Abstract

This paper aims to investigate a reaction-diffusion model which describes in-host infection for Mycobacterium tuberculosis (Mtb) allowing random motion (i.e. linear diffusion) and chemotaxis (i.e. non-linear diffusion) of macrophages and bacteria populations. In particular, chemotaxis-driven aggregation of macrophages plays a fundamental role in the development of the Mtb infection and the production of chemokine - located in the infection site - represents an attractant for the uninfected macrophages, therefore we consider chemotaxis between infected macrophages and uninfected macrophages. The linear stability of the endemic equilibria is investigated. In particular, by looking for conditions guaranteeing that an equilibrium, stable in the absence of diffusion, becomes unstable when diffusion is allowed, the formation of Turing patterns - that biologically represent the formation of granuloma initiated by the immune cells - is investigated. The weakly nonlinear analysis is performed to deeply explore the patterns amplitude. Furthermore, via numerical simulations, the formation of Turing patterns is analysed.

Abstract

A mathematical model for anaerobic digestion in plug-flow reactors is proposed on the basis of mass balance considerations. The model consists of a system of parabolic partial differential equations for the variables representing the concentrations of the bio-components constituting the waste matrix and takes into account convective and diffusive phenomena. The plug-flow reactor is modelled as a one-dimensional domain; the waste matrix moves in the direction of the reactor axis and undergoes diffusive phenomena which reproduce the movement of the bio-components along the reactor axis due to a gradient in concentration. The velocity characterizing the convection of the waste matrix is not fixed a priori but it is considered as an additional unknown of the mathematical problem. The variation in the convective velocity allows to account the mass variation occurring along a plug-flow reactor due to the conversion of solids. The equation governing the convective velocity is derived by considering the density of the waste matrix within the reactor constant over time and the sum of the volume fractions of the bio-components constituting the waste matrix constrained to unity. The waste matrix undergoes biochemical transformations catalysed by anaerobic microbial species which lead to the production of gaseous methane, the final product of the anaerobic digestion process. Biochemical processes are modelled using a simplified scheme and a differential equation is used to describe the dynamics of the produced gaseous methane. A finite difference scheme is used for the numerical integration. Model consistency is showed through numerical simulations which investigate the effect of the variation of some operating parameters on process performance. The model is then applied to a real case scenario of engineering interest. Abstract

Introduction: Over the years, disease registers have been increasingly considered a source of reliable and valuable population studies. However, the validity and reliability of data from registers may be limited by missing data, selection bias or data quality not adequately evaluated or checked. This study reports the analysis of the consistency and completeness of the data in the Italian Multiple Sclerosis and Related Disorders Register. Methods: The Register collects, through a standardized Web-based Application, unique patients. Data are exported bimonthly and evaluated to assess the updating and completeness, and to check the quality and consistency. Eight clinical indicators are evaluated. Results: The Register counts 77,628 patients registered by 126 centres. The number of centres has increased over time, as their capacity to collect patients. The percentages of updated patients (with at least one visit in the last 24 months) have increased from 33% (enrolment period 2000–2015) to 60% (enrolment period 2016–2022). In the cohort of patients registered after 2016, there were ≥ 75% updated patients in 30% of the small centres (33), in 9% of the medium centres (11), and in all the large centres (2). Clinical indicators show significant improvement for the active patients, expanded disability status scale every 6 months or once every 12 months, visits every 6 months, first visit within 1 year and MRI every 12 months. Conclusions: Data from disease registers provide guidance for evidence-based health policies and research, so methods and strategies ensuring their quality and reliability are crucial and have several potential applications.

In the cohort of patients registered after 2016, there were >= 75% updated patients in 30% of the small centres (33), in 9% of the medium centres (11), and in all the large centres (2).Clinical indicators show significant improvement for the active patients, expanded disability status scale every 6 months or once every 12 months, visits every 6 months, first visit within 1 year and MRI every 12 months.ConclusionsData from disease registers provide guidance for evidence-based health policies and research, so methods and strategies ensuring their quality and reliability are crucial and have several potential applications.

33. Evidentiary and Charging Matters in the Context of Prosecuting Returning Foreign Fighters Before National Courts

Abstract

States have been reluctant to repatriate their foreign fighters and families for a variety of reasons. One of these is of a legal nature, namely that prosecution at home would be too difficult because of a lack of evidence. After a brief overview of possible other options, this chapter will focus on the prosecution of returning foreign fighters before national courts. While acknowledging that securing evidence is and will remain difficult, this chapter points to a number of evidentiary and charging opportunities that show potential in somewhat overcoming this challenge. These could assist prosecutors in focusing on what should have their priority, namely the actual acts committed, such as war crimes and other international crimes. This chapter will also demonstrate that, in addition to membership of a terrorist organisation, there are other charges out there that do not require establishing what the returning foreign fighter has actually done while being abroad. The aim of presenting these in this chapter should not be seen as an endorsement—their critical examination will make this quite clear—but as a demonstration of the fact that the refusal to repatriate and prosecute can only be explained by a lack of political will, and not because of a lack of prosecutorial options.

34. Returning Foreign Fighters: Responses, Legal Challenges and Ways Forward

Abstract

This book, a follow-up publication to the 2016 volume Foreign Fighters under International Law and Beyond, zooms in on the responses that the international community and individual States are implementing in response to (prospective and actual) returning foreign fighters (FFs) and their families, focusing on returnees from Syria and Iraq to European countries. As States and international organisations are still ‘learning by doing’, the role of the academic community is to help steer the process by bridging the divide between international standards and their implementation at the national level and between security concerns and human rights law. Furthermore, the academic community can and should assist in identifying ways forward that are both effective, sustainable and international law-compliant. Those are, ultimately, the goals that the present volume seeks to pursue. The observations, recommendations and calls for caution included in this book will be useful in future debates on (returning) FFs, both in the academic world and in the world of policy makers and practitioners, as well as to the public at large.

35. Family Courts as part of the States’ Counter-Terrorism Toolkit: A Welcome Development for Children of FTFs?

Abstract

The case for family courts having a bigger or exclusive role in making and monitoring arrangements for the families and, in particular, the children of alleged foreign terrorist fighters (FTFs) is strong. These courts can be a conduit for a whole of government approach in respect of children potentially impacted by radicalisation or extremism and/or children of alleged FTFs. As part of the broader rule of law framework, the family justice system works in cooperation with the criminal justice system to ensure the proper administration of justice in both of these jurisdictions. What makes family law better suited to addressing cases of (potentially) radicalised children or children of alleged FTFs, however, is the best interests of the child principle—a foundational principle of family law regardless of the jurisdiction. What this chapter proposes to do is to put forward the case for family courts and pertinent child support and protection services as a more long-term and sustainable response to the plight of accompanied and orphaned children of alleged FTFs: a response that respects the applicable international legal obligations.

Abstract

Background and purpose: In relapsing-remitting multiple sclerosis patients (RRMS) disability progressively accumulates over time. To compare the cumulative probability of 6-month confirmed disability-worsening events using a fixed baseline or a roving Expanded Disability Status Scale (EDSS) reference, in a real-world setting. Methods: A cohort of 7964 RRMS patients followed for 2 or more years, with EDSS scores recorded every 6 months, was selected from the Italian Multiple Sclerosis Register. The overall probability of confirmed disability-worsening events and of confirmed disability-worsening events unrelated to relapse was evaluated using as reference a fixed baseline EDSS score or a roving EDSS score in which the increase had to be separated from the last EDSS assessment by at least 6 or 12 months. Results: Using a fixed baseline EDSS reference, the cumulative probability of 6-year overall confirmed disability-worsening events was 33.2%, and that of events unrelated to relapse was 10.9% (33% of overall confirmed disability-worsening events). Using a roving EDSS, the proportions were respectively 35.2% and 21.3% (61% of overall confirmed disability-worsening events). Conclusions: In a real-world setting, roving EDSS reference scores appear to be more sensitive for detecting confirmed disability-worsening events unrelated to relapse in RRMS patients.

Abstract

The pathophysiological mechanisms of Moyamoya angiopathy (MA), which is a rare cerebrovascular condition characterized by recurrent ischemic/hemorrhagic strokes, are still largely unknown. An imbalance of vasculogenic/angiogenic mechanisms has been proposed as one possible disease aspect. Circulating endothelial progenitor cells (cEPCs) have been hypothesized to contribute to vascular remodeling of MA, but it remains unclear whether they might be considered a disease effect or have a role in disease pathogenesis. The aim of the present study was to provide a morphological, phenotypical, and functional characterization of the cEPCs from MA patients to uncover their role in the disease pathophysiology. cEPCs were identified from whole blood as CD45dimCD34+CD133+ mononuclear cells. Morphological, biochemical, and functional assays were performed to characterize cEPCs. A significant reduced level of cEPCs was found in blood samples collected from a homogeneous group of adult (mean age 46.86 ± 11.7; 86.36% females), Caucasian, non-operated MA patients with respect to healthy donors (HD; p = 0.032). Since no difference in cEPC characteristics and functionality was observed between MA patients and HD, a defective recruitment mechanism could be involved in the disease pathophysiology. Collectively, our results suggest that cEPC level more than endothelial progenitor cell (EPC) functionality seems to be a potential marker of MA. The validation of our results on a larger population and the correlation with clinical data as well as the use of more complex cellular model could help our understanding of EPC role in MA pathophysiology.

Abstract

Background: Multiple sclerosis (MS) is characterized by phenotypical heterogeneity, partly resulting from demographic and environmental risk factors. Socio-economic factors and the characteristics of local MS facilities might also play a part. Methods: This study included patients with a confirmed MS diagnosis enrolled in the Italian MS and Related Disorders Register in 2000–2021. Patients at first visit were classified as having a clinically isolated syndrome (CIS), relapsing–remitting (RR), primary progressive (PP), progressive-relapsing (PR), or secondary progressive MS (SP). Demographic and clinical characteristics were analyzed, with centers’ characteristics, geographic macro-areas, and Deprivation Index. We computed the odds ratios (OR) for CIS, PP/PR, and SP phenotypes, compared to the RR, using multivariate, multinomial, mixed effects logistic regression models. Results: In all 35,243 patients from 106 centers were included. The OR of presenting more advanced MS phenotypes than the RR phenotype at first visit significantly diminished in relation to calendar period. Females were at a significantly lower risk of a PP/PR or SP phenotype. Older age was associated with CIS, PP/PR, and SP. The risk of a longer interval between disease onset and first visit was lower for the CIS phenotype, but higher for PP/PR and SP. The probability of SP at first visit was greater in the South of Italy. Discussion: Differences in the phenotype of MS patients first seen in Italian centers can be only partly explained by differences in the centers’ characteristics. The demographic and socio-economic characteristics of MS patients seem to be the main determinants of the phenotypes at first referral.

Abstract

Malignant mesothelioma (MM) is an aggressive asbestos-related cancer, against which no curative modalities exist. Oncolytic virotherapy is a promising therapeutic approach, for which MM is an ideal candidate; indeed, the pleural location provides direct access for the intra-tumoral injection of oncolytic viruses (OVs). Some non-human OVs offer advantages over human OVs, including the non-pathogenicity in humans and the absence of pre-existing immunity. We previously showed that caprine herpesvirus 1 (CpHV-1), a non-pathogenic virus for humans, can kill different human cancer cell lines. Here, we assessed CpHV-1 effects on MM (NCI-H28, MSTO, NCI-H2052) and non-tumor mesothelial (MET-5A) cells. We found that CpHV-1 reduced cell viability and clonogenic potential in all MM cell lines without affecting non-tumor cells, in which, indeed, we did not detect intracellular viral DNA after treatment. In particular, CpHV-1 induced MM cell apoptosis and accumulation in G0/G1 or S cell cycle phases. Moreover, CpHV-1 strongly synergized with cisplatin, the drug currently used in MM chemotherapy, and this agent combination did not affect normal mesothelial cells. Although further studies are required to elucidate the mechanisms underlying the selective CpHV-1 action on MM cells, our data suggest that the CpHV-1-cisplatin combination could be a feasible strategy against MM.
