4. Evaluation of Evolux™ Intraocular Lenses in Cataract Surgery: Clinical Outcomes and Patient Satisfaction.

Abstract

Objective: This study aimed to evaluate the initial clinical outcomes of and patient satisfaction with the newly introduced extended monofocal Evolux™ intraocular lens following cataract surgery. Methods: A retrospective study was conducted, involving 18 patients (36 eyes) who received Evolux™ lenses bilaterally. The inclusion criteria comprised individuals over 40 years old with no active eye diseases, excluding cataracts, and no postoperative complications. Various parameters were evaluated, including distance, intermediate, and near visual acuity; refraction; defocus curves; dependency on spectacles; and patient satisfaction. Patient satisfaction and visual quality of life were measured using the Catquest-9SF and QOV questionnaires. Results: The average age of the patients was 72.7 years, with the majority being women (77.8%). The results demonstrated excellent postoperative visual acuity at different distances. The mean logMAR values for uncorrected visual acuity were −0.04 for distance vision, 0.15 for intermediate vision, and 0.35 for near vision. The defocus curve indicated good tolerance, with visual acuity exceeding 20/20 in significant defocus ranges. Most patients expressed high satisfaction with their vision without spectacles, particularly for distance and intermediate vision. The need for spectacles for near vision was notably reduced. Furthermore, there was a low incidence of photic phenomena like halos and glare, contributing to the overall high patient satisfaction. Conclusions: Evolux™ lenses were found to be an effective choice for cataract surgery, providing excellent visual acuity at various distances and high patient satisfaction. [ABSTRACT FROM AUTHOR]

Methods: Prospective non-comparative single-center study enrolling 29 patients (55–71 years) undergoing bilateral cataract surgery with implantation of the trifocal diffractive IOL Liberty 677CMY (Medicontur Medical Engineering Ltd., Zsámbék, Hungary). Visual and refractive outcomes as well as PROMs were evaluated during a 3-month follow-up: measurement of uncorrected and corrected distance (UDVA, CDVA), intermediate (UIVA, DCIVA) and near visual acuities (UNVA, DCNVA), defocus curve, patient satisfaction, photic phenomena perception, spectacle independence, and difficulty in performing some vision-related activities. Results: A total of 100%, 92%, and 80% of patients achieved a postoperative binocular UDVA, UIVA, and UNVA of 20/25 or better, respectively. Likewise, 100%, 80%, and 84% of patients achieved a postoperative binocular CDVA, DCIVA, and DCNVA of 20/25 or better, respectively. In the defocus curve, all mean visual acuity values were better than 0.15 logMAR for all defocus levels. A total of 95.8%, 95.8%, and 91.7% of patients referred to be satisfied with their distance, intermediate, and near visual vision, respectively. Mean overall Catquest Rasch calibrated score was −3.12 ± 0.98. Most of the patients were spectacle independent: far (95.8%), intermediate (95.8%) or near vision (91.7%). No bothersome or minimal to moderately bothersome halo, starburst, and glare was perceived by 83.3%, 83.4%, and 83.3% of patients, respectively. Conclusions: The trifocal IOL evaluated provides a visual acuity improvement, with high levels of spectacle independence, patient satisfaction, and perceived visual quality associated.

Abstract

This article presents the findings of a long-term study on the clinical outcomes, safety, and efficacy of the Visian implantable collamer lens (ICL) for myopia correction. The study followed patients for a mean duration of 17.52 years and found that the ICL procedure resulted in favorable visual outcomes with a low risk of complications. The study also found that the ICL did not cause a significant loss of endothelial cell density and had an optimal vaulting range to minimize complications. However, a small number of cases experienced complications such as angle-closure glaucoma and cataracts. 13. Comparison of wound architecture in implantable collamer lens surgery: Self-sealing single-plane opposite clear corneal incision versus main surgical incision

Abstract

Purpose: Incision architecture can play an important role in corneal astigmatism management through peripheral corneal relaxing incisions. The aim of this study was to compare the incision architecture of single-plane opposite clear corneal incisions (OCCIs) and main surgical incisions (MSIs) in patients undergoing implantable collamer lens (ICL) surgery. Methods: A retrospective cross-sectional tomographic analysis of MSI and OCCI architectures was performed 6 months after ICL surgery. Image acquisition was performed using spectral-domain anterior segment optical coherence tomography. Results: A total of 31 OCCIs and 24 MSIs were evaluated. The mean incision angle was 42.83 ± 5.69 degrees for MSIs and 48.26 ± 6.07 degrees for OCCIs (p < 0.01), and the mean MSI and OCCI length was 1146.70 ± 150.48 µm and 976.68 ± 140.19 µm, respectively (p < 0.01). The mean increase in epithelium depth in the wound was 37.63 ± 11.91 µm in the MSI group and 47.64 ± 15.45 µm in the OCCI group (p = 0.02). Endothelial misalignment was observed in both types of incisions. However, the misalignment with MSI was greater than with OCCI, 106.67 ± 31.84 µm versus 83.75 ± 23.39 µm (p = 0.01), respectively. Conclusion: Both types of incisions, OCCI and MSI, were shown to be safe with complete wound sealing and healing 6 months postoperatively. The MSIs performed in the temporal position were more angled and longer, with greater endothelial retraction and minor epithelial thickening in the wound area compared with astigmatic incisions without manipulation.

14. Management of low astigmatism in implantable collamer lens surgery: opposite clear corneal incisions versus toric implantable collamer lens

Abstract

PURPOSE: To compare 2 techniques to correct low astigmatism during implantable collamer lens (ICL) surgery: astigmatic opposite clear corneal incisions (OCCIs) and toric ICL (T-ICL). SETTING: Arruzafa Ophthalmological Hospital, Cordoba, Spain. DESIGN: Randomized prospective comparative study. METHODS: The study comprised 152 myopic eyes undergoing ICL surgery. Patients were separated into 2 groups: Group 1 (57 patients; 76 eyes) received a spherical ICL with OCCIs and Group 2 (53 patients; 76 eyes) received a T-ICL. The inclusion criteria were refractive astigmatism up to 1.50 diopters (D), regular corneal astigmatism up to 2.00 D (Sim K, Pentacam), and agreement between the refractive and topographic corneal cylinders (discrepancies less than 30 degrees axis or 0.50 D). The outcomes were evaluated after a 1-month follow-up. RESULTS: The T-ICL group achieved a mean postoperative spherical equivalent refraction and refractive astigmatism of -0.04 ± 0.17 D and -0.03 ± 0.12 D, respectively, vs -0.14 ± 0.33 D and -0.20 ± 0.36 D, in the OCCI group ( P < .001). Postoperative refractive astigmatism of less than 0.25 D was achieved in 94.74% of cases in the T-ICL group vs 73.68% in the OCCI group. Undercorrection of corneal astigmatism occurred in the OCCI group with a surgically induced astigmatism of 0.48 ± 0.24 D and correction index = 0.46. CONCLUSIONS: Both the T-ICL and OCCI techniques provided excellent results in terms of safety and efficacy. T-ICL surgery was shown to be more predictable and accurate for correcting low astigmatism with a lower postoperative spherical equivalent and less residual astigmatism compared to incisional management.

21. Cataract surgery astigmatism incisional management. Manual relaxing incision versus femtosecond laser-assisted arcuate keratotomy. A systematic review

Abstract

Purpose: This systematic review aims to compare corneal astigmatism correction in cataract surgery through corneal relaxing incision, manually and femtosecond laser assisted. Methods: The study was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement recommendations. We used PubMed, Scopus, and Web of Science (WOS) as databases from January 2010 to March 30, 2021. Patients with keratoconus, corneal ectasia, and a previous history of eye surgery were excluded because our aim was to analyze only healthy eyes. Results: A total of 1025 eyes were evaluated from 946 patients (mean age was 68.90 ± 5.12) in manual incision group articles, while 1905 eyes of 1483 patients (mean age was 65.05 ± 4.57) were evaluated in femtosecond laser arcuate keratotomy (FLAK) articles. The mean uncorrected distance visual acuity (UDVA) was 0.19 ± 0.12 and 0.15 ± 0.05 logMAR for manual incision and FLAK articles, respectively (p = 0.39). The mean correction index (CI) was similar in both groups: 0.77 ± 0.18 in manual incision and 0.79 ± 0.17 in femtosecond laser assisted incision (p = 0.70). Refractive stability was found after 3 months and no serious complications were reported during the follow-up in any group. Conclusion: Both techniques are safe and moderately effective in corneal astigmatism correction in cataract surgery. FLAK represents a more precise and predictable approach. However, since visual and refractive outcomes appear to be similar in both cases, the cost-benefit analysis is controversial.

22. Early patient-reported outcomes of cataract surgery with implantation of the trifocal liberty 677MY intraocular lens: A pilot study

Abstract

Purpose: To report our first clinical and patient-reported outcomes in the early postoperative period with a new model of trifocal diffractive intraocular lens (IOL). Methods: Pilot prospective analysis of the results of 26 eyes of 13 patients (age, 44–79 years) undergoing cataract surgery with implantation of the trifocal diffractive IOL Liberty 677MY (Medicontur Medical Engineering Ltd., Zsámbék, Hungary). The following clinical outcomes were evaluated during the first postoperative month: measurement of distance, intermediate and near visual acuity, binocular defocus curve, and level of spectacle independence, patient satisfaction, perception of photic phenomena, and difficulty in performing several vision-related daily tasks by means of a questionnaire. Results: Mean binocular LogMAR uncorrected distance, intermediate and near visual acuities were −0.03 ± 0.13, 0.21 ± 0.16, and 0.16 ± 0.09, respectively. Furthermore, 100.0%, 84.6% and 92.3% of patients achieved a binocular corrected distance, distance-corrected intermediate and distance-corrected near visual acuity of 20/32 or better, respectively. In the defocus curve, visual acuities were better than 0.2 logMAR for defocus levels between +1.00 and −3.00 D. Spectacle independence was referred by all patients, with any of them reporting dissatisfaction with the outcome of the surgery. The postoperative vision did not lead to difficulties in the daily life in 92.3% of patients. Bothersome halos, glare or starbursts were only reported by only 7.7% of patients. Conclusions: The trifocal IOL evaluated provides a successful visual rehabilitation with minimal photic phenomena associated, leading to high levels of spectacle independence and patient satisfaction.

Abstract

PURPOSE: To compare two aspheric ablation profiles in myo- pic refractive surgery using different asphericity targets. METHODS: Patients underwent laser in situ keratomileu- sis (LASIK) with the WaveLight EX500 laser platform (Alcon, WaveLight Laser Technologie). Asymmetric surgery was per- formed, programming the wavefront-optimized (WFO) abla- tion profile in one eye and the custom-Q (CQ) profile in the contralateral eye. The patients were divided into two groups following a systematic randomization method. The Q-target programmed for the preoperative Q group was equal to the preoperative asphericity of the CQ profile, and for the -0.6 Q-target group, the Q-target was set to -0.6. RESULTS: The study included 100 patients (200 eyes). Both groups had comparable safety and efficacy indexes greater. The continuously evolving ophthalmic industry together with ongoing advances in biomedical research have made corneal refractive ablation surgery the technique of choice in low and medium myopia surgery. Several authors have reported high safety and efficacy indexes in both laser in situ ker- atomileusis (LASIK)1,2 and photorefractive keratec- tomy.3,4 However, the main challenge for clinicians and engineers is to control higher order aberrations than 1. A similar oblate shift in postoperative asphericity was seen in both groups regardless of the ablation profile and programmed Q-target. Asphericity was 0.33 ± 0.34 and 0.35 ± 0.29 (P = .18) in the preoperative Q group and 0.26 ± 0.28 and 0.26 ± 0.27 (P = .89) in the -0.6 Q-target group for WFO and CQ, respectively. A lower spherical aberration was found with CQ compared to WFO when the Q-target was set to -0.6: 0.211 ± 0.121 versus 0.144 ± 0.114 (P < .01). However, no statisti- cally significant differences were found when the preopera- tive Q-target was used. CONCLUSIONS: WFO and CQ treatments are similar in terms of refractive and visual outcomes. Abstract

El síndrome de Sjögren (SS) es una enfermedad de tipo autoinmune con importante afectación ocular. Su etiología es aún desconocida con un numero de factores elevados como posibles generadores de la enfermedad. El cambio en la microbiota intestinal ha sido reportado como un factor diferencial entre los sujetos sanos y los sujetos con SS. Diferencias en la microbiota intestinal han sido reportadas en otras patologías sistémicas autoinmunes. Para no tener factores de confusión entre otras patologías sistémicas autoinmunes se ha realizado un estudio en pacientes estrictamente seleccionados y con ausencia de otras alteraciones inmunitarias, siendo seleccionados solo los pacientes con síndrome de Sjögren primario (SSP). Los objetivos de esta tesis doctoral han sido la caracterización de microbiota fecal en pacientes con SSP comparados con pacientes sanos, la determinación de parámetros inflamatorios en plasma de pacientes con SSP comparados con pacientes sanos y su relación con microbiota fecal, así como la determinación de permeabilidad intestinal a través de zonulina plasmática. En esta tesis doctoral se ha realizado un estudio de pacientes con SSP para poder establecer si los cambios en la microbiota intestinal son asociados a la presencia de esta enfermedad de forma aislada, o bien si en el SSP no existen diferencias con los sujetos sanos. De este modo podríamos centrar investigaciones posteriores en otras patologías autoinmunes como la artritis reumatoide (AR) y el lupus eritematosis sistémico (LES), entre otras. Algunos autores han estudiado patologías autoinmunes encontrando diferencias en la microbiota intestinal lo que nos ha llevado a profundizar en el estudio del SSP como entidad aislada sin otras enfermedades autoinmunes asociadas que nos lleven a posibles confusiones. Se han estudiado parámetros de inflamación y de permeabilidad intestinal para intentar establecer relaciones entre estos parámetros y los cambios de la microbiota intestinal, así como se ha intentado establecer o aportar información sobre la etiopatogenia de la enfermedad. Los resultados obtenidos muestran que la microbiota intestinal de los pacientes con SSP difieren tanto a nivel taxonómico como a nivel funcional con respecto a controles sanos. El perfil de microbiota intestinal de nuestros pacientes con SSP presento una menor diversidad y riqueza siendo el filo Bacteroidetes el grupo más representado. Se ha encontrado un aumento de la producción de citoquinas proinflamatorias y la disminución de la liberación de citoquinas antiinflamatorias (IL-10). Se necesitan mas estudios para comprender mejor la realidad del impacto de la disbiosis en el curso del SSP y para concebir estrategias preventivas o terapéuticas para contrarrestar la inflamación impulsada por el microbioma. Sjögren's syndrome (SS) is an autoimmune disease with significant ocular involvement. Its etiology is still unknown with a high number of factors as possible generators of the disease. The change in the gut microbiota has been reported as a differential factor between healthy subjects and subjects with SS. Differences in the intestinal microbiota have been reported in other autoimmune systemic pathologies. In order to avoid confounding factors between other autoimmune systemic pathologies, our study has been carried out in strictly selected patients and with the absence of other immune alterations, only selecting patients with primary Sjögren's syndrome (SSP). The objectives of this doctoral thesis have been the characterization of the fecal microbiota in patients with SSP compared to healthy patients, the determination of inflammatory parameters in plasma in SSP patients compared to healthy patients and their relationship with fecal microbiota as well as the determination of intestinal permeability through plasma zonulin. In this doctoral thesis, a study of patients with SSP has been carried out in order to establish if changes in the intestinal microbiota are associated with the presence of this disease in isolation, or if there are no differences in SSP with healthy subjects. In this way, we could focus further research on other autoimmune diseases such as rheumatoid arthritis (AR) and systemic lupus erythematosis (LES), among others. Some authors have studied autoimmune pathologies finding differences in the intestinal microbiota, which has led us to delve into the study of SSP as an isolated entity without other associated autoimmune diseases that lead us to possible confusion. Inflammation and intestinal permeability parameters have been studied to try to establish relationships between these parameters and changes in the intestinal microbiota, as well as attempts to establish or provide information on the etiopathogenesis of the disease. The results obtained show that the intestinal microbiota of patients with SSP differ both taxonomic level and functional level with respect to healthy controls. The intestinal microbiota profile of our patients with SSP presented less diversity and richness, the Bacteroidetes phylum was the most represented group An increase in the production of pro-inflammatory cytokines and a decrease in the release of anti-inflammatory cytokines (IL-10) have been found. More studies are needed to understand the impact of dysbiosis on the course of SSP and to devise preventive or therapeutic strategies to counter microbiome-driven inflammation.

Abstract

El síndrome de Sjögren (SS) es una enfermedad de tipo autoinmune con importante afectación ocular. Su etiología es aún desconocida con un numero de factores elevados como posibles generadores de la enfermedad. El cambio en la microbiota intestinal ha sido reportado como un factor diferencial entre los sujetos sanos y los sujetos con SS. Diferencias en la microbiota intestinal han sido reportadas en otras patologías sistémicas autoinmunes. Para no tener factores de confusión entre otras patologías sistémicas autoinmunes se ha realizado un estudio en pacientes estrictamente seleccionados y con ausencia de otras alteraciones inmunitarias, siendo seleccionados solo los pacientes con síndrome de Sjögren primario (SSP). Los objetivos de esta tesis doctoral han sido la caracterización de microbiota fecal en pacientes con SSP comparados con pacientes sanos, la determinación de parámetros inflamatorios en plasma de pacientes con SSP comparados con pacientes sanos y su relación con microbiota fecal, así como la determinación de permeabilidad intestinal a través de zonulina plasmática. En esta tesis doctoral se ha realizado un estudio de pacientes con SSP para poder establecer si los cambios en la microbiota intestinal son asociados a la presencia de esta enfermedad de forma aislada, o bien si en el SSP no existen diferencias con los sujetos sanos. De este modo podríamos centrar investigaciones posteriores en otras patologías autoinmunes como la artritis reumatoide (AR) y el lupus eritematosis sistémico (LES), entre otras. Algunos autores han estudiado patologías autoinmunes encontrando diferencias en la microbiota intestinal lo que nos ha llevado a profundizar en el estudio del SSP como entidad aislada sin otras enfermedades autoinmunes asociadas que nos lleven a posibles confusiones. Se han estudiado parámetros de inflamación y de permeabilidad intestinal para intentar establecer relaciones entre estos parámetros y los cambios de la microbiota intestinal, así como se ha intentado establecer o aportar información sobre la etiopatogenia de la enfermedad. Los resultados obtenidos muestran que la microbiota intestinal de los pacientes con SSP difieren tanto a nivel taxonómico como a nivel funcional con respecto a controles sanos. 43. Development of machine learning-based models for vault prediction in implantable collamer lens surgery according to implant orientation.

Abstract

Purpose: The main objective was to develop a prediction model based on machine learning to calculate the postoperative vault as well as the ideal implantable collamer lens (ICL) size, considering for the first time the implantation orientation in a Caucasian population., Setting: Arruzafa Ophthalmological Hospital (Cordoba, Spain) and Barraquer Ophthalmology Center (Barcelona, Spain)., Design: Multicenter, randomized, retrospective study., Methods: Anterior segment biometric data from 235 eyes of patients who underwent ICL lens implantation surgery were collected using the anterior segment optical coherence tomography (AS-OCT) CASIA II, to train and validate five types of multiple regression models based on advanced machine learning techniques. To perform an external validation a dataset of 45 observations was used., Results: The Pearson correlation coefficient between observed and predicted values was similar in the five models in the external validation, with least absolute shrinkage and selection operator (LASSO) regression being the highest (r = 0.62, p < 0.001), followed by random forest regression model (r = 0.60, p < 0.001) and backward stepwise regression (r = 0.58, ρ < 0.001). In addition, the predictions generated by the different models showed closer agreement with the actual vault compared with the Nakamura formulas. Using the new methods, about 70% of the observations had a prediction error below 150 µm., Conclusions: Advanced forms of regressions models based on machine learning allow satisfactory calculation of the ideal lens size, offering greater precision to surgeons customizing surgery according to implant orientation., (Copyright © 2025 Published by Wolters Kluwer on behalf of ASCRS and ESCRS.)
