Abstract

Somatic mutations in the Notch gene cause uncommitted cells to express the Notch pathway, which controls cell determination during development, and the cells stuck in the proliferative undifferentiated state cause neoplasia. The normal differentiation of cells is also affected if the pathway is expressed in nonterminally differentiated cells or if it is suppressed in cells which normally express the pathway. Studies conducted on human epithelium, as cervical tissue, show that in situ and invasive squamous cell carcinomas and adenocarcinomas show activity of the Notch gene which the normal epithelial cells do not.

Abstract

Psammoma bodies are tiny layered concretions found in some pathological conditions. They were first described in cancer of the uterine endometrium (lining of the uterus) in 1961. Since that time, this special form of endometrial cancer has become recognized as a distinct disease, now called uterine papillary serious carcinoma (UPSC). The distinction is significant, since UPSC shows a great propensity for spread to the upper abdomen. Patients with UPSC generally have a poor prognosis. The treatment of the disease is primarily surgical, but the propensity for disease spread indicates that adjuvant treatment is necessary. The previous experience of the authors has indicated that adjuvant chemotherapy is not particularly effective in cases of UPSC. They now report their experience using adjuvant radiotherapy, given in the form of whole-abdominal radiation therapy (WART). Nine UPSC patients underwent WART after their recovery from surgery. The total dose of radiation given averaged 4,500 cGy. (A Gy, or Gray, is a dose of radiation equivalent to one joule of energy absorbed per kilogram of tissue. It is equal to 100 rads.) Three of the nine patients also received an additional boost of radiation to the vaginal apex. After an average follow-up time of just over two years, six of the nine patients relapsed, and four of these relapsed patients have died. It is worth noting that two of the three patients who remain disease-free received additional radiation to the vaginal apex. Furthermore, in three of the six patients who relapsed, recurrent cancer was found in the vaginal apex. These results suggest that although WART is apparently of little value for routine use, irradiation of the vaginal apex may prove useful. Abstract

Although growing numbers of tubal carcinomas in carriers of BRCA1and BRCA2germline mutations have been reported, very little is known about the nature and frequency of their possible precursor lesions. The aim of this study is to investigate the occurrence of atypical proliferative tubal lesions in grossly normal fallopian tubes from 26 women with BRCA1and BRCA2germline mutations who underwent prophylactic salpingo-ooophorectomy and whose ovaries were histologically negative for carcinoma. Fallopian tubes from 49 women who had undergone hysterectomy with salpingo-oophorectomy for uterine leiomyoma served as controls. In the 22 BRCA1-mutated women, there were two in situcarcinomas and two atypical hyperplasias of the tubal epithelium. The tubes of the BRCA2-mutated women and of the 49 control women did not show any atypical proliferation. The frequency of proliferative lesions of the tubal epithelium, including in situcarcinoma, appears to be increased in BRCA1mutation carriers. Removal and thorough examination of the fallopian tubes at the time of surgical prophylaxis for ovarian cancer is therefore recommended.
