Abstract

[Background] P. aeruginosa bacteremia is a common and severe infection carrying high mortality in older adults. We aimed to evaluate outcomes of P. aeruginosa bacteremia among old adults (≥ 80 years)., [Methods] We included the 464/2394 (19%) older adults from a retrospective multinational (9 countries, 25 centers) cohort study of individuals hospitalized with P. aeruginosa bacteremia. Bivariate and multivariable logistic regression models were used to evaluate risk factors for 30-day mortality among older adults., [Results] Among 464 adults aged ≥ 80 years, the mean age was 84.61 (SD 3.98) years, and 274 (59%) were men. Compared to younger patients, ≥ 80 years adults had lower Charlson score; were less likely to have nosocomial acquisition; and more likely to have urinary source. Thirty-day mortality was 30%, versus 27% among patients 65–79 years (n = 894) and 25% among patients < 65 years (n = 1036). Multivariate analysis for predictors of mortality among patients ≥ 80 years, demonstrated higher SOFA score (odds ratio [OR] 1.36, 95% confidence interval [CI] 1.23–1.51, p < 0.001), corticosteroid therapy (OR 3.15, 95% CI: 1.24–8.01, p = 0.016) and hospital acquired P. aeruginosa bacteremia (OR 2.30, 95% CI: 1.33–3.98, p = 0.003) as predictors. Appropriate empirical therapy within 24 h, type of definitive anti-pseudomonal drug, and type of regimen (monotherapy or combination) were not associated with 30-day mortality., [Conclusions] In older adults with P. aeruginosa bacteremia, background conditions, place of acquisition, and disease severity are associated with mortality, rather than the antimicrobial regimen. In this regard, preventive efforts and early diagnosis before organ failure develops might be beneficial for improving outcomes.

Abstract

[Introduction] There is no consensus regarding optimal duration of antibiotic therapy for Pseudomonas aeruginosa bacteremia. We aimed to evaluate the impact of short antibiotic course., [Methods] We present a retrospective multicenter study including patients with P. aeruginosa bacteremia during 2009–2015. We evaluated outcomes of patients treated with short (6–10 days) versus long (11–15 days) antibiotic courses. The primary outcome was a composite of 30-day mortality or bacteremia recurrence and/or persistence. Univariate and inverse probability treatment-weighted (IPTW) adjusted multivariate analysis for the primary outcome was performed. To avoid immortal time bias, the landmark method was used., [Results] We included 657 patients; 273 received a short antibiotic course and 384 a long course. There was no significant difference in baseline characteristics of patients. The composite primary outcome occurred in 61/384 patients in the long-treatment group (16%) versus 32/273 in the short-treatment group (12%) (p = 0.131). Mortality accounted for 41/384 (11%) versus 25/273 (9%) of cases, respectively. Length of hospital stay was significantly shorter in the short group [median 13 days, interquartile range (IQR) 9–21 days, versus median 15 days, IQR 11–26 days, p = 0.002]. Ten patients in the long group discontinued antibiotic therapy owing to adverse events, compared with none in the short group. On univariate and multivariate analyses, duration of therapy was not associated with the primary outcome., [Conclusions] In this retrospective study, 6–10 days of antibiotic course for P. aeruginosa bacteremia were as effective as longer courses in terms of survival and recurrence. Shorter therapy was associated with reduced length of stay and less drug discontinuation.

Abstract

Parent training (PT) interventions constitute an empirically demonstrated alternative to promote effective parenting practices and prevent child behavioral and mental health problems. However, the dissemination of evidence‐based PT interventions across Latin America remains scarce. This qualitative study had the primary objective of evaluating the level of acceptability of a culturally adapted version of the PT intervention known as GenerationPMTO©, adapted for the Chilean context. According to qualitative reports provided by 24 Chilean caregivers exposed to the culturally adapted parenting intervention, the intervention was perceived by caregivers as useful for their parenting practices, as well as contextually and culturally relevant. Current qualitative findings indicate that the culturally adapted PT intervention holds promise for larger dissemination in the Chilean context. [ABSTRACT FROM AUTHOR]

Abstract

The dissemination of evidence‐based parent training (PT) interventions remains extremely limited in Latin American countries. This is concerning when considering the high prevalence of child maltreatment associated with punitive parenting practices across countries in the region. Furthermore, efforts to disseminate PT interventions must be conducted by adhering to the core parenting components that have established effectiveness for such interventions, while ensuring contextual and cultural relevance for focus populations. In this manuscript, we describe the cultural adaptation of an evidence‐based PT intervention in the context of Chile. This initial phase of adaptation was informed by the theoretical tenets of the Ecological Validity Model of cultural adaptation (Bernal et al., J. Abnorm. Child Psychol., 23, 1995, 67). According to findings from a qualitative thematic analysis conducted with five interventionists in training, therapists perceived that the intervention's core components were relevant to Chilean caregivers who participated in the parenting program. Interventionists also provided specific suggestions to enhance the intervention's contextual and cultural relevance. This investigation illustrates the importance of culturally adapting evidence‐based interventions according to comprehensive cultural adaptation frameworks, prior to engaging in large‐scale dissemination of adapted interventions in Latin American contexts. La lumbalgia mecánica es una condición cada vez más prevalente en el mundo laboral, a causa de muchos motivos, tales como la renovación de fuerza laboral, cambios de dinámicas industriales y empresariales que someten al trabajador a esfuerzos físicos repetitivos y de magnitud variable, incidentes y accidentes en el sitio de trabajo; lo que al final significa un trabajador con dolor e incapacidad funcional ya sea temporal o permanente, y los costos que esto significa tanto para el trabajador, como para la empresa. El presente diseño busca establecer un protocolo para la realización de una revisión sistemática, que permita establecer con mayor nivel de evidencia científica que el actualmente descrito en las Guías Colombianas Basadas en la Evidencia (GATISO), sentando un precedente en el manejo inicial de las lumbalgias agudas, optimizando al máximo el recurso físico e intelectual disponible para el manejo de estos trabajadores. Mechanical low back pain is an increasingly prevalent condition in the workplace, cause of many reasons, such as the workforce’s renewal, changes in industrial and business dynamics that at some point a worker must to do repetition efforts of varying magnitude, incidents and accidents at the workplace; What ultimately means a worker with pain and functional disability either temporary or permanent, and the costs either for worker as for the company. The present design seeks to establish a protocol for carrying out a systematic review, allowing it to establish a greater degree of scientific evidence that is currently in the Colombian Evidence-Based Guidelines (GATISO), setting a precedent in the initial management of acute low back pain , optimizing to the maximum the physical and intellectual resource available for the management of these workers. Abstract

Latino/a populations in the United States are negatively impacted by widespread mental health disparities. Although the dissemination of culturally relevant parent training (PT) programs constitutes an alternative to address this problem, there is a limited number of efficacious culturally adapted PT prevention interventions for low‐income Latino/a immigrant families with adolescents. The current manuscript describes the level of acceptability of a version of the GenerationPMTO® intervention adapted for Latino/a immigrant families, with an explicit focus on immigration‐related challenges, discrimination, and promotion of biculturalism. Qualitative reports were provided by 39 immigrant parents who successfully completed the prevention parenting program. The majority of these parents self‐identified as Mexican‐origin. According to qualitative findings, participants reported overall high satisfaction with immigration and culture‐specific components. Parents also expressed high satisfaction with the core GenerationPMTO parenting components and provided specific recommendations for improving the intervention. Current findings indicate the need to adhere to the core components that account for the effectiveness of PT interventions. Equally important is to thoroughly adapt PT interventions according to the cultural values and experiences that are relevant to target populations, as well as to overtly address experiences of discrimination that negatively impact underserved Mexican‐origin immigrant families. Due to the exploratory nature of this study, the efficacy and effectiveness of the adapted prevention intervention remains to be established in empirical research. [ABSTRACT FROM AUTHOR]

Abstract

La prevalencia de asma en adolescentes parece estar aumentando al igual que el tabaquismo. El presente estudio pretende conocer la prevalencia de asma en estudiantes de nuevo ingreso a la Universidad Autónoma de Ciudad Juárez (UACJ) y su relación con la exposición pasiva o activa al tabaco. Se realizó un estudio transversal y mediante un cuestionario estructurado y modificado basado en el Estudio Internacional de Asma y Alergias en la Niñez (ISAAC por sus siglas en inglés) se realizó el diagnóstico de asma y se determinó la exposición pasiva o activa al tabaco. Para el análisis estadístico se utilizó la prueba de Chi cuadrado ( ) para estudiar que las distribuciones de las variables (tabaquismo y género) son independientes unas de otras con relación a la categoría asma en estudiantes universitarios. El nivel de significación estadística para la prueba fue de ( ). De la población total el fueron hombres y el mujeres. El de los hombres sufrían de asma mientras que el de las mujeres fue diagnosticada con asma. De los estudiantes con asma el declararon ser fumadores activos, el estaban expuestos al tabaco de forma pasiva y el no fumaban. En este estudio no se encontró una correlación estadísticamente significativa entre el género y la prevalencia de asma con un valor de , con intervalo de confianza [ ] y tampoco con el tabaquismo activo o pasivo , [ ], a pesar de algunos reportes, que asocian a la población adolescente de sexo masculino con la prevalencia de asma y otros al sexo femenino como factor de riego para a esta enfermedad. Los datos muestran que en la población estudiada el tabaquismo activo o pasivo no se asocia con la prevalencia de asma, sin embargo es necesario realizar un estudio longitudinal más amplio.

In some ground transportation enterprises are presented problems of stability of the performance (big variations in theirs variables) and low use of the installed capabilities. Their maintenance system has low efficiency or efficacy. It can be seen in: high fuel consumptions, high transportation costs, low use of the route, and so on. The objective of the paper is to elaborate a measurement method of the maintenance system of such enterprises, to increase the effectiveness of their operations. The contributions were: the definition of maintenance classes with new performance indicators, estimating the standard error of the Coefficient of Technical Availability (CTA); the definition of the “Route Map” for the modernization process of the transportation fleet, and the definition of a control graph for each maintenance class of the fleets. Abstract

Organizational transformation for digitalization is a daily challenge for organizations. Successful change can be defined as the combined result of a number of factors, in which the attitude, trust and/or distrust of employees towards technology is of paramount importance. The aim of this study was to explore which factors most influence employees' trust in technology and how the risk they pose can be mitigated. The quantitative research analyzed 473 respondents (Smart PLS3, using SEM model) and came to the following conclusions. Employees' trust in technology depends primarily on the supportive role of management, and to a lesser extent on the digital readiness of the company and the training provided in the organization. The supportive role of management is a key element in the model, as it affects trust not only in a direct way, but also indirectly, through several pathways in the model. This means that the supportive role of leadership is clearly a decisive influence and its importance helps to assess the risk of trust or lack of trust. [ABSTRACT FROM AUTHOR]

34. Power Over Fiber in C-RAN With Low Power Sleep Mode Remote Nodes Using SMF.

Abstract

Power over fiber (PoF) with sleep mode operation in centralized radio access networks (C-RAN) of low power Remote Radio Heads (RRH) helps to reduce power consumption. This proposal includes bundles of single mode optical fibers (SMF) as part of the 5G C-RAN front-haul solution for providing control on power consumption by selectively activating some parts of the RRH. We experimentally demonstrate a PoF system based on 14.43 km of SMF feed by 2.24 W giving 226 mW electrical power at the RRH for control, battery charge, load operation and communication purposes. A bidirectional control channel is integrated in the central office and the RRH for providing the capability of entering in sleep mode operation and to provide information about the status of the battery and sensing elements at RRH. The optical data uplink/downlink operates over separate optical fibers shared by various RRHs and achieves low power consumption below 33 mW with low data rates. The measured PV cells conversion efficiency is above 30%. The RRH has two sleep modes of operation with a minimum power consumption of 5.8 mW. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: It has been well-established that parents of children with ADHD report significantly higher levels of parenting stress and home chaos, and lower levels of parental efficacy than parents of children without ADHD. Unfortunately, most of the extant ADHD literature has focused on European American children and families, resulting in a paucity of research focusing on ethnic minority families of children with ADHD. The current study aimed to expand what is known about Latinx parents of children with ADHD by exploring contextual and cultural factors, such as parental gender and acculturation, which may account for variations in parenting experiences within this population. Methods: The present study utilized secondary data analysis to analyze ratings of parenting stress, home chaos, and parental efficacy among a sample of Latinx mothers and fathers of children with ADHD (n= 46 dyads). Results: Results indicated that Latinx mothers of children with ADHD reported higher levels of parenting stress than Latinx fathers of children with ADHD; however, no significant parental gender differences were found in ratings of parental efficacy or home chaos. Additionally, several significant relationships were found between parental acculturation and family functioning variables. Conclusions: Latinx families of children with ADHD are an understudied and underserved population within the field of clinical psychology. The current study provides critical information on Latinx family functioning within the context of ADHD treatment, specifically pertaining to the complex interplay of parenting and acculturation variables. Abstract

Al seleccionar el sujeto ”mujer rural”, en una situación ubicable en Colombia, bajo el peso de una ley (731 de 2002) como objeto de estudio de esta tesis, se ha pretendido describir y analizar una concepción más amplia del principio de no discriminación, de la importancia no solo del género al decidirse por dispositivos legislativos que promueven cambios en las prácticas sociales, sino que el énfasis de ”rural”, indica un mayor conjunto de barreras sociales y jurídicas, que pretenden superarse, pero que señalan que este sujeto ha estado más abandonado por las políticas públicas que otros que alegan desigualdad y ejercicios de discriminación, como “mujer desplazada”, “mujer discapacitada”, “mujer afro-descendiente” u otros ejemplos. El conflicto armado por 60 años que ha vivido Colombia, que hoy con el proceso de paz ha reducido en amplios porcentajes sus impactos económicos, sociales y culturales, ha sido causa primordial del abandono de las políticas agrícolas por el mismo número de años, por un Estado limitado, que no ha cubierto todo el territorio, con instituciones débiles, de poca credibilidad y con presupuesto público limitado. El cuerpo del texto propone unos antecedentes históricos, porque el sujeto “mujer rural”, también debe leerse en un contexto más amplio, con reformas agrarias debilitadas o frustradas que hacen parte de los factores que originaron el conflicto de armas y lo han potenciado a mayores niveles de confrontación. Es prudente recordar que se ha tenido presente una norma supuestamente considerada en términos de impacto respecto de un grupo como el de la “mujer rural”, de la cual se hace un balance de manera práctica, para comprender como los instrumentos de ley son lentos o eficaces, de finalidad altruista o mezquina, con bajos o altos presupuestos que igualmente afectan los derechos económicos, sociales y culturales en su perspectiva y cumplimiento.

Abstract

Background: Despite the large number of evidence-based practices (EBPs) ready for implementation, they are the exception in usual care, especially for ethnic minority patients, who may not have access to trained health professionals. Providing EBP training as part of a graduate curriculum could help build the pipeline of professionals to provide quality care. Methods: We conducted a before-after study to determine whether we could implement a blended learning strategy (BL; i.e., in vivo and online training) to teach an EBP in university settings. Feasibility in this pilot was operationalized as knowledge acquisition, satisfaction, fidelity, acceptability, and usability. Using GenerationPMTO as the EBP, our aim was to train graduate students enrolled in Psychology, Social Work, and Family Therapy programs in the EBP in one academic year. Two therapists from a community agency were also students in this pilot. A total of 13 students from five universities were trained in the intervention. Adaptations were made to the intervention and training strategy to optimize training fidelity. Focus groups were conducted with the students to capture their perspective about the training. Results: Students demonstrated significant knowledge acquisition from baseline (Mean = 61.79, SD = 11.18) to training completion (Mean = 85.27, SD = 5.08, mean difference = − 23.48, 95% CI = − 29.62, − 17.34). They also reported satisfaction with the BL format, as measured by teaching evaluations at the end of the course. Instructors received acceptable fidelity scores (range of 7–9 in a 9-point scale). Qualitative findings from focus groups showed support for acceptability and usability of BL training. Conclusions: BL training in university settings can be conducted with fidelity when provided by appropriately trained instructors. BL that integrates EBP and adaptations may be uniquely applicable for training providers in low-resource and ethnically diverse settings. The BL enhanced knowledge of GenerationPMTO was acceptable and usable to students, and was delivered with high instructor fidelity to the training model. [ABSTRACT FROM AUTHOR]

All persons included are recognized authorities and either natives of, or long-term residents in the region. It will offer an invaluable reference point, in particular for scholars of the history of psychology, Latin American studies, the history of science, and global psychology; as well as for historians, psychologists and social scientists seeking international perspectives on the development of the discipline.
