6. Moderators of Theory-Based Interventions to Promote Physical Activity in 77 Randomized Controlled Trials

Abstract

A meta-analysis of randomized controlled trials (RCTs) has recently showed that theory-based interventions designed to promote physical activity (PA) significantly increased PA behavior. The objective of the present study was to investigate the moderators of the efficacy of these theory-based interventions. Seventy-seven RCTs evaluating theory-based interventions were systematically identified. Sample, intervention, methodology, and theory implementation characteristics were extracted, coded by three duos of independent investigators, and tested as moderators of interventions effect in a multiple-meta-regression model. Three moderators were negatively associated with the efficacy of theory-based interventions on PA behavior: intervention length (=14 weeks; ß = -0.22, p = 0.004), number of experimental patients (ß = -0.10, p = 0.002), and global methodological quality score (ß = -0.08, p = 0.04). Our findings suggest that the efficacy of theory-based interventions to promote PA could be overestimated consequently due to methodological weaknesses of RCTs and that interventions shorter than 14 weeks could maximize the increase of PA behavior.

Abstract

International audience; Background: A sedentary lifestyle is associated with higher mortality and the development of chronic pathologies. In children and adolescents, sedentary behavior for more than 2 h/day is associated with unfavorable body composition and physical condition, and lower levels of self-esteem, sociability and school level. However, more than 50% of children (6-10 years) spend on average more than 3 h/day in front of a screen, and this time continues to increase (+20 min from 2007 to 2015) [1]. In this context, designing and testing effective interventions to decrease sedentary behavior in children is a major public health research gap. Objective: The CIPRES intervention is co-constructed with key local actors, multilevel with a socio-ecological approach [2], and theory-based on the transcontextual model [3]. The CIPRES intervention aims to reduce sedentary time in school-age children (7-10 years). A secondary aim is to evaluate the effect of the CIPRES intervention on sedentary according to social deprivation level. Methods: The intervention will be evaluated by a cluster-randomized and controlled study. The target population will be made up of 1000 children in CE2-CM1 from primary schools in 4 towns in Toulon area with different levels of social deprivation. The study involves 2 phases: (1) Co-construction of an intervention for sedentary prevention adapted to the local context by using a participatory approach including representatives of each "level" of actor (i.e., children, teachers, parents, associative and municipal staff, educational advisers). (2) Implementation of the intervention in the local context [4]: the intervention lasts 6 weeks and aims to reduce sedentary behavior at different times in the child's life: school, after-school and family times. The effects of the intervention will be assessed before (T0), after the intervention (T1) and 6 months after the end of the intervention (T2). Main outcomes include sedentary behavior, level of physical activity, sleep, and variables of the transcontextual model (i.e., motivation, attitudes, norms, perceived control, intentions), and will be measured by accelerometry, diary or questionnaires. Results/discussion: A pilot intervention was co-constructed and a feasibility study is currently underway. This feasibility study involves around 90 children from primary schools. The first results on the feasibility of the implementation and the pre- vs post-intervention effects on children's sedentary time and variables of the transcontextual model will be available by summer 2021. Conclusion: The CIPRES study will assess a multilevel intervention aiming at reducing sedentary behavior in school-aged children. The intervention is based on the socio-ecological perspective and relies on a participatory design, in line with recent concerns regarding complex public health programs, social inequalities and the need to combine both agentic and structural strategies in such interventions.

Abstract

Although the iron-heart disease hypothesis is prevalent, the epidemiological findings are incongruent. The relationship of serum ferritin with early cardiovascular disease (CVD), particularly atheroscterosis, has not been evaluated extensively, particularly with accounting for inflammation. We examined this association in a case-control study of 124 age- and sex-matched pairs embedded in the population-based random sample (MONICA survey) in Southwest France, taking into account inflammation status. Cases had [greater than or equal to] 2 carotid atherosclerotic plaques and controls had none. Inflammation was assessed using several markers, including serum [alpha]-1 acid glycoprotein (AGP) and high sensitivity C-reactive protein. There was an interaction of inflammation with group (case/control) for serum ferritin. In adults without elevated AGP, serum ferritin was significantly greater in atherosclerotic cases than in adults in the control group. In models adjusted for CVD risk factors, the odds of atherosclerosis increased with the increase in serum ferritin in individuals without elevated AGP; for every 10-[micro]g/L increase in serum ferritin, the risk for atherosclerosis increased by 3% (odds ratio [95% CI]: 1.03 [1.01-1.06]). In conclusion, carotid atherosclerosis was positively associated with serum ferritin in individuals free from subclinical inflammation based on AGP. Abstract

Objective. The present meta-analysis aimed to determine the overall effect of cognitive behavior therapy combined with physical exercise (CBTEx) interventions on depression, anxiety, fatigue, and pain in adults with chronic illness; to identify the potential moderators of efficacy; and to compare the efficacy of CBTEx versus each condition alone (CBT and physical exercise). Methods. Relevant randomized clinical trials, published before July 2017, were identified through database searches in Pubmed, PsycArticles, CINAHL, SportDiscus and the Cochrane Central Register for Controlled Trials. Results. A total of 30 studies were identified. CBTEx interventions yielded small-to-large effect sizes for depression (SMC = -0.34, 95% CI [-0.53; -0.14]), anxiety (SMC = -0.18, 95% CI [-0.34; -0.03]) and fatigue (SMC = -0.96, 95% CI [-1.43; -0.49]). Moderation analyses revealed that longer intervention was associated with greater effect sizes for depression and anxiety outcomes. Low methodological quality was also associated with increased CBTEx efficacy for depression. When compared directly, CBTEx interventions did not show greater efficacy than CBT alone or physical exercise alone for any of the outcomes. Conclusion. The current literature suggests that CBTEx interventions are effective for decreasing depression, anxiety, and fatigue symptoms, but not pain. However, the findings do not support an additive effect of CBT and exercise on any of the four outcomes compared to each condition alone.

Abstract

Metabolomics is now widely used to characterize metabolic phenotypes associated with lifestyle risk factors such as obesity. The objective of the present study was to explore the associations of body mass index (BMI) with 145 metabolites measured in blood samples in the European Prospective Investigation into Cancer and Nutrition (EPIC) study. Metabolites were measured in blood from 392 men from the Oxford (UK) cohort (EPIC-Oxford) and in 327 control subjects who were part of a nested case-control study on hepatobiliary carcinomas (EPIC-Hepatobiliary). Measured metabolites included amino acids, acylcarnitines, hexoses, biogenic amines, phosphatidylcholines, and sphingomyelins. Linear regression models controlled for potential confounders and multiple testing were run to evaluate the associations of metabolite concentrations with BMI. 40 and 45 individual metabolites showed significant differences according to BMI variations, in the EPIC-Oxford and EPIC-Hepatobiliary subcohorts, respectively. Twenty two individual metabolites (kynurenine, one sphingomyelin, glutamate and 19 phosphatidylcholines) were associated with BMI in both subcohorts. The present findings provide additional knowledge on blood metabolic signatures of BMI in European adults, which may help identify mechanisms mediating the relationship of BMI with obesity-related diseases.

Abstract

International audience; A meta-analysis of randomized controlled trials (RCTs) has recently showed that theory-based interventions designed to promote physical activity (PA) significantly increased PA behavior. The objective of the present study was to investigate the moderators of the efficacy of these theory-based interventions. Seventy-seven RCTs evaluating theory-based interventions were systematically identified. Sample, intervention, methodology, and theory implementation characteristics were extracted, coded by three duos of independent investigators, and tested as moderators of interventions effect in a multiple–meta-regression model. Three moderators were negatively associated with the efficacy of theory-based interventions on PA behavior: intervention length (≥14 weeks; β = −.22, p = .004), number of experimental patients (β = −.10, p = .002), and global methodological quality score (β = −.08, p = .04). Our findings suggest that the efficacy of theory-based interventions to promote PA could be overestimated consequently due to methodological weaknesses of RCTs and that interventions shorter than 14 weeks could maximize the increase of PA behavior.

Abstract

Introduction: The objective of this study is to investigate factors associated with risks of perinatal death in a disadvantaged, high-migrant French district with mortality rates above the national average.Material and Methods: The study design is a perinatal audit in 2014 in all 11 maternity units in the Seine-Saint-Denis district (25 037 births). The data come from medical chart abstraction, maternal interviews and peer assessor confidential review of deaths. A representative sample of live births in the same district, from the 2010 French Perinatal Survey, was used for comparisons (n = 429). The main outcome measures were stillbirth and neonatal death (0-27 days) at ≥22 weeks of gestation.Results: The audit included 218 women and 227 deaths (156 stillbirths, 71 neonatal deaths); 75 women were interviewed. In addition to primiparity and multiple pregnancy, overweight and obesity increased mortality risks (50% of cases, adjusted odds ratios [aOR] 1.7, 95% confidence interval [CI] 1.1-2.8, and aOR 1.9 [95% CI 1.1-3.2], respectively) as did the presence of preexisting medical/obstetric conditions (28.6% of cases, aOR 3.2, 95% CI 2.0-5.3). Problems accessing or complying with care were noted in 25% of medical records and recounted in 50% of interviews. Assessors identified suboptimal factors in 73.2% of deaths and judged 33.9% to be possibly or probably preventable. Care not adapted to risk factors and poor healthcare coordination were frequent suboptimal factors. Possibly preventable deaths were higher (P < .05) for women with gestational diabetes or hypertension (44.6%) than women without (29.0%).Conclusions: Preventive actions to improve healthcare referral and coordination, especially for overweight and obese women and women with medical and obstetrical risk factors, could reduce perinatal mortality in disadvantaged areas.
