3. O Impacto da Cardiopatia Grave nas Causas de Óbito e Sobrevida após Aposentadoria por Invalidez

Resumo Fundamento: As doenças não comunicáveis são responsáveis por mortes prematuras e limitações. A aposentadoria por invalidez está associada a condições crônicas, especialmente a doenças cardiovasculares. A II Diretriz Brasileira de Cardiopatia Grave definiu critérios para enquadramento das doenças cardiovasculares. Poucos estudos abordam esse tema em instituições federais. Objetivos: Avaliar sobrevida e causas de óbito de servidores aposentados por invalidez na UFRJ, com ênfase no impacto da cardiopatia grave. Métodos: Estudo de coorte retrospectivo baseado nos registros de aposentadorias e óbitos ao longo de 15 anos. As aposentadorias foram divididas em três grupos: integral por cardiopatia grave, integral por outras doenças e proporcional. As causas de óbito foram obtidas a partir das certidões de óbito. Foram avaliadas taxa de mortalidade, sobrevida e a presença de diagnósticos concordantes entre a aposentadoria e o óbito. Foram utilizados testes qui-quadrado, log-rank, modelos de Cox e curvas de Kaplan-Meier. Significância estatística com intervalo de confiança de 95%, considerando p < 0,05. Resultados: Foram 630 aposentadorias, 368 (51,4%) no sexo feminino, com idade média de idade de 52,9 (DP=7,8) anos, e 169 (26,8%) óbitos. A mortalidade foi maior nos professores (37,0%; p=0,113), na faixa etária entre 65 e 70 anos (48,4%; p=0,004), no sexo masculino (34,0%; p=0,001), e nas aposentadorias integrais por cardiopatia grave (41,5%; p < 0,001). Diagnósticos concordantes entre aposentadoria e óbito foram mais frequentes em professores (74,1%; p=0,026) e nas aposentadorias integrais por cardiopatia grave (72,7%; p < 0,001). Conclusões: O diagnóstico de cardiopatia grave confere maior taxa de mortalidade e menor sobrevida aos aposentados por invalidez, e sua presença em maior frequência nos diagnósticos de aposentadoria e óbito ressalta sua importância neste contexto.

Introduction: Cardiovascular diseases represent an important cause of morbidity and mortality during pregnancy, cardiomyopathy is notable as a syndrome of heart failure. Objective: To characterize patients with peripartum cardiomyopathy according to clinical and epidemiological variables of interest for the investigation. Methods: A descriptive and cross-sectional study of 18 patients with diagnosis of peripartum cardiomyopathy was carried out. They were assisted at Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba from October, 2015 to December, 2022. Results: In the series there was a prevalence of patients over 35 years (32.6 average age), besides African descendant (50.0 %), chronic hypertension (44.4 %) and multiparity (8.9 %) as risk factors and the left ventricle failure as clinical manifestation. The ejection fraction of this ventricle was regularly diminished and the pharmacological treatment response was satisfactory in all the sample. Conclusions: Peripartum cardiomyopathy is of low incidence in this center; however, due to its seriousness, the early diagnosis and the specialized staff intervention are necessary to avoid complications. Heart diseases are potentially fatal pathologies considered a risk factor for anxiety disorders. This paper aims to present a systematic literature review on anxiety in cardiology. Five databases were selected: Scielo, PsycINFO, Web of Science, and PubMed. The descriptors "cardiologia" and "ansiedade" and their equivalents in English, "anxiety" and "cardiology" were used. The inclusion criteria were papers published in Portuguese, English, or Spanish between 2017 and 2021. Twenty-six studies were selected, and four topics stood out: socio-demographic profile, anxiety and cardiac rehabilitation, anxiety-related risk and protective factors, and anxiety and depression. 9. Epidemiología de las enfermedades cardiovasculares: una revisión narrativa.

- Abstract
Cardiovascular diseases (CVD), primarily ischemic heart disease and stroke, are the leading cause of global mortality and a major contributor to disability. The burden of CVD is influenced by genetic, metabolic, behavioral, environmental and social determinants. However, low physical activity and blood pressure play a central role as risk factors for CVD. The prevalent cases of CVD are 523 million in 2019 and the number of deaths from these pathologies increased constantly to reach 18.6 million in the same period, so, currently, heart disease and strokes were the first and the second cause of death in America, while globally, stroke remained the second cause of death. It is evident that monitoring the burden of cardiovascular diseases is of vital importance, this article reviews the total epidemiology of cardiovascular diseases, including the underlying causes of cardiovascular death and related risk factors, using global estimates and available population-level data sources. on incidence, prevalence, lethality, mortality and health risks in Latin American countries and the world in the last five years, hence this narrative update of documentary design is proposed, which aims to contribute to the knowledge of this important problem of public health, contributing to prevention actions and effective interventions to curb the burden of CVD diseases, especially in regions with lower sociodemographic indices. La atención médica y terapéutica a niños con cardiopatías presenta desafíos significativos, especialmente cuando factores subjetivos limitan su acceso a los tratamientos convencionales. Este artículo se centra en la aplicación de la cultura física como una herramienta efectiva para contribuir a la rehabilitación de niños cardiópatas que no pueden acceder a terapias convencionales en su comunidad. Se destaca la importancia de comprender y abordar esta problemática para mejorar la calidad de vida de estos niños. Para llevar a cabo esta investigación, se emplearon diversos métodos científicos, incluyendo histórico-lógicos, de análisis-síntesis, inductivo-deductivos y de enfoque de sistema, así como métodos empíricos como la observación y pruebas de medición. La muestra de estudio consistió en cuatro pacientes con cardiopatías en la comunidad de Nuevo Manzanillo, permitiendo un análisis de los mismos.El análisis de los resultados evidenció los efectos positivos del ejercicio físico sistemático en la capacidad funcional, el consumo máximo de oxígeno y la frecuencia cardíaca, entre otros aspectos. Tras la implementación de un programa de ejercicios durante cuatro meses, se observó una mejora significativa en la recuperación de los pacientes, alcanzando todos ellos una rehabilitación completa al final del periodo de estudio.

Introducción: Las enfermedades cardiovasculares representan una causa importante de morbilidad y mortalidad durante la gestación, entre las que se destaca la miocardiopatía, que cursa como un síndrome de insuficiencia cardíaca. Objetivo: Caracterizar a pacientes con miocardiopatía periparto según variables clínicas y epidemiológicas de interés para la investigación. Métodos: Se realizó un estudio descriptivo y transversal de las 18 pacientes con diagnóstico de miocardiopatía periparto, asistidas en el Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba en el período comprendido desde octubre de 2015 hasta diciembre de 2022. Resultados: En la serie predominaron las pacientes mayores de 35 años de edad (edad promedio de 32,6 años), además de la descendencia africana (50,0 %), la hipertensión arterial crónica (44,4 %) y la multiparidad (8,9 %) como factores de riesgo y la insuficiencia del ventrículo izquierdo como manifestación clínica. La fracción de eyección de dicho ventrículo estuvo regularmente disminuida y la respuesta al tratamiento farmacológico fue satisfactoria en el total de la muestra. Conclusiones: La miocardiopatía en el periparto es de baja incidencia en este centro; sin embargo, por la gravedad que representa, se impone el diagnóstico temprano y la intervención del personal especializado para evitar complicaciones.

16. Síndrome de Williams-Beuren y la importancia del diagnóstico oportuno. Reporte de caso

Objetivo: Se presenta un paciente con Síndrome de Williams-Beuren (SWB) con manifestaciones clínicas en el primer año de vida, resaltando la disfunción cognitiva/retraso global, alteraciones cardiovasculares y faciales como elementos clave para fomentar la sospecha clínica y la importancia del diagnóstico oportuno con el fin de disminuir la morbilidad y mortalidad del paciente. Descripción del caso: Paciente masculino de 12 meses de edad, producto de segunda gestación, cesárea a las 37,5 semanas, peso al nacer: 2400 g, talla 47 cm y perímetro cefálico 34 cm. Asiste a cita de control con pediatría a los 3 meses de edad donde se observa dacrioestenosis, dificultad para alimentarse y dismorfias faciales menores. En los antecedentes familiares; padres jóvenes, sin historial de enfermedades genéticas, no consanguíneos, el resto de antecedentes sin relevancia clínica. Se realizó seguimiento por 12 meses debido a la severidad y presentación inusual de algunas manifestaciones clínicas que ocurren en fases tardías. Conclusiones: El SWB ha sido descrito por sus manifestaciones clínicas variables, sin embargo, su identificación e intervención multidisciplinaria es oportuna al conocerse las alteraciones multisistémicas de acuerdo a la edad. El SWB debe considerarse dentro de los diagnósticos diferenciales en pacientes pediátricos con retraso del desarrollo psicomotor, anomalías cardiovasculares severas y/o dimorfismos faciales ya que desempeñan un rol clave en la sospecha clínica de esta patología, reduciendo el gasto de los recursos del sistema de salud y mejorando la calidad de vida del paciente de acuerdo a sus necesidades.

Objetivo: traçar o perfil epidemiológico de cardiopatas que contraíram COVID-19 no primeiro ano pandêmico. Método: estudo observacional, do tipo coorte retrospectiva. Foram incluídos cardiopatas com resultado de PCR positivo, de março a dezembro de 2020. Excluíram-se os registros com dados incompletos para as variáveis analisadas. Na análise bivariada, foram calculadas as razões de prevalência (com IC 95%) como medida de associação entre a variável dependente (cardiopatia) e as variáveis independentes (idade, sexo, escolaridade e zona de residência) empregando-se o teste do χ2 de Mantel Haenszel. Resultados: identificou-se associação com significância estatística em relação às variáveis faixa etária, sexo e escolaridade. O maior risco de ocorrer desfecho desfavorável, foi para a faixa etária acima de 70 anos (59,97%), seguido do intervalo de idade de 60 a 69 anos (38,31%), sexo masculino (45,28%) e escolaridade para o nível superior. Conclusão: evidenciou-se que os cardiopatas requerem maiores cuidados após o diagnóstico da doença, especialmente pelo fato deste grupo apresentar um maior índice de mortalidade.

Several autoimmune rheumatic diseases (ARD) are responsible for the increased risk of cardiovascular comorbidities, whether due to atherosclerosis, thrombus formation, vasculitis or myocardial inflammation. The improvement of therapeutic practices and the increase in life expectancy of patients brought a new look at cardiovascular complications resulting from rheumatological disorders. This article considers heart disease in Rheumatic Diseases. The results demonstrated that physical activity is one of the most important behavioral interventions, as a beneficial impact on the probability of developing, suffering symptomatically or dying from CVD. Thus, maintaining a healthy lifestyle reduces risk factors for CVD, including obesity, dyslipidemia, hypertension, diabetes mellitus and possibly even inflammation. It also showed efficacy in preventing acute coronary syndromes. Exercise seems to help manage established CVDs, be it aerobic exercise or resistance training. They improve myocardial contractility and quality of life in patients with chronic heart failure. [ABSTRACT FROM AUTHOR]

Objetivo. Evaluar las características epidemiológicas, clínicas, quirúrgicas, patológicas y desenlaces en el seguimiento de los pacientes receptores de trasplante cardiaco en el Instituto Nacional Cardiovascular durante 2010-2020. Materiales y métodos. Estudio retrospectivo descriptivo de pacientes sometidos a trasplante cardiaco en un centro de referencia nacional, se describen las características clínicas, quirúrgicas, de laboratorio, de patología y la supervivencia de los pacientes hasta los 10 años de seguimiento. Resultados. Ochenta y seis pacientes fueron trasplantados en 10 años, la mediana de edad fue de 41 años (RIQ 28-56), con predominancia de varones (66,3%). Las tres primeras causas de indicación de trasplante cardiaco fueron: cardiomiopatía dilatada (48,9%), cardiopatía isquémica (17,4%) y miocarditis (6,9%). El tiempo de isquemia total fue de 160 min (RIQ 129,7-233,5). La sobrevida al año, cinco y diez años fue de 84,8%, 73,6% y 65,7%, respectivamente. La principal causa de muerte fue la infecciosa (39,1%) y de origen desconocido (26%). Conclusiones. La principal etiología de la insuficiencia cardiaca en el receptor de trasplante cardiaco en el Perú, en los últimos años, fue la cardiomiopatía dilatada no isquémica. Se observa que la tasa de sobrevida fue similar a la de registros internacionales; sin embargo, la tasa de mortalidad de causa infecciosa y muerte de origen desconocido es alta, lo cual plantea un desafío en el manejo de los pacientes postrasplante.

Introducción. La infección por el HIV-1 induce un estado de inflamación crónico en el que participan los inflamasomas. El incremento de los parámetros inflamatorios es mayor en individuos con replicación viral activa que en aquellos con control de la replicación viral. Este proceso desencadena alteraciones metabólicas relacionadas con cambios en el perfil lipídico, lo cual podría incrementar el riesgo de eventos cardiovasculares, incluso en pacientes con terapia antirretroviral. Objetivo. Establecer si existe correlación entre la expresión de los componentes de los inflamasomas y los marcadores de riesgo cardiovascular en individuos con control de la replicación viral y en aquellos con replicación viral activa con terapia antirretroviral o sin ella. Materiales y métodos. Se estudiaron 13 individuos con control de la replicación viral y 40 con replicación viral activa (19 sin terapia antirretroviral y 31 con terapia). Se evaluaron los marcadores clásicos de riesgo cardiovascular y se cuantificó mediante RT-PCR la expresión de los componentes de los inflamasomas (NLRP1, NLRP3, NLRC4, AIM2, ASC, IL-1β, IL-18 y caspasa-1), TLR2, TLR4, TGF-β e IL-10. Resultados. Se observó que los pacientes con replicación viral activa y con terapia antirretroviral presentaron un incremento en la expresión de TLR2, TLR4 e IL-18, comparados con los controladores del HIV-1. Además, mostraron grandes valores de triglicéridos y lipoproteína de muy baja densidad (Very Low Density Lipopretein, VLDL), lo que se correlaciona positivamente con la expresión de los componentes de los inflamasomas NLRP1, NLRP3, NLRC4, AIM2, ASC y caspasa-1. Conclusión. El aumento en la expresión de los componentes de los inflamasomas en los individuos con replicación viral activa y con terapia antirretroviral se correlacionó con las concentraciones de triglicéridos y VLDL, lo que sugiere el papel de la activación inmunitaria y la terapia antirretroviral en el riesgo cardiovascular.

Objectives: to analyze the prognostic nutritional index and factors associated with mortality in children and adolescents with heart disease who underwent cardiac surgery. Methods: this is a longitudinal, retrospective study that included 98 children and adolescents with heart disease from 0 to 14 years old, and assessed the prognostic nutritional index and nutritional status, through the body mass index for age, weight for height, weight for age and height for age. Multiple logistic regression analysis was performed. Results: malnutrition was present in 27 patients, 68 were categorized as having a low prognostic nutritional index and 16 died. In the adjusted analysis, malnutrition (OR=4.11; CI95%=1.26-13.40; p=0.019), the low body mass index for age (OR=4.14; CI95%=1.26-13.61; p=0.019), low weight for height (OR=4.15; CI95%=1.29-13.35; p=0.017) and low weight for age (OR=5.20; CI95%=1.39-19.43; p=0.014) were associated with mortality. Conclusions: malnutrition, low body mass index for age, weight for height and weight for age had shown a significant association with mortality. Despite being an easily applicable indicator of nutritional status, the findings suggest no association between the prognostic nutritional index and mortality in patients with congenital heart disease after cardiac surgery. This is an integrative review study on the self-care of elderly patients with heart disease. The general objective is to know the possibilities of the gerontologist nurse's role to support the self-care of elderly people with heart disease. Scientific articles were searched on the VHL platform, being found 68 studies, after refining the research according to the theme, there remained 4 researches that made up the scope of the study. With 3 studies conducted in 2015 and 1 in 2017, 2 qualitative and 2 clinical trials. Several strategies for self-care were used: group and individual care and the use of telephone messaging technology. It is concluded that gerontological nurses in their practice can use group strategies and/or individual monitoring to establish self-care. Both actions were effective in promoting health and thus favoring the quality of life of elderly patients with heart disease. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: HIV-1 infection induces a chronic inflammatory state in which inflammasomes participate. The increase in inflammatory parameters is higher in individuals with active viral replication (progressors) than in those with viral control (HIV-1 controllers). This process triggers metabolic alterations related to changes in the lipid profile, which could increase the risk of cardiovascular events, even in patients with antiretroviral therapy. Objective: To establish whether there was a correlation between the expression of inflammasome components and cardiovascular risk markers in HIV-1 controllers and progressors with or without antiretroviral therapy. Materials and methods: We studied 13 HIV-1 controllers and 40 progressors (19 without antiretroviral therapy and 31 with therapy) and evaluated in them classic markers of cardiovascular risk. Using RT-PCR we quantified the expression of inflammasome components (NLRP1, NLRP3, NLRC4, AIM2, ASC, IL-1β, IL-18, and caspase-1), TLR2, TLR4, TGF-β, and IL-10. Results: Progressors with antiretroviral therapy had an increased expression of TLR2, TLR4, and IL-18 compared to HIV-1 controllers. They also showed high levels of triglycerides and VLDL, which positively correlated with the expression of the inflammasome components NLRP1, NLRP3, NLRC4, AIM2, ASC, and caspase-1. Conclusion: Progressors receiving antiretroviral therapy exhibited an increased expression of the inflammasome components, which correlated with the levels of triglycerides and VLDL. Studies show that there is a low adherence to cardiovascular rehabilitation (CVR) implying limitations as to expected benefits. It is important to investigate whether the diversification of interventions already applied in CVR, such as the adoption of exergaming, can provide greater adherence to this treatment. Objective: To review studies that investigated the intragroup adherence to CVR of patients using exergaming and comparing, intergroup, with patients undergoing conventional CVR. Methods: This was a systematic review that included original intervention studies indexed in databases and published up to April 2021, without language restrictions. The search was performed in PubMed/Medline, Web of Science, Scopus, Cochrane, and PEDro, in April 2021. Results: A total of 8 studies were included. In 3 of them, the average adherence to CVR sessions with exergaming was 77%. In another study, about 60% of the participants were more than 80% adherent to the recommended playing time. In the fifth study, the average time of exergaming use was 28 ± 13 minutes/day, which is higher than the recommended 20 minutes/day. In the sixth study, the median of individual attendance was 96%. In the seventh, there was 100% adherence to the CVR sessions. And in the eighth study, there was a significant increase in adherence at 12 weeks of CVR with exergaming (72.87% vs. 82.80%) and a reduction when exergaming was discontinued (82.80% to 65.48%, p < 0.05). Conclusion: The best available evidence demonstrates a positive effect of exergaming on CVR patient adherence. [ABSTRACT FROM AUTHOR]

Introduction: platelet antiaggregant are part of the treatment of patients with ischemic heart disease. Objective: to characterize the response of platelet antiaggregant in patients with ischemic heart disease. Methods: observational, descriptive and cross-sectional study in patients with ischemic heart disease treated at the "Abel Santamaría Cuadrado" General Teaching Hospital between January 2018 and November 2019. The sample consisted of 146 patients receiving standard doses of ASA (tablet 125 mg ). The patients were randomly distributed into two groups: ASA groups (n= 73) and the ASA + clopidogrel group (n= 73). Results: male sex (60,9 %) and dyslipidemia as a risk factor (74,6 %) predominated. A greater number of patients without response to aggregation was found in the patients of the ASA group (10,3 % vs 6,8 %). The main risk factors for non-responders were obesity (ASA: 70,6 % vs ASA + clopidogrel: 75 %) and diabetes mellitus (ASA: 70,6 % vs ASA + clopidogrel: 87,5 %). The largest number of non-responders had an LVEF < 30 % (10,3 % of patients). Conclusions: therapeutics with ASA and ASA + clopidogrel achieve adequate platelet antiaggregation in patients with ischemic heart disease. Patients who receive only ASA tend to present a null response to antiaggregant therapy to a greater extent. Non-responders to antiaggregant therapy were mostly diabetic and obese, with a history of acute coronary syndrome and a reduced left ventricular ejection fraction.

Introducción: La ecografía obstétrica constituye una herramienta indispensable en la evaluación rutinaria de la consulta prenatal, en el diagnóstico de gestaciones múltiples, trastorno en el crecimiento fetal, anomalías placentarias y errores en la estimación de la edad gestacional. Objetivo: Describir y conocer la prevalencia de malformaciones congénitas diagnosticadas por ultrasonido en el servicio de Obstetricia del Centro Médico Docente la Trinidad. Métodos: Se realizó un estudio observacional, descriptivo de tipo retrospectivo. La población estuvo conformada por pacientes que acudieron al servicio de Obstetricia y Ginecología para el diagnóstico prenatal, que hayan presentado diagnostico ecográfico de alguna malformación fetal, en el periodo comprendido enero de 2011 junio 2021. Resultados: Se incluyeron a 58 pacientes embarazadas, la edad gestacional promedio fue de 15,9,5 ± 4,5 semanas, el estrato socioeconómico II fue del 94,6%. Las malformaciones cardíacas fueron las más frecuentes en el 25,9%, seguido de las del sistema nervioso central y abdomen con 24,1% cada uno. Al especificar las patologías, la translucencia nucal fue la más frecuente (19,0%), seguido de la ectasia renal/dilatación pielocalicial/hidronefrosis (12,1%). Conclusión: Las malformaciomes congénitas más prevalente fueron las cardíacas. Los resultados resaltan la importancia de establecer una vigilancia epidemiológica continua que permita un diagnóstico temprano y tratamiento oportuno de las malformaciones congénitas.

Objetivo: Identificar os cuidados à mulher internada no Centro de Terapia Intensiva por miocardiopatia periparto. Metodologia: Revisão integrativa da literatura realizada em três bases de dados durante o mês de maio de 2021 utilizando estratégias de buscas que combinavam através dos operadores booleanos AND, OR os termos exatos e alternativos: Cardiopatias/Heart diseases; Insuficiência cardíaca/Heart failure; Cuidados críticos/Critical care; Gravidez/Pregnancy; Puerpério/Puerperium; Período periparto/Peripartum period. Seguindo critérios de inclusão e exclusão pré-estabelecidos o número de artigos selecionados para esta revisão foi sete. Resultados: Os cuidados apresentados nos estudos foram encontrados de forma mais abrangente, sem designar as atribuições que deveriam ser executadas por cada uma das categorias de profissionais que atua na unidade de terapia intensiva. Quanto aos tipos de cuidados encontrados, destacaram-se aqueles relacionados ao monitoramento ou estabilização das funções cardíacas, renais e respiratórias, tendo em vista que são esses os principais sistemas afetados pela patologia. Também foram citados cuidados referentes ao transplante cardíaco. Considerações finais: A miocardiopatia periparto ainda é uma patologia pouco evidenciada no âmbito científico, havendo necessidade de ampliar o desenvolvimento de estudos acercar da temática, sobretudo com diretrizes que norteiem as práticas assistenciais.

Cardiovascular diseases are aggravated by various factors, including chronic use of certain medications. There are worldwide reported cases of sudden strokes or myocardial infarction in previously non-ischemic patients while using medications for other chronic diseases, such as diabetes, gout, hypertension, and endocrine diseases. The mechanism is quite complicated, and the underlying causes were obscure. In line with this reported toxicity are patients with hyperuricemia on uric acid lowering therapy. The goal of the present investigation was to assess the cardiotoxic effect of allopurinol based on the levels of troponin that were quantified by fully automated COBAS technique in apparently healthy renal stone patients. To do so, serum samples were collected from these patients at baseline (before starting allopurinol therapy), and at the end of the course of the therapy. The troponin level was significantly elevated after allopurinol therapy compared to baseline. The study concluded that allopurinol could have a significant cardiotoxic effect in heart tissues and should be used cautiously in patients with preexisting ischemic cardiac diseases. Objective: to establish the epidemiological profile of patients with heart disease who contracted COVID-19 in the first pandemic year. Method: observational, retrospective cohort study. Patients with heart disease with positive PCR results from March to December 2020 were included. Records with incomplete data for the variables analyzed were excluded. In the bivariate analysis, the prevalence ratios (with CI 95%) were calculated as a measure of association between the dependent variable (cardiopathy) and the independent variables (age, sex, education and area of residence) using the χ² test of Mantel Haenszel. Results: an association with statistical significance was identified in relation to the variables age, sex and education. The highest risk of an unfavorable outcome was for the age group over 70 years old (59.97%), followed by the age range from 60 to 69 years old (38.31%), male sex (45.28%) and schooling to higher level. Conclusion: it was evidenced that patients with heart disease require greater care after the diagnosis of the disease, especially because this group has a higher mortality rate. - Abstract
Background: The myxomatosis degeneration is a degenerative cardiac valve disease, with a higher incidence in male and senile canids. The diagnosis is made by a doppler echocardiography exam. Although there are few reports on the occurrence of cardiopathies in wild dogs (Cerdocyon thous), some studies on their cardiological parameters can be found. Considering this, and knowing that the cardiopathies in wild canids are common post mortem findings, the objective of this study is to describe the echocardiography diagnosis of a case of myxomatous degeneration of the atrioventricular valves in 1 wild dog (Cerdocyon thous) living in captivity. Case: It was treated at the Diagnostic Imaging Department of the Veterinary Hospital of the Federal University of Mato Grosso (HOVET-UFMT), 1 wild dog (C. thous), male, living in captivity with approximately 10-year-old, directed by the Center of Medicine and Research in Wild Animals of Cuiabá, to perform echocardiography exam. The patient was submitted to anesthesia for proper examination, which was used Esaote® machine model MyLabFive VET with sector scan transducer (4.0 -7.5 MHz). The longitudinal, transverse and apical scan planes were obtained through left and right parasternal windows. The evaluation of M mode exposed ejection fraction and shortening increased, of 81% and 47%, respectively, however it showed no increase in systolic and diastolic values of left ventricle, nor in right cavities on subjective evaluation. The relation between the left atrium (LA) and the aorta (Ao) remained normal, at 1.2 mm, with dimensions of 13.4 mm from the AO and 16.3 mm from LA, compatible with species parameters or domestic canines. The atrioventricular valves showed thickening and irregularities in their cusps, with great intensity in the left atrioventricular valve (LAV). The Doppler mode analysis revealed a turbulent systolic flow into the left atrium and right atrium, constituting transvalvular LAV and right atrioventricular valve- (RAV) regurgitation, both observed through the blood flow in colored Doppler and measured through the reflux velocity of 4.02 m/s of LAV and 2.17 m/s of RAV by the continuous Doppler, showing insufficiency of intense degree of LAV and moderate degree of RAV, no evidence of pulmonary hypertension. On the other hand, the relation between wave E and wave A (E/A) was 1.0, with increased transvalvular velocities and values of 0.95 m/s for wave E and A. The isovolumetric mitral relaxation time was approximately 76 m/s. The value of the pressure derivative (dp) in relation to time (dt) dp/dt measured from the LAV reflux was 1257 mmHg, within the limit considered normal for canines. Four months after the diagnosis, the patient died due to complications of chronic renal failure. Discussion: Despite being a commonly diagnosed pathology in domestic canids, the myxomatous degeneration of atrioventricular valves is still little reported in wild canids. The evaluation of the results showed that although there was severe LAV regurgitation, there was no hypertrophy or compensatory dilation of the left cavities. However, there was a compensatory increase in the shortening fraction together with the ventricular relaxation deficit. The diagnosis of this condition in Cerdocyon thous demonstrates that the pathology can affect animals of advanced age and that its incidence needs to be determined in these captive species, in order to understand the real impact of this disease in these populations. Patients with ankylosing spondylitis (AE) may present cardiac manifestations, mainly aortic root dilation and valvular disease. Rehabilitation programs to improve cardiovascular capacity are important for maintaining the quality of life of these patients, who tend to evolve with their disease impairments and dysfunctions, when not corrected. The objective of this study was to report the clinical case of a 67-yearold male patient diagnosed with ankylosing spondylitis for twenty years and with cardiac structural evolutions. The rehabilitation program involves stretching exercises, aerobics, resistance and health education. The Borg Subjective Effort Scale was used to analyze the patient's effort during exercises and activities of daily living. However, it maintained its functional capacity in daily and instrumental life activities over the years even with the structural evolution of the disease due to the constant practice of rehabilitation. Objective: The aim of this study was to assess the association between the Sense of Coherence (SoC) and Quality of Life (QoL) in adolescents with heart disease. Methods: A cross-sectional study was carried out with 164 adolescents with congenital or valvular heart disease, aged between 10 and 18 years, treated in a referral center in the city of Recife - Brazil. The information collected contains census data, type of heart disease, economic status identified according to the Brazilian Criteria for Economic Classification (ABEP), as well as an evaluation of the SoC and the QoL. Results: The SoC (50.09) and QoL (72.23) exhibited high average scores. The SoC was positively correlated with all dimensions of the QoL scale (p<0.001). The social and school dimensions, respectively, presented the highest and lowest scores. The linear regression analysis revealed that the SoC influenced the school and emotional dimensions. Conclusions: This study demonstrates that the SoC is a protective factor in the life of adolescents. This factor helps on the improvement of perception of QoL and on successfully dealing with daily adversities and chronic stress. [ABSTRACT FROM AUTHOR]

Observe the trend in the literature on studies that associate obesity with heart disease. Integrative literature review, through 15 articles published in the Latin American and Caribbean Literature in Health Sciences databases, Regional Library of Medicine, Online Scientific Electronic Library, Nursing Database and Google Scholar, in the last 10 years. Most articles referred to obesity as a risk factor for cardiovascular disease. Most of the samples are composed of young adults with overweight and obesity. The lack of awareness about the risk factors for cardiovascular diseases was pointed out as a worrying factor in self-care with health. Looking at the current scenario of this population will promote subsidies to nurses and health professionals for the elaboration of care that cooperates to promote health and prevent heart disease. O objetivo foi avaliar o estado nutricional de pacientes cardíacos e comparar as recomendações existentes na literatura. Estudo quantitativo e transversal, realizado em pacientes cardíacos hospitalizados. A avaliação ocorreu por meio de aferição de peso e estatura e pela NRS 2002, os cálculos para necessidade energética foram descritos por Harris-Benedict e compararam as recomendações da ASPEN, ESPEN, Aquilani e Garnes. Foram avaliados 51 pacientes, seguindo a média de idade, IMC de sobrepeso e maioria sem risco nutricional. As recomendações calculadas se encontraram entre as descritas na literatura. O perfil nutricional de pacientes com cardiopatias encontra-se alterado, sendo recomendado de 25 a 30 kcal/dia devido à baixa ingestão alimentar deles.

Objetivo: identificar, avaliar e transmitir novos dados e recomendações aos profissionais de saúde. Método: trata-se de uma revisão integrativa da literatura realizada nas bases Scopus, Cinahl, Ibecs, Medline e Lilacs. Resultados: inicialmente foram encontrados 248 artigos, após exclusão dos artigos que não se enquadraram nos critérios de inclusão foram selecionados 6, conclusão: Concluímos que existe um alto índice de mortalidade em idosos por alterações cardíacas, associação elevada de comorbidades e recursos simples de diagnóstico nas fases das alterações cardíacas por Chikungunya. Torna-se necessário garantir uma maior atenção ao atendimento inicial e acompanhamento do idoso com esta arbovirose envolvendo melhores práticas na rotina da assistência clínica, ampliação do uso de exames cardiológicos de imagem e laboratoriais em unidades de saúde e de pesquisas científicas por parte dos profissionais de saúde.

Objetivo: compreender as principais complicações ocasionadas pelo vírus SARS-CoV-2 no sistema cardíaco do público idoso. Métodos: trata-se de uma revisão sistemática da literatura de abordagem qualitativa e característica descritivo-exploratória realizada no ano de 2020. Resultados: as complicações cardíacas mais prevalentes na população idosa foram miocardite, arritmias, insuficiência cardíaca, infarto agudo, choque cardiogênico, lesão miocárdica aguda e parada cardiorrespiratória. O principal biomarcador cardíaco foi a troponina, apresentando elevação superior ao percentil 99°, evidenciando a necessidade de tratamento em Unidade de Terapia Intensiva. Considerações Finais: o aparecimento desse vírus causou grande prejuízo no campo da saúde, especialmente a população idosa, que apresenta risco elevado de óbito ao contrair essa doença. Destarte, é de extrema importância se ater a mensuração da troponina sérica no público alvo e realizar monitorização longitudinal, utilizando para tal a telecardiologia, uma vez que diminuem as chances de contaminação entre infectado e profissional de saúde.

Objective: To identify nursing interventions and analyze the survival of pediatric patients who used extracorporeal membrane oxygenation in the post-op period of cardiac surgery. Methods: This is a prospective historical observational study with a quantitative approach. Data on the main nursing interventions were captured in the medical records during the immediate postoperative period. Subsequently, telephone contact was made to analyze the survival of children who used extracorporeal membrane oxygenation in the postoperative period of cardiac surgery. Results: There was a predominance of male gender 68.8%, infants 66.7%, white ethnicity 83.3%. We found 82 nursing procedures. In the patients who had hospital discharge (33.3%), it was performed for follow-up. Conclusion: The main nursing interventions for pediatric patients after cardiac surgery using extracorporeal membrane oxygenation indicated in this study were: multiparametric monitoring, care for prevention and / or control of bleeding, change of decubitus position, bandage change and observation of the patient's clinical change. Elderly dogs are frequently affected by diseases in the cardiovascular and respiratory system, being often asymptomatic until they are indicated for a surgical procedure that requires preoperative evaluation, at which point many clinical findings can be detected, to the point of need for treatment prior to surgical intervention. Therefore, the work aims to detect the main cardiorespiratory manifestations and their correlation to data found in complementary exams and demographic information of 138 animals treated at a service specialized in cardiology and respiratory diseases, from 2017 to 2021, for risk purposes surgical. It was possible to observe a greater predominance of elderly, small and female animals. Among heart diseases, chronic degenerative mitral disease was the most prevalent, followed by heartworm disease. Regarding respiratory affections, chronic bronchitis and tracheal collapse were more commonly diagnosed, followed by bronchopneumonia and brachycephalic syndrome. When observing the cardiac rhythm through the electrocardiogram, sinus arrhythmia, sinus rhythm and sinus arrest were the main findings. As for laboratory findings, it was possible to observe an increase in mean hematimetry, a reduction in mean hematocrit and platelets. However, some animals showed azotemia, leukocytosis and eosinophilia. It was observed that patients with cardiac diseases in stages of congestion have reduced blood pressure, increased hematimetry and tachyarrhythmias, such as sinus tachycardia and/or premature atrial complexes. Older patients tend to have reduced global white blood cell values and more likely to manifest increased blood pressure. Heartworm patients have an increase in eosinophils. [ABSTRACT FROM AUTHOR]
