19% presented perfusion defects. Abstract

Background/Objectives: Bioelectrical impedance vector analysis (BIVA) has been considered a promising technique in monitoring the nutritional and hydration status of patients with different types of diseases. The aim of this study was to assess the nutritional status provided by direct parameters of bioelectrical impedance analysis (BIA), BIVA and phase angle (PA), in patients with cervical and endometrial cancer undergoing surgical treatment, associating to other parameters of nutritional status and surgical outcomes. Subjects/Methods: In a prospective cohort, 208 women eligible to surgical treatment, admitted from January to December 2015, were enrolled. Patients were assessed according to the body mass index (BMI), Patient Generated Subjective Global Assessment (PG-SGA) and BIA. The PA was categorized as below and above percentiles 25 and 50 of studied population. Results: According to BMI and PG-SGA, most of them were classified as obese (69%) and well nourished (84%), respectively. PA was significantly lower in patients with endometrial cancer, PG-SGA B or C, and in those who remained longer in hospital. PA below 25th percentile was also associated with surgical complications. Comparison of BIVA detachment of our population with a reference population showed significant impedance vector displacement, characterized by decreased reactance value and increased resistance value in our group of patients. Conclusions: PA was associated with other parameters of nutritional status and surgical outcomes. Abstract

The Ectodermal Dysplasias generally present orofacial manifestations, such as skeletal discrepancies and dental alterations. Therefore, the role of a paediatric dentist in the detection and recognition of these repercussions can be crucial in early diagnosis of the disease. The oral rehabilitation of paediatric patients with this condition is extremely important, ideally, at a very early stage, yet contributing for the re-establishment of normal chewing, swallowing and phonetics functions, and, naturally, aesthetics increase. The purpose of this narrative review aims to elucidate dentists about their role in the detection, diagnosis, treatment and monitoring of the Ectodermal Dysplasia’ oral manifestations in paediatric patients, through the presentation of general physical and specific craniofacial characteristics.

39. Nutrition transition in a middle-income country: 22-year trends in the Seychelles

Abstract

BACKGROUND/OBJECTIVES: There is little objective information regarding nutrition transition in African countries. We assessed trends in nutrition patterns in the Seychelles between 1989 and 2011. SUBJECTS/METHODS: Population-based samples were obtained in 1989, 1994 and 2011 and participants aged 25-44 are considered in this study (n = 493, 599 and 471, respectively). Similar, although not identical, food frequency questionnaires (FFQs) were used in each survey and the variables were collapsed into homogenous categories for the purpose of this study. RESULTS: Between 1989 and 2011, consumption frequency offish (5 + /week) decreased from 93 to 74%, whereas the following increased: meat (5 + /week) 25 to 51%, fruits (1 + /week) 48 to 94%, salty snacks (1 + /week) 22 to 64% and sweet snacks (1 + /week) 38 to 67% (Po 0.001 for all). Consumption frequency decreased for home-brewed alcoholic drinks (1 + /week) 16 to 1%, but increased for wine (1 + /week) 5 to 33% (both Po0.001). Between 2004 and 2011, consumption frequency decreased for rice (2/day) 62 to 57% and tea (1 + /day) 72 to 68%, increased for poultry (1 + /week) 86 to 96% (all Po0.01), and did not change for vegetables (70.3 to 69.8%, P = 0.65). CONCLUSIONS: Seychelles is experiencing nutrition transition characterized by a decreased consumption frequency of traditional staple foods (fish, polished rice), beverages (tea) and of inexpensive home brews, and increased consumption frequency of meat, poultry and snacks. Food patterns also became more varied along with a broader availability of products in the 22-year interval. Abstract

Currently, in machine learning, there is a growing interest in finding new and better predictive models that can deal with heterogeneous data and missing values. In this thesis, two learning algorithms are proposed that can deal with both issues. The first learning algorithm that is studied consists of a neural network based on similarity measures, the Similarity Neural Network (SNN). It is a two-layer network, where the hidden layer computes the similarity between the input data and a set of prototypes, and the output layer gathers these results and predicts the output. In this thesis, several variants of this algorithm are proposed and it is analyzed which one performs better. Some of these variants are the way to choose the prototypes or how to set the parameters of the activation function. A full analysis is performed in the experiments section. Secondly, an Ensemble of SNNs is also proposed. The purpose of using an ensemble is to increase predictive performance, reduce variability and reduce learning time complexity. This second learning algorithm combines the predictions of a set of SNNs and gives the response of the ensemble based on these predictions. For this algorithm, several ensemble learners are proposed (in other words, different ways to combined these predictions). These variants are analyzed with a set of experiments. The main goal of this thesis is to understand these two methods, derive training algorithms and compare them with traditional learning algorithms, such as the classical Random Forest. The results of the experiments show a competitive performance of both methods, obtaining similar results than the Random Forest and improving it in some problems. 16 datasets with heterogeneous data and missing values are tested, some of them large and difficult problems. About the SNN, with these experiments, it is found that adding regularization to the network has a high influence on the model. About the ensemble, the experiment results suggest that the simplest ensemble learner (mean or majority vote of the SNNs) is the one that performs better. Among the two proposals, both get similar and quite good performance metrics but the ensemble obtains slightly better predictions.

Abstract

Trichlorfon [TCF] is an organophosphate compound used to eliminate or control a variety of parasites in farmed fish. The physiological and biochemical responses of juvenile Nile tilapia were studied after 96 hours of exposure to TCF. The experiment was conducted randomly with two treatments: control and TCF (0.5 mg L-1). No mortality or changes in hematological profile were recorded for the fish. On the other hand, we found that TCF exposure caused behavioral, metabolic and hormonal changes that modified the response patterns of the immune and enzymatic defense system in muscle and liver tissues. Increased plasma cortisol and glucose combined with the reduction of acetylcholinesterase activity in muscle and liver tissue after 96-h of TCF exposure indicates chemical stress. In response to this harmful situation, we observed changes in the immune system that lead to an increased number of leukocytes, thrombocytes, monocytes, lysozyme and leukocyte respiratory burst activity. Furthermore, we associate the increased activity of glutathione-S-transferase in liver tissue as a strategy fish use to combat the effects of TCF. Thus, we conclude that TCF affects the physiologic parameters of Nile tilapia, which can generate irreversible situations of recovery of the clinical condition.
