Objectives: Describe the characteristics of the different cardiology medical residencies in Latin America. Method: Cross-sectional study that aims to evaluate the characteristics of cardiology residencies in Spanish-speaking countries of Latin America, through self-administered electronic surveys. Results: Three hundred seven residents of 147 residences were surveyed. Mean age was 31 years and 63% were male. Ninety eight percent carry out their training in the capital city. The average total training time is 4.8 years. Forty four percent complete their residency in internal medicine prior to starting cardiology, and 10% have no prior training. In cardiology training is 3 years in most countries. Fifty four percent present academic activities every day and 16% only once or less, consisting of theoretical classes (93%), clinical cases (85%), bibliographic workshops (69%), and writing scientific papers (68%). Supervision is carried out by the chief resident (45%), resident coordinator (44%), resident instructor (27%) or the department head (54%), while 2.6% do not present supervision. The main rotations were echocardiography (99%), hemodynamics (96%), coronary unit (93%), and electrophysiology (92%). Residents highlighted the need to improve academic activities (23%) and scientific production (12%). La simulación clínica busca garantizar el desarrollo de competencias clínicas, orientada hacia una correcta calidad de los cuidados y la seguridad de los pacientes. El objetivo de este estudio, en ese sentido, fue identificar la eficiencia de la simulación clínica en la enseñanza de la cardiología. Para ello, se realizó una revisión sistemática mediante la utilización del protocolo PRISMA. La información se recopiló en el primer cuatrimestre del año 2024, mediante los buscadores en PubMed, Scopus y Web of Science, por título, considerando los documentos que desde 2019 a 2024 mostraran alguna asociación entre los términos MeSH utilizados: “simulation training”, “education” y “cardiology”. Se localizaron 2117 artículos; tras el cribado se contó con 117; de la evaluación para elegibilidad quedaron 70, y finalmente se incluyeron 19 artículos útiles para esta revisión. La simulación clínica en la enseñanza de cardiología, se concluye, está soportada pedagógicamente en un modelo universitario formativo en el que se estimula el aprendizaje significativo para alcanzar las competencias establecidas en los currículos de los planes de estudio de la carrera de medicina.

Introduction: the constant evolution of cardiovascular medicine has provided significant advances in the treatment of various cardiac conditions, highlighting cardiac catheter ablation as a highly relevant intervention. Over the years, this technique has become an effective alternative for controlling cardiac arrhythmias, offering patients the prospect of an improved quality of life. Objective: this literature review provides updated information on nursing care in cardiac catheter ablation, benefiting healthcare professionals involved in the care of these patients. Methodology: the review was based on the analysis of articles present in the PubMed and Scielo databases. Conclusion: Studies emphasize the crucial role of team assistance in promoting safe recovery by addressing pain, monitoring vital signs, educating patients, and considering the emotional impact of the procedure. The conclusion highlights that, although catheter ablation is a valuable option in the management of arrhythmias, its effectiveness depends on adequate and safe performance, with the implementation of rigorous postoperative care and effective management of complications being essential. Heart diseases are potentially fatal pathologies considered a risk factor for anxiety disorders. This paper aims to present a systematic literature review on anxiety in cardiology. Five databases were selected: Scielo, PsycINFO, Web of Science, and PubMed. The descriptors "cardiologia" and "ansiedade" and their equivalents in English, "anxiety" and "cardiology" were used. The inclusion criteria were papers published in Portuguese, English, or Spanish between 2017 and 2021. Twenty-six studies were selected, and four topics stood out: socio-demographic profile, anxiety and cardiac rehabilitation, anxiety-related risk and protective factors, and anxiety and depression. The results indicate that healthy anxiety levels play a protective role in promoting patient self-care. However, maladaptive anxiety levels hinder patients from coping with the disease. Background: The Ignacio Chávez National Institute of Cardiology (NIC) has positioned itself as an international benchmark in cardiovascular medical education, standing out for its high academic standards and human quality. Objective: This study aims to identify the employment and academic profile of cardiology fellowship graduates from the National Institute of Cardiology of Mexico (NIC). Methods: A cross-sectional and retrospective study was conducted, involving 473 graduates from the period 2000-2015. Data was collected through an electronic survey. Results: A total of 221 graduates agreed to participate, accounting for 46.7% of the total number of graduates. Among the participants, 85.2% were male. The majority of graduates (82.4%) held Mexican nationality, while the remaining individuals represented various Latin-American nationalities. Following the completion of the cardiology fellowship, 89% of graduates pursued further postgraduate degrees. Within this group, 95.4% completed at least one additional fellowship, while 4.6% pursued a master's or PhD program. The most popular fellowship program chosen was Interventional Cardiology. On average, NIC graduates dedicate 10.8 hours per day to their work, with 6.4 hours dedicated to cardiology and 4.3 hours to their other postgraduate degree. Those employed in both public and private health systems allocate 6.1 hours and 5.2 hours per day, respectively. Additionally, 75% of graduates are involved in teaching activities. Currently, 84% of graduates are employed in Mexico. Conclusion: The majority of NIC graduates are physicians who further their academic training by pursuing additional postgraduate programs. They work longer hours per day compared to the general Mexican population, with a significant focus on clinical cardiology. 235 p., Debido al elevado coste de los nuevos, la reutilización de dispositivos cardíacos usados puede ser una opción factible y segura de dotar de un tratamiento vital a muchos pacientes en necesidad, especialmente cuando la alternativa sería no tener ningún dispositivo. La presente tesis doctoral tuvo como objetivo identificar las perspectivas sobre la potencial donación de dispositivos para su reutilización en países de ingresos medios y bajos, además de estudiar la funcionalidad de dispositivos explantados post mortem del entorno. Las perspectivas generales previamente descritas sobre la donación de dispositivos cardíacos reutilizables desde países de ingresos altos a países de ingresos medios y bajos son mayoritariamente positivas en todos los grupos estudiados. Respecto a las perspectivas a nivel local, la mayoría de facultativos especialistas en terapias de electroestimulación cardíaca de la Asociación del Ritmo tienen perspectivas positivas hacia la reutilización de dispositivos cardíacos implantables en países de ingresos medios y bajos. Asimismo, la mayoría de pacientes portadores de dispositivos cardíacos implantables tienen perspectivas positivas hacia la donación de sus dispositivos a países de ingresos medios y bajos. Aplicando un protocolo de manejo estandarizado y mediante una recogida semestral, uno de cada dos dispositivos cardíacos explantados post mortem en funerarias o crematorios disponen de batería suficiente, integridad y funcionamiento adecuados como para poder reutilizarse.

Resumo Fundamento Apesar de progresso significativo na melhoria da qualidade do tratamento de doenças cardiovasculares, lacunas persistem em termos de falha na adesão às recomendações de diretrizes. Objetivo Este estudo avalia os efeitos da implementação de um programa de melhoria da qualidade adaptado do Programa Get with the guidelines® da American Heart Association sobre a adesão às diretrizes para síndrome coronária aguda (SCA), fibrilação atrial (FA) e insuficiência cardíaca (IC). Métodos Avaliamos dados demográficos, medidas de qualidade, e desfechos em curto prazo em pacientes com SCA, FA, e IC incluídos no programa Boas Práticas em Cardiologia (BPC) entre 2016 e 2022. Resultados Este estudo incluiu 12167 pacientes em 19 hospitais no Brasil. A idade média foi 62,5 [53,8-71] anos, 61,1% eram do sexo masculino, 68,7% apresentaram hipertensão, 32% diabetes mellitus, e 24,1% dislipidemia. Introducción: Transthyretin-mediated hereditary amyloidosis is an autosomal dominant disease with progressive systemic involvement that affects the function of multiple organs and is associated with high morbidity and mortality. Patients present neurological, cardiac, autonomic, ophthalmological and renal involvement, among others, related to the patient’s genotype. These alterations affect quality of life and are directly related to early mortality. Peripheral neuropathy is an early marker of the disease. Its identification allows early treatment with disease-modifying therapies aimed at stabilizing disease progression, which, along with adequate follow-up, has a positive impact on patients’ quality of life and prolongs life expectancy. Objective: To structure a series of recommendations appropriate to the local setting on treatment and follow-up in patients with hereditary transthyretin-mediated amyloidosis. Materials and methods: Delphi methodology was used, and a cut-off point of 75% favorability was established. Results and conclusions: A series of recommendations were presented based on the available evidence and international recommendations, applicable to the local setting, on diagnostic methods that facilitate early detection of the affected patient and the parameters for initiation of treatment and follow-up that improve the quality of life, decrease complications, and increase the life expectancy of these patients. This review summarizes the impact of gender affirming hormone therapy used in the transgendered population and the classic and emerging risk factors on cardiovascular outcomes and surrogate markers of cardiovascular health. There is a growing body of evidence that people who are transgender and gender diverse are impacted by disparities across a variety of cardiovascular risk factors compared with their peers who are cisgender. Previously, disparities have been reported in cardiovascular morbidity and mortality across this group as a result of a higher prevalence of non-healthy life style. However, recent research suggests that there are additional factors playing a role in this differences: there is the hypothesis that the excess of cardiovascular morbility and mortality has been driven by psychosocial stressors across the lifespan at multiple levels, as structural violence (e.g., discrimination, lack of affordable housing, lack of access to health care, etc.) Lack of information and research in this population is an important limitation; therefore, a multifaceted approach that integrates best practice into research, health promotion and cardiovascular care for this understudied and growing population is clearly needed. The need to improve access to health services for the transgender community has become evident, especially concerning cardiovascular risk, which is higher compared to the general population. Surgical procedures and hormone therapies are common in this population to affirm gender identity, but they pose challenges as they are associated with disruptions in lipid metabolism, body fat concentration, and insulin resistance. Additionally, there is an increased risk of adverse cardiovascular events such as venous thromboembolism, stroke, and myocardial infarction. The influence of sex hormones on the electrophysiological properties of the heart has been studied, highlighting gender differences that may predispose the transgender population to cardiac arrhythmias. Exogenous hormone therapy, for both transgender women and men, can affect the QT interval and increase the risk of arrhythmias, including atrial fibrillation. The Gender-Diverse (TGD) population in Mexico faces significant health challenges, such as limited access to healthcare and a prevalence of adverse conditions. Cardiology is crucial for this population due to a high prevalence of risk factors and cardiovascular diseases. Despite a lack of precise data, it is estimated that 0.5-1.5% of the population identifies as TGD. They encounter sociocultural challenges, including discrimination and stigma, contributing to health issues and a lack of treatment access. Cardiovascular risk factors, hormone self-administration, and barriers to healthcare access are prominent concerns. Opportunities for improvement involve strengthening medical education, implementing inclusive policies, and promoting research and data collection. The development of specific clinical guidelines and inclusive health programs is suggested. The theory of minority stress emphasizes addressing chronic psychosocial stressors and recognizing the influence of stress factors on health. Integrating healthcare services for all populations in cardiology and fostering resilience are key strategies. Objetivo: Determinar la prevalencia de cardiopatías congénitas (CC) en un hospital de segundo nivel que ameritaron tratamiento quirúrgico. Material y métodos: Estudio descriptivo, realizado en pacientes con diagnóstico de CC en una unidad de segundo nivel de atención del Instituto Mexicano del Seguro Social (IMSS) en Puebla, México durante el periodo de 2016-2017, se incluyeron expedientes de recién nacidos (RN) a término hasta los 14 años, analizando variables sociodemográficas, tipo de CC y corto circuito, presencia de anomalías asociadas y envío a un tercer nivel de atención para tratamiento quirúrgico. La información fue recolectada y analizada mediante el programa SPSS Statistics v25. Resultados: La prevalencia hospitalaria de CC que requirieron envío a tercer nivel de atención para tratamiento quirúrgico fue 6.8% en 2016 y 6.6% en 2017, la mediana de edad fue 1 año. El 77.2% de las CC fueron acianógenas, la persistencia del conducto arterioso (PCA) fue el corto circuito más frecuente y 19.2% presentó síndrome de Down como anomalía congénita asociada. Conclusión: Obtuvimos una prevalencia similar a otras regiones de México, siendo las CC acianógenas la causa más frecuente de los tratamientos quirúrgicos. La detección y referencia oportuna mejorara la atención y calidad de vida en estos pacientes.

Resumo Fundamento A Sociedade Brasileira de Cardiologia promove anualmente a prova para obtenção do título de especialista em Cardiologia, sendo a Comissão Julgadora do Título de Especialista em Cardiologia responsável pela sua organização e aplicação. A análise psicométrica dos itens de uma prova, por meio da Teoria de Resposta ao Item (TRI) pode fornecer informações robustas e contribuir para o aprimoramento contínuo dessa avaliação. Objetivos Avaliar as propriedades psicométricas da prova do Título de Especialista em Cardiologia no ano de 2019, em relação aos parâmetros da TRI. Métodos Estudo observacional, com a análise psicométrica das 120 questões da prova realizada por 1120 (mil cento e vinte) candidatos para a obtenção do título de especialista em Cardiologia, no ano de 2019. Resultados A análise da prova pela TRI mostrou 32,2% dos itens com poder de discriminação “alto” ou “muito alto”, 49,2% dos itens categorizados como “fácil” ou “muito fácil” e 41,5% apresentavam alta probabilidade de acerto ao acaso . Foram identificados 69 itens com problemas em relação aos parâmetros da TRI e, portanto, com baixo poder de avaliar a proficiência do candidato. Conclusões A análise psicométrica da prova de título de Especialista em Cardiologia pela TRI demonstrou um alto percentual de questões fáceis, com cerca de dois terços dos itens com alta probabilidade de acerto ao acaso. Esses dados poderão desencadear uma série de discussões e propostas para a construção das futuras provas em cardiologia.

Resumo Fundamento Diante de dados demográficos referentes às áreas de atuação dos cardiologistas no Brasil, a coordenação do Programa de Residência Médica em Cardiologia do Instituto do Coração (PRM INCOR) entendeu a necessidade de uma atualização de seu conteúdo programático, a fim de adaptar o processo de formação à realidade profissional do cardiologista. Objetivo O presente artigo tem como objetivo descrever à comunidade científica as atualizações recentemente implementadas no PRM INCOR. Métodos No artigo, descrevemos as atualizações recentes do PRM INCOR, comparando a grade teórica pregressa e a atual. Expomos também o racional por trás de tais mudanças com dados de literatura relacionados à atuação do médico cardiologista no mercado de trabalho. Resultado Houve uma redução da carga horária destinada a estágios de terapia intensiva, e um incremento nas atividades ambulatoriais relacionadas a medidas de prevenção primária e secundária. Além disso, o programa passou por uma reformulação de seu conteúdo didático, organizado agora por núcleos de competência. Conclusão A atualização da grade curricular decorre da necessidade de adequar o PRM INCOR à realidade atual do mercado de trabalho brasileiro. O grupo envolvido na atualização está ciente que se trata de um processo dinâmico e que pode exigir modificações no decorrer do tempo.

Introducción: Las enfermedades cardiovasculares son la principal causa de muerte en el mundo, y las enfermedades oncológicas están aumentando en prevalencia. Los medicamentos oncológicos pueden tener efectos secundarios cardiovasculares, y la cardiooncología es una subespecialidad de la cardiología que se ocupa de la prevención y el tratamiento de las complicaciones cardiovasculares relacionadas con el cáncer. Puntos importantes del editorial: Las antraciclinas son un grupo de medicamentos oncológicos que pueden causar cardiotoxicidad, lo que puede causar una variedad de síntomas, incluyendo fatiga, disnea, edema y dolor en torácico. En casos graves, la cardiotoxicidad puede provocar insuficiencia cardíaca. Otros medicamentos oncológicos que pueden causar cardiotoxicidad incluyen los anticuerpos anti-HER2, el fluorouracilo y la gemcitabina. Los inhibidores de la tirosinkinasa y los inhibidores del factor de crecimiento derivado del endotelio también pueden causar problemas cardiovasculares, como hipertensión, formación de coágulos sanguíneos y arritmia. Los pacientes con cáncer también tienen un mayor riesgo de desarrollar complicaciones tromboembólicas venosas en miembros inferiores. Conclusión: El diagnóstico temprano y el tratamiento de las complicaciones cardiovasculares relacionadas con el cáncer son esenciales para mejorar la supervivencia de los pacientes con cáncer. La cardiooncología es una especialidad emergente y multidisciplinaria que requiere la participación de oncólogos, hematólogos, radiooncólogos y cardiólogos. El equipo de cardiooncología trabaja en conjunto para evaluar el riesgo cardiovascular de los pacientes con cáncer, prevenir las complicaciones cardiovasculares y tratar las complicaciones cardiovasculares que ocurren.

O artigo examina a constituição da cardiologia como especialidade médica no Brasil nas décadas de 1930 e 1940. No contexto de implementação do sistema de proteção social pelo governo de Getúlio Vargas, as doenças cardiovasculares ganharam visibilidade no debate sobre a saúde e a produtividade dos trabalhadores urbanos. Alguns médicos defenderam que o diagnóstico, o tratamento e a prevenção dessas enfermidades requeriam conhecimentos e práticas especializados, como a eletrocardiografia. A disseminação dessa tecnologia no Brasil se beneficiou dos intercâmbios com médicos estadunidenses, intensificados na época pela Política da Boa Vizinhança. Argumentamos que o estabelecimento da cardiologia como especialidade resultou da associação entre demandas para a reprodução da força de trabalho, interesses socioprofissionais no campo médico e redes transnacionais de cooperação científica interamericana.

The relationship between cancer diagnosis and cardiovascular diseases is complex, with newly diagnosed patients facing a higher risk of coronary disease, heart failure, and atrial fibrillation. Compared to the general population, they have two to six times more risk of dying from cardiovascular causes. Cardiovascular complications arising from chemotherapy and radiotherapy, along with social and healthcare access disparities, complicate the collection of accurate data on the incidence of cancer and cardiotoxicity in marginalized populations. Among the LGBTQ community, certain types of cancer are more prevalent, and hormone administration for gender affirmation is also under study. The delay in cancer screening in the transgender population results in late detections and deaths from cancer. Research on cancer in the transgender population and cardiotoxicity is limited, but special attention is needed to develop detection and prevention strategies in specific situations, such as hormone-dependent tumors. Introducción: es bien conocida la utilidad del strain global longitudinal (SGL) del ventrículo izquierdo (VI) en pacientes que reciben antineoplásicos. Estudios recientes sugieren una conducta similar del strain longitudinal de la pared libre (SLPL) del ventrículo derecho (VD), pero en la literatura aún existen muchas controversias que limitan su valoración, por lo que este estudio es un aporte a esta necesidad de practicidad. Objetivo: evaluar el comportamiento del SLPL del VD en los pacientes tratados con quimioterapia y/o radioterapia, y su correlación con el SGL del VI. Metodología: se aplicó un protocolo de recolección de datos que incluía variables demográficas y antecedentes de pacientes con diagnóstico de cáncer, tratados con quimioterapia y/o radioterapia, que acudieron a realizarse un ecocardiograma con strain, desde noviembre 2020 a marzo 2021. El ecocardiograma se hizo acorde con los estándares de las guías de la Sociedad Americana de Ecocardiografía, que incluyera SGL del VI y el SLPL del VD. Los datos fueron analizados en programa SPSS. Resultados: de 90 pacientes, 6 excluidos para un total de 84 recopilados. 89 % mujeres, edad promedio 59 ±11 años, 73 % cáncer de mama. 54 % hipertensos, 27 % obesos, 23 % con dislipidemia, 19 % diabéticos y 11 % eran o fueron fumadores. 46 % había recibido quimioterapia y radioterapia concomitantes y el 48 % había recibido tratamiento hace más de un año. En hallazgos ecocardiográficos: fracción de eyección del VI (FE) promedio 64±7; sin embargo, 82 % tenía SGL del VI reducido (44 % menor a -15 %). De manera similar, el 74 % tenía reducción del SLPL del VD, pero solo el 15 % de la onda S del Doppler tisular del VD estaba anormal, 7 % del TAPSE estaba reducido y 6 % el cambio del área fraccional (CAF). Introduction: cardiology is one of the medical specialties with the most systematic reviews and meta-analyses. Studying the methodology of the reviews and analyzing their statistical heterogeneity is essential to guarantee their scientific validity. Objective: to describe the comparison of association measures, statistical models and degree of heterogeneity in meta-analyses of systematic reviews of pharmacological interventions in cardiology, published between 2000-2005 and 2011-2016. Methodology: analytical study based on the description and comparison of statistical methods of systematic reviews of pharmacological interventions in cardiology, published in the Cochrane library. For the qualitative variables, absolute and relative frequencies were estimated, while for the quantitative ones, means and standard deviations, or medians and interquartile ranges, were determined, depending on their distribution. The Student's t test was used to establish the difference in means and the Chi square for the difference in proportions. Results: 54 systematic reviews were included, with a total of 1.053 meta-analyses, 6 reviews with 240 meta-analyses between 2000-2005, and 48 reviews with 813 metaanalyses between 2011-2016. Most meta-analyses used standard treatment as the comparison group (56.6%), measured nominal qualitative outcomes (86.3%), determined relative risks (63.3%), and applied fixed-effect models (57.8%). In the 2011-2016 period, an average of the Higgins Index was found to be 17.5 lower than in the 2000-2005 (p<0.05). Conclusion: there was evidence of a decrease in statistical heterogeneity and an increase in the implementation of random effects models, which accounts for greater rigor when it comes to demonstrating truly significant results. Background: Infectious endocarditis (IE) is an infection of the cardiac endothelium that can usually be caused by bacteria and fungi. The superficial growth of these microorganisms (MOs) can cause embolism in various organs, such as kidneys, lungs, skin, brain and central nervous system, leading to death if not treated properly. IE caused by HACEK bacteria (Haemophilus spp., Aggregatibacter actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens and Kingella kingae) mainly affects patients with previous heart disease or artificial valves, and is characterized by an insidious course, with an average delay in diagnosis of one month (when caused by Haemophilus spp.) to 3 months (when caused by Aggregatibacter or Cardiobacterium spp.). Aim: To report a case of IE caused by MOs from the HACEK group. Method: This is the report of a patient treated at the Hospital Universitário São Francisco na Providência de Deus - HUSF, located in the city of Bragança Paulista - SP, Brazil. Case Report: This report describes the case of a 50-year-old male patient with a history of persistent fever, sweating, chills, fatigue and weight loss. After clinical evaluation, the patient was diagnosed with IE, based on clinical, laboratory and imaging findings, including microcytic anemia, leukocytosis with left shift, elevated PCR and VHS, presence of diastolic murmur, in addition to evidence of cardiac valve involvement in the transthoracic echocardiogram. Blood cultures identified the bacteria A. aphrophilus as the cause of IE, and treatment with ceftriaxone was initiated. Due to significant valve dysfunction, the patient underwent cardiac surgery with replacement of the tricuspid valve with a mitral prosthesis, followed by outpatient/cardiological monitoring and dental evaluation. Conclusion: HACEK MOs can cause IE in patients of different age groups, including children, and their ability to affect both native and prosthetic valves highlights the importance of a broad clinical approach and a high index of suspicion for accurate diagnosis. Furthermore, maintaining oral health is an important factor in preventing these infections, and the use of imaging tests, such as CT scans and echocardiograms, and blood cultures play a fundamental role in identification and appropriate treatment of patients with IE caused by HACEK. Therefore, understanding these aspects is essential to improve the diagnosis and clinical management of these infections, which can lead to substantial morbidity and mortality if not detected and treated in a timely manner. El Coronavirus 2019 (COVID-19) generó una alta mortalidad, que describe complicaciones cardiovasculares, inflamatorias y trombóticas expresadas y diagnosticadas mediante la elevación de biomarcadores cardíacos, como la porción N-Terminal del Pro-BNP (NT-ProBNP), siendo asociado elevación con mortalidad. Se diseñó una investigación no experimental de cohorte retrospectivo que buscó determinar la utilidad pronóstica del NT-proBNP en la mortalidad en los pacientes por COVID-19 hospitalizados de enero a octubre de 2021 en el Centro Médico Docente La Trinidad, para lo cual se revisaron las historias clínicas de los pacientes con COVID-19 con niveles de NT ProBNP, biomarcadores cardiacos y se hizo seguimiento por 60 días desde el inicio de sus síntomas. Se revisaron las historias clínicas de 46 pacientes entre 42 y 93 años con un promedio de 69 años, 24 hombres; se aplicó test de Chi cuadrado (X2 0,7466 y p= 0,3875) y cálculo de Riesgo Relativo de 1.615 y una Reducción del Riesgo Relativo de 61,5%, demostrando que aquellos pacientes con este marcador elevado casi duplicaban su riesgo de mortalidad respecto a quienes no tuvieron elevación con NT-ProBNP, sólo se demostró asociación estadísticamente significativa entre la mortalidad y la elevación del NT-proBNP en los pacientes con patología endocrinológica (p=0,021) y elevación de la troponina I ultrasensible (OR 39 IC 95% 2,14-713); no se demostró asociación con el resto de variables clínicas. La muestra no permite ningún modelo matemático predictivo y por tanto no se pudo concluir su valor predictivo de manera estadística por lo que se recomienda realizar un estudio prospectivo que controle las variables estudiadas.

Introduction and objectives: The coronavirus disease 2019 (COVID-19) spread quickly around the world. Although mainly a respiratory illness, there is growing interest in non-respiratory manifestations, particularly cardiovascular ones. At our center, mobilization of cardiologists with intensive care training was needed. Our aim is to describe patients with severe COVID-19 admitted to a Portuguese intensive care unit (ICU), the cardiovascular impact of the disease and the experience of cardiologists working in a COVID-19 ICU. Methods: Data from adult patients with COVID-19 admitted to the ICU of Centro Hospitalar de Vila Nova de Gaia/Espinho between 16 March 2020 and 21 April 2020 were analyzed retrospectively. Results: Thirty-five patients were admitted. Mean age was 62.6±6.0 years and 23 (65.7%) were male. Dyslipidemia was the most common cardiovascular risk factor (65.7%, n=23), followed by hypertension (57.1%, n=20). Mean ICU stay time was 15.9±10.0 days. Patients had high rates of mechanical ventilation (88.6%, n=31) and vasopressor support (88.6%, n=31). Low rates of new onset left systolic dysfunction were detected (8.5%, n=2). One patient required venoarterial extra-corporeal membrane oxygenation. Mortality was 25% (n=9). Background: Patients with coronary heart disease and their families often face numerous changes in their lives. Recommendations on sexual activity (SA) should be included in the management of these patients. The objective of this study was to evaluate the degree of knowledge and professional attitude regarding the patient's SA. Objective: A descriptive and analytical cross-sectional study was carried out. The data were obtained from a structured, virtual and anonymous survey that was carried out among cardiologists. Results: Three hundred forty-five surveys were analyzed. In total, 63.8% considered sexual dysfunction as a cardiovascular risk marker. Likewise, 68.1% considered it relevant or very relevant to ask patients about SA. Regarding counseling, it is believed that it should be initiated primarily by the cardiologist. A more active attitude regarding the re-initiation of SA after a cardiovascular event, a greater interest in SA training, more testosterone orders and more indications of phosphodiesterase inhibitors were observed in professionals older than 60 years and male compared to younger or female physicians, respectively. Conclusions: This study showed that the degree of knowledge of the patients about the aspects related to SA was poor. 49. Evolución clínica de los pacientes sometidos a implante de válvula aórtica transcatéter (TAVI)

- Abstract
La estenosis aórtica es la enfermedad valvular cardíaca adquirida más frecuente en la población de edad avanzada. Su prevalencia en mayores de 80 años es superior 4%, y está en aumento progresivo debido al envejecimiento de la población1,2. En los pacientes con estenosis aórtica grave sintomática, el remplazo valvular aórtico quirúrgico ha sido el tratamiento de elección ya que mejora la supervivencia y la calidad de vida respecto al tratamiento médico. Sin embargo, en estos pacientes por la presencia de comorbilidades asociadas a la edad, se desestima el tratamiento quirúrgico por el alto riesgo de intervención que supera los beneficios, con unos pobres resultados del tratamiento médico (un riesgo de mortalidad por cualquier causa a los 5 años de casi el 93%)2,3. En este contexto, el desarrollo de la técnica de implante de una válvula aórtica transcatéter (TAVI), inicialmente descrita por Andersen e introducida para pacientes no operables por Cribier en el año 20024,5, ha supuesto una opción terapéutica actualmente ya aceptada para el tratamiento de pacientes ancianos con alto riesgo quirúrgico6,7,8, debido a la evidencia clínica proveniente de ensayos aleatorizados: mejora su pronóstico y calidad de vida comparado con el tratamiento conservador y con la sustitución valvular quirúrgica9,10. Por tanto, dada la menor esperanza de vida y elevada carga de comorbilidades de esta población, la TAVI representa una opción terapéutica a valorar. Sin embargo, es una técnica que presenta un coste económico alto, por lo que se hace necesario valorar con datos del mundo real el impacto clínico de este procedimiento11. En resumen, en este estudio se pretende valorar la eficacia clínica del implante de TAVI en nuestros pacientes, en el contexto de la vida real de la práctica clínica diaria (fuera de los estudios clínicos, sin límites en la inclusión de pacientes), y obtener datos de su supervivencia tras el implante., Grado en Medicina
