BackgroundProstate cancer (PrCa) is one of the most hereditable human cancers, however, only a small fraction of patients has been shown to carry deleterious variants in known cancer predisposition genes.MethodsWhole-exome sequencing was performed in multiple affected members of 45 PrCa families to select the best candidate genes behind part of the PrCa missing hereditability. Recurrently mutated genes were prioritised, and further investigated by targeted next-generation sequencing in the whole early-onset and/or familial PrCa series of 462 patients.ResultsPRUNE2 stood out from our analysis when also considering the available data on its association with PrCa development. Ten germline pathogenic/likely pathogenic variants in the PRUNE2 gene were identified in 13 patients. The most frequent variant was found in three unrelated patients and identical-by-descent analysis revealed that the haplotype associated with the variant is shared by all the variant carriers, supporting the existence of a common ancestor.DiscussionThis is the first report of pathogenic/likely pathogenic germline variants in PRUNE2 in PrCa patients, namely in those with early-onset/familial disease. Importantly, PRUNE2 was the most frequently mutated gene in the whole series, with a deleterious germline variant identified in 2.8% of the patients, representing a novel prostate cancer predisposition gene.

The aim of this article is to assess the most relevant factors influencing customer trust in the Portuguese banking sector following the Global Financial Crisis. It also aims to evaluate the impact of trust on satisfaction and satisfaction on loyalty. To address the research objectives and the hypotheses posed, a quantitative study with a descriptive design was conducted. Data was collected through an online survey administered to a sample of bank clients residing in Portugal. The findings indicate that respondents generally trust Portuguese banking institutions, although this trust has been affected by the Global Financial Crisis. The bank's reputation and financial performance were identified as critical factors in the respondents' choice of bank. Additionally, the results suggest that both global and domestic financial conditions, bank reputation, client satisfaction, and overall trust significantly influence client loyalty to the bank. This study provides valuable insights into client behavior and perceptions of banks, emphasizing the importance of factors such as trust, client satisfaction, and bank reputation in shaping client loyalty. Palliative care is a holistic and multidisciplinary care that focuses on the patient and their family/caregivers, prioritising comfort and quality of life. Primary healthcare involvement is crucial to making palliative care practices accessible, early, and effective. This article reports the case of a woman diagnosed with metastatic gastric adenocarcinoma (pT4bN3M1), with rapid progression, whose follow-up and palliative guidance were carried out by her family doctor, given the patient's and her family's reluctance to be referred to palliative care. This case report reinforces the role and impact of the family doctor in timely and correct coordination with palliative care, as well as the need to dispel misconceptions associated with palliative care, particularly the idea that it is exclusive to end-of-life situations and an alternative to curative treatments. Em 1940, o governo português aprovou um plano nacional de construção de escolas primárias denominado “Plano dos Centenários” que projectava um aumento de 60% do número de estabelecimentos de ensino primário. Através de uma análise estatística e após uma exaustiva recolha de dados, identificam-se vários factos relativos à escolarização em Portugal a nível geográfico, de género, e de tipo de escola à data do início do Plano dos Centenários e descreve-se a distribuição das novas escolas primárias por distritos, concelhos e freguesias prevista no plano. Em 1940, a distribuição de escolas e o aproveitamento dos recursos escolares era muito desigual a nível geográfico, e existiam grandes diferenças entre os géneros. O plano delineava uma maior expansão em termos relativos nas áreas mais desfavorecidas do Sul e interior do país. Finalmente, utiliza-se o caso do distrito de Évora para demostrar o atraso na execução do plano In 1940, the Portuguese Government approved the denominated “Plano dos Centenários,” a massive plan of primary school construction all over the country. The plan projected a 60% increase in the number of primary schools. Based on a thorough data collection and statistical analysis, we establish several facts regarding the situation of formal education and schooling levels in Portugal prior to the beginning of the plan by region, gender and type of school, as well as provide a description of the plan’s strategy regarding the location of new schools. In 1940, the distribution of schools was uneven at the geographical level, and there were great gender differences in educational achievement. The plan involved a larger relative expansion of schools in the most disadvantaged areas of the country. Finally, we describe the delays in the plan’s execution in a particular example, the Évora region En 1940, le gouvernement portugais a approuvé le «Plano dos Centenários»: un ambitieux projet de construction d’écoles primaires dans tout le pays. Le plan était décisif pour le pays car il prévoyait une augmentation de 60% du nombre d’écoles primaires. Grace a une base de données approfondie et une analyse statistique, nous établissons plusieurs faits concernant la place de l’éducation formelle et les niveaux de scolarisation au Portugal avant le début du plan, et ceci par région, sexe et type d’école. En 1940, la répartition des écoles était inégale au niveau géographique, et il y avait de grandes différences dans les résultats scolaires entre les sexes. Nous proposons une description de la stratégie d’implantation de nouvelles écoles du plan et, enfin, nous décrivons l’exécution du plan à l’aide de l’exemple particulier de la région d’Evora.

21. High-tech aid tool to monitor postural stability in Parkinson’s Disease

Dissertação de mestrado integrado em Engenharia Biomédica, Parkinson’s disease (PD) is a neurodegenerative disease that affects around 1% of the population over 65 and has increased in prevalence in recent years. One of the most disabling motor symptoms and a major contributor to falls is postural instability, which threatens the independence and well-being of people with PD. Usually, physicians assess this symptom with a traditional clinical examination named pull test, which, although easy to administer without requiring any instruments, it is a difficult test to standardize and lacks sensitivity to small but significant changes. Thus, other approaches based on high technologies have emerged to provide objective metrics and long-term data on postural stability, complementing clinical assessment. Wearable sensors appeared as a promising tech-based solution to better capture postural instability and eliminate the subjectivity of postural-associated clinical examinations. This dissertation proposes the design, development and validation of a postural assessment tool to perform more objective evaluations of postural instability during basic dynamic day-to-day activities. To achieve this goal, the following steps were accomplished: (i) create a dataset based on 3D motion data of PD patients performing the pull test and dynamic activities using an inertial measurement unit (IMU); (ii) extract relevant features from the data collected, conduct an extensive statistical search, and find correlations to clinical scales; (iii) implement a tool based in artificial intelligence (AI) to classify the level of postural instability through the data collected. Different deep learning models were designed and several combinations of data input were considered in order to find the best model to predict the pull test score. Overall, satisfactory results were achieved as the statistical analysis revealed that many features were considered relevant to distinguish between the scores of the pull test, for diagnostic purposes and also to differentiate the several stages of the disease and levels of motor disability. Regarding the AI-based tool, the results suggest that the combination of IMU-based data with deep learning may be a promising solution for postural instability assessment. The model that achieved the best performance in the testing phase with unseen data presented an accuracy, precision, recall and F1-score of approximately 0.86. The results also show that when fewer daily activities are included in the dataset, the complexity of the model reduces, making it more efficient. Despite the promising results, more data should be collected to assess the actual performance of the model as a postural assessment tool., A doença de Parkinson (DP) é uma doença neurodegenerativa que afeta cerca de 1% da população acima de 65 anos e cuja prevalência tem aumentado nos últimos anos. Um dos sintomas motores mais incapacitantes e um dos principais contribuintes para quedas é a instabilidade postural, que ameaça a independência e o bem-estar das pessoas com a DP. Normalmente, o teste utilizado para avaliar a instabilidade postural é o pull test, que, embora fácil de executar e não necessitando de qualquer instrumento, é um teste difícil de padronizar e com falta de sensibilidade para detetar pequenas alterações que podem ser significativas. Assim, os sensores vestíveis surgiram como uma solução promissora para capturar a instabilidade postural e eliminar a subjetividade dos exames clínicos associados à postura. Esta dissertação tem como objetivo o idealizar, desenvolver e validar um instrumento para realizar avaliações mais objetivas da instabilidade postural durante atividades dinâmicas básicas do dia-a-dia. Para atingir esse objetivo, as seguintes etapas foram realizadas: (i) criar um dataset baseado em dados de movimento 3D de pacientes com a DP enquanto executam o pull test e atividades dinâmicas através de uma unidade de medida inercial; (ii) extrair características relevantes dos dados adquiridos, realizar uma extensa pesquisa estatística e encontrar correlações com escalas clínicas; (iii) implementar uma ferramenta baseada em inteligência artificial (IA) para classificar o nível de instabilidade postural através dos dados recolhidos. É de notar que diferentes frameworks de deep learning foram projetados e vários datasets foram considerados de modo a encontrar o melhor modelo para prever a pontuação da escala do pull test. No geral, os resultados alcançados foram satisfatórios, pois o estudo estatístico revelou que muitas das características extraidas dos sinais recolhidos foram consideradas relevantes para distinguir entre as pontuações do pull test, para fins diagnósticos e também para diferenciar os estágios da doença e os níveis de incapacidade motora. Em relação à ferramenta baseada em IA, os resultados apresentados sugerem que o deep learning pode ser promissor na área de avaliação de instabilidade postural através de IMUs. O modelo que obteve o melhor desempenho apresentou uma exatidão, precisão, sensibilidade e F1-score no teste de aproximadamente 0.86. Os resultados também mostram que dataset com um menor número de actividades diferentes incluídas leva a que o modelo se torne menos complexo, tornando-o mais eficiente. Apesar dos resultados promissores, mais dados devem ser recolhidos para avaliar o real desempenho do modelo como ferramenta de avaliação postural.

In this article, we analyze the issue of the psychological impact of the coronavirus disease 2019 (Covid-19) in view of its incidence in a context of exclusion and social precariousness. We refer to subjective experiences of rupture of social and symbolic bonds that are related to different forms of belonging: economic, political, racial, and cultural. The threat that the pandemic represents and the ways of coping with it reveal central aspects of society nowadays, considering the singularity of its different layers. From a psychoanalytical point of view, stressing the articulation of subjectivity/social bond, we study the idea that the phenomenon of Covid-19 intensifies the processes of exclusion, already present in the contemporary social scene, with nefarious consequences for the psyche of subjects whose existence is marked by social invisibility. In the context of a situation of social insecurity, exacerbated by unprecedented economic, political, and health crisis, individuals face a daily struggle for survival, which can result in a traumatic experience of subjective urgency. En este artículo, analizamos el tema del impacto psicológico de la coronavirus disease 2019 (Covid-19) en vista de su incidencia en un contexto de exclusión y precariedad social. Se trata de experiencias subjetivas de ruptura de vínculos sociales y simbólicos que se relacionan con diferentes planes de pertenencia colectiva: económico, político, racial y cultural. Las formas de enfrentamiento y la propia amenaza que representa la pandemia revelan la sociedad actual, teniendo especialmente en cuenta la singularidad de sus diferentes estratos. Desde un punto de vista psicoanalítico, destacando la articulación subjetividad/vinculo social, exploramos la idea de que el fenómeno de Covid-19 produce efectos de profundización de los procesos de exclusión, ya existentes en la escena social contemporánea, con consecuencias nefastas para la psique de sujetos cuya existencia está marcada por la invisibilidad social. Ante una situación creciente de desprotección, agravada por la crisis económica, política y sanitaria, el sujeto enfrenta una lucha diaria por la supervivencia, lo que puede resultar en una experiencia traumática de urgencia subjetiva. Neste artigo, é analisada a questão do impacto psicológico produzido pela coronavirus disease 2019 (Covid-19) tendo em vista sua incidência num contexto de exclusão e precariedade social. Trata-se de experiências subjetivas de ruptura de laços sociais e simbólicos que são relativas a diferentes planos de pertencimento coletivo: econômico, político, racial e cultural. Os modos de enfrentamento e a própria ameaça que a pandemia representa são reveladores da sociedade atual considerando-se, particularmente, a singularidade de suas diferentes camadas. A partir de um referencial psicanalítico, com ênfase na articulação subjetividade/laço social, é explorada a ideia de que o fenômeno da Covid-19 produz efeitos de aprofundamento de processos de exclusão, já existentes na cena social contemporânea, com consequências nefastas para o psiquismo de sujeitos cuja existência é marcada pela invisibilidade social. 28. Colesteatoma primário do canal auditivo externo: Relato de caso pediátrico

- Abstract
Relata-se o caso de uma criança de 12 anos de idade, do sexo feminino, com queixas de otalgia e otorreia do ouvido esquerdo com 3 meses de evolução. Ao exame objetivo era evidente uma lesão polipóide que obliterava em completo o canal auditivo externo (CAE). A tomografia computadorizada de ouvido caracterizou uma extensa massa tecidular de partes moles, que ocupava e alargava o CAE, com remodelação e erosão das respetivas paredes, aspetos compatíveis com provável colesteatoma do canal auditivo externo. Foi realizada abordagem cirúrgica, com canalplastia e excisão completa do colesteatoma que tinha origem aparente na parede póstero- superior do CAE. Não houve evidência de atresia ou estenose do CAE. Admitiu-se um CCAE primário. O pós-operatório decorreu sem intercorrências e sem evidência de recidiva a um ano. Baseando-nos nas análises genealógica e epistemológica propostas por Foucault e Lantéri-Laura, nosso objetivo principal é discernir quais são as principais linhas discursivas que constituíram o saber em torno da perversão, produzindo um sujeito dito perverso. Emerge de maneira fundamental dessa leitura como a ideia de transgressão de limites acompanha a história dos perversos: seja o limite moral imposto pela autoridade divina, seja o limite jurídico determinado pela lei penal, seja o limite do normal definido pela medicina, a perversão sempre aponta para o excesso transgressor. Tais leituras constituem o solo epistemológico sobre o qual Freud se apoia na concepção de uma metapsicologia cujo eixo central é a sexualidade. O discurso freudiano subverte o estatuto da perversão ao afirmá-la como dimensão inescapável da sexualidade e da constituição da subjetividade humana. Based on the genealogical and epistemological analyzes proposed by Foucault and Lanteri-Laura, we aim to discern which are the main discursive lines that constituted the knowledge around perversion, producing a so-called perverse subject. It emerges in a fundamental way from this reading how the idea of transgression of limits accompanies the history of the perverse: whether the moral limit imposed by divine authority, whether the legal limit determined by criminal law, or the normal limit defined by medicine, perversion always points towards the transgressive excess. Such readings constitute the epistemological ground on which Freud relies to develop the conception of a metapsychology whose central axis is sexuality. The Freudian discourse subverts the status of perversion by affrming it as an inescapable dimension of sexuality and of the constitution of human subjectivity. Sur la base des analyses généalogique et épistémologique proposées par Foucault et Lanteri-Laura, notre objectif principal est de discerner quelles sont les principales lignes discursives qui constituaient les connaissances autour de la perversion, produisant un sujet dit pervers. Il ressort de manière fondamentale de cette lecture comment l’idée de transgression des limites accompagne l’histoire du pervers : soit la limite morale imposée par l’autorité divine, soit la limite légale déterminée par le droit pénal, ou la limite normale définie par la médecine, la perversion pointe toujours dans la direction de l’excès transgressif. Ces lectures constituent le terrain épistémologique sur lequel Freud s’appuie pour la conception d’une métapsychologie dont l’axe central est la sexualité. Le discours freudien subvertit le statut de perversion en l’affrmant comme une dimension incontournable de la sexualité et de la constitution de la subjectivité humaine. Basado en los análisis genealógico y epistemológico propuestos por Foucault y por Lanteri-Laura, nuestro principal objetivo es discernir cuáles son las principales líneas discursivas que constituyeron el conocimiento en materia de perversión, produciendo un sujeto denominado perverso. De esta lectura surge de manera fundamental la idea de que la transgresión de los límites acompaña la historia de lo perverso: ya sea el límite moral impuesto por la autoridad divina, el límite legal determinado por el derecho penal o el límite normal definido por la medicina, la perversión siempre señala el exceso transgresor. Tales lecturas constituyen el fundamento epistemológico en el que Freud se basa para la concepción de una metapsicología cuyo eje central es la sexualidad. El discurso freudiano trastorna el estatuto de la perversión al afirmarla como una dimensión ineludible de la sexualidad y de la constitución de la subjetividad humana.

Com base em um projeto de intervenção social clínica, fundamentado na psicanálise e dirigido ao atendimento de professores do ensino básico da rede municipal do Rio de Janeiro, são analisados neste artigo aspectos envolvidos em sua experiência subjetiva. É priorizada a questão de sua relação com a esfera do trabalho, tendo como foco central as ressonâncias psíquicas do confronto sofrido por eles com a situação da pandemia da Covid-19. Trata-se de abordar seu impacto na rotina das atividades de trabalho desses sujeitos, na área da Educação, já marcadas pela precariedade social, fonte de vulnerabilidade psíquica. O isolamento social imposto pela pandemia, aliado a outras consequências, evidenciou, e mesmo acentuou esse quadro, objeto de especial atenção na proposta clínica, cujas linhas mestras são brevemente trazidas no texto. Basado en un proyecto de intervención social clínica, apoyado en la psicoanálisis y dirigido a la asistencia a profesores de la enseñanza de la red municipal en Rio de Janeiro, analizamos aspectos envueltos en su experiencia subjetiva. La cuestión de su relación con la esfera del trabajo es priorizada, habiendo como foco central las resonancias psíquicas del confronto que ellos sufren con la situación de la pandemia de Covid-19. Es cuestión de abordar su impacto en la rutina de las actividades de trabajo de esos sujetos, en el área de la Educación, ya marcadas por la precariedad social, fuente de vulnerabilidad psíquica. El aislamiento social impuesto por la pandemia ha evidenciado tal cuadro, objeto de atención en la propuesta clínica, cuyas líneas maestras están brevemente expuestas en el texto. En tenant comme base un projet d’intervention social clinique, en ayant comme fondement la psychanalyse, projet consacré au soin de professeurs de l’enseignement assuré par le reseau municipal de Rio de Janeiro, dans cet article des aspects liés à leur expérience subjective seront analysés. La question de leur relation avec le champs du travail sera privilegiée dont l’axe central sont les ressonances psychiques de la confrontation de ces sujets avec la pandémie du Covid 19. Il s’agit d’aborder son impact sur leur quotidien de travail, dans le champs de l’Education, déjà marqué par la precarité social, source de vulnérabilité psychique. L’isolement social, imposé par la pandémie a evidentié et a aussi acentué ce cadre, objet d’attention dans la proposition clinique dont les grandes lignes sont syntétiquement presentées dans ce texte. Based on a project of social clinical intervention, founded on psychoanalysis and addressed to the assistance of teachers of basic education in the local network in Rio de Janeiro, we examine in this article some features involved in their subjective experience. The issue of their relation to the sphere of work is prioritized, having as a central focus the psychic resonances of the confrontation they endured in the situation of Covid-19 pandemic. It’s a matter of approaching its impact on the routine of work of these individuals in the area of education, already marked by social precariousness, source of psychic vulnerability. The social isolation imposed by the pandemic highlighted and even reinforced such picture, object of attention in the clinical proposition whose guidelines are briefly developed in the text.

BACKGROUND: Germline ATM mutations are suggested to contribute to predisposition to prostate cancer (PrCa). Previous studies have had inadequate power to estimate variant effect sizes. OBJECTIVE: To precisely estimate the contribution of germline ATM mutations to PrCa risk. DESIGN, SETTING, AND PARTICIPANTS: We analysed next-generation sequencing data from 13 PRACTICAL study groups comprising 5560 cases and 3353 controls of European ancestry. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Variant Call Format files were harmonised, annotated for rare ATM variants, and classified as tier 1 (likely pathogenic) or tier 2 (potentially deleterious). Associations with overall PrCa risk and clinical subtypes were estimated. RESULTS AND LIMITATIONS: PrCa risk was higher in carriers of a tier 1 germline ATM variant, with an overall odds ratio (OR) of 4.4 (95% confidence interval [CI]: 2.0-9.5). Neste artigo, a adolescência é apresentada a partir de múltiplas dimensões que a caracterizam, fundamentada por formulações de distintos, porém complementares, campos disciplinares sobre essa experiência de vida. A proteção do adolescente é considerada uma atribuição coletiva, incluindo, além da autoproteção, um amplo conjunto relacionai e institucional com seus recursos diferenciados, necessários e desejáveis para o asseguramento da qualidade de vida. Tendo como foco a negligência, em suas diferentes figuras, a adolescência é explorada em sua dimensão relacionai, segundo a perspectiva de três áreas: a Medicina, o Serviço Social e a Psicanálise. O artigo é finalizado com um relato de atendimento produzido pelas autoras inspirado em demandas cotidianas de uma unidade de saúde, envolvendo uma adolescente em situação marcada por formas distintas de negligência, reflexão na qual se evidencia a riqueza de uma análise interdisciplinar acerca das temáticas exploradas., En este artículo se presenta la adolescencia desde las múltiples dimensiones que la caracterizan, a partir de formulaciones de campos disciplinares diferentes pero complementarios sobre esta experiencia de vida. Adolescent protection is considered a collective attribution, including, in addition to self-protection, a broad relational and institutional set with its differentiated, necessary and desirable resources to ensure quality of life. Focusing on neglect, in its different figures, adolescence is explored in its relational dimension, according to the perspective of three areas: medicine, social work and psychoanalysis. The article ends with a report produced by the authors, inspired by the daily demands of a health unit, involving an adolescent subjected to different forms of neglect, a reflection in which the richness of an interdisciplinary analysis on the themes explored is highlighted.

The identification of recurrent founder variants in cancer predisposing genes may have important implications for implementing cost-effective targeted genetic screening strategies. In this study, we evaluated the prevalence and relative risk of the CHEK2 recurrent variant c.349A>, G in a series of 462 Portuguese patients with early-onset and/or familial/hereditary prostate cancer (PrCa), as well as in the large multicentre PRACTICAL case&ndash, control study comprising 55,162 prostate cancer cases and 36,147 controls. Additionally, we investigated the potential shared ancestry of the carriers by performing identity-by-descent, haplotype and age estimation analyses using high-density SNP data from 70 variant carriers belonging to 11 different populations included in the PRACTICAL consortium. The CHEK2 missense variant c.349A>, G was found significantly associated with an increased risk for PrCa (OR 1.9, 95% CI: 1.1&ndash, 3.2). A shared haplotype flanking the variant in all carriers was identified, strongly suggesting a common founder of European origin. Additionally, using two independent statistical algorithms, implemented by DMLE+2.3 and ESTIAGE, we were able to estimate the age of the variant between 2300 and 3125 years. By extending the haplotype analysis to 14 additional carrier families, a shared core haplotype was revealed among all carriers matching the conserved region previously identified in the high-density SNP analysis. These findings are consistent with CHEK2 c.349A>, G being a founder variant associated with increased PrCa risk, suggesting its potential usefulness for cost-effective targeted genetic screening in PrCa families.

- Abstract
The identification of recurrent founder variants in cancer predisposing genes may have important implications for implementing cost-effective targeted genetic screening strategies. In this study, we evaluated the prevalence and relative risk of the CHEK2 recurrent variant c.349A>G in a series of 462 Portuguese patients with early-onset and/or familial/hereditary prostate cancer (PrCa), as well as in the large multicentre PRACTICAL case-control study comprising 55,162 prostate cancer cases and 36,147 controls. Additionally, we investigated the potential shared ancestry of the carriers by performing identity-by-descent, haplotype and age estimation analyses using high-density SNP data from 70 variant carriers belonging to 11 different populations included in the PRACTICAL consortium. The CHEK2 missense variant c.349A>G was found significantly associated with an increased risk for PrCa (OR 1.9; 95% CI: 1.1-3.2). A shared haplotype flanking the variant in all carriers was identified, strongly suggesting a common founder of European origin. Additionally, using two independent statistical algorithms, implemented by DMLE+2.3 and ESTIAGE, we were able to estimate the age of the variant between 2300 and 3125 years. By extending the haplotype analysis to 14 additional carrier families, a shared core haplotype was revealed among all carriers matching the conserved region previously identified in the high-density SNP analysis. These findings are consistent with CHEK2 c.349A>G being a founder variant associated with increased PrCa risk, suggesting its potential usefulness for cost-effective targeted genetic screening in PrCa families.

BACKGROUND: Mutations in BRCA2 cause a higher risk of early-onset aggressive prostate cancer (PrCa). The IMPACT study is evaluating targeted PrCa screening using prostate-specific-antigen (PSA) in men with germline BRCA1/2 mutations.OBJECTIVE: To report the utility of PSA screening, PrCa incidence, positive predictive value of PSA, biopsy, and tumour characteristics after 3 yr of screening, by BRCA status.DESIGN, SETTING, AND PARTICIPANTS: Men aged 40-69 yr with a germline pathogenic BRCA1/2 mutation and male controls testing negative for a familial BRCA1/2 mutation were recruited. Participants underwent PSA screening for 3 yr, and if PSA > 3.0 ng/ml, men were offered prostate biopsy.OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: PSA levels, PrCa incidence, and tumour characteristics were evaluated. Statistical analyses included Poisson regression offset by person-year follow-up, chi-square tests for proportion t tests for means, and Kruskal-Wallis for medians.RESULTS AND LIMITATIONS: A total of 3027 patients (2932 unique individuals) were recruited (919 BRCA1 carriers, 709 BRCA1 noncarriers, 902 BRCA2 carriers, and 497 BRCA2 noncarriers). After 3 yr of screening, 527 men had PSA > 3.0 ng/ml, 357 biopsies were performed, and 112 PrCa cases were diagnosed (31 BRCA1 carriers, 19 BRCA1 noncarriers, 47 BRCA2 carriers, and 15 BRCA2 noncarriers). Higher compliance with biopsy was observed in BRCA2 carriers compared with noncarriers (73% vs 60%). Cancer incidence rate per 1000 person years was higher in BRCA2 carriers than in noncarriers (19.4 vs 12.0; p = 0.03); BRCA2 carriers were diagnosed at a younger age (61 vs 64 yr; p = 0.04) and were more likely to have clinically significant disease than BRCA2 noncarriers (77% vs 40%; p = 0.01). No differences in age or tumour characteristics were detected between BRCA1 carriers and BRCA1 noncarriers. The 4 kallikrein marker model discriminated better (area under the curve [AUC] = 0.73) for clinically significant cancer at biopsy than PSA alone (AUC = 0.65).CONCLUSIONS: After 3 yr of screening, compared with noncarriers, BRCA2 mutation carriers were associated with a higher incidence of PrCa, younger age of diagnosis, and clinically significant tumours. Therefore, systematic PSA screening is indicated for men with a BRCA2 mutation. Further follow-up is required to assess the role of screening in BRCA1 mutation carriers.PATIENT SUMMARY: We demonstrate that after 3 yr of prostate-specific antigen (PSA) testing, we detect more serious prostate cancers in men with BRCA2 mutations than in those without these mutations. We recommend that male BRCA2 carriers are offered systematic PSA screening.

In this article, we analyze the issue of the psychological impact of the coronavirus disease 2019 (Covid-19) in view of its incidence in a context of exclusion and social precariousness. We refer to subjective experiences of rupture of social and symbolic bonds that are related to different forms of belonging: economic, political, racial, and cultural. The threat that the pandemic represents and the ways of coping with it reveal central aspects of society nowadays, considering the singularity of its different layers. From a psychoanalytical point of view, stressing the articulation of subjectivity/social bond, we study the idea that the phenomenon of Covid-19 intensifies the processes of exclusion, already present in the contemporary social scene, with nefarious consequences for the psyche of subjects whose existence is marked by social invisibility. In the context of a situation of social insecurity, exacerbated by unprecedented economic, political, and health crisis, individuals face a daily struggle for survival, which can result in a traumatic experience of subjective urgency.
