- Abstract
Major surgical societies advised using non-operative management of appendicitis and suggested against laparoscopy during the COVID-19 pandemic. The hypothesis is that a significant reduction in the number of emergent appendectomies was observed during the pandemic, restricted to complex cases. The study aimed to analyse emergent surgical appendectomies during pandemic on a national basis and compare it to the same period of the previous year. This is a multicentre, retrospective, observational study investigating the outcomes of patients undergoing emergent appendectomy in March–April 2019 vs March–April 2020. The primary outcome was the number of appendectomies performed, classified according to the American Association for the Surgery of Trauma (AAST) score. Secondary outcomes were the type of surgical technique employed (laparoscopic vs open) and the complication rates. One thousand five hundred forty one patients with acute appendicitis underwent surgery during the two study periods. 1337 (86.8%) patients met the inclusion criteria: 546 (40.8%) patients underwent surgery for acute appendicitis in 2020 and 791 (59.2%) in 2019. According to AAST, patients with complicated appendicitis operated in 2019 were 30.3% vs 39.9% in 2020 (p = 0.001). We observed an increase in the number of post-operative complications in 2020 (15.9%) compared to 2019 (9.6%) (p < 0.001). The following determinants increased the likelihood of complication occurrence: undergoing surgery during 2020 (+ 67%), the increase of a unit in the AAST score (+ 26%), surgery performed > 24 h after admission (+ 58%), open surgery (+ 112%) and conversion to open surgery (+ 166%). In Italian hospitals, in March and April 2020, the number of appendectomies has drastically dropped. During the first pandemic wave, patients undergoing surgery were more frequently affected by more severe appendicitis than the previous year's timeframe and experienced a higher number of complications. Major surgical societies advised using non-operative management of appendicitis and suggested against laparoscopy during the COVID-19 pandemic. The hypothesis is that a significant reduction in the number of emergent appendectomies was observed during the pandemic, restricted to complex cases. The study aimed to analyse emergent surgical appendectomies during pandemic on a national basis and compare it to the same period of the previous year. This is a multicentre, retrospective, observational study investigating the outcomes of patients undergoing emergent appendectomy in March–April 2019 vs March–April 2020. The primary outcome was the number of appendectomies performed, classified according to the American Association for the Surgery of Trauma (AAST) score. Secondary outcomes were the type of surgical technique employed (laparoscopic vs open) and the complication rates. One thousand five hundred forty one patients with acute appendicitis underwent surgery during the two study periods. 1337 (86.8%) patients met the inclusion criteria: 546 (40.8%) patients underwent surgery for acute appendicitis in 2020 and 791 (59.2%) in 2019. According to AAST, patients with complicated appendicitis operated in 2019 were 30.3% vs 39.9% in 2020 (p = 0.001). We observed an increase in the number of post-operative complications in 2020 (15.9%) compared to 2019 (9.6%) (p 24 h after admission (+ 58%), open surgery (+ 112%) and conversion to open surgery (+ 166%). In Italian hospitals, in March and April 2020, the number of appendectomies has drastically dropped. During the first pandemic wave, patients undergoing surgery were more frequently affected by more severe appendicitis than the previous year's timeframe and experienced a higher number of complications. - Abstract
Background: In recent years, the emergency management of acute left colonic diverticulitis (ALCD) has evolved dramatically despite lack of strong evidence. As a consequence, management strategies are frequently guided by surgeon’s personal preference, rather than by scientific evidence. The primary aim of IPOD study (Italian Prospective Observational Diverticulitis study) is to describe both the diagnostic and treatment profiles of patients with ALCD in the Italian surgical departments. Methods: IPOD study is a prospective observational study performed during a 6-month period (from April 1 2015 to September 1 2015) and including 89 Italian surgical departments. All consecutive patients with suspected clinical diagnosis of ALCD confirmed by imaging and seen by a surgeon were included in the study. The study was promoted by the Italian Society of Hospital Surgeons and the World Society of Emergency Surgery Italian chapter. Results: Eleven hundred and twenty-five patients with a median age of 62 years [interquartile range (IQR), 51–74] were enrolled in the IPOD study. One thousand and fifty-four (93.7%) patients were hospitalized with a median duration of hospitalization of 7 days (IQR 5–10). Eight hundred and twenty-eight patients (73.6%) underwent medical treatment alone, 13 patients had percutaneous drainage (1.2%), and the other 284 (25.2%) patients underwent surgery as first treatment. Among 121 patients having diffuse peritonitis, 71 (58.7%) underwent Hartmann’s resection. However, the Hartmann’s resection was used even in patients with lower stages of ALCD (36/479; 7.5%) where other treatment options could be more adequate. Conclusions: The IPOD study demonstrates that in the Italian surgical departments treatment strategies for ALCD are often guided by the surgeon’s personal preference.

Several authors characterized the photo-induced absorption (PA) in a-Si:H by performing suitable physical models describing the optical modifications of the illuminated material. Usual investigation techniques study the phenomenon by analysing transmitted or reflected probe light directed on the film surface. Alternatively, we report an in-guide analysis performed through properly designed planar a-Si:H/SiO2 waveguides, fabricated on a c-Si substrate by plasma enhanced chemical vapour deposition (PECVD). The technique allows a wider interaction region between the probe and the pump beams, then the experimental setup sensitivity improves highly. As probe, we used a coherent beam emitting at 1550 nm wavelength and different pump lights in the visible spectrum. We observed the amplitude modulation induced over the probe radiation traveling through the waveguide when an intermittent visible pump was shone over it. LEDs emitting at 644, 612 and 590 nm wavelengths, at intensities up to 0.85 mW/cm(2), were alternatively used. The PA effect shows a linear increasing on the pump intensity and is stronger for longer wavelength pumps. A numerical analysis of the optical propagation was performed to evaluate the absorption variation: at the highest pump intensity the maxima values for Deltaalpha(1550) were 2.15, 1.7 and 0.88 cm(-1), respectively, for pump emitting at 590, 612 and 644 nm.

- Abstract
A novel rib waveguide-integrated switch is proposed and theoretically discussed. The device is based on the total internal reflection phenomenon (TIR) and the thermo-optic effect (TOE) in hydrogenated amorphous silicon (a-Si:H) and crystalline silicon (c-Si). It takes advantage of a bandgap-engineered a-Si:H layer to explore the properties of an optical interface between materials showing similar refractive indexes but different thermo-optic coefficients. In particular, thanks to modern plasma-enhanced chemical vapor deposition techniques, the refractive index of the amorphous film can be properly tailored to match that of c-Si at a given temperature. TIR may be achieved, however, at the interface by acting on the temperature, because the two materials have different thermo-optic coefficients. The switch is integrated in a 4-mum-wide and 3-mum-thick single-mode rib waveguide. The substrate is a silicon-on-insulator (SOI) wafer with an oxide thickness of 500 nm. The active middle region optimal length is 282 mum. The device performance is analyzed at a wavelength of 1.55 mum. It is shown that the output crosstalk and insertion loss are less than -26 and 3.5 dB, respectively.

Credit risk has always been an important issue for banks and other financial intermediaries. A reliable and consistent computing system is necessary to simplify the decision making process in uncertain domains such as the assessment of credit risk. In recent years fuzzy logic techniques have been in their wide-ranging use in modeling of uncertainties, vagueness, impreciseness and the human thought process. Therefore in this paper we present a fuzzy logic based intelligent system for the assessment of credit risk. It aims at efficiently and intelligently managing the process, handling its complexity, and monitoring large amounts of dynamic information in a distributed way, by using a society of intelligent agents, each carrying out a different function autonomously. Simple Summary Although the effectiveness of screening in reducing the mortality of breast and cervical cancer in the EU is established, the impact of these cancers continues to be substantial among women. The aims of this study were to evaluate the attendance to breast and cervical cancer screening and the role of related determinants in an area of Southern Italy. Only 49.7% of the sample reported to have undergone mammography in the previous two years, 27.7% within an organized program and 22% as an opportunistic procedure. The attendance to cervical cancer screening interval of three years was reported by 56.1% of women, 16.1% within an organized program and 40% as an opportunistic procedure. A very low attendance was thus detected for both breast and cervical cancer organized screening programs. A strong commitment to the promotion of these programs is urgently needed, also to support their role in the reduction of inequalities of attendance of disadvantaged women. The miscellaneous codex Par. gr. 1630 (mid-14th century), written by the scribe Chariton, of the monastery τῶν Ὁδηγῶν, contains, among many other texts, a selection of epigrams from the so-called Sylloge Parisina, fully preserved by a 12th/13th-century manuscript, the Par. suppl. gr. 352. The aim of this paper is to analyse the principles followed by Chariton in designing his (informal) anthology. Particular attention will be paid to a synonymic substitution that might be explained by his moral scruples, as they also appear from his selection of poems with respect to his model.

Several authors characterized the photo-induced absorption (PA) in a-Si:H by performing suitable physical models describing the optical modifications of the illuminated material. Usual investigation techniques study the phenomenon by analysing transmitted or reflected probe light directed on the film surface. Alternatively, we report an in-guide analysis performed through properly designed planar a-Si:H/SiO 2 waveguides, fabricated on a c-Si substrate by plasma enhanced chemical vapour deposition (PECVD). The technique allows a wider interaction region between the probe and the pump beams, then the experimental setup sensitivity improves highly. As probe, we used a coherent beam emitting at 1550 nm wavelength and different pump lights in the visible spectrum. We observed the amplitude modulation induced over the probe radiation traveling through the waveguide when an intermittent visible pump was shone over it. LEDs emitting at 644, 612 and 590 nm wavelengths, at intensities up to 0.85 mW/cm 2 , were alternatively used. The PA effect shows a linear increasing on the pump intensity and is stronger for longer wavelength pumps. A numerical analysis of the optical propagation was performed to evaluate the absorption variation: at the highest pump intensity the maxima values for Δ α 1550 were 2.15, 1.7 and 0.88 cm −1 , respectively, for pump emitting at 590, 612 and 644 nm.

Based on the total internal reflection (TIR) phenomenon and the thermo-optic effect in hydrogenated amorphous silicon (a-Si:H) and crystalline silicon (c-Si), a symmetric rib optical waveguide integrated switch is proposed and theoretically discussed. The device exploits the similar refractive index coupled to the different thermo-optic coefficient in the two materials. The possibility of alloying and doping for the band-gap engineering of a-Si:H, by means of the gas phase composition during the modern plasma enhanced chemical vapour deposition process, which takes place at temperatures as low as 220 degrees C, makes this semiconductor ideal for this type of application. In particular the refractive index at room temperature of the amorphous film can be properly tailored to match that of c-Si in order to achieve the light switching when the device experiences a given temperature change. TIR may be achieved however at the interface by acting on the temperature, because the two materials have different thermo-optic coefficient. The integrated single-mode rib waveguide is 4 μm wide and 3 μm high. The substrate is a SOI wafer with an oxide thickness of 500 nm. The switch has a quite short operation length of about 280 μm. The device performance is analyzed at the wavelength of 1.55 μm. It shows that the output crosstalk and insertion loss are less than -26.9 dB and 3.5 dB, respectively.

29. Community-Acquired and Healthcare-Associated Sepsis: Characteristics and in-Hospital Mortality in Italy.

- Abstract
Background : The main aim of the study was to analyse characteristics of sepsis according to the setting of occurrence and to identify predictors of sepsis-related in-hospital mortality. Methods : 544 medical records of adult patients with a diagnosis of sepsis were consulted and divided into two groups according to the setting where sepsis originated: community-acquired (CA) and healthcare-associated (HA) sepsis. Results : Overall, 257 (47.2%) patients had HA sepsis and the in-hospital death rate was 33.6%. Results of the multiple logistic regression revealed that patients with HA sepsis were significantly more likely to have been admitted from another hospital or ward, to have a ≥1 Charlson's index, to be immunesuppressed, and to have undergone a surgical intervention during hospitalization. In-hospital deaths were significantly associated with older age, admission from another hospital or ward, need of haemodialysis and mechanical ventilation (MV), whereas they were less likely in patients with HA sepsis as compared with CA sepsis. Conclusion : Community-acquired and HA sepsis show distinct clinical, prognostic and risk factors profiles, and should be managed according to their differential characteristics.

- Abstract
The aim of this study was to verify feasibility and safety of single incision laparoscopic cholecystectomy using conventional surgical instruments. Twenty patients underwent single incision laparoscopic cholecystectomy. Indications for cholecystectomy were symptomatic cholelithiasis. Operative time, postoperative length of stay, intraoperative and postoperative complications, postoperative pain (by visual analogue scale), and patient cosmetic satisfaction were considered. All 20 procedures were successfully concluded without additional skin incisions. The mean operative time was 69 ± 16 min with a trend toward a reduction in operating time with increasing surgical experience. Intraoperative bile leakages for gallbladder rupture were observed in only three cases. No postoperative complications were recorded. The median pain scale value was 3 and the median value of cosmetic satisfaction was 3. The mean postoperative length of stay was 1.3 ± 0.9 days. Single incision laparoscopic cholecystectomy with conventional surgical instruments is a feasible and safe procedure, but additional studies in larger series are needed to confirm our results.

- Abstract
Objective: This study was designed to describe the surgical technique for single-incision laparoscopic right colectomy and present preliminary short-term results. Laparoscopic surgery has been fully validated as alternative, minimally invasive treatment for different benign and malignant conditions. In the attempt to reduce even more the surgical trauma, natural orifices transluminal endoscopic surgery (NOTES™) and single-incision laparoscopic surgery (SILS) have been proposed. Although the lack of proper instrumentations makes NOTES™ not fully suitable for advanced procedures, SILS might play a significant role, although, to date, only limited series and few case reports of single-incision right colectomy are present in the literature., Methods: After signed, informed consent was obtained, patients with malignant tumors or large polyps of the right colon underwent single-incision colonic resection through a 3-cm incision using two different single-port devices and articulated or coaxial curved instruments. Preliminary results were analyzed retrospectively., Results: A total of 36 patients were selected for SILS procedure. There were no intraoperative complications or conversions to the standard laparoscopic procedure. One patient had a postoperative urinary tract infection and one prolonged ileum that did not required any surgical intervention. No complications were reported in all the remaining cases. The mean postoperative stay was 5 ± 1.2 days (range, 4-14), and mean lymph node retrieval and tumor-free margins was 24 ± 7 (range, 29-15) and 8 ± 3 (range, 6-12) cm, respectively., Conclusions: Our preliminary results show that single-incision laparoscopic right colectomies are feasible and safe from the oncological point of view; nevertheless larger, randomized experiences are needed to demonstrate the benefits of SILS compared with standard laparoscopic resections.

Advances in technology continue at a rapid pace affecting several aspects of life; medicine is no exception. Minimally invasive surgery is, by its nature, a highly technological specialty of surgical practice. The development of new instruments and new technologies during the last few years have allowed complex procedures that were previously considered unsuitable for minimally invasive surgery. The aim of this article is to review some of these advances and the impact they are having on the managements of different surgical problems.
