Graph representation learning methods opened new avenues for addressing complex, real-world problems represented by graphs. However, many graphs used in these applications comprise millions of nodes and billions of edges and are beyond the capabilities of current methods and software implementations. We present GRAPE (Graph Representation Learning, Prediction and Evaluation), a software resource for graph processing and embedding that is able to scale with big graphs by using specialized and smart data structures, algorithms, and a fast parallel implementation of random-walk-based methods. Compared with state-of-the-art software resources, GRAPE shows an improvement of orders of magnitude in empirical space and time complexity, as well as competitive edge- and node-label prediction performance. GRAPE comprises approximately 1.7 million well-documented lines of Python and Rust code and provides 69 node-embedding methods, 25 inference models, a collection of efficient graph-processing utilities, and over 80,000 graphs from the literature and other sources. Standardized interfaces allow a seamless integration of third-party libraries, while ready-to-use and modular pipelines permit an easy-to-use evaluation of graph-representation-learning methods, therefore also positioning GRAPE as a software resource that performs a fair comparison between methods and libraries for graph processing and embedding.

Many real-world problems are naturally modeled as heterogeneous graphs, where nodes and edges represent multiple types of entities and relations. Existing learning models for heterogeneous graph representation usually depend on the computation of specific and user-defined heterogeneous paths, or in the application of large and often not scalable deep neural network architectures. We propose Het-node2vec, an extension of the node2vec algorithm, designed for embedding heterogeneous graphs. Het-node2vec addresses the challenge of capturing the topological and structural characteristics of graphs and the semantic information underlying the different types of nodes and edges of heterogeneous graphs, by introducing a simple stochastic node and edge type switching strategy in second order random walk processes. The proposed approach also introduces an ''attention mechanism'' to focus the random walks on specific node and edge types, thus allowing more accurate embeddings and more focused predictions on specific node and edge types of interest. Empirical results on benchmark datasets show that Hetnode2vec achieves comparable or superior performance with respect to state-of-the-art methods for heterogeneous graphs in node label and edge prediction tasks., Comment: 25 pages (references excluded), 9 figures

X-ray-based analytics are routinely applied in many fields, including physics, chemistry, materials science, and engineering. The full potential of such techniques in the life sciences and medicine, however, has not yet been fully exploited. We highlight current and upcoming advances in this direction. We describe different X-ray-based methodologies (including those performed at synchrotron light sources and X-ray free-electron lasers) and their potentials for application to investigate the nano-bio interface. The discussion is predominantly guided by asking how such methods could better help to understand and to improve nanoparticle-based drug delivery, though the concepts also apply to nano-bio interactions in general. We discuss current limitations and how they might be overcome, particularly for future use in vivo.

The SARS-CoV-2 global pandemic will disproportionately impact countries with weak economies and vulnerable populations including people with dementia. Latin American and Caribbean countries (LACs) are burdened with unstable economic development, fragile health systems, massive economic disparities, and a high prevalence of dementia. Here, we underscore the selective impact of SARS-CoV-2 on dementia among LACs, the specific strain on health systems devoted to dementia, and the subsequent effect of increasing inequalities among those with dementia in the region. Implementation of best practices for mitigation and containment faces particularly steep challenges in LACs. Based upon our consideration of these issues, we urgently call for a coordinated action plan, including the development of inexpensive mass testing and multilevel regional coordination for dementia care and related actions. Brain health diplomacy should lead to a shared and escalated response across the region, coordinating leadership, and triangulation between governments and international multilateral networks.

- Abstract
Selenium (Se) compounds hold promise as potential therapeutics for cancer due to their diverse biological functions and redox‐regulating properties. In this study, the encapsulation of a hydrophobic selenium compound inside 5 µm sized biodegradable polyelectrolyte capsules consisting of poly‐L‐arginine and dextran is investigated. While the encapsulated form of the compound showed comparable cellular uptake and cytotoxicity as the free drug, e.g. no improved anti‐cancer efficacy is achieved, the investigation yielded crucial insights. Utilizing subcellular resolution synchrotron X‐ray fluorescence imaging (XFI), the intracellular location of the Se compound is successfully visualized by demonstrating the degradation of the capsules and the following redistribution of the compound, without exogenous labeling. By applying a quantitative fitting approach, the intracellular Se mass is determined to be 0.09 ± 0.01 pg after degradation of the carrier. This highlights the potential of XFI as a powerful tool for tracking intracellular dynamics of metal‐based drugs, which may facilitate drug development in the clinic. [ABSTRACT FROM AUTHOR]

We analyze the adsorption of the proteinogenic amino acids (AAs) glutamine, glutamic acid, lysine, tyrosine, proline, and valine onto bare iron oxide nanoparticles (approx. 10 nm). Aiming to identify the governing principles of low molecular weight coronae, which remain underinvestigated, our study covers broad concentration ranges up to the solubility limit of the AAs. Isothermal experiments reveal that the highly soluble AAs valine, proline, and lysine form extensive multilayers on the nanoparticle surface, and infrared measurements indicate intermolecular interactions, particularly with valine and lysine, for higher AA contents. Conversely, the low solubility of tyrosine and glutamic acid restricts their adsorption capacity, despite their higher partitioning on the solid surface. Parameters derived from fitting a classic saturation model seem to align with well‐documented physicochemical properties such as the hydrophobicity and the complexity indices – a promising first step towards formulating design principles. Scaling these parameters by the AA solubility reveals a clear correlation with the adsorption behavior. In adsorption experiments with AA model mixtures, sequential incubation increases the adsorption capacity for valine and proline, whereas simultaneous incubation with these AAs reduces tyrosine's capacity. Future studies should seek to elucidate adsorption patterns to advance our understanding of corona growth and evolution mechanisms. [ABSTRACT FROM AUTHOR]

- Abstract
Proactive control of hospital processes significantly influences patient performance and satisfaction. The objective of the research is to develop a dashboard for the evaluation of patient flow management in hospital institutions. Descriptive quantitative study was developed and previous methodologies were analyzed. A procedure was developed for the preparation of a dashboard for the evaluation of patient flow management in hospital institutions, structured in five stages and nine steps, including procedures for the identification of key variables and aggregation of indicators. A dashboard was prepared for the General Surgery service of a Clinical Surgical Teaching Hospital, which is composed of 18 indicators, 6 synthetic indexes and an integral indicator. The integral indicator obtained a value of 0.79 (4), which shows the effectiveness of the actions implemented during the deployment of the improvement model. Among the positive implications for health managers is the possibility of monitoring strategies with a process vision., El control proactivo de los procesos hospitalarios influye de forma significativa en el rendimiento y satisfacción de los pacientes. El objetivo de la investigación es desarrollar un tablero de control para la evaluación de la gestión del flujo de pacientes en instituciones hospitalarias. Se desarrolló un estudio cuantitativo descriptivo y se analizaron metodologías precedentes. Se elaboró un proceder para la confección de un tablero de control para la evaluación de la gestión de los flujos de pacientes en instituciones hospitalarias, estructurado en cinco etapas y nueve pasos, incluye procedimientos para la identificación de variables clave y agregación de indicadores. Se confeccionó un tablero de control para el servicio de Cirugía General de un hospital Clínico Quirúrgico Docente, que está compuesto por 18 indicadores, 6 índices sintéticos y un indicador integral. El indicador integral obtuvo un valor de 0,79 (4), lo que es muestra de la eficacia de las acciones implementadas durante el despliegue del modelo de mejora. Entre las implicaciones positivas para los gestores sanitarios está la posibilidad de monitorear las estrategias con una visión de proceso.

Introduction: Frailty is defined as a state prior to disability, characterized by greater vulnerability to external stressors. Frailty contributes to negative outcomes such as falls, hospitalization, disability, and mortality. The objective of this study is to identify the prevalence and evaluate the factors associated with frailty in patients of the geriatric outpatient service of the Hospital Universitario San Ignacio. Materials and methods: a cross-sectional study was carried out including patients seen for the first time in the geriatric outpatient clinic of the HUSI in Bogotá, Colombia between August 2016 and March 2020. Results: the prevalence of frailty was 35.4%. Statistically significant variables associated with frailty were age over 80 years (OR 2.07), female gender (OR 1.40), multimorbidity (OR 2.13), and nutritional status (OR 2.23). When the analysis was adjusted for performance variables, only multimorbidity (OR 2.46 95% CI 1.62 – 3.75 p 0.001), slow walking speed (OR 5.15 95% CI 3.0 – 8.60 p 0.001) and low calf circumference (OR 1.60 CI 95% 1.03 – 2.50 p 0.06) were statistically significant. Conclusion: the prevalence of frailty found in our study is higher compared to national reference surveys, the associated variables coincide with those found in the literature such as older age, female sex, number of comorbidities and malnutrition. In addition, an association was found with other performance measures such as low calf circumference and slow walking speed., Introdução: A fragilidade é definida como um estado anterior à incapacidade, caracterizado por maior vulnerabilidade a estressores externos. A fragilidade contribui para resultados negativos, como quedas, hospitalização, incapacidade e mortalidade. O objetivo deste estudo é identificar a prevalência e avaliar os fatores associados à fragilidade em pacientes do ambulatório de geriatria do Hospital Universitário San Ignacio (HUSI). Materiais e métodos: foi realizado um estudo transversal incluindo pacientes atendidos pela primeira vez no ambulatório de geriatria do HUSI em Bogotá, Colômbia, entre agosto de 2016 e março de 2020. Resultados: a prevalência de fragilidade foi de 35,4%. As variáveis estatisticamente significativas associadas à fragilidade foram idade acima de 80 anos (OR 2,07), sexo feminino (OR 1,40), multimorbidade (OR 2,13) e estado nutricional (OR 2,23). Resultados: la prevalencia de fragilidad fue del 35,4%. Las variables estadísticamente significativas asociadas con fragilidad fueron edad mayor de 80 años (OR 2.07), sexo femenino (OR 1.40), multimorbilidad (OR 2.13) y estado nutricional (OR 2.23). Cuando se ajustó el análisis por variables de desempeño, solamente la multimorbilidad (OR 2.46 IC 95% 1.62 – 3.75 p 0.001), la velocidad de la marcha lenta (OR 5.15 IC 95% 3.0 – 8.60 p 0.001) y el perímetro de pantorrilla bajo (OR 1.60 IC 95% 1.03 – 2.50 p 0.06) fueron estadísticamente significativos. Conclusión: la prevalencia de fragilidad encontrada en nuestro estudio es mayor comparada con las encuestas referentes nacionales, las variables asociadas coinciden con las encontradas en la literatura como mayor edad, sexo femenino, numero de comorbilidades y malnutrición. Además, se encontró asociación con otras medidas de desempeño como perímetro de pantorrilla bajo y velocidad para la marcha lenta.

[Resumo] O obxecto deste estudo son as concesións hidroeléctricas españolas, dentro das cales se realiza especial mención ao caso galego. Con respecto a elas, analízanse a súa finalización e reversión á Administración, así como distintas fórmulas para a súa nova explotación. Para levar a cabo esta análise estúdanse a evolución histórica e a regulación actual tanto do sector eléctrico como do dereito de augas. Unha vez observados ambos os campos, investígase o concepto de concesións de dominio público e, dentro destas, a regulación das concesións hidroeléctricas, especialmente a súa finalización e reversión. Posteriormente, inclúese un estudo de dereito comparado en que se aprecian as distintas formas xurídicas de explotación dos recursos hidroeléctricos nos países con maior potencia instalada no ámbito tanto europeo como mundial, así como a estrutura do mercado de produción hidroeléctrica dos devanditos países. Por último, analízase a conveniencia de continuar ou non coa explotación deste recurso e as distintas formas xurídicas de facelo. Para iso, obsérvanse as vantaxes e os inconvenientes dunha explotación privada, pública e mixta dos aproveitamentos, e propóñense distintos cambios normativos e formas xurídicas a través das cales se pode levar cabo esta explotación ou mellorar o modelo existente., [Resumen] El objeto del presente estudio son las concesiones hidroeléctricas españolas y, dentro de ellas, se realiza especial mención al caso gallego. Con respecto a estas se analiza su finalización y reversión a la Administración, así como distintas fórmulas para su nueva explotación. Para llevar a cabo este análisis se realiza un estudio de la evolución histórica y su regulación actual tanto del sector eléctrico como del derecho de aguas. Una vez observados ambos campos se investiga el concepto de concesiones de dominio público y, dentro de éstas, la regulación de las concesiones hidroeléctricas, especialmente su finalización y reversión. Posteriormente, se incluye un estudio de derecho comparado en el que se aprecian las distintas formas jurídicas de explotación de los recursos hidroeléctricos en los países con mayor potencia instalada tanto a nivel europeo como mundial, así como su estructura del mercado de producción hidroeléctrica. Por último, se analiza la conveniencia de continuar o no con la explotación de este recurso y las distintas formas jurídicas de hacerlo. Para ello, se observan las ventajas e inconvenientes de una explotación privada, pública y mixta de los aprovechamientos y se proponen distintos cambios normativos y formas jurídicas a través de las que puede llevarse a cabo esta explotación o mejorar el modelo existente., [Abstract] The object of this study is the Spanish hydroelectric concessions and, within them, special mention is made of the Galician case. With respect to these, their completion and reversion to the Administration is analysed, as well as different ways for its new exploitation. In order to carry out this analysis, a study is made of the historical evolution and current regulation of both the electricity sector and water law. Once both issues have been observed, the concept of public domain concessions is investigated and, within these, the regulation of hydroelectric concessions, especially their completion and reversion. Subsequently, a study of comparative law is included in which the different legal forms of exploitation of hydroelectric resources in the countries with the highest installed power are appreciated, both at a European and world level, as well as their structure of the hydroelectric production market. Finally, the convenience of continuing or not with the exploitation of this resource and the different legal forms of doing so are analysed. For this, the advantages and disadvantages of a private, public and mixed exploitation of the hydroelectric uses are observed and different regulatory changes and legal forms are proposed through which this exploitation can be carried out or improve the existing model.

Green urbanism and sustainable cities are concepts that focus on the design and planning of urban environments that promote the quality of life of inhabitants and minimize environmental impact. This bibliometric review explored research and academic publications related to city design and planning from an ecological and sustainable perspective. Through a comprehensive analysis, architectural and urban planning approaches that integrate ecological elements into the design of buildings and public spaces, the implementation of green infrastructure, and the promotion of sustainable mobility were identified. The social, economic and environmental impacts of green urban interventions were also assessed, as well as the policies and strategies used to promote urban sustainability. The main purpose of this research focused on identifying current trends, emerging research areas and best practices in the field of green urbanism and sustainable cities. The results of this research provide valuable information for practitioners, researchers and decision-makers in the field of urban planning and urban planning. [ABSTRACT FROM AUTHOR]

37. Biomolecular interactions and cellular effects of steroidal and metallosupramolecular metallodrugs

The work described herein concerns the effect on the anticancer activity and the ability to reach their possible intracellular targets of certain steroidal metallodrugs and metallosupramolecular cylinders. Chapter 1 surveys the background to the project, surveying different DNA-binding modes, explaining their importance in the anticancer properties of metallodrugs and showing an overview of the different strategies used for enhanced delivery of these metallodrugs. In Chapter 2 the synthesis of new steroidal DNA covalent-binding platinum(II) complexes together with techniques to purify previously synthesised steroidal complexes are presented. Their cytotoxicity, cellular uptake and biomolecular interaction are investigated, showing that the coupling of the steroid confers activity to otherwise inactive complexes, modifying their DNA binding mode and cellular uptake and distribution. Chapter 3 explores the coupling of similar steroidal delivery vectors to non-covalent metallodrugs, presenting simple synthetic pathways to create such complexes in a single step. Their anticancer activity and DNA-binding affinity are investigated: surprisingly showing that this coupling has negative effects. In Chapter 4 the cytoxicity and cellular behaviour of metallosupramolecular cylinders are studied. It is shown that these complexes can cross the cellular membrane, concentrating in the nuclei where they can interact with cellular DNA.

Rapid advances in DNA sequencing promise to enable new diagnostics and individualized therapies. Achieving personalized medicine, however, will require extensive research on highly reidentifiable, integrated datasets of genomic and health information. To assist with this, participants in the Personal Genome Project choose to forgo privacy via our institutional review board- approved "open consent" process. The contribution of public data and samples facilitates both scientific discovery and standardization of methods. We present our findings after enrollment of more than 1,800 participants, including whole-genome sequencing of 10 pilot participant genomes (the PGP-10). We introduce the Genome-Environment-Trait Evidence (GET-Evidence) system. This tool automatically processes genomes and prioritizes both published and novel variants for interpretation. In the process of reviewing the presumed healthy PGP-10 genomes, we find numerous literature references implying serious disease. Although it is sometimes impossible to rule out a late-onset effect, stringent evidence requirements can address the high rate of incidental findings. To that end we develop a peer production system for recording and organizing variant evaluations according to standard evidence guidelines, creating a public forum for reaching consensus on interpretation of clinically relevant variants. Genome analysis becomes a two-step process: using a prioritized list to record variant evaluations, then automatically sorting reviewed variants using these annotations. Genome data, health and trait information, participant samples, and variant interpretations are all shared in the public domain-we invite others to review our results using our participant samples and contribute to our interpretations. We offer our public resource and methods to further personalized medical research.

- Abstract
Semaglutide, a GLP-1 receptor agonist, reduces the risk of major adverse cardiovascular events (MACE) in people with overweight or obesity, but the effects of this drug on outcomes in patients with atherosclerotic cardiovascular disease and heart failure are unknown. We report a prespecified analysis of the effect of once-weekly subcutaneous semaglutide 2·4 mg on ischaemic and heart failure cardiovascular outcomes. We aimed to investigate if semaglutide was beneficial in patients with atherosclerotic cardiovascular disease with a history of heart failure compared with placebo; if there was a difference in outcome in patients designated as having heart failure with preserved ejection fraction compared with heart failure with reduced ejection fraction; and if the efficacy and safety of semaglutide in patients with heart failure was related to baseline characteristics or subtype of heart failure.
