ObjectiveThe aim of this study was to analyze the virulence genes， antibiotic resistance， and molecular typing of Vibrio parahaemolyticus （V.parahaemolytics） isolates from multiple sources in Wuxi City between 2016 and 2021.MethodsMultiplex real-time PCR， microbroth dilution， and pulsed field gel electrophoresis （PFGE） were used to detect virulence genes （tlh/tdh/trh）， evaluate antibiotic resistance， and perform molecular typing of 204 V. parahaemolyticus strains isolated from various surveillance samples in Wuxi City. χ2 tests were used for statistical analysis.ResultsAmong the V. parahaemolyticus isolates， the prevalence of tlh， tdh， and trh was 100% （204/204）， 82.35% （168/204）， and 2.45% （5/204）， respectively. A significant difference was found in the prevalence of tdh between food/environmental sources and patient-source isolates （P < 0.001）. Most （96.08%， 196/204） isolates were resistant to cefazolin， and 2.94% （6/204） of the isolates displayed a multidrug resistance pattern （resistant to three or more drugs）. Strains from both food/environmental and patient sources were significantly （P < 0.05） resistant to ampicillin， tetracycline， methomyl/sulfamethoxazole， and ciprofloxacin. The 204 V. parahaemolyticus strains were clustered into 123 PFGE band types with 49.1%-100.0% similarity， which could be differentiated into 18 different genetic clusters with 85% similarity.ConclusionMost V. parahaemolyticus isolates from patient sources in Wuxi City harbored the tdh gene. The isolated V. parahaemolyticus strains showed the highest resistance to cefazolin. The PFGE types were polymorphic， and no major bands were found.

This paper interprets the content and recommendations of the guidelines on infection prevention and control in long-term care facilities put forward by the World Health Organization （WHO） during the 2019 coronavirus disease （COVID-19） pandemic， and actively explores the key points of nursing and infection prevention and control measures for the long-term care facilities under the background of repeated outbreaks， with the aim of providing care measures and infection prevention and control measures that suit our national conditions to improve the living standards of the elderly and protect them from viral infection amid the recurring pandemic.

Objective: To investigate the prevalence of vitamin D deficiency (VDD) in children/adolescents in extreme southern China. Methods: This multicenter, cross-sectional study included 21,811 children aged 0–18 years from 18 districts in Hainan Province, using a multistage stratified random sampling method from January 2021 to March 2022. Results: Serum 25(OH)D levels decreased with age (p trend <0.001). VDD prevalence increased significantly from 3.7% (95% CI: 3.2, 4.3) in children aged 0–3 years to 43.5% (95% CI: 42.1, 45.0) in those aged 13–18 years. Girls and urban residents showed higher deficiency rates. Adolescents (13–18 years) had the highest prevalence of VDD (43.5%), while toddlers (0–3 years) had the lowest (3.7%). Factors influencing vitamin D status included gender, urban residency, and breastfeeding duration. Seasonal variations showed higher deficiency rates in autumn, particularly among preschoolers. Regional differences were noted, with the highest deficiency in semiarid and subhumid zones for various age groups. Conclusion: A significant increase in VDD with age, particularly among adolescents, urban girls, and during autumn, emphasizing the need for targeted strategies. [ABSTRACT FROM AUTHOR]

ObjectiveTo learn the epidemiological features of foodborne disease in Baiyun District， Guangzhou City from 2018 to 2020， so as to provide scientific basis for the development of targeted prevention and control measures.MethodsThe descriptive analysis method was used to analyze the data of 1 226 cases of foodborne diseases reported by surveillance hospitals in Baiyun District， Guangzhou City from 2018 to 2020. The epidemiological and clinical characteristics were described， and the Logistic regression analysis was conducted on the influencing factors of timely outpatient rate.ResultsFrom 2018 to 2020， there were 567 cases of foodborne diseases in males （46.25%， 567/1 226） and 659 cases in females （53.75%，659/1 226） in Baiyun District， Guangzhou City. The age of cases mainly ranged from 21 to 30 years old （37.19%，456/1 226）. The occupational distribution of cases was dominated by civil servants， employees of enterprises and institutions. The peak period was June to October. Family（41.80%， 754/1 804） was the main suspicious eating place. Meat and meat products were the main types of suspect foods. Catering service industry was the main processing and packaging method. Multivariate Logistic regression showed that old age， high hospital grade， vomiting times were positively correlated with the timeliness of health-seeking， diarrhea and diarrhea times were more negatively correlated.ConclusionThe high incidence of foodborne diseases in Baiyun District， Cuangzhou City was in hot seasons， and families and food service organizations were the high risk places. Food safety supervision and health education should be further targeted to prevent and control the occurrence and prevalence of food-borne diseases．

Objective To explore the clinical value of artificial intelligence (AI) in the diagnosis of intracranial aneurysms by head and neck CT angiography (CTA). Methods A total of 117 patients with clinically suspected intracranial aneurysms admitted in our hospital from August 2020 to January 2021 were enrolled, and their head and neck CTA and digital subtraction angiography (DSA) data were collected and retrospectively analyzed. Taking DSA results as the gold standard, the sensitivity and accuracy of AI in the diagnosis of intracranial aneurysms were calculated. In addition, the results of AI automatic measurement were recorded, and the consistency test was conducted with the results of DSA manual measurement. Results Among the 117 suspected patients, there were 105 cases diagnosed with intracranial aneurysms by DSA, with a total of 136 aneurysms. ①Diagnostic efficiency: The sensitivity and accuracy of AI for aneurysm diagnosis were 87.6% and 79.5%, respectively, taking patients as unit. While taking lesions as unit, the sensitivity of AI diagnosis was 55.2%, 81.5%, 92.9% and 79.4% for aneurysms < 3 mm, 3.5 mm, >5 mm, as well as for the total number of aneurysms, respectively; And the accuracy was 53.8%, 75.3%, 86.3% and 72.0%. ②Quantitative analysis: The results of AI and DSA showed good consistency in the measurement of the total aneurysm length diameter, total short diameter, and >5 mm aneurysm length diameter (ICC=0.778, 0.822 and 0.722, P < 0.001); But poor consistency was seen in the measurement for < 3 mm and 3~5 mm aneurysm length diameter (ICC=0.113, 0.142, P>0.05). Conclusion AI has a high value in the application of head and neck CTA in the diagnosis of intracranial aneurysms, with its measurement results to be used as a reference standard in tumors with length diameter >5 mm, it can be further promoted and applied as an auxiliary diagnostic tool.

BACKGROUND: The psychological resilience of patients with traumatic lower extremity fractures is relevant and has been studied in the postoperative rehabilitation phase; yet, few studies have focused on the early preoperative phase. OBJECTIVE: This study aims to explore preoperative psychological resilience in patients with traumatic lower extremity fractures. METHODS: This single-center cross-sectional survey design study was conducted over 5 months from December 2022 to April 2023 in a tertiary hospital in Shanghai, China. We used a convenience sampling method to assess psychological resilience in traumatic lower extremity fracture patients electronically. We administered the online surveys using the Questionnaire Star applet prior to surgery using the following tools: (a) general information questionnaire, (b) Numerical Rating Scale, (c) Connor–Davidson Resilience Scale, and (d) General Self-Efficacy Scale. RESULTS: A total of N = 175 patients were studied, of which n = 166 patients fully completed the survey for a response rate of 95%. Patients with traumatic lower extremity fractures demonstrated a low degree of psychological resilience preoperatively (M = 49.28, SD = 13.64). Patients' psychological resilience and self-efficacy showed a significant positive correlation (r =.577). Factors associated with patients' psychological resilience in multivariate linear analyses included level of education, medical insurance, and self-efficacy (p <.05). CONCLUSION: We found that preoperative psychological resilience in patients with traumatic lower-extremity fractures is poor and influenced by various factors; thus, future efforts should focus on providing timely, targeted interventions. BACKGROUND: The association between infection with cagA-positive H. pylori and an elevated susceptibility to gastric cancer has been firmly established. PIM2 is known to be overexpressed in various types of cancers; however, the specific mechanism by which cagA influences the regulation of PIM2 expression in gastric cancer remains unidentified at present. MATERIALS AND METHODS: A mutant NCTC11637 Δ cagA strain of H. pylori and the eukaryotic expression vector pcDNA-cagA were constructed for evaluating PIM2 expression levels in gastric cancer cells (HGC27, SGC7901, and AG) co-cultured with the NCTC11637 and NCTC11637 Δ cagA strain, as well as pcDNA-cagA and the empty vector pcDNA3.1 (+). RESULTS: Co-culturing gastric cancer cells with NCTC11637 significantly increased PIM2 expression levels (P < 0.001) compared to the negative control group. Additionally, the expression of PIM2 in cells co-cultured with NCTC11637 was higher than that co-cultured with NCTC11637 Δ cagA (P < 0.001). Furthermore, successful construction of the eukaryotic expression vector pcDNA-cagA resulted in a significant increase in PIM2 mRNA expression levels after its transfection into gastric cancer cells compared to the control group after 48 hours. CONCLUSIONS: The findings indicate that H. pylori/cagA A could be one of the key factors in regulating PIM2 expression levels, potentially influencing the progression of H. pylori-related Gastric Cancer.
